ABSTRACT

Abstract Objectives: to analyze protocols and direct Brazilian women to prevent perinatal seizures during the CO VID-19 pandemic, based on the positive perinatal experience. Methods: exploratory analysis of document analysis. In the first phase, from April 11 to May 14, in order to search for words with keywords, 402 documents were found. The second phase consisted of reading the set of selected documents and dividing them into categories. The analysis was carried out in light of the guidelines for a positive perinatal experience of the World Health Organization. Results: as well as directing flow detection phase protocols to mice that show or do not receive COVID-19 symptoms and are being updated with new evidence of systemic evidence. It appears that there are several guidelines for prenatal, partial and puerperium, divergences in some documents for example umbilical cord clamping, skin-to-skin contact, newborn bath. Conclusions: analyze the permissible conclusions that most recommendations are in line with preconceived notions of positive perinatal experience, but is necessary to adaptation to the Brazilian context.


Resumo Objetivos: analisar à luz da experiência positiva na perinatalidade, protocolos e diretrizes brasileiras acerca da atenção à saúde perinatal durante a pandemia da COVID-19. Métodos: pesquisa exploratória de análise documental. Na primeira fase, de 11 de abril a 14 de maio de 2020, foram realizadas as buscas com palavras-chave,encontrando-se 402 documentos. A segunda fase consistiu na leitura dos sete documentos selecionados e divisão do conteúdo nas categorias. A análise foi realizada à luz das orientações para a experiência positiva na perinatalidade da Organização Mundial da Saúde. Resultados: as diretrizes e protocolos fazem distinção do fluxo de atendimento para mulheres que apresentam ou não sintomas da COVID-19 e são atualizados à medida que novas evidências surgem.Constatou-se que dentre as diversas orientações para pré-natal, parto e puerpério, divergências em algumas categorias como clampeamento de cordão umbilical, contato pele a pele, banho do recém-nascido. Conclusões: a análise permitiu concluir que a maioria das recomendações estão em consonância com os preceitos para a experiência positiva na perinatalidade, mas carecem de adaptação ao contexto brasileiro.

ABSTRACT

Abstract Objectives: to present scientific recommendations for perinatal care in the context of the COVID-19 pandemic. Methods: a narrative review was carried out between March and September in 2020 from BIREME, the Scientific Electronic Library Online and the Virtual Health Library databases, based on the descriptors: "Perinatal Care''; "Coronavirus"; "Coronavirus Infection''; "Obstetrics"and "Newborns". The information was grouped into four categories, namely, prenatal care; labor and birthcare; care for puerperal women and the newborn. Results: 14 publications were found, 9 scientific articles and 5 technical standards pursuant to the Ministry of Health in Brazil. In the first category, generally, it is recom-mended that prenatal consultations should be continued, if necessary, by means of telemedi-cine; hospitalization of positive pregnant women in case symptoms are severe and consid-ering pregnant women's clinical and emotional aspects. In the second, screening and testing on suspected cases, discouraging skin-to-skin contact and breastfeeding in the first hour of life. In the third, change the arrangement of beds in shared accommodations and have early discharge. In the fourth category, it is also recommended late clamping of the umbilical cord and observe differences in laboratorial testing of asymptomatic neonates. Conclusions: scientific evidence based on studies with methodological designs and more robust analyzes are necessary to guide perinatal care in the context of the harm-free COVID-19 pandemic.


Resumo Objetivos: apresentar as recomendações científicas para a atenção perinatal no contexto da pandemia de COVID-19. Métodos: revisão narrativa realizada entre março a setembro de 2020, nas bases de dados da BIREME, Scientific Electronic Libary Online e Biblioteca Virtual em Saúde, a partir dos descritores: "Assistência Perinatal", "Coronavírus", "Infecção por Coronavírus", "Obstetrícia" e "Recém-Nascido". As informações foram agrupadas em quatro categorias: assistência pré-natal, assistência ao trabalho de parto e nascimento, assistência à puérpera e assistência ao recém-nascido. Resultados: foram encontradas 14 publicações, 9 artigos científicos e 5 normas técnicas do Ministério da Saúde do Brasil. Em linhas gerais, na primeira categoria, recomenda-se a permanência da realização das consultas pré-natais, se necessário por meio de telemedicina; internamento de gestantes positivas em caso de sintomatologia grave e consideração aos aspectos clínicos e emocionais das gestantes. Na segunda, triagem e testagem dos casos suspeitos, desencorajamento do contato pele a pele e amamentação na primeira hora de vida. Na terceira, mudança na disposição de leitos no alojamento conjunto e alta precoce. Na quarta categoria, clampeamento tardio do cordão umbilical, divergências quanto a testagem laboratorial dos neonatos assintomáticos. Conclusões: evidências científicas baseadas em estudos com desenhos metodológicos e análises mais robustas são necessárias para nortear a assistência perinatal livre de danos no contexto da pandemia por COVID-19.

Proporcionar condiciones térmicas y sistemas de ventilación adecuados que eviten la dispersión de patógenos es fundamental para proteger la salud de los pacientes, de los profesionales médicos y de enfermería, y del resto del personal, así como para el funcionamiento general de los equipos sensibles. Esta segunda versión presenta recomendaciones generales para evitar la transmisión del SARS-CoV-2 a través de sistemas de calefacción, ventilación y aire acondicionado en establecimientos de salud.


Providing adequate thermal conditions and ventilation systems that prevent the dispersion of pathogens, is fundamental to protect the health of patients, caregivers and staff, and to the overall operation of sensitive equipment. This technical note presents general recommendations to prevent the transmission of SARS-CoV-2 through heating, ventilation, and air conditioning systems in ​health care facilities.

ABSTRACT

Objetivo: Observar e relatar aspectos relacionados à segurança em um serviço de hidroterapia do município de Goiânia, Goiás. Casuística e Métodos: Trata-se de um estudo observacional realizado entre agosto e dezembro de 2017, no período matutino, em um serviço de Fisioterapia de Goiânia, Goiás. Primeiramente, realizou-se uma revisão da literatura buscando artigos científicos no Google Acadêmico, Scielo, PubMed, Medline e literatura especializada (normas técnicas, portarias e resoluções). O levantamento abrangeu a literatura nacional e internacional, artigos originais gratuitos e disponíveis na íntegra, sendo utilizados como descritores: acessibilidade, padrões da hidroterapia, risco, contraindicações, perigo e precauções. Em seguida, tomando como referência as normas e recomendações para serviços de hidroterapia encontradas na literatura, foi construída uma tabela constando os aspectos de segurança observados. Resultados: De 31 itens elencados e observados neste estudo, quatro (12,9%) não puderam ser avaliados, 14 (45,2%) atenderam aos padrões e recomendações e 13 (41,9%) apresentaram necessidade de reflexão e melhoria, uma vez que não atenderam às exigências apresentadas nas normas vigentes. Conclusão: O serviço de hidroterapia observado requer a adoção de algumas medidas de segurança, recomendando-se um olhar mais cuidadoso frente ao assunto e um esforço coletivo no sentido de ampliar a discussão sobre segurança nesta área, a fim de tornar os profissionais conscientes de seus atos e do pleno atendimento às normas técnicas vigentes


Objective: Observe and report safety-related aspects in a hydrotherapy service in the city of Goiânia, Goiás. Casuistry and Methods: This is an observational study carried out between August and December of 2017, in the morning period, in a Physiotherapy service in the city of Goiânia, Goiás. Initially, a literature review was carried out searching for scientific articles on Google Scholar, Scielo, PubMed, Medline and specialized literature (technical standards, ordinances and resolutions). The survey covered national and international literature, original articles were free and available in full, being used as descriptors: accessibility, hydrotherapy standards, risk, contraindications, danger and precautions. Then, with the rules/recommendations for a hydrotherapy service taken from the literature review, a table was built and during the internship, the safety aspects contained in the table were observed by that service. Results: Of 31 items listed and observed in this study, four (12.9%) could not be evaluated, 14 (45.2%) met the standards and recommendations and 13 (41.9%) generated energy for reflection and improvement, since they did not meet the requirements of current regulations. Conclusion: The observed hydrotherapy service requires the adoption of some safety measures, recommending a more careful look at the subject and a collective effort to expand the discussion on safety in this area, in order to make professionals aware of their acts and full compliance with current technical standards.

ABSTRACT

Abstract Spondyloarthritis (SpA) is a group of chronic inflammatory systemic diseases characterized by axial and/or peripheral joints inflammation, as well as extra-articular manifestations. Over some decades, nonsteroidal anti-inflammatory drugs (NSAIDs) have been the basis for the pharmacological treatment of patients with axial spondyloarthritis (axSpA). However, the emergence of the immunobiologic agents brought up the discussion about the role of NSAIDs in the management of these patients. The objective of this guideline is to provide recommendations for the use of NSAIDs for the treatment of axSpA. A panel of experts from the Brazilian Society of Rheumatology conducted a systematic review and meta-analysis of randomized clinical trials for 15 predefined questions. The Grading of Recommendations, Assessment, Development and Evaluation methodology to assess the quality of evidence and formulate recommendations were used, and at least 70% agreement of the voting panel was needed. Fourteen recommendations for the use of NSAIDs in the treatment of patients with axSpA were elaborated. The purpose of these recommendations is to support clinicians' decision making, without taking out his/her autonomy when prescribing for an individual patient.(AU)

RESUMEN El gran desarrollo del programa materno infantil en Cuba y la gran atención que a este se brinda hace que muchos médicos limitan a la embarazada al reposo y todavía hay muchas mujeres que se enfrentan al parto sin un entrenamiento previo. Por la importancia de este tema en materia de salud materno infantil se realizó esta revisión bibliográfica con el propósito de aumentar el nivel de conocimientos de los profesionales sobre los beneficios de la misma y lograr un trabajo en equipo del personal que integre el programa, para de esta manera, conseguir el máximo beneficio para las madres, bebés y sus familias. Está demostrado que le hace bien a la mujer embarazada y al bebé. Una gestante preparada es una gestante colaboradora. Por tanto, el entrenamiento junto a hábitos sanos de vida conllevara a que la embarazada tenga un parto en las mejores condiciones físicas y psíquicas, para de esta forma lograr un hijo sano, fuerte y una gran gratificación para toda la familia. La participación activa del padre conseguirá mayor armonía familiar (AU).


SUMMARY The great development of the maternal-infantile program in Cuba and the great attention provided to this, leads to the limitation of pregnant women to rest by doctors, and there are still many women facing the childbirth without a previous training. Due to the importance of this topic in the matter of maternal-infantile health, the authors carried out the current bibliographical review with the purpose of increasing the level of the professionals' knowledge about its benefits and of achieving a team work of the staff integrating the program, and that way, to get the maximum benefit for mothers, babies and their families. It is proved that it makes well to pregnant woman and to the baby. A trained pregnant woman is a collaborative pregnant woman. Therefore, training together with healthy life habits leads pregnant women to childbirth in the better physical and health conditions, delivering this way a healthy, strong child and achieving a great gratification for the whole family. The father's active participation will get bigger family harmony (AU).

Introducción: En las últimas décadas el uso de la posición sentada ha disminuido en frecuencia a causa de 2 complicaciones mayores: el embolismo aéreo venoso y la hipotensión intraoperatoria. Sin embargo es innegable que la posición sentada ofrece una serie de ventajas al neurocirujano, el anestesiólogo y al electrofisiólogo. Materiales y métodos: Estudio retrospectivo de pacientes operados en dos instituciones de Tucumán, entre enero de 2015 y diciembre de 2019. Resultados: Se operaron un total de 119 pacientes en posición sentada por vía posterior. Conclusión: Se presentó la técnica de posición semisentada paso a paso y consejos específicos. Se ilustró la utilidad de la misma mediante la presentación de casos representativos


Introduction: In the last decades, the use of the sitting position has been abandoned due to 2 major complications: venous air embolism and intraoperative hypotension. However, it is undeniable that the sitting position offers a series of advantages to the neurosurgeon, the anesthesiologist and the electrophysiologist. Materials and methods: Retrospective study of patients operated at two institutions in Tucumán, between January 2015 and December 2019. Results: A total of 119 patients were operated in a sitting position and posterior approach. Conclusion: The sitting position technique was presented step by step in detail, with the key steps and a series of tricks. The usefulness of the position was illustrated by presenting representative cases
