RESUMO

In Freud's day, his case studies, coupled with the findings of others (e.g., Breuer, Charcot), provided evidence that the Oedipus complex and his dynamic theory of repression (i.e., the unconscious interactions between id, ego, and superego) as causal agents in neurotic symptoms was suspect. With the aid of over 75 years of research since Freud's death, today's psychodynamic-orientated clinicians have discarded many of Freud's tenets related to the Oedipus complex. Modern psychoanalysts have focused their attention instead on a patient's personal relationships in their early life, their current life, and in their interactions with their therapist (transference).

RESUMO

The question of transference has been extensively covered in nursing literature. Faced with a patient approaching the end of life, caregivers, emotionally shaken, sometimes lose their therapeutic bearings. By drawing on the principles of countertransference, they can support these patients with all the necessary empathy, without becoming lost in the complexity of transference issues.

RESUMO

The author contends that psychoanalytic theory has generally presented religious beliefs as developmentally immature or pathological. This viewpoint has resulted in a neglect of religion on the part of psychoanalysts and an avoidance of their religious life by patients. Even though there has been an evolution from the traditional Freudian foundational approach to religion as an "illusion" to the inclusion of psychoanalytical training within some Christian institutes and attributions that psychoanalysis, itself, is a religion, religious beliefs should be included in psychotherapy because they can become involved in transference and countertransference issues in ways that are ignored if religious issues are not discussed in therapy. The author presents clinical material to illustrate this problem.

RESUMO

OBJECTIVE: Insight and affect awareness are correlated with outcome in a number of studies across different treatment orientations. In this study, we perform a full mediational analysis to examine whether improvement of both cognitive insight and affect awareness are mediators of the specific effects of transference work in dynamic psychotherapy. METHOD: This was a dismantling randomized controlled clinical trial specifically designed to study long-term effects of transference work (exploration of problematic patterns in the therapeutic relationship). One hundred outpatients were randomly assigned to 1 year of dynamic psychotherapy with and without transference work interventions. The outcome variables were the Interpersonal Functioning scale (clinician rated) and Inventory of Interpersonal Problems (patient self-report). Quality of Object Relations was moderator. Using structural equation modeling, we tested change during treatment (1 year) of Insight and Tolerance for Affects as mediators of long-term outcome (over the 1-year study period). RESULTS: For both outcome measures, the best model supported a mediated moderation model where the effects of transference work, for patients with low Quality of Object Relations, were mediated via both change of Insight and Tolerance for Affects. The effect of Insight on outcome was significantly reduced due to an indirect effect via Tolerance for Affects. A number of alternative models allowed us to rule out alternative pathways with some confidence. CONCLUSIONS: Both improved insight and affect awareness seem to be mechanisms for long-term effects of transference work. Our results bridge the gap between mainstream clinical theory and empirical research. (PsycINFO Database Record (c) 2019 APA, all rights reserved).

RESUMO

In this essay I wish to present some reflections on Jordan Belfort, the protagonist of the movie "The Wolf of Wall Street" from a psychoanalytic prism. The movie, "The Wolf of Wall Street", is a 2013 black comedy film directed by Martin Scorsese and adapted by Terence Winter from Belfort's memoir (2007) of the same name. This movie has already been analyzed from cultural and historical perspectives, with the protagonist representing American culture of the 1980s. I will first summarize some of these views, and then present my psychoanalytic perspective of Jordan's wish to become "Master of the Universe" (Wolfe, 1987; Grunberger, 1993), as expressed through his abuse of drugs, hyper-sexuality, and his aggressive and self-destructive behavior. As the craving for omnipotence and immortality is a universal wish that has existed from time immemorial, I will draw an analogy between certain aspects and symbolic elements in "The Wolf of Wall Street" and Wagner's (1848-1872) four epic operas "Der Ring des Nibelungen." I will conclude with a brief reference to the charismatic appeal of a man like Jordan to the general public.

RESUMO

This paper investigates the concept of "total situation" which, even though introduced into psychoanalytic thinking via sister disciplines, such as sociology, the neurosciences, etc., has gradually acquired a relatively prominent position in current therapeutic practice. It is used as a metaphor for the envelopment of the unfolding transferential and related events in the analytic process. Irrespective of whether one focuses on the individual analytic condition or the group-analytic one, contemporary psychoanalytic perspectives include both the bi-personal unconscious interactions and the various levels of the total situation in their conceptualizations of the nature of the process. Such a complex approach in conceptualization can only be achieved through the so-called binocular vision of the analyst.

RESUMO

Hypnosis predates psychoanalysis, when autohypnotic pathologies were identified through the lens of hypnosis, and labeled "hypnoid hysteria" in the language of the day. The broad spectrum of disorders then subsumed under that term is still reflected in ICD-10's subset, "F44-Dissociative (Conversion) Disorders." Freud initially embraced both hypnoid hysteria and hypnosis, but came to abandon hypnosis and, by extension, hypnoid hysteria as well. Since that fateful decision, which I term herein Freud's "Inaugural Category Mistake," references to both hypnosis and hypnoid pathology largely vanished from the psychoanalytic mainstream, thereby neglecting conditions afflicting a significant portion of the mentally ill, and needlessly restricting the therapeutic repertoire of psychoanalysis. This contribution argues that psychoanalysis could best re-embrace hypnosis and hypnoid pathology together, as a related pair, and would benefit from doing so. Two examples of the differences of understanding and interventions such a rapprochement might encourage are offered: (a) how hypnoid pathology alters the transference and countertransference; and (b) how the appropriate use of hypnosis alters the nature of interpretation.

RESUMO

In this article, I will describe the way in which I work with enactment-prone dissociative patients in the transference. This approach requires an appreciation of the phenomena of hypnosis and the auto-hypnotic aspects of some forms of dissociation. Essentially, I learn from the patient and my interactions with the patient how hypnotic phenomena and auto-hypnotic defenses manifest themselves in the therapeutic relationship in order both to understand them and ultimately to bring them under conscious control. Because of the fluidity and turbulence of these states, I use the analogy of catching a wave, in which timing and balance are essential, albeit elusive factors in effecting a successful treatment. The importance of having experience with many patients, attending conferences, seeking supervision, and undergoing one's own therapy will be also discussed as important prerequisites for the clinician endeavoring to utilize this type of approach. This preparation, this quest for such a "balance," is modeled after the so-called tripartite model of training employed in psychoanalytic training institutes. I will offer clinical material to illustrate this approach, which I have described as "psychoactive psychotherapy." In such treatments, the clinician may be taken by surprise and is likely to be thrown "off balance" from time to time. The mutually shared understanding of such moments is essential to regaining clinical balance in the therapeutic setting, and can lead to if not create important turning points in the treatment process.

RESUMO

The relationship between hypnosis and psychoanalysis has always been complex, and often acrimonious. When Freud abandoned hypnosis as both the foundation of his theory building and his clinical intervention of choice, the two approaches and the theories associated with them have often seemed separated by enormous rifts, theoretical, clinical, and political. Yet throughout their considerable estrangement for over more than a century, each has much to offer to the other. In this contribution, I briefly review efforts by psychoanalysts to conceptualize and understand hypnosis, offer an introduction to the relatively new post-modern intersubjective approach to psychoanalysis, and demonstrates how the relational concepts associated with that perspective can be of significant help in preparing patients for hypnosis, and in crafting interventions geared to the dynamics of the individual patient, which enhance the likelihood of clinical success. Key relational elements are illustrated with clinical vignettes.

In the first part of the paper, Little outlines the technical difficulties raised by transference interpretations in certain patients, who do not fit within a clearly defined group, which prompts her to question the type of transference developed by these patients. The absence of the "as if" quality in this type of transference leads the author to re-evaluate both our understanding of, and technical approach to, these treatments. In the second part of the article, Little endeavours to insert her clinical reflections within the context of psychoanalytic thought of the time. She postulates that certain phenomena be considered in reference to her concept of "basic unit of being" as a primal state of undifferentiation

The author moves away from a narrow definition of the concept of countertransference, which one might conceive as a response to transference, to an expansive theory of countertransf eren ce, which furthermore comprises a precession of countertransference, in turn conditioning the development of the transf eren ce. He argues for the speci:ficity of psychoanalytic thinking, precisely in that it takes countertransference as a starting point, differentiating it from other more intellectual approaches to the question. He maintains the idea of non-neutrality in analytic listening and the relationship between this and identi:fication, inscribed within a situation conducive both to the elucidation of and resistance to unconscious phenomena. He explores the connections between different types of thinking, regression and countertransference

Drawing upon his experience in psychotherapy with schizophrenic inpatients, the author sets out his understanding of psychotic transference movements, and the way in which the analyst is summoned in his countertransference. He defines four modalities: a first, in which, drawing upon the pre-symbiotic in Mahler, he discusses autistic relationships in which investment is not human, or is inanimate or very partialized; a second, in which the link is symbiotic and highly ambivalent; a third, in experienced as attacks on the object; and a fourth, in which confusion predominates in the relationship and the object must take on the subject's thinking. All of this is extensively illustrated by clinical material and discussed primarily with authors such as Bion, Rosenf eld and Little

Departing from the Freudian notion of psychical working-over [Verarbeitung], this article explores working through [Durcharbeiten], conceived as overcoming the analysand's vanous forms of resistance, vital to the psychoanalytic process. Having already prompted various developments in the analysis of neurotics, the high incidence of non-neurotic disorders necessitates a reconsideration of what working through entails in these cases. To do this the author turns to the model of the resistances described by Freud in Inhibition, Symptom and Anxiety. Of the resistances emanating from the ego, those of the defence mechanisms are characterized by the predominance of pre-repressive defences. The transference resistances derive from primitive forms of transference, adopting diverse modalities. The superego resistances stem from the action of a sadistic superego upon a masochistic ego, while those of the id include the compulsion to repeat "beyond the pleasure principle". This is the analysis of early and unrepresentable trauma. The notion of Verarbeitung may be considered a form of binding [Bindung]; the attempt to bind the energy-released by trauma-with a representation, particularly through construction. A clinical vignette is also considered

A historical look at the metapsychological theorization of transference reveals that it pertains first and foremost to the procedurality involved in the diverse modalities of psychic work. A transposition of the unconscious processes onto another must take place in arder for the processes to become efficient, by way of an identification with the thought processes of this other. The transference of authority onto another anticipates the future founding identification. The psychoanalytic treatment of patients presenting with a somatic condition enables a reflection upan the involvement of the mechanism of transposition in the foundation of the transference in the session

This article sets out to clarify certain fundamental questions of psychoanalysis to doctors and psychologists in hospital training. Psychoanalytic paradigms have shifted throughout history to adapt to the different observational vertices of psychic reality. As is currently occurring in the neurosciences, the primary focus seems to be based around understanding the capacity for thinking and symbolizing and thus, inevitably, study has been oriented towards psychosis, the illness most disorganizing to thought. In this paper, the author seeks to understand the full meaning of the concept of "insight", to trace the reasons for its fragility or indeed complete absence in this illness, while at the same time endeavouring to draw sorne technical conclusions in terms of treatment approaches

This paper investiga tes various countertransf eren ce modalities from the conceptual framework of the oedipal situation as a process that groups together distinct forms of working through triangularity. The capacity to bear the triangular situation and the "third" (in prototypical form, the paternal penis or the father himself) is bound up with the capacity to tolerate the link between differentiated objects, both capacities together underpinning the constitution and maintenance of the mental space. To know and recognize varying intensities and forcefulness in the analyst's countertransference experience allows him to connect with the nature of the relationships between the patient's objects, the vicissitudes of their relationship with the "third", and their abilities and inabilities to sustain an interna! space which fosters thoughts and personal and distinctive meanings. Various examples are used to demonstrate the clinical manifestation of these ideas, presenting the clinical material of a young woman to illustrate how the countertransf eren ce process helps the analyst both to experience and understand the patient's difficulties in maintaining a relationship with the "third", and to consider what it means in relation to the mental space in the patient, the analyst and the analytic encounter

The experience of intimacy may be regarded an emotional one: What is intimacy in the practice of psychoanalysis? How is it constructed, which factors facilitate and impede it? The article begins with sorne overarching considerations on intimacy in life. It defines intimacy as an emotional experience of contact with oneself and with another subject. It examines how intimacy is experienced in the analytic relationship, given that the encounter between two subjects with their subjectivities generates a zone of emotional turbulence that, when tolerated, transformed symbolically and, consequently, thought, can open the way for knowledge of the intimacy of one's own emotions and those of the other. The emotional experience of contact with the other and the connection between the two enables understanding of what is occurring within oneself and the other. Later, it suggests that intimate experience be considered along a gradient, at the extremes of which (autistic isolation and narcissistic fusion) intimacy would not be possible, with varying degrees between the two. The study goes on to describe the phases of intimacy. Lastly, clinical vignettes are introduced to illustrate the above

Based on respective papers by Carlos Paz, who conceptualizes the case of the Wolf Man as a "melancholic-hypomaniacal structure", we consider the re-analysis of Pankejeff with Mack Brunswick, focusing on the psychotic transference of this patient from the vertex of melancholia, suggesting that in this structure, in the absence of a signifying articulation in phantasm, a compensation occurs insofar as the transference object remains present. The psychotic episode is elicited in the transference, in that the wolves return as the J ouissance of the Other, awakened and activated by the abandoning effect of Freud, and by the transference link with the woman-mother analyst, which produce an effect of calling or signalling to the signifier in the Real. In the fantasy of Being Freud's favourite patient we do not detect a neurotic phantasm, rather a "statement of certainty" which we would categorize as a "maniacal conviction". In the same way, throughout the paper, we test out a series of theoretic-clinical approaches and links between Sigmund Freud, Melanie Klein andJacques Lacan
