RESUMO

A new kind of electrophilic alkynylthiolating reagent, called N-alkynylthio phthalimide, is designed and synthesized herein. This electrophilic sulfenylating reagent can be easily prepared in three steps from commercially available phthalimide and corresponding silver acetylide. Furthermore, the N-alkynylthio phthalimides are demonstrated to be efficient alkynylthio transfer reagents that can react with various C-nucleophiles, including ß-ketoesters, aryl boronic acids, and Grignard reagents to afford a diverse range of alkynyl thioethers under mild conditions.

BACKGROUND: Sleep benefit (SB) refers to the poorly understood phenomenon in Parkinson's disease (PD) in which patients wake up in the morning with improved motor function. Although previous studies have suggested that several related factors are associated with SB, this relationship remains controversial. OBJECTIVE: This systematic review and meta-analysis aimed to identify the possible related factors of SB in PD. METHODS: We searched PubMed, EMBASE and WanFang databases to collect eligible articles. We calculated pooled estimates of odds ratios (ORs) or the mean deviation (MD) with 95% confidence intervals. RESULTS: We found 3 related factors associated with SB: the duration of PD (MD 1.22, 95% CI: 0.21-2.23), sleep efficiency (MD -4.48,95% CI: -7.24- -2.44), and on-state MDS-UPDRS-Ⅲ total score (MD 3.05, 95% CI: 0.53-5.57). CONCLUSION: PD patients with SB are more likely to have a long duration of PD, a low sleep efficiency and a high MDS-UPDRS-Ⅲ total score. Our work helps obtain a better understanding of sleep SB in PD and its underlying mechanisms. More studies need to be conducted to evaluate the associations between clinical factors in PD and the SB phenomenon.

The worldwide rise of shared bicycle use has changed the way people travel. Here we analyze shared bicycle use from the perspective of the theory of planned behavior, and propose a model to investigate factors influencing shared bicycle usage in China. A total of 211 shared bicycle users selected from 28 provinces throughout China completed a self-reported survey. Structural equation modelling (SEM) was used to delineate the pathway from shared bicycle usage. The SEM model demonstrated that: (1) shared bicycle use intention was significantly associated with four variables, namely travel attitude(ß = 0.491, t = 24.569), social norms(ß = 0.149, t = 6.771), travel habits(ß = 0.146, t = 7.226) and perceived behavioral control (ß = 0.190, t = 11.110); (2) shared bicycle use behavior was significantly affected by shared bicycle use intention(ß = 0.406, t = 15.936), and also by travel habits(ß = 0.320, t = 11.921); (3) shared bicycle use behavior was also affected by demographic variables (gender, age) and situational factors (distance). The conclusions of this study provide useful data for operators of bicycle services and government policy makers.

BACKGROUND The present study was designed to evaluate the predictive value of head-neck computed tomography angiography (CTA) combined with ABCD2 score scale for patients with cerebral infarction of vertebrobasilar transient ischemic attack (TIA). MATERIAL AND METHODS A total of 92 patients with TIA who were admitted to our hospital from January 2014 to June 2015 were enrolled in this study. ABCD2 score and CTA combined with ABCD2 score were assessed. RESULTS The incidence of cerebral infarction was highest in the high-risk group, followed by the middle-risk group and low-risk group. The incidence of cerebral infarction was related to the degree of stenosis in head-neck CTA, which was highest in the severe stenosis group, followed by the moderate stenosis group and mild stenosis/normal group, with significant differences. The incidence of cerebral infarction in patients with cerebral artery stenosis was correlated with the incidence of cerebral infarction in the head and neck CTA, which was severe > medium > normal/low (P<0.05). CONCLUSIONS The ABCD2 score can accurately predict the early development from TIA to cerebral infarction. If it is used in combination with head-neck CTA; CTA combined ABCD2 score can further improve the accuracy of prediction, which makes it feasible for use in prediction of the development of vertebrobasilar TIA to cerebral infarction.

OBJECTIVE: To understand the distribution and characteristics of historical Oncomelania hupensis snail environments in Baoshan District, Shanghai City, so as to provide the reference for adjusting and formulating the monitoring program of the snail situation in the future. METHODS: A retrospective survey and a field survey were conducted to collect the information of historical O. hupensis snail environments and set up the database. The distribution and characteristics of them were analyzed. RESULTS: There were 53 historical O. hupensis snail environments which distributed in three towns, and the accumulative area was 1 635 530 m2 in Baoshan District, Shanghai City. Class I, II, III and IV accounted for 73.58%, 5.66%, 16.98% and 3.77% respectively. The complete transformation (Class I and Class II) was done in 42 places which covered the area of 1 622 043 m2, in which Class I environments and Class II environments accounted for 79.25% and 99.18% of the total, respectively. CONCLUSIONS: There has been a great change in the historical O. hupensis snail environments in Baoshan District, Shanghai City. The information of the field investigation should be enriched and updated at different times to guide the investigation of O. hupensis snails in the future.

In this study, a field experiment was conducted to estimate the effects of simulated nitrogen (N) deposition (low-N: 30 kg N·hm-2·a-1, moderate-N: 60 kg N·hm-2·a-1, high-N: 90 kg N·hm-2·a-1) on ecological stoichiometry of leaf litter in Moso bamboo forest under conventional management (CM) and intensive management (IM) practices. The results showed that compared with CM, IM significantly increased C, N and P concentrations of leaf litter by 9.3%, 32.4% and 22.7%, respectively, but significantly decreased C:N, C:P and N:P ratios by 17.4%, 54.3% and 44.6%, respectively. In CM Moso bamboo plots, low- and moderate-N deposition significantly increased C, N and P concentrations of leaf litter but decreased C:N, C:P and N:P, high-N deposition significantly increased C, N concentrations, C:P and N:P, but decreased P concentration. In IM Moso bamboo plots, low-N deposition significantly increased P concentration but decreased C concentrations, C:P and N:P, moderate-N deposition significantly increased N and P concentrations but decreased C concentration, C:N, C:P and N:P, high-N deposition significantly increased C:N, C:P and N:P but decreased P concentration. The interaction of management intensity and N deposition significantly influenced stoichiometry of leaf litter, except for C:N. The P concentration of leaf litter was significantly correlated with P concentration in soils.

Objective: The objective of this article is to study the etiology of community-acquired pneumonia in children with airway malacia. Methods: We retrospectively reviewed the medical records of 428 pneumonia patients. All patients underwent bronchoscopy, and bronchoalveolar lavage samples were processed for microbiological assessment. Results: In a total of 428 cases reviewed, 60 were found to have airway malacia. Pathogens were identified in 44 of the 60 specimens (73.3%), with 32 being single-pathogen infections. The most common pathogen was respiratory syncytial virus (RSV; 20%). Mixed-pathogen infections were observed in 12 patients. Airway malacia patients were younger than those without malacia (10.5 vs. 50 months, respectively; p < 0.001). Compared with those without airway malacia, wheezing, cyanosis and admission to the pediatric intensive care unit were more common in children with airway malacia and their hospital stay was longer. Conclusion: RSV was the most common pathogen in those with airway malacia. Airway malacia was found to aggravate infectious pneumonia.

The microstructure and properties of Super304H weld joints were developed and studied. The Creq and Nieq composition points of the deposited metal were all located in the austenite region. The quantity and size of the Nb-phase in the deposited metal with 0.28wt.% Nb were lower than tnat in the other two deposited metals. The phases were mainly γ and Nb(C, N). The content of Nb(C, N) phases in the deposited metal with the 0.6 wt.% Nb content was approximately 2.2 times of the deposited metal with the 0.28 wt.% Nb content. The Nb-phases in the deposited metal with 0.28 wt.% Nb content were lower in quantity, weaker in the strengthening effect, of lower yield strength and lower in size than that the deposited metal with the 0.6 wt.% Nb content.

The inhibitory impacts of spikes on LFP theta rhythms(4-8Hz) are investigated around sporadic spikes(SSs) based on intracerebral EEG of 4 REM sleep patients with temporal lobe epilepsy(TLE) under the pre-surgical monitoring. Sequential interictal spikes in both genesis area and extended propagation pathway are collected, that, SSs genesis only in anterior hippocampus(aH)(possible propagation pathway in Entorhinal cortex(EC)), only in EC(possible propagation pathway in aH), and in both aH and EC synchronously. Instantaneous theta power was estimated by using Gabor wavelet transform, and theta power level was estimated by averaged over time and frequency before SSs(350ms pre-spike) and after SSs(350ms post-spike). The inhibitory effect around spikes was evaluated by the ratio of theta power level difference between pre-spike and post-spike to pre-spike theta power level. The findings were that theta power level was reduced across SSs, and the effects were more sever in the case of SSs in both aH and EC synchronously than either SSs only in EC or SSs only in aH. It is concluded that interictal spikes impair LFP theta rhythms transiently and directly. The work suggests that the reduction of theta power after the interictal spike might be an evaluation indicator of damage of epilepsy to human cognitive rhythms.

Gastric cancer (GC) is one of the most frequent epithelial malignancies worldwide. The gastrointestinal (GI) peptide gastrin is an important regulator of the secretion and release of gastric acid from stomach parietal cells, and it also plays a vital role in the development and progression of GC. The aim of the current study was to investigate the role and underlying mechanism of gastrin and autophagy in regulating GC tumorigenesis. Gastrin-17 amide (G-17) was applied in the GC cell lines SGC7901 and MGC-803. The results showed that G-17 maintained the high viability of SGC7901 and MGC-803. The expression of autophagy marker proteins LC3II and Beclin1 was significantly increased, while the autophagy substrate p62 was obviously decreased in the gastrin group compared with the control group. Moreover, G-17 strengthened the expressions of AMPKα, Ras, Raf, MEK, and ERK1/2. Additionally, administration of AMPKα siRNA counteracted the effect of gastrin in SGC7901 cells. Finally, in an in vivo study of the tumor growth and survival rate of rats, the levels of AMPKα/Ras/Raf/MEK/ERK were significantly increased in the gastrin group and decreased following AMPKα shRNA injection. In conclusion, these findings indicate that gastrin plays a tumorigenic role by promoting autophagy in GC and may provide a novel therapeutic target for GC treatment.

Hearing impairment, or deafness (in its most severe form), is one of the most common human sensory disorders. There have been several reports of autosomal dominant mutations in the POU4F3 gene, which is associated with non-syndromic hearing loss. In this study, we identified a novel heterozygous mutation (c.602delT, p.L201fs) in the gene POU4F3 by taking advantage of whole-exome sequencing, which was validated by Sanger sequencing and completely co-segregated within a large hearing impaired Chinese family. We have focused on this pedigree since 2002, and we have mapped a deafness locus named DFNA42 (which has been renamed DFNA52, OMIM entry 607683) via a genome-wide scan. Furthermore, we analyzed this mutational variant and found that it was located at the beginning of the first functional domain of POU4F3, which could theoretically impair the function of POU4F3. We have identified a novel frameshift mutation in the POU4F3 gene. Further functional studies of variants of this specific gene are needed to illustrate the pathogenic mechanism(s) that underlie hearing impairment.

In this study, a field experiment was conducted using simulated N deposition method in conventionally managed (CM) and intensively managed (IM) Moso bamboo forests to test the responses of five soil enzymes (including ß-fructofuranosidase, cellulase, nitrate reductase, hydrogen peroxidase and urease) to simulated N deposition of four treatment levels of 0, 30, 60 and 90 kg N·hm-2·a-1. The results showed that compared with CM, IM significantly increased soil ß-fructofuranosidase, cellulase, urease activities by 55.5%, 112.9% and 28.6%, respectively, but significantly decreased nitrate reductase activity by 31.5%, and had no significant effect on hydrogen peroxidase activity. Nitrogen deposition significantly decreased soil ß-fructofuranosidase (20.0%-49.4% and 36.2%-45.1%), cellulase (20.5%-46.3% and 18.3%-49.0%), nitrate reductase (67.9%-85.2% and 15.2%-34.2%) activities in both CM and IM plots, respectively, and also urease activity (23.1%-47.6%) in IM, but enhanced the urease activity (8.1%-50.6%) in CM, and had no significant effects on hydrogen peroxidase activity. The combination of N deposition and management types significantly impacted the four soil enzyme activities except for hydrogen peroxidase.

BACKGROUND: Although many studies have investigated the relationship between the introduction of the New Cooperative Medical Scheme (NCMS) in rural China in 2003 and increased use of medical services, the effect on health status, objectively measured, is seldom reported. In Anhui Province a chronic disease scheme (CDS) for reimbursing part of the cost of outpatient care is designed to improve management of those with chronic conditions, including diabetes. METHODS: A follow-up study was designed in which patients with diabetes aged 40-70 years who had recently (in 2010) been granted a chronic disease card were individually matched on age, sex and village with a patient with diabetes not yet in the scheme. Each subject gave a fingertip sample of blood to give the concentration of glycosylated hemoglobin (HbA1c), a measure indicating blood glucose control during the previous 3 months. This measure was made on recruitment and at 12 month follow-up: information on use of health services, quality of life and financial burden was also collected at the two contacts. RESULTS: Of 602 pairs initially recruited, 528 pairs were contacted at follow-up and are the subject of this report. To distinguish between outcomes associated with application and those of membership of the scheme, the primary analysis was of 256 pairs in which one had been a member of the CDS throughout and the other never applied. No difference between pairs on HbA1c was found either at recruitment or follow-up but those in the CDS reported more hospital visits, more tests and more use of high level hospitals. However they had poorer scores on quality of life scales (SF-12, EQ-5D) and were more likely to report that the financial costs were very burdensome. Those recently applying for the scheme, or being accepted since recruitment, had lower HbA1c scores. CONCLUSIONS: On-going membership of the CDS was associated with increased use of services but this did not appear to result in better management of blood glucose or improved quality of life. Those who had recently joined the scheme had signs of improvement, suggesting a need for active follow-up to maintain and reinforce early gains.

Circulating microRNA has recently emerged as a promising biomarker for cardiovascular disease. This study sought to evaluate the diagnostic and prognostic value of circulating miR-133a as a marker of acute myocardial infarction in acute chest pain patients undergoing coronary angiography.Plasma was collected from 312 patients with chest pain on admission in the emergency department and 67 healthy controls. MiR-133a was detected using real-time quantitative PCR and enhanced accu-TnI, creatinine kinase-MB mass, and myoglobin were measured by immunoassay. End-point events (serious adverse cardiovascular events which require hospitalization or cardiovascular death) were examined in the AMI (acute myocardical infarction) group within 1, 6, 12, and 24 months.The miR-133a level was higher in AMI patients than in non-AMI patients (P < 0.001). In the ROC analysis, the sensitivity of miR-133a in diagnosis of AMI is 0.61 and the specificity is 0.68. In the prognostic analysis, only 1 endpoint event was observed in the non-AMI group; the amount of cases with end-point events in the AMI group at 1,6,12, and 24 months were 8, 19, 28, and 35, respectively. The cutoff value of miR-133a was determined using the median value of the AMI group and separated the patients into a positive group and a negative group. The Kaplan-Meier survival curve showed no significant difference in survival was detected in AMI patients between the miR-133a positive group and negative group after follow-up (12-month: x2 = 1.353, P = 0.245; 24-month: x2 = 3.722, P = 0.054). After adjusting for age, gender, Killip classes, prior myocardiac infarction history, myoglobin, LVEF (left ventricular ejection fraction), diabetes, hypertension, smoking and systolic blood pressure, miR133a had a significant association with the risk of events at 12 months (HR = 2.869, P = 0.024) and 24 months (HR = 3.936, P = 0.001).In patients undergoing coronary angiography, circulating miR-133a is upregulated in AMI patients, but it does not provide enough accuracy for clinical AMI diagnosis because it also rises in unstable angina patients. Its prognostic value in AMI is uncertain mainly for the number of cases with end-point event was small and may be further validated in a larger, better designed study.

OBJECTIVE: To explore the sample unit size effects of spatial pattern of Oncomelania hupensis population in the marshland schistosomiasis epidemic areas. METHODS: The push-broom method was performed to survey the snails in the study site in a basic unit size of 0.33 m×0.33 m in Poyang Lake region of Jiangxi Province. After combining different amount of survey cells, the aggregation indicators, regression models and aggregating factor index were employed to study the spatial distribution pattern. RESULTS: The snail population was in aggregation pattern and the smaller sampling unit could produce higher degrees of aggregation. The regression models showed that the snails distributed in the form of individual groups with attraction with each other and the aggregation was related with the snail density. The clustering factor index demonstrated that the aggregation was introduced by aggregating behavior habit of the snails and environmental factors, or only by snail habit itself, and the aggregating habit of snails could be expressed better by the smaller sampling unit. CONCLUSIONS: The spatial pattern of O. hupensis population in the marshland schistosomiasis epidemic areas is in aggregation pattern, and the sample unit size can affect the calculation results of the related biological indices.

OBJECTIVE: To optimize and simplify the survey method of Oncomelania hupensis snail in marshland endemic region of schistosomiasis and increase the precision, efficiency and economy of the snail survey. METHODS: A quadrate experimental field was selected as the subject of 50 m×50 m size in Chayegang marshland near Henghu farm in the Poyang Lake region and a whole-covered method was adopted to survey the snails. The simple random sampling, systematic sampling and stratified random sampling methods were applied to calculate the minimum sample size, relative sampling error and absolute sampling error. RESULTS: The minimum sample sizes of the simple random sampling, systematic sampling and stratified random sampling methods were 300, 300 and 225, respectively. The relative sampling errors of three methods were all less than 15%. The absolute sampling errors were 0.221 7, 0.302 4 and 0.047 8, respectively. CONCLUSIONS: The spatial stratified sampling with altitude as the stratum variable is an efficient approach of lower cost and higher precision for the snail survey.

Systematic reviews (SRs) of randomized controlled trials (RCTs) have demonstrated acupuncture's effectiveness in stroke rehabilitation. The current study reviews the quality of evidence in SRs of acupuncture in stroke rehabilitation, and rates the strength of recommendation for its use based on this evidence using the GRADE (grading of recommendations, assessment, development and evaluations) approach. A comprehensive literature search was performed using multiple databases (e.g., Medline, Embase) with advanced search strategies. Two authors independently selected articles, collected data, and assessed the methodological quality of each identified SR according to AMSTAR (a measurement tool to assess systematic reviews) and OQAQ (Oxman and Guyatt's overview quality assessment questionnaire). Outcomes related to stroke rehabilitation were evaluated. SRs of high methodological quality (AMSTAR score ≥9 and OQAQ score ≥7) were graded using GRADE. Ultimately, acupuncture yields benefits in stroke rehabilitation (neurological function improvement: RR = 1.34; swallowing improvement: RR = 1.61, 1.49, 1.07; disability: SMD = 0.49 or 0.07). Poor evidentiary quality and insufficient information about harm led to weak recommendations. In conclusion, acupuncture may improve stroke rehabilitation, as the GRADE approach indicated a weak recommendation for acupuncture's usage in this context.

Nasopharyngeal carcinoma (NPC) is a common disease in the southern provinces of China with a poor prognosis. To better understand the pathogenesis of NPC and identify proteins involved in NPC carcinogenesis, we applied iTRAQ coupled with two-dimensional LC-MS/MS to compare the proteome profiles of NPC tissues and the adjacent non-tumor tissues. We identified 54 proteins with differential expression in NPC and the adjacent non-tumor tissues. The differentially expressed proteins were further determined by RT-PCR and Western blot analysis. In addition, the up-regulation of HSPB1, NPM1 and NCL were determined by immunohistochemistry using tissue microarray. Functionally, we found that siRNA mediated knockdown of NPM1 inhibited the migration and invasion of human NPC CNE1 cell line. In summary, this is the first study on proteome analysis of NPC tissues using an iTRAQ method, and we identified many new differentially expressed proteins which are potential targets for the diagnosis and therapy of NPC.

BACKGROUND: Reference intervals are important for interpretation of clinical laboratory tests. The platelet (PLT) indices such as the mean platelet volume (MPV) and platelet distribution width (PDW) are newer hematological parameters, which have been recently reported as clinically valuable biomarkers. However, there are not many studies that have estimated the reference intervals for these parameters in healthy Chinese Han adults. OBJECTIVE: The objectives of this study were to establish reference values of PLT indices [including PLT count, MPV, PDW, platelet-large cell ratio (P-LCR), and plateletcrit (Pct)] for healthy Chinese Han adults. We also aimed to determine the region-based differences of PLT indices in China. METHODS: A total of 4,642 volunteers with a mean age of 43 were recruited from six regions of China. PLT indices were performed on Sysmex XE-2100 hematology analyzers, whose traceability was well verified. RESULTS: There were significant region-based differences for all PLT indices. Reference people in Chengdu had the lowest mean PLT count and Pct, but the highest MPV, PDW, and P-LCR among the six regions. Therefore, we derived the reference intervals in Chinese Han population excluding Chengdu reference people for PLT indices as PLT count: (127-341) × 10(9)/l; MPV: (9.20-13.30) fl; PDW: 9.90-19.00%; P-LCR: 18.10-52.00%; Pct: 16.00-41.00%. CONCLUSIONS: Region-specific reference intervals are essential as there were statistically significant region-related differences in the PLT parameters. The reference intervals established in this study differed from the existing reference values. Chengdu region may need proper specific reference ranges, which apply to their people, for all PLT parameters.
