[Esophageal-pulmonary fistula in a patient with squamous cell carcinoma of the esophagus]. / Fistule oesophago-pulmonaire au cours d'un carcinome épidermoïde oesophagien.

Acquired eso-respiratory fistulae are usually consecutive to an eso-tracheal or an eso-bronchial fistula. Esophago-pulmonary fistulae have been rarely described. We report a case of esophago-pulmonary fistula in a patient with esophageal carcinoma. Our patient presented progressive necrotizing pneumonia. CT scan of the thorax demonstrated necrosis of the esophagal wall and communication between the esophagus and the lung parenchyma. Furthermore, the biochemical analysis of the lung abcess fluid revealed a high level of amylase. Outcome was poor despite drainage of the lung abcess and insertion of an esophageal stent. Based on this case, we reviewed the cases of esophago-pulmonary fistulae described in the literature.

Poor localization of the destruction focus seems to result from low procoagulant activity of pulmonary macrophages. If the activity of purulent and destructive process stands high the ability of phagocytes to synthesize and release proteinases is realized fully. epsilon-Aminocaproic acid prohibits phagocytic proteolytic activity from growing under the influence of stimulators. Correction of defective leukocytic proteolysis and mononuclear activity can be done with prodigiozan, streptokinase and epsilon-aminocaproic acid.

The dependence of the activity of lysosomal enzymes upon the degree of the pyo-destructive process and the character of the course of the postoperative period has been established on the basis of studying the activity of lysosomal enzymes of leukocytes of blood and pleural exudate, titre of natural and heterophyllic antibodies, components of the kallikrein-kinin system in pyo-destructive diseases of the lungs and pleura. Pathogenetically substantiated ways of prevention of postoperative infectious complications are proposed.
