RESUMO

"Innovation" has become a ubiquitous term in the business of health care. The concept of disruptive innovation adds a new complexity to this popular notion. The emergence of artificial intelligence, virtual reality, and venture capitalists generates excitement, doubt, and curiosity for stakeholders across health care organizations. As companies such as Amazon, Apple, CVS, and JP Morgan enter this sector, disruptions will emerge that aim to improve the quality of care while reducing overall cost. The purpose of this article is to differentiate the concepts of innovation and disruptive innovation, explore their significance to the nursing profession, and outline 3 key roles nurse leaders can occupy as they navigate the future ever-changing trajectory of health care.

RESUMO

OBJECTIVE: Shortages in the speciality nursing workforce, both nationally and internationally are driving the need for the development of an evidence-based model to inform recruitment and retention into speciality nursing practice. This study aimed to identify the factors influencing rapid and early career transition into speciality nursing practice. METHODS: A comprehensive systematic review of the literature was undertaken using a convergent qualitative synthesis design where results from qualitative, quantitative and mixed methods studies were transformed into qualitative findings. Databases included CINAHL, Medline, Scopus and PsycINFO. Search terms were: nurse, early career, rapid career, transition, specialty, and Medical Subject Heading terms included: professional development and educational, nursing, and continuing. Using validated tools, papers were independently assessed by a minimum of two reviewers. RESULTS: Twenty-three research articles were included. There were no randomized control trials. Through thematic analysis and matrix mapping of the results, the TRANSPEC model was developed. The model outlines three phases of transition: pre-entry, incomer and insider. There has been little focus on pre-entry with programs being designed at the incomer and insider phases. Impacting on these phases are three concepts: the self (professional and personal), the transition processes (informal and formal) and a sense of belonging. The overarching theme influencing the phases and concepts is the context of practice. Enablers and inhibitors influence successful transition and therefore impact on recruitment and retention. Each nurse's transition is influenced by time. CONCLUSIONS: For successful transition, the enablers and inhibitors impacting on the three concepts, phases and the context of practice need to be considered when developing any program. It is apparent that while previous studies have focused on the transition processes, such as curricula, the development of the self and a sense of belonging are also essential to successful transition. Further studies should include the pre-entry phase.

Method: a review of the general written press through the analysis of news published in the following newspapers: El País, El Mundo, La Vanguardia and ABC, including in their headlines or lead paragraphs the terms "Enfermería" ("Nursing"), "enfermera" ("female nurse"), or "enfermero" ("male nurse"). The Flesch-Szigriszt Index was used in order to assess the linguistic readability of the headlines and lead paragraphs, calculated through the INFLESZ program. Results: in total, 534 pieces of news were retrieved. By newspapers, 185 (34.64%) pieces of news were collected from La Vanguardia, 165 (30.90%) from El Mundo, 100 (18.73%) from ABC and 84 (15.73%) from El País. Of these articles, 31.46% (n= 168) dealt with "Working conditions", 13.30% (n= 71) with "Research-University", 12.17% (n= 65) with "events" and 9.74% (n= 52) with "nursing prescription". The inclusion of the term "enfermero" ("male nurse") (n=179) was higher than the use of "enfermera" ("female nurse") in the articles collected. The level of readability of headlines and lead paragraphs was "somewhat difficult". Conclusions: the male term "enfermero" is more usual than the female "enfermera" in the Spanish written press. A third part of the news retrieved dealt with working conditions. Readability was not adequate

RESUMO

OBJECTIVE: to report the experience of the development of nursing knowledge from nursing theories and points of contact with the nurse's praxis. METHOD: case report on the reflections on contents addressed in the discipline offered in the Graduate Program in Nursing in a federal public university. The students presented seminars approaching the main nursing theories, generating extensive discussion and correlating them with the nurse's practice. Review and reflection on the contents addressed. RESULT: two themes were defined: the training of nursing undergraduate students and the development of knowledge in relation to the concepts of nursing theories; and care models and nurses' critical reasoning about the applicability of nursing theories. FINAL CONSIDERATION: the reflections produced permeated the development and acquisition of knowledge and of representative meanings of healthcare practice from the assumptions and constructs of the theories.

RESUMO

Advanced practice is now possible in nursing practice in France. It is starting to be taught at university. Significant changes to practices are taking place in the short term, for nurses. In this context, it is interesting to look back at the foundations of nursing autonomy rooted in the origins of the scope of nursing practice.

RESUMO

OBJECTIVE: To analyze the nursing errors reported by the journalistic media and interpret the main implications of this communication for the visibility of this problem. METHOD: Documental research, qualitative, descriptive and exploratory, with data collected in news reports from Brazil and Portugal, analyzed through hermeneutics with resources of Atlas Software. RESULTS: We analyzed 112 news items published between 2012 and 2016 that resulted in six categories: Year - highest occurrence in 2012; Age group of the patient - children; Professional category - nurses; Type of error - medication; Outcome - death; Possible attributed cause - occupational conditions. FINAL CONSIDERATIONS: Nursing mistakes are a challenge for the profession, and the way they are communicated by the media is not very explanatory, contributing to a negative visibility of the profession, and to making society insecure. Improving the way they are served in the media contributes to the visibility of the problem without affecting the professional image.

RESUMO

This article puts forward a theoretical model of nursing practice, elaborating and illustrating how the model can be used by nursing practitioners, students, and educators. Experiences with using the model in a pilot study with a clinical supervisor and three nursing students is reported. Implications of the model for working with competence development is discussed, both in relation to issues raised by Weeks et al. (2017) specifically, and to competence-based approaches more generally. A key point is that a focus on competence alone may not provide students with adequate conceptual tools for interpreting and acting competently in practical situations. The proposed theoretical model is seen as providing such a tool.

RESUMO

Based on a Lived Experience Description written by an experienced nurse in Denmark, this article offers an ontological and existential-phenomenological exploration of aesthetic dimensions of excellence in nursing. In the research of Patricia Benner and colleagues, excellence in nursing is described as a matter of intuitive pattern recognition based on clinical experience and narrative understanding. In this article, and based on phenomenological reflections and philosophical inspirations from the Danish philosopher Dorthe Jørgensen and the French philosopher Maurice Merleau-Ponty, I suggest an extended understanding of excellence in nursing, and I argue that the work and research of Benner seem to overlook ontological and aesthetic aspects of excellence in nursing. This article argues that aesthetic aspects of excellence in nursing can be comprehended as moments where nurses sense a surplus of meaning and moments where the nurse and the patient share and are subjected to communal and unfolding life phenomena. Aesthetic dimensions of being in nursing can, from an ontological approach, best be described as similar to the way human beings listen to a piece of music or to how artists try to give a voice to the world: as a resonant way of being and becoming within the moment.

RESUMO

AIM: To evaluate horizontal violence and bullying in the nursing workforce of an oncology inpatient and outpatient academic centre and to test the Horizontal Violence Scale in an outpatient setting. BACKGROUND: Horizontal violence (HV) and bullying in the workplace are dysfunctional behaviours that can affect nursing staff and patient care. The impact of bullying and HV is multilevel, affecting patient safety and satisfaction, nursing retention and employee satisfaction, while creating poor patient outcomes. METHODS: A quantitative cross-sectional descriptive design was used to examine prevalence of HV and bullying and the relationship between HV and bullying of nursing department personnel in an oncology setting. RESULTS: The study findings revealed significant positive relationships between bullying and HV in both inpatient and outpatient settings. CONCLUSION: Nurses experience diverse workplace violence, which could decrease their professional quality of life and affect their turnover intention no matter the work setting. IMPLICATIONS FOR NURSING MANAGEMENT: The findings of this study clearly link the presence of bullying and HV, which includes, emotional, physical, verbal and defiant behaviours to both inpatient and outpatient workplace settings. A positive organisational culture, which can be shaped by nursing leaders, can create a work environment that can thwart workplace violence.
