RESUMO

Convergence of principles of palliative care and integrative medicine has led to the introduction of the new practice of integrative palliative care in which integrative therapies (including mind-body modalities, traditional Chinese medicine, Ayurveda, and dietary supplements) are used to provide symptom management for patients who are dying or experiencing the sequelae of serious illness and its treatment. We propose an East-West Integrative palliative care model using non-drug therapies, such as acupuncture, diet, exercise, and stress management that shift the paradigm from suppressing the symptoms of illness to addressing both the root cause of the symptoms and the imbalance and declining homeostatic reserve that perpetuate these symptoms. This whole-person model expands the reach of palliative care, prolonging a better quality of life and allowing the patient to maintain as many activities as possible by preventing symptoms and improving function. Through this approach we reframe the dialogue such that patients are "living better" rather than "dying better" when faced with serious illness or death. In this article, we provide an overview of the principles of palliative care, integrative medicine, and the novel area of integrative palliative care, and propose an East-West integrative palliative care model that incorporates and broadens the scope of these existing approaches.

RESUMO

In 1951, ZHU Lian set up Acupuncture Experimental Institute, the predecessor of Acupuncture and Moxibustion Hospital (AMH) of China Academy of Chinese Medical Sciences. Being the founder of AMH, ZHU Lian had paved a road to ensure the correct academic growth and hospital development of AMH because of her outstanding clinical medical thinking and achievements in acupuncture-moxibustion. ZHU Lian's medical practice mode is reviewed and analyzed from 5 aspects, i.e. emphasis on clinical practice, inheritance and development, medical capacity, research orientation and self-growth. Based on this medical practice mode, the development orientation of AMH has been explored.

RESUMO

Through checking the archives from China Academy of Chinese Medical Sciences (CACMS), the event that the experts from the former Soviet Union visiting China and learning acupuncture-moxibustion in 1956 was sorted out. In 1956, under the framework of Sino-Sovien Agreement on Scientific and Technology Cooperation, 3 medical experts assigned by the former Soviet Union came to China for the investigation and study of acupuncture-moxibustion. Institute of Acupuncture and Moxibustion, CACMS was in charge of this activity at that time. The Institute had developed a detailed plan, in which, the teaching schedule was designed on the base of New Acupuncture and Moxibustion written by ZHU Lian and ZHU Lian herself gave the lecture. After 3-month study, the experts had mastered the basic theory and clinical techniques of acupuncture-moxibustion and had launched the relevant clinical and scientific work of acupuncture-moxibustion successfully after returning to their country. This event opens up the foreign exchange of acupuncture-moxibustion in New China and provides the foundation for international training and higher education of acupuncture-moxibustion thereafter.

RESUMO

Professor ZHENG believes that the interaction of internal factors and external factors leads to qi and blood obstruction on muscle surface and skin, thus eczema occurs. This disease is classified into 3 types, i.e. invasion of damp heat, spleen deficiency and damp retention, and blood deficiency and wind dryness, corresponding to acute stage, sub-acute stage and chronic stage successively in western medicine. The treatment principle refers to "regulating qi, detoxification, removing stasis and tonifying for deficiency" in Zhuang medicine. In local skin lesion, surrounding acupuncture, medicated thread moxibustion and lotus-needling and cupping for removing stasis are adopted. Regarding holistic regulation, acupuncture is applied to the points around the umbilicus and traditional acupoints to regulate qi, tonify deficiency and balance zangfu functions.

RESUMO

By searching the randomized controlled trials (RCTs) of acupuncture and moxibustion from CNKI since its inception date to december 31 of 2019, the development status and hot trend of RCTs of acupuncture and moxibustion in China were summarized. The CiteSpace and VOSviewer software were used to perform keyword co-occurrence analysis, clustering analysis, time-zone analysis and citation-burst analysis, and visual map was drawn. As a result, a total of 60 995 articles were included, which were published in 1027 academic journals with 1787 keywords. The publication date was from 1975 to 2019. During the past 45 years, the publications of RCTs on acupuncture and moxibustion had shown an overall growth trend with characteristics of the times. The RCTs of manual acupuncture ranked the top, and its proportion of publications every 5 years was stable in the past 30 years. Since 1994, the hot words such as electroacupuncture, warming needling, auricular point sticking and various acupoint therapies had emerged; meanwhile, the spectrum of diseases had broadened, and an evolutionary trend corresponding to therapies and disease systems had been formed. In recent decade, the RCTs using moxibustion therapy have increased significantly, and the hot words such as "sub-health" "winter diseases being treated in summer" and "acupoint application/ moxibustion during the dog days" had indicated that acupuncture clinical research was further inclined to the field of chronic disease prevention and health services, which was in line with social development and the needs of the times.

RESUMO

XIAO Shao-qing's understanding and evolution of acupoint compatibility methods and rules are summarized. XIAO Shao-qing's understanding of acupoint compatibility rules shows a process from simple listing to systematic improvement, which is reflected as the increase or decrease of the number of acupoint compatibility methods, the revision of the concept and connotation of acupoint compatibility methods, and the adjustment of the example content of acupoint compatibility rules. His clinical prescriptions of acupuncture and moxibustion highlight the academic idea of taking the indications of meridians and acupoints as the basis, using syndrome differentiation and treatment as the core, and selecting acupoints for symptoms as the auxiliary.

RESUMO

Wang Bing's perspective included two aspects. One was the annotation of the classical text, focusing on the medical interpretation and the precise interpretation of concepts and terminology, in particular, the annotation of the scriptures related to acupuncture and moxibustion which had a wide and far-reaching influence on the acupuncture and moxibustion literature in the Song Dynasty. Another was the adaptation of classical text which can be divided into two categories: the adaptation for revealing the original meaning and applying esoteric language; the adaptation of his own understanding. In the second type of adaptation, Wang Bing deliberately concealed the supplementary style of "Words written in red".Wang Bing most likely wrote and annotated the "Seven Chapters of Su wen", which was an important pawn in his adaptation of Suwen. In terms of history and logic, Wang Bing's problems in his large-scale adaptation of Suwen look far more than his achievements. The deception and misdirection caused by the second kind of adaptation concealed deliberately by Wang Bing is still unknown.

RESUMO

The historical artifacts displayed in the Museum of Western study on Chinese Medicine at Yunnan University of Chinese Medicine came from a variety of Chinese medical schools in America and European countries. They are in the memorial galleries for some well-known figures, such as George Soulié de Morant (Su Lie), Jacques-André Lavier (La Wei Ai), Felix Mann (Man Fu Li) and Manfred Porkert (Man Xi Bo), representing the development and status of respective Chinese medical schools in America and European countries. The displayed artifacts are nearly 3,000 photos, manuscripts, documents and more than 500 hours of audiovisual materials, including instruments for acupuncture and moxibustion, books, passports, letters and even supplies for their life. The displayed artifacts demonstrate the process and the access of people in the western societies to know, learn and take use of Chinese medicine.

RESUMO

The differences in the objective, starting point, disease spectrum and interventions of acupuncture-moxibustion clinical trials at home and abroad are collected. By taking two articles of acupuncture-moxibustion clinical trials in foreign countries accepted by JAMA as example, the reasons are analyzed on the dual character of negative and positive results obtained in acupuncture-moxibustion trials of foreign countries. The therapeutic regimens in acupuncture-moxibustion clinical trials in foreign countries are lack of TCM thinking, the manipulation of interventions have not displayed the basic principle of TCM in treatment of diseases, which is separated the theory from the practice in treatment with Chinese herbal medicine and acupuncture-moxibustion. Besides, the advantages in therapeutic effect of acupuncture-moxibustion have not been truly reflected. Regarding the dual character of negative and positive results in acupuncture-moxibustion clinical trials of foreign countries, it is suggested that acupuncture-moxibustion has not been thoroughly understood in foreign countries and its research content have not been in compliance with the theory of traditional acupuncture-moxibustion.

RESUMO

The appropriate sample size estimation is very important in the design of clinical trials. However, insufficient or inappropriate sample size estimation is still a prominent problem in the currently published acupuncture and moxibustion clinical trials. At present, the superiority test, non-inferiority test and equivalence test have been widely used in acupuncture and moxibustion clinical trials. This article focuses on the application, calculation methods and PASS11 software using of these three hypothesis test types. In view of the problems in the estimation of sample size in acupuncture and moxibustion clinical trials, the particularity of sample size estimation in acupuncture and moxibustion is summarized from the aspects of parameter setting, ratio of intervention group and control group, and multi-group comparison, in order to guide acupuncture clinical researchers to correctly estimate sample size when conducting clinical trials.

RESUMO

This study evaluates the analgesic efficacy of low-dose hydromorphone administered via pharmacopuncture at Governing Vessel 20 (GV20) for postoperative pain management following canine ovariohysterectomy. Fifty clinically healthy female dogs undergoing ovariohysterectomy were allocated to receive hydromorphone [0.1 mg/kg body weight (BW)] intramuscularly (IM, n = 25) or hydromorphone (0.01 mg/kg BW) pharmacopuncture at GV20 (GV, n = 25) following extubation. This was a prospective, blinded, randomized clinical trial. Pain and sedation scores were evaluated using the Glasgow Composite Measure Pain Scale Short Form (CMPS-SF) at 1, 2, 3, 4, and 12 hours following study treatment. Time of treatment failure (CMPS-SF ≥ 6/24) was recorded and analyzed using Kaplan-Meier survival analysis. Patient demographics and duration of surgery and anesthesia were analyzed using the appropriate unpaired Student's t-test. The Glasgow CMPS-SF and sedation score were analyzed using a repeated measures 2-way analysis of variance (ANOVA) followed by Bonferroni post-test where appropriate. Significance was set a P < 0.05. There were no significant differences in patient demographics, anesthesia and surgery duration, and study treatment failure. The Glasgow CMPS-SF scores were significantly higher for IM compared with GV [2 (0 to 8) versus 1 (0 to 6), respectively; P = 0.044] at 4 hours. Sedation scores were significantly higher for IM compared with GV at 2 [2 (1 to 3) and 1 (1 to 3), respectively; P = 0.0004] and 4 [1 (1 to 3) and 1 (1 to 2), respectively; P = 0.03] hours. Pharmacopuncture with low-dose hydromorphone provided adequate postoperative analgesia in dogs undergoing ovariohysterectomy with reduced sedative effects. Pharmacopuncture is a good alternative in dogs when reduced dosing of opioids is recommended.

RESUMO

BACKGROUND AND PURPOSE: This study aimed to identify the research evidence on acupoint stimulation (AS) for cancer-related fatigue (CRF) management. METHODS: Randomised controlled trials that utilised AS for CRF management were retrieved. The Cochrane Back Review Group Risk of Bias Tool was used for quality appraisal. RevMan 5.3 was used for meta-analysis. RESULTS: Fifteen studies were included. Both the overall (SMD = -0.95, p = 0.008) and sub-group (acupuncture: SMD = -1.25, p = 0.002; short-term AS: SMD = -0.95, p = 0.02; medium-term AS: SMD = -0.96, p = 0.003) analyses indicated that AS was more effective in alleviating CRF than standard treatment/care. A comparison between the true and sham AS interventions favoured the true AS for CRF management, although the difference did not reach statistical significance. CONCLUSION: This study identified a promising role of AS in improving CRF. However, the study findings should be interpreted prudently due to the limited quality and sample sizes of some of the included studies.

RESUMO

The paper explains academician SHI Xue-min's understanding on hypertension and the concrete application of twirling acupuncture technique for reinforcing and reducing in treatment of hypertension, and explores the theoretic basis of the first and second definitions of twirling acupuncture technique for reinforcing and reducing proposed by the academician SHI Xue-min. It is believed that the dysfunction of the sea of qi and the disharmony of the liver and spleen are the etiologies of hypertension, thus, the treatment principles are suggested as "activating blood circulation, dispersing wind, smoothing liver qi and strengthening spleen functions". Renying (ST 9) is selected as the main acupoint, while Hegu (LI 4), Taichong (LR 3), Quchi (LI 11) and Zusanli (ST 36) are used as the adjuvant acupoints. The twirling acupuncture technique is applied to achieve reinforcing or reducing. Four elements are determined to the successful application of this needling technique, i.e. direction of exertion, strength of exertion, manipulation duration of twirling needle, and the interval duration between two treatments of acupuncture. Eventually, the specification on acupuncture treatment for hypertension can be effectively promoted.

RESUMO

The discovery of modern evolutionary anatomy shows that the persistent median artery in the upper arm is a common variant with an increasing trend. This phenomenon can explain well the transition from the eleven meridians described in the Han silk and bamboo slips to the twelve meridians finalized in Neijing and the addition of the hand- jueyin meridian in Han dynasty. After systematic analysis and comparison, the author proposes the hypothesis that "yin meridians are arteries, and yang meridians are nerve-like structures" to explain the meridian theory prior to acupuncture and acupoints in the Qin and Han dynasties. It is clear that over two thousand years ago, Chinese physicians already described the distribution, physiology, and pathology of the six main arteries and six important nerves in the human extremities. The finding suggests that it was the addition of acupuncture and acupoints that had changed the direction of TCM development in Neijing or later and promoted the maturation of qi and blood, meridians, and zangfu theories.

RESUMO

To summarize the application value of acupuncture in perioperative stress response. Perioperative acupuncture can not only effectively relieve pain and stress response during operation, but also relieve psychological stress response represented by preoperative anxiety before operation, and regulate adaptive immune response after operation. Acupuncture, as a safe non-drug therapy, shows its core advantage of participating in the multidisciplinary intervention of enhance recovery after surgery (ERAS). The future studies need to explore and evaluate the role of acupuncture during perioperative period from multiple dimensions, and gradually reveal the mechanism of acupuncture while establishing the evidence-based basis for acupuncture during perioperative period.

RESUMO

In recent years, many studies have confirmed the positive effect of acupuncture and moxibustion in the treatment of irritable bowel syndrome (IBS), and a lot of researches have been made to explore its neurobiological mechanisms. The neurobiological mechanisms include two aspects 1) remission of visceral hypersensitivity via peripheral and central pathways (spinal cord and brain), and 2) improvement of intestinal motility via vagus nerve and enteric nervous system (ENS)- interstitial cells of Cajal (ICC)-smooth muscle cells (ENS-ICC-SMC) network, and 5-HT, vasoactive intestinal peptide (VIP), neuropeptide Y, etc.. At the same time, we also discussed the existing problems in the research of the underlying neurobiological mechanisms of acupuncture and moxibustion for IBS. The existing problems in this field mainly include 1) fragmentization or lack of integrity in researches, and 2) limitation of the reductionism method. For this reason, we should draw lessons from the research methods of systematic science to study the neurobiological mechanisms of acupuncture and moxibustion in the treatment of IBS, including the detailed links of peripheral and central nervous transmission of acupoint stimulation signals, and the interactive action modes (two-way benign regulation, multi-target regulation, saturation regulation, etc.).

RESUMO

The schematic diagram is an indispensable part of research article in life science, which can effectively and intuitively show the specific content of the article with simple composition. As an intuitive expression of the interdisciplinary subject at the early stage, the schematic diagram in the literature has gained an all-round improvement in the quality along with the deve-lopment of computer technology and modern drawing tools. In contrast, the level of the schematic diagram appears to be lagged behind in the field of acupuncture research papers. In order to improve the drawing level of this field, we took the Five Shu-points of Hand and Foot Jueyin Meridian as an example, and drew some vector diagrams of their anatomical structures including the body surface, skeleton, nerves, arteries and veins by using the Adobe Illustrator image software, through which we sum up a simple and easy-to-learn process including application skills and key points needing attention. We hope these methods can play a role in the acupuncture research in the future.
