RESUMO

The proliferation of digital technology is progressively changing dentistry. Thanks to continual improvements in CAD/CAM devices and dental materials, it is possible nowadays to carry out a treatment plan for oral rehabilitations with fully digital approaches and noninvasive concepts. The availability of digital resources allows clinicians to increase the predictability of enhanced esthetics and good functional results. There is an increasing number of patients today who are affected by excessive tooth wear and may benefit from these kinds of treatments. This article provides a step-by-step documentation of a full-mouth rehabilitation performed with a digital approach and additive CAD/CAM composite resin restorations. An innovative functional evaluation is also documented and discussed. The initial situation was assessed and compared with the rehabilitation project through a snap-on device. After the intraoral adjustment and validation, the final rehabilitation was performed according to the information obtained in the provisional phase and digitally transmitted to the laboratory.

El aumento en la demanda de tratamientos de odontología estética ha conducido al desarrollo de una comunicación multidisciplinaria entre las diferentes especialidades para lograr el éxito. La sonrisa gingival es un problema estético frecuente que consiste en una exposición excesiva de la encía durante la sonrisa y cuyas modalidades de tratamiento van desde una gingivectomía hasta el levantamiento de un colgajo para la remoción de hueso excedente. Las restauraciones con carillas directas de resinas compuestas son adecuadas para la conservación y la mejora estética del sector anterior, con óptima estética y durabilidad, además de ser consideradas un tratamiento mínimamente invasivo debido a sus conceptos conservadores de preservar la estructura dentaria.El presente caso clínico describe el tratamiento de un paciente joven con sonrisa gingival y alteración en forma y color de los dientes anteriores, mediante procedimientos mínimamente invasivos tales como la gingivectomía y la rehabilitación con carillas directas de resinas compuestas, con el objetivo máximo de preservar la estructura dentaria y una mayor predictibilidad estético-funcional. The increasing demand of patients for esthetics dentistry treatments has led to the development of multidisciplinary communication between approaches among different specialties to achieve treatment success. The gummy smile is a frequent esthetic problem, which consists of excessive exposure of the gum when smiling. Treatment modalities range from gingivectomy to flap lift for the removal of excess bone. Restoration with direct veneers of composite resins is suitable for the preservation and esthetic improvement of the anterior sector, with optimal aesthetics and durability. In addition, minimally invasive treatment can be considered due to its conservative concepts of preserving the dental structure.The present case describes the treatment of a young patient with gummy smile and alteration in shape and color of the anterior teeth, through minimally invasive procedures such as gingivectomy and rehabilitation with direct veneers of composite resins with the objective of preserving the dental structure and greater esthetic - functional predictability. The present case report describes the treatment of a young patient with gummy smile. Purpose: To investigate the effect of full-mouth rehabilitation on the levels of Streptococcus mutans and Streptococcus sobrinus in plaque samples of children suffering from severe early childhood caries using the quantitative reverse transcription polymerase chain reaction method.
Methods: A dental examination was performed to determine the decayed, missing, and filled primary teeth and surfaces (dmft and dmfs, respectively) in three- to five-year-old children using the World Health Organization's caries diagnostic criteria. Supragingival plaque samples were collected from all tooth surfaces with a sterile universal scaler at baseline and six months after full-mouth rehabilitation. The Wilcoxon signed-rank test was used to compare the mean values of dmft and dmfs scores and the mean values of S. mutans and S. sobrinus levels between the pre- and post-treatment period.
Results: At six months, the mean relative quantification values of S. mutans and S. sobrinus showed significant reductions (P > 0.05) in the post-treatment samples but the mean dmft scores remained the same.
Conclusion: Full-mouth rehabilitation is effective in reducing levels of S. mutans and S. sobrinus, thus diminishing the cariogenic challenge in the oral cavity.

Los signos de desgaste y rechinamiento de los dientes están asociados con el bruxismo y se observan comúnmente en los consultorios dentales. En este reporte, las autoras describen un caso de bruxismo probablemente inducido por el medicamento anticonvulsivo Valpromed®, que fue tratado con éxito mediante una férula dental. El paciente llegó a la clínica declarando que deseaba mejorar su estética. En el examen clínico, se observó un desgaste moderado de las superficies oclusales de los molares asociado con antecedentes de rechinamiento nocturno, característico del bruxismo del sueño. Hace aproximadamente 5 meses, el paciente comenzó la terapia con Valpromed® por orden del neurólogo, debido a episodios de migraña. El medicamento se prescribió para evitar el riesgo de convulsiones ya que el paciente había sufrido de neurocisticercosis durante la infancia. Según los consejos del especialista en rehabilitación oral y la literatura revisada, las autoras concluyeron que el bruxismo podía atribuirse a la terapia con el medicamento anticonvulsivo. Para el tratamiento, se escogió una técnica mínimamente invasiva. Aunque el paciente todavía está en controles, la férula dental parece ser un enfoque prometedor para el tratamiento de esta afección. Signs of tooth wear and grinding are associated with bruxism, which is commonly observed in dental offices. Here, the authors describe a case of bruxism probably induced by the anti-seizure drug Valpromed®, that was successfully treated with a dental splint. A man was attended in the dental clinic for improvement of dental aesthetics. On clinical examination moderate overwear of the occlusal surfaces of molars was observed associated with a history of nightly grinding, characteristic of sleep bruxism. Approximately 5 months previously, the patient had started therapy with Valpromed® due to episodes of migraine. The drug had been prescribed by a neurologist to prevent convulsions since the patient had had neurocysticercosis during childhood. Based on the advice of an oral rehabilitation specialist and the literature reviewed, it was concluded that the presence of bruxism in this patient may be attributed to the therapy with the anti-seizure medication. Although the patient is still under controls, the dental splint seems to be a promising approach for the treatment of this condition. The patient was treated with a minimally invasive dental splint and is monitored periodically, showing good results. (AU)

A busca por restaurações estéticas e duradouras com resultados mais semelhantes à dentição natural está cada vez mais evidente, a adequação dos materiais e das técnicas foram a saída para atender à exigência estética e mecânica dos pacientes. O objetivo desse trabalho é relatar um caso clínico no qual aborde uma reabilitação oral complexa por meio de próteses fixas metalocerâmicas. A paciente apresentava problemas estéticos e funcionais e ao exame clínico foi contatado próteses fixas metalocerâmicas desadaptadas e desgastes severos nas duas arcadas. O desgaste resultou em uma desarmonia no seu sorriso, além de uma discrepância no contorno gengival em incisivos laterais, caninos e pré-molares. No planejamento clínico realizou-se gengivoplastia para adequar contorno gengival e próteses fixas do tipo metalocerâmica dos elementos 14 ao 26. Concluímos que as restaurações com as coroas metalocerâmicas se mostraram uma excelente alternativa para reabilitações protéticas amplas, atendendo todas as expectativas estéticas e funcionais da paciente possibilitando uma melhora significativa em sua qualidade de vida(AU)


The search for aesthetic and lasting restorations with results more similar to natural dentition is increasingly evident, the adequacy of materials and techniques have been the output to meet the aesthetic and mechanical demands of patients. The aim of this paper is to report a clinical case in which a complex oral rehabilitation using fixed metalloceramic prostheses is addressed. The patient presented aesthetic and functional problems and the clinical examination was contacted maladaptive fixed metalloceramic prostheses and severe wear on both arches. The wear resulted in a disharmony in his smile, as well as a discrepancy in the gingival contour in lateral, canine and premolar incisors. In the clinical planning, gingivoplasty was performed to adapt the gingival contour and the metalloceramic fixed prostheses of elements 14 to 26. We conclude that the restorations with the metalloceramic crowns proved to be an excellent alternative for broad prosthetic rehabilitation, meeting all the aesthetic and functional expectations of the patient. enabling a significant improvement in their quality of life(AU)

The purpose of this case report is to present implant-retained maxillary and mandibular complete overdentures in a patient with Marfan syndrome. The patient initially presented with generalized periodontitis (stage IV, grade C). Due to the progressive nature of periodontal disease, the patient elected to have implant-retained maxillary and mandibular complete dentures. Bilateral maxillary sinus augmentation was performed 6 months before full-mouth extraction, alveoloplasty, and immediate implant placement. Maxillary and mandibular immediate overdentures were delivered. After 4 months of healing, the final overdenture was fabricated. The patient was seen regularly throughout the healing process for peri-implant maintenance. Soft-tissue grafts were completed to increase the thickness of the mucosa around the implants. The patient has been followed for 2 years and is functioning well without major complications. For patients with Marfan syndrome, implant-retained prostheses are a viable treatment option in the presence of a failing dentition.

This research study aimed at evaluating the content related to pediatric dentistry addressed in civil service examinations in the state of Paraíba through the analysis and classification of questions. Questions related to pediatric dentistry addressed in civil service examinations for the dentist position in the Family Health Program, held in the state of Paraíba, between the years 2001 and 2017 were evaluated. Across the state, 107 civil service examinations were identified and, when applying the exclusion criteria, 9 identical tests were found, consequently, being removed, totaling 98 examinations. After the analysis, 3,949 questions were obtained and only 54 questions were related to pediatric dentistry, representing 1.36% of total questions. The questions were classified according to the topic and the cognitive level according to the BLOOM's revised taxonomy. Results showed that topics related to oral rehabilitation and surgical management, including questions regarding anesthesia, surgery, pulp therapy, endodontic treatment, operative and restorative dentistry, dental materials, traumatic injuries and prosthesis/rehabilitation corresponded to 42% of the questions. The most prevalent cognitive domains were recall and analyze, corresponding to 45% and 29%, respectively. Thus, questions concerning oral rehabilitation and surgical management were the most addressed in civil service examinations, in which the domains remember and analyze were predominant. There were no questions requiring complex cognitive domain such as assessing and creating.


Esse trabalho teve como objetivo verificar o conteúdo de Odontopediatria abordado em concursos públicos da Paraíba através da análise e classificação de questões. Foram extraídas questões que versavam sobre os assuntos da área de Odontopediatria de certames disponíveis realizados e encerrados, de todas as cidades do estado da Paraíba para o cargo de cirurgião-dentista do Programa Saúde da Família entre os anos de 2001 e 2017. De todo o estado foram coletados 107 certames e ao aplicar os critérios de exclusão, foram removidas 9 provas idênticas, totalizando 98. Após análise destas, obteve-se 3949 questões e dessas apenas 54; 1,36% do total, versavam sobre Odontopediatria. As questões foram classificadas de acordo com o tópico genérico de que se tratavam e o nível cognitivo que exigiam de acordo com a Taxonomia Revisada de BLOOM. Os resultados mostraram que o tópico referente à reabilitação bucal e manejo cirúrgico, foi o de maior prevalência correspondendo a 42% das questões e as dimensões cognitivas mais prevalentes foram lembrar e analisar que corresponderam a 45% e 29%, respectivamente. Foi possível concluir que questões referentes à reabilitação bucal e manejo cirúrgico foram as mais abordadas em concursos públicos no contexto da Odontopediatria e que as dimensões lembrar e analisar foram as de maior prevalência. Não houve ocorrência de questões que exigiam dimensões cognitivas mais complexas, como avaliar e criar.

The development of technologies that include face scanning and dental software has improved workflows in dentistry. Digital files make it possible to create a 3-dimensional virtual image of the patient that helps the clinician plan treatment and make decisions, reducing uncertainty and improving communication. This report describes the complete-mouth rehabilitation of a patient with severe tooth wear. The treatment adopted a minimal intervention approach, applying adhesive veneers in areas where the tooth structure was affected and used a computer-aided design and computer-aided manufacturing (CAD-CAM) workflow to prepare nanoceramic composite resin materials with a high filler loading.

Head and neck cancer occupy the seventh place among the most common neoplasms in the world, with an annual incidence of approximately 640,000 new cases. Radiotherapy treatment has been highlighted, because there is greater tissue preservation, possibility of regression of the disease, decreased tumor volume, prevention of possible tumor recurrences and metastases, and even total cancer cure, but patients undergoing radiotherapy are at high risk of osteoradionecrosis if they perform surgical interventions in the region, such as dental implants and dental extraction, making prosthetic rehabilitation of partially edentulous patients after treating oral cancer something complex and often omitted; although it is a crucial factor in restoring oral function. Thus, the aim of the present study was to describe the planning, treatment and follow-up of a twelve-month prosthetic approach to rehabilitate the lower arch shape and function of a partially edentulous patient undergoing head and neck radiotherapy with a full denture mucus supported and tooth retained. After twelve months of follow-up, it was possible to evidence the functional and aesthetic clinical success, with preserved occlusal pattern, soft and bone tissue around and dental roots. (AU)


O câncer de cabeça e pescoço ocupa o sétimo lugar entre as neoplasias mais comuns do mundo, com uma incidência anual de aproximadamente 640.000 novos casos. O tratamento com radioterapia tem se destacado, pois há maior preservação tecidual, possibilidade de regressão da doença, diminuição do volume tumoral, prevenção de possíveis recorrências e metástases tumorais e até cura total do câncer, mas os pacientes submetidos à radioterapia apresentam alto risco de osteorradionecrose se realizar intervenções cirúrgicas na região, como implantes dentários e extração dentária, fazendo com que a reabilitação protética, de pacientes parcialmente dentados depois de tratar o câncer bucal, algo complexo e frequentemente omitido; embora seja um fator crucial para o reestabelecimento da função oral. Assim, o objetivo do presente estudo foi descrever o planejamento, tratamento e acompanhamento de doze meses de uma abordagem protética para reabilitar forma e função do arco inferior de um paciente parcialmente desdentado, submetido a radioterapia de cabeça e pescoço, com o uso de uma prótese total mucossuportada e dente retida. Após doze meses de acompanhamento, foi possível evidenciar o sucesso clínico funcional e estético, com padrão oclusal, rebordo alveolar e remanescentes dentários preservados (AU)

Introdução:Aenorme dificuldade de deglutição e fala apresentada em pacientes que sofreram maxilectomiatotal ou parcial por consequência de patologias bucais, afeta grande parte da população que realizou tal procedimento. A alternativa de minimizar o impacto causado pela sequela deixada após cirurgia em maxila está na confecção de um obturador palatino, que tem como objetivo obstruir a comunicação oronasal.Objetivo:Este trabalho objetivou descrever um relato de caso evidenciando o passo a passo clínico e laboratorial de uma prótese total obturadorae a satisfação do paciente com a mesma.Método:Paciente do gênero masculino, maxilectomizado há seisanos devido a presença de um CarcinomaAdenóideCístico foi reabilitado com prótese total obturadora superior e prótese total convencional inferior. A reabilitação superior seguiu os mesmos passos de uma prótesetotal convencional, com exceção da moldagem funcional, na qual foi feita a moldagem da comunicação bucosinusal com silicona de condensação antes da moldagem do selado periférico e moldagem funcional propriamente dita. Na etapa laboratorial, foi necessáriaa realização de três prensagens a fim de deixar a prótese oca na porção obturadora.Resultados:O tratamento restabeleceu a dimensão vertical perdida, proporcionou uma oclusão adequada, se mostrou melhor esteticamentee o paciente mostrou-se extremamente satisfeito com a reabilitação.Conclusões:Como conclusão, obteve-se uma prótese total obturadora superior com qualidades funcionais e estéticas satisfatórias. Ademais, ao comparar com a antiga, a novamostrou-se satisfatória de acordo com a percepção do usuário epossui um menor peso, favorecendo uma melhor retenção (AU).


Introduction:The enormous difficulty in swallowing and speaking presented in patients who underwent total or partial maxillectomy as a result of oral pathologies affects a large part of the population that performed this procedure. The alternative of minimizing the impact caused by the sequelae left after maxillary surgery is by making a palatal obturator, which aims to obstruct oronasal communication.Objective:This study aimed to describe a case report highlighting the clinical and laboratory step by step of a complete obturator prosthesis and patient satisfaction with it. Methods:A male patient who had been maxillectomized for 6 years due to the presence of a cystic adenoid carcinoma was rehabilitated with superior obturator total denture and inferior conventional total denture. The superior rehabilitation followed the same steps of a conventional total prosthesis, except for the functional molding, in which the bucosinusal communication with condensation silicon was molded before the peripheral sealing molding and the functional molding itself. In the laboratory stage, it was necessary to perform three presses in order to leave the hollow prosthesis in the obturator portion.Results:The treatment restored the lost vertical dimension, provided an adequate occlusion,was better aesthetically and the patient was extremely satisfied with the rehabilitation.Conclusions:In conclusion, a superior total obturator prosthesis with satisfactory functional and aesthetic qualities was obtained. Moreover, when compared to the old one, the new one was satisfactory according to the user's perception and has a lower weight, favoring a better retention (AU).


Introduccion: La enorme dificultad para tragar y hablarpresentada en pacientes que han sufrido una maxillectomía total o parcial como resultado de patologías orales afecta a gran parte de la población que realizó este procedimiento. La alternativa de minimizar el impacto causado por las secuelas que quedan después de la cirugía maxilar es hacer un obturador palatino, que tiene como objetivo obstruir la comunicación oronasal. Objetivo: Este estudio tuvo como objetivo describir un informe de caso que muestra la clínica y el laboratorio paso a paso de una prótesis obturadora completa y la satisfacción del paciente con ella. Método:Un paciente masculino que había sido maxillectomizado durante seis años debido a la presencia de un carcinoma adenoide quístico fue rehabilitado con una prótesis obturadora total superior y una prótesis total inferior convencional. La rehabilitación superior siguió los mismos pasos de una prótesis total convencional, excepto el moldeado funcional, en el cual la comunicación bucosinusal con silicio de condensación se moldeó antes del moldeado de sellado periférico y el moldeado funcional en sí. En la etapa de laboratorio, fue necesario realizar tres prensas para dejar la prótesis hueca en la porción del obturador. Resultados:El tratamiento restableció la dimensión vertical perdida, proporcionó una oclusión adecuada, fue mejor estéticamente y el paciente quedó extremadamente satisfecho con la rehabilitación. Conclusiones:Como conclusión, se obtuvo una prótesis obturadora superior con cualidades funcionales y estéticas satisfactorias. Además, en comparación con el anterior, el nuevo fue satisfactorio de acuerdo con la percepción del usuario y tiene un peso menor, lo que favorece una mejor retención (AU).

El desgate dental es la pérdida de tejido duro de etiología múltiple y puede deberse a factores funcionales o parafuncionales. Dependiendo de esto, se debe elegir un tratamiento restaurador según la gravedad del desgaste, ya que puede resultar en la pérdida de dimensión vertical de la oclusión (DVO), sensibilidad dental y disminución de la calidad de vida. En estos casos, la elección de la estrategia de restauración debe seguir protocolos de tratamiento basados en la evidencia, que incluyan técnicas de restauración directas e indirectas, así como tratamientos mínimamente invasivos a partir de resinas compuestas que surgen como una alternativa a las restauraciones convencionales de cerámica. El presente caso clínico describe el tratamiento de un paciente con desgaste dental severo mediante la rehabilitación oral con resinas compuestas a través de técnicas directas e indirectas, con el objetivo de preservar al máximo la estructura dentaria y tener una mayor predictibilidad estético-funcional. Dental wear inolves the loss of hard tissue of multiple etiology and may be due to functional or parafunctional factors. Accordingly, restorative treatment should be chosen considering the severity of the wear, since it can result in the loss of vertical dimension of the occlusion (DVO), dental sensitivity and a reduction in the quality of life. The choice of a restoration strategy should follow evidence-based treatment protocols, including direct and indirect restoration techniques such as minimally invasive treatments based on composite resins which are being increasingly used as an alternative to conventional ceramic restorations. The present clinical case describes the treatment of a patient with severe dental wear who underwent oral rehabilitation with composite resins using direct and indirect techniques with the aim of preserving tooth structure and greater aesthetic - functional predictability. RATIONALE: Full-mouth rehabilitation of patients with bruxism and severely worn dentition poses a great challenge to clinicians. Several treatment planning methods and restorative materials are used to treat tooth wear in modern dentistry. Clinicians should be able to select the most suitable treatment planning methods and materials for individual patients depending on their specific situation. PATIENT CONCERNS: A 47-year-old male was referred for evaluation of a severely worn dentition. DIAGNOSES: Clinical and radiographic evaluation revealed tooth wear in the entire dentition. The interocclusal distance at rest was 4 mm, and the patient had a parafunctional habit of bruxism. INTERVENTIONS: A digital smile design was used to formulate a treatment plan. Full-mouth rehabilitation was performed using a combination of conventional and digital materials and methods. OUTCOMES: The full-mouth restoration showed satisfactory functions and esthetics. No complications were observed in the restorations, supporting tissues, and temporomandibular joints during 2-year follow-up. LESSONS: In clinical practice, it is important to determine the optimal combination of the available methods for treatment planning. This case report details the formulation of a unique treatment plan for the dental rehabilitation of a severely worn out dentition, which is considered challenging due to the limitations imposed by biological tissues and restorative materials. The use of conventional and digital tools for treatment planning, patient education, and treatment execution was demonstrated.

Background: The aims of this study are to present sociodemographic and familial characteristics, clinical and systemic findings, dental treatment needs, and concomitant dental anomalies in patients with amelogenesis imperfecta (AI) and to evaluate time-varying conditions in these long-term follow-up patients. Materials and Methods: Records of patients with AI who were examined in the Department of Pediatric Dentistry between 1999 and 2017 were reviewed. Information about sociodemographic characteristics, history of AI and consanguinity in family, systemic conditions, reasons for referral to the clinic, oral hygiene habits and gingival health, occlusion findings, and performed treatments were gathered. Dental anomalies in radiographs were also evaluated. Baseline and final situations of the patients were assessed. Statistical analyses were performed. Results: Of 75 patients aged 3-15 years with follow-ups up to 12 years, 34 had AI in their families and 15 were born from consanguineous marriages. Nephrocalcinosis has been observed in 5 patients. Main reasons for referral to the clinic were related to esthetic and hypersensitivity concerns. Twenty-two patients had gingivitis, and during follow-up process, gingival problems could not be completely prevented due to poor oral hygiene habits. Vertical dimension loss, open-bite, and cross-bite were seen in 16, 15, and 10 patients, respectively. Of the patients, 63% experienced restorative, 33% stainless steel crown, 17% endodontic, 8% prosthetic treatments, and 24% had retreatment needs. Concomitant dental anomalies were dens invaginatus, taurodontism, ectopic eruption, delayed eruption, hypodontia, and pulpal calcification. Conclusion: Early diagnosis and interventions considering the time-varying conditions with long-term follow-ups provide significant improvements in clinical maintenance of patients with AI.

Resumen El síndrome 3M es un desorden autosómico recesivo, heterogéneo, poco común, llamado así por los tres investigadores que lo describieron por primera vez, Miller, Mckusck y Malvaux. Las características principales son retraso en el crecimiento prenatal y postnatal severo, dismorfias faciales y anomalías radiológicas. En sus manifestaciones estomatológicas presentan dolicocefalia, abombamiento frontal, cara triangular, labios gruesos, cejas pobladas, hipoplasia maxilar severa, facies melancólicas, retraso en la erupción dental severa y falta de crecimiento del macizo facial. El objetivo del artículo es la descripción de casos gemelares de 8 años y 6 meses de edad, que acuden a la Clínica de Atención Dental Avanzada de la Universidad de Monterrey, con confirmación del síndrome 3M por prueba sanguínea a los 4 años de edad por parte del Servicio de Genética del Hospital Universitario. Se realiza rehabilitación bucal de ambas niñas, y, posteriormente, son referidas a interconsulta con ortodoncia interceptiva. MÉD.UIS.2019;32(2): 59-65


Abstract 3M syndrome is very rare, it's a heterogeneous autosomal recessive disorder named after 3 researches who described it for the first time, Miller, Mckusck and Malvaux. Whose main characteristic are; delayed prenatal growth, severe postnatal growth, facial dysmorphia, radiological abnormalities, presence of dolichocephaly, frontal bulging, triangular face, thick lips, raised eyebrows, severe maxillary hypoplasia, melancholic facies, delayed severe dental eruption, lack of facial mass growth. The objective of the article is the description of twin cases that come to the Advanced Dental Care Clinic of the University of Monterrey; 8 years 6 months old, with confirmation of the 3M syndrome, with a blood test at 4 years of age by the Genetics Service of the University Hospital. RESUMO

Acromegaly is a rare disease that can have serious consequences. This disease is not widely known by stomatologists, although cranio-facial manifestations are important and rapid changes in the oral cavity are frequent and sometimes dramatic. The prosthetic management of patients with acromegaly is not easy because it is necessary to wait for the stabilization of the disease. If this is not the case, the bone changes prevent the stability of prostheses over time. Moreover, because of bone remodeling, some treatments are contra-indicated, which limits our therapeutic choices. Through this clinical case, we will focus on the oral manifestations of patients with acromegaly and the method of prosthetic management.

Introdução:A avaliação do sorriso deve abranger diversos aspectos desde a composição facial até características morfológicas dos elementos dentários e, dessa forma, se qualquer um desses aspectos forem afetados, o sorriso se torna esteticamente inadequado.Objetivo:apresentar um caso clínico cujo plano de tratamento foi composto pela confecção de facetas diretas em resina composta dos elementos antero-superiores a fim de devolver a estética adequada para a paciente. Descrição do caso:Paciente, 16 anos, do sexo feminino, apresentava má oclusão (Classe III de Angle) compensada ortodonticamente, agenesia de incisivos laterais e inversão dental dos elementos 23 e 24. A resolução estética deste caso utilizou a técnica direta com resina composta por meio do protocolo correto no quesito proporção áurea e mimetização.Conclusão:As restaurações diretas em resina composta são uma solução simples, efetiva e prática na intervenção de desarmonias de forma e tamanho dental e, portanto, foi possível reanatomizar os dentes anteriores devolvendo estética, função e satisfação ao paciente (AU).


Introduction:Smile evaluation should cover several aspects from facial composition to morphological characteristics of dental elements and, therefore, if any of these aspects are affected, the smile becomes aesthetically inadequate.Objective:to present a clinical case whose treatment plan was composed by the making of dental veneers in composite resins of the upper and lower elements in order to obtain a suitable aesthetic.Case description:A 16-year-old female patient presents malocclusion (Class III of Angle) orthodontically compensated, agenesis of lateral incisors and dental inversion of elements 23 and 24. The aesthetic resolution of this case used the direct technique with composite resin by means of the correct protocol in relation to golden proportion and mimicry.Conclusion:Direct restorations in composite resin are a simple, effective and practical solution in the intervention of disharmonies of form and dental size and, therefore, it was possible to reanatomize the anterior teeth restoring aesthetics, function and patient satisfaction (AU).

Extraction is commonly presented as a treatment alternative for restorable teeth, and patients are biased to choose it for a variety of reasons. However, without subsequent rehabilitation, patients' health, function, and esthetics could be compromised. This article explores problematic outcomes where extraction was offered as a treatment option when teeth were restorable; a case report example demonstrates this issue. The case involved a patient who was diagnosed with moderate to severe periodontitis and had severe attrition and lingual erosion on his maxillary teeth. Despite being given the option of full-mouth rehabilitation, ie, periodontal and restorative treatment, due to financial reasons the patient chose to have all maxillary teeth extracted and receive a maxillary denture. Respecting the patient's autonomy, the dental team performed surgical extraction of the maxillary teeth and an alveoloplasty and delivered an interim denture. The patient did not adapt well to the denture, and several postoperative issues transpired, which required unanticipated surgical procedures. Dissatisfied with the treatment, the patient continued to need dental appointments more than a year after the extractions, and his oral health, function, and esthetics have still not been restored. The dental community must educate patients regarding extraction being an irreversible, last-resource procedure, and mainly indicated only when teeth restoration is not possible. Presenting extraction together with more conservative options for restorable teeth may contribute to patients' misconceptions that it is a treatment alternative as good as any other.

ABSTRACT Introduction: The cemento-ossifying fibroma is a benign bone neoplasm that affects mainly the female sex during the third or fourth decades of life. This lesion has a slow growth and the treatment is the surgical removal of the tumor, because radiotherapy is not indicated. Objective: to describe and discuss a case of central cemento-ossifying fibroma. Case report: A 41-year-old female patient sought dental treatment due to tooth mobility in the anterior region of the mandible. In this region of the mandible, an increase in volume was noted with a firm consistency on palpation, covered by normal mucosa, and displacement of teeth. Radiographically, the presence of an extensive lesion in this region was observed. An incisional biopsy was performed which led to the final diagnosis of central cemento-ossifying fibroma. Subsequently the tumor was completely removed. An autogenous bone graft was performed and four osseointegrated implants were installed to rehabilitate the patient. Implant-supported prostheses (lower arch) and a complete denture (upper arch) were installed to restore esthetics and function of the patient's oral cavity. Conclusion: Despite being a benign tumor, the central cemento-ossifying fibroma caused functional and aesthetic damage to the patient and required a complex rehabilitation treatment. After approximately 5 years of tumor removal, there was no recurrence of the lesion or signs of peri-implant or periodontal diseases, evidencing the success of the treatments(AU)


RESUMEN Introducción: El fibroma cemento-osificante es una neoplasia ósea benigna que afecta principalmente al sexo femenino durante la tercera o cuarta décadas de la vida. Esta lesión tiene un crecimiento lento y el tratamiento es la extirpación quirúrgica del tumor, porque la radioterapia no está indicada. Objetivo: Describir y analizar un caso de un fibroma cemento-osificante central. Presentación caso: Paciente de 41 años de edad buscó tratamiento dental debido a la movilidad dental en la región anterior de la mandíbula. En esta región de la mandíbula se observó un aumento de volumen con una consistencia firme a la palpación, cubierta por la mucosa normal y desplazamiento de los dientes. Radiográficamente, se observó la presencia de una lesión extensa en esta región. Se realizó una biopsia incisional, que indicó el diagnóstico final del fibroma cemento-osificante central. Después el tumor fue completamente extirpado. Se realizó un injerto óseo autógeno y, posteriormente, se instalaron cuatro implantes osteointegrados para rehabilitar al paciente. Se instalaron prótesis soportadas por implantes (arco inferior) y una dentadura (arco superior) para restablecer la estética y la función de la cavidad bucal de la paciente. Conclusiones: A pesar de ser un tumor benigno, el fibroma cemento-osificante central causó daños funcionales y estéticos al paciente y requirió un complejo tratamiento de rehabilitación. Después de 5 años de la extirpación del tumor, no hubo recurrencia de la lesión. Además, no hubo signos de enfermedades periimplantarias y/o periodontales, lo que demuestra el éxito del tratamiento(AU)

Objetivo: Presentar una técnica quirúrgica de disyunción con bisturí piezoeléctrico y expansión de rebordes delgados en maxilar inferior que permita la colocación de implantes en la posición tridimensional correcta en el mismo acto quirúrgico. Casos clínicos: Se presenta la resolución de dos casos clínicos de pacientes que acudieron a la Cátedra de Odontología Integral Adultos de la Facultad de Odontología de la Universidad de Buenos Aires. Ambos requerían la colocación de implantes en sectores posteroinferiores. Las imágenes tomográficas mostraban adecuada altura del reborde alveolar pero deficiente espesor para la colocación de implantes en la posición tridimensional correcta. Se decidió realizar una técnica de disyunción horizontal del reborde alveolar con bisturí piezoeléctrico, expansión del reborde y colocación de implantes e injerto óseo particulado de forma simultánea. Luego de 3 meses, se efectuó la segunda cirugía para iniciar la rehabilitación protésica. Conclusión: La técnica de disyunción horizontal del reborde alveolar mediante el uso del bisturí piezoeléctrico permitió ubicar al implante en la posición tridimensional correcta dentro del tejido óseo nativo sin la necesidad de realizar regeneraciones complejas en rebordes alveolares delgados (AU)


Aim: Present the horizontal split crest technique with a piezoelectric scalpel and the immediate implant placement in the correct tridimensional position, in thin mandibular alveolar ridges. Cases report: The surgical resolutions of 2 clinical cases of patients who attended the Adult Integral Dentistry Chair of the Faculty of Dentistry of the UBA, are presented. Both patients required implant placement in posterior madibular residual ridge. The tomographic images showed adequate height of the alveolar ridge but poor thickness for implants placement in a correct three-dimensional position. It was decided to perform a horizontal split crest technique of the alveolar ridge with piezoelectric scalpel, ridge expansion, and immediate implant placement with xenograft. After 3 months, the second surgery was performed to start with the prosthetic rehabilitation. Conclusion: The horizontal split crest technique using a piezoelectric scalpel, allowed immediate implant placement in the correct tridimensional position within native bone tissue, avoiding a guided bone regeneration technique, in these cases with thin alveolar ridges (AU)
