PURPOSE: The aim of the study is to evaluate the retinal and choroidal microvascular changes via optical coherence tomography angiography in patients who received hydroxy-chloroquine. METHODS: In total, 28 eyes of 28 patients (24 females, and 4 males) receiving treatment with hydroxy-chloroquine were assessed in this cross-sectional cohort study (hydroxychloroquine group). The high-and low-risk groups consisted of patients receiving hydroxychloroquine for ≥5 years (14 eyes of 28 patients) and <5 years (14 eyes of 28 patients), respectively. A total of 28 age- and gender-matched volunteers were enrolled as the control group. The macular flow area (superficial, deep, and choriocapillaris), superficial and deep vessel density, foveal avascular zone area, central foveal thickness, and subfoveal choroidal thickness parameters were measured by optical coherence tomography angiography. RESULTS: The mean age of the 28 patients who received hydroxychloroquine and the 28 age-matched controls was 45.5 ± 11.1 years (range: 29-70 years) and 44.5 ± 13.9 years (range: 28-70 years), respectively. In patients who received hydroxychloroquine, the values for the superficial, deep, and choriocapillaris macular flow areas were 13.578 ± 0.30, 13.196 ± 0.31, and 17.617 ± 0.42, respectively. In controls, these values were 16.407 ± 0.95, 13.857 ± 0.31, and 18.975 ± 0.76, respectively (p<0.05 for all). The superficial, deep, and cho-riocapillaris flow areas were significantly smaller in patients who received hydroxychloroquine than those in controls (p<0.05 for all). Superficial and deep vessel densities were significantly reduced in patients who received hydroxychlo-roquine in all regions (i.e., foveal, parafoveal, temporal, superior, nasal, and inferior) (p<0.05 for all). Moreover, significant difference was observed between the groups in the foveal avascular zone area (superficial and deep), central foveal thickness, and subfoveal choroidal thickness (p<0.05 for all). CONCLUSIONS: Retinochoroidal microvascular flow and vessel density of the macular area were significantly decreased in patients who received hydroxychloroquine. Hy-droxychloroquine may damage the retinochoroidal mi-cro-vascular architecture. Optical coherence tomography angiography may contribute to the early detection of hy-dro-xychloroquine-induced retinal toxicity.

Optical coherence tomography angiography (OCTA) is a novel, non-invasive imaging technology, which could acquire volumetric angiographic information. Numerous studies have reported the potential clinical use of OCTA in a variety of common retinal disorders. Polypoidal choroidal vasculopathy (PCV) is characterized by the formation of branching choroidal vascular networks (BVN) with terminal dilatations (polyps). Indocyanine green angiography (ICGA) remains the golden diagnostic standard for PCV. The clinical application of OCTA in PCV is also widely investigated recent years. But the results are controversially interpreted. In addition to various diagnostic accuracy of PCV from different studies, the clinical application of OCTA in PCV is limited. With the constant innovation of fundus imaging techniques, OCTA is reaching greater investigation depth and become more accurate at picking up blood flow signals, which also improves the diagnostic accuracy of PCV. In this paper, we reviewed the clinical application and research progress of OCTA in PCV, in order to provide some assistant for clinical practice and correct interpretation of the reports. In recent years, much attention is being paid to studying the state of the choroid in various eye pathologies, but changes in the choroid of pregnant women with disorders of glucose metabolism remain unexplored. PURPOSE: To study choroidal thickness in pregnant women with disorders of carbohydrate metabolism in the III trimester. MATERIAL AND METHODS: The study included 376 pregnant women in the third trimester of pregnancy: 49 of them had type 1 diabetes (T1D), 248 had gestational diabetes (GD), and 79 were healthy pregnant women who comprised the control group. Three subgroups were distinguished among patients with GD, depending on the timing of the development of GD in the I (160 patients), II (56 patients), or III (32 patients) trimesters. Optical coherence tomography was performed to measure choroidal thickness in the foveal region. RESULTS: In patients with GD, no significant differences in average indexes of choroidal thickness in subgroups with different timing of diabetes development were observed, however, the pregnant women of the GD subgroup with beginning of its development in the first trimester had the smallest indexes of average choroidal thickness. In pregnant women with GD who received insulin, choroidal thickness was lower than in patients who did not receive it, although statistical significance of that was not established. In patients with T1D and diabetic retinopathy (DR), choroidal thickness was significantly lower compared to patients with T1D without DR, GD and control groups. Obtained data indicates the presence of changes in the choroid and development of diabetic choroidopathy in pregnant women with T1D and DR. CONCLUSION: The study of choroidal changes in pregnant women with disorders of carbohydrate metabolism can help identify criteria for manifestation of diabetic microangiopathy, choroidopathy, and predict the progression of DR.

Many ocular diseases are associated with choroidal changes. Therefore, it is crucial to be able to segment the choroid to study its properties. Previous methods for choroidal segmentation have focused on single cross-sectional scans. Volumetric choroidal segmentation has yet to be widely reported. In this paper, we propose a sequential segmentation approach using a variation of U-Net with a bidirectional C-LSTM(Convolutional Long Short Term Memory) module in the bottleneck region. The model is evaluated on volumetric scans from 40 high myopia subjects, obtained using SS-OCT(Swept Source Optical Coherence Tomography). A comparison with other U-Net-based variants is also presented. The results demonstrate that volumetric segmentation of the choroid can be achieved with an accuracy of IoU(Intersection over Union) 0.92.Clinical relevance- This deep learning approach can automatically segment the choroidal volume, which can enable better evaluation and monitoring at ocular diseases.

Since the thickness and shape of the choroid layer are indicators for the diagnosis of several ophthalmic diseases, the choroid layer segmentation is an important task. There exist many challenges in segmentation of the choroid layer. In this paper, in view of the lack of context information due to the ambiguous boundaries, and the subsequent inconsistent predictions of the same category targets ascribed to the lack of context information or the large regions, a novel Skip Connection Attention (SCA) module which is integrated into the U-Shape architecture is proposed to improve the precision of choroid layer segmentation in Optical Coherence Tomography (OCT) images. The main function of the SCA module is to capture the global context in the highest level to provide the decoder with stage-by-stage guidance, to extract more context information and generate more consistent predictions for the same class targets. By integrating the SCA module into the U-Net and CE-Net, we show that the module improves the accuracy of the choroid layer segmentation.

RESUMO

Automatic detection of age-related macular degeneration (AMD) from optical coherence tomography (OCT) images is often performed using the retinal layers only and choroid is excluded from the analysis. This is because symptoms of AMD manifest in the choroid only in the later stages and clinical literature is divided over the role of the choroid in detecting earlier stages of AMD. However, more recent clinical research suggests that choroid is affected at a much earlier stage. In the proposed work, we experimentally verify the effect of including the choroid in detecting AMD from OCT images at an intermediate stage. We propose a deep learning framework for AMD detection and compare its accuracies with and without including the choroid. Results suggest that including the choroid improves the AMD detection accuracy. In addition, the proposed method achieves an accuracy of 96.78% which is comparable to the state-of-the-art works.

Precise three-dimensional segmentation of choroidal vessels helps us understand the development and progression of multiple ocular diseases, such as agerelated macular degeneration and pathological myopia. Here we propose a novel automatic choroidal vessel segmentation framework for swept source optical coherence tomography (SS-OCT) to visualize and quantify three-dimensional choroidal vessel networks. Retinal pigment epithelium (RPE) was delineated from volumetric data and enface frames along the depth were extracted under the RPE. Choroidal vessels on the first enface frame were labeled by adaptive thresholding and each subsequent frame was segmented via segment propagation from the frame above and was in turn used as the reference for the next frame. Choroid boundary was determined by structural similarity index between adjacent frames. The framework was tested on 33 mm SS-OCT volumes acquired by a prototype SS-OCT system (PlexElite 9000, Zeiss Meditec, Dublin, CA, US), and vessel metrics including perfusion density, vessel density and mean vessel diameter were computed. Results from human subjects (N = 8) and non-human primates (N = 6) were summarized.Clinical Relevance- Accurate 3D choroid vessel segmentation can help clinicians better quantify blood perfusion which can lead to improved diagnosis and management of retinal eye diseases.

We aimed to evaluate choroid structural changes using swept-source optical coherence tomography (SS-OCT) following hemodialysis initiation in diabetic and nondiabetic patients with end-stage kidney disease (ESKD). In this multicenter, prospective, cross-sectional study, diabetic (DM group; 30 eyes; 16 patients) and nondiabetic patients (NDM group; 30 eyes; 15 patients) with ESKD were evaluated after hemodialysis initiation. SS-OCT findings were analyzed using a manual delineation technique and binarization method before the first and last hemodialysis sessions, conducted approximately 2 weeks apart. Subfoveal choroidal thickness changes and mean large choroidal vessel layer thickness were significantly greater in the DM group (-13.3% ± 2.5% and -14.5% ± 5.2%, respectively) than the NDM group (-9.5% ± 3.1% and -9.2% ± 3.4%, respectively; p = 0.049 and p = 0.02, respectively). Binarized SS-OCT analysis revealed that the mean subfoveal choroidal area was significantly larger in the DM group (-21.9% ± 6.5%) than the NDM group (-17.2% ± 5.9%; p = 0.032). The change ratio in mean luminal area values was significantly greater in the DM group (-27.7% ± 8.7%) than the NDM group (-17.7% ± 5.8%; p = 0.007). The DM group exhibited substantial changes in the choroidal layer, possibly reflecting choroidal vascular disorders caused by diabetes.

Aim: This study aimed to determine the effects of strabismus surgery on choroidal thickness in paediatric patients with Spectral Domain-Optical Coherence Tomography (SD-OCT). Material and Methods: Sixty eyes of 60 children with esotropia were included. The mean age was 9.5 ± 3.1 years. The choroidal thickness was measured using SD-OCT, which was performed before the surgery and 1 week, 1 month and 3 months postoperatively. At the same time, the patients' visual acuity was evaluated. Results: There were no statistically significant differences between the preoperative and postoperative values in terms of the visual acuity (P > 0.05). When compared to the preoperative values, decreases were found in the choroidal thicknesses in all of the areas measured during the first week evaluation (P < 0.05). In the postoperative first month, significant decreases were also found in the choroidal thicknesses in all the areas measured (P < 0.05); however, in the postoperative third month, there were no changes in any of the areas (P > 0.05). Conclusions: We observed that there was a significant decrease in the choroidal thickness during the early postoperative period and no significant change in the late postoperative period in paediatric patients of double horizontal muscle surgery. Moreover, these early changes in the choroid in paediatric patients show that it does not cause a functional loss.

PURPOSE: To compare macular choroidal thickness of idiopathic epiretinal membrane (ERM) and fellow eyes, and before and after vitrectomy in terms of the morphological features on spectral-domain optical coherent tomography (SD-OCT). METHODS: Eighty-four patients with unilateral idiopathic ERM were involved. Patients were categorized into: Group 1, ERM without membrane contraction; Group 2, ERM with membrane contraction and retinal folding; and Group 3, ERM with membrane contraction and macular edema. Twenty-two patients received surgical removal of ERM. Choroidal thickness was compared between ERM and fellow eyes, and before and after treatment. RESULTS: Mean choroidal thickness was significantly greater in ERM eyes than in fellow eyes (234.4 ± 22.5 vs 220.6 ± 20.8 µm; P<0.01). Group 1 (n = 20) showed no significant difference in choroidal thickness between ERM and fellow eyes. Eyes in Group 2 (n = 27) and Group 3 (n = 37) showed statistically significant differences in mean choroidal thickness between ERM and fellow eyes (229.6 ± 23.8 vs 220.8 ± 19.6 µm; 242.6 ± 27.8 vs 221.0 ± 21.8 µm, respectively; P<0.05). In Group 2 (n = 8) and Group 3 (n = 16), choroidal thickness in ERM eyes decreased significantly at 1 month and 6 months after surgery, compared with that before surgery (P<0.05 for all comparisons). CONCLUSIONS: Membrane contraction contributed to the increase in choroidal thickness in idiopathic ERM patients. This finding may help to elucidate the pathophysiologic features of idiopathic ERM as well as the response to treatment in these patients.

INTRODUCTION: We report a case of type III uveal effusion syndrome (UES) suspected to be related to pachychoroid spectrum disease. PATIENT CONCERNS: A 42-year-old man became aware of visual field constriction and deterioration of visual acuity in his right eye. DIAGNOSIS: Upon examination, a bullous non-rhegmatogenous retinal detachment was observed in the inferior 2 quadrants of the right eye fundus, and the subretinal fluid moved with postural changes. The axial length in that eye was 22.36 mm, thus indicating no nanophthalmia. Preoperative indocyanine green angiography revealed dilated choroidal vessels in the posterior pole of the right eye and mild leakage in the late phase. Optical coherence tomography examination revealed choroidal thickening in both eyes. INTERVENTIONS: For treatment, we first performed sclerotomy, and the intraoperative findings showed no thickening of the sclera. Following surgery, reattachment of the retina was not achieved. OUTCOMES: Thus, we next performed vitrectomy, which led to successful reattachment of the retina. LESSONS: In this case, we theorize that pachychoroid spectrum disease might have been involved in the pathogenesis of type III UES.

INTRODUCTION: Swept-source optical coherence tomography is a useful non-invasive device that is used to understand better the role of choroid in the pathogenesis of diabetic retinopathy. AIM: To measure choroidal thickness in diabetic eyes and to correlate it with established systemic risk factors, the severity and the therapy of diabetic retinopathy. METHOD: Prospective cross-sectional study using swept-source optical coherence tomography has been performed. Choroidal and macular thickness maps of 117 eyes of 60 diabetic patients were compared to data from 45 eyes of 24 healthy controls. In all diabetic patients, the systemic risk factors (duration and type of diabetes, blood hemoglobin A1C level, hypertension), the type (no, non-proliferative or proliferative) and the therapy of diabetic retinopathy were recorded, and their relation to choroidal thickness was evaluated using multiple regression models. RESULTS: A significantly thinner choroid was measured in diabetic patients compared to controls (p<0.05). Analysing the whole cohort, aging (p<0.001) and the presence of hypertension (p<0.05) showed significant correlation with choroidal thinning. In diabetic patients, the duration of diabetes significantly correlated with choroidal thinning (p<0.05). In multivariable analysis, the duration of diabetes remained a significant predictor of choroidal thickness (ß -0.18; p = 0.02). A significantly thinner choroid was measured in patients with proliferative retinopathy and in patients after panretinal photocoagulation treatment compared to nonproliferative retinopathy (p<0.05). CONCLUSION: Diabetes mellitus itself and diabetic retinopathy progression affects choroidal thickness significantly. Choroidal thickness is affected significantly by systemic risk factors (age, the presence of hypertension, disease duration). We investigated whether response to photodynamic therapy (PDT) with intravitreal aflibercept injection (IAI) for polypoidal choroidal vasculopathy (PCV) differs depending on fellow eye condition. A retrospective review was conducted for consecutive 60 eyes with PCV treated with PDT combined with IAI as well as 2-years of follow-up data. Fellow eyes were divided into 4 groups; Group 0: no drusen, Group 1; pachydrusen, Group 2; soft drusen, Group 3: PCV/fibrovascular scarring. Best-corrected visual acuity improved at 24-months irrespective of groups and there were no significant differences in visual improvement among treated eyes among the 4 groups. Within 2-years, 35 (58.3%) required the retreatment. The need for retreatment including additional injection and the combination therapy was significantly less in Group 1(12.5%) compared to the others (P = 0.0038) and mean number of additional IAI was also less in Group 1 compared to the others (P = 0.017). The retreatment-free period from the initial combination therapy was longest in Group 1 (23.6±1.1 months) (P = 0.0055, Group 0: 19.1±6.9, Group 2: 12.8±7.9, Group 3: 11.5±9.9). The need for retreatment was significantly different according to fellow-eye condition. Among PCV patients, pachydrusen in fellow eyes appear to be a predictive characteristic for a decreased treatment burden at 2 years.

OBJECTIVE: To investigate the effects of silicone oil (SiO) on macular thickness (MT) and subfoveal choroidal thickness (SFCT) in patients with macula-off rhegmatogenous retinal detachment (RRD) undergoing pars plana vitrectomy (PPV). MATERIAL AND METHODS: In this prospective study, 70 eyes of 70 patients who received SiO tamponade for the treatment of macula-off RRD were treated with PPV and a 5000-cSt SiO endotamponade followed by subsequent SiO removal. MT and SFCT were measured 1 day before and 3 months after SiO removal using spectral-domain optical tomography (SD-OCT) and enhanced depth imaging optical tomography (EDI-OCT). The patients were divided into 3 groups according to the length of time that the SiO was present: group 1 (3-6 months), group 2 (6-9 months), and group 3 (9-18 months). RESULTS: A total of 70 eyes of 70 patients with a mean age of 57.22±9.83 years (range: 30 years to 75 years) were included in the SiO (5000-cSt) study. SiO was extracted after a mean duration of 8.67±5.33 months (range, 3-18 months) after PPV. In the 1st group, BCVA increased from 1.83±0.32 log MAR before PPV to 0.85±0.41 log MAR at 3 months after silicone removal (P<0.001). In the 2nd group, BCVA increased from 1.76±0.38 log MAR before PPV to 0.86±0.48 log MAR at 3 months after silicone removal (P<0.001). In the 3rd group, BCVA increased from 1.89±0.28 log MAR before PPV to 1.08±0.63 log MAR at 3 months after SiO removal (P=0.001). There was no statistically significant change in MT in the difference values of each group. As the length of SiO presence in the eye increased, significant thinning was observed on measurement of SFCT. Differences in the SFCT values were -14.91µm, -18.76µm, and -51.50µm in groups 1, 2, and 3 respectively (P=0.004). CONCLUSIONS: A significant decrease in macular and choroidal thicknesses after SiO removal was observed. Presence of SiO endotamponade for 9 months was associated with subfoveal choroidal thinning and decreased final visual acuity in eyes undergoing RRD surgery. SD-OCT and EDI-OCT may be recommended for the treatment and follow-up of patients with complications caused by the use of SiO tamponade.

Purpose: The purpose of this study was to evaluate macular preganglionic function and to verify its relationship with retinal and choroidal morphology in patients with intermediate age-related macular degeneration (iAMD) patients. Methods: All included patients performed multifocal electroretinogram (mfERG) for investigating on macular function from the central 15° of foveal eccentricity, spectral domain optical coherence tomography (SD-OCT) for studying retinal structure, enhanced depth imaging OCT (EDI-OCT) for the measure of choroidal vascularity index (CVI), and OCT-angiography (OCTA) for the evaluation of vessel density (VD) in the superficial and deep capillary plexus, and choriocapillaris (CC) layer. Results: Twenty-seven patients with iAMD and 20 age-matched control eyes were analyzed. Significantly (P < 0.01) delayed and reduced mfERG responses in the central 0 to 2.5°, paracentral 2.5 to 5°, and overall 0 to 5° areas, as well as increased CVI values in both foveal (1 mm centered to the fovea) and fovea + parafovea areas (3 mm centered to the fovea), increased foveal and parafoveal (annular area of 1-3 mm centered to the fovea) retinal pigment epithelium thickness, and volume and parafoveal outer retinal volume were found in iAMD eyes as compared to controls. Moreover, iAMD eyes showed significantly (P < 0.01) reduced foveal and parafoveal OCTA-VD values in the CC layer when compared to controls. In the iAMD group, not significant (P > 0.01) correlations were found between morphological and functional parameters. Conclusions: Our findings support a dysfunction of photoreceptors and bipolar cells in both foveal and parafoveal areas in the presence of outer retina, CC, and choroidal structural changes, however, not significantly correlated. The observed enlargement of luminal choroidal area (measured by CVI) is possibly compensatory to CC vascular insufficiency.

PURPOSE: Indocyanine green angiography (ICGA) was compared with swept-source optical coherence tomography angiography (SS-OCTA) for the detection of polypoidal choroidal vasculopathy (PCV). DESIGN: Retrospective, cross-sectional. METHODS: Patients with treatment-naïve PCV based on ICGA imaging underwent same-day SS-OCTA imaging at Kyung Hee University Medical Center between April 2017 and November 2018. ICGA and SS-OCTA images were graded independently. SS-OCTA images were graded using both flow and structural information. Images were graded for the number of polypoidal lesions and the total lesion area, which included both the polypoidal lesions and the branching vascular networks (BVNs). RESULTS: A total of 31 eyes from 30 patients were enrolled. Polypoidal lesions were identified in all eyes using both modalities, and there was agreement on the number of polypoidal lesions in 17 eyes (55%). In 12 eyes (39%), SS-OCTA graders identified a greater number of polypoidal lesions, and in 2 eyes (6%) ICGA graders identified more lesions. There was no significant difference in the lesion area measurements (standard deviation = 1.09, P = .08). The lesion with the largest difference in area measurements resulted from focal areas of atrophy, misdiagnosed as polypoidal lesions on ICGA, and a low-lying serous retinal pigment epithelial detachment erroneously identified as part of the BVN by ICGA graders. SS-OCTA imaging correctly diagnosed the focal areas of atrophy and the serous retinal pigment epitheial detachment. CONCLUSIONS: SS-OCTA imaging was comparable to ICGA for the diagnosis of treatment-naïve PCV. However, SS-OCTA might be better than ICGA in correctly identifying both polypoidal lesions and BVNs in treatment-naïve PCV.
