RESUMO

HYPOTHESIS: Factor Va (FXa) and Xa (FVa) can assemble on the phosphatidylserine (PS) membrane (of platelet) to form prothrombinase complex and contribute to blood clotting. Very recently, we discovered that Ca-zeoliteacts as a type of reinforced activated inorganic platelet to enable assembly of prothrombinase complex and display an unusual zymogen (prothrombin) activation pattern. Inspired but not constrained by nature, it is of great interest to understand how FVa and FXa assembly on the inorganic surface (e.g., zeolites) and perform their biocatalytic function. EXPERIMENTS: Given the important role of FVa C1-C2 domains in the assembly and activity of the prothrombinase complex, in this work, molecular dynamics simulations were performed to investigate the binding details of FVa A3-C1-C2 domains on the PS membranes and Ca2+-LTA-type (CaA) zeolite surface. FINDINGS: We found that different from the natural PS membrane, FVa light chain repeatedly exhibits a strong C2 domain anchoring interaction on the CaA zeolite. It mainly arises from the porous surface structure of CaA zeolite and local highly dense solvation water clusters on the CaA zeolite surface restrict the movement of some lysine residues on the C2 domain. The anchoring interaction can be suppressed by reducing the surface negative charge density, so that FVa light chain can change from single-foot (only C2 domain) to double-foot (both C1-C2 domain) adsorption states on the zeolite surface. This double-foot adsorption state is similar to natural PS membrane systems, which may make FVa have higher cofactor activity.

In order to estimate the associations of different adiposity indicators and long-term adiposity changes with risk of incident type 2 diabetes (T2DM), we conducted a 10-year prospective cohort study of 7441 adults in Guizhou, China, from 2010 to 2020. Adiposity was measured at baseline and follow-up. Cox proportional hazard models were used to estimated hazard ratios (HRs) and 95% confidence intervals (95% CIs). A total of 764 new diabetes cases were identified over an average follow-up of 7.06 years. Adiposity indicators, body mass index (BMI), waist circumference (WC), waist-height ratio (WHtR), and long-term adiposity changes (both weight change and WC change) were significantly associated with an increased risk of T2DM (adjusted HRs: 1.16-1.48). Significant non-linear relationships were found between weight/WC change and incident T2DM. Compared with subjects with stable WC from baseline to follow-up visit, the subjects with WC gain ≥9 cm had a 1.61-fold greater risk of T2DM; those with WC loss had a 30% lower risk. Furthermore, the associations were stronger among participants aged 40 years or older, women, and Han Chinese. Preventing weight or WC gain and promoting maintenance of normal body weight or WC are important approaches for diabetes prevention, especially for the elderly, women, and Han Chinese.

We aimed to explore the association between the efficiency score and the risk of hypertension. We conducted a prospective cohort study of 2412 adults aged 40 years or above without hypertension in Guizhou, China from 2010 to 2020. The data envelopment analysis input-oriented DEA-CCR model was used to calculate the efficiency scores. The Cox regression model was used to assess the relationship between the efficiency score and incident hypertension. The dose-response relationship was evaluated by restricted cubic spline. Quantile regression was used to analyze the effect of efficiency scores on SBP and DBP. A total of 857 new hypertension cases were identified with a mean follow-up of 6.88 years. The efficiency score was lower in the new hypertension cases than participants without hypertension (0.70 vs. 0.67). After adjusting for possible confounding factors, the HR of hypertension risk was 0.20 (95%CI: 0.09, 0.42) for per 0.1 increase in the efficiency score. The dose-response relationship showed a non-linear relationship between the efficiency score and hypertension risk. Our results showed that the efficiency score was a cost-effective tool to identify those at a high risk of hypertension, and suggested targeted preventive measures should be undertaken.

To solve the problem of strong adhesion and excessive blood loss caused by the use of hydrophilic zeolite gauze (Z-Gauze) in uncontrollable bleeding, we have modified the surface of commercial Z-Gauze with a paraffin coating and prepared a hydrophobic dressing PZ-Gauze. After paraffin coating, the adhesion of Z-Gauze was reduced without an obvious decrease in coagulation activity. The clotting time of the hydrophobic PZ-Gauze was reduced from 378.3 to 154.6 s compared with that of cotton gauze, and the peeling force was decreased from 348.8 to 84.7 mN compared with that of Z-Gauze. Besides, PZ-Gauze can efficiently cut down the blood loss during treatment. On the basis of in vitro and in vivo experiments, it is confirmed that surface hydrophobic modification does not change the procoagulant performance because of the maintained cation exchange capacity of zeolites, and the reduced blood loss as well as enhanced difficulty for fibrin adhesion is attributed to its hydrophobicity. This is different from the traditional gauze procoagulant theories, where gauze hydrophilicity and procoagulant performance are always positively correlated.

Leukemia is the most common malignancy affecting children. The morphologic analysis of bone marrow smears is an important initial step for diagnosis. Recent publications demonstrated that artificial intelligence is able to classify blood cells but a long way from clinical use. A total of 1,732 bone marrow images were used for the training of a convolutional neural network (CNN). New techniques of deep learning were integrated and an end-to-end leukemia diagnosis system was developed by using raw images without pre-processing. The system creatively imitated the workflow of a hematologist by detecting and excluding uncountable and crushed cells, then classifying and counting the remain cells to make a diagnosis. The performance of the CNN in classifying WBCs achieved an accuracy of 82.93%, precision of 86.07% and F1 score of 82.02%. And the performance in diagnosing acute lymphoid leukemia achieved an accuracy of 89%, sensitivity of 86% and specificity of 95%. The system also performs well at detecting the bone marrow metastasis of lymphoma and neuroblastoma, achieving an average accuracy of 82.93%. This is the first study which included a wider variety of cell types in leukemia diagnosis, and achieved a relatively high performance in real clinical scenarios.

BACKGROUND: Poor balance has been cited as one of the key causal factors of falls. Timely detection of balance impairment can help identify the elderly prone to falls and also trigger early interventions to prevent them. The goal of this study was to develop a surrogate approach for assessing elderly's functional balance based on Short Form Berg Balance Scale (SFBBS) score. METHODS: Data were collected from a waist-mounted tri-axial accelerometer while participants performed a timed up and go test. Clinically relevant variables were extracted from the segmented accelerometer signals for fitting SFBBS predictive models. Regularized regression together with random-shuffle-split cross-validation was used to facilitate the development of the predictive models for automatic balance estimation. RESULTS: Eighty-five community-dwelling older adults (72.12 ± 6.99 year) participated in our study. Our results demonstrated that combined clinical and sensor-based variables, together with regularized regression and cross-validation, achieved moderate-high predictive accuracy of SFBBS scores (mean MAE = 2.01 and mean RMSE = 2.55). Step length, gender, gait speed and linear acceleration variables describe the motor coordination were identified as significantly contributed variables of balance estimation. The predictive model also showed moderate-high discriminations in classifying the risk levels in the performance of three balance assessment motions in terms of AUC values of 0.72, 0.79 and 0.76 respectively. CONCLUSIONS: The study presented a feasible option for quantitatively accurate, objectively measured, and unobtrusively collected functional balance assessment at the point-of-care or home environment. It also provided clinicians and elderly with stable and sensitive biomarkers for long-term monitoring of functional balance.

BACKGROUND: Telehealth is an effective means to assist existing health care systems, particularly for the current aging society. However, most extant telehealth systems employ individual data sources by offline data processing, which may not recognize health deterioration in a timely way. OBJECTIVE: Our study objective was two-fold: to design and implement an integrated, personalized telehealth system on a community-based level; and to evaluate the system from the perspective of user acceptance. METHODS: The system was designed to capture and record older adults' health-related information (eg, daily activities, continuous vital signs, and gait behaviors) through multiple measuring tools. State-of-the-art data mining techniques can be integrated to detect statistically significant changes in daily records, based on which a decision support system could emit warnings to older adults, their family members, and their caregivers for appropriate interventions to prevent further health deterioration. A total of 45 older adults recruited from 3 elderly care centers in Hong Kong were instructed to use the system for 3 months. Exploratory data analysis was conducted to summarize the collected datasets. For system evaluation, we used a customized acceptance questionnaire to examine users' attitudes, self-efficacy, perceived usefulness, perceived ease of use, and behavioral intention on the system. RESULTS: A total of 179 follow-up sessions were conducted in the 3 elderly care centers. The results of exploratory data analysis showed some significant differences in the participants' daily records and vital signs (eg, steps, body temperature, and systolic blood pressure) among the 3 centers. The participants perceived that using the system is a good idea (ie, attitude: mean 5.67, SD 1.06), comfortable (ie, self-efficacy: mean 4.92, SD 1.11), useful to improve their health (ie, perceived usefulness: mean 4.99, SD 0.91), and easy to use (ie, perceived ease of use: mean 4.99, SD 1.00). In general, the participants showed a positive intention to use the first version of our personalized telehealth system in their future health management (ie, behavioral intention: mean 4.45, SD 1.78). CONCLUSIONS: The proposed health monitoring system provides an example design for monitoring older adults' health status based on multiple data sources, which can help develop reliable and accurate predictive analytics. The results can serve as a guideline for researchers and stakeholders (eg, policymakers, elderly care centers, and health care providers) who provide care for older adults through such a telehealth system.

Background: Anemia caused by left ventricular outflow tract obstruction in patients with hypertrophic obstructive cardiomyopathy (HOCM) has been reported, however, large clinical studies confirming this association are lacking. The objective of the present study was to investigate the relationship between left ventricular outflow tract (LVOT) pressure gradient and hemoglobin in patients with hypertrophic cardiomyopathy (HCM). Methods: Patient demographics, laboratory and echocardiography data from 310 patients diagnosed with HCM from our hospital who had undergone echocardiography from July 2014 to March 2019 were collected from medical records. Patients were classified into HOCM and non-HOCM groups. Results: Compared to the non-HOCM group, patients in the HOCM group had a lower hemoglobin level (112.2 ± 16.7 vs. 132.9 ± 22.2 g/L, p < 0.001). In addition, significant negative correlations between hemoglobin and LVOT pressure gradient were found in males (r = -0.568, p < 0.001) and females (r = -0.589, p < 0.001). Receiver operating characteristic curve analysis revealed that the best cut-off value for hemoglobin to predict HOCM in male patients was 128 g/L with 74.19% sensitivity and 75.51% specificity (area under the curve: 0.763, p < 0.001). For female patients, the cut-off value was 125 g/L, with a sensitivity and specificity of 89.39% and 48.48%, respectively (area under the curve: 0.718, p < 0.001). Conclusions: Our results indicate that hemoglobin level is inversely proportional to the LVOT gradient pressure and has value for predicting outflow tract obstruction in patients with HCM.

BACKGROUND: The serum alanine aminotransferase (ALT) level is a critical parameter for evaluating liver injury in non-alcoholic fatty liver disease (NAFLD). However, the currently accepted upper limits of normal (ULN) for serum ALT (ULN-ALT) are debated, as they may be excessively high. METHODS: A total of 1638 children aged 6-16 years, comprising 507 children with normal BMI (500 healthy children and 7 children with NAFLD), 199 overweight children, and 932 obese children, were included in the analysis. We re-evaluated the ULN-ALT in 500 healthy Chinese children using the 95th percentiles of serum ALT levels as revised ULN-ALT. Fatty liver was identified by ultrasound examination. RESULTS: Significant positive correlations between serum ALT levels and body mass index (BMI) were detected in overweight boys (r = .399, P < .001), obese boys (r = .398, P < .001), and obese girls (r = .392, P < .001). The prevalence percentages of NAFLD were 93.6%, 75.8%, and 37.9% in obese boys with serum ALT levels of >50, 25-50, and ≤25 U/L and were 81.6%, 67.9%, and 20.6% in obese girls with serum ALT levels of >40, 20-40, and ≤20 U/L, respectively. CONCLUSION: Serum ALT levels significantly correlated with abnormal BMI values in children, suggesting a rigorous BMI threshold is needed to establish the cutoffs for serum ULN-ALT in children. Besides, the revised serum ULN-ALT can uncover mild liver injury in obese children with NAFLD.

It is challenging to develop a low-cost household water treatment (HWT) that simultaneously deliver an effective and robust way for safe and reliable water supply. Here, we report a simple flow-through filter made by zeolite-cotton packing in a tube (ZCT) as low-cost HWT device to remove heavy metal ions from contaminated water. The zeolite-cotton is fabricated by an on-site template-free growth route that tightly binds mesoporous single-crystal chabazite zeolite onto the surface of cotton fibers. As a result, the ZCT set-up with optimized diameter achieves both high adsorption efficiency, proper flow rate, reliable supply and strong stability at the same time. After flowed through the set up packed with 10 g of zeolite-cotton, 65 mL 1000 ppm Cu2+ solution was purified down to its safety limit (<1 ppm). Notably, their efficiency remains unaltered when filtering several ions simultaneously. In a simulated purification process, 8 L of water contaminated by Cu2+, Cd2+ and Pb2+ could be transformed into drinking water and it enables the removal of heavy metals to concentrations of below 5 ppb (µg L-1). We also show that the ZCT can be used for disinfection by introducing Ag-exchanged zeolite-cotton without contaminating the water with Ag ions (<0.05 ppm).

Achieving rapid definitive hemostasis is essential to ensure survival of patients with massive bleeding in pre-hospital care. It is however challenging to develop hemostatic agents or dressings that simultaneously deliver a fast, long-lasting and safe treatment of hemorrhage. Here, we integrate meso-/micro-porosity, blood coagulation and stability into a flexible zeolite-cotton hybrid hemostat. We employ an on-site template-free growth route that tightly binds mesoporous single-crystal chabazite zeolite onto the surface of cotton fibers. This hemostatic material maintains high procoagulant activity after water flow treatment. Chabazite particles are firmly anchored onto the cotton surface with < 1% leaching after 10 min of sonication. The as-synthesized hemostatic device has superior hemostatic performance over most other clay or zeolite-based inorganic hemostats, in terms of higher procoagulant activity, minimized loss of active components and better scalability for practical applications (a hemostatic T-shirt is hereby demonstrated as an example).

Aggregation-induced emission (AIE) active cellulose nanocrystals (TPE-CNCs) were synthesized by attaching tetraphenylethylene (TPE) to cellulose nanocrystals (CNCs). The structure and morphology of TPE-CNCs were characterized by FT-IR, XRD, ζ-potential measurements, elemental analysis, TEM, atomic force microscopy (AFM), and dynamic laser light scattering (DLS). Fluorescent properties of TPE-CNCs were also further studied. Unlike aggregation-caused quenching (ACQ), TPE-CNCs emitted weak fluorescence in the dilute suspensions, while emitting efficiently in the aggregated states. The AIE mechanism of TPE-CNCs was attributed to the restriction of an intramolecular rotation (RIR) process in the aggregated states. TPE-CNCs displayed good dispersity in water and stable fluorescence, which was reported through the specific detection of nitrophenolic explosives in aqueous solutions by a fluorescence quenching assay. The fluorescence emissions of TPE-CNCs showed quantitative and sensitive responses to picric acid (PA), 2,4-dinitro-phenol (DNP), and 4-nitrophenol (NP), and the detection limits were 220, 250, and 520 nM, respectively. Fluorescence quenching occurred through a static mechanism via the formation of a nonfluorescent complex between TPE-CNCs and nitrophenolic analytes. A fluorescence lifetime measurement revealed that the quenching was a static process. The results demonstrated that TPE-CNCs were excellent sensors for the detection of nitrophenolic explosives in aqueous systems, which has great potential applications in chemosensing and bioimaging.

Asthenozoospermia is a common cause of male infertility, the aetiology of which remains unclear in 50-60% of cases. The current study aimed to characterize metabolic alterations in asthenozoospermic seminal plasma and to explore the signalling pathways involved in sperm motility regulation. At first, high-performance liquid chromatography-electrospray ionization-tandem mass spectrometry was used to detect the targeted metabolic network of arachidonic acid (AA). Metabolomic multivariate data analysis showed significant distinction of AA metabolites between asthenozoospermic and healthy seminal plasma. AA as well as its lipoxygenase (LOX) and cytochrome P450 metabolites were found to be abnormally increased, while cyclooxygenase (COX) metabolites were complicatedly disturbed in asthenozoospermic volunteers compared with those in healthy ones. In vitro experiments and western blot analysis of sperm cells revealed a decrease in sperm motility and upregulation of sperm phosphor-P38 induced by AA. P38 inhibitor could increase AA-reduced sperm motility. Also, all the inhibitors of the three metabolic pathways of AA could block AA-induced P38 mitogen-activated protein kinase (MAPK) activation and further improve sperm motility. We report here for the first time that an abnormal AA metabolic network could reduce sperm motility via P38 MAPK activation through the LOX, cytochrome P450 and COX metabolic pathways, which might be an underlying pathomechanism of asthenozoospermia.

Zeolite is a multifunctional material, which recently exhibited promising prospects in emerging biological and medical applications. This study reported a new bio-inorganic hybrid of zeolite-fibrin clot composite serving as haemostatic agent in haemophilia A. The zeolite-fibrin clot composite promoted haemocompatibility and helped to achieve short clotting time both in vitro (22 ± 3 s vs. >600 s) and in vivo (4.5 min vs. >60 min) compared to control in coagulation factor VIII deficiency bleeding model. Therefore, in situations of surgical operation or accidental injury, this effective and ready-to-use haemostatic agent may provide rapid haemostasis for haemophilia A patients.

A solid surface morphology is of great importance for the fundamental research in the field of hydrophobic materials. Commercial sand core funnels (SCs) are embedded with multilevel pore size and surface roughness, which are excellent models to study the mechanism of surface wettability. This article described a simple, green, and facile method to fabricate hydrophobic surfaces on SCs via reacting with perfluorooctyltriethoxysilane (PFTS) vapor. Systematic analyses on the reaction, properties, and applications of the PFTS-modified SCs were conducted, which involved the reaction time and temperature, water resistance, mechanical durability, self-cleaning test, surface adhesion, and underoil superhydrophobicity. The water contact angle of the modified SCs increased with a decrease of the pore size and an increase of the surface roughness of the sand core particles. The wettability of the modified SCs agrees well with the intermediate states between Wenzel and Cassie-Baxter. The PFTS-modified SCs retained excellent chemical stability in rigid conditions and good mechanical properties. The hydrophobic SCs showed oil/water separation performance with excellent efficiency, reusability, and high flux. Especially for the PFTS-modified SCs with small pore sizes, water-in-oil emulsion separation was successfully realized. The easily accessible, relatively cheap raw materials and facile process in this work are very desirable to obtain a specific wetting surface, which will offer promising applications in various fields.

A dual physical crosslinking (DPC) strategy based on the synergistic effects of electrostatic and dipole-dipole interactions was used to construct strong and tough hydrogels. The DPC hydrogels were synthesized by one-step copolymerization of acrylic acid (AAc) and acrylonitrile (AN) monomers in chitosan (Ch) solutions. The electrostatic and dipole-dipole interactions served as dynamic but highly stable associations, endowing the hydrogels with high strength, toughness and resistance to swelling. The mechanical properties of the DPC hydrogels strongly depended on the content of components. By adjusting the concentrations of Ch, AAc and AN in the prepolymerization solution, the hydrogels displayed the maximum tensile strength (0.82 MPa) and toughness (2.72 MJ m-3) while being subjected to a strain of more than 800%. The dual noncovalent bonding strengthening mechanism offered a universal strategy for enhancing the comprehensive mechanical properties of hydrogels. Moreover, the DPC hydrogels displayed good adhesive capability, reversible painting and moldable behavior.

Acute promyelocytic leukemia (APL) is characterized by t(15;17)(q22;q21), resulting in a PML-RARA fusion that is the master driver of APL. A few cases that cannot be identified with PML-RARA by using conventional methods (karyotype analysis, FISH, and RT-PCR) involve abnormal promyelocytes that are fully in accordance with APL in morphology, cytochemistry, and immunophenotype. To explore the mechanisms involved in pathogenesis and recurrence of morphologically diagnosed APL, we performed comprehensive variant analysis by next-generation sequencing in 111 pediatric patients morphologically diagnosed as APL. Structural variant (SV) analysis in 120 DNA samples from both diagnosis and relapse stage identified 95 samples with RARA rearrangement (including 94 with PML-RARA and one with NPM-RARA) and two samples with KMT2A rearrangement. In the eligible 13 RNA samples without any RARA rearrangement at diagnosis, one case each with CPSF6-RARG, NPM1-CCDC28A, and TBC1D15-RAB21 and two cases with a TBL1XR1-RARB fusion were discovered. These uncovered fusion genes strongly suggested their contributions to leukemogenesis as driver alternations and APL phenotype may arise by abnormalities of other members of the nuclear receptor superfamily involved in retinoid signaling (RARB or RARG) or even by mechanisms distinct from the formation of aberrant retinoid receptors. Single-nucleotide variant (SNV) analysis in 77 children (80 samples) with RARA rearrangement showed recurrent alternations of primary APL in FLT3, WT1, USP9X, NRAS, and ARID1A, with a strong potential for involvement in pathogenesis, and WT1 as the only recurrently mutated gene in relapsed APL. WT1, NPM1, NRAS, FLT3, and NSD1 were identified as recurrently mutated in 17 primary samples without RARA rearrangement and WT1, NPM1, TP53, and RARA as recurrently mutated in 9 relapsed samples. The survival of APL with RARA rearrangement is much better than without RARA rearrangement. Thus, patients morphologically diagnosed as APL that cannot be identified as having a RARA rearrangement are more reasonably classified as a subclass of AML other than APL, and individualized treatment should be considered according to the genetic abnormalities.

OBJECTIVE: In the present study, we aimed to characterize the pathological development of menopausal osteoporosis, as well as to explore potential biomarkers and metabolic pathways involved in osteoporosis. METHODS: Urine samples from 322 female participants categorized by menopause status and different bone conditions were collected and analyzed based on a gas chromatography-mass spectrometry (GC-MS) approach. Multivariate and univariate statistical analyses were carried out for urinary metabolomic profile characterization and comparison. RESULTS: Seventeen metabolites in the low bone mineral density (BMD) groups were clearly differentiated from those in normal BMD groups. Among these 17 differentiating metabolites, taurine, ß-alanine, and 5-hydroxycaproic acid were found to be potential biomarkers of osteoporosis. The taurine metabolic pathway and the ß-alanine metabolic pathway were found to be related to menopause and bone loss. CONCLUSIONS: Based on the GC-MS metabolomic platform, four typical pathological phases during the progression of postmenopausal osteoporosis were described. Several differentiating metabolites and metabolic pathways were found to be closely related to the pathology of postmenopausal osteoporosis. Our results provided a solid foundation for further studies on early diagnosis and pathomechanistic evaluation.

Osteosarcoma is the most common primary bone tumor with highly invasive characteristic and low long-term survival. Recently, epithelial-mesenchymal transition (EMT) is reported as a key event in cancer invasion and metastasis. Oridonin, a bioactive diterpenoid, has been proved to possess anti-cancer effects. However, the effect of oridonin on EMT and metastasis of osteosarcoma is unclear. In this study, we investigated the underlying mechanism of oridonin on EMT and metastasis of osteosarcoma. We found that oridonin inhibited migration and invasion of MG-63 and 143B cells. Moreover, oridonin increased the protein expression of E-cadherin and decreased that of N-cadherin and Vimentin. Oridonin upregulated the transcription of E-cadherin and downregulated N-cadherin and Vimentin. Oridonin inhibited the protein and mRNA levels of Snail and Slug. Furthermore, oridonin inhibited TGF-ß-induced phosphorylation of Smad 2/3, prevented Smad dimer translocation into the nucleus. Finally, we established metastatic models of osteosarcoma 143B cells, and found that oridonin inhibited lung metastasis in vivo. Oridonin increased the protein expression of E-cadherin and reduced N-cadherin and Vimentin. Oridonin inhibited the protein expression of Snail and Slug as well as Smad 2/3 activation. In conclusion, our study demonstrated that oridonin inhibited EMT and TGF-ß1-induced EMT by inhibiting TGF-ß1/Smad2/3 signaling pathway in osteosarcoma.
