BACKGROUND: Muscle stretching exercises preserve corporal flexibility and decrease the retraction and shortening of myofascial and articular structures. These exercises are recommended for the treatment of fibromyalgia (FM). The purpose of the study was to verify and compare the effect of muscle stretching exercises on FM patients based on the global posture reeducation method against segmental muscle stretching exercises, both used in concert with an educational approach rooted in cognitive behavioral therapy. METHODS: Forty adults with FM were randomly allocated into two groups: global and segmental. The two kinds of therapies were performed in 10 individual sessions once a week. Two assessments were made: one at baseline and one at the end of therapy. The primary outcome variable was pain intensity (Visual Analog Scale). The secondary outcome variables were multidimensional pain (McGill Pain Questionnaire), the pain threshold at tender points (dolorimetry), attitudes toward chronic pain (Survey of Pain Attitudes-Brief Version), body posture (Postural Assessment Software Protocol), postural control (Modified Clinical Test of Sensory Interaction on Balance), flexibility (sit-and-reach test), the impact of FM on quality of life (Fibromyalgia Impact Questionnaire, FIQ), and self-reported perceptions and body self-care. RESULTS: At the end of treatment, there were no statistically significant differences between the groups in the outcome variables. Furthermore, the groups presented lower pain intensity (baseline vs. final; global group: 6 ± 1.8 vs. 2.2 ± 1.6 cm, p<0.01; segmental group: 6.3 ± 2.1 vs. 2.5 ± 1.7 cm, p<0.01), higher pain threshold (p ≤ 0.01), lower total FIQ score (p < 0.01), and greater postural control (p < 0.01) after treatment. CONCLUSIONS: Muscle stretching exercises based on global posture reeducation and segmental muscle stretching exercises, both used in concert with an educational approach rooted in cognitive behavioral therapy, reduced the pain intensity and impact of FM on quality of life. These exercises also improved FM patients' pain threshold at tender points, attitudes toward chronic pain, and postural control. There were no differences between global posture reeducation and segmental muscle stretching exercises. TRIAL REGISTRATION: ClinicalTrials.gov NCT02384603 . Registered on 10 March 2015.

The aim of this study was to estimate the prevalence of low-back pain (LBP) and to identify the level of functional disability in elderly individuals in different populations. From January 1985 to October 2018, a search was performed using the following databases: Embase, LILACS, SciELO, Scopus, Medline, and the Web of Science. The descriptors were low-back pain, back pain, lower-back pain, prevalence, and elderly in Portuguese and English. Two independent reviewers conducted a search for studies and evaluated their methodological quality. The search strategy returned 2186 titles, and 35 were included in this review. The studies evaluated 135,059 elderly individuals aged between 60 and 102 years, and the prevalence of LBP ranged from 21% to 75%. The levels of functional disability, as well as functional difficulties, activities of daily living, and physical capacity, were identified in 60% of the studies. This review indicated a high prevalence of LBP in elderly individuals and functional disability that affects factors important for independence. However, the studies used different methodologies, suggesting that more studies be conducted with scientific accuracy, methodological quality, and low risk of bias to contribute to the proposal of preventive actions for elderly populations.

The new 2010 American College of Rheumatology criteria have not been widely used yet and the COPCORD (Community-oriented program for control of Rheumatic Diseases) methodology has increased the quality of studies on the prevalence of rheumatic diseases in general.


RESUMO O presente estudo teve como objetivo atualizar a revisão de literatura sobre a prevalência da fibromialgia (FM) publicada em 2006. Foi feito levantamento bibliográfico do período de 2005 a 2014 nas bases de dados Medline, Web of Science, Embase, Lilacs e SciELO e identificaram-se 3.274 registros. Cinco pesquisadores selecionaram os estudos, de acordo com os critérios de inclusão: estudos que obtiveram a prevalência da FM. Foram excluídos estudos da FM em doenças. Na triagem pelo título e resumo, foram excluídos 2.073 artigos irrelevantes. Foram avaliados quanto à elegibilidade os textos completos de 210 artigos, incluíram-se nesta revisão 39 estudos, descritos em 41 artigos. Os estudos selecionados foram agrupados em quatro categorias: a) prevalência da FM na população em geral; b) prevalência da FM em mulheres; c) prevalência da FM em áreas rurais e urbanas; d) prevalência da FM em populações especiais. A literatura aponta valores de prevalência da FM na população em geral entre 0,2 e 6,6%, em mulheres entre 2,4 e 6,8%, nas áreas urbanas entre 0,7 e 11,4%, nas rurais entre 0,1 e 5,2% e em populações especiais entre 0,6 e 15%. Esta atualização de revisão de literatura mostra um aumento expressivo de estudos de prevalência da FM ao redor do mundo. Os novos critérios do Colégio Americano de Reumatologia de 2010 foram ainda pouco usados e a metodologia Copcord (Programa Orientado para a Comunidade para Controle de Doenças Reumáticas) aumentou a qualidade dos estudos de prevalência de doenças reumáticas em geral.

The systematic review aimed to evaluate the effectiveness of massage in fibromyalgia. An electronic search was conducted at MEDLINE, SCiELO, EMBASE, ISI, PEDro, SPORTDiscus, CINAHL, Cochrane CENTRAL and LILACS (Jan 1990-May 2013). Ten randomized and non-randomized controlled trials investigating the effects of massage alone on symptoms and health-related quality of life of adult patients with fibromyalgia were included. Two reviewers independently screened records, examined full-text reports for compliance with the eligibility criteria, and extracted data. Meta-analysis (pooled from 145 participants) shows that myofascial release had large, positive effects on pain and medium effects on anxiety and depression at the end of treatment, in contrast with placebo; effects on pain and depression were maintained in the medium and short term, respectively. Narrative analysis suggests that: myofascial release also improves fatigue, stiffness and quality of life; connective tissue massage improves depression and quality of life; manual lymphatic drainage is superior to connective tissue massage regarding stiffness, depression and quality of life; Shiatsu improves pain, pressure pain threshold, fatigue, sleep and quality of life; and Swedish massage does not improve outcomes. There is moderate evidence that myofascial release is beneficial for fibromyalgia symptoms. Limited evidence supports the application of connective tissue massage and Shiatsu. Manual lymphatic drainage may be superior to connective tissue massage, and Swedish massage may have no effects. Overall, most styles of massage therapy consistently improved the quality of life of fibromyalgia patients.

Introdução: Os exercícios físicos têm sido benéficos para o controle dos sintomas da fibromialgia (FM).Objetivos: Comparar os efeitos de exercícios de alongamento muscular com os exercícios aeróbicos na dor, número de tender points, sono, ansiedade e depressão de pacientes com FM. Materiais e métodos:Participaram do estudo 19 pacientes com diagnóstico de FM, segundo critérios do Colégio Americano de Reumatologia, divididos em dois grupos: grupo alongamento, com 12 pacientes, que realizou um programa de 8 sessões de 45 minutos de exercícios de alongamento, uma vez a cada semana; grupo aeróbico com 7 pacientes, que realizou caminhada em esteira ergométrica por 30 minutos. Ambos foram orientados a realizar em exercícios em casa nos demais dias. A dor foi avaliada pela escala analógica visual; o limiar de dor dos tender points com o dolorímetro; sono pelo Post Sleep Inventory; ansiedade pelo Inventário de Ansiedade Traço-Estado e depressão pela Escala de Depressão de Beck. Resultados: Os dados mostram que o ganho clínico foi maior no grupo alongamento em relação à dor (25%), número de tender points (15%), sono(38%) e depressão (22%) ao se comparar com o grupo aeróbico (ganho clínico de 2% na dor e 4% no sono; piora de 3% no número de tender points e 18% na depressão). O ganho clínico da ansiedade foi mais importante no grupo aeróbico (8% no traço de ansiedade e 10% no estado de ansiedade) em comparação ao grupo alongamento (piora de 3% no traço de ansiedade e melhora de 2% no estado de ansiedade). Conclusão: Este estudo sugere que os exercícios de alongamento são mais eficazes que os exercícios aeróbicos na dor, número de tender points, sono e depressão da FM. Os exercícios aeróbicos parecem produzir um efeito mais importante na diminuição da ansiedade em comparação aos exercícios de alongamento


INTRODUCTION: The physical exercises have been beneficial for the control of the fibromyalgia (FM) symptoms.OBJECTIVES: To compare the effects of stretching exercises to aerobic exercises on pain, number of tender points, sleep, anxiety and depression of patients with FM. MATERIALS AND METHODS: Nineteen patients with diagnosis of FM according to the American College of Rheumatology criteria were divided into two groups: the stretching group, with 12 patients, performed a program composed by 45 minutes eight sessions of stretching exercises once a week, and the aerobic group, with seven patients, carried out walking on a treadmill for 30 minutes. Both groups received orientation to exercise at home on the other days. Pain intensity, tenderness threshold at the tender points, sleep, anxiety and depression were evaluated by, respectively, visual analogic scale, dolorimeter, Post Sleep Inventory (PSI), Trait-State Anxiety Inventory (IDATE) and Beck Depression Scale (BDS). RESULTS: The data show that the clinical improvement was higher in the stretching group in relation to pain (25%), number of tender points (15%), sleep (38%) and depression (22%), compared to aerobic group (clinical improvement in pain was 2% and in sleep, 4%; the number of tender points and depression became worse, respectively 3% and 18%). The anxiety clinical improvement was more important in the aerobic group (8% in the trait anxiety and 10% in the state anxiety) compared to stretching group (the trait anxiety become worse at 3% and the state anxiety clinical improvement was 2%). CONCLUSION: This study suggests that the stretching exercises are more effective than the aerobic exercises on pain, number of tender points, sleep and depression in FM. On the other hand, the aerobic exercises seem to produce a more important effect on anxiety reduction compared to stretching exercises.

Introdução: A fibromialgia é uma síndrome caracterizada por dor crônica e generalizada e sensibilidade dolorosa à palpação em tender points. É comum estar associada também a fadiga, rigidez matinal, distúrbios do sono, ansiedade e depressão. Os profissionais da área de saúde têm utilizado as terapias alternativas e complementares em tratamentos multidisciplinares, e a massoterapia vem apresentando evidências de eficácia no tratamento da fibromialgia. Objetivo: Este estudo teve como objetivo verificar o efeito de duas técnicas na dor, ansiedade e qualidade de vida de duas mulheres fibromiálgicas. Método: Ambas foram avaliadas antes e após o tratamento, com relação ao limiar de dor dos tender points com a dolorimetria, intensidade da dor com a Escala Visual Analógica, ansiedade com o Inventário de Ansiedade Traço-Estado e qualidade de vida com o Questionário de Impacto da Fibromialgia. Os sujeitos foram submetidos a oito sessões de massagem, um com a técnica de fricção em oito tender points específicos, e o outro com deslizamento superficial e profundo na região onde esses tender points estão localizados. Resultados: Nos dois casos, houve aumento do limiar de dor nos tender points, inclusive nos que não foram diretamente massageados, redução da intensidade da dor e do impacto da fibromialgia na qualidade de vida. No Sujeito 1, houve redução da ansiedade estado, enquanto a ansiedade-traço permaneceu inalterada, e o Sujeito 2 apresentou redução apenas na ansiedade traço. Conclusão: Ambas as técnicas foram eficazes no tratamento dos sujeitos com fibromialgia, aumentando o limiar de dor nos tender points, diminuindo a intensidade da dor e a ansiedade e melhorando a qualidade de vida, com resultados mais expressivos na massagem de deslizamento superficial e profundo. Devido à ínfima amostra, sugerese a realização de estudos controlados de boa qualidade com número maior de sujeitos.


Introduction: Fibromyalgia is a syndrome characterized by chronic widespread pain and tenderness on palpation in tender points. Commonly it is also associated with fatigue, morning stiffness, sleep disturbances, anxiety and depression. Frequently, health care professionals have been using complementary and alternative therapies, and massage therapy has been showing evidences of efficacy in the treatment of fibromyalgia. Objective: This study has the goal of verifying the effect of two techniques on the pain, anxiety and quality of life of two women with fibromyalgia. Method:80th subjects were evaluated before and after the treatment, regarding the pain threshold of tender points with dolorimetry, pain intensity with the Visual Analogue Scale, anxiety with the State-Trait Anxiety Inventory, and quality of life with the Fibromyalgia Impact Questionnaire. The subjects received eight sessions of massage, one of them with friction technique on eight specific tender points, and the other with superficial and deep sliding in the area where these tender points are located. Results: In these cases, there was an increase of pain threshold of tender points, including those that were not directly massaged, and a decrease of pain intensity and impact of fibromyalgia over quality of life. In Subject 1, there was a reduction of state anxiety, as trait anxiety remained unchanged, and Subject 2 presented reduction only intraitanxiety. Conclusion: 80th techniques were efficient in the treatment of the subjects with fibromyalgia, increasing the pain threshold of tender points, decreasing the pain intensity and anxiety and improving the quality of life. The results were more expressive with the superficial and deep sliding massage. Due to the insufficient sample, it is suggested that well-designed controlled studies with a greater number of subjects should be undertaken.

OBJETIVO: Desenvolver uma versão transcultural do Fibromyalgia Impact Questionnaire (FIQ) para a população brasileira e analisar sua validade e eficácia quando aplicado em pacientes com fibromialgia. PACIENTES E MÉTODOS: Participaram do estudo 44 pacientes com fibromialgia (FM), diagnosticados segundo os critérios do American College of Rheumatology (ACR), 1990. Baseados nas orientações de Guillemin et al(22), foram convidados quatro professores de língua inglesa, um reumatologista e dois fisioterapeutas. O procedimento seguiu as etapas: tradução inicial por dois professores de inglês, avaliação das duas traduções para uma versão única, versão para a língua inglesa por dois professores de inglês nativos, reunião de consenso com dois professores de inglês, reumatologista e fisioterapeutas para versão teste, avaliação da equivalência cultural, versão final, avaliação da confiabilidade e reprodutibilidade. A versão teste foi aplicada em 20 pacientes com FM, tendo em todas as questões o item "não-aplicável". Substituição de possíveis questões com mais de 15 por cento de respostas "não-aplicável" por outras de mesmo conceito, resultando na versão final. Aplicação dessa versão em 24 pacientes com FM por dois avaliadores que fizeram a entrevista no mesmo dia com intervalo de uma hora e, após um período de sete dias da primeira avaliação, o questionário foi reaplicado pelo primeiro avaliador. RESULTADOS: Na aplicação da versão teste não houve questões com mais de 15 por cento de respostas "não-aplicável". Sendo assim, não foi mudado o texto para a versão final. Porém, houve dificuldade de compreensão das escalas visuais analógicas (questões 4 a 10). Com isso, foram acrescidas "carinhas" nos dois extremos: à esquerda uma "carinha" feliz e à direita, uma infeliz. Esse processo deu origem à versão final. Na avaliação da confiabilidade, os resultados do avaliador 1 (primeira e segunda aplicação) e do avaliador 2 foram comparados, bem como as duas aplicações do avaliador

INTRODUÇAO E OBJETIVO: este trabalho teve como objetivo realizar uma revisão da literatura sobre a prevalência da fibromialgia (FM) na população a partir dos critérios propostos pelo American College of Rheumatology (ACR). MÉTODOS: foi realizado levantamento bibliográfico do período de 1990 a 2005 nas bases de dados MedLine, Lilacs, Embase e ISI. Foram utilizadas as palavras-chave "fibromialgia" e "prevalência" e as correspondentes em inglês, "fibromyalgia" e "prevalence". Foram selecionados 97 artigos e, após leitura dos resumos, foram excluídos os que se referiam à prevalência em doenças. Somente 30 abordavam o tema prevalência da fibromialgia na população. Os artigos selecionados foram agrupados em cinco categorias: a) prevalência da FM em populações adultas; b) prevalência da FM em mulheres; c) prevalência da FM em crianças e adolescentes; d) prevalência da FM em populações específicas; e) prevalência de dor crônica e difusa na população, segundo os critérios do ACR. RESULTADOS: a literatura aponta a prevalência da FM na população com valores entre 0,66 e 4,4 por cento, sendo mais prevalente em mulheres do que em homens, especialmente na faixa etária entre 35 e 60 anos. Os estudos com crianças e adolescentes e em grupos especiais são escassos e pouco conclusivos. A prevalência de dor crônica difusa na população em geral também tem poucos estudos, com valores entre 11 e 13 por cento. CONCLUSAO: mais estudos sobre prevalência de dor crônica e difusa devem ser estimulados, assim como os de prevalência na população adulta, crianças e jovens.

CONTEXTO: Fibromialgia é uma síndrome reumática caracterizada por dores músculo-esqueléticas difusas e crônicas e sítios dolorosos específicos à palpação, chamados de tender points, freqüentemente associados a fadiga, distúrbios do sono, rigidez matinal e, em alguns casos, dispnéia e ansiedade. Devido ao seu caráter crônico, a síndrome geralmente causa impacto negativo na qualidade de vida dos fibromiálgicos. OBJETIVO: Comparar a eficácia de instrumentos que avaliam a qualidade de vida de fibromiálgicos mensurada pelo Fibromyalgia Impact Questionnaire(FIQ) e pelo Medical Outcomes Study 36-item Short-Form Healthy Survey (SF-36), e a ansiedade avaliada pelo Inventário de Ansiedade Traço-estado (IDATE). TIPO DE ESTUDO: Transversal. LOCAL: Ambulatório de Reumatologia do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HC/FMUSP). MÉTODOS: Participaram do estudo 80 sujeitos: 40 com fibromialgia (grupo teste) e 40 saudáveis (grupo controle). Três questionários (dois para avaliação de qualidade de vida - FIQ e SF-36 û; e um para ansiedade - IDATE) foram aplicados aos indivíduos dos dois grupos em uma única entrevista. Toda a análise estatística foi realizada utilizando-se o Teste "t" Student e o teste de Correlação de Pearson (r), com significância p < 0,05. Além disso, o teste estatístico Qui-quadrado de Pearson, para homogeneidade, foi usado para comparar o grau de escolaridade entre os grupos teste e controle. RESULTADOS: Os resultados obtidos mostram que houve diferença estatisticamente significante entre os grupos (p = 0,00), indicando que os fibromiálgicos têm pior qualidade de vida e níveis mais altos de ansiedade. A correlação entre os três questionários foi alta (r = 0,90). DISCUSSAO: O impacto negativo na qualidade de vida decorrente da Fibromialgia tem sido relatado em muitos estudos, nos quais os protocolos de avaliação são os principais istrumentos de medida. O FIQ é um instrumento utilizado em vários estudos clínicos para avaliar a função física. Este estudo comprovou a eficiência do FIQ para avaliar o impacto da fibromialgia sobre a qualidade de vida. CONTEXT: Fibromyalgia is a syndrome characterized by chronic, diffuse musculoskeletal pain, and by a low pain threshold at specific anatomical points. The syndrome is associated with other symptoms such as fatigue, sleep disturbance, morning stiffness and anxiety. Because of its chronic nature, it often has a negative impact on patients' quality of life. OBJECTIVE: To assess the quality of life and anxiety level of patients with fibromyalgia. TYPE OF STUDY: Cross-sectional. SETTING: Rheumatology outpatient service of Hospital das Clínicas (Medical School, Universidade de São Paulo). METHODS: This study evaluated 80 individuals, divided between test and control groups. The test group included 40 women with a confirmed diagnosis of fibromyalgia. The control group was composed of 40 healthy women. Three questionnaires were used: two to assess quality of life (FIQ and SF-36) and one to assess anxiety (STAI). They were applied to the individuals in both groups in a single face-to-face interview. The statistical analysis used Student's t test and Pearson's correlation test (r), with a significance level of 95%. Also, the Pearson chi-squared statistics test for homogeneity, with Yates correction, was used for comparing schooling between test and control groups. RESULTS: There was a statistically significant difference between the groups (p = 0.000), thus indicating that fibromyalgia patients have a worse quality of life and higher levels of anxiety. The correlations between the three questionnaires were high (r = 0.9). DISCUSSION: This study has confirmed the efficacy of FIQ for evaluating the impact of fibromyalgia on the quality of life. SF-36 is less specific than FIQ, although statistically significant values were obtained when analyzed separately, STAI showed lower efficacy for discriminating the test group from the control group. The test group showed worse quality of life than did the control group, which was demonstrated by both FIQ and SF-36. Even though STAI was a less efficient instrument, it presented significant results, showing that fibromyalgia patients presented higher levels of anxiety, both on the state and trait scales. Thus, patients with fibromyalgia had higher levels of tension, nervousness, preoccupation and apprehension, and higher propensity towards anxiety. CONCLUSION: The three instruments utilized showed efficiency in evaluating fibromyalgia patients. FIQ was found to be the most efficient instrument for discriminating and assessing the impact of fibromyalgia on their quality of life. It can be concluded that such patients have a worse quality of life and higher levels of anxiety.

Objetivo: avaliar a dor, o nível de estresse e a incidência de eventos estressantes em pacientes com fibromialgia. Métodos: participaram do estudo 63 indivíduos do sexo feminino, sendo 32 do grupo teste (pacientes com fibromialgia) e 31 do grupo controle (indivíduos sem fibromialgia). O limiar de dor foi avaliado nos 18 tender points com auxílio de um algômetro. Também foi utilizado o Questionário de dor da McGill. Para a avaliação do estresse foram aplicados dois protocolos: Lista de Sintomas de Estresse e Inventário de Eventos de Vida Estressantes. Os sujeitos foram avaliados individualmente em uma única entrevista. Foi utilizado o teste de Kruskal-Wallis para avaliar a significância dos dados. Resultados: houve diferença estatisticamente significante entre os grupos quanto à dor. A avaliação da dor pelo Questionário de dor da McGill demonstrou que o grupo controle (GC) selecionou somente descritores (palavras que qualificam a dor) das categorias sensorial e afetiva, sendo os de maior escore “latejante” (45,2%) e “pontada” (45,2%), ambos da categoria sensorial. No grupo teste (GT) a freqüência das palavras selecionadas foi maior e foram escolhidos descritores em todas as categorias (sensorial, afetiva, avaliativa e mista). O escore mais alto do GT (81,3%) foi da palavra “cansativa” (categoria afetiva). A soma dos valores associados aos descritores escolhidos em cada categoria do Questionário de dor da McGill demonstrou que os grupos teste e controle diferiram nas categorias sensorial, afetiva, avaliativa e mista. Em relação à sensibilidade dolorosa nos tender points também houve diferença estatisticamente significante, sendo que o GT apresentou limiar de dor mais baixo que o GC. As pacientes com fibromialgia apresentaram maior nível de estresse do que o grupo controle, porém, o número de eventos de vida estressantes não diferiu de forma significativa entre os dois grupos. Conclusões: as pacientes com fibromialgia apresentaram maior número de descritores escolhidos em todas as categorias do Questionário de dor da McGill e limiar de dor mais baixo em todos os tender points. Observou-se que, apesar das pacientes com fibromialgia apresentarem maior nível de estresse, o número de eventos de vida estressantes não está significativamente aumentado, sugerindo que o problema seja a forma como esta paciente lida com o estresse e não a intensidade dos eventos que ela vivência.
