Abstract: This work is based on psychoanalysis and historical-dialectical materialism, according to which human beings are constituted by a historical and social process. Based on Lacan's appropriation of Marx's work, we argue that the unconscious is a historical and political instance, constituted in interaction with the context in which the subject is inserted. We then propose that sexual division of labor is inscribed in the unconscious and gender identity is constituted based on this symbolic content. The awareness of this unequal division is hampered by an ideological fantasy.

Abstract: This article proposes an analogy between poetic function and psychoanalytic interpretation. Departing from the Lacanian postulation known as lalangue, we intend a parallel between this concept and the use of language as it is performed in concretist poetry. Through the literary analysis of two Brazilian poems of this school, Chico Buarque's Construction and Arnaldo Antunes's The pulse, we suggest the understanding of Lacan's assertion ("the unconscious is a savoir-faire with lalangue") as a consideration of the poetic aspect of the interpretative technique in psychoanalysis.

BACKGROUND: Nursing practice is a key driver of quality care and can influence newborn health outcomes where nurses are the primary care givers to this highly dependent group. However, in sub-Saharan Africa, nursing work environments are characterized by heavy workloads, insufficient staffing and regular medical emergencies, which compromise the ability of nurses to provide quality care. Task shifting has been promoted as one strategy for making efficient use of human resources and addressing these issues. AIMS AND OBJECTIVES: We aimed to understand the nature and practice of neonatal nursing in public hospitals in Nairobi so as to determine what prospect there might be for relieving pressure by shifting nurses' work to others. METHODS: This paper is based on an 18-month qualitative study of three newborn units of three public hospitals-all located in Nairobi county-using an ethnographic approach. We draw upon a mix of 32 interviews, over 250 h' observations, field notes and informal conversations. Data were collected from senior nursing experts in newborn nursing, neonatal nurse in-charges, neonatal nurses, nursing students and support staff. RESULTS: To cope with difficult work conditions characterized by resource challenges and competing priorities, nurses have developed a ritualized schedule and a form of 'subconscious triage'. Informal, organic task shifting was already taking place whereby particular nursing tasks were delegated to students, mothers and support staff, often without any structured supervision. Despite this practice, nurses were agnostic about formal institutionalization of task shifting due to concerns around professional boundaries and the practicality of integrating a new cadre into an already stressed health system. CONCLUSION: Our findings revealed a routine template of neonatal nursing work which nurses used to control unpredictability. We found that this model of nursing encouraged delegation of less technical tasks to subordinates, parents and other staff through the process of 'subconscious triage'. The rich insights we gained from this organic form of task shifting can inform more formal task-shifting projects as they seek to identify tasks most easily delegated, and how best to support and work with busy nurses.

The nurse leader is ethically responsible for creating diverse and inclusive spaces for both nurses and those we care for. The leader must openly acknowledge and address the negative influence of bias and prejudice within health care organizations. This requires both an awareness of the influence of unconscious bias within the organization and appropriate actions to mitigate its impact. This process begins with an in-depth examination of one's own biases and continues through actions at the individual and organizational levels. Nurse leaders are well positioned to address and mitigate the negative influence of bias within organizations. This article discusses the influence of unconscious bias as well as strategies that the nurse leader can use to build diverse and inclusive organizations.

Recent findings suggest that priming may be useful for facilitating disclosure in investigative interviews; however, the effects of priming on behavioral outcomes have been mixed. The current studies attempted to replicate the increase in information disclosure when the concept of "openness" is primed. We assessed the separate and combined influence of context reinstatement instructions and activation of the concept of openness (via lexical primes in Experiment 1, via contextual and embodiment primes in Experiment 2) on information disclosure. In doing so, we introduced a novel paradigm to investigate factors contributing to the elicitation of sensitive personal information that participants provided in written (Experiment 1) or verbal (Experiment 2) form. Participants (Experiment 1: N = 173; Experiment 2: N = 194) completed a checklist of illegal behaviors and misdeeds, then engaged in an unrelated task that was used to administer the priming manipulation (either the concept of "open" or "closed," or a neutral prime). Participants then described a life event related to the most serious illegal behavior to which they had admitted, following either a direct request for information or a context reinstatement instruction. Across both experiments, context reinstatement led to robust increases in information disclosure. Although we failed to replicate prior effects of priming on disclosure, our observed effect sizes fell within the confidence intervals of previous studies. A meta-analytic assessment of priming across the two studies suggested a small but significant increase in information elicitation, suggesting that investigators are best served using evidence-based interviewing tactics during investigative interviews. This paper explores the relation of concepts of the unconscious to notions of the imagination, and both to the dynamics of shame. In this discussion dreams occupy a central place, since they are so intimately related to human relationships and to the human imagination. What is seen, not seen, concealed, relied upon for others not to understand, and what is imagined in the responses of others and of oneself-these are essentially shame dynamics, since our identity is determined by relationships.

Abstract This paper investigates the child's relationship with knowledge. Clinical practice and analytical research with children with difficulties to access school knowledge revealed the unconscious dynamics in which knowledge occupies a place in the family symptom. A theoretical discussion will be presented considering that access to knowledge bears a structural dimension of impediment, which is the impossibility of knowing in the face of castration. Therefore, we will investigate two paths to discuss what prevents access to knowledge. The first addresses the relationship between demand and desire and how it affects children. The second discusses the relationship between knowledge and truth, exploring the relationships and boundaries between both, adding to this discussion a third essential term: jouissance.

This essay challenges the most basic tenet of Jung's analytical psychology, namely, the existence of the collective unconscious. Despite the fact that there are purported to be universal processes and ontological features of mind throughout all psychoanalytical schools of thought, Jung's is unique in the history of psychoanalytic ideas for positing a supraordinate, autonomous transpersonal psyche that remains the source, ground, and wellspring from which all unconscious and conscious manifestations derive. This bold claim is analyzed through a close inspection of Jung's texts that questions the philosophical justification for postulating a supernatural macroanthropos or reified collective mind. Pointing out the problems of agency and fallacies of hypostatization, it is not necessary to evoke a transpersonal cosmogony to explain how universality suffuses individual subjectivity within social collectives. Here we may conclude that the collective unconscious construct is a signifier for the common psychological dynamics and characteristics of shared humanity. In this sense, the myth of the collective unconscious is better understood as a metaphor for a higher abstraction or ideal principle ordained with numinous value.

As fixações pulsionais comparecem, muitas vezes, livres do ocultamento fantasmático no inconsciente.

The contemporary clinic confronts us with subjects subject to massive impositions of jouissance. Drive fixations often appear free from the phantasmatic occultation in the unconscious. A kind of bond prevails with the Other that goes back to the more archaic forms of relation of the self to the object. Faced with this scenario, we ask what the status of the phantasm would be today. Our guiding principle will be the hypothesis of a debasement of the symbolic efficacy of paternal authority in containing drive's egoism, which manifests itself in the radicalization of the calls to unlimited jouissance. The phantasmatic grammar is a symbolic-imaginary screen that allows for interpreting the libidinal position of each one when faced the real of the desire of the Other. The disavowal of the paternal function as an agent of castration produces a voracious subject, whose weakness of self results in psychotic alterations of character.(AU)

In this paper the author asks, "How long is the life of an intersubjective field?" She proposes that it is possible for the field to remain active and instructive even after formal sessions have ended: This occurs in her case of Carla, a young woman who terminates prematurely. Carla enters treatment in a downward spiral of severe trauma symptoms that began subsequent to her rape, a decade earlier. Although Carla's symptoms diminish and the analysis continues to be productive, it suddenly ends in an impasse, leaving the analyst perplexed and feeling professionally insufficient. Months later, she has three dreams pertaining to Carla and her rape. Largely employing Jessica Benjamin's recognition theory and her representation of the intersubjective third, as well as contemporary Bionian thinking, the paper depicts how countertransference dreaming is one example of how the intersubjective field can carry on the psychoanalytic function-even outside of formal treatment.

RATIONALE: Life-threatening anaphylactic shock is a rare (1 in 1 million) but documented occurrence in response to subcutaneous immunotherapy. Immediate administration of Epinephrine (Epi) is critical to save lives in these situations. The current protocol for systemic reactions in immunotherapy is for the prescribing physician to reassess the dosing and schedule as well as the risk:benefit assessment for the therapy and determine whether or not to proceed. PATIENT CONCERNS: The patient revealed concerns regarding the neurologic sequela sustained after undergoinig life-threatening anaphylactic shock. DIAGNOSIS: The patient was diagnosed with anaphylactic shock and treated appropriately. INTERVENTIONS: The patient experienced shortness of breath and was promptly administered 2 shots of 0.3mg Epi followed by a loss of consciousness (LOC) and a series of 4 consecutive seizures accompanied with LOC and urinary incontinence. Seizures as a manifestation of anaphylaxis are rare with 1 study claiming 13% of cases of anaphylaxis having LOC and only 1.5% cases with loss of bladder or bowel control. OUTCOMES: This case is one of continued subcutaneous immunotherapy after the patient had an initial systemic reaction suspicious for anaphylaxis 6 months before the life-threatening anaphylaxis, both induced by immunotherapy. In both instances, there was a significant amount of neurologic involvement. Neurologic sequela included a transient tremor and permanent deficits in vision, fine motor coordination evidenced by a change in handwriting. LESSONS: The current protocol was followed in this patient but still ended up almost ending her life. This protocol seems to be inadequate with regards to potential fatality. Even though a very small number, some patients face life-threatening adverse effects after apparently very low-risk immunotherapies. Therefore, reevaluating the treatment protocol with addition of a longer post-shot observation step and discontinuing treatment in the case of adverse events may help minimize the overall risk of any fatal outcome.

Psychoanalysis will be approached, as a practical and clinical thinking, as the conquest of immanence. In this context, the notion of drive gains prominence, demanding in return its development to be magnified. As a practical concept, understood as knowledge in act, the drive designates an exercise of ethical consistency. A review of the lacanian versions of the unconscious ethical status should situate properly the drive vector. Described by elements of immanence - the strength, the "live" and the "active" - such vector is the "real" in psychoanalysis. The language in this field presents the appearance of an indigenous language.

Continuous flash suppression (CFS) has become a popular tool for studying unconscious processing, but the level at which unconscious processing of visual stimuli occurs under CFS is not clear. Response priming is a robust and well-understood phenomenon, in which the prime stimulus facilitates overt responses to targets if the prime and target are associated with the same response. We used CFS to study unconscious response priming of shape: arrows with left or right orientation served as primes and targets. The prime was presented near the limen of consciousness and each trial was followed by subjective rating of visibility and a forced-choice response concerning the orientation of the prime in counterbalanced order. In trials without any reported awareness of the presence of the prime, discrimination of the prime's orientation was at chance level. However, priming was elicited in such unconscious trials. Unconscious priming was not influenced by the prime-target onset-asynchrony (SOA)/prime duration, whereas conscious processing, as indicated by the enhanced discriminability of the prime's orientation and conscious priming, increased at the longest SOAs/prime durations. These results show that conscious and unconscious processes can be dissociated with CFS and that CFS-masking does not completely suppress unconscious visual processing of shape.

This paper discusses how we interpret and experience time and how such experiences affect our understanding of the topographical as well as the structural models. Following the thought of both ancient Hindu teachings (the Upanishads) and contemporary findings from quantum mechanics, the paper frames the discussion within a unitary experience of both mind and everyday experience. The function and role of clinical interpretations are also discussed. Following the tradition articulated in Roy Schaffer's action language model as well as insights from existentialism, the paper offers a deeper appreciation of individual agency and its role in self-understanding and personal growth.

This article develops several foundations of the Jungian psychology from the direct elaborations of its creator the Swiss psychologist Carl Gustav Jung, perspective also called, by many psychologists of this theory, like complex psychology. At the same time, some key ideas regarding Edgar Morin's complex thinking are presented, whose sustenance is derived from the complexity paradigm. In this exercise are found guidelines that could provide an epistemological support to some Jungian concepts, proposing, in turn, another look at this psychological current. Thus, as Jung proposed, it is understood that the recognition of the unconscious depths is as necessary for the subject as the understanding of his conscious and the dynamics between both. Recalling that, according to the complex thinking, it is in the concurrence, from where a more real approach towards the understanding of the systems is possible. It is understood then that the unconscious should not be explored exclusively in relation to that which afflicts the subject, as a repetition (circular), but also as to its creative facet unconscious that with the help of consciousness, it becomes recursive (spiral) and source of healing.

(AU)

In the present work, we analyze the narratives centered on the revelation of sexual identity as retold by an individual who says he´s "gay" in order to present his preconscious thoughts as well as the wishes and defenses that stand out in these narrations. In the results, the predominant languages detected are the urethral phallic and the secondary oral. The initial scenes are staged in his preadolescent stage. One of the recollections is central in that it makes it possible to demonstrate the correlation between the sexual arousal process and the constitution of the preconscious. In the study of defenses, the failure of repression stands out as well as how it takes on a pathogenic nuance, sustained in secondary defenses, such as disavowal and conversion to the contrary. To conclude, the results are articulated with the theory, to show how the children's narrations are present insofar as unconscious phantasies are not processed. (AU)
