The aim of this study was to investigate how Brazilian dentists perceive and manage dentin hypersensitivity (DH) in their clinical routine. A 13-item questionnaire-based survey was developed and sent electronically to a convenience sample of dentists. The questionnaire assessed the personal and dental practice characteristics of the sample, the occurrence of DH in their daily clinical practice, and management strategies. The data were analyzed descriptively and together with the chi-square test (a = 0.05). A total of 353 responses were obtained from September 2017 to March 2018. Of all the respondents, 62% were females, 49.9% reported fewer than five years of dental practice, and 70.5% were self-identified as private practitioners. Most of the dentists reported an estimated frequency (30-60%) of patients with DH in their practice. The most frequently cited (91.79%) trigger of DH was air blast and/or scratching with a probe. The first-choice strategy to manage DH was a dentin desensitizer (48.16%). The number of years in clinical practice did not influence DH relapse frequency (p = 0.76) significantly, or consider DH treatment as a problem (p = 0.22). The present findings indicate that, regardless of clinical experience, dentists in Brazil still consider DH management a challenge in their daily dental practice. In addition, the results suggest that guidelines should be developed to disseminate the available knowledge regarding this condition in ways that may influence decision-making processes among practitioners.

PURPOSE: Dentin hypersensitivity (DH) is a prevalent painful condition of the teeth, the management of which lies in the obliteration of patent dentinal tubules. The purpose of this study was to compare the efficacy of 5% potassium nitrate (PN), low-level laser therapy (LLLT), and combination of low-level laser therapy and 5% potassium nitrate (LLLT+PN) in patients with fluorotic and non-fluorotic hypersensitive teeth. MATERIALS AND METHODS: Ninety self reporting patients with DH completed the randomised, double-blind study. Patients were equally divided into fluorosis group (FG), with moderate to moderately severe fluorosis and the non-fluorosis group (NFG) and subdivided into 3 groups to receive PN, LLLT (810 nm, 1 W) alone and LLLT+PN. Each participant had at least one tooth eliciting a response of ≥ 3 on a visual analog scale (VAS) to evaporative, thermal and electric tactile stimulus (digital scratch-o-meter). The teeth were evaluated at baseline, 30 min post treatment, 1 week, 4 weeks and 12 weeks. The magnitude of pressure applied by the scratch-o-meter was also assessed for all three interventions. RESULTS: Statistically, LLLT+PN was more effective in alleviating DH at all time intervals compared to baseline. However, statistically significant results (p ˂ 0.05) were seen with LLLT+PN at 12 weeks in FG. Magnitude of pressure applied showed statistically significant differences with LLLT+PN at 30 min post treatment. CONCLUSION: LLLT+PN alleviated DH at all time intervals, with a greater effect in the fluorotic group. However, LLLT+PN was more efficacious in reducing DH at 12 weeks post treatment in the fluorotic group.

PURPOSE: To compare the clinical efficacy of toothpastes containing Pro-Argin and NovaMin as dentin hypersensitivity (DH) treatment. MATERIALS AND METHODS: A systematic review and meta-analysis based on PRISMA was conducted (PROSPERO registration CRD42018095367). Electronic searches were performed in Pubmed, Scopus, Cochrane Library, Web of Science, Virtual Health Library and Open Grey until June 2018, with the terms [pro-argin OR arginine] AND [novamin OR calcium sodium phosphosilicate OR calcium sodium phospho silicate]. Randomised and non-randomised clinical trials comparing DH reduction in adults given Pro-argin-containing toothpastes and NovaMin-containing toothpastes were included. Study selection and quality assessment with Cochrane tool were performed. In the meta-analysis, the comparison between desensitising toothpastes was assessed by standardised mean difference (SMD) with 95% confidence intervals (CI). Certainty of evidence was evaluated with GRADE. RESULTS: Five studies fulfilled the inclusion criteria. In four included studies, a reduction in the DH under tactile stimulus between baseline and post-application times for both toothpastes was demonstrated. Three studies were suitable for meta-analysis. No statistically significant difference between the two toothpastes for DH reduction was observed at immediate (SMD = -1.05, CI = -3.52;1.41), 2-week (SMD = -0.55, CI = -2.59;1.48) and 4-week (SMD = -0.49, CI = -2.78;1.81) follow-up. Certainty of the evidence was very low. Included studies presented a high risk of bias. CONCLUSIONS: Pro-argin-containing and NovaMin-containing toothpastes showed effectiveness for DH reduction. No statistically significant difference between the two toothpastes was found. Thus, both can be prescribed to treat DH in adults with equivalent effectiveness up to four weeks.

BACKGROUND: Dentine hypersensitivity can impact functional status and everyday activities such as eating and talking. This study aimed to assess changes in oral health-related quality of life measures in individuals with dentine hypersensitivity following long-term use (24 weeks) of a commercially available toothpaste marketed for dentine hypersensitivity relief. METHODS: This study was conducted across two sites and enrolled 75 adults with ≥2 non-adjacent sensitive teeth. Participants were assigned to twice-daily brushing with toothpaste containing 0.454% w/w stannous fluoride (1100 ppm fluoride). Every 4 weeks, participant-reported outcomes were assessed using the Dentine Hypersensitivity Experience Questionnaire (DHEQ), a condition specific oral health-related quality of life scale that has five domains and includes questions on social and emotional impact, restrictions, adaptations and effect on life overall. Responses to a clinically applied evaporative (air) stimulus were assessed using the examiner-observed Schiff sensitivity scale and Labelled Magnitude Scales (LMS), which included dentine hypersensitivity-specific descriptors of intensity, duration, tolerability and descriptive qualities of the participant's response. RESULTS: Participant-reported outcomes demonstrated reduction of the impact of dentine hypersensitivity over time on health-related quality of life, as measured by the DHEQ. This reached statistical significance from Week 8 onwards (p < 0.0001 versus baseline) for the Total DHEQ score, with scores continually decreasing at each timepoint. Most domain scores followed a similar pattern. Statistically significant reductions were also detected for the examiner-observed Schiff Sensitivity Scale scores at all timepoints (including at 4 weeks) (p < 0.05), which were mirrored by LMS responses. The toothpaste was generally well-tolerated. CONCLUSIONS: These results show that long-term use of a sensitivity toothpaste containing 0.454% w/w stannous fluoride has a beneficial, ongoing, impact on the oral health-related quality of life of people with dentine hypersensitivity. PURPOSE: This study investigated the role of desensitising-only and desensitising/whitening versions of arginine and calcium carbonate-containing (ACC) and calcium sodium phosphosilicate-containing (CSPS) toothpastes on surface loss and permeability of root dentine. MATERIALS AND METHODS: Slabs of human root dentine were embedded and exposed to citric acid solution to create a lesion resembling hypersensitive dentine. Sixty specimens were divided into six groups and exposed to slurries made with ACC (Colgate Sensitive Pro-Relief) and CSPS (Sensodyne Repair & Protect) toothpastes, in their desensitising or desensitising/whitening versions, an ordinary toothpaste (Crest Cavity Protection) or distilled water (DI). The other 60 specimens were brushed with either slurries or DI and assessed for surface loss. All specimens were analysed for dentine permeability. Data were subjected to analysis of variance (ANOVA) and Fisher's least statistically significant difference tests. RESULTS: ANOVA (p = 0.018) showed that in the group brushed with DI surface loss was lower than in the groups brushed with toothpastes, but there was no statistically significant difference among the ordinary, desensitising-only and desensitising/whitening toothpastes. Dentine permeability was not influenced by brushing (p = 0.125). Toothpastes affected dentine permeability (p = 0.004), with the groups submitted to desensitising-only and desensitising/whitening toothpastes presenting significantly lower permeability than those submitted to DI, but no difference existed between the group treated with the ordinary toothpaste and DI. CONCLUSION: In comparison to desensitising-only, desensitising/whitening toothpastes neither accounted for increased surface loss nor impaired tubule occlusion of root dentine.

AIM: To evaluate effects of a 0.454% stannous fluoride test toothpaste on dentine hypersensitivity (DH) applied by fingertip, then 3 days' brushing, versus a sodium monofluorophosphate-based control. MATERIALS AND METHODS: In three randomized clinical studies, DH was assessed using evaporative (Schiff scale) and tactile (Yeaple probe) stimuli. Participants applied toothpaste to two sensitive teeth by fingertip (60 s each); DH was re-assessed, prior to brushing. Test treatment participants brushed their sensitive teeth, with all participants then brushing all teeth for ≥60 s, twice daily for 3 days. DH was re-assessed. Data were analysed by study and then pooled. RESULTS: In two studies, test treatment significantly reduced DH versus control treatment after fingertip application and 3 days' brushing (both measures). In one study, both treatments significantly reduced DH without between-treatment differences. Mean Schiff differences (95% confidence intervals) for fingertip/3d were as follows: Study 1: -0.09 (-0.280, 0.092)/ -0.18 (-0.442, 0.072); Study 2: -0.72 (-0.839, -0.610)/ -1.02 (-1.150, -0.882); and Study 3: -0.26 (-0.387, -0.123)/ -0.92 (-1.055, -0.793). Pooled analysis indicated test treatment significantly reduced DH versus control (both timepoints, both measures). Toothpastes were generally well-tolerated. CONCLUSION: Studies indicated that single, fingertip application of a SnF2 toothpaste reduced DH versus a control. DH relief increased over 3 days.

AIM: This in vitro study was designed to evaluate and compare the effectiveness of a dentifrice containing 8% arginine and calcium carbonate, diode laser (810 nm) and the combined use of diode laser (810 nm) and the same dentifrice in occluding dentinal tubules. MATERIALS AND METHODS: Forty sectioned dentine discs were observed in this study. The Samples were randomly divided into 4 groups. In group A, a dentifrice containing arginine and calcium carbonate was applied. In group B, the specimens were subjected to diode laser (810 nm) irradiation. In group C, the same dentifrice was applied and the specimens were subjected to diode laser (810 nm) treatment. Group D was left untreated and served as the control. The specimens were observed under the scanning electron microscope. Then the photomicrographs of each specimen were taken which were uploaded into Photoshop 7®, and the diameters of the dentinal tubules of each specimen were measured and the data were analyzed statistically. RESULTS: The overall comparison between different treatment modalities using one-way analysis of variance (ANOVA) and multiple comparisons of two modalities assessed using post hoc Tukey method revealed that there is a highly significant difference (p value = 0.0001) in the dentinal tubule diameter and rate of occlusion in different treatment modalities. CONCLUSION: The study concluded that all three treatment modalities caused significant occlusion of dentinal tubules. The combined use of desensitizing toothpaste and diode laser (810 nm) is the most effective, then comes the diode laser (810 nm) alone and finally the desensitizing toothpaste alone. CLINICAL SIGNIFICANCE: This study suggests an effective treatment of dentinal hypersensitivity using diode laser and desensitizing toothpaste.

PURPOSE: To evaluate the ability of an arginine-containing dentifrice to occlude dentin tubules. METHODS: Dentin discs were divided equally into premolar and molar groups, which were then utilized in three treatment groups: a blank control group (distilled water treatment), a negative control group (common dentifrice with calcium carbonate) and an experimental group [dentifrice with 8% (w/w) arginine]. Each dentin disk was brushed with the dentifrice twice daily for 7 consecutive days. After this period, each disc was separated into two equal halves. One half was used for scanning electron microscopy (SEM) and energy-dispersive spectrometer (EDS) examinations, while the other half was brushed with distilled water twice daily for another 7 days prior to SEM observation. RESULTS: The plugging rate in the arginine dentifrice group was significantly higher and more sustainable than in the negative control group. The surface deposition of calcium and phosphorus on the dentin discs in the arginine dentifrice group was also significantly higher. CLINICAL SIGNIFICANCE: This study provided evidence that using arginine as an active ingredient in dentifrice can improve its ability to occlude dentin tubules, thus supporting future efforts to improve dentin hypersensitivity.

Objetivos: O seguinte trabalho relatou por meio de um caso clínico a eficácia do dessensibilizante Nano-P® e do Flúor acidulado em um paciente com hipersensibilidade dentinária. Métodos: a boca da paciente foi dividida para a aplicação de cada tratamento. Nos quadrantes 1 e 3 foi utilizado o dessensibilizante Nano P® e nos quadrantes 2 e 4 foi utilizado o flúor acidulado, com aplicações semanais por 4 semanas. A hipersensibilidade dentinária foi avaliada através da Escala Visual Analógica (EVA) e por meio de estímulos osmótico e tátil. Resultados: Para ambos os tratamentos realizados, foi constatado a diminuição no grau de hipersensibilidade relatado pela paciente ao longo das consultas, com tempos de ação diferentes para cada agente dessensibilizante, devido à suas características e mecanismos de ação diferenciados, bem como diferenças em relação ao conforto relatado nas duas técnicas pela paciente participante do estudo. Conclusões: Os agentes dessensibilizantes avaliados neste estudo foram eficazes na redução da sensibilidade dentinária após 5 consultas de avaliação(AU)


Aim: The following work reported in a clinical case the efficacy of Nano-P® desensitizer and acidulated fluoride in a patient with dentin hypersensitivity. Methods: the patient's mouth was divided for the application of each treatment. In the quadrants 1 and 3 the Nano P® desensitizer was used and in the 2 and 4 quarters the acidulated fluoride was used, with weekly applications for 4 weeks. Density hypersensitivity was evaluated by the Visual Analogue Scale (VAS) through osmotic and tactile stimulation. Results: For both treatments, it was observed a decrease in the degree of Hypersensitivity reported by the patient during the consultations, with different action times for each desensitizing agent, due to its differentiated characteristics and mechanisms of action, as well as differences in relation to the comfort reported in the two techniques by the patient participating in the study. Conclusions: The desensitizing agents evaluated in this study were effective in reducing dentin sensitivity after 5 evaluation visits(AU)

BACKGROUND: Dentine hypersensitivity (DH) can occur after gum recession or enamel loss and may impact quality of life. Treatments include toothpastes that decrease DH by occluding dentine tubules. One effective occluding ingredient used in toothpastes is stannous fluoride (SnF2), but this can be unstable in aqueous formulation. These three studies aimed to characterise the short-term effects of an experimental, anhydrous SnF2 dentifrice on DH. METHODS: Three examiner-blind, parallel-group studies evaluated DH in participants with the condition after a single brushing and after 3d brushing with an experimental anhydrous 0.454% SnF2/polyphosphate toothpaste (Test) or a toothpaste containing 0.76% sodium monofluorophosphate (Control). Test treatment participants brushed two pre-identified sensitive teeth first, then their remaining dentition for ≥1 min ('focused brushing'). Control treatment participants brushed their whole dentition for ≥1 min. DH was measured after single brushing and after 3d twice-daily use, via evaporative (air) (Schiff Sensitivity Scale) and tactile (Yeaple probe) stimuli and analysed using an ANCOVA model. RESULTS: In all studies, after 3d treatment, the Test toothpaste/brushing regimen significantly reduced DH compared to the Control regimen by both evaporative and tactile stimuli assessment (p < 0.0001 for all). The Test regimen also significantly reduced DH from baseline at both time-points by both measures in all studies (p < 0.0001 for all). Mean Schiff sensitivity score differences (95% confidence intervals) between Test and Control regimens after 3d were: Study 1: - 0.45 (- 0.577, - 0.319); Study 2: - 0.40 (- 0.505, - 0.300); Study 3: - 1.31 (- 1.500, - 1.128). Mean tactile score differences were: Study 1: 11.30 (7.927, 14.662); Study 2: 3.57 (2.531, 4.614); Study 3: 24.54 (20.349, 28.736). After single use, in Studies 2 and 3, the Test toothpaste/brushing regimen significantly reduced DH versus Control by both measures (p < 0.001 for all); in Study 1, treatment differences were not significant. Toothpastes were generally well-tolerated. CONCLUSIONS: Taken together, these studies indicated focused brushing with an experimental anhydrous 0.454% SnF2/polyphosphate toothpaste reduces DH compared to brushing with a conventional toothpaste after single use, with greater reduction after 3d. Background: Dentin hypersensitivity (DH) affects all age groups in a population and is perceived as pain to any stimuli. From time immemorial, researchers have sought herbal or natural solutions to treat hypersensitivity. Propolis is one such natural, nontoxic resinous substance produced by honey bees, which is useful in various applications in dentistry and effective in treating dentinal hypersensitivity. Aim: The aim of this in vitro study is to assess the effect of propolis varnish on occlusion of dentinal tubules thus aiding in the treatment of dentinal hypersensitivity. The objective is to evaluate the effectiveness of the proposed treatment using scanning electron microscope (SEM) imaging. Materials and Methods: Twenty naturally extracted teeth were collected and stored until in vitro assessment. Discs obtained from each tooth were divided into two groups of 20 each - I (test) and II (control), with each tooth acting as its own control. Propolis varnish was applied only to the acid-etched surface of the exposed dentin of discs in the test group, whereas all the discs were subjected to SEM imaging. Results: Statistical analysis showed a significant reduction in open tubules (P < 0.001) from 160 ± 6.97 before treatment to 61.20 ± 9.10 after propolis varnish application in the test group. Conclusion: This study showed the promise of propolis varnish as a natural treatment modality for DH.

OBJECTIVES: To compare a 3.14% potassium oxalate strip and 8% arginine calcium carbonate toothpaste for the reduction of dentine hypersensitivity after 2 and 4 weeks. METHODS: This was an examiner-blind, parallel study in 80 healthy adults with dentine hypersensitivity (Schiff score >2) in >1 tooth. After acclimatisation, participants were randomised to the oxalate desensitising strip with fluoride toothpaste or the arginine desensitising toothpaste control which also contained fluoride. Products were applied under supervision of study staff after measuring baseline sensitivity, thereafter the strip or control toothpaste (fingertip application) was applied after 1 and 2 weeks, and teeth brushed twice-daily with the fluoride (test group) or the fluoridated arginine control toothpaste. Sensitivity was assessed following airblast (Schiff and VAS) and tactile stimuli (Yeaple probe) at baseline, 2 and 4 weeks. RESULTS: Both groups showed significant reductions from baseline in VAS, Schiff and Yeaple sensitivity scores after 2 and 4 weeks (p < 0.0005). The oxalate group had significantly lower Schiff and higher Yeaple probe scores compared to control after both time points (p < 0.0002 and p < 0.05), but while scores favoured the oxalate group, there were no significant differences in VAS. CONCLUSIONS: This study demonstrated application of a 3.14% potassium oxalate strip combined with toothbrushing with paste was more effective in pain management of dentine hypersensitivity than brushing with arginine toothpaste. CLINICAL SIGNIFICANCE: Treatment of sensitive teeth with the oxalate strip reduced dentine hypersensitivity after 2 and 4 weeks to a significantly greater degree than a positive control sensitivity toothpaste demonstrating that oxalate strips are an effective targeted treatment for dentine hypersensitivity sufferers.

OBJECTIVES: To compare the treatments used to treat dentin hypersensitivity (DH), based on its efficacy and effect duration. METHODS: Medline/PubMed, Cochrane Library, EMBASE and ClinicalTrials were searched for articles published between 1 January 2008 and 14 November 2018, in English, Portuguese or Spanish, reporting clinical trials, completed and with results. This systematic review protocol was registered in PROSPERO, number CRD42019121986. RESULTS: Seventy-four randomised clinical trials were included in the systematic review, reporting patients from 16 to 65 years old, with a clinical diagnosis of DH, that evaluate the efficacy of a desensitising product, compared to pre-treatment, used the evaporative method stimulation and the visual analogue scale. These studies evaluated 5366 patients and at least 9167 teeth. Seven follow-up periods were considered corresponding to an immediate, medium or long-time effect. Sixty-six studies were included in the quantitative synthesis. Glutaraldehyde with HEMA, glass ionomer cements and Laser present significant immediate (until 7 days) DH reduction. Medium-term (until 1 month) reduction was observed in stannous fluoride, glutaraldehyde with HEMA, hydroxyapatite, glass ionomer cements and Laser groups. Finally, long-term significant reduction was seen at potassium nitrate, arginine, glutaraldehyde with HEMA, hydroxyapatite, adhesive systems, glass ionomer cements and LASER. CONCLUSIONS: All active ingredients show efficacy in DH reduction in different follow-up times. Only in-office treatments are effective in immediate DH reduction, maintaining its efficacy over time. For long-time effects, at-home treatments can also be used. More standardised evaluation protocols should be implemented to increase the robustly of the results.

Dentine hypersensitivity (DH) is a common and harrowing dental condition. A novel BioMin-F technology that contains 5% fluorocalcium phosphosilicate bioactive glass has been introduced. It forms fluorapatite, which is more stable towards acid erosion. There is a lack of literature with the utility of this toothpaste in treating DH. Therefore, the authors of this randomised clinical trial have aimed to compare and evaluate the efficacy of 5% fluorocalcium phosphosilicate with an 8% arginine and calcium carbonate and placebo toothpaste. A total of 75 patients clinically diagnosed with DH were randomly divided into three groups: Group A, 5% fluorocalcium phosphosilicate; Group B, 8% arginine and calcium carbonate; and Group C, placebo. The DH was evaluated by tactile and evaporative stimuli, and a visual analogue scale (VAS) was used for evaporative stimuli at pre-baseline, baseline (15 days) and post-baseline (1 month). The results showed symptoms of DH were reduced in all three groups. However, Group A showed a better reduction of DH than the other two groups. The toothpaste containing 5% fluorocalcium phosphosilicate was reported to be more efficacious than the other two toothpastes in managing DH. This novel development opens up a unique opportunity in the prevention and management of DH and may also be beneficial in preventing acid erosion of the tooth surface and in the maintenance of oral hygiene by reducing the effects of plaque accumulation and gingival inflammation.

OBJECTIVES: To evaluate the performance of calcium/phosphate desensitizing agents when used for dentin permeability and tubule occlusion. METHODS: 1 mm-thick dentin specimens were immersed in 17% EDTA solution and allocated into 7 groups: 1. Clinpro White Varnish, 2. Clinpro XT Varnish, 3. Teethmate Desensitizer, 4. Desensibilize Nano P, 5. Nupro prophylaxis paste, 6. Duraphat (reference product), and 7. Control (no treatment). After treatment, specimens were submitted to erosion-abrasion cycling for 5 days. Dentin permeability was assessed by hydraulic conductance (n = 10) and environmental scanning electron microscopy ESEM (n = 8) post-EDTA, post- treatment and post-cycling. The percentage of permeability (%Lp) was calculated post-treatment and post-cycling. ImageJ software was used to obtain the number of open dentin tubules (ODT) in the micrographs. Data were statistically analyzed (α = 0.05). RESULTS: Post-treatment, the %Lp values of all treatments were significantly lower than the control, with Nupro presenting higher %Lp values than Duraphat. All the groups presented significantly lower %Lp values in the post-treatment in comparison with the post-cycling period, except the control. Post-treatment, all groups showed lower numbers of ODTs than the control, except Nupro. Clinpro WV, Clinpro XT, Duraphat and Nano P presented the best results. Post-cycling, there was no significant difference among groups. Clinpro WV and Duraphat presented lower numbers of ODTs post-treatment than they had post-cycling, and the control had a higher number of ODTs post-treatment than they had post-cycling. CONCLUSIONS: Most treatments were efficient in reducing both dentin permeability and number of ODTs after treatment; however, none of the products were able to resist the erosive-abrasive challenges.

Objective: A single-blind randomized clinical trial was conducted to evaluate the effectiveness of desensitizing agents with different action mechanisms in reducing cervical dentin hypersensitivity (CDH) after four application sessions, with 24-week follow-up. Materials and methods: Sixty patients with CDH were selected in the study and were allocated in three groups of treatment: Desensibilize KF 2%, Clinpro XT Varnish, and Photon Lase III (100 mW, 4 J/cm2-1 J/cm2 each point, 10 sec per point with wavelength of 808 nm). There were four application sessions performed, with a 48-h interval between each one. The evaporative stimuli and visual analog scale were used to evaluate the CDH level at baseline, immediately after treatment, and at 2, 4, 8, and 24 weeks after the application. Mixed-model effects test was used for comparison (α = 0.05).Results: All three groups showed significant reduction in CDH from baseline to each all-subsequent follow-up. All the groups maintained the CDH reduction, and presented no statistical differences between each other after treatment (p = 0.885), 2 (p = 0.857), 4 (p = 0.928), 8 (p = 0.206), and 24 weeks (p = 0.073) of follow-up.Conclusions: The four-session protocol was an effective approach in reduction of CDH (even after 24 weeks), regardless of desensitization mechanism.

Objective: The aim of this article was to describe and discuss a clinical case in which the Nd:YAG laser and a desensitizing agent were associated in a protocol for dentin hypersensitivity (DH) in teeth with molar-incisor hypomineralization (MIH). Background data: DH is a common condition among the population, being one of the main reasons for patients to seek for a dental treatment. MIH can be considered a condition that induces DH. Methods: The patient was referred for evaluation, complaining of hypersensitivity during eating or drinking cold foods. Clinical evaluation revealed the presence of MIH on the first lower molars. Aiming to treat DH, two strategies for dentin desensitization were associated: the use of a high-power laser (Nd:YAG laser, 1.064 nm; Lares Research), with a 300 µm quartz optical fiber, 1 W of power, 100 mJ of energy, 10 Hz of repetition rate, and 85 J/cm2 of energy density, followed by the application of two layers of a desensitizing agent (Gluma Desensitizer). Results: DH was evaluated immediately, after 1 week and after 1 month of the treatment. Clinical outcomes were satisfactory, confirming the efficacy and considerable durability of the protocol used for the reduction of DH originated from MIH. Conclusions: The association of Nd:YAG laser and a desensitizing agent for controlling DH was effective, showing to be an interesting protocol.

PURPOSE: To evaluate the effect on dentin of chondroitin sulfate and L-arginine on dentin tubule occlusion. METHODS: The dentin samples were activated by submersion in an aqueous ( aq. ) solution of chondroitin sulfate ( ChS) or L-arginine prior to application of a commercial or custom-made toothpaste. After rinsing with water and ultrasonication, adhesion to dentin and occlusion of dentin tubules were evaluated by scanning electron microscopy and the elemental composition of the deposits was evaluated by energy-dispersive x-ray spectroscopy. RESULTS: Rinsing a dentin sample with a solution of ChS resulted in an increase in the adherence of dentifrices containing either titanium dioxide (TiO2 ) or calcium-based nanoparticles [ hydroxyapatite ( HA\ or calcium carbonate( to the dentin surface. ChS does not appear to enhance the adherence of dentifrices lacking TiO2. Pretreatment by L-arginine improved adherence of calcium carbonate nanoparticles, but less efficiently than ChS. Addition of nanoparticles of hydroxyapatite or calcium citrate to dentifrices improved their adherence to dentin without any pre-treatment. CLINICAL SIGNIFICANCE: The significant increase in adherence to the dentin surface of dentifrices of either TiO2 or calcium-supplying nanoparticles to the dentin surface following pre-treatment with ChS or L-arginine opens the door to the development of two-step dental treatments, which accomplish dentin tubule occlusion and help to deliver active dentifrice components to the dentin surface. The ability of the aqueous pastes of nanoparticles of hydroxyapatite or calcium citrate to occlude dentin tubules enables the formulation of desensitizing dentifrices, which also supply the mineral and organic nutrients to the tooth surface.

PURPOSE: To compare the efficacy of a 2.5% w/w smaller particle-size (ca 4 µm) calcium sodium phosphosilicate (CSPS) dentifrice (test) to an occluding dentifrice (8% w/w arginine) (comparator) and a negative control dentifrice in an 8-week, randomised, controlled, parallel-group, stratified (maximum baseline Schiff sensitivity score) study in adults with dentin hypersensitivity. MATERIALS AND METHODS: The sensitivity of two selected teeth was assessed at baseline and after 1, 2, 4 and 8 weeks in response to evaporative (air) (Schiff sensitivity score and visual analogue scale [VAS]) and tactile (tactile threshold) stimuli. RESULTS: Of 151 randomised subjects, 147 completed the study. Statistically significant changes from baseline were reported for test and comparator dentifrices at all weeks on all measures (p < 0.05) with no statistically significant differences between them. The negative control dentifrice group was statistically significantly different from baseline in all measures by week 8 (p < 0.05). Test and comparator dentifrices demonstrated statistically significantly greater improvements in Schiff sensitivity from week 2 compared to the negative control (p < 0.05). There was a statistically significant difference in favour only of the comparator dentifrice over the negative control at week 8 when examining tactile threshold (p = 0.0435) and at week 4 with VAS (p = 0.0425), with no other between-group differences. The dentifrices were generally well tolerated. CONCLUSION: No statistically significant differences were found between a small particle size 2.5% w/w CSPS dentifrice and an 8% w/w arginine dentifrice in terms of a dentine hypersensitivity decrease.
