Purpose: To describe trends and correlation between antibacterial exposure and bacterial resistance from hospitalized patients in a hospital in southern China. Patients and methods: This study used hospital-wide data regarding antimicrobial resistance and consumption between January 1, 2014 and December 31, 2018. Antibacterial consumption was expressed as antimicrobial use density (AUD). The changes in trends and associations between antibacterial utilization and resistance were analyzed using linear regression and time series analysis. Results: The total AUD of all antimicrobials decreased year by year (50.66 in 2014 vs 44.28 in 2018, P=0.03). The annual use of antimicrobials, such as penicillins, monobactams, aminoglycosides, macrolides, and lincosamides, significantly decreased (P<0.05), while the annual use of quinolones and tetracyclines significantly increased (P<0.05). Among the top ten isolated bacteria, antimicrobial resistance trends of Escherichia coli, Pseudomonas aeruginosa, Candida albicans, Staphylococcus aureus, and Staphylococcus epidermidis significantly decreased (P<0.05). Significant positive correlation was found between AUD of carbapenems and resistance rate of Acinetobacter baumannii to imipenem (ß=32.87, P<0.01), as well as the correlation between AUD of quinolones and resistance rate of Enterococcus faecium to levofloxacin (ß=104.40, P<0.01). Conclusion: The consumption of antibiotics and antibiotic resistance has been significantly improved in this tertiary hospital. Additionally, the efforts of China's antibiotic management may be suggested by the relationship between indicated antibiotic resistance and consumption. However, overall AUD levels and poor control of the use of antibiotics, such as quinolones and tetracyclines, still require strengthened management.

RESUMO

This article reviews the current status of the overuse of intravenous (IV) infusions in China and implications to patient safety, and analyzes factors associated with the overuse. Although many factors contribute to the overuse of IV infusions in China, we focus on the construction of an IV infusion management system and tackling cultural problems as the first step to address issues of IV therapy in this commentary.

This study categorizes the types of inappropriate intravenous prescriptions in hospitalized patients in China. Prescription data from 2016 were retrospectively analyzed based on predefined categories of inappropriateness. Of the 123,521 patients included, 89.2% received intravenous medications, contributing to 80% of the patients' hospital medication costs. Of significant concern, antibiotics and traditional Chinese medicines were administered to 44.3% and 14.5% of hospitalized patients, respectively. Overall, 11.4% of all intravenous prescriptions were classified as inappropriate, with improper diluent and diluent volumes being the primary cause. A team-based collaborative approach is necessary to address this widespread issue in China.

The present study assessed the expression of the DNA doublestrand repair (DDR) proteins ATM serine/threonine kinase (ATM), checkpoint kinase 2 (CHEK2) and γH2A histone family member X (γH2AFX) in oral leukoplakia (OL) and evaluated their clinical significance and usefulness as biomarkers for predicting OL transformation. Retrospectively, ATM, CHEK2 and γH2AFX protein levels were evaluated using immunohistochemical analysis in 61 OL, 33 oral squamous cell carcinoma (OSCC) and 15 normal oral mucosa tissues. OL tissues were classified into two groups according to the epithelial dysplasia pathology: The low risk dysplasia group (n=41) and the high-risk dysplasia group (n=20). The results of the present study revealed that the expression of ATM and γH2AFX in OSCC was significantly increased compared with that in OL with low-risk dysplasia and normal oral mucosa tissues. There was no statistically significant difference in CHEK2 expression among the groups. ATM expression was correlated with that of γH2AFX in OSCC tissue. The prognostic values of the DDR proteins and their correlation with clinical and pathological parameters were evaluated further in 99 OL patients with low risk dysplasia. Multivariate analysis revealed that increased expression of ATM and γH2AFX was significantly associated with an increased risk of malignant transformation. Immunohistochemical analysis of ATM and γH2AFX protein expression provided useful prognostic information on the carcinogenesis of OL. Increased ATM and γH2AFX expression may indicate a poor prognosis.

Periodontitis is a chronic oral inflammatory disease caused by microorganisms. Human ß­defensin­3 (HBD­3) is an endogenous antimicrobial peptide that inhibits a broad spectrum of microorganisms. Cell sheet technology has been widely applied in tissue and organ reconstructions. In the current study, it was aimed to investigate the anti­inflammatory effect of periodontal tissue engineered by HBD­3 gene­modified periodontal ligament cell (PDLC) sheets, and to identify a suitable method of promoting the regeneration of periodontal tissues. Western blot analysis and antimicrobial tests were used to confirm the expression of HBD­3. The effect of the cell sheets on anti­inflammatory activity and bone remodeling in a dog model of periodontitis was demonstrated by immunohistochemistry. The results demonstrated that the transfected PDLCs stably expressed HBD­3. Periodontal pathogens were susceptible to the antimicrobial activity of the cell sheets. In addition, the cell sheets relieved the bone resorption caused by inflammation in the in vivo model. HBD­3 may potentially be applied in the treatment of periodontitis and may function as osteogenic promoter via its anti­inflammatory effect.

Periodontitis is an oral chronic inflammatory disease induced by microorganisms that can destroy tooth­supporting structures. Human ß­defensin­3 (HBD­3) is a type of endogenous antimicrobial peptide that inhibits a broad spectrum of microorganisms. The objectives of the present study were to transfect human periodontal ligament cells (HPDLCs) and human bone marrow stromal cells (HBMSCs) with lentivirus containing the HBD­3 gene, determine the transfection efficiency, and investigate the antimicrobial activity of the experimental cells against periodontal pathogens. Fluorescence microscopy was used to calculated the transfection efficiency. Western blot analysis and ELISA were conducted to confirm the expression of HBD­3 at the protein level. The effect of the HBD­3 gene on the antimicrobial activity of the cells were demonstrated by antimicrobial tests. The results of the present study demonstrated that the transfected HPDLCs and HBMSCs stably expressed HBD­3. In addition, periodontal pathogens and caries­causing bacteria were susceptible to the antimicrobial activity of the cells. Both HPDLCs and HBMSCs hold potential for use as seeding cells in cell­ and gene­based therapies for periodontal disease. The lentiviral vector containing HBD­3 resulted in broad­spectrum antimicrobial activity against a variety of oral organisms, and could potentially be applied in the treatment of oral infectious diseases, including periodontitis.

PURPOSE: To investigate the interferon-alpha level in peripheral blood of patients with primary Sjogren's syndrome (pSS), and explore preliminarily the correlation between pSS and interferon-alpha. METHODS: ELISA and real-time PCR were used to evaluate the interferon-alpha level in peripheral blood in 37 patients with pSS and 24 patients with non-pSS. Statistical analysis was performed by Student's t test or Fisher's exact test using SAS6.12 software package. RESULTS: The results demonstrated that 43.24% (16/37) patients with pSS had detectable IFN-alpha concentration in plasma (>or=12.5 pg/mL), the other 21 patients (56.76%) with pSS had undetectable IFN-alpha concentration(<12.5 pg/mL). All 24 controls had undetectable IFN-alpha expression.There was significant difference between the two groups(P=7.02 x 10(-5)). Furthermore, the mRNA expression levels of IFN-alpha gene in peripheral blood cells were over-expressed in patients with pSS(P=0.0124). CONCLUSIONS: The study reveals that interferon-alpha level in peripheral blood with pSS patients is overexpressed.

PURPOSE: To retrospectively analyze the advantages and disadvantages of imaging methods commonly used for diagnosis of gland submandibular diseases. METHODS: One hundred and eighty-two patients with submandibular gland surgery were retrospectively studied to compare the accuracy rate between preoperative imaging and postoperative diagnosis.The data were analyzed using SAS8.0 software package.Fisher's exact test was used to compare the difference between B type ultrasonography, CT and MRI. RESULTS: Compared with postoperative pathological results,diagnostic accuracy rate of B type ultrasonography, CT and X-ray was 85.34%, 79.66% and 92.31%, respectively. MRI had a higher accuracy rate than B type ultrasonography or CT.(P<0.05). CONCLUSION: B type ultrasonography, CT and X-ray examination have misdiagnosis especially in submandibular sialadenitis,which is one of the most important causes of misdiagnosis leading to excision of the glands.MRI has higher application value in diagnosis of submandibular gland diseases.

RESUMO

PURPOSE: To evaluate the role of MR Sialography(MRS) in diagnosis of submandibular sialolithiasis. METHODS: Twenty-one patients with submandibular sialolithiasis were scanned by MRS, the results of MRS were compared with those after proper treatment. RESULTS: The results of MRS were consistent with those after proper treatment. CONCLUSION: MRS has high diagnostic accuracy in diagnosis of sialolithiasis including submandibular sialadenitis.
