BACKGROUND: Atypical eye gaze on emotional faces is a core feature of alexithymia. The inferior frontal gyrus (IFG) is considered to be the neurophysiological basis of alexithymia-related emotional face fixation. Our aim was to examine whether anodal high-definition transcranial direct current stimulation (HD-tDCS) administered to the right (r)IFG would facilitate eye gaze of emotional faces in alexithymia individuals. METHOD: Forty individuals with alexithymia were equally assigned to anodal or sham HD-tDCS of the rIFG according to the principle of randomization. The individuals then completed a free-viewing eye tracking task (including happy, sad, and neutral faces) before and after 5 consecutive days of stimulation (twice a day). RESULTS: The results showed that twice a day anodal HD-tDCS of the rIFG significantly increased the fixation time and fixation count of the eye area on happy and neutral faces, but there was no significant effect on sad faces. According to the temporal-course analysis, after the intervention, the fixation time on neutral faces increased significantly at almost all time points of the eye tracking task. For happy faces, the improvement was demonstrated between 500 and 1000 ms and between 2500 and 3500 ms. For sad faces, the fixation time improved but not significantly. CONCLUSIONS: Applying high-dose anodal HD-tDCS to the rIFG selectively facilitated eye gaze in the eye area of neutral and happy faces in individuals with alexithymia, which may improve their face processing patterns.

The research into emotional regulation in eating disorders (EDs) has shown specific impairments and maladaptive coping strategies in patients, and there is an increasing interest in the role of the emotional domain in the treatment outcome. This study aims to evaluate the effect of a specialized inpatient treatment characterized by both an intensive and comprehensive standardized multidisciplinary programme based on cognitive-behavioural therapy and a flexible and personalized component implemented by third-wave interventions. A cohort of 67 female ED patients (anorexia nervosa = 28, bulimia nervosa = 28 and binge eating disorder = 11) underwent an evaluation of emotional regulation difficulties, alexithymia and dissociative symptomatology at admission to a specialized ED ward. The psychological modifications were subsequently re-evaluated upon discharge, after an inpatients treatment of 60 days, examining specific changes in the specific psychopathology. A significant improvement after specialized ED treatment was shown in alexithymia, emotional regulation difficulties and dissociation symptoms, with higher effect sizes in patients with higher alexithymia scores. As regards the specific effect of the psychological improvement, changes into alexithymia scores have shown specific correlations with ED psychopathology (p < 0.010) and with difficulties in emotional regulation (p < 0.010) in patients with higher alexithymia levels at admission. Emotional regulation and dissociation should therefore be evaluated in ED patients and may be improved with specific therapeutic approaches, while alexithymia remains a clinical trait, even with a significant reduction.

OBJECTIVE: The anxiety of life that comes with the pandemic process increases the health anxiety and the level of perceived stress. However, there are uncertainties about which individuals are more sensitive. This study aims to investigate the effects of alexithymic characteristics on health anxiety and perceived stress. MATERIALS AND METHODS: The authors invited the participants to study via social media and e-mail. The data of 793 individuals, aged 18-65, collected over the internet (Google Forms) between November and December 2020 were statistically evaluated. Evaluations were made with the sociodemographic data form, the General Health Questionnaire-12 (GHQ-12), the Health Anxiety Scale (HAS), the Toronto Alexithymia Scale (TAS-20), and the Perceived Stress Scale (PSS). RESULTS: In mediation analyzes between TAS subscales and HAS, Difficulty in Identifying Feelings (DIF) most strongly predicted HAS (B=0.469, p<0.001) and indirectly affected HAS only through GHQ (CS: 0.08, B=0.108, SE:0.021, CI: 0.070, 0.153). However, both PSS (CS: 0.0128, B=0.084, SE:0.027, CI: 0.032, 0.139) and GHQ (CS: 0.02, B=0.139, SE:0.028, CI: 0.090, 0.198) played a mediating role between Difficulty Describing Feelings (DDF) and HAS. CONCLUSIONS: The present study suggests that individuals with alexithymic features are more sensitive to stress during periods of health-related increased stress, such as pandemics, and that individuals with alexithymic features should be given priority in psychotherapeutic interventions.

Despite being a longstanding and well-established concept, alexithymia is unfamiliar for many clinicians. This article aimed to address the alexithymia concept from a clinical perspective based on a review of the research on alexithymia intervention. Several strategies are proposed to help clinicians better work with alexithymic clients in psychotherapy. Alexithymia assessment, its impact on the therapeutic alliance, and the difficulties in emotional tasks are highlighted points. Considering alexithymia will inform clinicians' current diagnosis and conceptualization and provide specific targets and venues for intervention, increasing the effectiveness of psychotherapy.

A lack of personalized approaches in non-medication pain management has prevented these alternative forms of treatment from achieving the desired efficacy. One hundred and ten female patients with fibromyalgia syndrome (FMS) and 60 healthy women without chronic pain were assessed for severity of chronic or retrospective occasional pain, respectively, along with alexithymia, depression, anxiety, coping strategies, and personality traits. All analyses were conducted following a 'resource matching' hypothesis predicting that to be effective, a behavioral coping mechanism diverting or producing cognitive resources should correspond to particular mechanisms regulating pain severity in the patient. Moderated mediation analysis found that extraverts could effectively cope with chronic pain and avoid the use of medications for pain and mood management by lowering depressive symptoms through the use of distraction mechanism as a habitual ('out-of-touch-with-reality') behavior. However, introverts could effectively cope with chronic pain and avoid the use of medications by lowering catastrophizing through the use of distraction mechanism as a situational ('in-touch-with-reality') behavior. Thus, personalized behavior management techniques applied according to a mechanism of capturing or diverting the main individual 'resource' of the pain experience from its 'feeding' to supporting another activity may increase efficacy in the reduction of pain severity along with decreasing the need for pain relief and mood-stabilizing medications.

BACKGROUND: Infertility causes emotional and psychological problems, including anxiety, depression, low self-efficacy, and chronic mental stress in women. These emotional problems can negatively affect fertility treatment. Numerous studies have reported the clinical therapeutic effects of acupuncture on emotional problems; however, the efficacy and safety of acupuncture treatment for emotional problems in infertile women remain unclear. This protocol aims to evaluate the efficacy and safety of acupuncture for treating emotional problems in infertile women. METHODS: We will search the following databases from their inception to April 30, 2021: MEDLINE, EMBASE, Cochrane Library, Korean Medical Databases (KoreaMed, Korean studies Information Service System, Korean Traditional Knowledge Portal, Oriental Medicine Advanced Searching Integrated System, Research Information Sharing Service, and National Digital Science Library), and Chinese databases (CNKI and Wan Fang Database). We will include randomized controlled trials on acupuncture for emotional problems in infertile women. There will be no restrictions regarding language or publication date. The primary outcome will be assessed using an emotion-related assessment scale. The risk of bias of the included studies will be measured using the Cochrane risk of bias assessment tool. For meta-analysis, RevMan Version 5.4 software will be used to combine the relative risks for dichotomous outcomes, as well as the mean differences or standardized mean differences for continuous outcomes, with both having 95% confidence intervals. RESULTS: Based on current evidence, this study will assess the effectiveness and safety of acupuncture for emotional problems in infertile women. CONCLUSION: This study will provide evidence for evaluating the acupuncture efficacy for infertile women with emotional problems. REGISTRATION NUMBER: INPLASY202150082.

BACKGROUND/OBJECTIVE: Brief transdiagnostic psychotherapies are a possible treatment for emotional disorders. We aimed to determine their efficacy on mild/moderate emotional disorders compared with treatment as usual (TAU) based on pharmacological interventions. METHOD: This study was a single-blinded randomized controlled trial with parallel design of three groups. Patients (N = 102) were assigned to brief individual psychotherapy (n = 34), brief group psychotherapy (n = 34) or TAU (n = 34). Participants were assessed before and after the interventions with the following measures: PHQ-15, PHQ-9, PHQ-PD, GAD-7, STAI, BDI-II, BSI-18, and SCID. We conducted per protocol and intention-to-treat analyses. RESULTS: Brief psychotherapies were more effective than TAU for the reduction of emotional disorders symptoms and diagnoses with moderate/high effect sizes. TAU was only effective in reducing depressive symptoms. CONCLUSIONS: Brief transdiagnostic psychotherapies might be the treatment of choice for mild/moderate emotional disorders and they seem suitable to be implemented within health care systems


ANTECEDENTES/OBJETIVO: Las psicoterapias breves son un posible tratamiento para los trastornos emocionales. Nuestro propósito fue determinar su eficacia en los trastornos emocionales leves/moderados en comparación con el tratamiento habitual basado en intervenciones farmacológicas. MÉTODO: Este estudio fue un ensayo clínico aleatorizado simple ciego con diseño paralelo de tres grupos. Los pacientes (N = 102) fueron asignados a psicoterapia breve individual (n = 34), psicoterapia breve grupal (n = 34) o tratamiento habitual (n = 34). Los participantes fueron evaluados antes y después del tratamiento con los siguientes instrumentos: PHQ-15, PHQ-9, PHQ-PD, GAD-7, STAI, BDI-II, BSI-18 y SCID. Se realizaron análisis por protocolo y por intención de tratar. RESULTADOS: Las psicoterapias breves fueron más efectivas que el tratamiento habitual para la reducción de síntomas y diagnósticos de los trastornos emocionales. El tratamiento habitual solo fue efectivo en reducir los síntomas depresivos. CONCLUSIONES: Las psicoterapias breves pueden ser el tratamiento de elección para los trastornos emocionales leves/moderados y podrían implementarse en los sistemas de salud

Home and Community-Based Services (HCBS) Medicaid waivers for Serious Emotional Disturbance (SED) extend Medicaid eligibility to youth who otherwise would be financially ineligible and finance a broad array of highly specialized mental health services specific to the needs of youth with SED. This study examines whether these policies are associated with greater public health insurance coverage among youth with severe mental health diagnoses. It also assesses, among youth with severe mental health diagnoses who have public health coverage, whether waiver policies are associated with reduced reports of unmet mental health treatment need and increased reports of adequate mental health coverage. Analysis uses CMS reported data on state HCBS Medicaid waivers in conjunction with data from the National Survey of Children's Health for the years 2016 through 2018. Multi-level, fixed-effects logistic regression models demonstrate that living in a state with an HCBS Medicaid waiver is associated with significantly increased odds of having public insurance among children with concurrent private health coverage (OR 1.89), reduced odds of unmet mental health needs among youth with public coverage (OR 0.45), but not significantly associated with reported adequacy of mental health insurance coverage.

Parent-mediated interventions can reduce behavioral and emotional problems in children with ASD. This report discusses the development of the first group parent intervention targeting behaviors and anxiety in children with ASD, across the spectrum of cognitive and language ability. 'Predictive Parenting' was developed from the clinical observation (and emerging evidence base) that children with ASD struggle with 'prediction' and anticipating change. It integrates well-established parenting strategies within an ASD-specific framework. The concept was co-created with patient and public involvement panels of parents and adults with ASD. A feasibility study found the programme is acceptable and accessible. Qualitative feedback from participants was largely positive, and critiques were used to inform a larger, pilot randomized controlled trial of the intervention.

Trabalho realizado a partir de um relato de caso. A abordagem terapêutica com o uso do medicamento homeopático é baseada na utilização do princípio da similitude (similia similibus curentur). Esse princípio implica no uso de medicamentos que causam efeitos semelhantes aos sintomas da doença, a fim de estimular uma reação do organismo contra sua própria patologia ou sofrimento. No tratamento homeopático, a individualidade e a totalidade sintomática característica de cada indivíduo é essencial para a correta escolha desse remédio. Pelos sintomas e queixas do paciente e através da repertorização, chegamos ao medicamento Stramonium, que é um dos principais medicamentos descritos na literatura para o "sentimento de abandono". Work carried out from a case report. The therapeutic approach using homeopathic medicine is based on the use of the principle of similitude (similia similibus curentur). This principle indicates the use of drugs that cause effects similar to the symptoms of the disease, in order to stimulate a reaction of the organism against its own pathology or suffering. The individuality and the total symptomatic characteristic of each individual is essential for the correct choice of the only suitable medicine for homeopathic treatment. Following the assessment of symptoms and patient complaints in repertorization, we come to the medication: Stramonium, which is one of the main drugs described in the literature for the "feeling of abandonment".(AU)

Healthy sleep, positive general affect, and the ability to regulate emotional experiences are fundamental for well-being. In contrast, various mental disorders are associated with altered rapid eye movement (REM) sleep, negative affect, and diminished emotion regulation abilities. However, the neural processes mediating the relationship between these different phenomena are still not fully understood. In the present study of 42 healthy volunteers, we investigated the effects of selective REM sleep suppression (REMS) on general affect, as well as on feelings of social exclusion, cognitive reappraisal (CRA) of emotions, and their neural underpinnings. Using functional magnetic resonance imaging we show that, on the morning following sleep suppression, REMS increases general negative affect, enhances amygdala responses and alters its functional connectivity with anterior cingulate cortex during passively experienced experimental social exclusion. However, we did not find effects of REMS on subjective emotional ratings in response to social exclusion, their regulation using CRA, nor on functional amygdala connectivity while participants employed CRA. Our study supports the notion that REM sleep is important for affective processes, but emphasizes the need for future research to systematically investigate how REMS impacts different domains of affective experience and their neural correlates, in both healthy and (sub-)clinical populations.

Este artigo constituiu-se com base nas experiências com a psicanálise aplicada na assistência aos colaboradores de uma instituição hospitalar ao longo da construção de um projeto de pesquisa coordenado pela Profa. Dra. Tania Coelho dos Santos e financiado pelo CNPq. Consiste numa abordagem dos impasses psíquicos de funcionários e dos consequentes fenômenos clínicos-institucionais encontrados. A atenção psicológica aos colaboradores recebeu maior destaque durante a pandemia Covid-19. Em resposta ao medo do desamparo e ameaça de morte recolhemos reações emocionais intensas e comportamentos disruptivos no ambiente de trabalho. Como consequência o serviço de psicologia organizou-se para tratar das urgências subjetivas deflagradas no âmbito institucional


Cet article a été basé sur les expériences de la psychanalyse appliquée à l'assistance des employés d'un hôpital développées lors de la construction d'un projet de recherche coordonné pour la Prof. Tania Coelho dos Santos et financé par le Conseil national pour le développement scientifique et technologique. Il consiste en une approche des impasses psychiques des employés et des phénomènes cliniques-institutionnels qui en découlent. L'attention psychologique offerte aux employés a gagné de l' importance pendant la pandémie de Covid-19. Nous avons observé des réactions émotionnelles intenses et des comportements perturbateurs sur le lieu de travail en réponse à la peur causée par l'impuissance et la menace de mort. C'est pourquoi le service de psychologie a été réorganisé pour faire face aux urgences subjectives déclenchées à l'intérieur de l' institution


This article was based on the experience assisting employees of a hospital through applied psychoanalysis, developed during the construction of a research project coordinated by ProfessorTania Coelho dos Santos and financed by the National Council for Scientific and Technological Development. It consists of an approach to the psychic impasses of employees and the consequent clinical-institutional phenomena encountered. Psychological attention to the employees gained greater prominence during the Covid-19 pandemic. In response to the fear of helplessness and the threat of death, we observed intense emotional reactions and disruptive behaviors in the workplace. As a consequence, the psychology service was organized to deal with the subjective emergencies triggered inside the institution

INTRODUCTION: About 66% of stroke survivors present with cognitive or physical consequences, which are often complicated by emotional instability. Alexithymia is defined as "a difficulty in identifying and describing feelings", although there is no consensus on the exact diagnosis and treatment. PATIENT CONCERNS: A 36-year-old right-handed man, affected by ischemic stroke (which occurred about 3 months before admission) involving the right hemisphere (ie, the fronto-parieto-temporal region) with left hemiparesis and behavioral abnormalities, came to our observation for intensive rehabilitation. He was treated unsuccessfully with a traditional and behavioral training. DIAGNOSIS: Alexithymia due to ischemic stroke. INTERVENTIONS: Then, a specific combined protocol using computerized emotional and virtual emotional training was applied in a semi-immersive virtual reality environment using the BTS-Nirvana device. OUTCOMES: At the end of this novel rehabilitation approach, the patient showed a significant improvement in emotional skills, cognitive performances, and coping strategies. CONCLUSIONS: Virtual reality, in addition to standard therapy, may be a valuable tool in improving emotional abnormalities due to brain lesions, such as alexithymia.

BACKGROUND: Instruments for monitoring the clinical status of adolescents with emotional problems are needed. The Clinical Outcomes in Routine Evaluation-Outcome Measure (CORE-OM) according to theory measures problems/symptoms, well-being, functioning and risk. Documentation of whether the theoretical factor structure for CORE-OM is applicable for adolescents is lacking. METHODS: This study examined the factor structure and psychometric properties of the CORE-OM based on two samples of adolescents (age 14-18): youths seeking treatment for emotional problems (N = 140) and high school students (N = 531). A split half approach was chosen. An exploratory factor analysis (EFA) was performed on the first half of the stratified samples to establish the suitability of the model. A Confirmatory Factor Analysis (CFA) with the chosen model from the EFA was performed on the second half. Internal consistency and clinical cut-off scores of the CORE-OM were investigated. RESULTS: The best fitting model only partially confirmed the theoretical model for the CORE-OM. The model consisted of five factors: 1) General problems, 2) risk to self, 3) positive resources 4) risk to others and 5) problems with others. The clinical cut-off score based on the all-item total was higher than in an adult sample. Both the all-item total and general problems cut-off scores showed gender differences. CONCLUSION: The factor analysis on CORE-OM for adolescents resulted in a five-factor solution, and opens up for new subscales concerning positive resources and problems with others. A 17-item solution for the general problems/symptoms scale is suggested. We advise developers of self-report instruments not to reverse items, if they do not intend to measure a separate factor, since these seem to affect the dimensionality of the scales. Comparing means for gender in non-clinical samples should not be done without modification of the general emotional problem and the positive resources scales. Slightly elevated CORE-OM scores (up to 1.3) in adolescents may be normal fluctuations.

BACKGROUND: There is growing evidence that positive psychology interventions (PPIs) enhance positive mental health and lead to a decrease in symptoms of psychopathology. This study examines the effectiveness of a 10-week multicomponent PPI (the Inspired Life Program; ILP) in promoting positive mental health and reducing symptoms of depression and negative affect in a sample of rural poor adults in Ghana. METHODS: Using a quasi-randomized controlled trial design, participants from four rural poor communities were randomly allocated to intervention (n = 40) or control (n = 42) conditions. The intervention group participated in the 10-week ILP. Both groups completed a battery of mental health measures, including the Mental Health Continuum-Short Form, at pre-intervention, immediately after the intervention, and at 3 months follow-up. Hierarchical linear modeling was applied to evaluate whether the intervention was effective. RESULTS: There was a greater improvement in positive mental health, with a marked reduction in symptoms of depression in the intervention group compared to the control group, immediately and 3 months after the intervention. There were also larger increases in the proportion of flourishers in the intervention group compared to the control group, immediately and three months after the intervention. CONCLUSIONS: The observed effects of the ILP intervention program suggest that group-based PPIs can promote positive mental health and buffer against psychopathology among people living in rural poor communities in sub-Saharan Africa.

Attention-Deficit Hyperactivity Disorder (ADHD) is frequently associated with emotion dysregulation (ED) that is characterized by excessive and inappropriate emotional reactions. Children with ADHD and ED present significant social, academic and family functioning impairments. These findings indicate that ED should be regularly monitored in children with ADHD and should be managed with targeted therapeutic interventions. However, few studies have evaluated the efficacy of psychosocial interventions to manage ED in children with ADHD. The aim of this systematic review was to assess the effects of psychosocial interventions for children with ADHD and ED, particularly their benefits and limitations. This review followed the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) model. A systematic literature search of different databases in February 2018 allowed the identification of five randomized controlled trials, one quasi-experimental study, and four open-label uncontrolled studies. Analysis of the results reported in these studies suggested that psychosocial interventions can improve severe irritability and aggressive behavior in children with ADHD and ED. However, the short trial duration, the lack of follow-up and of control group in several studies, and the heterogeneity of the outcome measures affected the result interpretation. Future studies should use standardized measures of ED and larger samples.

Borderline personality disorder (BPD) recognizes several psychopathological dimensions related to prefrontal cortex impairments. Transcranial Direct Current Stimulation (tDCS) targeting the right prefrontal dorsolateral cortex (DLPFC) positively influence cognitive functions related to impulsivity in healthy subjects. A randomized double-blind study was designed to investigate whether tDCS could modulate core dimensions (impulsivity, aggression, affective dysregulation) of BPD. Also effects on decision making process and substances craving was assessed. Patients were randomized to receive active-tDCS at 2 mA versus sham-tDCS, once a day for 15 sessions. Anode was placed on the right DLPFC (F4), cathode on the left DLPFC (F3). Impulsivity and aggression measures were significantly reduced only in patients treated with active-tDCS. Decision-making process was marginally influenced by the active current. Craving intensity was reduced only in the active-tDCS sample. Both groups showed improvements in the affective dysregulation dimension and anxious and depressive symptoms. The application of bilateral tDCS targeting right DLPFC with anodal stimulation seems to improve core dimensions of BPD (mainly impulsivity and aggression) probably by restoring prefrontal activity. tDCS might be a potential tool for preventing self-harming behaviors.

PROBLEM: Alexithymia is defined as difficulties in defining and describing feelings, differentiating feelings and bodily sensations caused by emotional arousal, and a combination of constricted imaginary processes. This study aims to investigate the effect of the psychoeducation program provided to individuals with alexithymia and violent tendencies adolescents on the level of alexithymia. METHODS: This experimental study was conducted between February 2019 and April 2019. The sample was 80 volunteer adolescents (experimental group n = 40 and control group n = 40) who met the research criteria. Data were collected using the Sociodemographic Form, the Tendency to Violence Scale, and the Alexithymia Questionnaire for Children (AQC). The psychoeducation program was applied in the experimental group in 90-min sessions once a week throughout 9 weeks. FINDINGS: There was a statistically significant decrease in the AQC posttest mean scores after the training provided to the experimental group (p < .01). CONCLUSIONS: The psychoeducation provided to adolescents was found to reduce the level of alexithymia. It is recommended that similar studies should be conducted with larger groups.

In their daily practice dentists frequently have patients showing signs of stress-related oral manifestations in different forms. Stress-related aetiology of oral changes are still not investigated enough, and present a subgroup of psychosomatic diseases which had been recognized in medicine a long time ago. Recognition of such psychological or emotional disturbance needs deep evaluation "per exclusionem", and is beneficial for both the patient and clinician. Psychological management should be taken into consideration when treating patients with these psychosomatic disorders. Therapeutic approach comprises different forms of psychotherapy and medication as well.
