En la presente publicación se detallan las metas actualizadas de la OPS para productos alimenticios procesados y ultraprocesados en la Región de las Américas. Se describen los métodos usados para establecer estas metas y se incluye un cuadro con las metas para el 2022 y el 2025 correspondientes a las distintas categorías y subcategorías de alimentos. Este trabajo se basa en las metas establecidas en el 2015 y proporciona una herramienta importante para ayudar a los países a fijar las metas nacionales en materia de reducción del sodio

The emergence of multi-drug-resistant pathogens threatens the healthcare systems world-wide. Recent advances in phototherapy (PT) approaches mediated by photo-antimicrobials (PAMs) provide new opportunities for the current serious antibiotic resistance. During the PT treatment, reactive oxygen species or heat produced by PAMs would react with the cell membrane, consequently leaking cytoplasm components and effectively eradicating different pathogens like bacteria, fungi, viruses, and even parasites. This Perspective will concentrate on the development of different organic photo-antimicrobials (OPAMs) and their application as practical therapeutic agents into therapy for local infections, wound dressings, and removal of biofilms from medical devices. We also discuss how to design highly efficient OPAMs by modifying the chemical structure or conjugating with a targeting component. Moreover, this Perspective provides a discussion of the general challenges and direction for OPAMs and what further needs to be done. It is hoped that through this overview, OPAMs can prosper and will be more widely used for microbial infections in the future, especially at a time when the global COVID-19 epidemic is getting more serious.

BACKGROUND: In the United States, medical devices are regulated and subject to review by the Food and Drug Administration (FDA) before they can be marketed. Low-to-medium risk novel medical devices can be reviewed under the De Novo umbrella before they can proceed to market, and this process can be fairly cumbersome, expensive, and time-consuming. An alternate faster and less-expensive pathway to going to market is the 510(k) pathway. In this approach, if the device can be shown to be substantially equivalent in safety and effectiveness to a pre-existing FDA-approved marketed device (or "predicates"), it can be cleared to market. Due to the possibility of daisy-chaining predicate devices, it can very quickly be difficult to unravel the logic and justification of how a particular medical device's equivalence was established. From patients' perspective, this minimizes transparency in the process. From a vendor perspective, it can be difficult to determine the right predicate that applies to their device. METHODS: We map the connectivity of various predicates in the medical device field by applying text mining and natural language processing (NLP) techniques on data publicly made available by the FDA 78000 device summaries were scraped from the US FDA 510(k) database, and a total of 2,721 devices cleared by the 510(k) regulatory pathway in 2020 were used as a specific case study to map the genealogy of medical devices cleared by the FDA. Cosine similarity was used to gauge the degree of substantial equivalence between two medical devices by evaluating their device descriptions and indications for use. Recalls and complaints for predicate devices were extracted from the FDA's Total Product Life Cycle database using html scraping and web page optical character recognition to determine the similarity between class 1 recalled devices (the most severe form of device recall) and other substantially equivalent devices. A specific product code was used to illustrate the mapping of the genealogy from a De Novo device. RESULTS AND DISCUSSION: The ancestral tree for the medical devices cleared in 2020 is vast and sparse, with a large number of devices having only 1-2 predicates. Evaluation of substantial equivalence data from 2003-2020 shows that the standard for substantial equivalence has not changed significantly. Studying the recalls and complaints, shows that the insulin infusion pump had the highest number of complaints, yet none of the recalled devices bore significant degree of text similarity to currently marketed devices. The mapping from the De Novo device case study was used to develop an ancestry map from the recalled predicate (recalled due to design flaws) to current substantially equivalent products in the market. CONCLUSIONS: Besides enabling a better understanding of the risks and benefits of the 510(k) process, mapping of connectivity of various predicates could help increase consumer confidence in the medical devices that are currently in the marketplace.

Biofilms formed by methicillin-resistant S. aureus (MRSA) are among the most frequent causes of biomedical device-related infection, which are difficult to treat and are often persistent and recurrent. Thus, new and effective antibiofilm agents are urgently needed. In this article, we review the most relevant literature of the recent years reporting on promising anti-MRSA biofilm agents derived from the genus Streptomyces bacteria, and discuss the potential contribution of these newly reported antibiofilm compounds to the current strategies in preventing biofilm formation and eradicating pre-existing biofilms of the clinically important pathogen MRSA. Many efforts are evidenced to address biofilm-related infections, and some novel strategies have been developed and demonstrated encouraging results in preclinical studies. Nevertheless, more in vivo studies with appropriate biofilm models and well-designed multicenter clinical trials are needed to assess the prospects of these strategies.

ABSTRACT: Advances in medical device technology have led to greater connectivity with other devices, networks, and systems, raising concerns about associated security risks. Many clinicians, as well as the patients who rely on these lifesaving devices, are unaware of these risks and how they can be mitigated. This article describes several types of cyberattacks, including medjacking, hacking, and ransomware, and what nurses can do to guard against security threats and educate patients.

Establecer el cumplimiento de las normas de bioseguridad por parte de los profesionales que laboran en los laboratorios de la Facultad de Ciencias Químicas y de la Salud de la Universidad Técnica de Machala. Materiales y Métodos: La presente investigación es de tipo descriptiva, con enfoque cuantitativo. La modalidad de la investigación es de campo. La población estuvo constituida por 100 estudiantes que usan los diferentes laboratorios de la Facultad de Ciencias Químicas y de la Salud de la Universidad Técnica de Machala. Resultados: El 59% de la población contestó que sí, ha recibido capacitación acerca de las normas de bioseguridad en la institución y el 41% respondieron que no. El 73% de los estudiantes cumplen con las medidas de bioseguridad en el laboratorio. Se realiza el lavado de manos, con un equivalente a un 39% para Después de tener contacto con cualquier tipo de materiales, muestras biológicas, productos sólidos, sustancias o reactivos. Sobre si se cuentan con recipientes de descarte para elementos contaminados, y están debidamente identificados el cual corresponde a un 46,9%. A la pregunta sobre qué materiales se deposita en los guardianes dando un equivalente de 43% para las Jeringuillas, bisturí, sin embargo, las Lancetas, agujas. Conclusiones: Se pudo determinar que la gran parte de la población estudiada si cumplen con las normas de bioseguridad(AU)

To establish compliance with Biosafety standards by professionals working in the laboratories of the Faculty of Chemistry and Health Sciences of the Technical University of Machala. Materials and Methods: This is a descriptive research with a quantitative approach. The modality of the research is field research. The population consisted of 100 students who use the different laboratories of the Faculty of Chemistry and Health Sciences of the Technical University of Machala. Results: 59% of the population answered that they had received training on Biosafety norms in the institution and 41% answered that they had not. Seventy-three percent of the students comply with Biosafety measures in the laboratory. Hand washing is performed, with an equivalent of 39% for after having contact with any type of materials, biological samples, solid products, substances or reagents. Regarding whether there are disposal containers for contaminated items, and whether they are properly identified, this corresponds to 46.9%. To the question about what materials are deposited in the guards giving an equivalent of 43% for syringes, scapel powever, Lancets, needles. Conclusions: It could be determined that the great part of the population studied if they comply with Biosafety standards

Objetivo: Investigar os conhecimentos e cuidados desenvolvidos por enfermeiros de uma instituição hospitalar em relação às lesões por pressão relacionadas a dispositivos médicos. Métodos: Estudo de abordagem qualitativa exploratório-descritivo realizado em novembro de 2018, mediante aplicação de questionário semiestruturado com enfermeiros de unidades assistenciais de um hospital privado do Sul do Brasil. Os dados foram processados mediante análise temática de conteúdo. Resultados: Participaram 18 enfermeiros que desenvolvem assistência em unidades de atendimento à idosos. Dentre estes, 61,2% compreende a terminologia lesão por pressão relacionada a dispositivo médico, mas metade dos entrevistados apresentam fragilidades em conhecimentos relacionados a prevenção deste tipo de lesão. Dentre os cuidados empregados destacam-se, inspeção diária da pele durante o exame físico uma vez ao dia com 12 respondentes, ou inspeção da pele durante quadros de intercorrências relacionados a dor/desconforto com dez respondentes. A maioria deles (13) sinalizou a implementação de cuidados de enfermagem indicados na prescrição de enfermagem, 11 enfermeiros destacaram a notificação ao Núcleo de Segurança do Paciente e dez profissionais discutem ações de prevenção com a equipe multidisciplinar. Conclusão: Observou-se fragilidades apresentadas pelos enfermeiros quanto a identificação e avaliação clínica desse tipo de lesão de pele, caracterizando a necessidade de instrumentalização dos profissionais. Objective: To investigate the knowledge and care developed by nurses at a hospital in relation to pressure injuries related to medical devices. Methods: A qualitative exploratory descriptive study carried out in November 2018, using a semi-structured questionnaire with nurses from care units in a private hospital in southern Brazil. The data analysis was processed through thematic content analysis. Results: Participated 18 nurses who develop assistance in elderly care units. Among these, 61.2% understand the terminology pressure injury related to medical device, but half of the interviewees have weaknesses in knowledge related to the prevention of this type of injury. The care employed stands out, daily inspection of the skin during the physical examination once a day with 12 respondents, or inspection of the skin during conditions of complications related to pain / discomfort with ten respondents. Most of them (13) signaled the implementation of nursing care indicated in the nursing prescription, 11 nurses highlighted the notification to the Patient Safety Center and ten professionals discuss prevention actions with the multidisciplinary team. Conclusion: It was observed weaknesses presented by nurses regarding the identification and clinical evaluation of this type of skin lesion, characterizing the need for instrumentalization of professionals. Objetivo: Investigar el conocimiento y la atención desarrollada por las enfermeras en un hospital en relación con las lesiones por presión relacionadas con dispositivos médicos. Métodos: Estudio cualitativo exploratorio descriptivo realizado en noviembre de 2018, utilizando un cuestionario semiestructurado con enfermeras de unidades de atención en un hospital privado en el sur de Brasil. El análisis de datos se procesó mediante análisis de contenido temático. Resultados: Participaron 18 enfermeras que desarrollan asistencia en unidades de atención a personas mayores. Entre estos, el 61.2% entiende la terminología lesiones por presíon relacionada con el dispositivo médico, pero la mitad de los entrevistados tienen debilidades en el conocimiento relacionado con la prevención de este tipo de lesiones. Se destaca la atención empleada, la inspección diaria de la piel durante el examen físico una vez al día con 12 encuestados, o la inspección de la piel durante las condiciones de complicaciones relacionadas con el dolor / incomodidad con diez encuestados. La mayoría de ellos (13) señalaron la implementación de la atención de enfermería indicada en la receta de enfermería, 11 enfermeras destacaron la notificación al Centro de Seguridad del Paciente y diez profesionales discutieron las acciones de prevención con el equipo multidisciplinario. Conclusión: Se observaron debilidades presentadas por enfermeras con respecto a la identificación y evaluación clínica de este tipo de lesión cutánea, caracterizando la necesidad de instrumentalización de los profesionales (AU)

Objetivo: analisar o impacto na implantação do Projeto Lean, sob ótica de enfermeiros assistenciais. Método: estudo descritivo, exploratório, retrospectivo, documental, de abordagem qualitativa, com enfermeiros assistenciais lotados em um Hospital de Ensino. Os dados foram coletados por meio de entrevista semiestruturada, no ano de 2019. Utilizou-se Análise de Conteúdo na modalidade Temática. Resultados: Evidenciaram-se problemas como aumento da carga de trabalho e dimensionamento de pessoal incipiente. A equipe de enfermagem, durante a realização da assistência enfrenta limitações quando se refere ao dimensionamento inadequado e quantitativo insuficiente de profissionais e recursos materiais disponíveis, tornando o trabalho desgastante. Conclusão: O estudo mostrou a necessidade de dimensionamento de pessoal de enfermagem adequado bem como previsão e provisão de materiais médico-hospitalares para uma assistência de qualidade, buscando agregar valor ao paciente bem como sua satisfação com o atendimento na Instituição.(AU)

Objective: analyze the impact on the implementation of the Lean Program, on the perspective of assistant nurses. Method: descriptive, exploratory, retrospective, and documental study, of qualitative approach, with assistant nurses from a Teaching Hospital. The data was collected by semi structured interviews, in the year 2019. Content Analysis was used in the Thematic modality. Results: Problems such as the increase of working hours and incipient staff sizing were evidenced. The nursing staff, during the assistancefaces limitations when it is related to the inadequate sizing and insufficient qualitative of professionals and material resources available, making it an exhausting work. Conclusion: the study has shown the necessity of adequate nursing staff sizing as well as prediction and provision of medical and hospital materials for good quality assistance, seekingto add value to the patient as well as their satisfaction with the care provided at the institution.(AU)

Objetivo: analizar el impacto en la implementación del Proyecto Lean, desde la perspectiva de los enfermeros clínicos. Método: estudio descriptivo, exploratorio, retrospectivo, documental, con abordaje cualitativo, con enfermeros clínicos que en un Hospital Docente. Los datos fueron recolectados a través de entrevistas semiestructuradas, en el año 2019. Se utilizó Análisis de Contenido en la modalidad Temática. Resultados: Se evidenciaron problemas como aumento de la carga de trabajo y dimensionamiento del personal incipiente. El equipo de enfermería, durante la atención, se enfrenta a limitaciones en cuanto a dimensionamiento inadecuado y cantidad insuficiente de profesionales y recursos materiales disponibles, lo que hace que el trabajo sea agotador. Conclusión: El estudio mostró la necesidad de un adecuado dimensionamiento del personal de enfermería, así como la previsión y provisión de materiales médico-hospitalarios para una atención de calidad, buscando agregar valor al paciente, así como su satisfacción con la atención en la institución.(AU)

Objetivo: Refletir sobre os aspectos relacionados ao trabalho da equipe de Enfermagem, durante a pandemia da COVID-19 no Brasil. Métodos: Trata-se de uma reflexão sobre alguns aspectos das condições de trabalho e a exposição aos riscos à saúde dos profissionais de Enfermagem no Brasil, fundamentada nas informações do Observatório de Enfermagem e dos relatórios de fiscalização do Conselho Federal de Enfermagem (COFEN), e da Pesquisa Perfil da Enfermagem no Brasil. Resultados: Os profissionais de Enfermagem brasileiros tem enfrentado duras condições de trabalho, a exemplo da insuficiência e a inadequação dos Equipamentos de Proteção Individual (EPI) e o subdimensionamento das equipes, com isso aumentando os riscos à saúde dos trabalhadores. Conclusões: A pandemia exacerbou problemas enfrentados pelos profissionais de Enfermagem, e o déficit de EPI com o subdimensionamento das equipes e a sobrecarga de trabalho podem estar associados ao elevado número de óbitos de profissionais durante o período analisado.(AU)

Objective: To reflect on aspects related to the work of the Nursing team, during the COVID-19 pandemic in Brazil. Methods: This is a reflection on some aspects of working conditions and the exposure to health risks of nursing professionals in Brazil, based on information from the Nursing Observatory and inspection reports from the Federal Nursing Council (COFEN), and the Nursing Profile Survey in Brazil. Results: Brazilian Nursing professionals have faced harsh working conditions, such as the insufficiency and inadequacy of the Equipment of Individual Protection (EIP) and the undersizing of teams, thereby increasing the risks to workers' health. Conclusions: The pandemic has exacerbated problems faced by nursing professionals, and the deficit in EIP with the undersizing of teams and work overload may be associated with the high number of deaths of professionals during the analyzed period.(AU)

Objetivo: Reflexionar sobre los aspectos relacionados al trabajo del equipo de Enfermería, durante la pandemia de COVID-19 en Brasil. Métodos: Se trata de una reflexión sobre algunos aspectos de las condiciones de trabajo y la exposición a los riesgos a la salud de los profesionales de Enfermería en Brasil, basada en las informaciones del Observatorio de Enfermería, los informes de fiscalización del Consejo Federal de Enfermería (COFEN), y de la Investigación Perfil de la Enfermería en Brasil. Resultados: Los profesionales de Enfermería brasileños han enfrentado duras condiciones de trabajo, como la insuficiencia y la inadecuación de los Equipos de Protección Individual (EPI) y el tamaño insuficiente de los equipos de Enfermería, aumentando con esto los riesgos a la salud de los trabajadores. Conclusiones: La pandemia exacerbó problemas enfrentados por los profesionales de Enfermería, y el déficit de EPI con el tamaño insuficiente de los equipos de Enfermería y la sobrecarga de trabajo pueden estar asociados al elevado número de óbitos de profesionales durante el período analizado.(AU)

Alveolar osteitis (AO) is a common complication following the extraction of the teeth, particularly the lower third molars. It starts within a few days after the extraction and manifests mainly as pain in the extraction site. Several strategies of treatment are available in order to relieve pain and heal the extraction wound. Recently, a novel medical device combining hyaluronic acid (HA) and octenidine (OCT) was introduced for the treatment of AO. This series of case reports aims to summarize the initial clinical experiences with this new device and to highlight factors possibly interfering with this treatment. The medical documentation of five patients with similar initial situations treated for AO with HA + OCT device was analyzed in detail. Smoking and previous treatment with Alveogyl (Septodont, Saint-Maur-des-Fossés, France) were identified as factors interfering with the AO treatment with the HA + OCT device. In three patients without these risk factors, the treatment led to recovery within two or three days. The patient pretreated with Alveogyl and the smoker required six and seven applications of the HA + OCT device, respectively. According to these initial observations, it seems smoking and previous treatment with Alveogyl prolong the treatment of AO using the HA + OCT device that, in turn, shows a rapid effect if these risk factors are not present.

SUMMARY: The progress of biotechnology, medical instruments, and applied sciences contributes to a rapidly expanding space for the advancement of the medical field. Surgeons experience first-hand the limitations of current medical devices and thus have unique insight into problems that could be solved with new products. The process of turning an idea into a product capable of success in the marketplace, however, is often unfamiliar to surgeons. The authors seek to illuminate this process and provide an ordered list of tasks that can make bringing ideas to market more achievable for surgeons. The first step in this process is the generation and protection of a new idea. Next, the process of making an idea into a product is outlined. This phase involves team assembly, business planning, and product development. Market research and valuation are key to understanding how a product can be applied in the market, and meticulous research during this phase allows for informed decision-making that will help secure funding down the road. Finally, various options for financing are discussed and compared to help surgeon-entrepreneurs find an option that best fits their project, and steps to maximize leverage are described. The development of new products can be a complicated process for surgeons. Organized into four phases, with ordered instructional steps to advance through each phase, the process of bringing an idea to the market is clarified. Facilitating this process will possibly contribute to the continual improvement of medical and surgical abilities through the introduction of new devices and technologies.

Several studies have demonstrated improved clinical outcomes in pressure ulcer prevention using the SEM scanner, but none have compared it with other methods. In one of Scotland's health boards, 'hot spot' wards had been unable to reduce the number of hospital-acquired pressure ulcers (HAPUs) after several years of focused improvement work. In addition, other wards showed high use of dynamic therapy systems with associated costs. This review compares the use of a first generation SEM scanner versus a mattress and equipment selection pathway over a 6-week period. The findings show that the SEM scanner wards had zero HAPU while the equipment pathway wards developed a total of 4 HAPU. The two SEM scanner wards showed a 11% and 33% reduction in dynamic therapy use, while the pathway wards showed an average 40% increase. Consideration should be given to using SEM scanners to support staff decision-making to reduce HAPU development and dynamic therapy usage.

This article provides an introduction to the aetiology of medical device-related pressure ulcers (MDRPUs), describes the vicious cycle that leads to these injuries and highlights bioengineering methodologies and findings that connect the aetiology to the clinical practice of preventing MDRPUs. Specifically, the vicious cycle of MDRPUs is triggered by the sustained tissue deformations induced by a skin-contacting device. The primary, deformation-inflicted cell damage leads to a secondary inflammatory-oedema-related damage and then to tertiary ischaemic damage. Each of these three factors contributes to cumulative cell death and tissue damage under and near the applied device. The damage therefore develops in an escalated manner, as a result of the added contributions of the above three factors. This phenomenon is exemplified through two common clinical scenarios. First, through the use of continuous positive airway pressure (CPAP) masks, which are being applied extensively in the current COVID-19 pandemic, and, second, through the use of doughnut-shaped head positioners, which are applied to surgical patients and sometimes to bedridden individuals who receive intensive care in a supine position. These two medical devices cause intense, localised mechanical loads in the facial skin and underlying tissues (CPAP mask) and at the occipital scalp (doughnut-shaped positioner), where the soft tissues cannot swell in response to the inflammatory oedema as, in both cases, the tissues are sandwiched between the device and the skull. Accordingly, the two device types result in characteristic MDRPUs that are avoidable through appropriate prophylactic interventions, that is, preventive dressings under the CPAP mask and replacement of the doughnut device by a soft, shape-conforming support aid to alleviate and disperse the localised soft tissue deformations. Hence, understanding the aetiology of MDRPUs targets and focuses effective clinical interventions.

OBJECTIVE: To investigate the evolution of pressure-measuring devices used in compression treatment for venous leg ulcers and assess the most practical and effective devices to determine optimal pressure in compression therapy. DATA SOURCES: Relevant information was retrieved from databases including Google Scholar, PubMed, Wiley Online, and ScienceDirect without publication date restrictions. The keywords included venous leg ulcer, compression therapy, pressure measuring device, pressure sensor, and wireless system. STUDY SELECTION: Studies included in the review had to be published in English and discuss or compare pressure-measuring devices/sensors for compression therapy, the development of alternative sensors, and the applications of wireless technologies. Veterinary studies, conference proceedings, and unpublished articles were excluded. Applicable studies and articles were critically evaluated and synthesized. DATA EXTRACTION: After abstract review, 39 studies were identified. During full-text review, study details were collected using a data extraction form and organized into tables. Device attributes, accuracy, price, and limitations were categorized and analyzed. DATA SYNTHESIS: Studies disagree on the effectiveness and user-friendliness of existing pressure-measuring devices. These devices often impact user comfort and convenience, which are crucial factors in the adoption and use of wearable devices. Potential solutions for pressure-measuring devices with promising technologies were proposed: four feasible alternative sensors are described that could improve comfort and facilitate prolonged use under bandages. Advanced communication technologies may provide more convenience for users and practitioners. CONCLUSIONS: Conventional pressure-measuring devices used in compression therapy are not designed for the user's comfort and convenience. The use of flexible and stretchy pressure sensors (e-skin) provides good biocompatibility, conformability, and comfort and when integrated with near-field communication technology could address the drawbacks of current pressure-measuring devices.

During the COVID-19 pandemic, a shortage of personal protective equipment compromised efficient patient care and provider safety. Volunteers from many different backgrounds worked to meet these demands. Additive manufacturing, laser cutting, and alternative supply chains were used to produce, test, and deliver essential equipment for health care workers and first responders. Distributed equipment included ear guards, face shields, and masks. Contingent designs were created for powered air-purifying respirator hoods, filtered air pumps, intubation shields, and N95 masks.

During the COVID-19 pandemic, the rapid surge in demand for critical supplies and public health efforts needed to guard against virus transmission have placed enormous pressure on health systems worldwide. These pressures and the uncertainty they have created have impacted the health workforce in a substantial way. This paper examines the relationship between health supply chain capacity and the impact of the COVID-19 pandemic on Canada's health workforce. The findings of this research also highlight the impact of the pandemic on health workers, specifically the relationship between the health supply chain and the autonomy of the health workforce.
