RESUMO

This study investigated the factors associated with new carious lesions in one-to five-year-old children with developmental disabilities. This was a retrospective cohort that evaluated 64 dental charts of individuals with caries or fillings in their first dental appointment. The dependent variable was the occurrence of a new carious lesion or restoration. Gender, age, mother's education, sugar consumption, oral hygiene, mouth breathing, reports of xerostomia, gingival status, use of psychotropic drugs, use of asthma drugs, history of asthma, bronchitis, reflux or seizures and having at least one sibling were covariates. Cox proportional hazards regression model was used to estimate the raw and adjusted hazard ratios with their respective 95% confidence interval. The average time that individuals remained free of dental caries/restoration was 79.49 months (95%CI: 64.37 to 92.61). Increase in sucrose consumption increased the rate of caries recurrence (HR = 1.16; 95%CI: 1.04 to 1.30). Individuals who had poor oral hygiene had higher rate of new dental caries (HR = 3.88; 95%CI: 1.22 to 12.37) compared to those with good oral hygiene. The presence of mouth breathing decreased the rate of recurrence of the disease when compared to the nasal breathing (HR = 0.32; 95%CI: 0.15 to 0.70). Oral health-related behaviors and nasal respiration influenced the rate of dental caries recurrence in individuals with developmental disabilities.

RESUMO

The dentine hypersensitivity (DH) is able to impair the oral health related quality of life (OHRQoL). However, there isn't any specific validated questionnaire to be used in Brazil. The objective was to adapt and to validate the English version of the Dentine Hypersensitivity Experience Questionnaire (DHEQ-15) for use in Brazil. DHEQ-15 was cross-culturally adapted into the Brazilian-Portuguese language and then validated in a cross-sectional study with 100 participants recruited at a University clinic. Study sample comprised 2 groups: 100 individuals with DH, and 100 individuals without. The instrument was self-administered twice 7 to 10 days apart. The participants answered a global rating of oral health. The psychometric properties of the Brazilian version of DHEQ-15 were verified through internal consistency (Cronbach's α) and test-retest reliability (intraclass correlation coefficient - ICC), convergent (Spearman correlation) and discriminant (Mann-Whitney test) validity. The significance threshold was set at p<0.05. Sample comprised 69 men and 131 women, of mean age 30.4y. The Brazilian DHEQ-15 demonstrated very good internal consistency (α = 0.945). Test-retest reliability revealed excellent reproducibility (ICC = 0.959, p < 0.001). There was statistically significant correlation between the scores obtained on all DHEQ-15 domains and the global rating of oral health (p<0.001). Participants with DH scored significantly higher than those without DH (p<0.001). This study provides evidence supporting the cross-cultural validity of the Brazilian version of DHEQ-15 for use in Brazil.

RESUMO

BACKGROUND: The primary stability of dental implants can be evaluated by insertion torque (IT) and resonance frequency analysis (RFA). OBJECTIVE: Assess the supposed relationship between the IT and RFA. MATERIALS AND METHODS: A systematic review was performed based on the PRISMA. The electronic search was performed in the PubMed, Web of Science, SCOPUS, Cochrane Library electronic, OVID, and Scielo databases. Manual searches were also performed. There was no restrictions regarding year of publication or language. The articles identified were assessed independently by 3 trained researchers. Clinical trials reporting the RFA values by means of implant stability quotient (ISQ) and IT were included. RESULTS: The electronic and manual searches yielded 2017 studies. Twelve studies were included in the systematic review. There was no statistically significant correlation between ISQ and IT (rs = .366; P = .079). The quality of the evidence was downgraded by risk of bias and indirectness; and the certainty of the evidence was low. CONCLUSION: IT and RFA are independent and incomparable methods of measuring primary stability. Is important for clinicians to define only one method of evaluation for each implant.

RESUMO

OBJECTIVE: The aim of the present study was to determine how long the effects of laser and cyanoacrylate last when used as treatments for dentine hypersensitivity (DH). In addition, the predictive factors of DH recurrence and the prevalence of DH according to type of tooth were investigated. MATERIALS AND METHODS: Sixty-two patients contributed with 434 sensitive teeth. Two hundred sixteen teeth were randomized to receive laser treatment (wavelength of 795 nm) and 218 received cyanoacrylate treatment. Both treatments were applied three times at intervals of 48 hours. Dentine hypersensitivity was assessed by air stimulus at baseline and 1, 30, 90 and 180 days after treatment by a blinded researcher. Additionally, possible etiologic factors were assessed. The data were analyzed by Kaplan-Meier survival curves and by logistic regression. RESULTS: Both groups had signifi cant reductions in DH. Treatment failure occurred in 276 (63.6%) treated teeth and 158 (36.4%) teeth maintained the result of the treatment until the end of the study (censored teeth). The survival rate at 6 months was 36% for laser treatment, and 36% for cyanoacrylate. There was no signifi cant difference between the survival curves (p = 0.703). Gingival recession and abfraction increased the risk of treatment failure by 2.04 (p = 0.002) and 2.76 (p less than 0.001) times, respectively. CONCLUSIONS: Overall, these two approaches to DH treatment had a survival rate of 36%at 6 months. The most prevalent hypersensitive tooth is the first premolar. Gingival recession and abfraction predict the recurrence of DH irrespective of patient age and sex.

RESUMO

AIM: The aim was to compare the immunoexpression of extracellular matrix proteins in squamous cell carcinomas of tongue (SCCTo) and lower lip (SCCLi). METHODS: Eleven SCCTo and 11 SCCLi were selected and examined according to Bryne's method (1998). For immunohistochemical study utilized antibodies to fibronectin, tenascin and type I collagen. Histopathologic and immunohistochemical analysis were performed on the tumor invasive front. RESULTS: All SCCTo were classified in high score malignant grade and all SCCLi in lower score. Fibronectin showed strong immunorreactivity in the peritumoral basement membrane (BM) in 91% of SCCTo and all cases of SCCLi, while in the tumor stroma (TS) all cases of SCCTo and SCCLi had strong intensity. Tenascin had strong expression in BM of 91% cases of SCCTo and 63.4% of SCCLi and in TS had strong expression in 91% cases of SCCTo and 54.6% of SCCLi. Type I collagen demonstrated weak immunoreactivity in the TS of 72.7% cases of SCCTo and 63.4% of SCCLi. CONCLUSION: These results may suggest that the strong expression of fibronectin and tenascin proteins and the weak expression of type I collagen could play a role in the invasive process of oral SCC (AU)

RESUMO

Resonance frequency analysis (RFA) has become the main tool used to assess the osseointegration of dental implants. The objective of this study was to verify the relationship between the ISQ values with different prosthetic abutments and with the implant platform. The hypothesis was that ISQ values changes according to the abutment height. Twelve patients were included, whose contribution to the study was 31 dental implants (external hexagon connection implants, 4.1x10 mm). The temporary implant-supported crown and prosthetic components were removed and the following smartpegs were inserted, one at a time: type 1, in the implant platform (G1); type A3, in the microunit component with 1mm transmucosal height (G2) and type A3, in the microunit component with 5mm transmucosal height (G3). In all the smartpegs, RFA measurements were taken on mesial, distal, buccal and lingual surfaces. All evaluations were performed by a single calibrated examiner (ICC = 0.989). Data were analyzed by Friedman and Spearman correlation tests and log-linear marginal regression (p<0.05). The mean age of participants was 52.83 (± 3.77) years. There was statistically significant difference (p<0.001) among the mean ISQ of G1 (88.27 ±5.70); G2 (72.75 ±4.73) and G3 (66.33 ±3.67). There was statistically significant negative correlation between the ISQ and the measurement distance (rs:-0.852; p<0.001; R2:0.553). Measurement distance was significantly associated (p<0.001) with ISQ value in the log-linear regression. The abutment height has a significant impact on resonance frequency analysis measurements. The higher the transmucosal abutment height, the lower the implant stability quotient value. Clinically, the ISQ measured on the abutment cannot be compared with values measured on the implant platform.

RESUMO

OBJECTIVE: Temporomandibular disorder (TMD) is a variety of clinical problems that originate from the area of the temporomandibular joint (TMJ), masticatory muscles, and surrounding tissues. There are different treatment options; however, there is no evidence that low level laser treatment (LLLT) will last about six months. The aim of this study was to determine the survival rate of treatment with red (660 nm) and infrared (790 nm) laser in cases of TMDs. METHODS: In 19 subjects, one side of the face (half face) was randomly selected to receive intervention, in a total of 116 sensitive points. Pain was measured at baseline and time intervals of 24 h, 30, 90, and 180 days after treatment. Laser irradiation with 4 j/cm² in the TMJs and 8 j/cm² in the muscles was used in three sessions. Kaplan-Meier survival analysis and logistic regression were performed. RESULTS: Both treatments showed statistically significant results (p < 0.001). The survival rate for red and infrared laser was 0.24 and 0.30, respectively, at 180 days. Grinding teeth and headache were associated with recurrent pain. DISCUSSION: Both lasers were effective in the treatment of TMD symptoms and had a low survival rate at 180 days.

RESUMO

OBJECTIVE: This study aimed to evaluate the effect of initial periodontal clinical parameters on the final outcome of the treatment of periodontal recessions by means of a coronally positioned flap (CPF). MATERIALS AND METHODS: The CPF technique was used in a total of 39 Miller Class I/II gingival recessions of the upper canines and/or premolars. The clinical periodontal parameters evaluated were: probing depth, gingival recession height, gingival recession width, keratinized mucosa, attached keratinized mucosa, flap thickness and free gingival margin thickness. The degree of root coverage was evaluated 6 months and 1 year after the surgery. The data were analyzed by Student's t-test, Mann-Whitney and Pearson's correlation tests, as well as linear regression. RESULTS: A significant reduction in gingival recession height, gingival recession width, keratinized mucosa, and attached keratinized mucosa were observed. A significant negative correlation was found between gingival recession and root coverage percentage at 1 year, and a significant negative correlation between gingival margin thickness and coverage percentage at 1 year. The gingival recession and flap thickness significantly predicted the amount of root coverage. CONCLUSIONS: The initial dimensions of gingival recession and the mucosa thickness were correlated to the percentage of root coverage. The gingival recession width was the major predictor of root coverage.

OBJECTIVE: This study aimed to perform a review of the literature regarding the survival rate of dental implants with immediate loading using insertion torque of 30 Ncm. MATERIAL AND METHODS: A systematic review was performed based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and the Cochrane Handbook for Systematic Reviews of Interventions (PROSPERO CRD42014015323). The search was performed in the PubMed, Web of Science, Cochrane Library electronic, OVID, and Scielo databases. Manual searches were also performed. The articles identified were assessed independently by 3 researchers. Clinical trials reporting dental implants with immediate loading and 30 Ncm torque in patients ages 18 years or older were included. RESULTS: The searches yielded 589 studies. Six studies were included in the systematic review. The survival rate of dental implants was 96.8%. Three studies showed a low potential risk of bias. CONCLUSION: There is not strong evidence that insertion torque of 30 Ncm is enough for implant survival in cases of immediate loading.

RESUMO

OBJECTIVE: The aim of this study was to evaluate the influence of periodontal clinical parameters on the treatment of gingival recession by means of a coronally positioned flap (CPF). MATERIALS AND METHODS: In this clinical study, the coronally positioned flap technique was used in a sample of 14 individuals with Miller's Class I or II gingival recession of the canines and/or maxillary premolars, totaling 39 recessions. The clinical periodontal parameters evaluated were: probing depth (PD), gingival recession height (GRH), gingival recession width (GRW), clinical attachment level (CAL), keratinized mucosa (KM), attached keratinized mucosa (AKM), gingival thickness (GT) and free gingival margin thickness (GMT). The degree of root coverage was evaluated 3 months after the surgical procedures were performed. The data were analyzed using Student's-t, Mann-Whitney and Pearson's correlation tests, as well as the Cohen's d. RESULTS: After root coverage, there was no significant change in PD. Significant reductions in GRH and GRW were observed. In addition, a significant reduction in KM and AKM, and an increase in GMT were also observed. A significant negative correlation was found between initial GMT and gingival recession at 3 months, and a significant negative correlation between GT and periodontal recession at 3 months. There was large effect size for GRH, GRW, CAL, KM, AKM and GMT parameters. CONCLUSIONS: The initial gingival recession height, keratinized mucosa and free gingival margin thickness were the parameters that had the most influence on root coverage by the coronal sliding flap technique.

Although implant-supported prosthesis and prosthesis over natural teeth share several characteristics some aspects should be distinguished due to the different biomechanical behavior between implants and teeth. Occlusion is one of these aspects and is one of the main factors responsible for the success of osseointegration. Considering the diversity of treatments that can be performed with implants and aware that there is still no consensus on the ideal occlusal pattern for these prostheses, the objective of the study was to conduct a literature review seeking to address the occlusal aspects relevant to the success of treatment with prosthetic implants.
