RESUMO

A simulation focused on nursing theory was designed to replace one clinical day experience for 37 senior-level baccalaureate students. Pretest-posttest self-evaluations were used in the study to rate student confidence in using Locsin's technology competency as caring in nursing theory and in describing critical care equipment and monitoring technology. A dependent t-test showed improvements in confidence using theory in practice after the simulation and debriefing session. Using one-on-one debriefing sessions, students and faculty had an opportunity to discuss how nursing theory can guide nursing practice. Intentionally developed simulation and debriefing experiences can provide students experience using nursing theory.

Research concerning this issue demonstrates that nursing students initially feared interacting with and caring for patients with suicidal tendencies. However, there is a lack of research, which examines the care that is provided to patients that are suicidal, by nursing students. The aim of this study was to develop a theory to guide nursing students when caring for patients with suicidal tendencies on their psychiatric clinical practicum. A qualitative approach using Grounded Theory was used. A total of 22 nursing students who had provided care for suicidal patients were interviewed. The core category that emerged from the data was the 'changing of mindsets towards caring for suicidal patients and promotion of suicidal care competencies'. Other key categories linked to and enfolded within this core category were: suicidal risk assessment; protecting patients' safety; and, developing therapeutic communication competencies to advance suicidal care. This study could help fill a theory-practice gap for both psychiatric nursing teachers and students. Nurse teachers could use this theory as a map to help guide students caring for patients with suicidal tendencies and develop nursing students' suicidal care competencies.

RESUMO

This article provides a critical analysis of the Calgary Family Assessment Model using Jacques Donzelot's work. According to Donzelot, the family is exploited by (para)medical power, and becomes a fertile ground for practices of governmentality. Thus, the family is invited (and "trained") to play a health provider role in addition to the role of natural caregiver. The (para)medical apparatus imposes itself on the family's intimacy, alters its functioning, and tenuously governs its conduct. Our analysis shows that Donzelot's theoretical perspective conflicts with nursing theories.

This case report describes a nursing care experience that applied Swanson's Caring Theory to a powerless elderly patient who was suffering from vasculitis problems that had made her incapable of walking and had delayed her discharge from the hospital. The associated health problems, including impaired physical mobility, infection, and powerlessness, were affirmed after an integrated physical, psychological, social, and spiritual assessment was conducted during the nursing care period, which lasted between April 26th and May 5th, 2018. The five categories (knowing, being with, doing for, enabling, maintaining belief) of Swanson's Caring Theory were applied. The patient was encouraged to participate in the treatment plan and motivated to participate in rehabilitation. In addition, the caring capacity and care confidence of family members was enhanced in order to support the patient's further care after discharge. Furthermore, religious support was involved, which allowed the patient to communicate her emotions, reduce her stress, and achieve spiritual comfort. By providing care in the realms of physiology, psychology, and spirit, the author helped the patient regain her self-worth and enhance her positive beliefs in the future. This case report is hoped to provide medical staffs with a reference for facilitating the recovery of elderly patients experiencing disease-related powerlessness.

OBJECTIVE: to report the experience of the development of nursing knowledge from nursing theories and points of contact with the nurse's praxis. METHOD: case report on the reflections on contents addressed in the discipline offered in the Graduate Program in Nursing in a federal public university. The students presented seminars approaching the main nursing theories, generating extensive discussion and correlating them with the nurse's practice. Review and reflection on the contents addressed. RESULT: two themes were defined: the training of nursing undergraduate students and the development of knowledge in relation to the concepts of nursing theories; and care models and nurses' critical reasoning about the applicability of nursing theories. FINAL CONSIDERATION: the reflections produced permeated the development and acquisition of knowledge and of representative meanings of healthcare practice from the assumptions and constructs of the theories.

OBJECTIVE: To discuss the contribution of nursing theories in the conceptual expansion of good practices in the area for the construction of the unified health system (Sistema Único de Saúde - SUS). METHOD: Theoretical production of reflection, based on the neomodern framework of philosophy of science. RESULTS: The knowledge and political, social and epistemological commitments of nursing are discussed, highlighting the relationship of the theories with good practices for the construction and application of knowledge in the area in the context of the Brazilian health care system. Reflection on how middle-range theories can help in solving dichotomies between discipline, theory, practice, producers and consumers of knowledge is made. CONCLUSION: Original reflections on the contributions of disciplinary and professional knowledge of nursing theories to the creation of SUS are presented, defending a creation through the academy-service partnership, in which the theories start being incorporated into a new concept of nursing practices aligned to neomodern principles of science.

Objetivo: Identificar produções acadêmicas, nos programas stricto sensu de enfermagem, onde se adota referencial de Thompson e discutir concepções teórico-metodológicas e contribuição de Thompson para o conhecimento e prática da Enfermagem. Método: Estudo descritivo-documental, reflexivo, cuja análise constituiu-se por resumos de teses/dissertações por enfermeiros no Brasil, relacionadas às concepções teórico-metodológicas de Thompson. Resultados: Identificou-se 17 produções, 07 teses de doutorado (41,17%), 10 dissertações de mestrado acadêmico (58,83%), com destaque para a Escola de Enfermagem Anna Nery da Universidade Federal do Rio de Janeiro. Conclusão: Estudos de enfermagem permeados pela sociologia ampliam a visão de que uma técnica, uma estratégia educativa, uma tecnologia emergem em razão do cenário social, das necessidades permeadas por valores e da dinâmica existente entre ciência e sociedade. Acredita-se ser importante para o cenário cientifico e investigativo de enfermagem, compreender as modificações inerentes ao cuidar, advindas de um produto crítico-reflexivo social, que pode ser contemplado por adotarmos como norteador de análise, teóricos imersos no viés da sociologia


Objective: The study's purpose has been to identify academic productions, in the stricto sensu nursing programs, where Thompson's reference is adopted, and also to discuss the Thompson's theoreticalmethodological conceptions and its contribution to both Nursing knowledge and practice. Methods: It is a descriptive study with a documentary basis, also reflexive, whose analysis consisted of theses/dissertations abstracts done by nurses in Brazil, which were related to Thompson's theoretical-methodological conceptions. Results: 17 productions were identified, 07 doctoral theses (41.17%), 10 masters' dissertations (58.83%), with emphasis on the Escola de Enfermagem Anna Nery from the Universidade Federal do Rio de Janeiro. Conclusion: The nursing studies permeated by sociology can expand the view that a technique, an educational strategy, and a technology may emerge due to the social scenario, furthermore, the needs permeated by values and the dynamics between science and society. It is assumed to be important for both the scientific and nursing research scenario, understanding the inherent modifications of care that come from a critical-social reflexive product, which can be contemplated by adopting as the analysis guiding principle, then being seen as theorists immersed in the sociology trendline

This report describes our experience providing nursing care between 7th October and 7th November 2016 to a homosexual male who repeatedly engaged in stress-related suicide attempts and self-harm. Information was collected using comprehensive nursing assessment, observation, and interviews and the health problems of ineffective coping and disabled family coping were identified. Because of affection-related distress and the lack of recognition of his gender orientation by his family, the client engaged repeatedly in self-harm and attempted suicide on several occasions in order to cope with the self-perceived unbearable stress and stressful situations. The authors provided nursing care based on Watson's theory of human caring. We fulfilled the nature of caring, prompted respect and acceptance, and established a therapeutic, interpersonal relationship. We also adopted the concept of suicide intervention, and used the protective factors as the main coping strategies. These approaches allowed the client to acknowledge the events and reactions that may trigger his suicidal ideas, to develop better self-reflection and impulse control, and to mitigate the ideas and behaviors related to suicide and self-harm. Because constant conflicts between the client and his parents hampered a harmonious family relationship, we adopted family-centered care in order to enhance family resilience, which created room for mutual learning and development and reconstructed a healthy family relationship. Our successful results with this case encouraged our writing this paper in order to share this nursing experience with caregivers facing similar client situations and needs.

The term "Socratic method" is so pervasive in education across the disciplines that it has largely lost its meaning, and it has lost its roots in its originator-the historical Socrates. In this article we draw from the original source, Plato's ancient dialogues, to understand the theory and principles behind the questioning used in Socratic method. A deep understanding of Socratic method is particularly timely now as nursing leaders call for increased use of theory-based debriefing across the nursing curriculum. Socratic questioning is ideally suited as a method for debriefing in nursing classrooms because of its ability to enhance critical thinking and self-reflection of the learner and because of its basis in care for the learner through a learner-centred design. We present an analysis of the Socratic method in Plato's works and provide an overview of the key Socratic principles and techniques. We illustrate these principles and techniques with examples of how Socratic teaching can be applied in the nursing classroom, and we address the challenges and rewards for nursing faculty implementing Socratic method. Learning about Socratic method directly from Plato's dialogues can provide a richer and more robust understanding of this key pedagogical technique and help nurse educators practice more intentional and informed Socratic questioning and debriefing.

Nursing professional development practitioners led the evaluation of the graphic representation of a health system's professional practice model (PPM) based on evolving expectations and key constructs of a PPM. The aim was to capture direct care nurse perceptions to guide adoption of a revised graphical depiction of the PPM. The specific benefit for nursing professional development practitioners is the ongoing relatability of the PPM to nursing for sustainability of clinical excellence.

Objective. To describe the transformation of decisive moments that arise within the nurse-patient and family caregiver interaction to turn them into moments of care capable of favoring adaptation. Methods. In a high complexity hospital in the city of Bogotá (Colombia), a "nursing methodological research"-type study was conducted. It was developed in five stages: 1) identification of the institutional route of patients and their caregivers and, within it, the moments of encounter with nursing; 2) typical day of the nurse; 3) analysis of the nurse-patient and family caregiver encounters; 4) literature review on how to strengthen the nurse-patient and family caregiver relationship; and 5) proposal to transform decisive moments into moments of care. Results. Patients and their family caregivers usually experience six moments of encounter with nursing that include admission, assessment, satisfaction of basic needs, administration of medication, shift change, and discharge; all of them cross-cut by education and communication. Recognition of experiences during moments of encounter allowed transforming them into moments of nursing care. Conclusion. The transformation of decisive moments into moments of nursing care to favor adaptation of patients and their family caregivers is consequence of the nursing commitment and human nature expressed in every encounter of the care process.


Objetivo. Describir la transformación de los momentos de verdad que se dan en la interacción enfermera­paciente y cuidador familiar para convertirlos en momentos de cuidado capaces de favorecer la adaptación. Métodos. En una institución hospitalaria de alta complejidad de la ciudad de Bogotá (Colombia) se realizó un estudio del tipo "investigación metodológica en enfermería", el cual se desarrolló en cinco etapas: 1) identificación de la ruta institucional del paciente y su cuidador familiar y en ella los momentos de encuentro con enfermería; 2) Día típico de la enfermera; 3) Revisión de los encuentros enfermera ­ paciente y cuidador familiar; 4) Revisión de la literatura sobre cómo fortalecer la relación enfermera ­ paciente y cuidador familiar; y 5) Propuesta para la transformación de los momentos de verdad en momentos de cuidado. Resultados. El paciente y su cuidador familiar tienen generalmente seis momentos de encuentro con enfermería que incluyen el ingreso, la valoración, la atención de necesidades básicas, la administración de medicamentos, el cambio de turno y el egreso. La educación y la comunicación son transversales a ellos. El reconocimiento de experiencias en los momentos de encuentro permitió transformarlos en momentos de cuidado de enfermería. Conclusión. La transformación de los momentos de verdad en momentos de cuidado de enfermería para favorecer la adaptación del paciente y de su cuidador familiar son una consecuencia del compromiso y del tono humano de enfermería expresado en cada uno de los encuentros del proceso de atención.

The cue-response theory is herewith proposed to replace the coma cue-response conceptual framework as a nursing theory for care of patients with acquired brain injury (ABI). After ABI, nurses assess patients and develop an understanding of their condition by interpreting meaning from physiologic and observational or behavioral cues. These interpretations form the basis for optimizing the timing of discrete nursing interventions; the outcome of which influences the trajectory toward recovery or toward secondary brain injury. The cue-response theory applies specifically for nurses' use to determine which interventions should be used and when those interventions should be carried out. The theory recognizes the knowledge potential and knowledge produced in the context of nursing care of all patients with ABI, not just those with coma, and broadens our understanding of how the timing of nursing interventions directly impacts secondary brain injury and the brain entropy state.

The transition to quality and safety in the new graduate registered nurses' practice remains problematic directly impacting patient outcomes. Effective mentoring during transition serves to enhance experiential learning, allowing the development and establishment of safe, quality nursing practice. Comprehensive understanding of the transition process, including the barriers and effective enablers to transition is the key to effective mentoring. A theoretical framework guided by Duchscher's Stages of Transition Theory and Transition Shock Model and Benner's From novice to expert model can facilitate such understanding. Nurse Theorists play an important part in shaping nurse education and practice and have provided nurse educators and leaders an understanding to shape skill acquisition and the transition process for new graduate registered nurses. The res ultantresearchmodels and theory of these influencial nurses are pertinent to transition of new graduate registered nurses. This paper outlines the theories of Duchscher and Benner and how their research formed the theoretical framework to facilitate the measurement, understanding and improvement of the safety and quality of nursing care and impact the future nursing workforce.

Objetivo: discutir a aplicabilidade do Nursing Activities Score (NAS) à luz da teoria do conforto, relacionando a abrangência das áreas do cuidado descrita neste instrumento aos contextos de conforto propostos na teoria de Kolcaba. Metodologia: estudo teórico, de análise crítica, fundamentado nos princípios da teoria do conforto de Kolcaba. Reflexão: as atividades avaliadas pelo NAS são congruentes com os contextos de conforto propostos na teoria de Kolcaba, podendo contribuir para a promoção do conforto. Nota-se, no entanto, que há menor número de itens para avaliar os contextos socioculturais, psicoespirituais e ambientais. A maioria das atividades valoriza as necessidades orgânicas e biológicas dos pacientes. Conclusão: apesar das atividades do NAS mostrarem-se equivalentes com os contextos de conforto propostos por Kolcaba, não se pode afirmar que os cuidados prestados serão integralmente condizentes aos pressupostos da teoria, necessitando de estudos posteriores para validação clínica. Objective: To discuss the applicability of the Nursing Activities Score (NAS) in the light of the Comfort Theory, relating the range of care areas described in this instrument to the comfort contexts proposed in Kolcaba’s theory. Methodology: theoretical study, critical analysis, based on the principles of Kolcaba comfort theory. Reflection: the activities evaluated by the NAS are congruent with the contexts of comfort proposed in the Kolcaba theory, and can contribute to the promotion of comfort. It is noted, however, that there are fewer items to evaluate the sociocultural, psycho-spiritual and environmental contexts. Most activities value the organic and biological needs of patients. Considerations: Although the activities of the NAS are consistent with the comfort contexts proposed by Kolcaba, it cannot be said that the care provided will be fully compatible with the theoretical assumptions, requiring further studies for clinical validation.
