RESUMO

Hypertrophic cardiomyopathy (HCM) is a congenital disease caused by mutations in a number of sarcomere proteins. According to the type of mutation, clinical observations record similar clinical manifestations, myocardial pathological changes, and the timing of manifestation of the disease in HCM patients. OBJECTIVE: To study cardiomyocyte (CMC) ultrastructural changes in the interventricular septum (IVS) of patients with HCM and evaluate their specificity for this pathology. MATERIAL AND METHODS: IVS myocardial samples taken from 44 HCM patients aged 18-59 years at IVS myoectomy underwent an electron microscopic study. The diameter of CMCs and their nuclei was measured in semithin sections. RESULTS: A morphometric examination of the IVS myocardium in HCM patients revealed moderate hypertrophy of CMCs and their nuclei, the diameters of which averaged 23.7±4.4 and 5.2±0.9 µm, respectively. The IVS CMCs were characterized by the ultrastructural signs of hypertrophy: the larger size and number of structures ensuring contractile and synthetic functions; the myocytes contained higher amounts of myofibrils, intermyofibrillar mitochondria, granular endoplasmic reticulum cisterns, and free ribosomes. On the contrary, some CMCs had fewer myofibrils in the perinuclear region, which is an adaptive change under hemodynamic overload conditions. In addition, a number of myocytes displayed signs of dystrophic changes: the appearance of lipofuscin granules, myelin figures, phagosomes, lipid droplets, and vacuoles, which can fill all free sarcoplasmic zones. CONCLUSION: Ultrastructural changes characteristic of hypertrophy were found in IVS CMCs in HCM patients. In addition, there was partial myofibrillar loss and dystrophic changes in a number of myocytes, which are stereotypic compensatory-adaptive changes under hemodynamic overload conditions. All the above-mentioned changes in the CMC ultrastructure are characteristic of myocardial hypertrophy, but not specific for hypertrophic cardiomyopathy.

OBJECTIVE: To analyze cardiomyocyte (CMC) ultrastructural changes in the right ventricle outflow tract (RVOT) of infants with tetralogy of Fallot (TF) in the first years of life and to compare the findings with clinical parameters in these patients. MATERIAL AND METHODS: Intraoperative RVOT myocardial biopsy specimens obtained from 51 patients aged 3-33 months with TF during radical correction of defect were investigated. CMC diameter and length were measured using the semithin myocardial sections stained with periodic acid-Schiff. The ultrathin sections were examined in the electron microscope. RESULTS: The diameter of CMCs in the RVOT of infants with TF varied significantly (7.3-17.0 µm) and averaged 10.8±2.2 µm; a large number of multinucleated CMCs were observed. There were ultrastructural signs of incomplete differentiation of CMCs: active myofibril assembly in the free sarcoplasmic region; gap junctions on the lateral surfaces of CMCs; and centrioli in their sarcoplasm. Myofibrillogenesis in babies under 6 months increased in response to hemodynamic overload and hypoxemia. In addition, organelles suggestive of the synthetic activity of CMCs, such as cisterns and vesicles of the Golgi complex and granular endoplasmic reticulum, were detected in the sarcoplasm of a number of CMCs. TF infants' myocardium also displayed focal disorders of CMC interposition; the change in the shape of myocytes was accompanied by the appearance of additional lateral insert discs. Some CMCs showed the abnormal localization of the nucleus beneath the sarcolemma, sarcoplasmic bulging areas, and dystrophic changes. CONCLUSION: There were ultrastructural features characteristic for the myocardium that was at the state of active growth and differentiation (increases in the diameter and length of CMCs and in the number of nuclei; myofibrillogenesis; signs of synthetic and proliferative activity along with insignificant dystrophic changes) in the CMCs of myocardial RVOT in infants with TF in the first years of life.

AIM: to investigate changes in the diameter and length of hypertrophic cardiomyocytes (CMCs) in the dilated left ventricle (LV). SUBJECTS AND METHODS: Light microscopy, morphometry, and statistical analysis were used to investigate the status of the contractile apparatus and changes in the length, diameter of CHC and diameter of CMC nuclei, by using intraoperative dilated LV biopsy samples from 31 patients with valvular disorders and dilated cardiomyopathy. Morphological findings were compared with the clinical parameters of the patients. RESULTS: CMCs in the patients with the dilated LV were hypertrophic and were at different stages of restructuring with progressive myofibrillar loss (PML). In 81% of patients, the diameter of CMCs was not significantly changed as their zones of PML extended. The length of CMCs, which correlated with the enlarged LV cavity, was increased in 52% of patients during cell restructuring with PML. In 42% of patients, the CMC nuclear diameter increased during restructuring with PML, which appeared to be associated with CMC polyploidization; in some of these patients (19% of the total number of patients), the diameter of CMCs increased in parallel with the higher diameter of their nuclei. CONCLUSION: The findings suggest that after completion of their transverse growth, hypertrophic CMCs are involved in a restructuring process with PML. The findings are consistent with the hypothesis that dilatation of the hypertrophied LV cavity is related to the preferential elongation of CMCs with an inadequate increase in their diameter. The results of the investigation may assume that the higher CMC diameter that brings to completion before the entry of the cells into the restructuring process with PML, resumes in the cells, the ploidy of which increases in the course of restructuring with PML, triggering an additional mechanism for raising the CMC diameter at this stage of myocardial hypertrophy. The results are indicative of different mechanisms for increasing the diameter and length of hypertrophic CMCs, since the diameter of CMCs directly correlates with that of their nuclei, and the length increases as the zones of PML extend in the CMCs.

AIM: to analyze the morphological features of the myocardium of the atrial appendages in patients with different forms of atrial fibrillation (AF) and to compare the findings with the clinical parameters of patients. MATERIAL AND METHODS: Light and electron microscopies were used to examine the myocardium of the atrial appendages in adult patients with paroxysmal (PAF), persistent (PrAF), or long-standing persistent atrial fibrillation (LPAF) and in comparison group patients with sinus rhythm without history of AF. A morphometric method was employed to evaluate myocardial fibrosis and to measure the diameter of cardiomyocytes (CMCs); the degree of lipomatosis and amyloidosis was semiquantitatively determined; and the content of CMC myofibrils was estimated. Atrial natriuretic peptide content in the myocytes was measured by immunoconfocal microscopy. RESULTS: In all groups, the patients with AF were found to have signs of atrial structural remodeling: fibrosis, lipomatosis, isolated atrial amyloidosis, CMC hypertrophy with the phenomena of a partial loss of myofibrils without significant differences between these parameters in different groups. In PAF patients, atrial remodeling was accompanied by hypertrophy of a number of CMCs with their higher myofibrilar mass; the increased CMC size in the left atrial appendage prevented left atrial enlargement; the degree of amyloidosis negatively correlated with the CMC myofibrillar loss that was recorded in the minor CMCs; the degree of CMC myolysis positively correlated with mitral valve insufficiency and left atrial enlargement. In contrast to the clinical and morphological changes that are typical of PAF, in LPAF the increase in CMC sizes was positively correlated with left atrial enlargement and mitral annular dilatation; while the myofibrillar loss phenomenon was noted in the most hypertrophied CMCs; the degree of amyloidosis was positively correlated with CMC myofibrillar loss. In the patients with PrAF, the size of CMCs did not correlate with their myofibril content. CONCLUSION: The patients with PAF were ascertained to have opposite changes in the ratio of CMC hypertrophy to left atrial enlargement, amyloidosis, and CMC myofibrillar loss, hypertrophy of CMCs and their myofibril content in comparison with these indicators in LPAF.

AIM: to determine the diameter and length of cardiomyocytes (CMC) in the ventricular septum (VS) of patients with hypertrophic cardiomyopathy (HCM) and to analyze correlations of a change in CMC sizes with the anatomical features of the heart in the patients. MATERIAL AND METHODS: Longitudinal sections of intraoperative myocardial biopsy specimens taken from 23 patients aged 15-59 years with HCM were treated using immunohistochemical detection of connexin 43; the sizes of 50 CMCs were measured; a 4-point scale was used to assess the degree of myofibril loss (MFL) in these cells. The change in the diameter and length of the cells during their rearrangement as MFL gradually increased, as well as the correlations of CMC sizes with the anatomical parameters of the heart were analyzed. RESULTS: VS CMCs from the patients with HCM were hypertrophic and were in the early stages of rearrangement accompanied by MFL. During this rearrangement, CMCs in some patients grew in length and, less frequently, diameter. The average diameter of CMCs was directly correlated with VS thickness. The average length of the cells in the CMC population with a considerable degree of MFL also directly correlated with VS thickness. CONCLUSION: The findings could suggest that the factor raising VS thickness in HCM may be an increase in not only diameter of CMCs, but also in length of CMCs, which had impaired orientation in the VS - which are oriented perpendicular to their normal tangential position. The presence of such CMCs in the VS myocardium in patients with HCM can be discussed due to the typical large number of myocardial areas with the impaired parallel arrangement of CMCs.

Telocytes of placental villi were studied by electronic microscopy during physiological pregnancy. Ultrastructural features of telocytes indicating their heterogeneity and presence of at least three types of villi depending on their localization and kind were observed. All placental telocytes were characterized by small amount of organelles including mitochondria. Presence of long thin processes, which generated a branching network by contacting with each other, served as a typical feature of telocytes including telocytes of the stroma and intermediate villi. Telocytes were absent in the terminal villi.

Background: Nowadays autologous mesenchymal placental stromal cells (MSCs) may use to treat for various diseases both of the mother and the child. Stroma of the placenta villi is appropriated origin for cell culture isolation. Aim: of the study was to evaluate the possibility for selection and use of placental tissue for mesenchymal stromal cells. Materials and methods: The present study was based on 45 placental samples of women aged 27−38 yy. who underwent surgical delivery at 36−40 weeks of gestation. 30 of these women have been enrolled in the basic group including children with congenital abnormalities (CA). The comparison group consisted of 15 patients with physiological pregnancy. We performed histological examination (with hematoxylin and eosin staining), immunohistochemical examination (with use monoclonal antibodies CD90 (1:25; Abcam, UK), СD105 (1:500; Abcam, UK), CD44 (1:25; Dako), СD73 (1:200, Abcam, UK), and electron microscopy (by microscope Philips/FEI Corporation, Eindhoven, Holland). Eclipse 80i microscope (Nikon Corporation, Japan) was used to examine the immunohistochemical reactions as a brown staining. The evaluation of the intensity of reaction was conducted by NIS-Elements Advanced Research 3.2 program (Czech Republic). Student's t-test and analysis of variance were used to compare the mean values. Differences were considered statistically significant at p<0.05. Results: Interstitial cells of the stroma of the villi with CA had fibroblastic differentiation as revealed degenerative changes of the cells. The histologic examination with hematoxylin and eosin staining revealed significant fibrosis of the stroma of the placenta villi in CA group (p<0,01). Immunohistochemical study of stem and intermediate chorionic villi revealed no significant differences in staining of CD44+, СD90+, СD73+, and CD105+ cells if compared to the control group (p>0.05). Although CD105 expression was significantly lower in the CA group (0.058±0.0049) than in the control group (0.088±0.0039) (p<0.05). However, electron microscopy detected the villi interstitial stromal cells with fibroblastic differentiation in CA group. Conclusions: Thus, it is necessary to exclude placenta with obstetrical history, somatic, and congenital pathology of the mother and the child when selecting the placental cell culture. Moreover, choosing a sample the morphological structure of the placenta should be taken into consideration. However, congenital malformations of the fetus, pathology of the mother cultivate mesenchymal stromal cells of placentas is inappropriate and should be taken advantage of the donor cells.

Proliferating Ki-67+ cardiomyocytes were detected in the interventricular septum myocardium of adult patients with hypertrophic cardiomyopathy. In the same patients, the severity of hypertrophy and the degree of cardiomyocyte differentiation were assessed by the content of myofibrils, ultrastructural morphology, and the pattern of connexin 43-containing gap junction distribution. Adult Ki-67+ cardiomyocytes containing sarcomeric α-actin (sarc α-act+) in the sarcoplasm (diameter 23.9±6.9 µ) were detected in the myocardium of patients with hypertrophic cardiomyopathy; their relative content varied from 2 to 3084 cells per 1 million cardiomyocytes. Small early differentiating Ki-67+/sarc α-act+ cardiomyocytes with a thin cytoplasm layer (diameter 5.9±1.7 µ) constituted from 3 to 2262 cells per 1 million cardiomyocytes. These cells were found in the myocardium with the most pronounced structural changes: hypertrophy of cardiomyocytes with signs of their partial dedifferentiation.

RESUMO

Biodegradable film compositions based on natural biopolymer gelatin with immobilized colchicine were prepared and their efficiency in prevention of the adhesion process in the pericardium was evaluated on rabbit model of postoperative pericarditis. The use of gelatin-based biodegradable film compositions significantly reduced the intensity of adhesion formation in the pericardial cavity, while immobilization of anti-inflammatory drug colchicine amplified their anti-adhesion activity.

The myocardium of the right and left atrial appendages (auricles) in patients with paroxysmal, persistent, and permanent forms of atrial fibrillation was examined by histological methods and electron microscopy. Isolated atrial amyloidosis was detected in the left (50.0-56.3% patients) and in the right (45.0-55.6% patients) atrial appendages. In all cases, immunohistochemistry revealed atrial natriuretic peptide in fibrillary amyloid deposits. Ultrastructurally, amyloid masses formed clusters of myofibrils 8-10 nm in diameter. They were chaotically located in the extracellular space along the sarcolemma as well as in membrane invaginations, dilated tubules of cardiomyocyte T-tubular system, and vascular walls. Amyloidosis was predominantly observed in women; its degree positively correlated with age of patients and duration of atrial fibrillation but negatively correlated with atrial fibrosis. The study revealed positive (in permanent atrial fibrillation) and negative (in paroxysmal atrial fibrillation) correlation of amyloidosis with myofibril content in atrial cardiomyocytes.

RESUMO

Endomyocardial biopsy samples of the interventricular septum from patients with hypertrophic cardiomyopathy isolated during myectomy were examined. We revealed significant variability of cardiomyocyte ploidy (from 2.9c to 13.5c) that directly correlated with myocyte size; the maximum ploidy was found in young patients. Ultrastructural signs of increased contractile and synthetic activity of cells were observed in patients with high DNA content in cardiomyocytes.

RESUMO

Negative pressure wound treatment (NPWT) is one of the newest methods used in the treatment of wounds. It allows speeding up and optimizing the healing process and reducing the cost of treatment. Negative pressure stimulates proliferation of granulation tissue, provides a continuous evacuation of fluid and effectively cleans wound surface. The authors present to the reader the results of treatment of acute suppurative diseases of soft tissues with the method of topical negative pressure.

Mitral valves tissue samplings from children with complete (13 patients) and partial (6 patients) atrioventricular defects at the age of from I month to 3 years old were examined. The biopsy material was received during the repeat surgical operation on mitral valve, performed due to residual mitral valve regurgitation grade 3-4 at the period of time from 2 days to 1 year after radical defect correction. On histological examination the areas of myxomatous tissue degeneration occupying more than 50% of mitral valves surface were found in 6 (32%) of the 19 patients. There were dispersed star-shaped cells, architectonics disturbances, deposition of acid mucopolysaccharides and increased content of matrix metalloproteinase 13 in such areas of myxomatous degeneration. The sizes of these areas correlated with mitral valve regurgitation grade. After the radical correction of atrioventricular defect the sutures on the folds and fibrous ring of the mitral valve "cut through" reliably more often in patients with wider areas of myxomatous degeneration, which indicates poor prognosis. According to the ultrastructural classification the majority of mitral valve cells regarded as fibroblasts; there also were found cells with the signs of myogenic differentiation--myofibroblasts and isolated hystiocytes. According to the immunohistochemistry assay the cells phenotype regarded as fibroblastic and endothelial differentiation; in some patients there were found cells of smooth muscle origin.

Interventricular septum myocardium was studied in 40 patients with obstructive hypertrophic cardiomyopathy. Immunohistochemical assay revealed c-kit-positive resident cardiac stem cells in 82.5% patients. The content of the connective tissue and myofibrillar disarray zones and the degree of cardiomyocyte hypertrophy and myolysis were determined. In 30% cases, cardiomyocytes containing atrial natriuretic peptide were detected in the interventricular septum myocardium. The data were compared with clinical and functional parameters of patients. It was found that cardiac stem cells are present in patients, whose myocardium was characterized by increased density of the connective tissue, hypertrophy of mature cardiomyocytes, medium degree of myolysis in them, and accumulation of natriuretic peptide, a cardiac failure marker, in cardiomyocytes.

RESUMO

Light and electron microscopies were used to analyze cardiomyocyte structural changes in the dilated left ventricle in patients with dilated cardiomyopathy and valvular heart diseases. The patients were found to have cardiomyocyte hypertrophy and ultrastructural rearrangement with a tissue-specific reduction. There was hypertrophic cardiomyocyte lengthening that continued after these cells stopped growing thicker, as well as occurred due to the loss of myofibrils, which increased during the cell rearrangement, and directly correlated with the lower ejection fraction and higher end-systolic volume of the left ventricle.

We studied the content of resident myocardial stem cells, cardiomyocyte precursors, in myocardial biopsy specimens from the right ventricular outflow tract of patients of the first two years of life with tetralogy of Fallot. Myocardial resident stem cells were detected by the method of confocal immunohistochemistry using antibodies to c-kit and sarcomeric α-actin. The diameter of right ventricular cardiomyocytes was measured; the presence of myolysis zones was semiquantitatively evaluated. Electron microscopic analysis of right ventricular cardiomyocytes was performed. The data on the content of resident myocardial stem cells were compared with clinical and functional parameters of the patients and morphological peculiarities of the myocardium. C-kit-positive resident myocardial stem cells were detected in the right ventricle of 17.4% patients with tetralogy of Fallot. The content of resident myocardial stem cells in these patients varied from 4 to 45 (median 11) per 1 mln cardiomyocytes; this parameter was higher in patients with high content of small cardiomyocytes (diameter <10 µ) and cardiomyocytes with incomplete myofibril assembly in the right ventricular myocardium.

Ureteral endoprosthesis results are presented for 22 patients (9 males and 13 females) with long strictures of the upper urinary tract (UUT). Stenosis was caused by rectal tumor in 2 patients, by cancer of the uterine cervix in 5 patients. In a female with Burneville-Pringle disease metallic prosthesis was set in the pyeloureteral segment (PUS) of the solitary right kidney. Six patients had long strictures of the abdominal part of the ureter, 4 had PUS strictures, 3--of the ureter of the transplanted kidney and 1 had stricture of ureterocalicoanastomosis after Neivert operation. Endoprosthesis was established by a retrograde approach in 13 cases, via percutaneous antegrade approach--in 9 cases. In postoperative period 2 patients had exacerbation of chronic pyelonephritis, they received anti-inflammatory treatment. Patancy of the ureter was reestablished in all the cases and persisted for all the follow-up period (4-62 months). Late after operation a patients with Burneville-Pringle disease developed UUT obstruction because of ureteral compression by enlarged angiolipomatous nodes under endoprosthesis. Therefore, one more nitinol stent was set in the upper third of the right ureter retrogradely. Thus, ureteral endoprosthesis in long and recurrent strictures of the ureter, ineffective plastic surgery on the UUT is an operation of choice.

We performed subcutaneous bypass draining (SBD) of the upper urinary tract (UUT) in 12 patients (3 males, 9 females, mean age 64 years) in 2006-2008. Ureteral obstruction was caused by a tumor in 11 patients, one patient had extended obliteration of the left ureter after resection of the abdominal aorta aneurysm and two plastic operations on the UUT. All the patients with ureteral tumor obstruction had only one functional kidney. For SBD of the kidney we used Detoure stent in 2 cases, the nephrovesical bypass - in the rest cases. Surgery was made in the lateroposition of the patient which provided sumaltenous establishment of the renal and vesical ends of the stent. The kidney was also drained by the nephrostoma in 8 patients. The nephrostoma was removed after antegrade pyelography and pyelomanometry. Intraoperative complications were not registered. Suprapubic urine leak in 3 patients previously given radiotherapy was stopped by a long drain of the bladder. Obstruction of the distal stent part by a progressive tumor was diagnosed in 1 patient 3 months later. The bypass was removed and a nephrostomic drainage was made. The nephrovesical stent was changed in one case 5 months after SBD. Upon 6-32 month follow-up 3 patients died of cancer progression, the rest are still alive. Thus, SBD is indicated if stenting, ureteral endoprosthesis, constant nephrostomic drainage are impossible. In some cancer patients with ureteral obstruction in UUT drainage SBD is a method of choice with promising short- and long-term results.

RESUMO

The cardioprotective effect of cardioplegic solution based on histidine-containing dipeptides was evaluated on isolated rat heart under conditions of hyporthermia and long ischemia. The use of natural dipeptides in cardioplegic solutions promoted an increase in the buffer capacity of myocardial cells and creation of an additional anti-ischemic effect under conditions of long ischemia and hypothermia.
