RESUMO

BACKGROUND: Painful diabetic peripheral neuropathy (PDPN) has a great impact on an individual's quality of life. The current researchers' previous trial suggested that acupuncture was a promising adjunctive treatment for PDPN. However, the underlying mechanism of action of acupuncture treatment for PDPN is still unclear, especially its effects at the cerebral level. The aim of this trial will be to explore how acupuncture works in treating PDPN by using multimodal cerebral imaging. METHOD AND DESIGN: This will be a randomized controlled trial. A total of 150 participants will be recruited and assigned to one of three groups: the healthy group, the DM without PDPN group and the DM with PDPN group. Participants in the DM without PDPN and the DM with PDPN groups will each be further divided between an electroacupuncture group and a sham electroacupuncture group. Participants will receive six treatment sessions per week for 4 weeks. Multimodal cerebral imaging includes resting-state functional magnetic resonance imaging (rs-fMRI), diffusion tensor imaging (DTI) and magnetic resonance spectroscopy (MRS); this neurophysiological testing will be the primary outcome measure. Subjective pain scales and blood analysis will be a secondary outcome measure and will be used to assess the clinical efficacy of the intervention. Multimodal cerebral imaging will be used to detect cerebral activity changes in each treatment group. The clinical data and fMRI data will be analysed for all the groups. Multiple correlation regression analyses will be used to assess the association between changes in cerebral functional activity and the improvement of clinical outcomes after acupuncture treatment. DISCUSSION: This study is based on the results of the researchers' previous study, and using combined clinical and cerebral function changes, it will help evaluate the effects of acupuncture on PDPN. The investigation of acupuncture's central mechanism of action will further expand the understanding of acupuncture treatment of PDPN. RESUMO

The elements of ethical review related to clinical research of acupuncture and moxibustion is discussed to provide ideas for various institutions to carry out relevant ethical review. It is believed that the ethical review of clinical research of acupuncture and moxibustion needs to focus on the specificity of acupuncture and moxibustion. Starting from the basic theory of traditional Chinese medicine, the theory of meridians and acupoints and the theory of syndrome differentiation along meridians, the key contents of ethical review such as intervention methods, grouping design and placebo control should be considered, so as to standardize the clinical research of acupuncture and moxibustion and protect the health and rights and interests of participants.

RESUMO

The Autonomous Sensory Meridian Response (ASMR) is a tingling sensation across the scalp that occur in response to specific triggering audio and visual stimuli, connected with the Default Mode Network. Our study (N = 76) aimed to test the neurophysiology of ASMR by examining pupil diameter and brain activity. Assuming the idiosyncratic nature of ASMR, we expected results detecting opposite physiological outcomes considering pupil diameter and brain activation. We used a battery of self-reports to investigate psychological dimensions; for the physiological measures, we used two instruments: PupilCore and NeuroSky MindWave Mobile 2. The results showed an augmented pupillary diameter during the ASMR video, regardless of the perception of tingles. On the other hand, the arousal level during the ASMR video was lower than the other conditions. The difference between the two neurophysiological measures appeared as peculiar and can be considered as the promoting phenomenon for ASMR psychological outcomes.

This paper reviewes the clinical and scientific research history of professor XUE Chong-cheng and summarizes his academic thoughts and main achievements. Professor XUE Chong -cheng created the first personality and constitution inventory in China to lay the foundation for the discipline of TCM psychology; discovered meridian sensory conduction in phantom limbs and proposed the extension of the central model; invented a method for treating mental illness with a low-dose electroacupuncture convulsion therapy instrument; and interpreted the TCM medical model as a Time-Space-Social- Psychological-Biological medical model.

Professor YIN Ke-jing's experience of Jingluo Bietong acupuncture technique for shoulder arthralgia is summarized. According to the location of pain, shoulder arthralgia is divided into hand-taiyin type, hand-yangming type, hand-shaoyang type, hand-taiyang type and mixed type. The acupoint selection is based on Jingluo Bietong acupuncture technique, and Shugu (BL 65) or Feiyang (BL 58) are selected for hand-taiyin type, Taichong (LR 3) for hand-yangming type, Taixi (KI 3) and Dazhong (KI 4) for hand-shaoyang type, and Taibai (SP 3) or Diji (SP 8) for hand-taiyang type. In addition, combined with acupuncture at the blocking points of meridians and Xiangu (ST 43), better efficacy could be obtained.

XIAO Shao-qing's understanding and evolution of acupoint compatibility methods and rules are summarized. XIAO Shao-qing's understanding of acupoint compatibility rules shows a process from simple listing to systematic improvement, which is reflected as the increase or decrease of the number of acupoint compatibility methods, the revision of the concept and connotation of acupoint compatibility methods, and the adjustment of the example content of acupoint compatibility rules. His clinical prescriptions of acupuncture and moxibustion highlight the academic idea of taking the indications of meridians and acupoints as the basis, using syndrome differentiation and treatment as the core, and selecting acupoints for symptoms as the auxiliary.

The paper summarizes the ideas and characteristics of acupoint selection in treatment of mental disorder with acupuncture and moxibustion by professor FU Wen-bin. Professor FU believes that mental disorder is manifested as the simultaneous illness of "heart" and "body". On the base of "holism" for the unity of body and mind, professor FU proposes the treating idea as "soothing the liver, regulating the mind and focusing on the heart and the gallbladder in treatment". In clinical practice, the acupoint prescription on specifically regulating and harmonizing yin and yang is composed to calm down the mind. The eight confluent points and back-shu points of five zang organs are selected to regulate zangfu functions. The "ghost" points, the points on the second line of bladder meridian and the acupoints with specific effect are used to tranquilize the mind, open the orifices and consolidate therapeutic effect. Meanwhile, the complicated symptoms are also considered in treatment. The integrative and holistic pattern of acupuncture and moxibustion therapy as "acupuncture at the top priority, followed by moxibustion, and consolidation at the end" achieves the co-regulation of qi and blood and tranquilization for both the mental and the physical.

The discovery of modern evolutionary anatomy shows that the persistent median artery in the upper arm is a common variant with an increasing trend. This phenomenon can explain well the transition from the eleven meridians described in the Han silk and bamboo slips to the twelve meridians finalized in Neijing and the addition of the hand- jueyin meridian in Han dynasty. After systematic analysis and comparison, the author proposes the hypothesis that "yin meridians are arteries, and yang meridians are nerve-like structures" to explain the meridian theory prior to acupuncture and acupoints in the Qin and Han dynasties. It is clear that over two thousand years ago, Chinese physicians already described the distribution, physiology, and pathology of the six main arteries and six important nerves in the human extremities. The finding suggests that it was the addition of acupuncture and acupoints that had changed the direction of TCM development in Neijing or later and promoted the maturation of qi and blood, meridians, and zangfu theories.

Based on literature research and Delphi expert consensus method, the important acupoints for cancer pain was summarized to provide evidence basis for the formulation of Clinical Practice Guide of Acupuncture in the Treatment of Cancer Pain. Through systematic search of Chinese and English databases, 28 clinical studies regarding acupuncture for cancer pain were included. The acupoint selection methods and high-frequency acupoints were summarized and analyzed. Based on this, a Delphi questionnaire was designed and two rounds of questionnaire survey on 30 experts in acupuncture and tumor related fields in China and abroad were conducted. As a result, it was suggested that the individualized acupoint selection should be adopted for acupuncture treatment of cancer pain, with Zusanli (ST 36), Hegu (LI 4), Taichong (LR 3), Sanyinjiao (SP 6), Yanglingquan (GB 34) and ashi points as the main acupoints. Combined with clinical research evidence and expert consensus, the important acupoints for cancer pain were identified. However, clinical acupoint selection still needed further research and refinement.

OBJECTIVE: To analyze the rules of acupoint and medication selection of acupoint application therapy for functional constipation (FC) by data mining technology. METHODS: The clinical research literature regarding acupoint application therapy for FC from published to February 26, 2020 was searched in CNKI, VIP, Wanfang, SinoMed and PubMed. The prescriptions were extracted, and by using SPSS24.0 and SPSS Modeler14.0 software, the use of high-frequency acupoints and medication was summarized. The association rule analysis, cluster analysis and core prescription analysis of acupoints and medication were analyzed. RESULTS: A total of 122 prescriptions of acupoint application therapy were included, involving 32 acupoints. The core prescription of acupoints was Tianshu (ST 25), Dachangshu (BL 25), Shenque (CV 8) and Guanyuan (CV 4). The high-frequency meridians mainly included conception vessel, yangming stomach meridian of foot and taiyang bladder meridian of foot. The core prescription of medication was rheum officinale, mirabilite, immature bitter orange, mangnolia officinalis, common aucklandia root and borneol. CONCLUSION: The use of local acupoint and regulating-qi and purgating medication is an important principle of acupoint application therapy for FC.

Through analyzing the indication distribution of the different acupoints located at the upper limbs recorded in Science of Acupoints and Science of Meridians and Acupoints, the industry planning teaching materials of traditional Chinese medicine, it is discovered that the acupoints located at the upper arms are commonly selected for the treatment of scrofula and goiter, while the acupoints below the elbow at the hand meridians and those at the lower limbs of the foot meridians which running through the neck, do not have the similar indications. Based on a further analysis on the literature at ancient and modern times, it is believed that the acupoints located on the lateral side of the upper arms, especially those at the large intestine meridian of hand-yangming perhaps have the specific effect in treatment of scrofula and goiter.

OBJECTIVE: To develop a self deep breathing training device which can improve lung function compliance and blood oxygen saturation. METHODS: The device consists of four parts:flow tube, measuring cylinder, mobile phone holder and meridian guidance audio-visual synthesis training software. The flow tube measures the flow rate of inhaled gas, the metering cylinder measures the total amount of inhaled gas, and the mobile phone rack is equipped with a mobile phone storing the meridian guidance audio-visual synthesis training software. RESULTS: The device is reasonable in structure and flexible in operation, which can meet the requirements of self deep inspiration training under the guidance of training module. CONCLUSIONS: Deep inspiration training under the guidance of guidance training module can form "deep and slow" abdominal breathing, and then improve lung function.

BACKGROUND: This study aimed to compare surgically induced astigmatism (SIA) on the anterior and total cornea during cataract surgery through a 2.2 mm steep meridian incision. METHODS: The study included 69 left eyes of 69 patients who had undergone cataract surgery. The 69 eyes were classified into three subgroups according to the preoperative steep meridian. Following phacoemulsification, an intraocular lens was inserted into the bag. The keratometric measurements were taken 12 months postoperatively, on the anterior cornea (automated keratometer and anterior keratometry [K] from a rotating Scheimpflug camera) and total cornea (equivalent K reading [EKR] 3.0 mm, EKR 4.5 mm, total corneal refractive power (TCRP) 2.0 mm ring, TCRP 3.0 mm zone, TCRP 4.0 mm zone). The SIA was analyzed for each parameter. RESULTS: On the double-angle polar plot, the summated vector mean values of SIA determined by the automated keratometer and Scheimpflug anterior K were 0.28 diopter (axis: 177°) and 0.37 diopter (axis: 175°) in with-the-rule (WTR) astigmatism; 0.03 diopter (axis: 156°) and 0.18 diopter (axis: 177°) in oblique astigmatism; 0.15 diopter (axis: 96°) and 0.17 diopter (axis: 73°) in against-the-rule (ATR) astigmatism. The mean SIAs on the total cornea ranged from 0.31 to 0.42 diopter in WTR astigmatism; from 0.16 to 0.27 diopter in oblique astigmatism; from 0.04 to 0.11 diopter in ATR astigmatism. Mean magnitude SIA ranged from 0.41 to 0.46 diopter on anterior corneal surface and 0.50 to 0.62 diopter on total cornea. J0 and J45 of the posterior cornea showed no significant changes after cataract surgery, and the changes in J0 and J45 did not show any statistical differences between the anterior and total cornea (all p > 0.05). CONCLUSIONS: There were no differences in the summed vector mean values of SIA between the anterior cornea and the total cornea.

RESUMO

The "meridian theory" is the summary and sublimation of long-term medical practice of the Chinese ancient sages. Via reduction and reconstruction about the theoretical system, it has been put forward a doctrine of "binary structure" about the meridian and acupoints which, at the limbs, present a longitudinal arrangement, and those at the head-face and trunk present a horizontal distribution, being closely related to the "dematome" of the body. Centered on the ontogenesis of the embryonic "somite", a closely connected "structure-function unit" complex is formed with the development of the dermis-muscle-ogran-nerve within the homo-segment. This form constitutes the basic characteristics of pathological reaction rules and therapeutic effects of meridian acupoints.

In this paper, the articles of auricular acupuncture in recent 20 years are sorted out and analyzed, and the theory and mechanism of auricular acupuncture in different schools are summarized and compared. Eight theories are included totally, i.e. meridian-collateral theory and zangfu theory of classic TCM, as well as neurodoctrine, neurohumoral theory, embryology theory, holography theory, biological cybernetics and chronobiology theory in modern times. Each of them has its own rationality.Hereafter, through more rigour scientific deign and in accordance with clinical and experimental studies, auricular acupuncture theory should be explored more scientifically and universally so as to provide the theoretic guidance and evidences for clinical treatment with auricular acupuncture, and then to improve the clinical therapeutic effect and promote the application of auricular acupuncture.

The schematic diagram is an indispensable part of research article in life science,which can effectively and intuitively show the specific content of the article with simple composition. As an intuitive expression of the interdisciplinary subject at the early stage, the schematic diagram in the literature has gained an all-round improvement in the quality along with the deve-lopment of computer technology and modern drawing tools. In contrast,the level of the schematic diagram appears to be lagged behind in the field of acupuncture research papers. In order to improve the drawing level of this field, we took the Five Shu-points of Hand and Foot Jueyin Meridian as an example, and drew some vector diagrams of their anatomical structures including the body surface, skeleton, nerves, arteries and veins by using the Adobe Illustrator image software, through which we sum up a simple and easy-to-learn process including application skills and key points needing attention. We hope these methods can play a role in the acupuncture research in the future.

A newly-developed "Mouse Forelimb Fixator" and two types of "Batch Mice Moxibustion Device" on the basis of our "Mouse Safe and Fast Fixation Board" (developed in 2012) were introduced in the present paper. The Forelimb Fixator inserted into the base part of the apparatus in tenon and mortise style is used to control the mouse's posture with the forelimbs' acupoints fully exposed, and can realize simultaneous fixation of several mice at the same time. By using the mobility of the base of the single-hole moxibustion frame and the magnet, the distance between the acupoint surface and the tip of the ignited moxa stick can be accurately controlled, and several acupoints of different meridians can be simultaneously stimulated at the same time. Utilizing the porous transparent moxibustion board, the Batch Mice Moxibustion Device can meet the requirement of moxibustion at multiple acupoints at the same time. In addition, these devices are convenient in operation, innovative in creativity, save manpower and material resources, and help improve experimental efficiency and research on moxibustion.

OBJECTIVE: To explore the ultrasonic anatomical characteristics and needle-knife insertion approach of common tendon lesions in knee osteoarthritis (KOA), so as to provide the references for accurate release of KOA by needle- knife along tendon lesions based on meridian sinew theory. METHODS: Sixty patients with one-knee KOA were selected. High-frequency musculoskeletal ultrasound was used to collect sonograms at the anatomical positions of "Hedingci" "Binwaixia" "Binneixia" and "Yinlingshang". The anatomic levels were marked on the sonograms. The anatomic levels and sonographic features of lesions were compared and analyzed, and the relevant data of needle-knife simulation approach was measured. RESULTS: The "Hedingci" lesions were mainly located at the attachment of quadriceps tendon to patella and suprapatellar bursa. The "Binwaixia" and "Binneixia" lesions were mainly located at the attachment of retinaculum patellae laterale and retinaculum patellae mediale to patella and infrapatellar fat pad. The "Yinlingshang" lesions were mainly located at the attachment of goose foot tendon to medial tibial condyle and bursa of goose foot. With "Hedingci" as an example, when the needle-knife entry point was 1 cm above the patella, the attachment of quadriceps tendon to patella was released, and the average depth of needle-knife was (3.60±0.10) cm, and the needle body was perpendicular to the skin. The average depth of needle-knife for releasing suprapatellar bursa was (2.35±0.17) cm, and the needle body was 45° towards head. CONCLUSION: The musculoskeletal ultrasound could clearly show the local detailed anatomical level, ultrasonic characteristics and anatomical level of common tendon lesions of KOA, and could improve the accuracy of needle-knife along tendon lesions with non-direct vision, which has important reference value for needle-knife medical standardization and standardized operation.

To summarize professor QIU Xian-ling's clinical experience of acupuncture for treating chronic prostatitis. Professor QIU Xian-ling believes that the causes of chronic prostatitis are complex and diverse, and the main pathogenesis is kidney deficiency with excess. Professor QIU follows the traditional principles of syndrome differentiation and meridian differentiation, and adopts the combination of replenishing and reducing treatment and regulating the spirit and calming the mind. Guanyuan (CV 4), Huiyin (CV 1), Ciliao (BL 32) and Zhibian (BL 54) are the main acupoints that must be selected for treatment. During clinical treatment, according to the selected acupoints or the specific condition, special acupuncture methods such as deep needling with elongated needle method, lifting acupuncture method, three-in-one-retraction method, penetrating needling method and acupuncture combined with pressing method are used to achieve the purpose of comprehensive improvement of the local and overall state.

By collecting and analyzing the explanation/conception, acupoint name, acupoint location, indications, acupuncture and moxibustion techniques and contraindications of he-sea point recorded in Neijing (Inner Canon of Yellow Emperor) and Nanjing (Yellow Emperor 's Classic of Eighty-one Difficult Issues), it is found that conception of he-sea point of five-shu points has been specifically formed, the names of he-sea points of eleven meridians, the locations and the ways to locate them were recorded. But, the summaries relevant to the indications of the many he-sea points have not been included in teaching materials, thus, it needs to be further analyzed. The acupuncture-moxibustion techniques at he-sea point are flexible and the attention should be paid to distinguish "needling he-sea point in autumn" from "needling he-sea point in winter".
