This study investigated the structural characteristic and in vitro digestion and fermentation behaviours of polysaccharide from litchi pulp (LPII). The results showed that LPII was 161.24kDa, and consisted of arabinose, galactose, rhamnose and glucose. The glycosidic linkages were identified as α-L-Araf-(1→, →5)-α-Araf-(1→ and →3,6)-ß-D-Galp-(1→. After 4h gastric digestion, the molecular weight (Mw) of LPII was declined and the reducing sugars content (CR) was increased significantly. But, the intestinal digestion had no effect on the structure of LPII. In addition, after 24h gut microbiota fermentation, the neutral sugars content and pH value decreased while total bacteria amount, CR, the production of short-chain fatty acids, acetic acid and n-butyric acid increased. These results showed that LPII could be digested in gastric digestion and fermented in large intestine fermentation broth, but not changed in small intestinal fluid. The digestion and fermentation behaviours of LPII may be related with its structural features.

RATIONALE: Adenocarcinoma coexists with adjacent microscopic gastrointestinal stromal tumor (micro-GIST) is rare, especially in the rectum, where the gastrointestinal stromal tumors (GISTs) have the lower incidence rate. It is easy to ignore the concurrent micro-GIST due to the untypical symptoms. PATIENT CONCERNS: A 77-year-old male patient suffered from lower abdominal pain for 20 days and presented with per rectal bleeding for 10 days. He had the medical history of hypertension and diabetes for more than 25 years. DIAGNOSES: Endoscopy revealed that the patient had rectum adenocarcinoma and multiple rectum polyps. Besides, the gastrointestinal stromal tumor was diagnosed by the pathologist. INTERVENTIONS: The patient underwent surgery of laparoscopic rectum resection and prophylactic ileostomy and took 6 courses of Capecitabine tablets orally. OUTCOMES: One year after surgery, the patient had no local relapse by the CT scan. However, not long after the CT examination, he died of cardiovascular disease. LESSONS: Although micro-GIST may be noninvasive and asymptomatic, it may have the potential for transforming to malignancies. More attention should be paid to the patients diagnosed with gastrointestinal malignancy coexisting with micro-GIST.

BACKGROUND: Gastric neuroendocrine carcinomas (G-NECs) are rare. This study aimed to explore the feasibility and clinical efficacy of laparoscopic surgery in patients with advanced G-NECs. METHODS: The clinicopathological data of 175 G-NECs patients who underwent radical gastrectomy in a high-volume centre were collected. One hundred fifty-one cases with advanced G-NECs (laparoscopic gastrectomy [LG] = 30, open gastrectomy [OG] = 121) were finally selected for comparison of the short-term outcomes and oncologic efficacy. RESULTS: In the postoperative recovery, when comparing the OG group, the time to ambulation (3.2 d vs. 2.6 d, respectively, p = 0.049), the time to first flatus (4.1 d vs. 3.6 d, respectively, p = 0.050), the time to first soft diet (7.9 d vs. 6.7 d, respectively, p = 0.007), and the postoperative hospital stay (13.1 d vs. 11.4 d, respectively, p = 0.047) of the LG group were shorter. There was no significant difference in the postoperative complication rates between the OG and LG groups (19.8% vs. 23.3%, p = 0.671). The 3-year overall survival (OS) rate was 57.0% in the OG group and 64.4% in the LG group (p = 0.349). The 3-year disease-free survival (DFS) rate was 51.7% in the OG group and 57.4% in the LG group (p = 0.357). There was no significant difference in the 3-year OS and DFS rates between the LG and OG groups at each stage. The recurrence rate was 35.5% in the OG group and 33.0% in the LG group (p = 0.821). CONCLUSIONS: The short-term outcomes and oncologic efficacy of laparoscopic gastrectomy and open gastrectomy for advanced G-NECs are comparable.

This study investigated the contents of saponins and phenolic compounds in relation to their antioxidant activity and α-glucosidase inhibition activity of 7 colored quinoa varieties. The total saponin content was significantly different among 7 varieties and ranged from 7.51 to 12.12 mg OAE/g DW. Darker quinoa had a higher content of phenolic compounds, as well as higher flavonoids and antioxidant activity than that of light varieties. Nine individual phenolic compounds were detected in free and bound form, with gallic acid and ferulic acid representing the major compounds. The free and bound phenolic compounds (gallic acid and ferulic acid in particular) exhibited high linear correlation with their corresponding antioxidant values. In addition, the free phenolic extracts from colored quinoa exhibited higher inhibitory activity against α-glucosidase than the bound phenolic extracts. These findings imply that colored quinoa with abundant bioactive phytochemicals could be an important natural source for preparing functional food.

This study aimed to evaluate the regulation of lipid metabolism and mechanism of a sulfated polysaccharide from Gracilaria Lemaneiformis (GLP). The results showed that GLP significantly decreased the hepatic total cholesterol (TC) and triglyceride (TG) levels in the serum of high-fat diet mice and prevented body weight gain and alleviated the accumulation of fat. Furthermore, GLP may accelerate the conversion of cholesterol to bile acids by promoting the expression of LxRα and CYP7A1 genes and regulating the relative abundance of Bacteroides, Ruminococcus_1 and Lactobacillus. Pearson's correlation analysis showed that the relative abundance of Alistipes, Prevotellaceae_UCG-001, and Corprococcus_1 was negatively associated with the hydrophobic BAs CDCA and DCA levels, while the relative abundance of Lachnospiraceae_NK4A136_group and Roseburia was positively correlated with the hydrophilic BAs UDCA and TUDCA levels. This study suggests that GLP can be used as a functional food for regulating cholesterol metabolism.

RATIONALE: Situs invsersus totalis (SIT) is a rare anomaly featured by complete inversion of abdominal and thoracic organs. Adrenal metastasis is often encountered as part of advanced systemic dissemination, which is usually unresectable. Few published cases reported the adrenal metastasis from gastric cancer with SIT and the treatment of gastrectomy combined with adrenalectomy, especially with intraoperative radiotherapy (IORT). PATIENT CONCERNS: A 61-year-old SIT man found a mass on the right clavicle and the biopsy revealed a metastatic cancer. Around 14 years ago, he had a rectal cancer resection surgery and no sign of occurrence. Five months later, the patient had a pain in his right low abdomen and abdominal CT found a right adrenal mass. DIAGNOSES: Gastroscopy and the pathology revealed the gastric antrum cancer invading the duodenal bulb. Abdominal enhanced CT suspected the adrenal mass as a hematoma, but positron emission tomography computed tomography suspected it as the metastases of gastric cancer which is consistent with the pathology results. Finally, the SIT patient was diagnosed with primary gastric cancer invading duodenal bulb with solitary right adrenal metastasis. INTERVENTIONS: The patient was treated with curative distal gastrectomy and Billroth-II anastomosis with D2 lymphadenectomy. A total 18 Gy intraoperative radiotherapy (IORT) using low energy x-rays by Intrabeam were given after resection. OUTCOMES: The patient had liver metastasis in the seventh month after surgery but there is no sign of local recurrence until now. LESSONS: Gastric cancer with adrenal metastasis, especially with SIT is rare and intractable. The result suggested that active surgical treatment for resectable gastric cancer and solitary adrenal metastatic tumor, especially in combination with IORT may be an option in controlling local relapse and prolonging survival in selected patients.

Lactic acid bacteria fermentation is an important processing technology for fruits and vegetables. Bioactive compounds such as polysaccharides are altered during the fermentation process. Polysaccharides from longan pulp (LPs) were extracted after different fermentation times and their physicochemical and prebiotic properties were investigated, such as longan polysaccharides named LP-0 and LP-12 means they were extracted from longan pulp fermented for 0 and 12 h, respectively. The yield, contents of neutral sugar and uronic acid, molecular weight (Mw), and monosaccharide composition of LPs were significantly changed with different fermentation times. Specially, the yield and uronic acid content of LPs were first increase and then decline. LP-12 contained the smallest Mw (108.71 ±â€¯5.55 kDa) of the tested LPs (p < 0.05). When compared with unfermented LP-0, the glucose molar percentages of fermented LPs declined, while those of rhamnose and galactose increased, except for LP-6. Fermented LPs also exhibited a stronger stimulatory effect on Lactobacillus strain proliferation, with the proliferative effect of LP-12 being the strongest (p < 0.05). These results suggest that lactic acid bacteria fermentation can change the physicochemical properties and enhance the prebiotic activities of polysaccharides from longan pulp.

Longan polysaccharides are valuable compounds with many biological activities. Lactobacillus fermentum was selected to ferment longan pulp and the polysaccharides from unfermented and fermented longan pulp (denoted as LP and LP-F, respectively) were isolated. Their physicochemical, immunomodulatory and prebiotic activities were investigated. The results revealed that LP-F, the molecular weight of which was lower than that of LP, contained less neutral sugar, uronic acid and glucose but more arabinose, galactose, rhamnose and mannose. Compared with LP, LP-F had better solubility, lower apparent viscosity and particle size. LP-F exhibited stronger stimulation on macrophages secretion of NO and IL-6, as well as better proliferation of Leuconostoc mesenteroides and Lactobacillus casei. In summary, fermentation treatment could change the physicochemical properties and enhance the bioactivity of polysaccharides from longan pulp.

Longan pulp polysaccharides were extracted with hot water (LP-H), superfine grinding (LP-S) and superfine grinding-assisted enzymatic treatments (LP-SE). The yields, physicochemical properties and prebiotic activities of polysaccharides were investigated. Compared with LP-H and LP-S, the yield, sugar content, solubility, arabinose and mannose percentage of LP-SE increased while its apparent viscosity, particle size, Mw and glucose percentage declined. Three LPs contained similar glycosidic linkage of →3)-α-l-Araf-(1→, →3,6)-ß-d-Galp-(1→ and α-l-Rhap(l→, while they each contained specific glycosidic linkage of →4)-ß-d-Glcp(l→, →4)-ß-d-Galp-(1→ and →5)-α-l-Araf-(1→ in LP-H, LP-S and LP-SE, respectively. Moreover, LP-SE exhibited stronger stimulation than LP-H and LP-S on the proliferation of Lactobacillus plantarum, Lactobacillus bulgaricus, Lactobacillus fermentum and Leuconostoc mesenteroides. The results indicated three extraction methods had some effect on chemical composition and structure of polysaccharide. LP-SE extracted by superfine grinding-assisted enzymatic treatment exhibited the highest prebiotic activities, which have a great potential in applying in functional food and medical industry.

Idiopathic pulmonary fibrosis (IPF), characterized by irreversible scarring and progressive destruction of the lung tissue, is one of the most common types of idiopathic interstitial pneumonia worldwide. However, there are no reliable candidates for curative therapies. Hence, elucidation of the mechanisms of IPF genesis and exploration of potential biomarkers and prognostic indicators are essential for accurate diagnosis and treatment of IPF. Recently, efficient microarray and bioinformatics analyses have promoted an understanding of the molecular mechanisms of disease occurrence and development, which is necessary to explore genetic alternations and identify potential diagnostic biomarkers. However, high false-positive rates results have been observed based on single microarray datasets. In the current study, we performed a comprehensive analysis of the differential expression, biological functions, and interactions of IPF-related genes. Three publicly available microarray datasets including 54 IPF samples and 34 normal samples were integrated by performing gene set enrichment analysis and analyzing differentially expressed genes (DEGs). Our results identified 350 DEGs genetically associated with IPF. Gene ontology analyses revealed that the changes in the modules were mostly enriched in the positive regulation of smooth muscle cell proliferation, positive regulation of inflammatory responses, and the extracellular space. Kyoto encyclopedia of genes and genomes enrichment analysis of DEGs revealed that IPF involves the TNF signaling pathway, NOD-like receptor signaling pathway, and PPAR signaling pathway. To identify key genes related to IPF in the protein-protein interaction network, 20 hub genes were screened out with highest scores. Our results provided a framework for developing new pathological molecular networks related to specific diseases in silico.

In this study, dried longan pulp (DLP) was subjected to fermentation using selected strains of lactic acid bacteria (Lactobacillus plantarum subsp. Plantarum and Leuconostoc mesenteroides). We then studied changes in the free and bound phytochemical profile, antioxidant activity, free amino acid, and organic acid composition. Fermentation exhibited a 17.4% and 5.7% increase in the amount of free and total phenolic contents of DLP. Phenolic composition determined by HPLC revealed significant changes due to fermentation that were primarily in the contents of gallic acid, vanillic acid, 4-methylcatechol and p-coumaric acid, resulting in a 37.9% and 25.7% increase in free gallic acid and 4-methylcatechol, respectively. Fermentation was also found to enhance the ferric reducing antioxidant power of both free and total and the oxygen radical absorbance capacity of free phenolic fraction by 18.3%, 11.8%, and 37.4%, respectively. In addition, fermentation was observed to reduce the contents of free amino acids with bitter taste (phenylalanine, tyrosine and leucine), and increase amino acids (taurine, aspartic acid, cysteine, cysteine thiazoline and γ-amino-butyric acid) having antioxidant potential. Therefore, this study provides basis for the production of fermented longan-based functional products with improved antioxidant activity.

Changes of phenolic profiles and antioxidant activity of litchi pericarp during storage at 4 °C for seven days and at room temperature (RT) for 72 h were evaluated in this study. The contents of total phenolic and procyanidin decreased by 20.2% and 24.2% at 4 °C and by 37.8% and 47.8% at RT, respectively. Interestingly, the corresponding reductions of anthocyanins were 41.3% and 73%, respectively. Four phenolic compounds, including epicatechin, procyanidin A2, procyanidin B2, and quercetin-3-O-rutinoside-7-O-α-l-rhamnosidase were detected in litchi pericarp. Their contents after storage at 4 °C and at RT were decreased by 22.1⁻49.7% and 27.6⁻48.7%, respectively. The oxygen radical absorbance capacity (ORAC) and cellular antioxidant activity (CAA) of litchi pericarp decreased by 17.6% and 58.7% at 4 °C, and by 23.4% and 66.0% at RT, respectively. The results indicated that storage at 4 °C preserved more phenolics and retained higher antioxidant activity in litchi pericarp compared to storage at RT, suggesting that storage at 4 °C should be considered as a more effective method for slowing down the degradation of litchi pericarp phenolics.

RATIONALE: Hepatoid gastric adenocarcinoma is a rare type of gastric cancer. The phenomenon of neuroendocrine differentiation (NED) in gastrointestinal tumor needs further research. Both hepatoid adenocarcinoma and neuroendocrine differentiation are the factors leading to a poor prognosis of gastric cancer. However, there is still no specific treatment. PATIENT CONCERNS: A 60-year-old man who had a pain and distention in his upper abdomen presented melena. Gastroscopy and pathology revealed a gastric cancer. DIAGNOSES: Postoperative pathology revealed a hepatoid gastric adenocarcinoma. Immunohistochemical analysis showed a-fetoprotein (AFP), hepatocyte, synaptophysin (Syn), and chromogranin A (CgA) positive, and Ki67 60% positive. A-fetoprotein producing hepatoid gastric adenocarcinoma with NED is diagnosed. INTERVENTIONS: The patient was treated with an R2 radical gastrectomy, but refused chemotherapy. OUTCOMES: The AFP level was >2000 ng/mL (0-8.78) half a year after the surgery. There was no obvious abnormality from computed tomography (CT). The patient refused positron emission tomography computed tomography (PET-CT) and left the hospital. LESSONS: Hepatoid adenocarcinoma and neuroendocrine differentiation are the factors leading to a poor prognosis of gastric cancer. It relapses easily. Long-term follow-up and regular examinations are necessary to detect relapses.

Excessive use of synthetic nitrogen (N) fertilizer and lower nitrogen use efficiency (NUE) are threatening the wheat production in the middle and lower reaches of Yangtze River. Excess input of N fertilizers also results in severe environmental pollution, climate change and biodiversity loss. However, the study on reasonable nitrogen application and NUE improvement with the prerequisite of stable and high yield remains unexplored. In our study, the four different levels of nitrogen were applied to find out the nitrogen threshold which could be both friendly to environment and promise the stable and high yield. The experiment was carried out in Yangzhou University (Yangzhou, China). The wheat cultivar Yangmai 23 was selected as the research material. The four nitrogen levels were as follows: 0, 189, 229.5, and 270 kg ha-1. The results showed that the grain yield under the application of 229.5 kg ha-1 N was as high as that under 270 kg ha-1 N level, with the observation of 20.3% increase in agronomic efficiency. The N2O emission of 229.5 kg ha-1 N application was as low as that of 189 kg ha-1 N, but the grain yield and agronomic efficiency were significantly higher (11.9%) under 229.5 kg ha-1 treatment than the lower one. Taken together, this indicated the nitrogen level at 229.5 kg ha-1 could be identified as the fertilizer threshold, which will be beneficial for the future fieldwork.

Rice husks (RHs) as an agro-waste generated from rice production, while its application is limited. This study was designed to introduce a full utilization of rice husks, which extracted the phytochemical at first and then produced cellulose nanocrystals (CNCs) as the use of the residue. Furthermore, the phytochemicals extracted from rice husk was identified and its biological activity, including antioxidant activity, cellular antioxidant activity (CAA) and antiproliferative activity, had been detected as well. Results showed the bound fraction of rice husk had higher antioxidant than common fruit and grain. Free fraction of rice husk deserved to have further analysis in antiproliferative activity due to its low cytotoxicity. The CNCs produced by residue was using delignification process and acid hydrolysis treatments. The chemical composition of the residue obtained after phytochemical extraction was determined. CNCs morphological investigation was performed using an optical microscope and atomic force microscopy (AFM). Our strategy is to achieve a comprehensive utilization of rice husks with both economy and environment benefits.

BACKGROUND: In order to overcome the shortcomings of laparoscopic intersphincteric resection (Lap ISR), an alternative method of delivering intraoperative radiotherapy by Intrabeam X-rays radiotherapy system (XRS) is proposed in this paper. Intrabeam XRS is a device that uses low-energy X-rays source generated by a mobile controller unit, which is featured in accurate irradiation, reduced complications, and less exposure. The purpose of this study is to discuss the feasibility of Lap ISR with intra-operative radiotherapy using low-energy X-rays for locally advanced ultra-low rectal cancer in Asian woman. This novel proposed method will greatly increase the anus preserving probability and improved the quality of life. METHODS: A 53-year-old woman diagnosed with rectal adenocarcinoma had a strong desire to preserve the anal function and presented at the Jilin University Second Hospital, Jilin, China. The tumor's size was 4 cm × 3 cm. It was located 2 cm from the anus merge and invaded the levator ani muscle. Preoperative clinical staging was T4N1M0 and could be reached R0 resection. After the consent form was signed by the patient, Lap ISR combined with the applicator put through the anus (natural orifice) to the tumor bed was performed and prophylactic ileostomy synchronized the anastomosis. Patient only received 1-cycle chemotherapy regimen of oxaliplatin with capecitabine postoperatively due to personal reasons. Pre- or postoperative radiotherapy was not given. RESULTS: After clinical follow-up, until now, there is not any sign of local recurrence. Anus function and short-term complications are acceptable. The short-term effect is satisfying and we look forward to further assess the long-term effect. CONCLUSION: Laparoscopic intersphincteric resection with IORT using low-energy X-rays for the patients with late-stage ultra-low rectal cancer could provide an opportunity of preserving the anus function, and it is feasible for the selected patients. Local recurrence remains a major cause of therapeutic failure in patients with nasopharyngeal carcinoma (NPC) and the effective treatment of recurrent NPC (r-NPC) is still a challenge. Intensity-modulated radiotherapy (IMRT) is considered as a favorable technique in the management of r-NPC, especially for extensive lesions. However, local r-NPC is a highly heterogeneous disease and the survival outcome following salvage IMRT varies. Furthermore, due to varied samples and therapeutic protocols, no consensus has been reached in the establishment of prognostic values. Hence, we used Medline and Embase electronic databases to conducted a meta-analysis to generate the best estimation of the prognostic factors in local r-NPC following salvage IMRT. Finally, a total of 783 patients in seven studies were enrolled. Overall, the pooled HR for OS of recurrent T stage and recurrent tumor volume was 1.77 (95% CI = 1.15-2.39) and 2.12 (95% CI = 1.42-2.82), without any heterogeneity. In addition, despite a significant association was observed in the pooled HR of significant compliance for OS, however, significant heterogeneity was also observed (I2 = 76.6%, p = 0.039). Furthermore, no significant association was observed among the pooled HRs for OS in terms of age, gender, recurrent time interval, synchronous nodal recurrence, chemotherapy and total re-irradiation dose. Therefore, the present meta-analysis demonstrated that recurrent T stage and tumor volume may serve as the prognostic factors for OS in patients with r-NPC who received salvage IMRT. The other factors such as age, gender, and optimal re-irradiation dose warranted further investigation.

The present study was designed to determine the role of PKD in vascular remodeling (VR) in Spontaneously hypertensive rats (SHRs). Increased SBP, VR and PKD activation were prominent in SHRs. The SBP has a positive correlation with the activation of PKD in SHRs. The ratio of media to lumen (MT/LD), volume fraction of collagen (VFC), hydroxyproline, IL-6, TNF-α and nitrotyrosine content were significantly related to the activated PKD. It may be concluded that PKD plays a central role in VR, and the mechanism may be related to its regulation of hypertrophy, fibrosis, inflammation and oxidative stress.

Background: The aberrant expression of surface receptors on immunocytes may represent potential markers of tumor escape for nasopharyngeal carcinoma (NPC). The aim of this study was to investigate the expression of representative receptors on natural killer (NK) cells and NK group 2, member D (NKG2D) on immunocytes in the peripheral blood of patients with NPC. Methods: Patients (n = 64) with NPC prior to initiation of treatment were defined as the study group. Healthy volunteers (n = 31) served as the control group. The expression of NK cells and NKT cells; the triggering receptors NKp30, NKp44, and NKp46 on NK cells; the activating receptor NKG2D on NK cells, CD4+ T cells, and CD8+ T cells; and the inhibitory receptors CD158b and CD159a on NK cells were analyzed by flow cytometry in the two groups. Results: Here, our study showed that no differences were observed in terms of the numbers of NK cells or NKT cells, or the expression of CD158b and CD159a on the surface of NK cells between the two groups. Nevertheless, the expression levels of NKp30 and NKp46 on NK cells in the NPC patients were significantly lower than in the healthy individuals (P < 0.05). No differences existed in the expression of NKG2D on NK cells, but NKG2D on CD8+ T cells showed a markedly lower expression in the study group (P < 0.001). Conclusions: Our findings may reflect a possible mechanism of immune evasion for NPC. The enhancement of immunotherapy concerning NKp30, NKp46, and NKG2D may be an innovative treatment strategy for patients with NPC.
