RESUMO

The Scientific Investigation Committee of the American Academy of Restorative Dentistry offers this review of the 2020 professional literature in restorative dentistry to inform busy dentists regarding noteworthy scientific and clinical progress over the past year. Each member of the committee brings discipline-specific expertise to this work to cover this broad topic. Specific subject areas addressed include prosthodontics; periodontics, alveolar bone, and peri-implant tissues; implant dentistry; dental materials and therapeutics; occlusion and temporomandibular disorders (TMDs); sleep-related breathing disorders; oral medicine and oral and maxillofacial surgery; and dental caries and cariology. The authors focused their efforts on reporting information likely to influence day-to-day dental treatment decisions with a keen eye on future trends in the profession. With the tremendous volume of dentistry and related literature being published today, this review cannot possibly be comprehensive. The purpose is to update interested readers and provide important resource material for those interested in pursuing greater detail. It remains our intent to assist colleagues in navigating the extensive volume of important information being published annually. It is our hope that readers find this work useful in successfully managing the dental patients they encounter.

RESUMO

At present, robotic system has been applied in many aspects of the field of prosthetic dentistry, such as tooth preparation, oral implant surgery, full denture arrangement, prosthodontic material testing and robotic education of prosthodontics. The advantages of prosthodontic robotics lie in their ability to perform quantitative and precise operations whilerepeating the work flow indefinitely, which assist dentists to complete heavy and complicated daily treatment. In the research and development of prosthodontic robotics, the limitation of oral operation space should be fully considered, and robotics should have high safety and flexibility. The review briefly summarizes the application and existing problems of robotics in prosthodontics, and provides references for further development and design.

OBJECTIVES: To evaluate implant survival, success and prosthodontic outcomes in a variety of composite free flaps. MATERIAL AND METHODS: A retrospective analysis of patient records was used for data collection of implants placed into flaps. Free flap donor sites included fibula, iliac crest, scapula, medial femoral condyle and radial forearm. Kaplan-Meier survival curves were used to estimate survival. Cox regression was used to assess variables affecting survival, and binary logistic regression was utilised to determine effect of variables on prosthodontic outcomes. RESULTS: There were 150 implants placed into 45 fibula, 98 implants into 29 iliac crest, 62 implants into 22 scapula, 6 implants into 3 medial femoral condyle and 2 implants in radial forearm composite free flaps. There was no difference in survival or success between or within the various flaps. Active smoking, increasing age, male gender and radiotherapy were associated with implant failure. Of patients completing prosthodontic rehabilitation, 93% were successful. There was no difference in likelihood of failure between different flap types. Fixed implant-supported dentures were associated with poor oral hygiene compared with removable designs (OR2.9, 95%CI 1.1-7.8, p = .03). CONCLUSIONS: There is adequate survival and success of implants in common flaps. Caution is required when planning implants in patients exposed to radiotherapy and smokers. Ability to maintain adequate oral hygiene is an important prosthetic design consideration.

The correct implant site design and placement are the basic clinical techniques that must be known for implant restoration. For a long time, most implants have been placed by free hands, and the choice of site is mostly dependent on the accumulation of long-term experience of the surgeon. The selection of implant site guided by this experience analogy logic is often based on the surgeon's level of experience,which often makes it very easy to produce complications related to the implant restoration of the incorrect site. In contrast, a clinical program using digital guidance and real-time measurable verification has emerged based on the restoration-oriented implantation concept, which marks the formation of an accurate, measurable and verifiable whole-process digital implant prototype. Furthermore, from the perspective of surveying, the numerical requirements that digital implant restoration relies on are actually incomplete to the four elements of measurement, which leading to the doubts about its authenticity. This article will question the numbers in implant restoration, and conduct a preliminary demonstration, and propose a new reliable actual measurement and verification method of the correct location and the numerical requirements of the restoration space and a new clinical program that relies on numbers from the perspective of the evolution of digital restoration, guided implantology and actual measurement technology. And this article further discusses the current mainstream implant restoration technology based on experience analogy which cannot effectively support the whole process of digital implant restoration and provides a new logical cognitive basis for the final realization of the entire process of digital implant restoration.

La caries de la temprana infancia puede afectar severamente la dentición primaria que requiere tratamientos complejos y en ocasiones la extracción del diente afectado. Se presenta informe de caso sobre la rehabilitación dental integral de un paciente de 6 años, la evolución de su comportamiento y la mejora en su calidad de vida, autoestima e interacción social. El plan de tratamiento incluyó una fase de adaptación a la consulta odontológica, eliminación de caries, restauración dental, exodoncias y elaboración de dos prótesis removibles. Al inicio del tratamiento presentó temor y rechazo al tratamiento, pero con el avance del proceso rehabilitador odontológico y la evidencia de los resultados, el comportamiento del niño fue de aceptación y colaboración. Su evolución positiva de comportamiento se presentó también en sus relaciones sociales, aumentó la interacción con otras personas y su participación en juegos con otros niños. La rehabilitación protésica contribuyó a recuperar las funciones de masticación y fonación. La renovación de la estética dental produjo un significativo cambio de conducta, mejora en su autoestima con un impacto psicológico positivo en su calidad de vida


Early childhood caries can severely affect the primary dentition requiring complex treatments and sometimes the extraction of the affected tooth. A case report is presented on the comprehensive dental rehabilitation of a 6-year-old patient, the evolution of his behavior and the improvement in his quality of life, self-esteem and social interaction. The treatment plan included a phase of adaptation to the dental consultation, elimination of cavities, dental restoration, extractions and elaboration of two removable prostheses. At the beginning of the treatment, he presented fear and rejection of the treatment, but with the progress of the dental rehabilitation process and the evidence of the results, the child's behavior was one of acceptance and collaboration. Their positive behavior evolution was also present in their social relationships, increased interaction with other people and their participation in games with other children. Prosthetic rehabilitation contributed to recovering the functions of chewing and phonation. The renovation of dental aesthetics produced a significant change in behavior, an improvement in their self-esteem with a positive psychological impact on their quality of life

RESUMO

The provisional prosthesis is a prerequisite for prosthodontic rehabilitation. The purpose of this technique was to register facial and intraoral scans predictably. A 3D virtual patient was created through facial scans, intraoral scans, digital face-bow transfer, and digital cross-articulation technique. This virtual patient allowed predictable positioning of intraoral scans to a virtual articulator by using digital face-bow transfer. The resulting virtual patient facilitated the design of definitive prostheses following a facially generated treatment planning principle. In addition, the virtual articulator was used to improve occlusal design on the definitive prostheses and reduce the need for intraoral adjustment.

This narrative review aims to provide an overview of recent studies and case reports on three-dimensional (3D) printing, and to verify the applicability of 3D printers in the field of dental prostheses. This review was performed by conducting a search of PubMed. The clinical application of fabricating a prosthesis made with cobalt-chromium is considered possible depending on the material and hardware of the 3D printer. However, it is currently difficult to assess the clinical use of 3D-printed zirconia crowns. Further research is required, such as verification of materials used, margin morphology, and hardware. Clinically acceptable results have been reported for patterns using 3D printers. Interim restorations made using a 3D printer have been reported with good results that are considered clinically usable. Dentures made with 3D printers need further verification in terms of strength and deformation. Custom trays made with 3D printers are clinically useful, however, issues remain with design time and effort. Although several studies have reported the usefulness of 3D printers, further verification is required since 3D printers are still considered new technology.

This article focuses on radiographic imaging with regard to planning, treating, and maintaining partially and completely edentulous prosthodontic patients with dental implants. Cone-beam computed tomography (CBCT) is the preferred imaging method for pretreatment dental implant treatment planning. Radiographic guides containing radiopaque materials and/or fiducial markers transfer both the proposed prosthesis design and desired implant location for appropriate radiographic evaluation. The three-dimensional CBCT analysis provides information on the adjacent relevant anatomy, bone volume of the edentulous sites, and restorative space assessment.

In general dental practice, the use of implants is focused mostly on prosthodontic issues. That is, the replacement of missing teeth or the support of dental prostheses. However, there are other dental fields using implants such as orthodontics or maxillofacial prosthodontics. A classic way to measure success in implant dentistry is to look how long implants and the corresponding superstructure survive and are in function. Nevertheless, this alone is a very crude parameter. Therefore, biological and technical complications are taken in account additionally. Nonetheless, these objective measures do not well replicate the perception of the patient. That why, subjective measures, reflecting the perception of the patient are recommended to complement objective parameters. If these dental patient-reported outcome measures (dPROMs) are wisely chosen, they offer a wide variety of options. Besides comparing therapeutic effects by using the instruments' summary score only, dPROMs such as the Oral Health Impact Profile (OHIP) provide the opportunity to additionally assess patients` perceptions in the 4 dimensions of oral health-related quality of life. These are functional limitations, pain, esthetic issues as well as psychosocial impairment. Even the 5-item short form of the OHIP captures these dimensions and provides an efficient way to assess patients' perception with low administrative burden. This in turn offers new insights into the patient perspective and therefore helps improving shared decision making.

OBJECTIVES: To analyze the clinical performance of the intraoral digital impression (IDI) in the fixed prosthodontics. METHODS: Databases of Medline (Ovid), EMBASE, Cochrane Central Register of Controlled Trials, and CNKI were searched for randomized controlled trial (RCT) on the use of IDI in fixed prosthodontics until May 2020. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. A Meta-analysis was conducted when available. RESULTS: Eleven RCTs involving 618 patients were included in this study. A total of 2 and 3 studies had low and high risks of bias, respectively, and other included studies had a medium risk of bias. Results illustrated that the IDI group could shorten the impression-taken time [SMD=-5.63, 95%CI (-11.25, -0.01), P=0.05] and improve the accuracy of the marginal fit [SMD=-0.53, 95%CI (-0.84, -0.22), P=0.000 7] compared with the conventional impression group. However, no significant difference was observed in the internal fit. CONCLUSIONS: Evidence indicated a good clinical performance of IDI for fixed prosthodontics. Notably, high-quality studies are expected to further support the conclusion.

OBJECTIVE: To evaluate the outcome of systematic video training on tooth preparation in veneer restoration and the practicability of the application of the digital evaluation system of scan design and assessment software. METHODS: Ten residents were selected from a group enrolled on the first-year programme for the National Standard Training of Dentistry in the Department of Prosthodontics, College of Stomatology, Ninth People's Hospital Affliated with Shanghai Jiao Tong University, School of Medicine. First, each student prepared five teeth based on their knowledge and clinical experience, and then received systematic video training on veneer preparation. Before and after the training, the evaluation of the effects of training was conducted on the prepared teeth by measuring the continuity of the finishing line and tooth reduction amount automatically using the prepCheck 2.0 (Dentsply Sirona, Charlotte, NC, USA) CAD/CAM system. RESULTS: The results showed a significant difference in the quality of finishing line continuity pre- and post-training. Furthermore, the data for tooth reduction after training, which met standard values, improved remarkably, increasing from 32.40 ± 7.82% to 60.50 ± 5.48%. CONCLUSIONS: Video training could significantly enhance the quality of tooth preparation for veneers. Moreover, the digital evaluation system could serve as an ideal alternative for tooth preparation evaluation for preclinical students, offering new insights for clinical education.

Aim: Recording the maxillomandibular relationship is important in various prosthodontic treatments. Evidence of face-bow transfer resulting in improved outcome is conflicting. Hence the objective of this study is to determine the use of face-bow transfer in prosthodontics. Settings and Design: : Systematic review based on PRISMA guidelines.">. Materials and Methods: A protocol was developed prior, which covered all aspects of the review. The databases explored were MEDLINE database, ScienceDirect, and Cochrane collaboration library. The PICO model included participants who received a complete denture/partial denture prosthesis. Intervention was the use of the face-bow device. Comparator was prosthesis made with a simple procedure of not using a face-bow device for prosthodontic rehabilitation. Outcomes were patient satisfaction with dentures, the stability of the dentures, esthetics, and time taken for clinical and laboratory procedures. Only randomized clinical trials were included in this study. The methodological quality of the studies was assessed according to the Cochrane risk of bias tool. Statistical Analysis Used: Qualitative analysis. Results: A total of 144 articles were identified. On excluding 4 duplicates and screening the title and abstracts of the rest of the records based on exclusion criteria only 15 studies were selected for review. Conclusions: The use of the face-bow did not yield superior results for the quality of the prosthesis. Therefore, there is no evidence for the utility of face-bow transfer in complete denture treatment. However, no inference could be drawn for its utility in partial denture prosthodontics as there was no study to draw an inference.

Tooth preparation is the primary and core operation technique for prosthodontics. Microscopic tooth preparation can improve the accuracy and efficiency of the operation. Experts from Society of Prosthodontics, Chinese Stomatological Association formulated the standard operating procedure for microscopic tooth preparation, so as to standardize its design points and operating procedures, highlight the difference between this novel technique and the traditional naked eye tooth preparation and promote the application of microscopic tooth preparation.

Tooth preparation is a common operation in dental clinical practice. This procedure is irreversible and invasive from the point of view of tooth preservation. Conditions of the abutment tooth, treatment methods, and restoration materials for target restoration affect tooth preparation. To achieve the goals of tooth tissue preservation, dental pulp protection, and periodontal health, dentistry professionals agreed on the importance of minimizing the amount of tooth reduction. The foundations for realizing this consensus are as follows. First, the available restoration materials with improved comprehensive performance need less target restoration space. Next, teeth can be prepared under a digital guide, and the real-time measurement of restoration space can be verified due to the invention of digital technologies for the analysis of the quantity and shape of the prepared tooth and tooth measurement. Moreover, guiding methods for preparation have been developed from freehand operation under the naked eye based on accumulated personal experience to digital-guidance jointing microscope. These innovations indicate the creation of a prototype of guided prosthodontics that is precise and applies real-time measurement throughout the process of tooth preparation. From the perspective of the evolution of digital, guided, and micro prosthodontics, this article raised seven questions about the numerical value and quantitative data transfer of tooth preparation and evaluated the authenticity of existing numerical requirements from the perspective of the four elements of measurement. Identifying unified measuring methods and developing measuring tools with a precision of hundred or ten microns will be the key to solving the problem about the authenticity of numerical measurement. Furthermore, this paper summarizes the methods of how to control tooth reduction and explains in depth why the currently dominant tooth preparation technology, which is based on empiricism, cannot effectively achieve the goals in digital prosthodontics. Therefore, we strongly call for rebuilding the digital foundation of prosthodontic treatment immediately.

PURPOSE: The objective of this Critically Appraised Topic was to determine the level of evidence relative to the usefulness of the Frankfort mandibular plane angle in prosthodontic treatment. MATERIALS AND METHODS: The Patient Intervention Comparison Outcome (PICO) mesh heading received zero PubMed references, as did Frankfort Mandibular Plane Angle (FMA) as a determinant for dental occlusion and Frankfort Mandibular Plane Angle as a determinant of the occlusal scheme. Frankfort Mandibular Plane Angle alone received 168 PubMed citations that highlighted 2 DiPietro articles and a third from the orthodontics literature which was a Randomized Controlled Trial not relevant to the PICO. Four others, three prosthodontic and one orthodontic publication, were related to the PICO. A Google search revealed one additional article, which was a narrative review. RESULTS: Nine articles were related to the search, 2 of which were clinical trials that revealed no evidence to support the use of FMA as a diagnostic test. CONCLUSION: Based on the limited data presented above, there is a lack of evidence to support the use of FMA as a diagnostic procedure to predict outcomes, or dictate prosthodontic treatment.

PURPOSE: Patients in need of extensive prosthodontic treatment may need restoration of their occlusal vertical dimension (OVD) due to tooth wear, tooth loss, or changes that have occurred to existing prostheses over time. Prosthodontic treatment is based on the clinical application of the available evidence regarding interocclusal distance (IOD), the positional stability of rest vertical dimension (RVD), and the effect of altering the OVD. Hence, the purpose of this consensus document is to examine available data related to IOD, RVD, and alteration of the OVD. MATERIALS AND METHODS: The search was limited to Clinical trials, Randomized Controlled Trials, Systematic Reviews and Meta-analyses. Key words were healthy patient, mean, range, interocclusal rest distance; healthy patient, mean, range, freeway space; and dentistry, interocclusal gap, and no citations appeared. Dentistry, interocclusal distance, revealed 5 not relevant citations. Dentistry, inter occlusal rest space, and dentistry, interocclusal rest distance, both had the same single not relevant citation. Dentistry, freeway space revealed over 7,000 citations. Dentistry, occlusal vertical dimension, revealed 253 citations, 7 of which were related to the search question but only 1 which was different from the previous search. Mandible, rest vertical dimension, age changes, found 7 citations, none relative to the question. Expanding the search to include journal article found 260 citations with only one relevant to the question. Mandible, rest vertical dimension, alteration, harm revealed no citations; mandible, occlusal vertical dimension, alteration, revealed 15 citations, 1 of which was relevant; mandible, occlusal vertical dimension, changes, revealed 75 citations, none of which were relevant; mandible, occlusal vertical dimension, rehabilitation revealed 10 citations, none of which were relevant. Expanding the search strategy to include Journal article, mandible, occlusal vertical dimension, alteration, received 159 citations, 4 of which were relevant; mandible, occlusal vertical dimension, restoration revealed 208 citations, 1 of which was relevant. Numerous other articles were culled by going through the reference lists of the aforementioned articles. RESULTS: For IOD, 27 articles were found relevant to the search question, which confirmed a mean of 3.0 mm with ranges from 1 to 9 mm. Five articles revealed little evidence as to whether the RVD changes during life. For OVD, 20 articles, including 4 systematic reviews, revealed some evidence that skeletal growth continues from mid adolescence into mid adulthood; strong anecdotal evidence that some unopposed teeth will continue to erupt; no clinical evidence to support the concept that abraded teeth in occlusion in a patient with bruxism will undergo continuous eruption; and some evidence from clinical case reports that restoring OVD in patients with severe abrasion is a successful treatment. CONCLUSIONS: There is a range of dimensions for the interocclusal distance (IOD) with many normal dental patients functioning with a higher or lower IOD than the commonly used 3.0 mm average dimension. The resting vertical dimension (RVD) is a 3-dimensional range with little evidence related to changes in the RVD during life. However, aging can cause a decrease in muscle tone which could affect the RVD. The restoration of the OVD can be successfully accomplished if proper diagnosis and treatment planning are performed.

The objective of this review is to identify preventive measures for COVID-19 to safeguard dental professionals providing prosthodontic dental care. Electronic searches were performed in PubMed, Scopus, and Cochrane databases using the following descriptors and/or words: "COVID-19," "SARS-CoV-2," "Dental practice," "Prosthodontics," and "Dental infection control." Prosthodontists are at high risk for exposure to the novel coronavirus through aerosols and possibly contaminated surfaces and indirect contact with dental laboratories and dental technicians through impressions, dental stone casts, and fixed and removable prosthetic appliances. Therefore, preventive measures should be implemented, including performing emergency treatments only, rescheduling patients with suspected disease, disinfecting surfaces and prosthodontic materials with biocidal substances, and using protective equipment such as N95 masks, disposable gowns, and face-shields.
