OBJECTIVES: Despite the complexity of the edentulous anatomy, little evidence exists as to what impression techniques and materials should be employed for an optimal result. The aim of this in silico study was to evaluate the trueness of peripheral and inner seals of different edentulous jaw impressions. MATERIALS AND METHODS: Twelve maxillary edentulous participants (male = 8, female = 4; age 68.5 ± 11.7 years) participated in this study. Four different impression materials and techniques, irreversible hydrocolloid (Alginate; Blueprint X-Crème, Dentsply Sirona, PA, USA), polyvinyl siloxane impression (PVS; Aquasil Ultra+ Medium, Dentsply Sirona, PA, USA), and subsequently modified with ZnOE (PVSM) and an optical impression (Optical; 3Shape A/S, Copenhagen, Denmark), were tested against a control impression: low-fusing impression compound border molding (Kerr Corp., CA, USA) followed by a ZnOE impression (ZnOE; SS White impression paste, S.S. White Group, Gloucester, England). All impressions were scanned and analyzed using a custom-built 3D comparison software analyzing the vertical and horizontal trueness. RESULTS: The vertical discrepancy (peripheral seal) of the impression surface was significantly more true for PVSM than Alginate (p = 0.001), PVS (p = 0.019), and Optical groups (p < 0.001). Where the horizontal discrepancy (inner seal) was compared, the impression surface was more true for PVSM than Alginate (p < 0.001) and Optical (p < 0.001). PVS group was also significantly more true than Optical (p = 0.015). CONCLUSION: Impression techniques and materials may significantly influence the peripheral and inner seal of an edentulous jaw impression. CLINICAL RELEVANCE: When using a polyvinylsiloxane impression material for master edentulous impressions, a selective inner seal reline with a conventional zinc oxide eugenol impression paste can improve the inner seal.

STATEMENT OF PROBLEM: Information on the mechanical properties of the materials used for manufacturing computer-engineered complete dentures is scarce. PURPOSE: The purpose of this in vitro study was to evaluate the mechanical properties of 3 prepolymerized polymethyl methacrylate (PMMA) resins used in the fabrication of computer-aided design and computer-aided manufacturing (CAD-CAM) milled complete dentures (CDs), as well as 2 denture base polymers used for conventionally fabricated CDs. MATERIAL AND METHODS: Three CAD-CAM materials were evaluated: Degos Dental L-Temp, IvoBase CAD, and Zirkonzahn Temp Basic Tissue. Two materials used for conventionally manufactured dentures were also included as controls (Palapress and Paladon 65). Each material type was sectioned into bars for flexural strength, nanohardness, elastic modulus, and surface microhardness evaluation (n=8/material). Half of the specimens were stored in water for 30 days, while the other half was dry-stored. A 2-way ANOVA was conducted to detect the effect of material and storage on the evaluated properties (α=.05). Linear contrasts were conducted to compare the differences among the 3 types of CAD-CAM material and the conventional ones. RESULTS: Material type and storage had a significant influence on the flexural strength, nanohardness, elastic modulus, and surface hardness of the materials investigated (P<.001). The post hoc Scheffé test for flexural strength revealed a nonsignificant difference in the interaction between Degos L-Temp and Paladon (P=1.000). In terms of nanohardness, no difference was found when comparing Palapress with Paladon, as well as IvoBase CAD with Zirkonzahn Temp Basic (P=1.000). A nonsignificant interaction in terms of surface hardness was also found between IvoBase CAD and Palapress (P=.575). CONCLUSIONS: The tested materials showed variation in their mechanical properties, with satisfactory behavior of the CAD-CAM materials. However, the results obtained when testing the materials used for the conventional fabrication of complete dentures suggest that their use might still be advisable.

OBJECTIVE: The aim of this systematic review was to evaluate the effectiveness of removable complete denture disinfection using microwaves for the treatment of denture stomatitis. METHODS: This review identified studies comparing the efficacy of microwave prosthesis disinfection (I) to topical antifungal therapy (C) in the treatment of denture stomatitis (O), which included only individuals who used complete dentures and presented with denture stomatitis (P). The search was performed in six databases and identified 1524 records; after the removal of duplicates, 816 articles remained. Three articles were selected for qualitative synthesis and two for meta-analysis. Random-effects meta-analysis estimated the polled effects of microwave disinfection and antifungal therapy on the Candida counts and clinical manifestation of denture stomatitis. The GRADE approach was used to estimate the certainty of evidence. RESULTS: All included studies reported significant reductions in Candida counts and the frequency of denture stomatitis of groups subjected to microwave disinfection of dentures and topical antifungal therapy with nystatin. Significative differences between treatments were only detected for Candida quantification in the palate, within 90 days follow-up period (SMD = 0.47, 95% CI = 0.02-0.91). Meta-analyses did not show any further differences between treatments (p > .05), considering the Candida quantification in dentures and the frequency of clinical signs of denture stomatitis. The certainty of the evidence was considered as low, according to GRADE approach. CONCLUSION: Microwave disinfection of complete dentures can be considered as efficient as antifungal therapy for the treatment of denture stomatitis. However, further well-designed studies are necessary to confirm such evidence.

Objectives: Patients with xerostomia manifest various clinical signs of oral dryness, which has an impact on oral functions and wearing of dental prosthese?s, but the evidence of xerostomia-related changes in denture performance is unsatisfactorily documented. The purpose of this systematic review was to evaluate whether the available literature can answer the focused question "Is there an association between xerostomia and decreased denture performance among patients wearing removable dentures?" Data sources: Indexed databases were explored without time or language restrictions up to and including March 2019. All levels of available evidence including experimental studies, case reports, and case series were searched using different combinations of the following keywords: saliva, xerostomia, dentures, personal satisfaction, quality of life, oral dryness, and oral complaints. Nine studies were included for qualitative synthesis. Overall, five studies had a cross-sectional design and four studies were case-control studies. In these studies, the number of participants ranged between 35 patients and 493 patients with mean ages ?from 56 to 82 years; 66% of the patients were completely and 34% were partially edentulous.
Conclusion: All studies included patient satisfaction with dentures and recorded the presence of oral dryness. Six out of nine studies demonstrated that xerostomia is significantly associated with the decreased performance of removable dentures. Although the available evidence lacks feedback from randomized, controlled clinical studies, it implies a negative impact of oral dryness on specific denture functions such as speaking, chewing, and retention, which affects both complete and partial denture wearers.

RESUMO

Wear of artificial teeth is the most common complication associated with an implant-supported fixed complete denture (ISFCD). This wear alters the occlusal scheme, with loss of eccentric occlusal balance and vertical dimension of occlusion (VDO). This clinical report describes the fabrication of a new maxillary removable complete denture opposing an existing ISFCD with occlusal modifications. Both prostheses received splinted zirconia crowns cemented with composite resin cement.

OBJECTIVES: This RCT compares marginal bone level (MBL) change and the clinical parameters after a 3-year function in maxillary implant-supported fixed complete dentures (FCDs) treated with four-implants (4-I) or six-implants (6-I). MATERIAL AND METHOD: Three centres treated 56 patients with 280 implants allocated to the 4-I or 6-I group. Radiographic and clinical examinations were performed. The primary outcome was to investigate MBL change between the groups. RESULTS: Implant survival rates were 100% and 99% in the 4-I and 6-I groups, respectively. Considering the clustering effects, the MBL change was not significantly different between the groups over the 3-year follow-up. The MBL in the 4-I group was 0.30 ± 0.50 mm at baseline, 0.24 ± 0.31 mm at 1 year and 0.24 ± 0.38 mm at 3 years. In the 6-I group, MBL was 0.14 ± 0.32 mm at baseline, 0.16 ± 0.35 mm at 1 year and 0.12 ± 0.26 mm at 3 years. There was a statistically significant difference between the groups at BL and 3Y. No significant differences between the groups were reported for clinical parameters at each time point as well as in between the visits. The technical and biological complications rates were 1.6% and 6.0%, respectively. Prosthetic complications affected 25 FCDs (47.2%). CONCLUSION: Marginal bone level change revealed a stable condition in the 3-year period in the two groups. Few technical and biological complications occurred apart from the chipping/fracture of the prosthetic teeth. Four-implant is a feasible solution if the rehabilitation is oriented towards the most cost-effective treatment and towards avoiding bone augmentation procedures. Clinicians have to consider the potential required visits for prosthetic maintenance.

PURPOSE: To evaluate the effectiveness of liquid disinfectant soaps for the reduction of microorganisms present on maxillary complete dentures. MATERIALS AND METHODS: The selected patients (N = 28) were randomly divided into four groups (n = 7), and each group underwent all four disinfection treatments in a different order. The disinfection treatments evaluated were: 0.5% sodium hypochlorite (positive control); Dettol liquid soap; Lifebuoy liquid soap; and phosphate-buffered saline solution (negative control). The patients were instructed to immerse their maxillary dentures in the disinfectant solution for 8 hours (overnight) for 7 days, with the solutions in a randomized sequence with a washout period of 1 week between solutions. Biofilm samples of the dental prostheses were obtained before and after each treatment with a sterile swab, and the microbiologic material was diluted and plated in selective media for Candida spp. Colony-forming unit count (CFU/mL) was performed in each group. One-way ANOVA with Welch correction was used for analysis, with Games-Howell post hoc test with a significance level of .05. RESULTS: A 3-log reduction in microorganisms was considered effective compared to baseline. The highest incidence observed was for Candida albicans, which presented with a frequency between 66% and 92%, followed by C tropicalis, with a frequency between 7% and 33%, in all experimental groups. Sodium hypochlorite was able to reduce more than 3 log10 of microorganisms for all patients, showing high antifungal effectiveness for both C albicans and C tropicalis species. Regarding the experimental groups, both liquid soaps (Dettol and Lifebuoy) were effective in reducing the two types of microorganisms. CONCLUSION: Liquid soaps were effective at reducing biofilm and may be an alternative for disinfection of removable partial dentures or complete dentures.

RESUMO

The study of palatal rugae is shown to be scientifically valid to human identification due to the unique number and style of the palatal ridges. Dentures provide an array of data and specifics that allow for the individualization of their wearers. This article describes the identification of edentulous, skeletonized remains through the analysis of the palatal rugae printed on the complete upper denture and subsequent comparison with the palatal rugae of an old complete denture of an unknown missing person. The analyses focus on the form, classification, location, and size of the palatal rugae which, in conjunction with the information obtained from the anthropological examination, resulted in a positive identification of the cadaver. This method has a significant impact on the identification process, particularly when other identification methodologies and techniques cannot be implemented. This case report highlights the importance of palatal rugae in human identification in cases of edentulous cadavers.

BACKGROUND: The period of functional adaptation to a new conventional complete denture embraces many transitory issues, and this period is directly related to the rehabilitation success. OBJECTIVE: This clinical trial evaluated the influence of the height of mandibular ridge on the masticatory function of complete denture (CD) wearers during the adaptation period. METHODOLOGY: A total of 28 individuals wearing new CDs (NR, n=14, normal mandibular ridges, 64±12.5 years, 9 female; RR, n=14, resorbed mandibular ridges, 69±6.8 years, 9 female) were assessed at 24 hours, 30 days, three months and six months after the insertion of the CDs for masticatory performance (MP, sieves method), satisfaction with CDs (questionnaire) and maximum occlusal bite force (MOBF) (gnatodynamometer). The classification of the mandibular ridges followed the Kapur index. Data of MP and MOBF were analyzed by two-way ANOVA and satisfaction with CDs was analyzed by Generalized Estimating Equations (GEE), α=.05. RESULTS: Participants with NR presented better masticatory performance (p=.000 - NR 30.25±9.93%, RR 12.41±7.17%), general satisfaction (p=.047), retention of mandibular denture (p=.001), chewing ability (p=.037), and comfort of wearing a mandibular denture (p=.000). Regardless of the mandibular ridge, MP (p=.000) was higher at three (21.26±12.07%) and six months (24.25±12.26%) in comparison to 24 hours (18.09±10.89%), the MOBF (p=.000) was higher at three months (78.50±6.49 N) compared to 24 hours (57.34±5.55 N) and 30 days (62.72±5.97 N), and the comfort of wearing a mandibular denture (p=.002) at three months (1.61 ± 0.07) was greater than 24 hours (1.29±0.10) and 30 days (1.36±10). CONCLUSIONS: The study suggests that the participants with NR have higher MP and satisfaction with their CD, regardless of the follow-up period after the insertion of the new CD. After subjects received the CD, a period of 3 months was necessary for achieving better achievement MOBF, MP, and self-perceived comfort with the mandibular denture, regardless of the height of the mandibular ridge.

This cross-sectional observational study with 24 patients evaluated differences in bite force (BF) and masticatory performance (MP) between conventional complete denture (CCD) and implant mandibular overdenture (IMO) users and the correlation between these variables. The BF test was performed bilaterally with an occlusal force device. During the MP test, patients were asked to chew Optocal particles for 40 cycles. The Shapiro Wilk test was employed to verify the normality of the data, the student t test to identify differences between groups, and Pearson's correlation to investigate interrelationships between variables. A multiple linear regression was subsequently performed via the stepwise method. P values ≤0.05 were considered statistically significant. Unlike IMO users, CCD users presented a significant difference (25.6%) in BF between the dominant and non-dominant chewing side (p=0.04). IMO users presented significantly higher BF (p=0.01) without presenting a dominant side (p=0.38), and also performed significantly better for the following MP parameters: MPX50 values decreased by 27.25% in IMO users (p=0.01), MPB decreased by 48.38% (p=0.01), and ME 5.6 decreased by 53.25% (p=0.02), while ME2.8 increased by 151.57% (p=0.01). The BF and MPX50 in the IMO wearers group were negatively correlated (-0.57; p=0.05); this correlation coefficient was the only parameter included in the multivariate regression model. IMO users have higher BF and better masticatory performance than CCD users, especially in terms of chewed particles size reduction. MP is correlated with a higher BF in IMO users through better particle trituration.

RESUMO

Whilst increasing numbers of older people retain natural teeth, a sizable proportion of older people are still edentate. Replacement of missing teeth is important for function, aesthetics and to facilitate social interactions. The process of providing oral rehabilitation for edentate older adults can be challenging, but clinicians can utilise a number of treatment options including removable or implant retained prostheses. In this article, complete denture construction will be described using both conventional and copy denture techniques. Recent innovations in the process for constructing complete dentures using computer aided design and manufacturing (CAD-CAM) techniques will also be discussed. Dental implants can be used to help retain removable complete prostheses for edentate patients and the use of implant-retained overdentures is presented as a treatment alternative.

OBJECTIVE: To observe the changes of forward displacement of maxillary complete denture during centric occlusion, three different methods were used to record the changes of vertical overlap and the comfort level of patients before and after the selective grinding of the three dentures made according to maxillo-mandibular horizontal relationship record. METHODS: Twelve edentulous patients with normal stomatognathic system were recruited in this study. Three types of complete dentures for these 12 edentulous patients were made according to their different maxilla-mandibular horizontal relationship record methods. The amount of displacement of the maxillary complete denture, the vertical overlap of the anterior teeth as well as patient comfort level were recorded before and after selective grinding. Statistical analysis was performed using the SPSS 17.0 software package. RESULTS: Before selective grinding, the amount of displacement of denture A was significantly larger than those of dentures B and C (P<0.05). After selective grinding, there was no statistical difference among the three dentures (P>0.05). During selective grinding, the vertical overlap variation of denture A was significantly greater than those of dentures B and C (P<0.05). Before selective grinding, the comfort level of the denture A was the lowest (P<0.05), and no statistical difference was found between dentures B and C (P>0.05). After selective grinding, no statistical difference was found among the three dentures (P>0.05). CONCLUSIONS: Among the complete dentures with anatomical teeth, the dentures whose horizontal relationship was recorded at 1 mm before the apex of the Gothic arch apex and with checkbite are more in line with clinical repair requirements. Complete dentures whose horizontal relationship was recorded at the apex of Gothic arch need to be adjusted with selective grinding to meet the clinical restoration requirements.

Data sources Data was collected utilising Medline (OVID interface), Google Scholar and Cochrane Library. Systematic reviews with/without meta-analysis were selected which included patient satisfaction and oral health-related quality of life as outcome measures. The literature search performed included systematic reviews with publication dates ranging from 1946 to 2018. PROSPERO was also used to incorporate systematic reviews that had been completed after the last search in August 2018.Study selection Studies were selected using the PICO model, selecting systematic reviews analysing adult edentulous patients with conventional complete dentures and/or implant-retained overdentures. The systematic reviews chosen assessed satisfaction and oral health-related quality of life, while comparing results to adult edentulous patients with no replacement teeth or prosthesis.Data extraction and synthesis All of the articles located from the database searches were uploaded to reference management software. Articles were screened independently by two authors to reduce bias and to assess the articles against the predetermined inclusion criteria. The EndNote filter Service was applied to avoid duplication of articles. Guidance from the Centre for Reviews and Dissemination (University of York) was incorporated to present data narratively, with text and tables. Eight reviews were included in data synthesis.Results Of the eight reviews included, six were systematic reviews without meta-analysis, one systematic review with meta-analysis was included and one was meta-analysis. Conclusions The results of this systematic review highlight the greater benefits of implant retained overdentures compared with conventional complete dentures when assessing patient satisfaction and oral health-related quality of life. The superiority of implant retained overdentures is most evident when patients cannot tolerate conventional complete dentures. However, consideration most be given to the adaptive capabilities of patients and the financial implications of implant-retained overdentures.

RESUMO

The proposed denture replication method shows a predictable workflow for fabricating complete dentures for patients with existing dentures. The replication technique is a method of fabricating new removable complete dentures utilizing the existing prosthesis as the foundation for a new denture. The technique has some advantages such as allowing the clinician to copy the tooth shape and contours of the existing prosthesis while modifying borders and intaglio surfaces. The denture replication method links proven methods of obtaining functional impressions using soft reline materials to a straightforward digital process. Utilizing the new "copy-denture" feature in the 3Shape software, it is possible to significantly reduce the number of appointments and laboratory labor involved in denture fabrication. The denture replication method protocol is increasing efficiency by reducing the amount of clinical procedures and visits, resulting in decreased treatment time and costs while providing a satisfactory clinical solution.

The purpose of this study was to clarify the usage and understanding of denture adhesives among 1,825 denture wearers, aged 40 years and older in Japan, using a web-based survey. The NTTCom Online survey (a closed investigation) was conducted over a period of 4 days using a 13-item questionnaire about denture adhesives. Results showed that 67.1% of the respondents were satisfied with their dentures; 81.5% of respondents knew about denture adhesive, but only 347 (19.0%) had used the product. Products used by the 347 denture wearers included cream type (79.7%), cushion type (16.2%), and powder type (3.2%). The most common reason for the selection of a particular type was "Saw the product" (59.9%), followed by "Saw an advertisement" (19.3%), and "Explanation at a dental clinic" (8.4%). The response of "Ill-fitting denture" significantly affected the response of "Use of denture adhesives" (chi-square test; P < 0.001). Moreover, the sensitivity (0.56) was lower than the specificity (0.75). These findings show that denture adhesives were not always necessary for those with ill-fitting dentures.

OBJECTIVE: To compare the clinical efficacy of a kind of complete dentures fabricated using computer aided design/computer aided manufacturing (CAD/CAM) workflow with that of the complete denture fabricated using conventional workflow. METHODS: Twenty edentulous patients were included in this prospective, single-blind, self-controlled clinical trial. Two pairs of complete dentures were fabricated for each participant: one using the functional suitable denture (FSD) system with CAD/CAM, and the other using conventional fabrication workflow. In the final delivery appointment, the conventional dentures worn by the participant for 3 months at first. Then the participant swapped to wear digital denture for another 3 months. The patients' satisfaction and oral health related quality of life (OHRQoL) were measured using the visual analogue scale (VAS) and oral health impact profile (OHIP-20E) at baseline, 2 weeks, 1 month, 2 months, and 3 months following denture delivery. In addition, the masticatory efficiency of each pair of dentures was measured after three months adaptation period for each type of dentures. RESULTS: The VAS scores of the twenty patients in FSD denture group on general satisfaction, ease of cleaning, ability to speak, esthetics, stability and oral health status on these six domains were higher than that of conventional denture group 3 months after delivery. While the VAS cores in FSD denture group on comfort, ability to chew and ability to chew up were the same as those of conventional denture group. But there was no significant difference (P>0.05). The VAS score on each domain in FSD denture group was higher than that of conventional denture group 3 months after delivery, three domains of which reached statistical significance (P < 0.05), involving functional limitation, psychological discomfort and total. The masticatory performance of the FSD denture (1.20±0.54) was slightly higher than that for the conventional denture (1.16±0.53), but the difference did not reach statistical significance (P=0.691). CONCLUSIONS: The clinical efficacy of the FSD complete denture is comparable to that of the conventional complete denture. As for patient satisfaction and oral health related quality of life, FSD dentures received comparable scores as conventional complete dentures did.

Las articulaciones temporomandibulares y la musculatura asociada, permiten que la mandíbula se desplace en los tres planos del espacio (sagital, frontal y horizontal), de esta forma la mandíbula puede realizar movimientos mandibulares complejos. Hace varias décadas que los movimientos bordeantes mandibulares son estudiados, sin embargo, en un principio los estudios se realizaban en dos dimensiones (2D). En la actualidad, gracias al articulógrafo electromagnético (EMA) 3D es posible realizar el estudio de la cinemática mandibular en tres dimensiones (3D). El objetivo del siguiente estudio es analizar los movimientos mandibulares bordeantes (sagital, frontal y horizontal), por medio de la articulografía electromagnética 3D. En el estudio, participaron 11 personas de 31,9 ± 5,7 años en promedio (mujeres 30,2 ± 2,9 y hombres 34,0 ± 7,8) completamente dentados de primer molar a primer molar, sin trastornos temporomandibulares ni tratamiento previo de ortodoncia. Los archivos fueron procesados mediante scripts, desarrolladas en MATLAB®. Entre los resultados más relevantes, se encontró diferencia estadística entre hombres y mujeres en relación al área Frontal del polígono de Posselt, y los resultados del área del polígono sagital de todos los participantes, fue mayor que en estudios previos.


The temporomandibular joints and associated muscles allow the mandible to move in the three planes (sagittal, frontal and horizontal), in this way the jaw can perform complex mandibular movements. For several decades, mandibular border movements have been studied. However, studies were initially carried out in two dimensions (2D). At the present time, it is possible to analyze mandibular kinematics in three dimensions (3D), with the 3D electromagnetic articulograph (EMA). The objective of the study is to evaluate the mandibular border movements (sagittal, frontal and horizontal), using 3D electromagnetic articulation. In this analysis, 11 subjects 31.9 ± 5.7 years of age on average, participated (women 30.2 ± 2.9 and men 34.0 ± 7.8) completely dentate patients, from first to first molar, with no temporomandibular disorders or orthodontic pretreatment. Files were processed using scripts, developed in MATLAB®. Among the most relevant results, a statistical difference was found between men and women in relation to the Frontal area of Posselt polygon, and results of the sagittal polygon area of all the participants were greater than those reported in previous studies.
