OBJECTIVE: Although research over the past decades has investigated the impact of the personality dimensions of dependency and self-criticism on treatment outcome, little is known of how these personality features influence responsiveness to treatment in patients with severe, chronic forms of depression. METHOD: The present study uses data from the Tavistock Adult Depression Study, a randomized controlled trial investigating the effectiveness of long-term psychoanalytic psychotherapy (LTPP) compared with treatment as usual (TAU) for individuals diagnosed with treatment-resistant depression. Patients were rated with the Anaclitic-Introjective Depression Assessment Q-sort, which distinguishes between two more maladaptive (Submissive and Dismissive) and two less maladaptive (Needy and Self-Critical) subdimensions of dependent or anaclitic and self-critical or introjective depression. Multilevel modeling was used to compare individuals' growth curves of depression severity as measured by the Hamilton Rating Scale for Depression over the 18-month treatment period and 2-year follow-up. Rates of clinically significant change were also determined. RESULTS: As expected, depressed patients with more maladaptive dependent and self-critical features did not benefit from LTPP or TAU. Patients with less maladaptive self-critical features benefited from both LTPP and TAU, while those with less maladaptive dependent features showed considerable gains from LTPP but not from TAU, with medium to large effect sizes. CONCLUSIONS: Findings of this study are consistent with existing research suggesting the need to modify and tailor treatments in accordance with individuals' pretreatment personality features. Given the time and cost-intensive nature of longer-term treatment, this may be particularly important in patients with treatment-resistant depression. In this paper the 'analyst as a citizen in the world' is understood as the analyst interconnecting and harmonising with his or her environment, including not only society, but also nature and the universe. In this sense, Buddhism teaches the Oneness of Life and its Environment, and links are made between Mahayana Buddhism and Jung's understanding of the Self and individuation. The Logic of Lemma in the Flower Ornament Sutra indicates that all phenomena in the world can merge with each other without losing individuality; ten stages are described from the Lotus Sutra and links are made with the development in respect to the ego; Kenji Miyazawa and his work are given as examples to illustrate this. Causality and synchronicity are explored in terms of the interaction between the individual and the environment, and three examples are given where sometimes individual, egoic, causality is more of a feature and sometimes synchronicity has a greater prominence. The paper ends with an examination of tree drawings, made over a period of 50 years by junior high school students, which indicates the way that these individuals have portrayed themselves and their relation to their environment.

Beginning with Jung's reflections on individuation as a starting point, this article examines the analytic relationship in the context of our current reality. The latter exerts pressures of all sorts that do not favour individuation. Indeed, geopolitical commentators see present-day society as marked globally by excessive self-love that seeks to dominate, at the expense of the other. Given that such forms of dominance are now also very prevalent in the workplace, this article examines how to help the increasing number of individuals with highly successful professional careers who suffer from this pressure and who then seek analysis. 'Progressive movement', an indicator of and incentive to change, is examined by studying the narrative processes emerging from dream activity - including dreaming with the hands in the sand tray - in the co-transference. The article highlights how 'progressive movement', founded on narrative Competence and on the reflective function, may generate experiences of 'realistic hope'. The latter signals the start of an individuation process in the analytic couple and, simultaneously, the transformation of both the analyst and the analysand into good enough 'citizens in the world'.

In Freud's day, his case studies, coupled with the findings of others (e.g., Breuer, Charcot), provided evidence that the Oedipus complex and his dynamic theory of repression (i.e., the unconscious interactions between id, ego, and superego) as causal agents in neurotic symptoms was suspect. With the aid of over 75 years of research since Freud's death, today's psychodynamic-orientated clinicians have discarded many of Freud's tenets related to the Oedipus complex. Modern psychoanalysts have focused their attention instead on a patient's personal relationships in their early life, their current life, and in their interactions with their therapist (transference).

The evidence on potentially greater benefits of psychoanalysis (PA) vs. long-term psychodynamic psychotherapy (LPP) is scarce. This study compared the effectiveness of PA and LPP on personality and social functioning during a 10-year follow-up from the beginning of the treatments. The eligible patients, 41 self-selected for PA and 128 assigned to LPP, were 20-45 years of age and had anxiety or mood disorder. Outcomes were analyzed using ten standard measures of personality and social functioning, carried out 5-9 times during the follow-up. Different change patterns by time in PA and LPP emerged, suggesting less benefit of PA during the first years of follow-up and more benefit in most outcomes thereafter. Greater post-treatment improvement in PA than in LPP was seen up to 1-2 years after PA had ended in more mature defense style (DSQ), level of personality organization (LPO), more positive self-concept (SASB), more improved social adjustment (SAS-SR) and sense of coherence (SOC). However, at the 10-year follow-up the differences were non-significant. In conclusion, PA may give some additional benefits when long-term aims are linked to personality and social functioning. The relatively small differences and higher costs in comparison to LPP may restrict the feasibility of PA.

Identity development depends on the ability to say 'no.' Setting limits enables a relationship between two separate individuals to develop. Early trauma can leave the individual so vigilant to others' demands that internal prohibitions against intrusion remain silenced, which we conceptualize as a 'no' that could not be sufficiently articulated to keep the person safe. For those who have not been able to assert this fundamental limit, the consulting room provides a potential anchoring point to formulate and work through unconscious meanings. Being able to articulate and register the legitimacy of one's own no becomes an important challenge, as tensions regarding power and powerlessness, trust and distrust, are acted out within the consulting room. Case material illustrates how psychoanalytic ideas regarding transference, countertransference, and enactment help the clinician tolerate the intrusion of past into present, inviting the type of mentalization that moves towards repair rather than merely reenacting the trauma.

This case report sketches the psychoanalytically-informed supportive psychotherapeutic treatment of a single man through his life challenges over a twenty-year period. The decades-long challenges arise from obsessive compulsive disorder, pathologic self-effacement with insatiable and unusual erotic components, and severe visual impairment leading to blindness. Sadistically-tinged impulses arising in the countertransference are described.

The author contends that psychoanalytic theory has generally presented religious beliefs as developmentally immature or pathological. This viewpoint has resulted in a neglect of religion on the part of psychoanalysts and an avoidance of their religious life by patients. Even though there has been an evolution from the traditional Freudian foundational approach to religion as an "illusion" to the inclusion of psychoanalytical training within some Christian institutes and attributions that psychoanalysis, itself, is a religion, religious beliefs should be included in psychotherapy because they can become involved in transference and countertransference issues in ways that are ignored if religious issues are not discussed in therapy. The author presents clinical material to illustrate this problem.

Functional analytic psychotherapy (FAP), with its emphasis on the creation of a safe, evocative, attuned, authentic, and mutually vulnerable therapeutic relationship, offers strategies that are especially relevant for therapeutic beginnings that yield an engaging and potent treatment. The 5 rules of FAP provide behavioral specificity in the early tasks of therapy that can build a powerful alliance with clients: creating trust and safety, moving the conversation from content to in-the-moment process, evoking and naturally reinforcing client target behaviors related to authentic expression, instilling hope, being aware of clinical impact, and promoting generalization of in-session gains to daily life. It is hoped that FAP offers a conceptually clear and inspiring transtheoretical framework that sets the stage for a deeply meaningful and unforgettable therapy. Existen multitud de propuestas psicoterapéuticas que plantean Intervenciones corporales para abordar el sufrimiento psíquico, algunas de ellas faltas de un cimiento teórico sólido. Por otro lado, diferentes autores aceptan la presencia de experiencias arcaicas que mantienen su influencia a lo largo de la vida y que resultan inaccesibles a la palabra. Cuando la intervención verbal es insuficiente, se hace necesaria una reflexión sobre los límites de la acción terapéutica: la atención sobre el diálogo somático, el cuestionamiento del tabú de tocar o la integración de técnicas corporales que evoquen experiencias sensoriales y emocionales primigenias. Se revisará el significado del cuerpo para diferentes autores dentro del ámbito del psicoanálisis, tales como Freud, Reich, Bion, Winnicott, Anzieu o los teóricos del psicoanálisis relacional, en busca de un fundamento conceptual para las técnicas corporales


There are many psychotherapeutic proposals that offer corporal interventions to deal with psychic suffering, some of them lacking a solid theoretical foundation. On the other hand, different authors accept the presence of archaic experiences that maintain their influence throughout life and are inaccessible to the word. When verbal intervention is insufficient, it is necessary to reflect on the limits of therapeutic action: attention to somatic dialogue, questioning of the touch taboo or the integration of body techniques that evoke sensory and primordial emotional experiences. The meaning of the body within the psychoanalysis field, according to a range of authors such as Freud, Reich, Bion, Winnicott, Anzieu or relational psychoanalysis theorists, will be reviewed in search of a conceptual foundation for body techniques

O objetivo do artigo é apontar para as possibilidades (e limites) do uso do método psicanalítico em um contexto não clínico, especificando as aproximações e os distanciamentos entre a situação clínica e a situação de pesquisa empírica. Pressupomos que o psicanalista que ocupa o lugar de entrevistador fala de um lugar de poder e de saber que produz efeitos no entrevistado. Com o cuidado de não adotar um estilo "protocolar", mas buscando apresentar sistematicamente as condições mínimas da prática da psicanálise, destacamos algumas dessas condições. Assim como na clínica, na situação de pesquisa empírica existem dois fenômenos indissociáveis: o que ocorre do lado pesquisado ou analisando; e o que ocorre do lado do pesquisador; ou analista


Le but de l'article est de mettre en évidence les possibilités (et les limites) de l'utilisation de la méthode psychanalytique dans un contexte non clinique, en précisant les approximations et les différences entre la situation clinique et la situation de la recherche empirique. Nous supposons que le psychanalyste qui occupe la place de l'intervieweur parle d'un lieu de pouvoir et de savoir qui produit des effets chez l'interviewé. En prenant soin de ne pas adopter un style "protocolaire", mais en cherchant à présenter systématiquement les conditions minimales de la pratique de la psychanalyse, nous soulignons certaines de ces conditions. Ainsi que dans la clinique, dans la situation de la recherche empirique, il existe deux phénomènes indissociables: ce qui se passe du côté de la recherche ou de l'analyse; et ce qui se passe du côté du chercheur; ou analyste


The aim of the article is to point out the possibilities (and limits) of the use of the psychoanalytic method in a non-clinical context, specifying the approximations and distances between the clinical situation and the empirical research situation. We assume that the psychoanalyst who occupies the place of interviewer speaks of a place of power and of knowing that produces effects in the interviewee. With the care of not adopting a "protocolar" style, but seeking to presentsystematically the minimum conditions of the practice of psychoanalysis, we highlight some of these conditions. Cet article vise à remettre en question la direction du traitement dans un cas clinique présenté par la psychanalyste française Ruth Lebovici. L'affaire a été publiée en 1956 et s'intitule "Perversion sexuelle transitoire au cours d'un traitement psychanalytique". Cette étude met en évidence les effets des interprétations sauvages de l'analyste. Les propositions de Freud et de Lacan sur l'interprétation et, plus précisément, les commentaires de Lacan sur l'affaire, élucident les glissements subis parla technique psychanalytique


This article aims at the question of the direction of treatment in a clinical case presented by the French psychoanalyst Ruth Lebovici. RESUMO

The MATRIX is a tool designed to analyze psychoanalytic sessions. It is composed of two axes: focus (patient, therapist, and dyad) and dimension (space, content, and order). The MATRIX was found to be a reliable tool, and valid for identifying the therapist's theoretical inclinations in psychoanalytic-oriented psychotherapy sessions. In the present study, we suggest that the MATRIX can identify different (Kleinian, Bionian, and relational) approaches to projective identification, thus providing therapists and supervisors an easy-to-use tool for monitoring, and better managing, projective identifications during psychoanalytically oriented psychotherapy.

The aim of the present paper is to describe the development of a Brief Psychoanalytic Group therapy for contaminated sites and its application in the National Priority Contaminated Site of Casale Monferrato. Before presenting the core of the clinical intervention, a brief examination of some clinical features encountered working with malignant mesothelioma patients and their caregivers is offered. These aspects have been pivotal elements in the construction of a psychoanalytically oriented time-limited (i.e., 12 sessions) group therapy. This model of intervention was designed by one of the Authors (AG) and is aimed at reducing the impact of living in a threatening place where both physical well-being and health are put to the test. At a psychological level, in fact, living in contaminated sites arouses death anxieties, which can deeply compromise the quality of time remaining to live together with loved ones after a fatal cancer diagnosis.

Es peligroso evaluar el impacto de un fenómeno en la época en la que vivimos. El riesgo es doble: sea al idealizar el pasado, darse a la nostalgia y juzgar negativamente su época; o al idealizar al presente, sacrificar todo al modernismo y despreciar los valores del pasado en nombre del progreso. En los dos casos, el mecanismo de base es proyectivo: proyectar sobre épocas pasadas tanto en los buenos aspectos de nuestros objetos internos, como en los malos - en los dos casos, nuestro mundo interno está escindido y nuestra percepción de la realidad distorsionada. El presente intenta inscribirse en una aproximación atenuada de la inevitable pregunta '¿qué impacto ha tenido sobre la evolución de nuestras sociedades occidentales el descubrimiento freudiano del inconsciente dinámico y de la sexualidad infantil?' Una metáfora viene a la mente: aquella del mito de Pandora la cual, guiada por su curiosidad, abre la caja que custodiaba, a pesar de la prohibición de Zeus, dejando escapar los vientos maléficos sobre toda la humanidad. ¿Es acaso una curiosidad mórbida la que nos empuja a abrir la caja de Pandora al mundo inconsciente y de los deseos reprimidos? ¿Podemos atenuar el daño recurriendo a otra metáfora: aquella del aliento, imagen del espíritu que anima a los seres y les da la vida? ¿La exploración psicoanalítica y las teorías que la avalan representarían, bajo este ángulo, una conquista universal del pensamiento humano? ¿Qué formas haría falta darles para obtener de ellas unos efectos benéficos para la sociedad entera? Esta problemática será explorada a la luz de la evolución del status dado al pensamiento y a la práctica psicoanalítica en el espacio cultural francófonol


Il est risqué d'évaluer l'impact d'un phénomène sur l'époque dans laquelle on vit. Le risque est double: soit idéaliser le passé, donner dans la nostalgie et juger négativement son époque - soit idéaliser le présent, tout sacrifier au modernisme et rejeter les valeurs du passé au nom du progrès. Dans les deux cas, le mécanisme de base est projectif : projeter sur des époques révolues tantôt les bons aspects de nos objets internes, tantôt les mauvais – dans les deux cas, notre monde interne est clivé et notre perception de la réalité faussée. Le présent essai s'inscrit dans une approche tempérée de cette question incontournable «quel impact a eu sur l'évolution de nos sociétés occidentales la découverte freudienne de l'inconscient dynamique et de la sexualité infantile?» Une métaphore vient à l'esprit : celle du mythe de Pandore qui, guidée par sa curiosité, ouvrit la jarre dont elle était dépositaire, malgré l'interdiction de Zeus, laissant échapper des vents maléfiques sur toute l'humanité. Est-ce une curiosité morbide qui nous pousse à ouvrir la boîte de Pandore du monde inconscient et des désirs refoulés? Pouvons-nous tempérer le danger en recourant à une autre métaphore: celle du souffle, image de l'esprit qui anime les êtres et leur donne vie? L'exploration psychanalytique et les théories qui en rendent compte représenteraient-elles sous cet angle un acquis universel de la pensée humaine? Quelles formes faut-il leur donner pour en tirer des effets bénéfiques pour la société tout entière? Cette problématique sera explorée à la lumière de l'évolution du statut donné à la pensée et à la pratique psychanalytique dans l'espace culturel francophone

El psicoanálisis multifamiliar, que consiste en la reunión de varios pacientes con sus padres alrededor de co-terapeutas, es todavía poco conocido en Europa. El objetivo de este trabajo es promover este nuevo abordaje terapéutico en la adolescencia. Dicho dispositivo, por su parte, favorece el "desinvestimiento" de los objetos parentales, paradójicamente en presencia de aquellos. Apoyándonos sobre un material inter- y transgeneracional, y sobre los juegos identificatorios entre los participantes, intentaremos demostrar cómo las asambleas multifamiliares estimulan las capacidades asociativas, potencian la elaboración de los conflictos psíquicos y promueven un nuevo despliegue de la libido sobre objetos sustitutivos. Nuestra contribución aspira, en definitiva, a fomentar la orientación psicoanalítica de este tipo de abordaje terapéutico, también heredero de la orientación sistémica


Multi-family psychoanalysis, which consists in gathering several patients and their parents together with co-therapists, is barely known in Europe. The objective of this paper is to contribute to the promotion of this original therapeutic approach for adolescents. Indeed, this kind of therapy contributes to the various operations leading to parental objects' decathexis, paradoxically in their very presence. Using the inter- and transgenerational material and the play of identifications among participants, we point out how multi-family gatherings reinforce the ability to associate, to relaunch conflictuality and redeploy libido onto objects of substitution. Our contribution aims in fine at promoting the psychoanalytic approach of the multifamily therapies, which is also inspired by the systemic approach

RESUMO

Este trabajo es una invitación a la reflexión sobre nuestra escucha y quehacer en la clínica psicoanalítica contemporánea. Se parte de la base que los cambios socioculturales en nuestra sociedad postmoderna, entre ellos la irrupción e inmersión de las nuevas tecnologías, producen transformaciones y aparición de configuraciones subjetivas inéditas. Pensamos que este panorama nos convoca como profesionales a afinar nuestra escucha y ampliar nuestra mirada, si queremos acompañar y sostener a nuestros adolescentes. Somos partidarios del "uso" de dispositivos en la consulta si así lo solicitan, para facilitar la representación de su mundo interno, en la interacción continua entre el empuje pulsional y la relación objetal. Se presentan distintas viñetas clínicas, provenientes de casos del Programa de Apoyo a la Salud Mental Infanto Juvenil de Aragón (AAPIPNA), en las que se plasma el trabajo psicoterapéutico


This paper is an invitation to consider our listening and way of proceeding in our contemporary psychoanalytic clinical work. It takes as a starting point for reflection the sociocultural changes in our postmodern society, including the emergence and the immersion of new technologies, which produce transformations and the appearance of new types of subjective organizations. This scenario calls us as professionals to attune our listening and broaden our view, in order, to accompany and support our adolescents. We are in favor of the "use" of devices in the consultation room if we are requested to, to facilitate the representation of their internal world, in the continuous interaction between the drive and the object relationship. Different clinical vignettes are presented, all cases that belong to the Program of Support for Child and Adolescent Mental Health in Aragon (AAPIPNA), which shows the psychotherapeutic work

Les discours contemporains sur l'enfance sont traversés par la réflexion à propos de l'exercice de la parentalité. L'inquiétude des parents pour acquérir un savoir-faire avec cet enfant est traversée par le discours médical, selon lequel les enfants sont vulnérables à des épidémies qui apellent à une surveillance de la part des parents et les poussent à exiger l'entrée d'un tiers dans leur rapport aux enfants: parmi les plus récentes, l'épidémie d'autisme. Un souci excessif des signes cliniques d'une pathologie révèle paradoxalement un «ne pas vouloir savoir» de l'enfant dans son altérité. Cet essai théorique s’utilise de l'exemple de l'enfant autiste comme une métaphore représentative de l'enfant idéal, c'est-à-dire celui construit et nourri dans l'imaginaire parental. Cet enfant idéal ne fait pas appel à l'Autre et n'appelle pas les parents à l'exercice de la parentalité, dans la mesure où elle remet en cause le lieu de la transmission

RESUMO

While on the one hand therapists can count on a number of codified and standardized diagnostic procedures, on the other hand it is hard to believe that in their daily work therapists slavishly follow these standardized procedures. In a clinical assessment, the diagnosis seems to be the outcome of a naïve and fuzzy process that is strongly influenced by personal training, by, theoretical models, and by one's masters as well as the experience gained in the field. What happens inside the no man's land of the clinical encounter? Can we identify some landmarks in these "amoeboid" exploratory moves? This paper addresses diagnosis not only as a noun or name ("diagnosis"), but also as a verb ("diagnosing"), arguing that a diagnostic space opens up thanks to an oscillatory state of mind that emerges at the intersection of different ways of looking "through" the symptom.

En el siguiente trabajo se realiza una breve articulación de referencias de autores que, desde el marco teórico psicoanalítico, no sólo manifiestan la importancia del juego en su carácter técnico, sino también ético, y cons-titutivo tanto para un sujeto particular como para el psicoanálisis mismo. Se ilustrará el recorrido a la luz de un caso clínico


In the following article we give a brief reference of authors who, from the psychoanalytic theoretical framework, not only state the importance of the game in its technical character, but also its ethical and constitutive role both for a particular subject and for the psychoanalysis itself. The steps of a clinical case will be illustrated
