ABSTRACT
Frequencies, magnitudes, and distributions of occurrence can affect the events. The problem can be worse or the solution better if greater frequencies and magnitudes are presented with aggregated distribution in the production system. Indices, hence, are used to assist in decision-making on certain issues. The system formed by Caryocar brasiliense Camb. (Malpighiales: Caryocaraceae), a typical and economically important Brazilian Cerrado tree species, and its several arthropods are adequate to evaluate a new index. This study aimed to test an index to identify the loss and solution sources and their importance in the system's loss or income gain. The index is: Percentage of Importance Indice [...] separated the loss sources [e.g., Edessa rufomarginata De Geer, 1773 (Hemiptera: Pentatomidae) on fruits = 41.90%)] on the percentage of reduction of fruit production (e.g., 0.13%), calculated the attention level (e.g., 0.10/fruit), with a total lost production of 1.35% (≈ 307 total lost fruits). The % I.I. also separated the solution sources [e.g., Zelus armillatus (Lep. and Servi., 1825) (Hemiptera: Reduviidae) = 55.48%), the non-attention level (e.g., Z. armillatus: 0.394 for E. rufomarginata on fruit), with total income gain of 0.56% (≈ 128 total saved fruits) on the natural system (e.g., C. brasiliense trees). This index can calculate losses or the effectiveness of the solutions monetarily. Here I test the % I.I., an index that can detect the key loss and solution sources on the system, which can be applied in some knowledge areas.

O problema pode ser pior ou a solução melhor se maiores frequências e magnitudes forem apresentadas com distribuição agregada no sistema de produção. Índices, então, são usados para assistir na decisão de certas questões. O sistema formado pelo Caryocar brasiliense Camb. (Malpighiales: Caryocaraceae), uma espécie arbórea típica e economicamente importante do Cerrado brasileiro, e seus diversos artrópodes são adequados para avaliar um novo índice. A motivação deste trabalho foi testar um índice capaz de identificar as fontes de perda e de soluções, e suas importâncias em termos de perdas ou ganhos no sistema. O índice é: percentagem de importância [...] separou as fontes de perda [ex., Edessa rufomarginata De Geer, 1773 (Hemiptera: Pentatomidae) em frutos = 41,90%)] na percentagem de redução na produção de frutos (ex., 0,13%), calculando o nível de atenção (ex., 0,10/fruto), com um total de perda de produção de 1,35% (≈ 307 frutos totais perdidos). O % I.I. também separou as fontes de solução [ex., Zelus armillatus (Lep. and Servi., 1825) (Hemiptera: Reduviidae) = 55,48%)], o nível de não atenção (ex., Z. armillatus: 0,394 para E. rufomarginata em fruto), com total de ganho de 0,56% (≈ 128 total de frutos salvos) no sistema natural (ex., árvores de C. brasiliense). Esse índice pode calcular essas perdas ou a eficácia das soluções monetariamente. Aqui eu testo o % I.I., um índice capaz de detectar fatores chaves de perda e de soluções no sistema, capaz de ser aplicado em algumas áreas do conhecimento.

The demand for products to replace high-cost raw materials, such oil and fish meal, in the manufacture of feed for use in aquaculture, while also guaranteeing the nutritional quality of the diets, is increasing. Silage produced with fish and vegetables residues is a low-cost and efficient protein source. The objective of the present study was to evaluate the physiological and biochemical responses of tambaqui fingerlings fed four different levels of silage included in commercial feed with 28% crude protein, over two periods: 45 and 90 days. Each treatment was carried out over three replications, with 10 tambaqui in each 100 L experimental tank. At the end of each established period, blood samples were collected from five animals from each repetition to determine the hematological and biochemical variables. Body weight and total length, hepatosomatic and liposomal indices and hematocrit of specimens fed with diets supplemented with silage did not exhibit significant changes in both assessment period. After 45 days of feeding, the hemoglobin concentration increased when tambaqui were fed a diet including 20% silage. The red blood cell count, mean corpuscular volume and mean corpuscular hemoglobin did not change between treatments in either period. The total protein concentrations increased significantly in the plasma of tambaqui fed with diets with the inclusion of 5 and 10% of silage, evaluated after feeding for 45 days. It was found that the groups which had silage included in their diet did not exhibit significant alterations in the evaluated parameters, and the diet was therefore not consider harmful to the health of tambaqui. Therefore, the use of silage as a feed supplement during tambaqui farming is a sustainable alternative for producers, as it leads to a reduction of impacts of fish and vegetables waste disposal.
A procura por insumos que substituam produtos de alto custo, como óleo e farinha de peixe, na fabricação de rações para uso na aquicultura é crescente, sendo necessário garantir a qualidade nutricional das dietas. A silagem produzida a partir de resíduos de pescado e de vegetais apresenta-se como uma alternativa de baixo custo e eficiente fonte proteica. O objetivo do presente estudo foi avaliar as respostas fisiológicas e bioquímicas de alevinos de tambaqui alimentados com quatro níveis de inclusão de silagem em ração comercial com 28% de proteína bruta, em dois períodos: 45 e 90 dias. Cada tratamento foi realizado em três repetições, com 10 tambaquis em cada caixa experimental de 100 L. Ao término de cada período estabelecido, amostras sanguíneas foram coletadas de cinco animais de cada repetição para determinação das variáveis hematológicas e bioquímicas. Peso, comprimento total e índices hepatossomático e lipossomático de espécimes alimentados com silagem não mostraram alterações significativas em ambos os períodos de avaliação, bem como os valores de hematócrito. A concentração de hemoglobina de tambaqui após 45 dias de alimentação aumentou quando foi fornecida dieta com inclusão de 20% de silagem. A contagem de eritrócitos, volume corpuscular médio e hemoglobina corpuscular média não apresentaram alterações entre os tratamentos, em ambos os períodos. As proteínas totais aumentaram significativamente no plasma de tambaquis que receberam dietas com inclusão de 5 e 10% de silagem, avaliados após 45 dias de alimentação. Evidenciou-se que os grupos com inclusão de silagem na dieta não apresentaram alterações significativas nos parâmetros avaliados, assim não sendo prejudiciais à higidez do tambaqui. Portanto, o uso da silagem como suplemento alimentar durante o cultivo do tambaqui é uma alternativa sustentável para produtores, por promover a redução do descarte de resíduos de pescado e da agricultura.

Objetivo: analisar os custos operacionais de um pronto-socorro relacionados ao atendimento de pacientes COVID-19 em 2020 e 2021. Método: estudo transversal, descritivo de abordagem quantitativa. A mensuração dos custos utilizou-se da perspectiva do gestor hospitalar, por meio de microcusteio por absorção. Custos diretos, indiretos e variáveis, foram avaliados de cima para baixo (top-down). Resultados: o perfil predominante foi de homens, com idades entre 61 e 70 anos, casados, brancos e moradores de Londrina (Paraná, Brasil). O tempo médio de internação para pacientes graves foi 12,20 dias e, para os demais, 8,38 dias. O desfecho principal foi a alta hospitalar. Os custos operacionais em 2020 foram de R$28.461.152,87, já em 2021 os valores encontrados foram R$43.749.324,61. O custo médio do paciente-dia foi de R$2.614,45 em 2020 para R$3.351,93 em 2021. Conclusão: verificou-se aumento dos custos no período estudado. Conhecer os custos operacionais do pronto-socorro, possibilita o planejamento financeiro institucional contribuindo para qualificar a tomada de decisões gerenciais.
Objective: to analyze the operating costs of an emergency room related to the care of COVID-19 patients in 2020 and 2021. Method: cross-sectional, descriptive study with a quantitative approach. The measurement of costs was used from the perspective of the hospital manager, through absorption micro-costing. Direct, indirect and variable costs were evaluated from top to bottom (top-down). Results: the predominant profile was men, aged between 61 and 70 years, married, white and living in Londrina (Paraná, Brazil). The mean length of stay for critically ill patients was 12.20 days and for the others, 8.38 days. The main outcome was hospital discharge. Operating costs in 2020 were BRL 28,461,152.87, while in 2021 the values found were BRL 43,749,324.61. The average patient-day cost went from R$2,614.45 in 2020 to R$3,351.93 in 2021. Conclusion: costs increased in the study period. Be aware of the operational costs of emergency room, enablement or institutional financial planning, contributing to qualify management decision-making.

Los costos directos, indirectos y variables se evaluaron de arriba hacia abajo (top-down). Resultados: el perfil predominante fue el de hombres, con edad entre 61 y 70 años, casados, blancos y residentes en Londrina (Paraná, Brasil). La estancia media de internación de los pacientes críticos fue de 12,20 días y, de los demás, de 8,38 días. El resultado principal fue el alta hospitalaria. Los costos operacionales en 2020 fueron de R$ 28.461.152,87, mientras que en 2021 los valores encontrados fueron de R$ 43.749.324,61. Objetivo: analizar el costo-efectividad y calcular la relación costoefectividad incremental del tratamiento multicapa compresivo con respecto al inelástico (bota de Unna y estiramiento corto) según la literatura actual. Método: estudio cuantitativo de costo-efectividad a través de un modelo con ayuda del software TreeAge® para la elaboración del árbol de decisión. Los supuestos anunciados se obtuvieron mediante el uso de datos secundarios de la literatura para estimar el costo y la efectividad de los parámetros asumidos. Para ello, se realizó una revisión sistemática de la literatura con metaanálisis. Resultados: el árbol de decisión, después del Roll Back, mostró que la terapia multicapa prevaleció sobre las alternativas en el caso base, presentó un costo intermedio por aplicación, pero obtuvo la mayor efectividad. El gráfico del análisis de costo-efectividad también demostró que había un dominio extendido de la bota de Unna sobre el vendaje de estiramiento corto. El análisis de sensibilidad reveló que el vendaje multicapa sigue siendo la alternativa con mayor costoefectividad, dentro del umbral de disposición a pagar. Conclusión: la alternativa con mayor costo-efectividad fue el vendaje multicapa, considerado estándar de oro en la literatura. La segunda alternativa con mayor costo-efectividad fue la bota de Unna, la terapia más utilizada en Brasil.
Objective: to analyze the cost-effectiveness and calculate the incremental cost-effectiveness ratio of multilayer compressive treatment in relation to inelastic (Unna boot and short stretch) therapy according to the current literature. Method: quantitative study about cost-effectiveness through modeling with the aid of TreeAge® software for construction of the decision tree. The anticipated assumptions were obtained by using secondary literature data to estimate the cost and effectiveness of the assumed parameters. A systematic literature review with meta-analysis was performed for this end. Results: the decision tree after Roll Back showed that the multilayer therapy dominated the alternatives in the base case, representing an intermediate cost per application, although with the highest effectiveness. The cost-effectiveness analysis graph also showed extended dominance of the Unna boot in relation to the short stretch bandage. The sensitivity analysis showed that multilayer bandage remains a more cost-effective alternative, within the threshold of willingness to pay. Conclusion: the most cost-effective alternative was multilayer bandage, considered the gold standard in the literature. The second most cost-effective alternative was the Unna boot, the most used therapy in Brazil.
Objetivo: analisar a custo-efetividade e calcular a razão de custoefetividade incremental do tratamento compressivo multicamadas em relação ao inelástico (bota de Unna e curto estiramento) de acordo com a literatura atual. Método: estudo quantitativo sobre custo-efetividade por meio de modelagem com auxílio do software TreeAge® para a construção da árvore de decisão. Os pressupostos anunciados foram obtidos pelo uso de dados secundários de literatura para estimativa do custo e efetividade dos parâmetros assumidos. Para tal, foi realizada uma revisão sistemática de literatura com metanálise. Resultados: a árvore de decisão, após Roll Back mostrou que a terapia multicamadas dominou as alternativas no caso-base, representando custo intermediário por aplicação, porém, com a maior efetividade. O gráfico da análise de custo-efetividade também mostrou uma dominância estendida da bota de Unna em relação à bandagem de curto estiramento. A análise de sensibilidade mostrou que a bandagem multicamadas permanece como alternativa mais custo-efetiva, dentro do limiar de disposição para pagar. Conclusão: a alternativa com maior custo-efetividade foi a bandagem multicamadas, considerada padrão ouro na literatura. A segunda alternativa mais custo-efetiva foi a bota de Unna, terapia mais utilizada no Brasil.

Introduction: the study sought to determine how the passage of values between generations is processed, in view of the exposure to conditions of deprivation to which these people are subjected, while they carry the obligation to ensure the minimum for their children but do not have the necessary resources. This raises the question of how it is possible to affirm a notion of right and wrong within this social framework. this study analysed the transmission of values between generations in groups of people in unfavourable economic and social conditions. Methods: this is a qualitative descriptive study, interviews were conducted through group meetings to discuss issues related to morals. It was carried out with grandparents, their children, and their grandchildren who are residents of a peripheral neighbourhood of the capital of São Paulo and live in restricted economic conditions and under the daily impact of violence. These discussions involved the three generational components of three families. Different arrangements were made that involved the same generational group in an initial meeting and, in another meeting, the mixture of different generations and different families responding to pre-established themes that involved moral dilemmas focused on the daily cultural and social life of these families. The theoretical basis of this study was psychoanalysis, using the concepts of Identification, Ego Ideal, Ideal Ego, Oedipus, and the dynamics underlying the insertion of the father in the Oedipal triad.Results: the grandfathers were ambiguous because they have a reference to a law of morals transcending space and time. Parents, realising the ambiguity of these grandparents in relation to reality, reformulate these moral questions, basing their convictions on a law that is established within the contingencies of everyday life and, therefore, changeable. The adolescent grandchildren perceive this ambiguity in the parents and in their speech. Conclusion: there is greater identification of moral standards among grandchildren and grandparents than between children and parents. The interviewed parents were ambiguous in the way they act and speak, with the transgenerational reproduction model being more guided between grandchildren and grandparents than in the sequential temporal generational sequence.
Introdução: o estudo procurou averiguar como se processa a passagem de valores entre gerações, face à exposição a condições de privação a que são submetidas estas pessoas, enquanto carregam a obrigação de assegurar o mínimo aos filhos mas não dispõem dos recursos necessários. Isso levanta a questão de como é possível afirmar uma noção de certo e errado dentro dessa estrutura social. este estudo analisou a transmissão de valores entre gerações em grupos de pessoas em condições econômicas e sociais desfavoráveis. Métodos: trata-se de um estudo qualitativo descritivo, foram realizadas entrevistas por meio de encontros grupais para discussão de questões relacionadas à moral. Foi realizado com avós, filhos e netos moradores de um bairro periférico da capital paulista e que vivem em condições econômicas restritas e sob o impacto cotidiano da violência. Essas discussões envolveram os três componentes geracionais de três famílias. Diferentes arranjos foram feitos envolvendo o mesmo grupo geracional em um encontro inicial e, em outro encontro, a mistura de diferentes gerações e diferentes famílias respondendo a temas pré-estabelecidos que envolviam dilemas morais centrados no cotidiano cultural e social dessas famílias. A base teórica deste estudo foi a psicanálise, utilizando os conceitos de Identificação, Ideal do Ego. Resultados: os avôs se mostraram ambíguos pois possuem uma referência de Lei, de moral, deslocada no espaço e no tempo; os pais percebendo a ambiguidade destes avôs em relação à realidade reformulam estas questões morais pautando suas convicções em uma Lei que se estabelece dentro das contingências do cotidiano e, portanto, mutável; os netos adolescentes percebem esta ambiguidade nos pais e nos seus discursos. Conclusão: há maior identificação de padrões morais entre netos e avôs do que entre filhos e pais. Os pais entrevistados se mostraram ambíguos na forma de agir e de discursar, sendo o modelo de reprodução transgeracional mais pautado entre netos e avôs do que na sequência geracional temporal sequencial.

ABSTRACT Purpose: To present the results of a retrospective study regarding the clinical and economic impact of intracameral cefuroxime administration to prevent endophthalmitis during cataract surgery in a referral hospital. Methods: This study included 16,902 eyes from patients who had undergone cataract surgery between 2013 and 2017. From May 2014 onwards, all patients received routine intracameral injections of 1 mg cefuroxime (10 mg/1 mL) after phacoemulsification. The prophylactic efficacy was evaluated using the relative risk ratio, whereas the economic impact was evaluated using number needed to treat to avoid endophthalmitis. Results: Before introducing cefuroxime, 3,407 cataract surgeries were performed using the phacoemulsification technique, and 7 post-operatory cases of endophthalmitis occurred (0.2% incidence). After introducing the cefuroxime protocol, 13,495 surgeries were performed, and 4 endophthalmitis cases were registered (0.03% incidence). Cefuroxime was identified as a protective factor against the development of endophthalmitis [risk ratio = 14%, p=0.002, 95% confidence interval (CI) 95%, 4%-49%], with an economic impact of number needed to treat = 568. The potential savings with cefuroxime was approximately US $2,334.36 for every 568 patients treated. Conclusion: The incidence of endophthalmitis decreased by 86% (risk ratio = 14%, p=0.002, 95% CI, 4%-49%) after introducing intracameral cefuroxime prophylaxis at the study hospital. The results presented herein provide strong evidence for the use of cefuroxime in endophthalmitis prophylaxis after phacoemulsification surgeries, outperforming the alternative by providing both economic and clinical benefits.
RESUMO Objetivo: Apresentar os resultados de um estudo retrospectivo sobre o impacto clínico e econômico da administração de cefuroxima intracameral para prevenir endoftalmite nas cirurgias de catarata em um hospital de referência. Métodos: Este estudo incluiu 16.902 olhos de pacientes submetidos à cirurgia de catarata entre 2013 e 2017. A partir de maio de 2014, todos os pacientes receberam rotineiramente uma injeção intracameral de 1mg de cefuroxima (10mg/1mL) ao final da cirurgia de facoemulsificação. A eficácia da profilaxia foi avaliada usando o risco relativo e o impacto econômico foi avaliado com o número necessário para tratar para se evitar um caso de endoftalmite. Resultados: Antes da introdução do protocolo da cefuroxima, foram realizadas 3.407 cirurgias de catarata por facoemulsificação e ocorreram 7 casos de endoftalmite pós-operatória (incidência de 0,2%). Após a introdução do protocolo da cefuroxima, foram realizadas 13.495 cirurgias e registrados 4 casos de endoftalmite (incidência de 0,03%). A cefuroxima foi um fator de proteção no desenvolvimento de endoftalmite (risco relativo = 14%, p=0,002, Intervalo de Confiança de 95% [IC 95%], 4% - 49%) e o impacto econômico do número necessário para tratar = 568. A economia potencial com a cefuroxima foi de aproximadamente US$ 2.334,36 para cada 568 pacientes tratados. Conclusão: A incidência de endoftalmite diminuiu 86% (risco relativo = 14%, p=0,002, IC 95% 4% - 49%) desde a introdução da profilaxia com cefuroxima intracameral no hospital do estudo. Os resultados apresentados mostram forte evidência para o uso da cefuroxima na profilaxia da endoftalmite após cirurgias de facoemulsificação, por proporcionar economia de custos e benefício clínico.

El abordaje onto epistémico de este ensayo se desarrolla en tres grandes miradas: un desarrollo gnoseológico orientado a precisar diferentes posicionamientos de la economía feminista como un movimiento político, herramienta de lucha y resistencia para enfrentar un modelo económico misógino, patriarcal, excluyente y a espaldas de la vida. Seguidamente, se precisan aspectos relevantes de su historicidad, partiendo de las figuras más resaltantes de la economía neoclásica. El hombre dentro de esta perspectiva (Homus Economicus), es el motor de la producción de riqueza, el agente económico por excelencia. Modelo donde no aparecen las relaciones sociales, las mujeres, la vida Solo producir y consumir. Finalmente, se destaca toda una serie de avances y desafíos. Dentro de estas nuevas perspectivas, se pone especial atención a las relaciones de cuidado y afecto que se producen en los hogares como trabajo no remunerado. El cuidado de la vida humana toma el escenario, exigiendo nuevas perspectivas de análisis y la ampliación de fronteras a fin de destacar su relevancia social y económica; a partir de la utilización de estrategias para la valoración de la producción doméstica incorporando nuevas propuestas sobre el manejo de las estadísticas con lentes de género. The ontologic epistemic approach in this essay is developed in three main perspectives: a gnoseological development aimed at specifying different positions of feminist economics as a political movement, a tool of struggle and resistance to face a misogynistic, patriarchal, exclusionary economic model behind the back of life. Next, relevant aspects of its historicity are specified, starting from the most outstanding figures of neoclassical economics. Man within this perspective (Homus Economicus), is the engine of the production of wealth, the economic agent par excellence. Model where social relationships, women, life do, not appear... Just produce and consume. Finally, a whole series of advances and challenges are highlighted. Within these new perspectives, special attention is paid to the relationships of care and affection that occur in households as unpaid work. The care of human life takes the stage, demanding new perspectives of analysis and the expansion of borders in order to highlight its social and economic relevance; from the use of strategies for the valuation of domestic production, incorporating new proposals on the management of statistics with gender lenses. The government Budget, should be presented disaggregated, and the effect of expenditure and revenue policies should be analyzed in a differentiated and situated manner(AU)

Resumen Antecedentes: La emergencia sanitaria por COVID-19 causó un aumento de la demanda por hospitalización y costos elevados para el sistema de salud. Objetivo: Estimar los costos directos de la atención por COVID-19 desde la perspectiva del proveedor de servicios en un hospital de segundo nivel que fue reconvertido durante el primer año de la emergencia sanitaria. Material y métodos: Estudio observacional retrospectivo. La información sobre cantidades de bienes y servicios se obtuvo de los sistemas de información SINOLAVE y CVOED, así como de fuentes administrativas del hospital. Se definieron cuatro grupos de costos y se valoraron en dólares norteamericanos con precios unitarios de 2021. Resultados: La duración promedio de la estancia hospitalaria (n = 3241 pacientes) fue de 10.8 ± 8.2 días. El costo promedio de la atención por paciente fue de USD 6 557 ± 4 997. La terapia respiratoria con ventilación mecánica asistida fue utilizada por 13 % de los pacientes. Conclusiones: Los costos médicos de atención por COVID-19 representaron una gran cantidad de recursos. La mayor parte de los costos (95 %) se derivó de estancia hospitalaria, terapia respiratoria sin ventilación mecánica asistida, así como de costos relacionados con equipo de protección personal, higiene, adecuación a la infraestructura y pagos al personal médico.
Abstract Background: COVID-19 health emergency caused an increase in the demand for hospitalization and high costs for the health system. Objective: To estimate COVID-19 care direct costs from the perspective of the healthcare provider in a secondary care hospital that underwent conversion during the first year of health emergency. Material and methods: Retrospective, observational study. Information on quantities of goods and services was obtained from the SINOLAVE and CVOED platforms and from hospital administrative sources. Four cost groups were defined and estimated with 2021 unit prices in US dollars. Results: Mean hospital length of stay (n = 3,241 patients) was 10.8 ± 8.2 days. Average cost of care per patient was USD 6,557 ± 4,997. Respiratory therapy with assisted mechanical ventilation was used by 13% of patients. Conclusions: The costs of COVID-19 medical care represent a large amount of resources. Most part of the costs (95%) were derived from hospital stay, respiratory therapy without assisted mechanical ventilation and costs related to personal protective equipment, hygiene, infrastructure adaptation and payments to medical personnel.

Fundamento padecer diabetes puede influenciar la percepción del propio individuo y cambiar su rutina diaria, afectando así su calidad de vida; partiendo de ello, la profundización en el tema reviste gran importancia Objetivo determinar la relación entre calidad de vida, edad e ingreso económico en adultos mayores con diabetes mellitus tipo 2. Métodos se realizó un estudio descriptivo, de corte transversal, en 33 adultos mayores con diabetes mellitus tipo 2, seleccionados mediante un muestreo no probabilístico por conveniencia, en la comunidad de las Mariposas (Chillán, Chile), en el año 2019. Para el análisis de la calidad de vida fue aplicado el instrumento Diabetes 39. Se utilizaron las pruebas estadísticas de Kolmogorov-Smirnov y Rho de Spearman. Resultados existió predominio del sexo femenino, así como de personas casadas. La edad tuvo un valor medio de 71,45 años. Con respecto al ingreso económico, un 66,7 % de los participantes refirió imposibilidad para costear los gastos del mes. Entre las dimensiones de la calidad de vida analizadas, las más afectadas fueron: severidad de la diabetes (media de 55,05), energía/movilidad (media de 44,1), y calidad de vida (media de 43,9). La edad y el ingreso económico no mostraron una correlación significativa con ninguna de las dimensiones estudiadas. Conclusiones la calidad de vida de los adultos mayores con diabetes mellitus tipo 2 se ve afectada, sin que exista una relación significativa con la edad e ingresos económicos.

Methods a descriptive, cross-sectional study was carried out in 33 older adults with type 2 diabetes mellitus, selected by non-probabilistic convenience sampling, in the Las Mariposas community (Chillán, Chile), in 2019. The Diabetes 39 instrument was applied for quality of life analysis. The Kolmogorov-Smirnov and Spearman's Rho statistical tests were used. Results the female sex was the predominant, as well as married people. Age had a mean value of 71.45 years. In the economic income, 66.7% of the participants reported the impossibility of paying the expenses of the month. Among the life's quality dimensions analyzed, the most affected were: diabetes severity (mean 55.05), energy/mobility (mean 44.1), and quality of life (mean 43.9). Age and income did not show a significant correlation with any of the dimensions studied. Conclusions the older adults' quality of life with type 2 diabetes mellitus is affected, without a significant relationship with age and economic income.

Abstract Bovine pestiviruses are the causative agents of bovine viral diarrhea, a disease thatcauses severe economic losses in cattle. The aim of this study was to improve their diagnosisby developing a RT-qPCR to detect bovine pestiviruses A, B and H; and to set up a protocolfor collecting, shipping and preserving bovine pestiviral RNA on filter papers. The developedRT-qPCR showed high sensitivity in detecting these viruses in different matrices: viral stocks,semen and serum samples. With regard to the possibility of using the technique to test serumpools, it was possible to identify a positive serum sample within a pool containing 30 sera.In addition to evaluating the qPCR from fresh samples, the use of filter papers to sow bovinesamples was analyzed. The sampling method on two different filter papers using bovine blooddrops was a useful alternative for diagnostic purposes and allowed to preserve pestiviral RNAfor up to 12 months under refrigeration.
Resumen Los Pestivirus bovinos son los agentes causales de la diarrea viral bovina, una enfermedad que genera importantes pérdidas económicas en el ganado vacuno. El objetivo de este trabajo fue mejorar su diagnóstico mediante el desarrollo de una RT-qPCR para detectar los Pestivirus bovinos A, B y H y disenar un protocolo de recolección, envío y conservación de ARN viral en papeles de filtro. La RT-qPCR desarrollada demostró alta sensibilidad en la detección de estos virus en diferentes matrices: stock viral, suero y semen. Respecto de la posibilidad de usar la técnica para testear pools de suero, fue posible identificar un suero positivo dentro de un pool compuesto por 30 sueros. Además de evaluar la qPCR en muestras frescas, se analizó el uso de papeles de filtro para sembrar muestras de bovinos. La metodología de toma de muestras en dos tipos de papeles de filtro usando gotas de sangre fue una alternativa útil para el diagnóstico y permitió conservar ARN viral por hasta 12 meses a temperaturas de refrigeración.

Abstract Background and objectives: Anesthesiologists and hospitals are increasingly confronted with costs associated with the complications of Peripheral Nerve Blocks (PNB) procedures. The objective of our study was to identify the incidence of the main adverse events associated with regional anesthesia, particularly during anesthetic PNB, and to evaluate the associated healthcare and social costs. Methods: According to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we conducted a systematic search on EMBASE and PubMed with the following search strategy: (‟regional anesthesia" OR ‟nerve block") AND (‟complications" OR ‟nerve lesion" OR ‟nerve damage" OR ‟nerve injury"). Studies on patients undergoing a regional anesthesia procedure other than spinal or epidural were included. Targeted data of the selected studies were extracted and further analyzed. Results: Literature search revealed 487 articles, 21 of which met the criteria to be included in our analysis. Ten of them were included in the qualitative and 11 articles in the quantitative synthesis. The analysis of costs included data from four studies and 2,034 claims over 51,242 cases. The median claim consisted in 39,524 dollars in the United States and 22,750 pounds in the United Kingdom. The analysis of incidence included data from seven studies involving 424,169 patients with an overall estimated incidence of 137/10,000. Conclusion: Despite limitations, we proposed a simple model of cost calculation. We found that, despite the relatively low incidence of adverse events following PNB, their associated costs were relevant and should be carefully considered by healthcare managers and decision makers.

Abstract Background and objectives: In this systematic review, we carried out an assessment of perioperative costs of local or regional anesthesia versus general anesthesia in the ambulatory setting. Methods: A systematic literature search was conducted to find relevant data on costs and cost-effectiveness analyses of anesthesia regimens in outpatients, regardless of the medical procedure they underwent. The hypothesis was that local or regional anesthesia has a lower economic impact on hospital costs in the outpatient setting. The primary outcome was the average total cost of anesthesia calculated on perioperative costs (drugs, staff, resources used). Results: One-thousand-six-hundred-ninety-eight records were retrieved, and 28 articles including 27,581 patients were selected after reviewing the articles. Data on the average total costs of anesthesia and other secondary outcomes (anesthesia time, recovery time, time to home readiness, hospital stay time, complications) were retrieved. Taken together, these findings indicated that local or regional anesthesia is associated with lower average total hospital costs than general anesthesia when performed in the ambulatory setting. Reductions in operating room time and postanesthesia recovery time and a lower hospital stay time may account for this result. Conclusions: Despite the limitations of this systematic review, mainly the heterogeneity of the studies and the lack of cost-effectiveness analysis, the economic impact of the anesthesia regimes on healthcare costs appears to be relevant and should be further evaluated.

Abstract Background: High-fidelity (HF) pediatric patient simulators are expensive. This randomized study aimed to compare the quality and educational impact of a full-scale simulation workshop with an HF infant simulator (SimBaby™, Laerdal) or with a low-cost (LC) simulator composed of an inert infant manikin with SimBaby™ software that displays respiratory/hemodynamic parameters on a monitor for medical education in pediatric difficult airway management. Methods: After written informed consent, anesthetists and emergency or ICU physicians participated in teams (4 to 6 participants) in a training session that included direct participation and observation of two difficult intubation scenarios. They were randomized into two groups (HF group, n = 65 and LC group, n = 63). They filled out a simulation quality score (SQS, 0 to 50), self-evaluated their anesthetists' non-technical skills (ANTS) score (15 to 60), and an educational quality score (EQS, 0 to 60) immediately (T0, main criteria), as well as 3 (T3) and 6 (T6) months after the training session. Results: We enrolled 128 physicians. Direct participation SQS (39 ± 5 HF group versus 38 ± 5 LC group), observation SQS (41 ± 4 H F group versus 39 ± 5 LC group), ANTS scores (38 ± 4 HF group versus 39 ± 6 LC group), T0 SQS (44 ± 5 HF group versus 43 ± 6 LC group), T3 and T6 SQS were not different between groups. Conclusion: Our low-cost simulator should be suggested as a less expensive alternative to an HF simulator for continuing medical education in pediatric difficult airway management.

Abstract Background and objectives: Contribution margin per hour (CMH) has been proposed in healthcare systems to increase the profitability of operating suites. The aim of our study is to propose a simple and reproducible model to calculate CMH and to increase cost-effectiveness. Methods: For the ten most commonly performed surgical procedures at our Institution, we prospectively collected their diagnosis-related group (DRG) reimbursement, variable costs and mean procedural time. We quantified the portion of total staffed operating room time to be reallocated with a minimal risk of overrun. Moreover, we calculated the total CMH with a random reallocation on a first come-first served basis. Finally, prioritizing procedures with higher CMH, we ran a simulation by calculating the total CMH. Results: Over a two-months period, we identified 14.5 hours of unutilized operating room to reallocate. In the case of a random ''first come -first serve'' basis, the total earnings were 87,117 United States dollars (USD). Conversely, with a reallocation which prioritized procedures with a high CMH, it was possible to earn 140,444 USD (p < 0.001). Conclusion: Surgical activity may be one of the most profitable activities for hospitals, but a cost-effective management requires a comprehension of its cost profile. Reallocation of unused operating room time according to CMH may represent a simple, reproducible and reliable tool for elective cases on a waiting list. In our experience, it helped improving the operating suite cost-effectiveness.

Resumen Introducción: En Argentina, los medicamentos de alto costo (MAC) generan una carga económica elevada que deben afrontar las instituciones sanitarias. Sin embargo, no existe a la fecha un estu dio en Argentina que indique la magnitud del real problema de los MAC para la Seguridad Social. El presente trabajo, explora cuál es su impacto económico para una de las principales Obras Sociales del país. Métodos: Se realizó un estudio descriptivo con etapa analítica a partir de datos obtenidos en gerencia de prestaciones, área farmacia y área contable de la institución. Cada medicamento fue clasificado según recomendación de OMS (clasificación Anatómica-Terapéutica- Química-ATC). Los precios fueron consignados en tres valores: nominal al momento de adquisición, actualizado a pesos fin de 2021 utilizando el CER (coeficiente de estabilización de referencia), y en dólares (USD). Se evaluaron 105 324 dispensas de MAC, correspondientes a 258 011 unidades para 10 450 afiliados. Resultados: El gasto total anualizado fue 57 millones de dólares (USD), y por usuario 6220 USD. Solo 1.9% de los afiliados requirieron MAC, aunque el gasto fue del 21.9% de los ingresos (aportes + contribuciones). Los primeros 5 medicamentos que generaron el mayor gasto fueron enzalutamida, bevacizu mab, nivolumab, palbociclib, pembrolizumab. Las enfermedades oncológicas y reumatológicas representaron el 62.8% del gasto. Conclusión: A la luz de los resultados, se deduce que los MAC constituyen un riesgo potencial de desfinanciación del sistema de salud si son abordados de manera atomizada por cada subsector. Los MAC requieren de políticas globales de carácter nacional y/o regional.
Abstract Introduction: In Argentina, high-cost drugs (HCD) induce a high economic burden for all the health system sec tors. However, it does not exist in Argentina any data that indicates the real problem of HCD for Social Security. That is why, the present study explores the economic impact of the HCD for one of the main Institutions of the country. Methods: A descriptive study with an analytical stage was carried out based on data obtained from management, pharmacy and accounting area. Each drug was classified according to WHO recommendation (Anatomical-Therapeutic-Chemical-ATC classification). The prices were expressed in three ways: nominal value at the time of acquisition in local currency, updated using the CER (reference stabilization coefficient), and in US dollars. A total of 105 324 HCD dispensed were evaluated, which corresponded to 258 011 units destined to 10 450 patients. Results: Total annualized spend was US$57 million (US$6220 per patient). Only 1.9% of affiliates required HCD, although those expenses represented 21.9% of the institutions´ total income. The first 5 drugs associated to the highest expenditure were enzalutamide, bevacizumab, nivolumab, palbociclib, pembrolizumab. Oncological and rheumatological diseases represented 62.8% of the HCD costs. Conclusion: Considering the results obtained, it can be deduced that if the HCD problem is approached in a scattered way by each subsec tor, it will become a potential risk for health system defund. The HCD topic requires of global policies at national or even regional level.

Objective: The study aimed to describe the profile and economic burden of patients with depression from the perspective of the Brazilian Private Healthcare System (PHS). Methods: A two-step methodological quantitative-qualitative research design was performed: retrospective descriptive analysis of the Orizon database of patients with at least one claim of depression (F33, F38, or F39) in PHS (2013-2019) and experienced physicians perspective from an expert meeting. Results: 1,802 patients fulfilling the eligibility criteria counted BRL 74,978 million across the 4-year period. Over this period, nearly 60% of patients had a medical appointment (6.6 appointments per patient, on average), 61% had a psychologist, 9.8% had a psychiatrist appointment, and an average of 115.2 exams and 8.7 emergency visits per patient were performed. According to the experts, the economic impact of depression is more significant when considering the indirect costs related to productivity loss and impairment in occupational and interpersonal functioning. Conclusion: Identifying and diagnosing patients with depression and their real burden is challenging; even with significant costs identified in the claim database analyses in the Brazilian PHS, the real impact must be higher if indirect costs are considered. The depressive disorder should be prioritized in the Brazilian PHS to establish more adequate health policies.
Objetivo: O estudo teve como objetivo descrever o perfil e a carga econômica de pacientes com depressão na perspectiva do Sistema Único de Saúde (SUS). Métodos: Foi realizado um projeto de pesquisa quantitativo-qualitativo metodológico em duas etapas: análise descritiva retrospectiva do banco de dados Orizon de pacientes com pelo menos uma alegação de depressão (F33, F38 ou F39) no PHS (2013- 2019) e perspectiva de médicos experientes de uma reunião de especialistas. Resultados: 1.802 pacientes que preencheram os critérios de elegibilidade totalizaram R$ 74,978 milhões no período de 4 anos. Nesse período, cerca de 60% dos pacientes tiveram consulta médica (6,6 consultas por paciente, em média), 61% tiveram psicólogo, 9,8% consulta com psiquiatra e foram realizados em média 115,2 exames e 8,7 atendimentos de emergência por paciente . Segundo os especialistas, o impacto econômico da depressão é mais significativo quando considerados os custos indiretos relacionados à perda de produtividade e prejuízo no funcionamento ocupacional e interpessoal. Conclusão: Identificar e diagnosticar pacientes com depressão e sua real carga é desafiador; mesmo com custos significativos identificados nas análises da base de sinistros do SUS brasileiro, o impacto real deve ser maior se considerados os custos indiretos. O transtorno depressivo deve ser priorizado na APS brasileira para o estabelecimento de políticas de saúde mais adequadas.

Objetivo: Avaliar o impacto orçamentário do tratamento com iPARP como primeira linha de manutenção, comparado ao tratamento-padrão a partir de evidências de mundo real sob a perspectiva de um hospital público referência em oncologia no Rio de Janeiro. Métodos: Foi aplicada uma análise de impacto orçamentário para estimar a introdução das tecnologias iPARP, olaparibe e niraparibe, em comparação com o cenário referência, utilizando dados de eficácia e evidências de mundo real, e considerando os custos globais de tratamento da doença em cinco anos. Este estudo foi aprovado pelo Comitê de Ética em Pesquisa, CAAE: 95157018.9.0000.5274. Resultados: A análise demonstrou que o cenário referência apresentou um impacto orçamentário no valor de R$ 3.578.768,04 em cinco anos. No cenário alternativo, o custo incremental do olaparibe chegou a ser 23,8% maior, comparado ao niraparibe, atingindo um custo de R$ 23.736.459,20 versus R$ 18.076.951,81, respectivamente. Os parâmetros que apresentaram maior impacto nas análises para a tecnologia olaparibe foram a difusão da tecnologia e o preço do medicamento. Contudo, para o niraparibe, os parâmetros de maior impacto foram a duração do tratamento, a difusão da tecnologia e a dose utilizada, demonstrando maior suscetibilidade de variação. Conclusão: Os iPARP no tratamento de pacientes com carcinoma de ovário avançado, apesar de apresentarem custo incremental de aproximadamente R$ 23 milhões em cinco anos, apontam para uma potencial redução de custos associados à progressão da doença.
Objective: Assess the budgetary impact of treatment with iPARP as a first line of maintenance, compared to standard treatment based on real-world evidence from the perspective of a public hospital reference in oncology at Rio de Janeiro. Methods: A budget impact analysis was applied to estimate the introduction of iPARP, olaparib and niraparib technologies, compared to the reference scenario, using efficacy data and real-world evidence, and considering the global costs of treating the disease in five years. This study was approved by the Research Ethics Committee, CAAE: 95157018.9.0000.5274. Results: The analysis showed that the reference scenario presented a budgetary impact of R$ 3,578,768.04 in five years. In the alternative scenario, the incremental cost of olaparib reached 23.8% higher compared to niraparib, reaching a cost of R$ 23,736,459.20 versus R$ 18,076,951.81, respectively. The parameters that had the greatest impact on the analyzes for the olaparib technology were technology diffusion and drug price. However, for niraparib, the parameters with the greatest impact were the duration of treatment, the diffusion of the technology and the dose used, demonstrating greater susceptibility to variation. Conclusion: iPARP in the treatment of patients with advanced ovarian carcinoma, despite having an incremental cost of approximately R$ 23 million in five years, point to a potential reduction in costs associated with disease progression.

Objective: To assess healthcare resource utilization and hospitalization costs of patients with chronic obstructive pulmonary disease (COPD) exacerbations in the Brazilian private healthcare system. Methods: A retrospective cohort study, considering data from an administrative database of a private company (Orizon). Patients aged ≥40 years old and with at least one COPD-related claim identified by the ICD-10 code (J40 to J44) at any time during the eligibility period (January/2010 to December/2013) were included in the analysis. Follow-up was performed until December/2014, death or inactivation of a health plan. Sociodemographic characteristics, number of emergency visits, hospital admissions (number and length of stay), length of hospital stay in an intensive care unit (ICU), number of severe COPD exacerbations, therapeutic approach, and hospitalization costs were assessed. Results: The analysis included 8,254 COPD patients. Emergency visits, hospital admission, and exacerbation rates were 0.4, 0.2, and 0.1 per person-year, respectively. The mean length of hospital stays and the length of stay of patients requiring or not ICU stay were 16.6 (SD = 77.0), 8.7 (SD = 36.9), and 27.6 (SD = 109.7), respectively. Mean costs associated to emergency department visits and hospitalizations were 258.2 BRL (SD = 383.1) and 38,165.4 BRL (SD = 124,683.5), respectively. Hospitalizations costs without ICU stay were 11,810.1 BRL (SD = 31,144.1) and 74,585.3 BRL (SD = 182,808.1) for those with ICU utilization. Conclusion: Costs for COPD management during disease exacerbation are very high and may reach almost 75 thousand BRL per hospitalization. The prevention of COPD exacerbations and better disease control may reduce the economic burden on the private healthcare system in Brazil.
Objetivo: Avaliar a utilização de recursos e custos de pacientes com exacerbação da doença pulmonar obstrutiva crônica (DPOC) no sistema de saúde suplementar (SSS) do Brasil. Métodos: Estudo de coorte retrospectiva, considerando banco de dados administrativo de uma empresa privada (Orizon). Pacientes com ≥40 anos e pelo menos um registro de admissão relacionado à DPOC identificado com CID-10 J40-J44, entre janeiro/2010 e dezembro/2013, foram incluídos e acompanhados até dezembro/2014, morte ou inativação no plano. Características sociodemográficas, número de visitas de emergência, admissões hospitalares (número e tempo de hospitalização), tempo de hospitalização em unidade de terapia intensiva (UTI), número de exacerbações graves, estratégias terapêuticas e custos hospitalares foram as variáveis analisadas. Resultados: A análise incluiu 8.254 pacientes com DPOC. As taxas de visita à emergência, internação hospitalar e exacerbação da doença foram de 0,4, 0,2 e 0,1 por pessoa-ano, respectivamente. Os tempos médios de hospitalização, hospitalização sem utilização de UTI e hospitalização com necessidade de UTI foram de 16,6 (DP = 77,0), 8,7 (DP = 36,9) e 27,6 (DP = 109,7) dias, respectivamente. Os custos médios relacionados à visita de emergência e por hospitalização foram de 258,2 BRL (DP = 383,1) e 38.165,4 BRL (DP = 124.683,5), respectivamente. Os custos para pacientes que não utilizaram UTI foram de 11.810,1 BRL (DP = 31.144,1) e de 74.585,3 BRL (DP = 182.808,1) para aqueles com necessidade desse serviço. Conclusão: Os custos para o manejo dos pacientes com exacerbação da DPOC são muito elevados, podendo chegar a 75.000 BRL por hospitalização. A prevenção de exacerbações e o melhor controle da doença podem reduzir esse impacto econômico no SSS.
