RESUMO

Ru complexes based on lutidine-derived pincer CNN(H) ligands having secondary amine side donors are efficient precatalysts in the hydrogenation and dehydrogenation of N-heterocycles. Reaction of a Ru-CNN(H) complex with an excess of base produces the formation of a Ru(0) derivative, which is observed under catalytic conditions.

INTRODUCTION: Revisional surgery must be considered when insufficient weight loss is attained or weight is subsequently regained. This study aimed to investigate the value of Roux-en-Y gastric bypass (RYGB) as a revisional procedure after restrictive surgery. MATERIALS AND METHODS: An observational, retrospective study including patients initially operated on for morbid obesity with restrictive techniques (vertical-banded gastroplasty [VBG], adjustable gastric band [AGB], and sleeve gastrectomy) and reoperated with RYGB in our centre between December 1994 and January 2019. Demographic and anthropometric data, associated comorbidities (diabetes mellitus type II, arterial hypertension, dyslipidaemia, and chronic obstructive pulmonary disease) and surgery-related data (approach, complications, and hospital stay) were evaluated at 5 different time points: initial (prior to first intervention), after the first surgical intervention, before the second intervention (gastric bypass), after the gastric bypass, and at present. RESULTS: A total of 63 patients were included. VBG was the most frequent initial procedure (n = 33). The mean age was 39 ± 9.52 years, and the average initial weight was 143.53 ± 28.6 kg. Weight loss was achieved in all groups, with a median excess weight loss of 58% after the first surgery and 40.3% after gastric bypass. In terms of weight loss, the best results after the second surgery were obtained when the first surgery was AGB, with statistically significant differences. CONCLUSIONS: RYGB is effective as a conversion procedure after a previous restrictive surgery, obtaining a significant reduction in weight and BMI. It has an acceptable morbidity rate and is more effective after an AGB.

PURPOSE: The signs and symptoms of dry eye disease (DED) can affect the quality of life of patients. This study analyzes whether locking Y lateral canthopexy improves patients' quality of life and in what way it does so (based on the ocular surface disease index questionnaire) and DED signs (measured by the fluorescein breakup time, tear osmolarity, Schirmer test, and corneal staining) in patients with DED caused by a eyelid malposition or impaired blink dynamics. METHODS: This was a case-control study. Forty eyes of 20 patients with DED and lateral canthal disinsertion examined at the University Hospital La Ribera during a 1-year period from November 2016 to November 2017. The 40 eyes were divided into 2 groups: Group 1 eyes that underwent locking Y lateral canthopexy (i.e., a plication of the superficial lateral canthal tendon to the lateral orbital rim periosteum); and group 2 eyes that underwent observational treatment, without surgery. Symptoms and signs in OU were recorded separately at baseline and 1 month after surgery. RESULTS: One month after surgery, the ocular surface disease index reduced in group 1 eyes when compared with those of group 2 (p = 0.001), fluorescein breakup time showed an improvement in the operated eyes (group 1) (p < 0.001) and corneal staining was reduced in group 1 after surgery (p = 0.012). The osmolarity results did not change when compared with eyes from group 2 (p < 0.001). CONCLUSIONS: Locking Y lateral canthopexy is an effective surgical technique for treating patients with evaporative dry eye caused by a lateral canthal tendón disinsertion by restoring proper eyelid closure and improving blinking dynamics. It improves DED signs and symptoms, thereby enhancing the quality of life of patients.

A joint experimental/computational effort to elucidate the mechanism of dihydrogen activation by a gold(I)/platinum(0) metal-only frustrated Lewis pair (FLP) is described herein. The drastic effects on H2 activation derived from subtle ligand modifications have also been investigated. The importance of the balance between bimetallic adduct formation and complete frustration has been interrogated, providing for the first time evidence for genuine metal-only FLP reactivity in solution. The origin of a strong inverse kinetic isotopic effect has also been clarified, offering further support for the proposed bimetallic FLP-type cleavage of dihydrogen.

A highly regio-, diastereo-, and enantioselective, scalable Ir-catalyzed hydroarylation of electron-rich acyclic and tensioned cyclic olefins with heterobiaryls is described. The reaction of acyclic vinyl ethers, dihydrofuran, and norbornenes with a variety of aryl isoquinoline, quinazoline, and picoline derivatives takes place with simultaneous installation of central and axial chirality, reaching complete branched/linear or exo/endo ratios and excellent diastereo- and enantiomeric excesses when in situ formed [IrI/Tol-SDP] or [IrI/Tol-BINAP] complexes are used as the catalysts. Deuterium labeling experiments and a comprehensive computational study suggest that, despite fast double bond migratory insertion into Ir-H, the reaction proceeds through a modified Chalk-Harrod mechanism, starting with selectivity-determining insertion into Ir-CAryl. The regioselectivity is controlled by the electron-donating alkoxy group, whereas diastereo- and enantioselectivity have a complex origin, which depend on the relative orientation of the alkoxy group and the establishment of adequate π-π interactions between the biaryl and the phosphine.

Introduction: There is no agreement on the procedures to be used for diagnosis and treatment of gestational thyroid dysfunction. Controversy still exists on the normal range of thyroid-stimulating hormone (TSH) levels and use of gestational hypothyroidism (GH) screening. The aim of this study was to assess diagnosis and treatment of thyroid dysfunction during pregnancy in a group of Spanish hospitals. Study design: This was a retrospective, multicenter study in pregnant females with GH attending Spanish healthcare centers from March 2013 to July 2014. Variables analyzed included diagnosis criteria for GH (availability of universal screening for gestational thyroid disorders and TSH reference values (RVs) by trimester of pregnancy): risk factors for GH, iodine intake from food or supplementation, gestational age (at diagnosis/treatment) and l-thyroxine treatment. Results: Fourteen centers participated in the study. Universal screening was performed in only half of the centers, and only 14% had their own TSH RVs. Overall, 257 pregnant women were enrolled, 53.7% with hypothyroidism (HT) diagnosed before pregnancy (pre-GH) and 46.3% with HT diagnosed during pregnancy (intra-GH). A comparison of intra-GH and pre-GH women showed that intra-GH women made their first visit later (59.7% vs. 75.4% respectively before week 12, p = 0.007) and had more frequently high TSH levels (>2.5 μIU/ml) during the first trimester (94.4% vs. 67.0% respectively, p < 0.001). Conclusions: Our results suggest that GH may be underdiagnosed or inadequately diagnosed in most healthcare centers. These findings suggest the need of improving the current practice in Spain


Introducción: Los procedimientos a seguir para el diagnóstico y tratamiento de la disfunción tiroidea en la gestación no están del todo consensuados. Aún se discute el rango de normalidad de los valores de la hormona estimulante del tiroides (TSH) y el uso de screening para detectar hipotiroidismo gestacional (HG). El objetivo de este estudio es evaluar la forma de diagnóstico y tratamiento de la disfunción tiroidea durante la gestación en un grupo de hospitales de España. Diseño del estudio: Estudio retrospectivo, multicéntrico en mujeres embarazadas con HG atendidas en instituciones sanitarias españolas entre marzo de 2013 y julio de 2014. Las variables analizadas incluyeron criterios diagnósticos de HG (disponibilidad de screening universal para trastornos tiroideos gestacionales y valores de referencia de TSH según el trimestre gestacional); factores de riesgo de HG, ingesta de yodo mediante alimentos o suplementos, edad gestacional (al diagnóstico/tratamiento) y tratamiento con L-tiroxina. Resultados: Participaron un total de 14 centros. Únicamente la mitad de los centros empleaba el screening universal, y solo el 14% tenía valores de referencia de TSH propios. Se incluyeron un total de 257 embarazadas, 53,7% con diagnóstico de hipotiroidismo previo al embarazo (pre-HG) y 46,3% con hipotiroidismo diagnosticado durante el embarazo (intra-HG). Comparando los casos de pre-HG e intra-HG, las mujeres con intra-HG realizaban la primera visita más tarde (antes de la semana 12; 59,7% vs. 75,4% respectivamente, p = 0,007) y tenían más frecuentemente valores elevados de TSH (> 2,5 μUI/ml) durante el primer trimestre (94,4% vs. 67,0% respectivamente, p < 0,001). Conclusiones: Nuestros resultados sugieren que el HG puede estar infradiagnosticado o diagnosticado indebidamente en la mayoría de los centros sanitarios. Estos hallazgos sugieren la necesidad de mejorar la práctica actual en España

Objetivos: Valorar la eficacia de la cirugía de conversión en una unidad de cirugía bariátrica con 25 años de experiencia. Método: Estudio observacional retrospectivo de pacientes con obesidad tipo II o superior reintervenidos mediante cirugía de conversión por reganancia de peso, índice de masa corporal (IMC) residual > 35 kg/m2 o pérdida < 50% del exceso de peso. Se analizaron los datos demográficos y antropométricos, las comorbilidades y los datos perioperatorios en 5 periodos de tiempo: inicial, post-cirugía1, pre-cirugía2, post-cirugía2 y actualidad. Resultados: Se incluyeron un total de 112 pacientes con una media de edad de 40,2 años intervenidos inicialmente mediante gastroplastia vertical anillada (GVA) (32,1%), banda gástrica ajustable (BGA) (23,2%), bypass gástrico en Y de Roux (BGYR) (21,4%) y gastrectomía vertical (GV) (23,2%). Las técnicas de conversión, realizadas tras una mediana de 70 meses, incluyeron: BGYR (58,9%), GV (1,8%), bypass gástrico de una anastomosis (BAGUA) (11,6%), acortamiento de asa común (AAC) (24,1%) y derivación bilio-pancreática (DBP) (3,6%). Hubo una reducción del peso inicial de 144,2 ± 30,3 a 101,5 ± 21,8 kg tras la cirugía-1 y de 115,6 ± 24,0 a 91,5 ± 19,0 kg tras la cirugía-2. El peso en la actualidad es de 94,7 ± 16,4 kg tras una mediana de seguimiento de 27,5meses. Un grado de reducción similar ocurrió con el IMC. La mejoría de las comorbilidades se produjo sobre todo tras la primera intervención. Conclusiones: La cirugía de conversión provoca una reducción de peso que no supera a la pérdida alcanzada tras la primera cirugía, pero a diferencia de esta, logra estabilizar el peso a lo largo del tiempo. La tasa de morbilidad perioperatoria es aceptable y justificaría su aplicación, a pesar de que el impacto en las comorbilidades sea limitado

C-H bond activation at cationic [(η5-C5Me5)Ir(PMe2Ar')] centers is described, where PMe2Ar' are the terphenyl phosphine ligands PMe2ArXyl2 and PMe2ArDipp2. Different pathways are defined for the conversion of the five-coordinate complexes [(η5-C5Me5)IrCl(PMe2Ar')]+, 2(Xyl)+ and 2(Dipp)+, into the corresponding pseudoallyls 3(Xyl)+ and 3(Dipp)+. In the absence of an external Brønsted base, electrophilic, remote ζ C-H activation takes place, for which the participation of dicationic species, [(η5-C5Me5)Ir(PMe2Ar')]2+, is proposed. When NEt3 is present, the PMe2ArDipp2 system is shown to proceed via 4(Dipp)+ as an intermediate en route to the thermodynamic, isomeric product 3(Dipp)+. This complex interconversion involves a non-innocent C5Me5 ligand, which participates in C-H and C-C bond formation and cleavage. Remarkably, the conversion of 4(Dipp)+ to 3(Dipp)+ also proceeds in the solid state.

RESUMO

The synthesis and double asymmetric hydrogenation of (Z,Z)-1,3-diene-1,4-diyl diacetates is described. In this reaction C2/meso ratios up to 85 : 15 and enantioselectivities up to 97% ee have been achieved. As the hydrogenation products can be converted into chiral 1,4-diols, key starting materials for the preparation of the best catalysts used, this catalytic system enables a self-breeding chirality process.

Iridium complexes based on deprotonated lutidine-derived CNP* pincers 2a/2b selectively catalyzed the hydroboration of CO2 under mild conditions (1-2 bar CO2, 30 °C) to methoxyborane using HBcat (TOF up to 56 h-1) and to the formate level with HBpin (TOF up to 1245 h-1). Interestingly, an intriguing, positive water effect on the reaction rates has been observed. NMR spectroscopy and ESI-MS analysis of the hydroboration reactions have shown the formation of ligand-protonated [Ir(CNP)(CO)(BR2)H][B(R2)2] (R2 = catecholate, pinacolate) derivatives under catalytic conditions. Control experiments, however, have demonstrated that these derivatives are not catalytically competent species in the hydroboration of CO2.

Introducción: Los objetivos del estudio fueron valorar la utilidad de sellantes de fibrina para evitar seromas postoperatorios tras linfadenectomía axilar y comparar la incidencia de seroma axilar en 3 grupos de estudio (Tachosil(R), Tissucol(R) y control). Así mismo analizar posibles factores relacionados con la aparición del seroma. Métodos: Entre los años 2012 y 2015 se realizó un estudio prospectivo y aleatorizado en nuestro centro. Se incluyeron 110 pacientes consecutivas operadas de cáncer de mama que requirieron linfadenectomía axilar. Las pacientes fueron aleatorizadas en 3 grupos: Tachosil(R) (38 pacientes), Tissucol(R) (35) y control (37). Las variables estudio fueron: edad, peso, talla e índice de masa corporal, estadificación TNM del tumor, tipo histológico e inmunohistoquímico, número de ganglios extirpados, afectos, y uso de quimioterapia neoadyuvante. Las variables resultado fueron: débito obtenido por el drenaje desde el día 1 postoperatorio hasta la retirada del mismo, volumen total del drenaje axilar, aparición de seroma que precisara punción evacuación, volumen del seroma postoperatorio drenado y número de punciones evacuadoras hasta su resolución clínica. Resultados: No existieron diferencias significativas en el porcentaje de seromas, volumen y número de punciones evacuadoras necesarias para su resolución, ni tampoco respecto al débito a través del drenaje antes de su retirada entre los grupos de estudio. Los valores pronósticos para desarrollar seroma fueron: edad >56 años y débito al 6.° día posquirúrgico >70ml. Conclusiones: No se evidenciaron ventajas al aplicar sellantes de fibrina en relación con el volumen seroso drenado y con la formación de seroma axilar posquirúrgico


Introduction: The objectives of our study were to evaluate the usefulness of fibrin sealants in preventing the appearance of seromas after axillary lymphadenectomy and to compare the incidence of axillary seroma in three groups (Tachosil(R), Tissucol(R) and control). We also analysed the possible factors related to the development of seroma. Methods: Between 2012 and 2015, we carried out a prospective, randomised study at our centre including 110 consecutive patients undergoing breast cancer surgery who required axillary lymphadenectomy. Patients were randomised into 3 groups: Tachosil(R) (38 patients), Tissucol(R) (35) and control (37). Study variables consisted of age, weight, height and body mass index (BMI), tumour-node-metastases (TNM) stage, histological and immunohistochemical type, application of neoadjuvant chemotherapy, and the number of excised and metastasised lymph nodes. Outcome variables consisted of daily output from postoperative day 1 to the withdrawal of drainage, the total volume of axillary drainage, the appearance of postoperative seromas requiring puncture evacuation, the volume of the postoperative seroma drained and the number of seroma puncture evacuations required until clinical resolution. Results: There were no significant differences in the percentage of seromas, volume, number of puncture evacuations needed for resolution of the seroma or total flow through the drainage tubes prior to withdrawal in the 3 study groups. Prognostic values for the development of axillary seroma were age >56 years and a drained volume >70ml by the 6th postoperative day. Conclusions: We did not identify any advantages in applying fibrin sealants in relation to the serous volume drained or the incidence of postoperative axillary seroma

RESUMO

Complex [Mo2(H)2{µ-HC(NDipp)2}2(THF)2], (1·THF), reacts with C2H4 and PhCH[double bond, length as m-dash]CH2 to afford hydrido-hydrocarbyl and bis(hydrocarbyl) derivatives of the Mo[quadruple bond, length as m-dash]Mo bond. Reversible migratory insertion and ß-hydrogen elimination, as well as reductive elimination and other reactions, have been uncovered. PhC[triple bond, length as m-dash]CH behaves instead as a Brönsted-Lowry acid towards the strongly basic Mo-H bonds of 1·THF.

RESUMO

A diastereo- and highly enantioselective dynamic kinetic resolution (DKR) of configurationally labile heterobiaryl ketones is described. The DKR proceeds by zinc-catalyzed hydrosilylation of the carbonyl group, thus leading to secondary alcohols bearing axial and central chirality. The strategy relies on the labilization of the stereogenic axis that takes place thanks to a Lewis acid-base interaction between a nitrogen atom in the heterocycle and the ketone carbonyl group. The synthetic utility of the methodology is demonstrated through stereospecific transformations into either N,N-ligands or appealing axially chiral, bifunctional thiourea organocatalysts.
