RESUMO

OBJECTIVE: evaluate the profile of the graduates of Nursing a public college from the perception of skills developed during graduation and the process of professional insertion. METHOD: quantitative, exploratory and descriptive study. The sample was composed of 216 graduates. The data was collected by a validated questionnaire and sent to a population of 470 egresses via electronic mail. For the analysis of the data, frequencies, mean and standard deviation were applied and, for the correlation, the chi-square test. RESULTS: the majority of the participants were female (88%) and the mean age was 29.62 years. The majority (65%) had an employment relationship, 14% worked in a single institution and 48% started working six months after graduation. Regarding the form of work, 56% work in care, with an average of 4.5 minimum wages and a weekly workload between 37 and 44 hours. The majority reported competence acquisition to practice the profession, assisting the patient in his integrality with ethics and applying technical and scientific concepts in care. CONCLUSION: the study made it possible to describe the singularities of nurses' education, their insertion in the world of work and the impact on the educational institution, as well as the presentation of specific competences from the perspective of the graduates themselves.

RESUMO

OBJECTIVE: To understand the education/training of nurses working in an intensive care unit. METHOD: Case study with qualitative approach, with an intentional sample. Data collection and analysis used different research techniques, mainly document analysis, interview and field observation. RESULTS: The data highlights feelings of well-being, satisfaction and motivation as important for education and training in a work context. Some organizational practices seem to promote interpersonal relationships and, consequently, increase the willingness of these professionals to adopt a reward perspective regarding continuing education and training, establishing a close relationship between the formal, the non-formal and the informal. FINAL CONSIDERATION: The attractiveness of this organization is related to the valorization and recognition that it can offer to the professionals. There is a reciprocity between a hospital that endorses up-to-date care and professionals who seek scientific evidence.

Objetivo: Investigar a perspectiva do circulante de sala operatória sobre a contagem cirúrgica para a segurança dos pacientes. Método: Estudo exploratório, qualitativo, realizado em um centro cirúrgico de um hospital de grande porte do Nordeste do Brasil, entre janeiro e março de 2018. Foi realizada Análise de Conteúdo das entrevistas realizadas com 11 circulantes de sala operatória por saturação teórica. Resultados: As falas foram organizadas em duas categorias temáticas ­ "Contagem cirúrgica para a segurança do paciente" e "Falhas no protocolo de contagem cirúrgica" ­ , revelando que a mudança da prática deve ser investida, visto que a maioria entende ou reconhece a importância da contagem cirúrgica para os pacientes. Conclusão: Há o reconhecimento empírico da relevância da contagem cirúrgica, devendo ser reforçada por meio de ações que garantam a compreensão da dimensão e a repercussão dessa prática na segurança dos pacientes.


Objective: To investigate the perspective of the operating room circulating nurse on surgical count for patient safety. Method: Qualitative exploratory study, performed at the surgical center of a large hospital in the Northeast of Brazil, between January and March 2018. We carried out a Content Analysis of the interviews conducted with 11 operating room circulating nurses using theoretical saturation. Results: The responses were organized in two thematic categories ­ "Surgical count for patient safety" and "Flaws in the surgical count protocol" ­, revealing the need to invest in practice change, as most interviewees understood or acknowledged the importance of surgical count for patients. Conclusion: The relevance of surgical count is empirically recognized and must be strengthened through actions that guarantee the understanding of the extent and impact of this practice on patient safety


Objetivo: Investigar la perspectiva del circulante de quirófano sobre el conteo quirúrgico para la seguridad de los pacientes. Método: Estudio exploratorio, cualitativo, realizado en un centro quirúrgico de un hospital de grande porte del Nordeste de Brasil, entre enero y marzo de 2018. Fue realizado Análisis de Contenido de las entrevistas realizadas con 11 circulantes de quirófano por saturación teórica. Resultados: Las conversas fueron organizadas en dos categorías temáticas ­ "Conteo quirúrgico para la seguridad del paciente" y "Fallas en el protocolo de conteo quirúrgico" ­, revelando que el cambio de la práctica debe ser invertida, visto que la mayoría entiende o reconoce la importancia del conteo quirúrgico para los pacientes. Conclusión: Hay el reconocimiento empírico de la relevancia del conteo quirúrgico, debiendo ser reforzada por medio de acciones que garanticen la comprensión de la dimensión y la repercusión de esa práctica en la seguridad de ldos pacientes

RESUMEN Objetivo: Entender la educación/formación de enfermeros que trabajan en un centro de cuidados intensivos. Método: se trata de una investigación cualitativa, en la que se ha recurrido, partiendo de una muestra intencional, al estudio de caso a modo de investigación. Para la recolección y el tratamiento de los datos se utilizaron diversas técnicas investigativas, con preponderancia del análisis documental, el interrogatorio mediante entrevista y la observación en el terreno. Resultados: Los datos denotan valores de bienestar, satisfacción y motivación de parte de los enfermeros para la educación y formación en el contexto laboral. Algunas prácticas organizacionales parecen promover las relaciones interpersonales y, como consecuencia, estos profesionales se disponen a adoptar dinámicas formativas permanentes, resultando en una relación estrecha entre lo formal, lo no formal y lo informal. Consideraciones Finales: La capacidad de atracción que el hospital tiene sobre los profesionales surge de la valorización y del reconocimiento que dicha organización demuestra. Existe una reciprocidad entre el hospital que legitima la atención actualizada y los profesionales que buscan la evidencia científica.


RESUMO Objetivo: Compreender a educação/formação dos enfermeiros que trabalham numa unidade de cuidados intensivos. Método: Pesquisa qualitativa, recorrendo-se ao modo de investigação estudo de caso, a partir de uma amostra intencional. Para a coleta e tratamento dos dados, fez-se uso de diversas técnicas de pesquisa, privilegiando a análise documental, o inquérito por entrevista e a observação no terreno. Resultados: Os dados evidenciam valores de bem-estar, satisfação e motivação dos enfermeiros para a educação e formação em contexto de trabalho. Algumas práticas organizacionais parecem promover as relações interpessoais e, consequentemente, a disposição dos profissionais para adotar dinâmicas formativas permanentes como recompensa, instituindo-se uma estreita relação entre o formal, o não formal e o informal. Considerações Finais: A capacidade de atração dos profissionais ao hospital está na valorização e reconhecimento que a organização consegue oferecer. Existe uma reciprocidade entre o hospital que legitima a assistência atualizada e os profissionais que procuram a evidência científica.

OBJECTIVES: The aim of this project was to develop and implement strategies to promote standardization and formal documentation of increased bleeding within the first 24 h after birth, in compliance with best practice. INTRODUCTION: Official data from Brazil shows a maternal mortality rate of around 52 to 75 deaths per 100,000 live births, 8% of which are caused by hemorrhage. Early diagnosis is promoted through standardization and formal documentation of the first signs of hemorrhage. METHODS: The current implementation project used the Joanna Briggs Institute Practical Application of Clinical Evidence System, a baseline audit was conducted on a sample size of 108 patients and 67 nursing staff. Based on the results of the baseline audit, strategies to address non-compliance were developed and implemented. A follow-up audit was conducted on a sample size of 110 patients and 68 nursing staff, using the same audit criteria. RESULTS: The baseline audit showed poor compliance with evidence-based best practice in every criterion. The implemented protocol for postpartum hemorrhage was well received by the nursing staff, promoting early diagnosis of increased bleeding within 24 h after birth and providing support for further medical treatment. There were significant improvements in every best practice criterion in the follow-up audit. CONCLUSION: The current implementation project managed to improve the nursing staff professional practice, reducing the absence of records while promoting greater awareness of the first signs of primary postpartum hemorrhage.

OBJECTIVE: To present the experience of elaboration and implementation of the Active Teaching Model to Promote Critical Thinking (MEAPC), associated to Problem-Based Learning (PBL), for undergraduate students in Nursing. METHOD: Case report on the experience of the educational intervention (MEAPC + PBL) with undergraduate students in Nursing, in a 20-hour course on Basic Life Support (BLS). The MEAPC was validated by judges to guide the analysis of clinical cases. Critical Thinking (CT) skills were assessed using the California Critical Thinking Skills Test. RESULT: The educational intervention took place in two phases: elaboration and implementation, allowing not only the production of knowledge about BLS, but also the development of CT and exchange of experiences for teaching-learning. CONCLUSION: The association of the MEAPC to the PBL in the course of BLS organized the learning, gave opportunity to acquire knowledge and to stimulate the skills of the CT.

Método: Reporte de experiencia en la intervención educativa (MEAPC + PBL) con estudiantes de grado en Enfermería, de una materia de 20 horas sobre Soporte Básico de Vida (SBV). El MEAPC fue validado por jueces con el objetivo de orientar el análisis de casos clínicos. Las habilidades de Pensamiento Crítico (PC) fueron evaluadas por el instrumento California Critical Thinking Skills Test. Resultado: La intervención educativa ocurrió en dos fases: la elaboración y la implementación, y permitió no sólo la producción de conocimiento sobre SBV, sino también el desarrollo del PC y el intercambio de experiencias para la enseñanza-aprendizaje. Conclusión: La asociación del MEAPC al PBL en la materia de SBV organizó el aprendizaje, posibilitó la adquisición de conocimientos y el estímulo a las habilidades del PC.

As habilidades de Pensamento Crítico (PC) foram avaliadas pelo instrumento California Critical Thinking Skills Test. Resultado: A intervenção educativa ocorreu em duas fases: elaboração e implementação, permitindo não apenas a produção de conhecimento sobre SBV, mas também o desenvolvimento do PC e troca de experiências para o ensino-aprendizado. Conclusão: A associação do MEAPC ao PBL em curso de SBV organizou o aprendizado, oportunizou aquisição de conhecimentos e o estímulo às habilidades do PC.

Objetivo: avaliar o perfil dos egressos de Enfermagem uma faculdade pública a partir da percepção de competências desenvolvidas durante a graduação e do processo de inserção profissional. Método: estudo quantitativo, exploratório e descritivo. Compôs-se a amostra por 216 egressos. Os dados foram coletados por questionário validado e encaminhado a uma população de 470 egressos via correio eletrônico. Para a análise dos dados, aplicaram-se frequências, média e desvio-padrão e, para a correlação, o teste qui-quadrado. Resultados: a maioria dos participantes era do sexo feminino (88%) e a média de idade foi de 29,62 anos. A maioria (65%) possuía vínculo empregatício, 14% trabalham em uma única instituição e 48% começaram a trabalhar seis meses após a conclusão da graduação. Quanto à forma de trabalho, 56% atuam na assistência, com média de 4,5 salários mínimos e carga horária semanal entre 37 e 44 horas. A maioria reportou aquisição de competência para exercer a profissão, assistindo o paciente na sua integralidade com ética e aplicando conceitos técnicos e científicos no cuidado. Conclusão: o estudo viabilizou a descrição das singularidades da formação do enfermeiro, da inserção no mundo do trabalho e o impacto para a instituição educacional formadora, bem como a apresentação das competências específicas a partir da ótica dos próprios egressos.


Objetivo: evaluar el perfil de egresados del curso de enfermería de una universidad pública, a partir del análisis de las competencias adquiridas durante su formación académica, así como de su proceso de inserción profesional. Método: estudio cuantitativo, exploratorio y descriptivo. La muestra fue realizada con 216 egresados. Los datos se recolectaron por medio de cuestionario validado y encaminado vía correo electrónico a 470 egresados. Para el análisis de los datos se consideraron frecuencias, media y desviación estándar y para la correlación se aplicó la prueba chi-cuadrado. Resultados: la mayor parte de los participantes era de sexo femenino (88%) con un promedio de edad de 29,62 años. La mayoría (65%) contaba con vínculo laboral, un 14% trabajaba en una única institución y un 48% comenzó a trabajar seis meses después de la conclusión de los estudios universitarios. En relación a la forma de trabajo, el 56% actúa en la asistencia, con un ingreso promedio de 4,5 salarios mínimos y una carga horaria semanal de 37 a 44 horas. La mayoría reportó haber adquirido competencia para ejercer su profesión atendiendo a los pacientes de forma integral, ética y aplicando conceptos técnicos y científicos para su cuidado. Conclusión: el estudio posibilitó describir singularidades de la formación del enfermero, su inserción en el mundo laboral y el impacto para la institución educativa formadora, así como la presentación de competencias específicas desde la óptica de los propios egresados.

Objetivos: Caracterizar os egressos da Licenciatura em Enfermagem da Escola de Enfermagem da Universidade de São Paulo e sua inserção no mercado de trabalho e conhecer as perspectivas sobre o curso. Trata-se de um estudo exploratório, de natureza descritiva e quantitativa. Resultados: Participaram da pesquisa 66 egressos do curso de Licenciatura, no período de 2002 a 2017. A coleta de dados foi realizada por meio de questionário eletrônico. Houve predominância do sexo feminino, solteiros, sem fi lhos, e de renda entre 7 a 10 salários mínimos. A maioria concluiu a licenciatura em 4 a 5 anos (69,7%). Parte dos egressos teve inserção profissional na Licenciatura (38%) e sete (10,6%) afirmaram exercer docência como atividade principal.Conclusão: Na fala dos participantes, percebeu-se algumas fragilidades em relação ao estágio curricular, bem como falta de articulação entre as disciplinas. A autonomia gerada durante o curso de licenciatura é citada como um fortalecedor da profissão. Foi possível observar que a docência é a segunda atividade mais exercida pelos egressos, contudo o curso é considerado fundamental para o crescimento pessoal e profissional.


Objective: To describe the alumni of the teaching credential in nursing program from the School of Nursing of the University of São Paulo and their employability, and to explore their perspectives about the course. This is an exploratory, descriptive, and quantitative study. Results: A total of 66 alumni from the teaching credential in nursing program who graduated between 2002 and 2017 participated in the study. Data collection were performed by using an electronic form. Participants were mainly female, single, childless, with an income between 7 to 10 minimum wages. The majority of the alumni concluded the course in 4 to 5 years (69.7%). Some of the alumni experienced teaching activities (38%) and seven (10.6%) participants described teaching as being their main current professional activity. Conclusion: Participants reported weaknesses regarding the clinical internship, as well as the lack of articulation among the disciplines. According to the alumni, the program generates professional autonomy, which empowers the profession. Although teaching was described as the second most commonly professional activity performed by the alumni, the program is considered fundamental for personal and professional development.

Hubo predominancia del sexo femenino, solteros, sin hijos, y de ingresos económicos entre 7 a 10 salarios mínimos (37,9%). La mayoría concluyó la licenciatura en 4 o 5anos (69,7%). Parte de los egresados tuvo inserción profesional en la Licenciatura (38%) y siete (10,6%) afirmaron ejercer la docencia como actividad principal.Conclusión: En el relato de los participantes se perciben algunas fragilidades con relación a la pasantía curricular, así como la falta de articulación entre las disciplinas. La autonomía generada durante el curso de licenciatura es citada como un fortalecedor de la profesión. Fue posible observar que la docencia es la segunda actividad más ejercida por los egresados, sin embargo, el curso es considerado fundamental para el crecimiento personal y profesional.

The objective of this study was to compare the effectiveness of Problem Based Learning (PBL) versus PBL associated with the Active Learning Model for Critical Thinking (ALMCT) to improve critical thinking (CT) in nursing students in an educational intervention on Basic Life Support (BLS). A two-arm randomized, single-blinded, parallel clinical trial was carried out with 108 Brazilian undergraduate nursing students who participated in a BLS course. PBL + ALMCT was used in the control group (CG), and PBL was used in the experimental group (EG). Pre- and post-tests were used to evaluate CT dispositions using the California Critical Thinking Dispositions Inventory (CCTDI), and CT skills were evaluated using the California Critical Thinking Skills Test (CCTST). The results did not indicate significant differences between the CG and EG groups for total CCTDI and total CCTST scales and for most of their subscales. However, there was a significant difference in the EG when the mean scores on the pre- and post-test were compared on the CCTDI analyticity subscale (t = 2073; p = 0.043) and on the CCTST analysis subscale (t = 2302; p = 0.025). We strongly recommend further studies using PBL associated with ALMCT and randomized clinical trials that last more than one month.

OBJECTIVE: This project aimed to promote evidence-based practice in the management of peripherally inserted central venous catheters (PICCs) in pediatric and adult patients in an intensive care unit (ICU). INTRODUCTION: A PICC provides secure vascular access to medication, blood and nutrition administration. However, their use in the ICU is limited. Gaps in training and education in the use of these catheters are barriers to improving practice and safety. METHODS: The project was conducted in an ICU of a cardiology teaching hospital in São Paulo, Brazil. The Joanna Briggs Institute Practical Application of Clinical Evidence System and Getting Research into Practice audit tools were used. A baseline audit of the management of PICCs in pediatric and adult patients was conducted, with a sample size of 22 patients and 180 nursing staff. After an educational program, a follow-up audit involving 14 patients and 180 nursing staff was conducted using the same audit criteria. RESULTS: The baseline audit indicated that PICC management concerning flushing had poor compliance of 2-20%. The results of the criteria related to the change in administration sets, including secondary sets and add-on devices, had better results, with moderate (65%) to high (100%) compliance. The follow-up audit showed improvement in all 10 criteria. Criteria related to flushing achieved 83% to 89% compliance; criteria related to dressing and change of administration sets achieved 100% compliance. CONCLUSIONS: Increased compliance with evidence-based best practices was achieved in all assessed audit criteria. The criteria that reached the highest compliance with best practice recommendations were related to the prevention of bloodstream infection and loss of the PICC due to obstruction. Future audits are planned to ensure sustainability.

RESUMO

OBJECTIVE: To understand/reveal the experiences of undergraduate students of the Nursing School of the Universidade de São Paulo in international academic mobility. METHOD: A cross-sectional, descriptive study with a qualitative approach conducted between February and July 2017. Data were collected using a semi-structured interview and submitted to content analysis. RESULTS: Twenty-two (22) students participated in the study. Five analytical categories emerged from the analysis of the interviews: Interinstitutional Relationships Dimension, Personal Dimension, Professional Dimension, Academic Dimension and Cultural Dimension. CONCLUSION: There are many advantages that international mobility can bring to vocational training. Greater governmental and institutional investment is considered necessary, but with mutual planning and monitoring by the institutions in order for it to contribute to the development of Nursing and the Country.

OBJECTIVE: To characterize the experiences of undergraduate students of the School of Nursing of the University of São Paulo (EEUSP) who participated in international mobility programs between January 2011 and July 2017. METHOD: Exploratory, descriptive study with quantitative approach. Of 68 reports, only 38 (56%) were considered valid and were submitted to descriptive statistical analysis. Data were categorized in general, institutional, academic and cultural aspects and cost of living. RESULTS: The main destination was Portugal and the years with most participation were 2012 and 2013. The mean stay was six months and the students took a mean of three to four courses. The main funder was the university of origin. CONCLUSION: Academic activities were limited to theoretical and practical courses, with little insertion in research. There is a need to increase investment in learning other languages and to expand partnerships with larger centers of foreign education and research.

OBJECTIVES: This implementation project aimed to identify the current practice in regards to central venous catheters (CVCs) maintenance to improve knowledge amongst nursing staff and to assess increased compliance with evidence-based best practice. INTRODUCTION: Central venous catheters are considered an important therapeutic resource for the administration of fluids, drugs, blood, collection of blood samples and hemodynamic monitoring. Despite the benefits, catheter use is associated with complications such as primary infection of the catheter-related bloodstream. METHODS: This project utilized the audit and feedback model using the Joanna Briggs Institute Practical Application of Clinical Evidence System. Nine of 10 criteria were audited through direct observation of nursing professionals or patient records in relation to CVC maintenance, and one criterion involved direct questioning of nursing staff. Baseline and follow-up audits were conducted in a 12-bed adult intensive care unit in a university hospital. RESULTS: The baseline audit revealed deficits between current practice and best practice in some criteria. Barriers to implementation of CVC maintenance best practice criteria were identified, and the strategies were implemented. The post-implementation (follow-up) audit showed improvement in compliance to best practice guidelines in all of the audit criteria, except in one criterion: the use of sterile gloves or surgical tweezers during the execution of the dressing. CONCLUSIONS: Best practice in CVC care was achieved in the hospital, strengthening and guiding nursing care, as well as highlighting the importance of nursing records throughout the care process. However, this project highlighted the need to improve compliance through follow-up audits and periodic training to support best practice.

OBJECTIVES: The objective of this project was to improve local practice in nasoenteric feeding discharge planning for cancer patients in a cancer teaching hospital in São Paulo, Brazil. INTRODUCTION: Nasoenteric tubes are commonly used for enteral nutrition in patients with cancer for over seven days during their recovery and this may be continued at home, with clear benefits. RESULTS: Baseline audit results indicated poor compliance (between 0% and 22%) with the current evidence, although good compliance (100%) was observed for three audit criteria. Implementation of the strategies, including changes to achieve higher participation of caregivers, resulted in an improvement of the audited criteria (100% compliance was maintained for three criteria, compliance increased from 22% to 67% for a further three criteria, with 0% compliance for one criterion). CONCLUSIONS: Despite the short time frame of this study, an increase in compliance with best practices proposed by the Joanna Briggs Institute was observed. Implementing a multidisciplinary meeting before patient discharge remains a challenge.

RESUMO Objetivo Compreender as experiências vivenciadas pelos estudantes de graduação da Escola de Enfermagem da Universidade de São Paulo na mobilidade estudantil internacional. Método Estudo transversal, descritivo, com abordagem qualitativa, realizado entre fevereiro e julho de 2017. Os dados foram coletados por meio de uma entrevista semiestruturada e submetidos à análise de conteúdo. Resultados Participaram do estudo 22 estudantes. Na análise das entrevistas, emergiram cinco categorias analíticas: Dimensão das Relações Interinstitucionais, Dimensão Pessoal, Dimensão Profissional, Dimensão Acadêmica e Dimensão Cultural. Conclusão São muitas as vantagens que o intercâmbio internacional pode trazer à formação profissional. Considera-se necessário maior investimento governamental e institucional, mas com planejamento mútuo e acompanhamento das instituições, de forma que contribua para o desenvolvimento da Enfermagem e do País.

Se recogieron los datos mediante una entrevista semiestructurada, los que se sometieron al análisis de contenido. Resultados Participaron en el estudio 22 estudiantes. En el análisis de las entrevistas, emergieron cinco categorías analíticas: Dimensión de las Relaciones Interinstitucionales, Dimensión Personal, Dimensión Profesional, Dimensión Académica y Dimensión Cultural. Conclusión Son muchas las ventajas que el intercambio internacional puede brindar a la formación profesional. Se considera necesario mayor inversión gubernamental e institucional, pero con planificación mutua y acompañamiento de los centros, de modo a que contribuya al desarrollo de la Enfermería y del País.

Método: Se trata de un estudio exploratorio, descriptivo de abordaje cuantitativo, de 68 reportes. De estos, solamente 38 (56%) se consideraron válidos y se sometieron al análisis estadístico descriptivo. Los datos se recogieron según los aspectos generales de la experiencia: institucionales, académicos, culturales y costo de vida. Resultados: El destino principal fue Portugal, entre los años 2012 y 2013, con permanencia media de seis meses y cursando, promedio, de tres a cuatro disciplinas. La principal financiadora fue la universidad de origen. Conclusión: Las actividades académicas se limitaron a disciplinas teórico-prácticas, sin involucrarse mucho en la investigación. Existe la necesidad de aumentar las inversiones para el aprendizaje de otros idiomas y aumentar los convenios con los grandes centros de enseñanza e investigación extranjeros.

Resultados: O principal destino foi Portugal, entre os anos de 2012 e 2013, com permanência média de seis meses, cursando em média três a quatro disciplinas. O principal financiador foi a universidade de origem. Conclusão: As atividades acadêmicas limitaram-se a disciplinas teórico-práticas, com pouca inserção na pesquisa. Há necessidade de aumentar o investimento na aprendizagem de outros idiomas e ampliar parcerias com grandes centros de ensino e de pesquisa estrangeiros.
