RESUMO

We sought to develop and characterize outer retinal degeneration induced by intravitreal injection of sodium iodate (SI) after vitrectomy in rabbits. To determine the effective dose of SI, the right eyes of 19 male New Zealand white rabbits received an intravitreal injection of SI or sham. Based on the dose-dependence results, 0.4 mg of SI in 0.05 mL of total volume was injected into the right eyes of 10 rabbits at two weeks after vitrectomy. In the dose-dependence study, localized retinal atrophy was observed with 0.3- and 0.4-mg SI injections without vitrectomy. Severe and diffuse retinal atrophy was identified by spectral-domain optical coherence tomography (SD-OCT) at one month after a 0.5-mg SI injection following vitrectomy. In the second experiment, 0.4 mg of SI in 0.05 mL was injected, and the severity of outer retinal degeneration was graded as one of two types according to electroretinography (ERG) response change. There was no response on ERG in complete retinal degeneration, 30% of all 10 rabbits. Intravitreal injection of 0.4 mg of SI into vitrectomized rabbit eyes induces diffuse outer retinal degeneration, and the degree of retinal degeneration can be evaluated through in vivo ophthalmic examination.

RESUMO

Phase-coherent transfer of optical frequencies over a long distance is required for diverse photonic applications, including optical clock dissemination and physical constants measurement. Several demonstrations were made successfully over fiber networks, but not much work has been done yet through the open air where atmospheric turbulence prevails. Here, we use an 18 km outdoor link to transmit multiple optical carriers extracted directly from a frequency comb of a 4.2 THz spectral width. In stabilization to a high-finesse cavity with a 1.5 Hz linewidth, the comb-rooted optical carriers are simultaneously transferred with collective suppression of atmospheric phase noise to -80 dBc Hz-1. Microwaves are also delivered by pairing two separate optical carriers bound with inter-comb-mode coherence, for example a 10 GHz signal with phase noise of -105 dBc Hz-1 at 1 Hz offset. Lastly, an add-on demonstration is given for multi-channel coherent optical communications with the potential of multi-Tbps data transmission in free space.

Metabolome-genome wide association studies (mGWASs) are useful for understanding the genetic regulation of metabolites in complex diseases, including type 2 diabetes (T2D). Numerous genetic variants associated with T2D-related metabolites have been identified in previous mGWASs; however, these analyses seem to have difficulty in detecting the genetic variants with functional effects. An exome array focussed on potentially functional variants is an alternative platform to obtain insight into the genetics of biochemical conversion processes. In the present study, we performed an mGWAS using 27,140 non-synonymous variants included in the Illumina HumanExome BeadChip and nine T2D-related metabolites identified by a targetted metabolomics approach to evaluate 2,338 Korean individuals from the Korea Association REsource (KARE) cohort. A linear regression analysis controlling for age, sex, BMI, and T2D status as covariates was performed to identify novel non-synonymous variants associated with T2D-related metabolites. We found significant associations between glycine and CPS1 (rs1047883) and PC ae C36:0 and CYP4F2 (rs2108622) variants (P<2.05 × 10-7, after the Bonferroni correction for multiple testing). One of the two significantly associated variants, rs1047883 was newly identified whereas rs2108622 had been previously reported to be associated with T2D-related traits. These findings expand our understanding of the genetic determinants of T2D-related metabolites and provide a basis for further functional validation.

BACKGROUND: The optimal extent of lymph node dissection in patients with descending colon cancer is still debatable. We designed this study to evaluate the distribution of lymph node metastasis and the appropriate extent of lymph node dissection in descending colon cancer patients. METHODS: We retrospectively reviewed the medical records of 118 descending colon cancer patients without distant metastasis, who underwent curative resection between January 2004 and December 2014. The distribution of lymph node metastasis was evaluated, and prognostic factors were analysed. RESULTS: The median follow-up period was 52 months (range 1-125 months). Twenty-six (22.0%) patients underwent high ligation of the inferior mesenteric artery (IMA), whereas 92 (78.0%) patients underwent ligation of the left colic artery, saving the IMA. Lymph nodes at the origin of the IMA showed no metastasis in any of the 26 patients who underwent high ligation of the IMA. After propensity score matching, 3-year disease-free survival (80.4% versus 92.9%, P = 0.471) and 5-year overall survival (81.8% versus 90.9%, P = 0.875) were not significantly different according to the type of IMA ligation. CONCLUSION: In patients with descending colon cancer, there was no lymph node metastasis at the origin of the IMA, and ligation of the IMA showed no prognostic benefit.

Although a bipedicled deep inferior epigastric artery perforator (DIEP) flap is widely accepted for slim patients with large breasts, we suggest DIEP flap-based breast reconstruction in which the superficial inferior epigastric artery (SIEA) is supercharged to the branch of thoracodorsal vessel as an alternative, which has not previously been well described. We report the case of a 48-year-old breast cancer patient who had a normal body mass index of 23.01 and relatively thin abdominal tissue, with large and ptotic (grade II ptosis) breasts. The mastectomy specimen weighed ~890 g, and the harvested abdominal tissue weighed ~700 g with a size of 32 × 12 cm2 . The elliptical-shaped flap was inset with a 90° counterclockwise rotation, and the lower one-third of the flap was folded to create a projection. Perfusion of flap was augmented by microvascular anastomosis between the contralateral SIEA and the serratus branch of a thoracodorsal vessel. With a supercharged DIEP flap, nearly the whole lower abdominal tissue (696 g, 99.4% of the elevated flap) could be transferred to obtain a symmetric contour, and there were no complications such as vascular obstruction, flap necrosis, and delayed wound healing during the postoperative course. Using the SIEA pedicle for contralateral abdominal perfusion with elongated branch of the thoracodorsal vessel, aesthetic inset and contouring of the reconstructed breast could be technically enhanced. The DIEP flap with the contralateral SIEA supercharged to the serratus branch of thoracodorsal vessel may be a feasible option for large ptotic breast reconstruction in thin patients.

OBJECTIVE: To evaluate flow pattern characteristics in the ascending aorta (AA) with four-dimensional (4D)-flow MRI and to determine predictors of aortic dilatation late after tetralogy of Fallot (TOF) repair. MATERIALS AND METHODS: This study included 44 patients with repaired TOF (25 males and 19 females; mean age, 28.9 ± 8.4 years) and 11 volunteers (10 males and 1 female, mean age, 33.7 ± 8.8 years) who had undergone 4D-flow MRI. The aortic diameters, velocity, wall shear stress (WSS), flow jet angle (FJA), and flow displacement (FD) at the level of the sinotubular junction (STJ) and mid-AA were compared between the repaired TOF and volunteer groups. The hemodynamic and clinical parameters were also compared between the aortic dilatation and non-dilatation subgroups in the repaired TOF group. RESULTS: The diameters of the sinus of Valsalva, STJ, and AA were significantly higher in the repaired TOF group than in the volunteer group (p = 0.002, p < 0.001, and p = 0.013, respectively). The FJAs at the STJ and AA were significantly greater in the repaired TOF group (p < 0.001 and p = 0.003, respectively), while velocities and WSS parameters were significantly lower. FD showed no statistically significant difference (p = 0.817). In subgroup analysis, age at TOF repair was significantly higher (p = 0.039) and FJA at the level of the AA significantly greater (p = 0.003) and mean WSS were significantly lower (p = 0.039) in the aortic dilatation group. FD were higher in the aortic dilatation group without statistical significance (p = 0.217). CONCLUSION: Patients with repaired TOF have an increased FJA, dilated AA, and secondarily decreased WSS. In addition to known risk factors, flow eccentricity may affect aortic dilatation in patients with repaired TOF.

PURPOSE: Previous studies have reported paradoxical survival prognoses for some node-negative and node-positive colon cancer patients. However, current guidelines recommend adjuvant chemotherapy (CT) only for node-positive patients. This study investigated the efficacy of adjuvant CT for patients who underwent radical surgery for colon cancer with solitary lymph node (LN) metastasis. METHODS: This study included 281 patients treated between 2004 and 2015. Patients were classified into no-CT (n = 39) and CT (n = 242) groups, and the survival outcomes and recurrence-related follow-up data were analyzed. RESULTS: The groups exhibited similarities in tumor sidedness, tumor differentiation, and pathologic stage. However, the age, ASA class, and preoperative CEA level were relatively lower in the CT group. Although the CT group had a higher 5-year overall survival (OS) rate than the no-CT group (88.4% vs. 65.3%, p < 0.001), the groups did not differ in terms of 5-year disease-free survival (DFS) (CT, 84.1% vs. no-CT, 83.3%, p = 0.490). A multivariate analysis identified adjuvant CT as an independent factor for OS but not for DFS. A highly examined LN count (≥ 12) was associated with improved DFS improvement. However, D3 LN dissection was not associated with DFS or OS. For DFS, intermediate/apical positive LNs received a high hazard ratio relative to pericolic/epicolic LNs (2.080, 95% confidence interval: 0.979-4.416), but this was not significant (p = 0.057). CONCLUSIONS: Adjuvant chemotherapy did not provide clear advantages for colon cancer with solitary LN metastasis. Further large studies that analyze several prognostic factors are needed to establish tailored adjuvant CT administration guidelines.

BACKGROUND: Anemia is a common cause among the elderly for increased mortality. Hemoglobin concentration can be affected by many factors, but the reference interval defined by the World Health Organization has not been adjusted for the previous half century. METHODS: Through using the dataset generated by the National Health Insurance (NHI) health screening program of Republic of Korea, here we attempt to present a close to actual hemoglobin concentration of the Korean population. Between January 2009 and December 2013, a total of 57,409,872 health screening events were registered in the NHI database. Following the exclusion criteria, 6,759,566 participants were enrolled for analyses. RESULTS: Significant portion of the study population was considered 'anemic', while the mean value (2.5% ~ 97.5%) of hemoglobin concentration from the study was 14.8 (12.5 ~ 16.8) g/dL in men and 12.8 (10.6 ~ 14.7) g/ dL in women. Concordant results of hemoglobin concentration declining with age were observed as previous studies have described, supporting the need for separate, possibly lower cutoff in the elderly. CONCLUSIONS: A considerable portion of the participants being categorized as anemia contests the accuracy of the current lower cutoff for anemia. From a large representative dataset, the need for adjustment to the lower cutoff for anemia is suggested.

BACKGROUND AND AIMS: The coronary artery disease-reporting and data system (CAD-RADS) was recently developed to standardize CAD classifications and incorporate clinical management. We aimed to investigate the prognostic value and additional risk stratification benefits of CAD-RADS compared to coronary artery calcium scores (CACS) and CAD extent classifications in ischemic stroke patients without cardiac symptoms. METHODS: From January 2013 to December 2014, 762 ischemic stroke patients with risk factors for CAD and without chest pain underwent coronary computed tomography angiography. CACS, CAD extent classification, and CAD-RADS scores were used to evaluate the computed tomography angiography images. The primary endpoint was major adverse cardiovascular events (MACEs), which were defined as cardiovascular death, nonfatal myocardial infarction, unstable angina requiring hospitalization, and revascularization. RESULTS: During the mean follow-up period of 3.36 years, 67 MACEs were recorded. Of the 762 patients, 23.5% were classified as CAD-RADS 0, 19.7% as CAD-RADS 1, 18.2% as CAD-RADS 2, 18.6% as CAD-RADS 3, 15.4% as CAD-RADS 4A, 2.2% as CAD-RADS 4B, and 2.4% as CAD-RADS 5. CACS, CAD extent classification, and CAD-RADS scores independently stratified the risk of future MACEs (all p < 0.05). The C-statistics revealed that both CAD extent classification and CAD-RADS scores improved risk stratification beyond CACS (C-index: 0.767 vs. 0.715; 95% confidence interval [CI] 0.026, 0.105), and 0.781 vs. 0.715; 95% CI 0.015, 0.086). CONCLUSIONS: In ischemic stroke patients without chest pain, CAD-RADS had prognostic value for future MACEs and better risk discrimination compared with CACS alone.

Biocompatible and biodegradable gelatin is a good candidate bioink for use in 3D bioprinting technologies, but viscous gelatin solution has a low printability. In order to improve the poor printability of gelatin, we optimized the rheological properties of gelatin solution. 3D gelatin scaffolds were then cross-linked using physical or chemical methods to maintain the 3D structure. The physicochemical and biological differences between the two types of cross-linked gelatin scaffolds were studied. Scanning electron microscopy images revealed that the morphologies of the resulting cross-linked 3D scaffolds maintained their structural stabilities. The physically cross-linked 3D scaffolds maintained their surface sizes without a significant decrease (less than a 3% reduction in the surface size was observed) after cross-linking. To evaluate the differences in cell affinity by two types of cross-linking method, human dermal fibroblasts cultured on the cross-linked 3D scaffolds. After 14 days of culturing, DNA assays showed that the cell proliferation rate of the physically cross-linked 3D scaffold was 44% higher than that of the chemically cross-linked 3D scaffold. In conclusion, the optimized physically cross-linked 3D scaffold retained its surface size without significant decreases after cross-linking, as required by 3D-printed patient-specific tissue engineered customized scaffolds, despite the use of water-soluble gelatin hydrogels.

PURPOSE: The colon originates from the midgut and hindgut, with subsequent differentiation into the right and left colon. The embryology, clinical symptoms, incidence, molecular pathways, and oncologic outcomes differ between right and left colorectal cancers. However, the differences have not been fully accepted. AIM OF THE STUDY: This study compared short- and long-term outcomes between right and left colon cancers. MATERIALS AND METHODS: This study included 966 patients who underwent laparoscopic resection with radical lymph node dissection for stage I, II, and III colon cancers between 2009 and 2014 at a tertiary teaching hospital. We excluded cases with fewer than 12 retrieved lymph nodes, emergency operations, synchronous or multiple cancers, and those located in the transverse colon and rectum. RESULTS: The right colon group included 343 (35.5%) patients and the left colon group 623. Female patients had a high incidence of right colon cancer (p < 0.001). Right colon cancer had longer operative times (p = 0.012), and more bleeding during the operation (p = 0.001). The size of the tumor was larger (p < 0.001) and more lymph nodes were harvested (p < 0.001) on the right side. Vascular (p = 0.006) and lymphatic (p = 0.004) invasion was greater in the right colon, but left colon cancer showed greater neural invasion (p = 0.008). Cancers on the right side also had a tendency to be poorly differentiated (p < 0.001). The groups did not differ in disease-free, overall, and cancer-specific survival rates for stage. CONCLUSION: Although the oncologic outcomes show no significant differences, colon cancer has characteristic, perioperative, and histopathologic differences according to its embryologic origin.

Exotic spin-dependent interactions between fermions have recently attracted attention in relation to theories beyond the Standard Model. The exotic interactions can be mediated by hypothetical fundamental bosons which may explain several unsolved mysteries in physics. Here we expand this area of research by probing an exotic parity-odd spin- and velocity-dependent interaction between the axial-vector electron coupling and the vector nucleon coupling for polarized electrons. This experiment utilizes a high-sensitivity atomic magnetometer, based on an optically polarized vapor that is a source of polarized electrons, and a solid-state mass containing unpolarized nucleons. The atomic magnetometer can detect an effective magnetic field induced by the exotic interaction between unpolarized nucleons and polarized electrons. We set an experimental limit on the electron-nucleon coupling [Formula: see text] at the mediator boson mass below 10-4 eV, significantly improving the current limit by up to 17 orders of magnitude.

We perform fast comb spectroscopy by dividing the probe comb into several sub-comb segments so as to produce multi-heterodyne beats focused around targeted molecular absorption lines. This concentrated scheme of comb spectroscopy is able to achieve a 30 dB signal-to-noise ratio with just a single shot measurement of 10 µs acquisition time. Such high signal sensitivity is verified by measuring separate absorption lines of H13C14N and 12CO2 gases simultaneously. In addition, atmospheric 12CO2 concentration over a 1.3 km open-air path is traced with a signal repeatability of 15 ppm at a 5 kHz update rate.

Stretchability and flexibility are two key requirements for manipulating the propagation of light in compact and high-performance lab-on-a-chip systems. These requirements are best met by embedding stretchable and flexible tuning elements such as volume phase gratings (VPGs) in polydimethylsiloxane (PDMS), making them attractive alternatives to conventional rigid optical elements. However, fabrication of these PDMS VPGs is a challenge, requiring extensive modifications to PDMS or complex multi-step processes that require long processing times. In this context, we propose the concept of "ultrafast volume holography" for the fabrication of stretchable photonic structures such as tunable VPGs directly in unmodified PDMS. Our concept translates insights in heat regulation via fs repetition rate control into volumetric patterning, forming periodic refractive index modulation of 1.95 × 10-4 in the PDMS without post-processing. VPGs formed are further demonstrated as active beam steering units and tunable spectroscopic optical elements.

BACKGROUND: The severity of parkinsonism and response to levodopa vary in patients with multiple system atrophy (MSA) because of the heterogeneity of nigrostriatal neuropathology. OBJECTIVE: To investigate the difference in clinical features between MSA patients with predominantly pre-synaptic nigrostriatal dysfunction and those with trans-synaptic nigrostriatal dysfunction. METHODS: We retrospectively analyzed clinical data of 61 patients with MSA who underwent both [18F]FP-CIT-PET and [18F]FDG-PET within 3 months of clinical evaluation, and who had ≤3 years of disease duration. Tracer uptake of the striatum on [18F]FP-CIT-PET and glucose metabolism of the striatum on [18F]FDG-PET were analyzed using eight striatal subregional volumes-of-interest templates. The patients were classified into two subgroups according to the predominant pre-synaptic tracer uptake loss of the posterior putamen on [18F]FP-CIT-PET (MSA-SNpc, n = 21) and trans-synaptic dopaminergic dysfunction reflected by both [18F]FP-CIT-PET and [18F]FDG-PET (MSA-STR, n = 40). RESULTS: Parkinsonian features were significantly more severe in the MSA-STR group than in the MSA-SNpc group (P = .005) and cerebellar ataxia was significantly more severe in the MSA-SNpc group (P = .036). The cerebellar type of MSA was significantly more common in the MSA-SNpc group (P = .001). There was no difference in age at onset, disease duration at the time of study, or Mini-Mental Status Examination scores between the groups. CONCLUSIONS: Patients with MSA showed distinct clinical features depending on whether the pattern of nigrostriatal dysfunction was predominantly pre-synaptic or trans-synaptic.

We report a multi-channel optical frequency synthesizer developed to generate extremely stable continuous-wave lasers directly out of the optical comb of an Er-doped fiber oscillator. Being stabilized to a high-finesse cavity with a fractional frequency stability of 3.8 × 10-15 at 0.1 s, the comb-rooted synthesizer produces multiple optical frequencies of ultra-narrow linewidth of 1.0 Hz at 1 s concurrently with an output power of tens of mW per each channel. Diode-based stimulated emission by injection locking is a key mechanism that allows comb frequency modes to sprout up with sufficient power amplification but no loss of original comb frequency stability. Channel frequencies are individually selectable with a 0.1 GHz increment over the entire comb bandwidth spanning 4.25 THz around a 1550 nm center wavelength. A series of out-of-loop test results is discussed to demonstrate that the synthesizer is able to provide stable optical frequencies with the potential for advancing diverse ultra-precision applications such as optical clocks comparison, atomic line spectroscopy, photonic microwaves generation, and coherent optical telecommunications.

Lipid levels in blood are widely used to diagnose and monitor chronic diseases. It is essential to identify the genetic traits involved in lipid metabolism for understanding chronic diseases. However, the influence of genetic traits varies depending on race, sex, age, and ethnicity. Therefore, research focusing on populations of individual countries is required, and the results can be used as a basis for comparison of results of other studies at the cross-racial and cross-country levels. In the present study, we selected lipid-related variants and evaluated their effects on lipid-related diseases in more than 14,000 subjects of three cohorts using the Illumina Human Exome Beadchip. A genome-wide association study was conducted using EPACTs after adjusting for age, sex, and recruitment area. A genome-wide significance cutoff was defined as p < 5E-08 in all the three cohorts. Sixteen variants represented the lipid traits and were classified as vulnerable to borderline hypertriglyceridemia, hyper-LDL-cholesterolemia, or hypo-HDL-cholesterolemia. Moreover, we compared the genetic effects of the 16 variants between ethnic groups and identified the missense variants in apolipoprotein A-V, cholesterol ester transfer protein, and apolipoprotein E as Asian-specific. Our study provides candidate genes as markers for chronic diseases through the evaluation of genetic effects.

BACKGROUND: Among rectal cancer patients, some of good responders after neoadjuvant chemoradiotherapy (nCRT) are considered for non-operative treatments to avoid postoperative morbidities and permanent stoma. However, oncologic feasibility of non-operative treatment has not been fully understood. METHODS: From 2008 to 2017, we retrospectively reviewed patient's records who had lower or mid rectal cancer and diagnosed to clinical complete response by magnetic resonance imaging after nCRT. Clinical differences and oncologic outcomes were compared among Radical surgery (RS), Local excision (LE) and Wait-and-see (WS) group. RESULTS: Number of 129, 25, 15 patients included to RS, LE, WS groups. Local recurrence was frequent type of recurrence in both of LE and WS group (RS; 31.3%, LE; 80%, WS; 66.7%), and many patients in WS group omitted salvage treatment (RS; 75%, LE; 100%, WS; 33.3%). 5-years local-recurrence/disease-free survival rate (LRFS, DFS) between RS and LE were similar between each group, but WS showed significantly inferior outcomes than that of RS (LRFS; p = 0.001, DFS; p = 0.001). In multivariate analysis, WS protocol (OR; 7.163, 95% CI; 1.995-25.715) and cT4 stage (OR; 8.206, 95% CI; 1.596-42.198) were independent factors for LRFS. CONCLUSIONS: Wait-and-see group showed high rate of rejection of salvage treatments for recurrence, and poor oncologic outcomes. However, recent low-level evidences reported favorable outcome of WS protocol when salvage treatment was followed after recurrence. It seems that the application of WS protocol should be postponed until the results of randomized-controlled trials are available. Local excision seems to be good alternative option to radical surgery when salvage treatment is followed.

Wearable and skin-attachable electronics with portable/wearable and stretchable smart sensors are essential for health-care monitoring devices or systems. The property of adhesion to the skin in both dry and wet environments is strongly required for efficient monitoring of various human activities. We report here a facile, low-cost, scalable fabrication method for skin-adhesive graphene-coated fabric (GCF) sensors that are sensitive and respond fast to applied pressure and strain. With octopus-like patterns formed on the side of the GCF that touches the skin, the GCF adheres strongly to the skin in both dry and wet environments. Using these characteristics, we demonstrate efficient monitoring of a full range of human activities, including human physiological signals such as wrist pulse and electrocardiography (ECG), as well as body motions and speech vibrations. In particular, both measurements of ECG and wrist-bending motions were demonstrated even in wet conditions. Our approach has opened up a new possibility for wearable and skin-adherent electronic fabric sensors working even in wet environments for health-care monitoring and medical applications in vitro and in vivo.
