Objetivo: descrever a escolha do parto domiciliar planejado acompanhado por enfermeira obstétrica em um centro urbano de grande porte, na perspectiva de mulheres brasileiras. Métodos: estudo qualitativo guiado pela Grounded Theory. Foram entrevistadas dez mulheres com idade entre 20 e 41 anos que tiveram parto domiciliar planejado acompanhadas por enfermeiras obstétricas. As participantes foram recrutadas por meio de rede social, acessando um grupo de mulheres que escreveram sobre seu parto domiciliar. Resultados: Emergiram duas categorias: Não vendo possibilidade de parir naturalmente no ambiente hospitalar e Pensando na segurança do parto domiciliar planejado. O hospital representou vários aspectos desfavoráveis como intervenções desnecessárias e solidão. As mulheres consideravam o lar um lugar seguro para parir, conectado aos cuidados de enfermeiras obstétricas. Conclusão: há mulheres que não desejam parir no hospital, preferindo parir em casa e do ponto de vista dos direitos humanos e dos cuidados desmedicalizados, as enfermeiras obstétricas devem apoiar as mulheres nessa sua decisão.


Objective: to describe the choice of planned homebirth attended by a nurse midwife in a large urban centre, from the perspective of Brazilian women. Methods: in this Grounded Theory study, ten women aged 20 to 41 years, who had a planned homebirth accompanied by a nurse midwife, were interviewed. Participants were recruited through a social network by accessing a group of women who wrote about their homebirth. Results: two categories emerged: seeing no possibility of giving birth naturally in the hospital environment; and thinking about the safety of a planned homebirth. Hospital represented several unfavourable aspects, such as unnecessary interventions and loneliness. Women thought of home as a safe place to give birth, connected with nurse midwife care. Conclusion: there are women who do not wish to give birth in hospital, but prefer to give birth at home and, from the point of view of human rights and de-medicalized care, nurse midwives should support women in their decision.


Objetivo: describir la elección del parto domiciliario planificado con enfermera obstétrica en un gran centro urbano, desde la perspectiva de mujeres brasileñas. Métodos: estudio cualitativo guiado por la Grounded Theory. Se entrevistó a diez mujeres entre 20 y 41 años que tuvieron parto domiciliario planificado, siendo acompañadas de enfermeras obstétricas. Las participantes fueron reclutadas a través de red social, accediendo a un grupo de mujeres que escribieron sobre su parto en domicilio. Resultados: surgieron dos categorías: las que no veían posibilidad de dar a luz naturalmente en el hospital y las que pensaron en la seguridad del parto domiciliario planificado. El hospital representó varios aspectos desfavorables como intervenciones innecesarias y soledad. Las mujeres consideraban que el hogar era un ambiente seguro para dar a luz, vinculado al cuidado de enfermeras obstétricas. Conclusión: hay mujeres que no desean dar a luz en el hospital, prefieren hacerlo en casa y, desde el punto de vista de los derechos humanos y de los cuidados sin la intervención de un médico, las enfermeras obstétricas deben apoyarlas en esa decisión.

Objetivo: avaliar o desempenho das atividades de vida diárias e identificar diagnósticos de enfermagem de pessoas em situação de rua. Métodos: estudo transversal, realizado de abril a julho de 2017, com 52 pessoas. Para coleta de dados, foi utilizado instrumento fundamentado no modelo teórico de Roper-Logan e Tierney. Foram consideradas dependentes as atividades de vida com frequência igual ou superior a 40%, para inferência dos diagnósticos utilizou-se referencial de Risner. Resultados: as atividades de vida dependentes foram: trabalho e distração (92,3%), morte (88,5%), sexualidade (76,9%), sono (71,2%), manter ambiente seguro (71,2%), respirar (61,5%), eliminar (53,8%) e alimentar-se (51,9%). Os diagnósticos mais frequentes: Disposição para comunicação melhorada (96%), Desobstrução ineficaz das vias aéreas (94%), Risco de contaminação (77%) e Controle de impulsos ineficaz (73%). Conclusão: evidenciou-se que pessoas em situação de rua possuem dependência de cuidados em atividades de vida diária relacionados a aspectos fisiológicos, sociais e emocionais.


Objective: to evaluate performance of activities of daily living (ADLs) and to identify nursing diagnoses for people living on the streets. Methods: in this cross-sectional study of 52 people, data were collected between April and July 2017 using an instrument based on the Roper-Logan-Tierney theoretical model. ADLs with frequency of 40% or more were considered to be care-dependent; diagnoses were inferred using a Risner framework. Results: dependent ADLs were: working and playing (92.3%), dying (88.5%), expressing sexuality (76.9%), sleeping (71.2%), maintaining a safe environment (71.2 %), breathing (61.5%), eliminating body wastes (53.8%), and eating (51.9%). The most frequent diagnoses were: improved communication disposition (96%), ineffective airway clearance (94%), contamination risk (77%), and ineffective impulse control (73%). Conclusion: people living on the streets were found to be dependent on care in physiological, social and emotional ADLs.

Métodos: estudio transversal, realizado entre abril y julio de 2017, con 52 personas. Para la recolección de datos se utilizó un instrumento basado en el modelo teórico de Roper-Logan y Tierney. Se consideraron como dependientes las actividades de vida con una frecuencia igual o superior al 40%. Para inferir los diagnósticos se utilizó el modelo referencial de Risner. Resultados: las actividades de la vida dependientes fueron: trabajo y distracción (92,3%), muerte (88,5%), sexualidad (76,9%), sueño (71,2%), mantener un ambiente seguro (71,2%), respirar (61,5%), eliminar (53,8%) y alimentarse (51,9%). Los diagnósticos más frecuentes: Mejora de la disposición comunicativa (96%), Limpieza ineficaz de la vía aérea (94%), Riesgo de contaminación (77%) y Control ineficaz de los impulsos (73%). Conclusión: se evidenció que las personas que viven en la calle son dependientes de cuidados en las actividades de la vida diaria relacionadas con los aspectos fisiológicos, sociales y emocionales.

RESUMO

This is a theoretical analysis of Constructivist Grounded Theory and its application in nursing research. It is organized in three topics: Characteristics of Grounded Theory; Philosophical basis of the constructivist strand; and Constructivist Grounded Theory data analysis particularities. The unique characteristics of Grounded Theory that set it apart from other approaches are simultaneous data collection and analyses, theoretical sampling, constant comparison at each stage of analysis, development of memos for reflexive and comparative analysis, and a coding system with an initial and a focused stage that is flexible and guides the analytical process of building a theory. Before embarking on a Constructivist Grounded Theory study, the literature is reviewed to hone the research question. The philosophical assumptions of symbolic interactionism and social constructivism influence the entire process. Constructivist Grounded Theory emphasizes the construction and interpretation of data recognizing social contexts, interaction, the point of view of participants, and their understandings according to the investigation time, place and situation.

RESUMO

OBJECTIVE: To trigger a reflection on the current working conditions of Nursing professionals in coping with the Covid-19 pandemic. METHOD: A theoretical-reflective study supported by studies from the Marxist perspective, national and international scientific articles, and official documents from the World Health Organization and the Federal Nursing Council. RESULTS: The daily work of Nursing professionals in the face of the Covid-19 pandemic presents unfavorable working conditions in Brazil and worldwide, with emphasis on the deficit of professionals, overload of activities, low pay, and personal protective equipment, often insufficient and inadequate, conditions that can lead to exhaustion, illness and death. CONCLUSION: This study can contribute to raising discussions about the need for improvements in the working conditions of Nursing professionals, especially in pandemic times and the impact on the health of these professionals.

AIM: To describe the expectations of children with cancer about the healing care environment during hospitalization based on Watson's Theory of Human Caring. DESIGN: A descriptive qualitative research design was used in this study. METHODS: Data were collected between December 2019 and February 2020 from purposively selected children with cancer in a paediatric haematology and oncology clinic at a university hospital in Turkey. Twelve participants (7 females and 5 males) aged 10-17 were recruited to carry out individual semi-structured interviews. All interviews were recorded via a digital audio recorder and transcribed verbatim. The content analysis method was used to analyse the data. FINDINGS: As a result of the analysis, three main themes with related sub-themes were identified that revealed the expectations of children with cancer: 'healing care behaviors', 'creating the physical environment of the hospital' and 'support from the social environment'. CONCLUSION: Watson's Theory of Human Caring may be a useful and applicable guide for drawing a conceptual framework of the expectations of children with cancer regarding a healing care environment. The study revealed these children's need for humanistic, compassionate, honest and problem-focused care. In addition, participants wished for improvements in the physical environment. They also stated that they feel lonely and need social support from their family, peers and society. IMPACT: This study presents a comprehensive picture of the expectations regarding a healing care environment (caring behaviours and physical and social aspects) of hospitalized children with cancer undergoing treatment. The qualitative findings generated by our study have the potential to facilitate these children's healthy adjustment to the hospital environment and procedures during hospitalization. Knowledge acquired through this study may be used to shape nursing care, enhance the healing care environment, and ensure that hospital design and setting are acceptable to and appropriate for paediatric patients.

OBJECTIVES: To analyze the abstracts that relate nursing to the Theory of Culture Care Diversity and Universality, published in the annals of the main national scientific events in the category, over a 12-year period. METHODS: documental research carried out in the annals of the Brazilian Nursing Congress and the National Nursing Research Seminar. Data collection took place from July/2018 to June/2019 using a form. Data was analyzed using descriptive statistics and content analysis. RESULTS: 99 studies were analyzed, and three categories emerged: Cultural diversity, emic perspective, and cultural context: possibilities for applying the theory; Incongruencies and peculiarities of the care sectors: limitations of the application of the theory; Knowledge production to improve care: implications for professional nursing practice. FINAL CONSIDERATIONS: the investigated scientific production enabled the construction of knowledge of cultural aspects related to nursing care and showed similarities and variations in care practices.

OBJECTIVES: to describe a medium-range nursing theory for the cardiovascular rehabilitation process. METHODS: theoretical development study, addressing the elaboration of medium-range theory, based on Roy's Adaptation Model, which combined theoretical deduction with induction strategies, based on literature review and concept analysis. RESULTS: the Medium-Range Theory for Nursing in Cardiovascular Rehabilitation presents eleven concepts and respective definitions, organized in relationships that represent elements, through modeling and image representation. Theoretical assumptions and propositions are presented. FINAL CONSIDERATIONS: the theory presented aims to contribute to the description of the cardiovascular rehabilitation process from the perspective of the adaptation of the person, family and groups, as well as to impact the nursing care implemented to patients in the process. The procedures advanced from conceptualization to operationalization, through the production of theoretical propositions that will guide future theoretical testing.

Objetivo: Elaborar um instrumento de consulta de enfermagem para a mulher idosa com Incontinência Urinária. Método: Estudo metodológico realizado em três etapas: identificação dos problemas adaptativos com base na teoria de Callista Roy por meio da revisão de literatura, os dados identificados foram coletados mediante formulário estruturado, o conteúdo foi avaliado pelas enfermeiras especialistas por meio da técnica de validação por consenso, culminando na elaboração do instrumento. Resultado: Considerou-se aprovados na validação por consenso 60 termos que indicavam problemas adaptativos, contendo 45 no modo fisiológico, seis no autoconceito, cinco na função do papel e quatro do modo interdependência. Conclusões: O instrumento elaborado dá subsídios para a investigação comportamental auxiliando na formulação dos diagnósticos de enfermagem, conhecimento do raciocínio clínico do enfermeiro para avaliação da eficácia na intervenção em relação ao comportamento apresentado pela paciente, propicia um cuidado holístico, por ser a adaptação uma característica inerente ao ser humano. Objective: To develop a nursing consultation instrument for elderly women with Urinary Incontinence. Method: Methodological study carried out in three stages: identification of adaptive problems based on the theory of Callista Roy through literature review, the identified data were collected using a structured form, the content was evaluated by specialist nurses using the validation technique by consensus, culminating in the elaboration of the instrument. Result: It was considered approved in the validation by consensus 60 terms that indicated adaptive problems, containing 45 in the physiological mode, six in the self-concept, five in the role function and four in the interdependence mode. Conclusions: The elaborated instrument provides subsidies for behavioral research, assisting in the formulation of nursing diagnoses, knowledge of the nurse's clinical reasoning to assess the effectiveness of the intervention in relation to the behavior presented by the patient, providing holistic care, as adaptation is a characteristic inherent to the human being. Objetivo: Analisar os pilares da Sistematização da Assistência de Enfermagem e seus elementos constituintes. Método: Estudo teórico-reflexivo, que estabeleceu como categorias analíticas Método, Pessoal, Instrumentos. Resultados: Se decompôs cada pilar estruturante da Sistematização, o que permitiu identificar seus elementos constituintes, e ampliou-se a compreensão do que é Sistematização da Assistência de Enfermagem, considerando-a como fenômeno distinto do Processo de Enfermagem. Conclusão: Descortinaram-se os elementos que compõem cada pilar da Sistematização da Assistência de Enfermagem, permitindo melhor entendimento quando se discute a organização do trabalho profissional a partir desta, bem como, ampliou-se a compreensão do termo, conceitual e operacionalmente. Objective: To analyze the pillars of the Nursing Care Systematization and its constituent elements. Method: Theoretical-reflective study, which established as analytical categories Method, Personnel, Instruments. Results: Each structuring pillar of the Systematization was decomposed, which allowed the identification of its constituent elements, and the understanding of what is Systematization of Nursing Care was expanded, considering it as a distinct phenomenon of the Nursing Process. Conclusion: The elements that make up each pillar of the Systematization of Nursing Assistance were revealed, allowing a better understanding when discussing the organization of professional work from it, as well as the understanding of the term, conceptually and operationally, was expanded. Objetivo: Analizar los pilares de la Sistematización del Cuidado de Enfermería y sus elementos constitutivos. Método: Estudio teórico-reflexivo, que establece como categorías analíticas Método, Personal, Instrumentos. Resultados: se descompuso cada pilar estructurador de la Sistematización, lo que permitió la identificación de sus elementos constitutivos, y se amplió la comprensión de lo que es la Sistematización de la Atención de Enfermería, considerándolo como un fenómeno distinto del Proceso de Enfermería. Conclusión: se revelaron los elementos que componen cada pilar de la Sistematización de la Asistencia de Enfermería, lo que permitió una mejor comprensión al analizar la organización del trabajo profesional a partir de ella, así como la comprensión del término, conceptual y operativamente, se amplió. OBJECTIVE: to describe a guideline for the use of the Delphi method to evaluate nursing theories, from the perspective of internal validation. METHOD: a methodological study, targeted at the development of a guideline for the use of the Delphi method in the evaluation of nursing theories. RESULTS: the Delphi method, principles of collective wisdom and levels of proficiency are used in the production of a guideline for organizing, searching, selecting and coordinating the activities of theoretical evaluators in teams. It distinguishes three phases for the theoretical evaluation process: Preparatory Phase (PP); Intermediate Phase (IP) and Theory Evaluation (TE) phase, incorporating Delphi-type selection procedures; search, selection and classification of judges/evaluators for the theory; definition of criteria for carrying out rounds and maintenance or removal of units of the theory evaluated. CONCLUSION: the developed guideline was able to adapt the elements of the Delphi method as a favorable strategy for the internal validation of nursing theories.

This analysis of Fawcett’s ideas will situate the role played by her model in the discipline in relation to its organization of knowledge and its role in the move to institutionalize the nursing discipline. By providing an in-depth epistemological analysis, locating the relationships between this concept and Popper’s work, and showing how Fawcett draws on a specific vision of science, the article presents a fresh look at the model while situating it within its assumptions. It also addresses some of its limitations and draws conclusions about its role in the move to institutionalize the nursing discipline.

RESUMO

This article presents Reed’s perspective on the development of nursing knowledge. Her concept is embedded in the perspective of intermodernism. For Reed, intermodernism is a philosophy for nursing science and practice and thus represents a way of looking at the content, structure, and process of scientific theory. Reed presents her concept in the form of a spiral path composed of the philosophical, practical, and theoretical dimensions of nursing knowledge. After an overview of Reed’s perspective, we offer an illustration of her proposal with Lawler’s theoretical development of somology. Lawler’s development pathway can be used to incorporate the components of Reed’s spiral path in a meaningful way. The discussion shows how Reed’s concept can improve the description and understanding of a nursing knowledge development process. Finally, the article provides a rare summary of Reed’s work in French.

RESUMO

The contemporary challenge in the field of health is undeniably that of devising a way to integrate humans and the environment that is beneficial to health. This article presents the universal cosmic imperative (UCI) philosophical perspective and the notion of the cosmic imperative that leads human beings to be creative in their relationship to nature. It begins by consolidating the exegesis of the internal evolution of Roy’s theory and identifying its influences. The epistemological and philosophical postulates upon which Roy’s theory was built are specified, and then the evolution of Roy’s thought is described in three main phases. The article then moves on to describe and explain the epistemological openings that Roy’s theory allows from the point of view of current health issues, especially environmental medicine, in a macro-meso-micro perspective of global health. Roy’s thinking is part of the question of the current turning point in the discipline of nursing (cure-care-healing) and the definition of its focus. Roy’s theory resists current theoretical developments, which it allows us to describe and question in a pattern that is fruitful for researchers.

RESUMO

OBJECTIVE: to reflect on Florence Nightingale's legacy and describe her contributions to critical holistic thinking in nursing. METHODS: this is a theoretical reflection, for which scientific productions on Florence Nightingale's environmental theory, as published in national and international journals, were based. RESULTS: Florence Nightingale's philosophy and teachings emphasize that the nurse must use her brain, heart and hands to create healing environments to care for the patient's body, mind and spirit. Nursing, since the time of Nightingale, has been building the holistic paradigm, in all schools of thought, with a view to a humanistic approach to the human being in their indivisible relationship with the environment. FINAL CONSIDERATIONS: Florence's contributions to holistic critical thinking in nursing are evident, constituting nurses' differential in clinical practice.

RESUMO

BACKGROUND: The Generation Z nursing student presents with a set of unique challenges from a faculty perspective since they have experienced technology and the use of screens since infancy. Nursing faculty require a different approach to this generation in order to communicate with, engage, effectively teach, and retain these students, all while revisiting their ideology of what defines student success. APPROACH: Newman's (1994) Theory of Health as Expanding Consciousness serves as a framework that nursing faculty can use to guide interactions and build relationships with Generation Z students to promote autonomy and critical thinking. The use of the mnemonic PROVE can help faculty outline strategies for use to improve metacognition and higher levels of consciousness in students of Generation Z. OUTCOME: By not prescribing to students "what to think", faculty can focus on helping students learn "how to" think and attain higher levels of consciousness.

RESUMO

PURPOSE: To provide a summary of research on ontology development in the Centre of eIntegrated Care at Dublin City University, Ireland. DESIGN: Design science methods using Open Innovation 2.0. METHODS: This was a co-participatory study focusing on adoption of health informatics standards and translation of nursing knowledge to advance nursing theory through a nursing knowledge graph (NKG). In this article we outline groundwork research conducted through a focused analysis to advance structural interoperability and to inform integrated care in Ireland. We provide illustrated details on a simple example of initial research available through open access. FINDINGS: For this phase of development, the initial completed research is presented and discussed. CONCLUSIONS: We conclude by promoting the use of knowledge graphs for visualization of diverse knowledge translation, which can be used as a primer to gain valuable insights into nursing interventions to inform big data science in the future. CLINICAL RELEVANCE: In line with stated global policy, the uptake and use of health informatics standards in design science within the profession of nursing is a priority. Nursing leaders should initially focus on health informatics standards relating to structural interoperability to inform development of NKGs. This will provide a robust foundation to gain valuable insights into articulating the nursing contribution in relation to the design of digital health and progress the nursing contribution to targeted data sources for the advancement of United Nations Sustainable Development Goal Three.

This article describes an experience providing nursing care to a patient under hemodialysis with the end-stage renal disease who had developed depression, loathing, and other, negative personal perceptions and gradually gave up on life goals due to hopelessness. The duration of nursing care, from August 23rd through August 29th, 2019, was revisited to identify the patient`s hopelessness in the dimensions of physiology, mental status, society, and spirituality. The author applied Swanson`s Caring Theory to facilitate the process of "knowing" and "being with" while performing direct nursing care and attentive listening to assist the patient to become more open-minded and to express personal perceptions toward the disease with the goal of further engaging the patient to increase self-awareness recognition, sense of loss, and negative perceptions. Through "doing for" and "enabling," the author reinforced infection control and identified the symptoms of fluid-overload, moisture, and salinity to raise the patient`s self-awareness and self-caring techniques and to lower the risk of hospitalization. Meanwhile, by "maintaining the patient`s belief," accompanied by the encouragement and attention from family members and providing linkages to patient-support communities, the patient was guided to identify and aggregate to foster positive thinking and self-worth to increase acceptance of living with the disease. Nursing personnel may apply Swanson`s Caring Theory to better consider the patient`s perspective, provide individual caring schemes, and strengthen the recognition, self-caring techniques and supportive systems of patients, increasing patient perceptions of self-worth, restoring their confidence, promoting their adaption to their disease, and improving attentiveness.

RESUMO

This essay is about what nurses can do when patients are at risk of dying alone during a hospitalization. A pilot program, "No One Dies Alone," is described as the program modifications required by the coronavirus pandemic. The centrality of being present with the dying person is discussed, with emphasis on Parse's concept of true presence.

El bajo peso al nacer constituye una preocupación mundial, los programas diseñados para prevenirlo dependen de la identificación de gestantes susceptibles. Existe escasa evidencia en la literatura acerca del cuidado por parte del profesional de enfermería dirigido a mujeres con factores de riesgo de tener niños con bajo peso al nacer desde la Atención Primaria de Salud. No obstante, hay teorías y modelos conceptuales que pueden ser aplicados para guiar las intervenciones. La teoría de los sistemas de enfermería es la que asumen los autores de esta investigación por considerarla de gran utilidad en la Salud Pública, debido a que, en el ámbito de la responsabilidad personal por la salud, centrar el autocuidado en la iniciativa propia de quienes lo requieren, entendiéndolo como "la práctica de actividades que los propios individuos, inician y desarrollan en su propio beneficio, en la mantención de su vida, salud y bienestar". El objetivo de este artículo fue determinar la importancia de la aplicación de la teoría de los sistemas de enfermería, para el abordaje de intervenciones de enfermería en la prevención del bajo peso al nacer desde la Atención Primaria de Salud.Las conclusionesdan luces sobrelas mujeres con diagnóstico de riesgo reproductivo preconcepcional y condiciones de riesgo para tener hijos con bajo peso al nacer, en particular, deben incorporar a su vida diaria autocuidados que les permitan prevenir complicaciones. Las teorías ayudan a adquirir conocimientos que contribuyen a perfeccionar prácticas cotidianas mediante la descripción, explicación, predicción y control de los fenómenos; además facilita a los profesionales autonomía de acción como guía en aspectos prácticos, educativos y de investigación.


Low birth weight is a global concern; programs designed to prevent it depend on the identification of susceptible pregnant women. There is little evidence in the literature about care by the Nursing professional directed to women with risk factors of having children with low birth weight from Primary Health Care. However, there are theories and conceptual models that can be applied to guide interventions. The Theory of Nursing Systems is the one assumed by the authors of this research because they consider it very useful in Public Health, because in the field of personal responsibility for health, it focuses self-care on the initiative of those who they require it, understanding it as "the practice of activities that individuals themselves initiate and develop for their own benefit, in maintaining their life, health and well-being." The goal of this study was to determine the importance of the application of the theory of nursing systems, for the approach of nursing interventions in the prevention of low birth weight from Primary Health Care.The conclusions shed light on the women with a diagnosis of preconception reproductive risk and risk conditions for having children with low birth weight, in particular, should incorporate self-care into their daily lives that allow them to prevent complications. Theories help to acquire knowledge that helps to improve daily practices through the description, explanation, prediction and control of phenomena; it also provides professionals with autonomy of action as a guide in practical, educational and research aspects.
