RESUMO

Best practice recommendations in cancer care increasingly call for integrated rehabilitation services to address physical impairments and disability. These recommendations have languished primarily due to a lack of pragmatic, generalizable intervention models. This perspective paper proposes a clinically integrated physical therapist (CI-PT) model that enables flexible and scalable services for screening, triage, and intervention addressing functional mobility. The model is based on (1) a CI-PT embedded in cancer care provider clinics, and (2) rehabilitation across the care continuum determined by the patient's level of functional mobility. The CI-PT model includes regular screening of functional mobility in provider clinics via a patient-reported mobility measure-the Activity Measure for Post-Acute Care, a brief physical therapy evaluation tailored to the specific functional needs of the individual-and a tailored, skilled physical therapist intervention based on functional level. The CI-PT model provides a pragmatic, barrier-free, patient-centric, data-driven approach to integrating rehabilitation as part of standard care for survivors of cancer. The model standardizes CI-PT practice and may be sufficiently agile to provide targeted interventions in widely varying cancer settings and populations. Therefore, it may be ideal for wide implementation among outpatient oncological settings. Implementation of this model requires a shared approach to care that includes physical therapists, rehabilitation administrators, cancer care providers, and cancer center administrators.

RESUMO

The American Physical Therapy Association (APTA), in conjunction with the Cardiovascular and Pulmonary Section of APTA, have commissioned the development of this clinical practice guideline to assist physical therapists in their clinical decision making when managing patients with heart failure. Physical therapists treat patients with varying degrees of impairments and limitations in activity and participation associated with heart failure pathology across the continuum of care. This document will guide physical therapist practice in the examination and treatment of patients with a known diagnosis of heart failure. The development of this clinical practice guideline followed a structured process and resulted in 9 key action statements to guide physical therapist practice. The level and quality of available evidence were graded based on specific criteria to determine the strength of each action statement. Clinical algorithms were developed to guide the physical therapist in appropriate clinical decision making. Physical therapists are encouraged to work collaboratively with other members of the health care team in implementing these action statements to improve the activity, participation, and quality of life in individuals with heart failure and reduce the incidence of heart failure-related re-admissions.

The purpose of this study was to determine and analyse Physical Education (PE) teachers' perceptions about inclusion of students with physical disabilities in regular PE classes and how such inclusion could be improved by the contribution of other professionals. The information was obtained through a questionnaire administered to 56 PE teachers and an interview with nine PE teachers and four physical therapists. The results reveal PE teachers' concern and uncertainty about providing quality attention to students with physical disabilities in their classes and their demand for specific training and support in class. They see the need for support from professionals such as Physical Therapists' when working with students with motor disabilities. We suggest further examination of the barriers faced by both professionals, their views on PE teachers and the physical therapists' role, as well as their willingness to collaborate with each other


O objetivo do estudo foi determinar e analisar as percepções dos professores que lecionam Educação Física (EF) a alunos com deficiência nas classes inclusivas de EF e como melhorar essa inclusão ao colaborar com o fisioterapeuta. A informação foi obtida através de um questionário administrado a 56 professores, que foi completado com entrevistas com nove deles e quatro fisioterapeutas. Os resultados mostram a preocupação e a incerteza dos professores sobre como servir os alunos com deficiência, a fim de oferecer uma educação de qualidade inclusiva e treinamento de demanda e suporte específico em suas aulas. Professores consideram necessário receber apoio de profissionais, como o fisioterapeuta, para que possam ajudar os alunos com deficiência motora. Sugerimos continuar a examinar as barreiras de ambos os profissionais, suas concepções de professores de EF e o papel do fisioterapeuta, bem como a sua vontade de colaborar entre si


El objetivo del estudio fue determinar y analizar las percepciones del profesorado de educación física que imparte clases al alumnado con discapacidad en las clases inclusivas de Educación Física y cómo mejorar esa inclusión cuando se colabora con el fisioterapeuta. Se obtuvo la información mediante un cuestionario administrado a 56 profesores, que se completó con entrevistas a nueve de ellos y a cuatro fisioterapeutas. Los resultados muestran la preocupación e incertidumbre del profesorado sobre cómo atender al alumnado con discapacidad para poder ofrecer una educación inclusiva de calidad y la demanda de capacitación y apoyos específicos en sus clases. El profesorado considera necesario el apoyo del fisioterapeuta como profesional adecuado para ayudar al alumnado con discapacidad motriz. Sugerimos seguir examinando las barreras de ambos profesionales, sus concepciones de EF y el papel del fisioterapeuta, así como su disposición a colaborar entre ellos

Introducción: La docencia clínica es diferente a la docencia de aula, pues está más estrechamente vinculada al desarrollo del perfil profesional de los estudiantes de la salud y ocurre en espacios de atención al paciente. Por lo mismo, requiere competencias específicas de los tutores clínicos que guían este momento formativo. Sujetos y métodos: Se realizó un estudio cuantitativo donde se encuestó a 70 tutores clínicos de cinesiología. A estos se les aplicó el cuestionario de competencias para tutores clínicos, previo consentimiento informado. Resultados: Los tutores clínicos tienen una percepción positiva de sus competencias pedagógicas, pero son más críticos de sus capacidades para diseñar recursos, evaluar y planificar. Igualmente, estas percepciones mostraron estar asociadas a la capacitación disciplinar, la capacitación pedagógica y la especialidad cinesiológica. Conclusión: Los resultados muestran la importancia del perfeccionamiento docente para el desarrollo de competencias pedagógicas, sobre todo de aquellas no vinculadas a la docencia directa. Igualmente, subrayan el carácter contextualizado de las competencias


Introduction: Clinical teaching is different from classroom teaching, because it is more closely linked to professional development. For this reason, it requires specific competences in clinical preceptors who guide this formative moment. Subjects and methods: A quantitative study was conducted, where 70 clinical preceptors of physiotherapy were surveyed. The Clinical Preceptors Competencies Questionnaire was applied to them, with prior informed consent. Results: Clinical tutors had a positive perception of their pedagogical competencies, but they are more critical of their capacities for resource design, evaluation and planning. Likewise, these perceptions are related to disciplinary training, pedagogical training and physiotherapy specialty. Conclusion: Results show the importance of teaching training for the development of pedagogical competencies, especially those not linked to direct teaching. The context dependent nature of competencies is also underlined

Hand surgery does not have to be expensive. Substituting evidence-based field sterility for main operating room sterility and using wide-awake, local anesthesia, no tourniquet (WALANT) surgery instead of sedation makes hand surgery much more affordable worldwide. This article explains how North Americans collaborated with Ghanaian hand surgeons and therapists to establish more affordable hand care in Kumasi. It describes how multiple nonprofit organizations collaborate to create trans-Atlantic Webinars and a reverse fellowship program to share hand surgery and therapy knowledge between North American and Ghanaian hand care providers.

Introducción: La empatía es un concepto importante en la interacción entre el profesional fisioterapeuta y el paciente, y contribuye a una mejor atención del mismo. La formación empática en estudiantes de fisioterapia debe ser asumida por las universidades desde el primer año de estudio por el carácter complejo de este atributo; sin embargo, cualquier intervención debe estar precedida por un diagnóstico empático exhaustivo. Objetivo: Determinar los niveles de empatía y de sus componentes en estudiantes de fisioterapia según los factores curso y género. Material y métodos: Estudio exploratorio y de corte transversal. Se estudió una muestra de 191 estudiantes. Los datos de empatía fueron recogidos mediante la aplicación de la escala de empatía de Jefferson. Fueron estimadas la mediana, los cuartiles 1 y 3, la diferencia intercuartílica y el intervalo de confianza de la mediana. Se estudiaron las posibles diferencias en la empatía y sus componentes mediante pruebas no paramétricas (mediana de Mood). Resultados: Hubo diferencias en la empatía y en el componente cuidado con compasión en el factor curso. Se manifiesta la declinación empática en el componente cuidado con compasión. La empatía y sus componentes no se diferenciaron en el género. Conclusiones: La diferencia encontrada en la empatía y en el componente cuidado con compasión, la ausencia de diferencias de empatía entre los géneros y la manifestación de declinación empática solo en el componente cuidado con compasión constituyen características específicas en los estudiantes examinados y representan un diagnóstico empático concreto y se constituye en la base para elaborar una estrategia de intervención que debe ser coherente con estos resultados


Introduction: Empathy is an important concept in the interaction between the physiotherapist and the patient, and contributes to a better care of the patient. Empathy training in physiotherapy students must be taught by the universities from the first year of study because of the complex character of this attribute. However, any intervention must be preceded by a comprehensive empathy diagnosis. Objective: To determine the levels of empathy and its components in physiotherapy students according to course and gender factors. Material and methods: Exploratory and cross-sectional study conducted on a sample of 191 students. The empathy data was collected by applying the Jefferson Empathy Scale. The median, 1st and 3rd quartile, interquartile difference and confidence interval of the median were estimated. The possible differences in Empathy and its components were analysed using of non-parametric tests (Mood's median). Results: There were differences in empathy and in the compassionate care component in the course factor. The empathy decline in the compassionate component was observed. Empathy and its components did not differ by gender. Conclusions: The differences found in empathy and in the compassionate care component, the absence of empathy differences in empathy between the genders, and observation of a decline in empathy only in the compassionate care component, were specific characteristics in the students examined. These represent a specific empathy diagnosis, and are the basis for developing an intervention strategy consistent with these results

Homelessness is a national epidemic and individuals experiencing homelessness have decreased access to healthcare services. At the same time, physical therapy (PT) educational programs are required to teach students about cultural competence, pro-bono service, and advocacy for underserved populations. Having PT students provide pro-bono services to this population in a supervised environment is a mutually beneficial scenario for the patients, students, and the PT program. AIMS: The study objective was to evaluate whether participating in a student-run free clinic (SRFC) at a homeless shelter could change PT students' attitudes towards individuals experiencing homelessness. METHODS: Forty DPT students completed the Health Professionals' Attitude Toward the Homeless Inventory (HPATHI) with 19 volunteering (experimental group) to participate in the SRFC. The HPATHI was administered again immediately following participation in the SRFC to assess if their attitudes had changed. RESULTS: Baseline scores differed between the control and experimental group (p=0.03). However, post-scores of the experimental group did not differ compared to pre-scores. The subsequent analysis estimated differences on 4 of 19 questions, specifically within the personal advocacy and social advocacy subscales. CONCLUSIONS: Overall, PT students' attitudes did not change significantly after one session of volunteering at an SRFC in a homeless shelter. However, small changes were seen in personal and social advocacy subscales of the HPATHI.

The evidence-based practice (EBP) model has served as a framework for clinical reasoning since Sackett first defined it in the late 1990s. Since then, this model has been implemented within various clinical healthcare settings by an array of healthcare practitioners including physical therapists. In the case of the latter, it is not clear how practicing physical therapists perceive the usefulness of this model within the clinical setting, nor to what extent they are able to apply it. The current study assessed practicing physical therapists on their perceptions of EBP as well as their general statistical knowledge. Although all participants agreed upon the import of the EBP model in their clinical skillset, not all could identify its common definition (75% provided correct responses), and responded their understanding of research methods were "strong" or "good" (90%), but for statistical methods were less so by comparison (76%). The majority of the respondents could not answer five questions assessing general knowledge in statistical design, with percent frequency of correct responses ranging from 7%, 24%, 0%, 31%, and 31%, respectively. These findings suggest support is warranted for practicing clinicians for appropriate application of the EBP model to occur within the clinical setting.

BACKGROUND: Developing cultural responsiveness among physiotherapists is considered essential to promote quality and equity in healthcare provision for our culturally diverse populations. The aim of this study was to evaluate how entry-level physiotherapy programs in Australia and Aotearoa New Zealand (NZ) design curricula to foster the development of cultural responsiveness in physiotherapy students. Further, the challenges of integrating educational content and approaches, and the perceptions of the effectiveness of these curricula were also explored. METHODS: A cross-sectional telephone survey with closed and open-ended questions, was conducted with 18 participants representing 24 entry-level physiotherapy programs (82% of all programs) in Australia and NZ between May and September 2017. Data were analysed descriptively in the form of frequencies, percentages or ratios as appropriate. Open-ended responses were thematically analysed. RESULTS: Results suggest variability in the structure, and teaching and assessment methods used across all programs. The majority of programs appeared to rely on didactic teaching methods, along with knowledge based and implicit assessment methods. The main challenges reported were that cultural responsiveness was thought to be perceived by academic staff as unimportant and that the curriculum was perceived to be 'overcrowded'. Participants also felt there was room for improvement despite perceiving the curriculum to be effective at fostering cultural responsiveness. CONCLUSION: Results provide insight into the educational content and approaches integrated in entry-level physiotherapy curricula in Australia and NZ, and suggest opportunities for further research and development to foster cultural responsiveness among physiotherapy students.

RESUMO A satisfação é determinada pela reação do paciente ao serviço recebido, sendo, portanto, um indicador sensível da qualidade do atendimento. O objetivo deste estudo foi comparar a satisfação dos usuários que realizam tratamento fisioterapêutico ambulatorial em clínicas públicas (CP), em clínicas privadas de convênio (CC) e em clínica-escola (CE). Foram avaliados 382 pacientes, com idade mínima de 18 anos, que haviam sido submetidos a, no mínimo, cinco atendimentos. Os pacientes foram divididos em três grupos e utilizou-se um questionário com perguntas sobre dados sociodemográficos e satisfação nos domínios interação paciente-terapeuta, acesso e atendimento da recepção, conveniência, ambiente e satisfação geral. A maior parte dos pacientes eram do sexo feminino (68,60%), com média de 51,96 anos de idade. Na comparação entre os serviços, a CE apresentou maior satisfação que a CP em equipe de apoio, conveniência e ambiente físico, e a CC em relação terapeuta-paciente e satisfação geral. A CC foi mais bem avaliada que a CP em conveniência e ambiente físico. A análise de correlação entre a satisfação geral e cada um dos domínios mostrou valores de bom a moderado para relação terapeuta-paciente, e os menores valores para o domínio conveniência. O questionário mostrou boa consistência interna e coerência nos três serviços (α≥0,94). Esses resultados representam um importante indicador da impressão dos usuários nos serviços investigados, permitindo melhor direcionamento na implementação de políticas públicas, privadas e acadêmicas, visando a melhora da qualidade dos atendimentos de fisioterapia.


RESUMEN Determinada por la reacción al servicio recibido, la satisfacción del paciente es un indicador sensible de la calidad de la atención. El objetivo de este estudio fue comparar la satisfacción de los usuarios que realizan tratamiento fisioterapéutico ambulatorio en clínicas públicas (CP), en clínicas privadas de convenio (CC) y en clínica-escuela (CE). Se evaluaron 382 pacientes, con edad mínima de 18 años, sometidos a por lo menos cinco atendimientos. Los pacientes fueron divididos en tres grupos y se utilizó un cuestionario con preguntas sobre datos sociodemográficos y satisfacción en los dominios interacción paciente-terapeuta, acceso y atención de la recepción, conveniencia, ambiente y satisfacción general. La mayoría de los pacientes eran mujeres (68,60%), con edad media de 51,96 años. En la comparación entre los servicios, la CE presentó mayor satisfacción que la CP en equipo de apoyo, conveniencia y ambiente físico, y la CC en la relación terapeuta-paciente y satisfacción general. La CC fue mejor evaluada que la CP en conveniencia y ambiente físico. El análisis de correlación entre la satisfacción general y cada uno de los dominios mostró valores de bueno a moderado para relación terapeuta-paciente, y los menores valores para el dominio conveniencia. El cuestionario mostró buena consistencia interna y coherencia en los tres servicios (α≥0,94). Estos resultados representan un importante indicador de la impresión de los usuarios en los servicios investigados, permitiendo un mejor direccionamiento en la implementación de políticas públicas, privadas y académicas, buscando la mejora de la calidad de las atenciones de fisioterapia.


ABSTRACT Satisfaction is determined by the patient's reaction to the service received, acting as a sensitive indicator of quality in medical care. The goal of this study was to compare the satisfaction of patients receiving outpatient physical therapy treatment in public clinics (PC), private healthcare clinics (PHC) and school clinics (SC). A total of 382 patients aged over 18 years old who had been to at least five appointments were divided into three groups. A questionnaire containing questions about sociodemographic data and overall satisfaction as well as satisfaction with the patient-therapist relationship, access to and support offered by the team, convenience and the environment was used. The majority of the patients was female (68.6%), their mean age being 52.0 years old. In the comparison of the services, the school clinic showed greater satisfaction rates than the public clinics in relation to the support team, convenience and physical environment; and greater satisfaction rates than the private healthcare clinics in relation the therapist-patient relationship and overall satisfaction. The private healthcare clinics were better evaluated than the public clinics in relation to convenience and physical environment. The correlation analysis between overall satisfaction and each variable showed good and moderate values for the therapist-patient relationship variable and the lowest values for the convenience variable. The questionnaire had good internal consistency and coherence for the three services (α≥0.94). These results represent an important indicator of the patients' perception about the services investigated, allowing the proper implementation of public, private and academic policies aimed at the improvement of the quality of physical therapy care.

O objetivo deste estudo foi analisar os efeitos de um programa de exercícios físicos aquáticos na flexibilidade e no alcance funcional de pacientes com Doença de Parkinson (DP). Para avaliar a flexibilidade foi utilizado o Teste de Flexibilidade da bateria AAHPERD (adaptado), enquanto que para verificar o alcance funcional, utilizou-se o Functional Reach Test (FRT). As variáveis foram analisadas pré e pós-intervenção. O programa de intervenção consistiu em 20 atendimentos, duas vezes por semana, 40 minutos de imersão em piscina aquecida a 33ºC, com exercícios aquáticos envolvendo Dupla Tarefa (DT) com uma progressão gradual de complexidade. A análise estatística consistiu no Teste T pareado para o comparativo pré e pós-intervenção. Os resultados encontrados mostraram melhora significativa nas variáveis analisadas no Teste de Flexibilidade da bateria AAHPERD (p<0,0001) e FRT (p<0,001) após a FA. Assim, conclui-se que o programa de exercícios aquáticos proposto foi capaz de promover melhora no alcance funcional e flexibilidade dos indivíduos com DP participantes da pesquisa...(AU)


The aim of this study is to analyze the effects of an aquatic physical exercise program on the flexibility and functional range of patients with Parkinson's disease (PD). To evaluate flexibility, the Flexibility Test of the AAHPERD (adapted) battery was used, while the Functional Reach Test (FRT) was used to verify the functional range. The variables were analyzed before and after intervention. The intervention program consisted of 20 visits, twice a week, 40 minutes of immersion in a heated pool at 33ºC, with aquatic exercises involving dual task (DT) conditions with a gradual progression of complexity. tatistical analysis consisted of paired T-test for the pre and post-intervention comparative. The results showed a significant improvement in the variables analyzed in the Flexibility Test of AAHPERD (p<0.0001) and FRT (p <0.001) after aquatic physical exercise program. Thus, it is concluded that the proposed aquatic exercise program was able to promote improvement in the functional scope and flexibility of individuals with PD participating in the research...(AU)

Negative changes in cardiovascular and autonomic variables in stroke survivors have encouraged the global scientific community to focus on investigating therapeutic strategies to mitigate stroke damage. The objective of the present study was to describe the effects of exercise training on cardiovascular and autonomic variables in stroke survivors. We used the PICO (population, intervention, control/comparison, and outcome variables) model for the search of articles in PubMed and Physiotherapy Evidence Databases from 2009 to December 2018. The following data were also recorded: type of study, author, year of publication, participants (time after stroke, sample size, and age) and benefits of exercise training. A total of 544 articles were initially selected, of which nine peer-reviewed articles met the search criteria. These nine studies enrolled 611 participants (middle-aged or elderly), and pointed to positive effects of training on maximal oxygen uptake, peak aerobic capacity, 6-minute walk test and resting heart rate. However, more well-controlled studies are needed to confirm the benefits of exercise training on cardiovascular and autonomic variables in this population

Para promover a qualidade de vida aos pacientes atendidos em um programa de reabilitação, há a necessidade e obrigação de humanizar o atendimento. O fisioterapeuta exerce um papel fundamental na reabilitação dos pacientes com sequelas ortopédicas, e, além de qualidade técnica, deve prezar pela qualidade relacional de sua assistência. avaliar a relação terapeuta paciente, investigando se há humanização do atendimento fisioterapêutico na especialidade de ortopedia. Estudo de corte transversal, no qual foi aplicado um questionário adaptado avaliativo individual, contendo 15 questões fechadas em nove domínios, em uma amostra de 59 indivíduos, os quais receberam atendimento na especialidade de fisioterapia ortopédica. Os dados foram analisados e apresentados em frequência e porcentagem. O atendimento fisioterapêutico foi caracterizado como humanizado, obtendo-se resposta com 100% em todos os domínios do questionário, nos quesitos que dizem respeito à relação terapeuta-paciente. A assistência fisioterápica, na especialidade de ortopedia, foi caracterizada como humanizada pelos pacientes.


Care should be humanized to promote the life quality of patients attended in a rehabilitation program. The physiotherapist has an important role in the rehabilitation of patients with orthopedic sequels. Besides technical quality, physiotherapists should appreciate the quality of their relationship. To evaluate the therapist-patient relationship by investigating whether humanization exists in physiotherapist attendance, especially in orthopedics. Transversal research applied a questionnaire adapted for individual evaluation, with 15 questions within nine dominions, in a sample of 59 agents receiving care in orthopedic physiotherapy. Data were analyzed, with frequency and percentages. Physiotherapeutic attendance was characterized as humanized, with one hundred percent response in all the domains of the questionnaire with regard to therapist-patient relationship. Physiotherapy, especially orthopedics, was characterized as humanized by patients.

Introduction: The temporomandibular joint (TMJ) acts during chew, swallowing, yawning, conversation and in activities involving jaw movements. Temporomandibular Dysfunction (TMD) is a set of disorders involving the masticatory muscles in the TMJ and associated structures. Joint mobilization aims to relief pain and restoring the function of the compromised body segment, increasing its degree of mobility. Objective: We aimed to evaluate the influence of oscillatory joint mobilization on TMD. Method: A descriptive case study with a patient who presented a clinical diagnosis of TMD in the Clinical School of Physiotherapy of the Leão Sampaio School, Juazeiro do Norte, CE, Brazil, from September 3 to October 6 of 2014, twice a week, lasting 40 minutes, a total of 10 visits. Results: The patient presented a gain in the range of motion of the mouth opening (from 17 mm to 46 mm), pain relief (from grade 6 to grade 0 on the Visual Analogue Scale) and improvement in muscle function. Conclusion: Oscillatory joint mobilization in TMD may be effective in increasing mandibular range of motion and opening of the mouth, as well as in reducing TMJ pain and masticatory musculature.


Introdução: A articulação temporomandibular (ATM) atua durante a mordida, a deglutição, o bocejo, a conversação e as atividades envolvendo movimentos do maxilar. Disfunção temporomandibular (DTM) é um conjunto de distúrbios envolvendo os músculos mastigatórios na ATM e estruturas associadas. A mobilização conjunta visa a dor de alívio e a restauração da função do segmento comprometido do corpo, aumentando seu grau de mobilidade. Objetivo: Pretendemos avaliar a influência da mobilização articular oscilatória na DTM. Método: Trata-se de um estudo de caso com paciente que compareceu para diagnóstico de DTM na Clínica Escola de Fisioterapia da UNILEÃO, Juazeiro do Norte, CE, Brasil, de 3 de setembro a 6 de outubro de 2014, duas vezes por semana, com duração de 40 minutos, totalizando 10 visitas. Resultados: O paciente apresentou um ganho no intervalo de movimento da abertura da boca (de 17 mm a 46 mm), alívio da dor (do grau 6 ao grau 0 na Escala Visual Analógica) e melhora na função muscular. Conclusão: A mobilização das articulações oscilatórias na DTM pode ser eficaz no aumento da amplitude de movimento mandibular e na abertura da boca, bem como na redução da dor da ATM e musculatura mastigatória.
