RESUMO

Actually, in Russian health care, the organization of community-based medical care of patients with mental disorders is considered as one of requested problems to be resolved. The in-patient substituting forms of specialized primary medical and social care for mental patients create optimal conditions for treatment and rehabilitation of this group of patients without separation from familiar social environment, that in many ways reduces "personality stigmatization" and promotes returning of individual to full-fledged social life, decreasing the load on the most expensive hospital care. One of such organizational forms implemented in Moscow under the new model of provision of mental care is out-patient mental module that provides specialized primary medical and social care and is tested in conditions of basic medical organization. Hence, the purpose of the study is to develop system of indices for continuous information support of management of implemented innovations.

Students with preexisting mental health conditions or disabilities may fair worse due to virus mitigation strategies during the COVID-19 pandemic. This study was conducted to understand the experiences of students with preexisting mental health conditions or disabilities at a public university during COVID-19. We examined disruptions in the management of preexisting mental health conditions or disabilities and its impact on psychological well-being. Students were surveyed about their health care experiences during the COVID-19 pandemic between June and September 2020. Linear regressions and mediation analyses were conducted to examine the relationships between disruption to care, mental health self-efficacy, and four psychological well-being outcomes (stress, anxiety, depression, and overall distress). Of the total (N = 1,082) study participants, 258 (24%) reported having a preexisting mental health condition(s) or disabilities (81% female; Mage = 23.47). Of those, 155 (61%) reported that COVID-19 disrupted health care delivery and management of their conditions or disabilities. Of those who reported this disruption, 51% (n = 109) of participants reported a disruption in their ability to see a health care professional and 58% (n = 69) reported either that they lost care or that the quality of the new telemedicine care was not sufficient. A series of linear regressions revealed significant relationships between disruption to care and the four psychological outcomes. Mediation analyses revealed that depression, stress, anxiety, and overall distress were mediated by self-efficacy in managing mental health. University administrators and health care providers should evaluate the scope of mental health care and telemedicine services for students to help long-term psychological effects of COVID-19.

RESUMO

The article considers problematic issues in hospital-replacing out-patient mental care management on the basis of the Psychiatric Hospital № 13 in 2015-2017. The analysis covered structure of mental patients, their age and social status. Also, patients under active dispensary monitoring and compulsory continuous out-patient treatment were analyzed. The new management component - «the out-patient psychiatric module¼ - is proposed to be introduced into the structure of mental dispensary.

Objetivo: compreender a percepção dos trabalhadores da equipe de enfermagem sobre a educação permanente em saúde mental. Método: estudo qualitativo, descritivo, exploratório, desenvolvido em três unidades de internação de um hospital geral. Realizou-se a coleta de dados por meio de entrevistas semiestruturadas com 15 trabalhadores da equipe de enfermagem. Fundamentou-se a análise dos dados no método de Análise do Fenômeno Situado. Resultados: aponta-se que os trabalhadores identificaram experiências com usuários de saúde mental marcadas por dificuldades no exercício do cuidado, em virtude da carência de conhecimento específico e experiência. Verificou-se que, quando as ações de educação permanente abordam temáticas técnicoassistenciais, motiva-se a necessidade de buscar informação, por meio das discussões entre pares e/ou da pesquisa individual. Conclusão: a educação permanente em saúde mental permite que a equipe de enfermagem repense as práticas, o processo de trabalho e o cuidar em saúde mental, de modo mais humanizado e em conformidade com a Reforma Psiquiátrica. Observa-se que a Enfermagem deve ser capaz de desenvolver habilidades para o manejo de situações específicas e gerar segurança e conforto aos profissionais para a oferta do cuidado.(AU)


Objective: to understand the perception of nursing team workers about permanent education in mental health. Method: qualitative, descriptive, exploratory study, developed in three inpatient units of a general hospital. Data were collected through semi-structured interviews with 15 workers on the nursing team. Data analysis was based on the Situated Phenomenon Analysis method. Results: the workers reported experiences with mental health patients marked by difficulties in the exercise of care due to the lack of specific knowledge and experience. When permanent education actions address technical-assistance issues, the need to seek information is motivated through peer discussions and/or individual research. Conclusion: permanent education in mental health allows the nursing team to rethink practices, the work processes, and mental health care ina more humane way and in accordance with the psychiatric reform. It is observed that the nursing team must be able to develop skills for handling specific situations and generate safety and comfort for professionals to provide care.

Se observa que na Enfermería debe ser capaz de desarrollar habilidades para el manejo de situaciones específicas y generar seguridad y comodidad para que los profesionales brinden cuidados.(AU)

Objectives: Poor oral health is an important determinant of general health in patients with mental illnesses. The present study was conducted to evaluate the dental and periodontal status of hospitalized female psychiatric patients in Jazan city, Saudi Arabia. Material and Methods: This study included 82 subjects: 37 psychiatric female patients, and 45 age- and gender- matched healthy controls. Dental caries experience (decayed, missing, and filled teeth index ( DMFT ) and periodontal health status were recorded. SPSS version 21 was used for data analysis, and p-value < 0.05 was considered statistically significant. Results: The mean ages were comparable among the two groups. Psychiatric patients showed poor oral hygiene practices, with only 56.6% reported regularly brushing their teeth as compared with 82.2% of controls (P < 0.05). Control subjects had significantly higher DMFT mean scores than psychiatric female patients. However, psychiatric patients had significantly higher mean scores of missing teeth (M component) and lower restored teeth (F component) as compared with controls. Moreover, the mean attachment loss scores were significantly higher in the psychiatric group (0.76±1.58) compared with controls (0.23; 0.44 p < 0.01). Conclusion: Psychiatric patients showed poor oral hygiene practices and a high prevalence of untreated dental caries and periodontal diseases. Effective oral health promotion strategies need to be implemented to improve the oral health and oral health practices of this group of people. (AU)


Objetivo: A má saúde bucal é um importante determinante da saúde geral em pacientes com doenças mentais. O presente estudo foi realizado para avaliar o estado dentário e periodontal de pacientes psiquiátricos do sexo feminino hospitalizados na cidade de Jazan, Arábia Saudita. Material e Métodos: Este estudo incluiu 82 indivíduos: 37 pacientes psiquiátricas do sexo feminino e 45 controles saudáveis pareados por idade e gênero. A experiência de cárie dentária (índice de dentes cariados, perdidos e obturados (CPOD) e o estado de saúde periodontal foram registrados. O progama SPSS versão 21 foi usado para a análise dos dados, e o valor de p <0,05 foi considerado estatisticamente significativo. Resultados: As médias das idades foram comparáveis entre os dois grupos. Pacientes psiquiátricos mostraram práticas de higiene bucal precárias, sendo que apenas 56,6% relataram escovar os dentes regularmente, em comparação com 82,2% dos controles (P <0,05). Os indivíduos controle tiveram pontuações médias de CPOD significativamente mais altas do que as pacientes psiquiátricas. Os pacientes tiveram escores médios significativamente maiores de dentes perdidos (componente P) e dentes restaurados menores (componente O) em comparação com os controles. Além disso, os escores médios de perda de inserção foram significativamente maiores no grupo psiquiátrico (0,76 ± 1,58) em comparação com os controles (0,23 ± 0,44;p <0,01). Conclusão: Pacientes psiquiátricos apresentaram práticas inadequadas de higiene bucal e uma alta prevalência de cárie dentária e doença periodontal não tratada. Estratégias eficazes de promoção da saúde bucal precisam ser implementadas para melhorar a saúde bucal e as práticas de saúde bucal desse grupo de pessoas (AU)

Resulta sorprendente que algunas publicaciones periódicas, editadas por personas con enfermedad mental en diversos entornos psiquiátricos, se remontan a hace casi dos siglos. Primero en las grandes instituciones psiquiátricas como parte integrante del tratamiento moral, para más recientemente dar el salto a otros entornos asistenciales y comunitarios, como son hospitales de día y comunidades terapéuticas. Sin embargo, a pesar de su dilatada historia y haber sobrevivido a las diferentes corrientes teóricas y asistenciales, siendo un fiel reflejo de sus postulados y métodos, llama la atención el poco caso e interés despertado en los ámbitos académicos en relación con su estudio y conservación, habiéndose perdido irremediablemente muchas de ellas para el futuro. El trabajo pretende rescatar del olvido dos de aquellos pioneros periódicos. Uno, "un periódico de considerable interés" en Essex (Inglaterra), que reclamaría para sí el honor de ser el primero de esas características en la historia, desplazando de esa posición a la Retreat Gazette de Connecticut (Estados Unidos). El segundo, el Sotto-Pancia, en Florencia, que precedió en el tiempo al Diario dell' ospizio di San Benedetto promovido por Lombroso en Pesaro y considerado hasta ahora el primero en Italia


It is surprising that some periodicals, edited by people with mental illness in various psychiatric settings, date back almost two centuries. First in large psychiatric institutions as an integral part of moral treatment, to make the leap more recently to other community settings, such as day hospitals and therapeutic communities. However, despite its long history and having survived the different theoretical and clinical models, being a faithful reflection of its postulates and methods, it is striking the little case and interest aroused in academic fields in relation to its study and conservation, having lost many of those publications for the future. The work aims to rescue from oblivion two of those pioneering periodicals. One, "a newspaper of considerable interest" in Essex (England), which would claim for itself the honour of being the first of these characteristics in history, displacing from that position the Retreat Gazette of Connecticut (United States). The second, the Sotto-Pancia, in Florence, which preceded to the Diario dell 'ospizio di San Benedetto promoted by Lombroso in Pesaro, that was considered so far the first in Italy

Objetivo: descrever a elaboração de protocolo para a identificação do paciente com transtorno mental agudo. Método: estudo exploratório e qualitativo, realizado de maio 2018 a janeiro 2019, por meio das etapas: revisão integrativa da literatura, questionário online respondido por 17 profissionais de saúde vinculados à Sociedade Brasileira para a Qualidade do Cuidado e Segurança do Paciente, e grupo focal com 04 especialistas em Saúde Mental. Para tratamento dos dados, utilizou-se a análise descritiva e comparativa. Resultados: na revisão não se encontraram artigos sobre identificação do paciente com transtorno mental agudo. Na consulta aos especialistas da segurança do paciente identificou-se que 82,3% não possuíam em suas instituições protocolo específico. No grupo focal evidenciou-se dificuldade na identificação deste paciente. Conclusão: acreditase que o protocolo com a inserção da pulseira fotográfica apresenta-se como uma ferramenta inovadora na redução de riscos associados à identificação deste paciente.


Objective: to describe the development of a protocol for identification of patients with acute mental disorders. Method: this qualitative exploratory study was carried out from May 2018 to January 2019 through an integrative literature review, an online questionnaire answered by 17 health personnel belonging to the Brazilian Society for Quality of Care and Patient Safety, and a focus group of four mental health experts. Results: no articles specifically on identification for patients with acute mental disorders were found in the review. The consultation of patient safety experts found that 82.3% had no specific protocol in their institutions. The focal group highlighted difficulties communicating with these patients. Conclusion: the protocol including the photographic bracelet is believed to constitute an innovative tool for reducing risks associated with identification of these patients.


Objetivo: describir el desarrollo de un protocolo para la identificación de pacientes con trastornos mentales agudos. Método: este estudio exploratorio cualitativo se realizó de mayo de 2018 a enero de 2019 a través de una revisión integradora de la literatura, un cuestionario en línea respondido por 17 miembros del personal de salud pertenecientes a la Sociedad Brasileña de Calidad de Atención y Seguridad del Paciente, y un grupo focal de cuatro personas de salud mental expertos. Resultados: en la revisión no se encontraron artículos específicos sobre identificación de pacientes con trastornos mentales agudos. La consulta de expertos en seguridad del paciente encontró que el 82,3% no tenía un protocolo específico en sus instituciones. El grupo focal destacó las dificultades para comunicarse con estos pacientes. Conclusión: se cree que el protocolo que incluye la pulsera fotográfica constituye una herramienta innovadora para reducir los riesgos asociados a la identificación de estos pacientes.

Mental disorders are widespread among the world's population and place a high burden on both the people affected and the economy. In this area of health care and prevention major deficits can be found. Health-enabling technologies are being developed in order to provide support in the therapy and diagnostics of mental disorders. However, it is not clear whether patients are open to these technologies and what they expect from a suitable usage. The main goal of this study is to find out what opinions, hopes and fears mentally ill persons have towards a supporting treatment with health-enabling technologies. Personal interviews were conducted with psychiatric patients for that purpose. The evaluation of the interview data revealed a predominantly positive mindset of the participants. In addition to the general question according to the acceptance, requirements and expectations for the use of health-enabling technologies were acquired. In this context the concern of an invasion of privacy was exposed as a major barrier.

The scientific and clinical interest in children with mentally ill parents increased in the last years. Those children belong to a high risk population so that prevention is urgently indicated. Due to genetic influences and partly to an impairment of the parent-child interaction because of the parent's illness there exists a higher risk for child abuse. They show a three to five time increased risk to develop mental problems which require treatment over the course of their lives. They show abnormalities in social, cognitive and emotional areas. Untreated mental disorders and associated behavioral problems in children often chronify and lead to permanent impairment of the emotional, social and also intellectual development. Early detection and treatment are indicated and of high relevance.

The article provides an overview of racism discourses in research and clinical practice in the health sector and discusses the individual and institutional effects of racism and discrimination on mental health. In addition, suggestions are provided as to which racism critical transformations in healthcare structures for mentally ill persons are necessary in order to enable equitable participation for people affected by discrimination and racism.

Shared Responsibility!? Interdisciplinary Care for Children of Mentally Ill Parents From the Perspective of Child and Youth Welfare Families with at least one mentally ill parent require professional help from several support systems considering the different burdens and complex needs. When designing help and care programs, it is important to focus on the quality of interdisciplinary cooperation and communal networking. Case-related and cross-case cooperation requires binding work structures as well as an overall strategy at the local level. The recent final report to the German Bundestag by the working group "The Children of Mentally Ill Parents" emphasized the relevance of the topic while putting forward recommendations. This article outlines the current academic discussion as well as developments in the design process of interdisciplinary care programs from the perspective of child and youth welfare.

Liaison and Diversion (L&D) has twin objectives: improving mental health outcomes and reducing re-offending. Early diversion from police custody seems promising, but evidence of benefit is required to sustain such programmes. To test the hypothesis that contact with L&D services while in police custody would lead to improved mental health outcomes and a reduction in type and level of offending, we used a pre-post service use design. National Health Service (NHS) records in two counties were searched for evidence that patients had been involved with L&D services while in police custody during the period July 2009-December 2017. We defined January 2009-July 2014 as the pre-intervention period and any time after contact as the post-intervention period. Data from the Police National Computer were gathered for each period for these individuals, to assess their pre-post L&D contact offending histories. NHS Trust data were similarly gathered to assess their pre-post use of mental health legislation. 4,462 individuals were identified who had used L&D services in police custody. There were statistically significant reductions in the amount of offending following contact with the L&D service (whether one or two contacts), regardless of offence type. Statistically significant reductions were also observed in use of the four most commonly used legislative powers for detaining patients in hospital on mental disorder grounds, regardless of offending status (prolific/non-prolific). Our results indicate positive associations between the L&D interventions and change in offending and use of compulsory hospital detention. Whilst our research does not allow a direct causal relationship to be established in either area, the findings go beyond other impact assessments of L&D which have either been with small samples or relied only on qualitative data or expert opinion.

Mental illness remains as one of the most stigmatizing conditions in contemporary western societies. This study sheds light on how mental health professionals and rehabilitants perceive stigmatization. The qualitative study is based on stimulated focus group interviews conducted in five Finnish mental health rehabilitation centers that follow the Clubhouse model. The findings were analyzed through inductive content analysis. Both the mental health rehabilitants and the professionals perceived stigmatization as a phenomenon that concerns the majority of rehabilitants. However, whereas the professionals viewed stigma as something that is inflicted upon the mentally ill from the outside, the rehabilitants perceived stigma as something that the mentally ill themselves can influence by advancing their own confidence, shame management, and recovery. Improvements in treatment, along with media coverage, were seen as the factors that reduce stigmatization, but the same conceptualization did not hold for serious mental illnesses. As the average Clubhouse client was thought to be a person with serious mental illness, the rehabilitation context designed to normalize attitudes toward mental health problems was paradoxically perceived to enforce the concept of inevitable stigma. Therefore, it is important for professionals in rehabilitation communities to be reflexively aware of these tensions when supporting the rehabilitants.

Germany, in comparison to other countries, provides a good quality of services to distinct groups of patients. But as far as total expenditure for mental health services is concerned, it belongs to the top European states. The differentiation of services since the Expert Commission to the parliament (1975) was associated by fragmentation of the therapeutic alliances and the patient's relations in his or her personal social system. The management of intake to hospital treatment seems to need further improvement. Treatment paths which integrate distinct sectors are clinically tested in other countries and should be hold up as examples for politicians to further improve our mental health services. With the available resources, better and economically sound results could be reached. This is important for the future, a) because of increasing utilization of mental health treatment, b) to guarantee the quality of services for the chronic, severely mentally ill patients.
