RESUMO

BACKGROUND: PBL approach has been widely used in many Chinese universities over the past decade. However, the effects of PBL approach on medical psychology education in China are inconsistent. The purpose of this study was to ascertain whether the PBL approach was superior to the lecture-based teaching method in the context of the medical psychology curriculum in China. METHODS: A systematic review and meta-analysis was performed to confirm the effectiveness of PBL in Chinese medical psychology. Corresponding databases were searched for available studies, where data were extracted to calculated Hedges' g and its 95% confidence interval in total and subgroup analyses. Subgroup analyses were also carried out. RESULTS: Nine studies with 551 cases and 496 controls were identified. The total examination scores of students in the PBL approach group were significantly higher compared with students in the traditional lecture-based teaching group under the random effect model (Hedges' g = 1.510, 95%CI 0.792-2.227, p<0.001). Subgroup analyses based on major and school system exhibited similar results. CONCLUSIONS: Our study supported the notion that the PBL approach may be applicable to Chinese medical psychology education.

Over the past few decades, pharmacists' work has changed from product-centered tasks to patient-centered care. In response to such social changes and needs, the pharmacy education course was also extended from 4 to 6 years, and the importance of the humanities in the curriculum (e.g., medical psychology, medical ethics, and communication) is now recognized. The Model Core Curriculum for Pharmacy Education, 2013 version, described 10 professional competencies for pharmacists (professionalism, patient-oriented attitude, communication skills, interprofessional team care, basic sciences, medication therapy management, community health and medical care, research, lifelong learning, and education and training) and stated that the humanities are a foundation of pharmaceutical education. However, a report by the Pharmaceutical Society of Japan (2014) expressed concern that clinical practice was not connected with knowledge of the humanities. It is educationally meaningful when pharmacists who studied the humanities can then offer the best medical care to patients. In order to utilize knowledge of the humanities in the clinical setting, educators need to provide opportunities for active learning. Furthermore, the humanities are useful to help pharmacists acquire meta-cognition.

RESUMO. A saúde mental é um nó-crítico para a gestão da Atenção Básica, convocada diariamente a acolher o sofrimento psíquico individual e coletivo contando com equipes mínimas. Pouco antes da VIII Conferência Nacional de Saúde, que estabeleceria bases para um novo sistema brasileiro de saúde, estruturava-se na periferia da cidade de Porto Alegre-RS um serviço de saúde comunitária antecipatório à Estratégia Saúde da Família, em que algumas equipes já contavam com psicólogos. Este artigo procura refletir acerca do atual lugar do psicólogo no nível primário de atenção, discutindo a experiência de mais de duas décadas de trabalho da psicologia como integrante de unidades de saúde daquele Serviço. Finalmente, discute o potencial da inserção da psicologia no nível primário ao considerar a complexidade de uma prática que envolve atenção, gestão, formação e participação, no processo de trabalho em equipe multiprofissional.


RESUMEN La salud mental es un crítico-nodo para la gestión de la atención primaria de salud, convocado al día para recibir a la angustia psicológica individual y colectiva y contando con equipos mínimos. Justo antes de la Conferencia Nacional de Salud VIII, que establecería bases de un nuevo sistema de salud brasileño, un servicio de salud comunitario fue estructurado en las afueras de Porto Alegre-RS de anticipación a la Estrategia Salud de la Familia, en el que algunos equipos ya contaban con psicólogos. En este artículo se pretende reflexionar acerca del lugar del psicólogo en el ámbito de la atención primaria, basado en la experiencia de más de dos décadas de trabajo de la psicología como parte de los centros de salud de ese Servicio. Por último, se analiza el potencial de la inserción de la psicología en el nivel primario a considerar la complejidad de una práctica que implica la atención, gestión, formación y participación en el proceso de trabajar en equipos multiprofesionales.


ABSTRACT. Mental health is a critical node for the management of Primary Health Care, convened daily to welcome the individual and collective psychological distress and counting on minimum teams. Shortly before the VIII National Health Conference, which would establish foundations for a new Brazilian health system, a community health service was structured on the periphery of Porto Alegre-RS anticipatory to the Family Health Strategy, in which some teams already counted on psychologists at that time. This article seeks to reflect on the psychologist's current place in the primary care level, based on more than two decades of experience work in psychology as part of the health care facilities of that service. Finally, it discusses the potential of the psychology's insertion at the primary level when considering the complexity of a practice that involves attention, management, training and participation in the process of working in multi-professional teams.

Objective: This study sought to analyze scientific publications on the psychologists' role in providing assistance for terminal patients under PC. Methods: This integrative review was carried out from June to August 2019 by searching online databases. The following Descritores da Ciência da Saúde (DeCS) [Health Science Descriptors] were used: Psychology, terminal patient, palliative care, terminal care, and review. These descriptors were combined using the Boolean operator "AND" with the purpose of refining search results. Results: The analysis of 15 publications allowed the emergence of two categories: "Forms of psychological interventions targeting patients and their relatives from the perspective of terminality and "Benefits of the psychologist's action for the end-of-life patient under PC". Conclusion: The number of studies on the subject was scarce, which justifies the need for further research to extend the knowledge about psychological assistance for end-of-life patients under palliative care


Objetivo: Analisar artigos publicados acerca da atuação do psicólogo voltada ao paciente terminal em cuidados paliativos. Métodos: revisão integrativa com coleta de dados, feita entre junho e agosto de 2019, em bibliotecas digitais e bases de dados. Foram utilizados os descritores (DeCS): Psicologia, Doente Terminal, Cuidados Paliativos, Assistência Terminal e Revisão, associados pelo operador "AND" com a finalidade de refinar os artigos para responder ao objetivo do estudo. Resultados: mediante análise de 15 publicações criaram-se duas categorias: formas de intervenção psicológica, direcionada ao paciente e à família, frente à terminalidade, e benefícios da atuação do psicólogo para o paciente em fim de vida no contexto dos cuidados paliativos. Conclusão: foi verificada a escassez de estudos sobre a temática, o que justifica a necessidade de novas pesquisas, que ampliem os conhecimentos acerca do atendimento psicológico aos pacientes em cuidados paliativos na terminalidade

Métodos: revisión integral con recolección de datos, realizada entre junio y agosto de 2019, en bibliotecas digitales y bases de datos. Se utilizaron los descriptores (DeCS): Psicología, Enfermedad terminal, Cuidados paliativos, Cuidado Terminal y Revisión, asociados por el operador "Y" con el fin de refinar los artículos para cumplir con el objetivo del estudio. Resultados: mediante el análisis de 15 publicaciones se crearon dos categorías: formas de intervención psicológica, dirigidas al paciente y la familia, frente a la terminalidad, y los beneficios del desempeño del psicólogo para el paciente al final de la vida en el contexto de los cuidados paliativos. Conclusión: se verificó la escasez de estudios sobre el tema, lo que justifica la necesidad de realizar más investigaciones para ampliar el conocimiento sobre la atención psicológica a los pacientes con cuidados paliativos con enfermedades terminales

Since the inception of large-scale human genome research, there has been much caution about the risks of exacerbating a number of socially dangerous attitudes linked to human genetics. These attitudes are usually labeled with one of a family of genetic or genomic "isms" or "ations" such as "genetic essentialism," "genetic determinism," "genetic reductionism," "geneticization," "genetic stigmatization," and "genetic discrimination." The psychosocial processes these terms refer to are taken to exacerbate several ills that are similarly labeled, from medical racism and psychological fatalism to economic exploitation and social exclusion. But as genomic information becomes more familiar in clinical and research settings as well as other life activities, do we need to continue to worry so much about this family of attitudes and their impact on existing problems? In genomics, the underlying anxiety has been that disclosure of genomic information will trigger a series of (seemingly unavoidable) negative responses that will affect individuals, their families, and their communities at large. The fundamental social challenges that hyperbolic genomic messaging, low genomic literacy, and "folk biology" help sustain remain to be addressed. If we hope to break the cycle of genomic isms and ations, we will have to get better at resisting overinterpretations of the relevance that genomics has for people's future potentials, ancestral vulnerabilities, community memberships, and ethnic affiliations.

"Psychological essentialism" refers to our tendency to view the natural world as emerging from the result of deep, hidden, and internal forces called "essences." People tend to believe that genes underlie a person's identity. People encounter information about genetics on a regular basis, as through media such as a New York Times piece "Infidelity Lurks in Your Genes" or a 23andMe commercial showing people acquiring new ethnic identities as the result of their genotyping. How do people make sense of new scientific findings that are inherently complex if they don't have years of specialized training and education at their disposal? Given the substantial overlap between a lay understanding of genetics and lay intuitions about essences, we argue that, when most people are thinking about genes, they are not really thinking about genes in the complex ways that good scientists are. Combating people's essentialist biases can be a formidable challenge. Although we have identified some promising results of trying to reduce people's genetic essentialist tendencies, there is still much to learn about how these essentialist biases can be countered. It is important to help people understand genetic information so they are able make well-informed decisions about their lives.

Since the start of the program to investigate the ethical, legal, and social implications (ELSI) of the Human Genome Project in 1990, many ELSI scholars have maintained that genetic testing should be used with caution because of the potential for negative psychosocial effects associated with receiving genetic information. More recently, though, some ELSI scholars have produced evidence suggesting that the original ELSI concerns were unfounded, exaggerated, or, at a minimum, misdirected. At least in the contexts that have been most studied, large negative impacts have not been found in the vast majority of people studied. What might explain the discrepancy between the original hypothesized outcomes and the growing impression that large negative effects appear to be few and far between? And if the original predictions of large negative psychosocial effects were simply wrong, is it time for ELSI researchers to move on? Should genetic testing be routinized, and would it be appropriate to relax or abandon the practice of engaging patients in a process of detailed informed consent before they receive genetic information? To confront those questions, we convened a conference entitled "Looking for the Psychosocial Impacts of Genomic Information" to review what is known about the negative impacts of genetic information on a variety of populations and in multiple medical and social contexts, to explore the implications of the findings, and to consider whether future research might benefit from different methods than have been used to date.

Measuring quality of care can transform care, but few tools exist to measure quality from the client's perspective. The aim of this study was to create concordant clinician and client self-report quality-of-care scales in a sample of community mental health clinicians (n = 189) and clients (n = 469). The client scale had three distinct factors (Person-Centered Care, Negative Staff Interactions, and Inattentive Care), while the clinician scale had two: Person-Centered Care and Discordant Care. Both versions demonstrated adequate internal consistency and validity with measures related to satisfaction and the therapeutic relationship. These measures are promising, brief quality assessment tools.

O estudo das emoções na prática clínica expõe sua influência na relação médico-paciente. Este artigo apresenta os resultados de uma pesquisa qualitativa cujo objetivo foi explorar o discurso dos médicos sobre as próprias emoções no atendimento a pacientes. Das entrevistas com oito médicos foram extraídos os seguintes temas de análise: função apostólica e o entendimento da emoção; defesas e estratégias perante as emoções; e a companhia de investimento mútuo: frustração e consequências. Os médicos demonstraram pouca percepção da influência das emoções sobre a própria atuação, apesar de terem relatado situações nas quais foram afetados por elas. Revelaram sofrimento em razão da dificuldade de elaborar conteúdos emocionais, usando distanciamento afetivo como um mecanismo de defesa. Conclui-se que a gestão emocional por parte do médico maximiza recursos na interação com o paciente, lançando novas perspectivas para a atuação médica.(AU)


The study of emotions in clinical practice shows its influence on the physician-patient relationship. This article presents the results of a qualitative study with the objective to explore the discourse of physicians on their own emotions in the delivery of care to patients. Interviews with eight physicians gave rise to the analysis themes: apostolic function and understanding of emotions; defenses and strategies in face of emotions; and the company of mutual investment: frustration and consequences. The physicians demonstrated a poor perception of the influence of emotions on their performance, despite having reported situations in which they were affected by them. They also revealed suffering due to the difficulty in elaborating emotional contents, using affective distancing as a defense mechanism. In conclusion, emotional management on the part of physicians maximizes resources in the interaction with patients, casting new perspectives for their medical performance.(AU)


El estudio de las emociones en la práctica clínica expone su influencia en la relación médico-paciente. Este artículo presenta los resultados de una investigación cualitativa, cuyo objetivo fue explorar el discurso de los médicos sobre las propias emociones en la atención a pacientes. De las entrevistas con ocho médicos se extrajeron los temas de análisis: función apostólica y el entendimiento de la emoción, defensas y estrategias antes las emociones y la compañía de inversión mutua: frustración y consecuencias. Los médicos demostraron poca percepción de la influencia de las emociones sobre la propia actuación, a pesar de haber relatado situaciones en las que fueron afectados por ellas. Revelaron sufrimiento debido a la dificultad de elaborar contenidos emocionales, usando el distanciamiento afectivo como un mecanismo de defensa. Se concluyó que la gestión emocional por parte del médico maximiza recursos en la interacción con el paciente, lanzando nuevas perspectivas para la actuación médica.(AU)

El objetivo de este artículo se basa en el principio de que el ser humano aprende de su pasado, de ahí que contar con la evolución histórica de la materia ‘Psicología médica’ enriquece la conciencia de quienes la imparten y fortalece sus raíces y las del Departamento de Psiquiatría y Salud Mental de la Facultad de Medicina de la Universidad Nacional Autónoma de México que la fundó, además de que establece las bases para proteger y fomentar su importancia dentro del currículo de la carrera de médico y da a conocer la materia a otras universidades que también consideran que es una asignatura indispensable en la formación de los médicos. Basándose en el análisis de diversas fuentes de información, se reconoce la importancia de sus profesores, de sus programas y del apoyo que brinda a la formación de los futuros médicos


This article is based on the principle that human beings learn from their past, hence having the historical evolution of ‘Medical Psychology’ course, enriches the conscience of those who teach it, strengthens their roots and those of the Department of Psychiatry and Mental Health of the Faculty of Medicine of the National Autonomous University of Mexico who founded it and, lays the basis for protecting and promoting its importance within the curriculum of medical career and makes it known to other universities that also consider that it’s indispensable in the formation of their doctors. Based on the analysis of different sources of information, it recognized the importance of their teachers, their programs and support provided for the training of future doctors

The present study aims at overviewing the different stages of the education of medical psychology in Hungary. In the perspective of the achievements of cognitive and affective neuroscience as well as developmental psychology, we intend to define the roots of doctor-patient relationship, certain determining factors of diagnosis forming and therapy efficiency as well as the essential components of empathy. We attest to the fact that the current psychosomatic medicine with its evidence-based achievements is a valid component and constitutive pillar of everyday medical care. The integrated university level instruction of medical psychology and behavioral sciences aims at consolidating this perspective. We also emphasize that the ill body hinders adequate psychological functioning and mental disorders provide risk factors concerning somatic illnesses. Through spreading health psychology perspective, we support the concept that in everyday medical care, health protection and the development of health conscious behavior relies on responsible, informed and committed decision-making patients as well as on similarly educated extended medical care teams. Medical humanity is the soul of health education. Beginning medical students are taught various aspects of basic medicine, such as biochemistry, anatomy, and immunology. However, cultivation of the humanistic aspects of medicine has received increasing attention in recent decades. We performed a comparison study based on a literature search and our experience with medical humanistic courses in Western and Chinese medical colleges. We found both similarities and disparities in humanities courses offered in Western medical colleges and Chinese medical colleges. The delivery of humanities courses, such as medical sociology, medical ethics, medical psychology, and medical history, is widespread and helps to prepare students for their transformation from medical students to skilful medical professionals. Both Western and Chinese medical colleges offer a variety of medical humanistic courses for undergraduate students. Although Chinese medical humanistic education has undergone major changes, it still requires improvement and educators can learn from Western practice. We hope that our analysis will contribute to education reforms in the medical field.

RESUMO

A psicologia está presente nos cursos médicos brasileiros desde o início do século XIX. Estudo investiga configurações e contribuições da psicologia na formação médica brasileira, a partir da análise das ementas disponíveis publicamente em sites de instituições de ensino superior. Estudo de análise documental e abordagem qualitativa. Resultados indicam que os conteúdos de psicologia estão colocados como secundários; referidos em ementas de disciplinas com carga horária reduzida ou pulverizados como subtema. Revela uma psicologia comprometida com a ideia de psiquismo individual, de transtornos mentais que não alcançam as proposições das Diretrizes Nacionais para a formação do profissional de saúde que articulam a psicologia ao ideário da promoção da saúde


Psychology has been present in Brazilian medical courses since the nineteenth century. This study investigates how psychology is presented in the medical education, based on the analysis of the curricula available publicly on websites of higher education institutions. This is a documentary analysis with a qualitative approach. The results indicate that the contents of psychology are placed as secondary; referred to in syllabus with reduced working hours or sprayed as subtopics. It reveals a psychology committed to the idea of individual psychism, of mental disorders that do not reach the propositions of the National Guidelines for the training of health professionals that articulate psychology to the idea of promoting health


La psicología está presente en los cursos médicos brasileños desde el inicio del siglo XIX. El estudio investiga configuraciones y contribuciones de la psicología en la formación médica brasileña, a partir del análisis de los menús disponibles públicamente en sitios de instituciones de enseñanza superior. Estudio de análisis documental y enfoque cualitativo. Los resultados indican que los contenidos de psicología se colocan como secundarios; que se refiere en los menús de asignaturas con carga horaria reducida o pulverizadas como subtema. Revela una psicología comprometida con la idea de psiquismo individual, de trastornos mentales que no alcanzan las proposiciones de las Directrices Nacionales para la formación del profesional de salud que articulan la psicología al ideario de la promoción de la salud

A avaliação da dor pode variar entre médicos e pacientes. Para compreender os sentidos que médicos atribuem à dor de seus pacientes, seis médicas foram entrevistadas e seus relatos submetidos a análise qualitativa, pelo método fenomenológico de Giorgi. Os resultados evidenciaram que a estrutura do sentido da dor se constitui centralmente pela ambiguidade, que atravessa não apenas o seu sentido conceitual, mas também a clínica e a intervenção. As análises indicam que as dificuldades enfrentadas pelos médicos na avaliação da dor são intensificadas pela falta de evidências objetivas e por seu caráter ambíguo e subjetivo, dependente do relato. Indicam também conflito entre a clínica e os paradigmas que sustentam explicações fisiológicas da dor. Conclui-se que o paradigma organicista é questionado pelo fenômeno doloroso que exige avaliação fundamentada em uma relação empática, na qual o critério avaliativo se sustente na confiança no relato do paciente e não apenas em dados objetivos


The pain evaluation may differ between physicians and patients. To understand the meanings doctors give to their patient's pain, we interviewed six physicians, and their reports were analyzed through Giorgi´s phenomenological qualitative method. The results revealed ambiguity as central to the pain meaning structure and it is present not only in its conceptual definition but also in its clinics and intervention. The analyses show that the absence of material evidence of pain and its ambiguous and subjective nature, subordinated to patient's reports, intensifies the difficulties faced by physicians in pain evaluation. The analyses also indicate a conflict between the paradigms that support physiological explanations of pain and its clinical manifestations. We conclude that the painful phenomenon defies the organicist paradigm and requires an empathetic evaluation, in which evaluative criteria must be the confidence in patient's reports not just in objective data


La evaluación del dolor puede variar entre médicos y pacientes. Para comprender los sentidos que los médicos atribuyen al dolor de sus pacientes, seis médicas fueron entrevistadas y sus relatos sometidos al análisis cualitativo del método fenomenológico de Giorgi. Los resultados evidenciaron que la estructura del sentido del dolor se constituye centralmente por la ambigüedad, que atraviesa no sólo su concepto, sino también la clínica y la intervención. Los análisis indican que las dificultades enfrentadas en la evaluación del dolor son intensificadas por la falta de evidencias objetivas y por su carácter ambiguo y subjetivo, dependiente del relato. También indican conflicto entre la clínica y los paradigmas que sustentan explicaciones fisiológicas del dolor. Se concluye que el paradigma organicista es cuestionado por el fenómeno doloroso que exige evaluación fundamentada en una relación empática, donde el criterio de evaluación se sustenta en la confianza en el relato y no solo en datos objetivos

El interés en la creatividad ha aumentado considerablemente en las últimas décadas, lo que supone un cambio notable a un área de investigación científica que en el pasado reciente fue considerada marginal o irrelevante científicamente. Sin embargo, este crecimiento en su popularidad, no siempre se acompaña del rigor exigible a las disciplinas científicas. El objetivo de este trabajo es intentar exponer algunas de estas dificultades y los avances en el campo del estudio de la creatividad como área de interés para la Psicología


Interest in creativity has increased considerably in recent decades, this represents a significant change to an area of scientific research that not so long ago was considered marginal or scientifically irrelevant. However, this growth in popularity is not always accompanied by the rigor required by scientific disciplines. The objective of this work is to try to summarize some of these difficulties and the advances in the field of the study of creativity as an area of interest for Psychology

Se realizó una investigación de corte pedagógico, resultado del trabajo metodológico realizado por los docentes de la Disciplina Psicología Médica II para perfeccionar la preparación metodológica del claustro docente en el tratamiento al contenido Historia Psicosocial. Se aprovechó el sistema de contenidos básicos de Psicología Médica II vinculados con la asignatura Propedéutica médica y semiología clínica. Los principales resultados se encuentran en la falta de correspondencia entre el modelo de Historia Psicosocial que se imparte en la asignatura Psicología Médica II con el que reciben los estudiantes en la asignatura Propedéutica, la desactualización de la bibliografía. El sistema de clases metodológicas desarrolladas dio respuesta al problema conceptual metodológico, se actualizaron los contenidos del tema Historia Psicosocial, se reelaboró la guía de la Clase Práctica y modificó la dinámica de la clase, con lo que se logró el incremento de la satisfacción de los profesores y estudiantes(AU)


A pedagogical research, as result of the methodological work carried out by the educational staff of Discipline Medical Psychology II to perfect the methodological preparation of the educational staff in the treatment to the contained Psychosocial History was carried out. It took advantage of the system of basic contents of Medical Psychology II linked with the subject medical Propedeutics and clinical semiology. The main results find in the fault of correspondence between the model of Psychosocial History that gives in the subject Medical Psychology II with which receive the students in the subject Propedeutics, the out-of-date of the bibliography. The system of methodological classes developed gave answer to the methodological conceptual problem, updated the contents of the subject Psychosocial History, rebuilded the guide of the Practical Class and modified the dynamics of the class, with what attained the increase of the satisfaction of the professors and students(AU)
