RESUMO

The significance of electricity for medicine in the modern industrial age should not be underestimated. Particularly in connection with neurasthenia, electrotherapeutic approaches also experienced a boom for domestic use. Thus, electrotherapy reached urology just as it was becoming established as a medical specialty. We analyzed urological manuals and textbooks and objects in the W. P. Didusch Center for Urologic History and the Museum zur Geschichte der Urologie in Berlin to present the wide range of indications for electrotherapy in the emerging field of urology from impotence to urethral strictures and try to highlight the variability of their importance over time.

RESUMO

At the turn of the second decade of the 21st century, advances in urology are driven by technobiologic progress and the strive for minimally invasiveness. Prostate disease benefits above all, with significant improvement in benign prostatic hyperplasia mini-invasive surgery and metastatic prostate cancer imaging. Chronic micturition syndromes also benefit from this trend, so as robotic surgery, whose latest promising innovation, the single port model, will have to find its place and confirm its non-inferiority as compared with the current latest multi-arm da Vinci robot.

PURPOSE OF REVIEW: One of the major functions of the Accreditation Council for Graduate Medical Education (ACGME) is to accredit all approved residency programs. This accreditation system is based on both common and program-specific requirements that form the foundation of all ACGME-accredited training programs. Embedded within the program requirements are the essential elements of the Competencies and Milestones. In this review article, we hope to provide the reader with an overview of the current Milestones and a preview of what lies ahead. RECENT FINDINGS: Milestones for resident education were implemented approximately 7 years ago. The milestones were intended to create a logical trajectory of professional growth which could be measured and tracked for each sub-specialty. However, substantial variability in both content and developmental progression was seen in many specialties. The ACGME has been actively reviewing the Milestones to insure that there exists harmony across all specialties. Much has been learned about the milestones since their implementation. As educators, we need to provide a robust and reproducible system for all to use. The future of resident education, Milestones 2.0, will provide the necessary groundwork for a more user friendly system that will allow adequate evaluation of our trainees.

OBJECTIVE: To provide an overview of the meaning and types of virtual reality (VR) system, its current applications in the field of urology and future implications. SYNTHESIS OF THE EVIDENCE: The concept of VR involves the generation of computer environments with which a user can interact directly. Urology technologies and surgical practices are constantly evolving and RV simulation has become an important complement in urologist training curricula, taking into account not only simulations in surgical techniques, but also in non-surgical techniques such as communication and decision making. VR approaches for image-guided surgery have demonstrated potential in the field of urology by supporting guidance for various disorders. An increasing number of pre and intraoperative imaging modalities have been used to create detailed surgical route maps. The tracing of these surgical roadmaps with the surgical vision of real life has been produced in different ways (electromagnetic, acoustic, optical ...), recommending the combination of several approaches to provide a superior result. One of the disadvantages of navigation systems is soft tissue deformations, requiring confirmatory images. Although early studies report that navigation surgeries provide results equal to or greater than conventional approaches, most of the work has been done in relatively small groups of patients, thus requiring studies with larger sample sizes. CONCLUSIONS: The development of VR offers urologists many opportunities, with surgical simulation being one of its most important applications today. Likewise, the first clinical studies have demonstrated the potential of augmented reality (2D and 3D models) to improve surgical accuracy, describing different navigation systems for different urological surgical interventions.

RESUMO

Imaging studies play a crucial role in the diagnosis of urologic diseases. X-ray and ultrasound studies are used as first-line diagnostic methods. Computed tomography and magnetic resonance imaging (MD-CT and MRI), radionuclide and hybrid methods allow to clarify diagnosis. Currently, the trend "from simple to complex" contributes to obtaining maximum information in the shortest possible time with a minimum cost.

RESUMO

A solving diagnostic and therapeutic problems in patients with various urologic diseases often requires a cooperation between different specialists which emphasizes the interdisciplinarity of our discipline. A number of clinical manifestations and diseases that urologists have to deal with and in which they definitely need a timely assistance from other specialists are described in the article. Determining the connecting links in the work of different specialists, we tried to recall the main " intersection points" and draw attention to the necessary cooperation.

Diagnosis and therapy of male sexual dysfunctions made enormous progress over the last 50 years. Starting with the development of hydraulic penile implants in 1973 meanwhile several effective and well-tolerated conservative treatments such as the PDE-5 inhibitors avanafil, sildenafil, tadalafil and vardenafil, transurethral PGE1 (MUSE) and self-injection therapy with a variety of vasoactive drugs like alprostadil, papaverine/phentolamine (bimix), PGE1/papaverine/phentolamine (trimix/triple drug) or VIP (aviptadil)/phentolamine-Invicorp) have been developed for the treatment of ED. More recently Li-ESWT has provided impressive results both in PDE-5i responders and non-responders and partly also in Peyronie's disease. Regarding premature ejaculation (PE) meanwhile with oral dapoxetine and topical lidocaine/prilocaine spray two effective treatment options are officially available which may be combined in men with severe PE and with PDE-5 inhibitors in men with PE and ED. Hormonal disorders such as hypogonadism, hyperprolactinemia and thyroid disorders may be linked to male sexual disorders and successfully treated with T-substitution, prolactin inhibitors or thyroid specific medications.Whereas vascular surgery for ED such as deep dorsal vein ligation/resection or arterial revascularization procedures dominated the 80ies and 90ies last century they have been considered outdated and are replaced in severe ED of organic etiology by the modern new three piece inflatable penile implants whose high technical standard meanwhile provide 10 years survival rates of about 70 %.

Introducción y objetivos: Hoy en día es casi imposible desvincular la mayoría de las fuentes de conocimiento e información modernos a las tecnologías 2.0. El objetivo de este trabajo es realizar una revisión de las redes sociales (RRSS) científicas (RRSS-C) y el papel que estas desempeñan en la urología actual. Material y métodos: Se realizó una búsqueda bibliográfica en la base de datos PubMed hasta julio de 2018. Se utilizaron los siguientes términos de búsqueda: "Redes sociales", "Urología", "Ciencia", "Investigación". Resultados: Las RRSS ofrecen servicios integrados y herramientas sencillas para la comunicación, la colaboración y la participación. Las instancias prototípicas populares de las redes son Facebook, Twitter o Instagram. Las RRSS no solo han afectado la vida privada y la comunicación personal, sino que también han tenido un alto impacto en el mundo empresarial y la ciencia. En este sentido, el término RRSS-C describe el uso de las plataformas de tecnologías 2.0 en el trabajo científico. Existen diferentes modelos de RRSS-C. Están los identificadores de autor, que son identificadores únicos que permiten gestionar la identidad profesional de cada investigador, distinguiéndolos de otros investigadores y asociando inequívocamente su trabajo. Los perfiles de autor nos ayudan a gestionar nuestro propio perfil académico y a controlar la información disponible sobre nosotros. De esta manera nos aseguramos de que otros investigadores encuentren información correcta y completa sobre nuestra carrera e investigación. Algunos ejemplos de RRSS-C son: ResearchGate, ORCID y Mendeley, entre otros. Conclusiones: Las RRSS-C no solo deben proporcionar información y servicios importantes para la literatura y búsqueda de esta, sino que también podrían ser un catalizador importante para promover servicios apropiados y útiles en el contexto de un nuevo concepto de ciencia, la ciencia 2.0


Introduction and aims: Nowadays, it is almost impossible not to link most of the sources of modern knowledge to information of 2.0 technologies. The aim of this review is to analyse the role of scientific social media (Sc-SoMe) and its potential applications in urology. Material and methods: A literature search was carried out using the PubMed database until July 2018. The research was performed with the following terms: "Social Media", "urology", "science", "research". Results: Social media (SoMe) offers integrated services and easy tools for communication, collaboration and participation. Popular prototypical platforms of SoMe are Facebook, Twitter or Instagram. SoMe not only influence private life and personal communication, but these also affect business and science sectors. In this sense, the term Sc-SoMe describes the impact and usage of 2.0 technologies platforms on scientific work. There are different models of Sc-SoMe such as author identifiers which are unique identifiers that allow managing the professional identity of each researcher, distinguishing them from other researchers and unequivocally associating their work and author profiles. This helps us manage our own academic profile and control the information available about us and ensure that other researchers are finding correct and complete information about our research and career. Examples of Sc-SoMe are: ResearchGate, ORCID, Mendeley among others. Conclusions: Sc-SoMe should not only provide important information and services for literature and literature search. These could also be an important catalyst for promoting appropriate and helpful services in the context of a new concept of science, the science 2.0
