RESUMO

MOTIVATION: In this work, we present an analytical method for quantifying both single-cell morphologies and cell network topologies of tumor cell populations and use it to predict 3D cell behavior. RESULTS: We utilized a supervised deep learning approach to perform instance segmentation on label-free live cell images across a wide range of cell densities. We measured cell shape properties and characterized network topologies for 136 single-cell clones derived from the YUMM1.7 and YUMMER1.7 mouse melanoma cell lines. Using an unsupervised clustering algorithm, we identified six distinct morphological subclasses. We further observed differences in tumor growth and invasion dynamics across subclasses in an in vitro 3D spheroid model. Compared to existing methods for quantifying 2D or 3D phenotype, our analytical method requires less time, needs no specialized equipment and is capable of much higher throughput, making it ideal for applications such as high-throughput drug screening and clinical diagnosis. It is essential that nurses can use technology and accurately record and interpret clinical data to deliver efficient and effective patient care. The purpose of this study was to determine the level of nursing faculty informatics competencies and to learn about barriers faculty find in teaching informatics. Using a cross-sectional design, faculty were surveyed about their knowledge and comfort in teaching informatics using the Self-Assessment of Informatics Competency Scale for Health Professionals instrument. The aggregate mean (SD) score of the Self-Assessment of Informatics Competency Scale for Health Professionals basic knowledge and skills competency subscale and role subscale were was 3.36 (0.437) and 3.35 (0.711), respectively, which showed overall proficiency with these competencies. The aggregate mean (SD) for the applied computer skills competency was 2.25 (0.842), which showed less proficiency in higher-level informatics skills. Reported barriers to integrating informatics into courses were technology issues (17.2%), lack of knowledge (15.5%), and a lack of resources (12.1%). Some faculty reported that the concept of informatics was confusing (3.5%) and students were not engaged in the concepts (3.5%). Faculty with informatics competence can help students advance the use of technology and informatics at the bedside to improve patient care.

RESUMO

Innovations in human-centered biomedical informatics are often developed with the eventual goal of real-world translation. While biomedical research questions are usually answered in terms of how a method performs in a particular context, we argue that it is equally important to consider and formally evaluate the ethical implications of informatics solutions. Several new research paradigms have arisen as a result of the consideration of ethical issues, including but not limited for privacy-preserving computation and fair machine learning. In the spirit of the Pacific Symposium on Biocomputing, we discuss broad and fundamental principles of ethical biomedical informatics in terms of Olelo Noeau, or Hawaiian proverbs and poetical sayings that capture Hawaiian values. While we emphasize issues related to privacy and fairness in particular, there are a multitude of facets to ethical biomedical informatics that can benefit from a critical analysis grounded in ethics.

OBJECTIVES: To identify and summarize the top bioinformatics and translational informatics papers published in 2021 for the IMIA Yearbook. METHODS: We performed a broad literature search to retrieve Bioinformatics and Translational Informatics (BTI) papers and coupled this with a series of editorial and peer reviews to identity the top papers in the area. RESULTS: We identified a final candidate list of 15 BTI papers for peer-review; from these candidates, the top three papers were chosen to highlight in this synopsis. These papers expand the integration of multi-omics data with electronic health records and use advanced machine learning approaches to tailor models to individual patients. In addition, our honorable mention paper foreshadows the growing impact of BTI research on precision medicine through the continued development of large clinical consortia. CONCLUSION: In the top BTI papers this year, we observed several important trends, including the use of deep-learning approaches to analyse diverse data types, the development of integrative and web-accessible bioinformatics pipelines, and a continued focus on the power of individual genome sequencing for precision health.

OBJECTIVE: To summarize significant research contributions on cancer informatics published in 2021. METHODS: An extensive search using PubMed/MEDLINE and Altmetric scores was conducted to identify the scientific contributions published in 2021 that address topics in cancer informatics. The selection process comprised three steps: (i) 15 candidate best papers were first selected by the two section editors, (ii) external reviewers from internationally renowned research teams reviewed each candidate best paper, and (iii) the final selection of two best papers was conducted by the editorial board of the IMIA Yearbook. RESULTS: The two selected best papers demonstrate some of the promises and shortcomings of real-world data. CONCLUSION: Cancer informatics is a maturing subfield of biomedical informatics. Applications of informatics methods to real-world data are especially notable in 2021.

RESUMO

The evolution of the informatics field, now with a well-accepted and crucial role in modern biomedicine and health care delivery, is the result of creative research over seven decades. The success is due in part to recognition that, throughout the process, investigators have documented not only what they have done but what they have learned, stimulating and guiding the next generation of projects. Such iterative experimentation, learning, sharing, and progressing is typical of all scientific disciplines. Yet progress depends on identifying key lessons, insights, and methods so that others can use them. This paper addresses the nature of scientific progress in informatics, recognizing that while the field is motivated by applications that can improve biomedicine and health, the scientific underpinnings must be identified and shared with others if the field is to progress optimally.

BACKGROUND:  The increasing use of technology in nursing practice requires nursing students to be competent in nursing informatics with an attitude of acceptance of technology in the healthcare environment. OBJECTIVES:  The objectives of the study were to determine final year nursing students' perceptions and skills in nursing informatics and their attitudes towards computerisation in nursing practice. METHOD:  The study population were 198 final year nursing students from a selected university in the Western Cape, South Africa. All-inclusive sampling was used. A descriptive survey was conducted using a self-administered questionnaire which included two validated scales, namely the validated Nursing Informatics Competency Assessment Tool (NICAT) and the Nurses' Attitudes towards Computerisation scale. Means and 95% confidence intervals (CI) of the ratings of the perceived relevance of nursing informatics skills in nursing practice, perceived levels of competence in nursing informatics skills and attitudes towards computers were calculated. RESULTS:  A total of 91 undergraduate respondents completed the survey. Computer literacy skills were rated overall as most relevant (4.23, 95% confidence interval [95% CI]: 4.06-4.40) and the skills perceived most competent (4.16, 95% CI: 3.81-4.22). The respondents had an overall positive score for attitudes towards computerisation in healthcare (67.34, s.d. = 10.40, 95% CI: 65.18-69.51). CONCLUSION:  The study concluded that computer literacy skills, informatics literacy skills and information management skills were relevant to nursing practice, despite varying levels of competence in these skills among nurses.Contribution: What key insights into the research results and its future function are revealed? INTRODUCTION: Institutionalized patients who require nutrition support regularly visit the Unit of Nutrition Support of the Hospital Pharmacy  Service. During these visits, prior to establishing a nutrition regime and  followup scheme, an initial nutritional status assessment is performed.  Telemedicine and Telepharmacy have expanded in recent years for remote  monitoring of institutionalized persons. OBJECTIVE: To evaluate the implementation of a Telemedicine informatics application for nutritional support surveillance of instutionalized  persons in nursing homes from a hospital pharmacy service. Method: A multidisciplinary team led by the Hospital Pharmacy Service was  created. Data of institutionalized persons in nursing homes needing artificial  nutritional support was extracted from the SILICON prescription system and  the internal ecords of the hospital pharmacy service. Nursing homes were  selected on the basis of their previous experience using the Telemedicine  informatics application TELEA. The following nutritional  support variables were  assessed: score on Mini-Nutritional Assessment questionnaire; a 24-h  food record; pressure ulcer control; and laboratory parameters. The existing  information flow between nursing homes and hospital specialists was analyzed.  The functionalities available on TELEA were considered. RESULTS: In 2021, over 300 institutionalized persons from 28 nursing homes  were incluided for nutritional support surveillance program of the hospital  pharmacy service. The project was implemented in two nursing homes serving  38 patients, although only 13 were involved in the nutrition surveillance  program of the Nutrition Support Unit. Nutritional status assessment and  nutritional support surveillance reports were adapted to the Telemedicine  informatics application. Paper reports were replaced  with electronic data or  online questionnaires available on the informatics application. An information  flow protocol was established. An instantaneous messaging and alert system  was activated, which allowed continuous communication. Some Telepharmacy  requests were categorized as preferential when the clinical status of the patient  so required. All the information generated during the nutritional status  assessment and nutritional support surveillance process was integrated into  the electronic medical history of each patient. CONCLUSIONS: TELEA-based nutritional support surveillance facilitates the  continuum of care by enabling direct communication between nursing homes  and secondary care for institutionalized persons. This model makes it possible  to record nutrition-related data on the electronic medical history of patients  through a Telepharmacy process. This model also eliminates paper  prescriptions and medical reports, and unnecessary travels. A nutritional status  profile should also be made available to facilitate nutrition surveillance in institutionalized persons with chronic diseases. That  would be the first step for a new integrated healthcare informatics application  for frail/polymorbid elderly patients.


Introducción: Los pacientes institucionalizados que requieren soporte  nutricional artificial acuden de forma periódica a las consultas de la Unidad de  Soporte Nutricional del Servicio de Farmacia. En ellas se realiza una  valoración nutricional inicial, a partir de la cual se establece la pauta nutricional  y el plan de seguimiento. La Telemedicina y la Telefarmacia se han  expandido en los últimos años para la monitorización remota de personas  institucionalizadas. OBJETIVO: Evaluar la implementación de una aplicación informática de Telemedicina para la vigilancia del soporte nutricional de personas  nstitucionalizadas en residencias de ancianos desde un servicio de farmacia hospitalaria.Método: Se formó un equipo multidisciplinar liderado por el Servicio de Farmacia. Los datos de pacientes institucionalizados que requieren soporte nutricional artificial se obtuvieron del programa SILICON y de los  registros internos del Servicio de Farmacia. Se eligieron las primeras  residencias candidatas según la experiencia previa en el manejo de la  aplicación informática de asistencia domiciliaria. Se analizaron llas variables de  soporte nutricional necesarias para el seguimiento nutricional: Mini  Nutritional Assessment, recordatorio de ingestas de las últimas 24 horas,  control de úlceras por presión y parámetros analíticos. Se analizó el flujo de  información existente entre las residencias sociosanitarias y la atención  hospitalaria y se valoraron las opciones disponibles a través de la plataforma  tecnológica de asistencia domiciliaria. RESULTADOS: El Servicio de Farmacia realizó seguimiento nutricional a más de  300 pacientes institucionalizados en 28 residencias sociosanitarias en el año  2021. El proyecto se implantó en dos residencias sociosanitarias que cuentan  con 38 pacientes institucionalizados, aunque solo 13 pacientes estaban en  seguimiento por la Unidad de Soporte Nutricional. Todos los registros  generados en la valoración y seguimiento nutricional se adecuaron a la  aplicación informática de asistencia domiciliaria, se cambiaron los registros en  papel por información incluida en la plataforma o cuestionarios online  facilitados a través de la misma. Además, se protocolizó el flujo de información  generada y se activó un sistema de mensajería con alertas que  permite una comunicación continua. En caso de que la situación clínica  del  paciente lo requiera se puede programar una teleconsulta preferente.Toda la  información generada en el proceso de valoración y seguimientonutricional de  cada paciente se integró en la historia clínica electrónica. CONCLUSIONES: El seguimiento nutricional a través de la aplicación informática de asistencia domiciliaria facilita la continuidad asistencial por el establecimiento de una comunicación directa entre las residencias  sociosanitarias y la atención hospitalaria, permitiendo la integración de la  información nutricional de los pacientes en la historia clínica electrónica  mediante un proceso de Telefarmacia. Se han eliminado las prescripciones en  papel, informes clínicos impresos y los desplazamientos innecesarios. El  desarrollo de este perfil podría ser extensible al seguimiento nutricional de  pacientes con patologías crónicas, y ser el precedente de un nuevo programa  de cuidado integral del paciente anciano frágil o pluripatológico.

Introduction: Informatics applied to health sciences has brought cutting-edge solutions to healthcare problems. However, the number of health professionals trained in "Health Informatics" is low. Virtual education, such as massive online open courses, provide the opportunity for training in this field. Objective: To estimate the global offer of massive online open biomedical informatics courses and characterize their content. Methods: A search for massive online open courses was conducted throughout December 2021 on 25 platforms offering these courses. The search strategy included the terms "health informatics" and "biomedical informatics". The application areas of biomedical informatics, platform, institution, duration, time required per week, language, and subtitles available for each course were evaluated. Data were analyzed descriptively, reporting absolute and relative frequencies. Results; Our search strategy identified 1333 massive online open courses. Of these, only 79 were related to health informatics. Most of these courses (n = 44; 55.7%) were offered through Coursera. More than half (n = 55; 69.6%) were conducted by U.S. institutions in english (n = 76; 96.2%). Most courses focused on areas of translational bioinformatics (n = 27; 34.2%), followed by public health informatics (n = 23; 29.1%), and clinical research informatics (n = 13, 16.5%). Conclusions: We found a significant supply of massive online open courses on health informatics. These courses favor the training of more professionals worldwide, mostly addressing competencies to apply informatics in clinical practice, public health, and health research.


Introducción: La informática biomédica ha traído consigo soluciones innovadoras a problemas sanitarios. Sin embargo, el número de profesionales sanitarios capacitados en informática biomédica es bajo. La educación virtual, como los cursos masivos abiertos en línea, brindan la oportunidad de formación en este campo. Objetivo: Estimar la oferta global de cursos masivos abiertos en línea sobre informática biomédica y conocer las características de su contenido. Métodos: Se realizó una búsqueda de cursos masivos abiertos en línea durante diciembre de 2021 en 25 plataformas que ofertan estos cursos. La estrategia de búsqueda incluyó los descriptores "" y "". Se evaluaron las áreas de aplicación de informática biomédica, la plataforma, la institución, la duración, el tiempo requerido por semana, el idioma y los subtítulos disponibles para cada curso. Se analizaron los datos descriptivamente, reportándose frecuencias absolutas y relativas. Resultados: Nuestra estrategia de búsqueda identificó 1333 cursos masivos abiertos en línea. De ellos, solo 79 estaban relacionados con informática de la salud. La mayoría de estos cursos (n = 44; 55,7%) se ofrecieron a través de Coursera. Más de la mitad (n = 55; 69,6%) fueron realizados por instituciones de Estados Unidos y en el idioma inglés (n = 76; 96,2%). La mayor parte de los cursos se centraron en áreas de bioinformática traslacional (n = 27; 34,2%), seguido de informática de la salud pública (n = 23; 29,1%), e informática de la investigación clínica (n = 13; 16,5%). Conclusiones: Se encontró una importante oferta de cursos masivos abiertos en línea sobre informática biomédica. Estos cursos favorecen la formación de más profesionales en todas las regiones del mundo, abordando en su mayoría competencias para aplicar la informática en la práctica clínica, la salud pública y la investigación en salud.

Solute carrier transporters are integral membrane proteins, and are important for diverse cellular nutrient transports, metabolism, energy demand, and other vital biological activities. They have recently been implicated in pancreatic cancer and other cancer metastasis, angiogenesis, programmed cell death and proliferation, cell metabolism and chemo-sensitivity. Here we report the study of 13 human solute carrier membrane transporters using the highly accurate AlphaFold2 predictions of 3D protein structures. In the native structures, there are hydrophobic amino acids leucine (L), isoleucine (I), valine (V) and phenylalanine (F) in the transmembrane alpha-helices. These hydrophobic amino acids L, I, V, F are systematically replaced by hydrophilic amino acids glutamine (Q), threonine (T) and tyrosine (Y), thus the QTY code. Therefore, these QTY variant transporters become water-soluble without requiring detergents. We present the superposed structures of these native solute carrier transporters and their water-soluble QTY variants. The superposed structures show remarkable similarity with RMSD ~ 1 Å-< 3 Å despite > 46% protein sequence substitutions in transmembrane alpha-helices. We also show the differences of surface hydrophobicity between the native solute carrier transporters and their QTY variants. Our study may further stimulate designs of water-soluble transmembrane proteins and other aggregated proteins for drug discovery and biotechnological applications.

PURPOSE: This study aims to review and evaluate available informatics platforms for research and management purposes of Lynch syndrome (LS) to identify gaps and needs for future development. METHODS: LS informatics tools were identified through literature search in four publication databases (1 and Scopus). First, the LS and functional elements of every informatics tools for LS were introduced. Then, current existing LS informatics tools were reviewed and explained. RESULTS: A detailed review of implemented studies shows that many types of informatics platforms are available for LS management (ie, prediction model, clinical decision support system, database website, and other tools for research and management purposes of LS). Moreover, several dimensions of existing LS informatics tools were discussed and features and positive findings were reported. CONCLUSION: Reviewing the literature reveals that several LS informatics tools were focused on gene-specific estimate, cancer risk prediction, identifying/screening patients, supporting personalized care of individuals with LS, and storing mismatch repair mutations information. Nevertheless, these platforms do not fully cover the care and research purposes. For instance, future developments of LS tools require more attention to dynamic knowledgebase, extra-colonic lynch-related cancers on the basis of precision medicine, variants of unknown significance, and support from diagnosis to surveillance for patient follow-up. Insights and recommendations provided in this study could help researchers and developers to meet the existing challenges in future developments.

Professor Josef Charvát, a prominent internist and endocrinologist, has been following the issues of cybernetics and computers from the very beginning. In the difficult times of the 1950s, he was heavily involved in the organization of seminars and meetings of workers in technical and medical fields interested in cybernetics. Later, in his books for the general public, he foresaw a significant application of computers in medicine. His predictions are quite accurate even 50 years after the publication of these ideas.

The Future of Nursing 2020 to 2030 report explicitly addresses the need for integration of nursing expertise in designing, generating, analyzing, and applying data to support initiatives focused on social determinants of health (SDOH) and health equity. The metrics necessary to enable and evaluate progress on all recommendations require harnessing existing data sources and developing new ones, as well as transforming and integrating data into information systems to facilitate communication, information sharing, and decision making among the key stakeholders. We examine the recommendations of the 2021 report through an interdisciplinary lens that integrates nursing, biomedical informatics, and data science by addressing three critical questions: (a) what data are needed?, (b) what infrastructure and processes are needed to transform data into information?, and (c) what information systems are needed to "level up" nurse-led interventions from the micro-level to the meso- and macro-levels to address social determinants of health and advance health equity?

PRCIS: Utilizing an automated pipeline for data extraction from electronic health records provides real-world information on the success of various glaucoma procedures, with tube shunt implantation associated with increased failure rates compared with trabeculectomy. BACKGROUND: We aimed to evaluate the long-term survival of glaucoma surgeries using an automated pipeline for extraction of outcomes from electronic health records. METHODS: A retrospective observational study from a single academic center. Patients undergoing trabeculectomy, Ex-PRESS shunt, Baerveldt, and Ahmed tube shunt insertion from 2009 to 2018 were identified from electronic health record procedure codes. Patient characteristics were identified from structured and unstructured fields using a previously validated natural language processing pipeline. RESULTS: Five hundred twelve patients underwent 711 glaucoma surgeries: 287 trabeculectomies, 47 Ex-PRESS shunts, 274 Baerveldt and 103 Ahmed tube implantations. The Median follow-up was 359 days. The mean baseline IOP was 24.4 mm Hg (SD 10.9), and 73.1% were on ≥3 medications. Compared with trabeculectomy, tube shunt surgery had a higher risk of failure (Baerveldt: Hazard Ratio (HR) 1.44, 95% CI 1.02 to 2.02; Ahmed: HR 2.01, 95% CI 1.28 to 3.17). Previous glaucoma surgery was associated with increased failure (≥2 previous surgeries: HR 2.74, 95% CI 1.62 to 4.64), as were fewer baseline medications (<3 medications: HR 2.96, 95% CI 2.12 to 4.13) and male sex (HR 1.40, 95% CI 1.03 to 1.90). At 1 year, tube shunt patients had a 2.53 mm Hg ( P =0.002) higher IOP compared with trabeculectomy patients. CONCLUSIONS: Baerveldt and Ahmed tube shunt implantation was associated with increased failure compared with trabeculectomy. Fewer baseline medications, previous glaucoma surgeries, and male sex were also risk factors for failure. These results demonstrate the utility of applying an informatics pipeline to electronic health records to investigate key clinical questions using real-world evidence.

RESUMO

Natural hazards (NHs) associated with climate change have been increasing in frequency and intensity. These acute events impact humans both directly and through their effects on social and environmental determinants of health. Rather than relying on a fully reactive incident response disposition, it is crucial to ramp up preparedness initiatives for worsening case scenarios. In this perspective, we review the landscape of NH effects for human health and explore the potential of health informatics to address associated challenges, specifically from a preparedness angle. We outline important components in a health informatics agenda for hazard preparedness involving hazard-disease associations, social determinants of health, and hazard forecasting models, and call for novel methods to integrate them toward projecting healthcare needs in the wake of a hazard. We describe potential gaps and barriers in implementing these components and propose some high-level ideas to address them.

PURPOSE: Multiple data formats in the MRS community currently hinder data sharing and integration. NIfTI-MRS is proposed as a standard spectroscopy data format, implemented as an extension to the Neuroimaging informatics technology initiative (NIfTI) format. This standardized format can facilitate data sharing and algorithm development as well as ease integration of MRS analysis alongside other imaging modalities. METHODS: A file format using the NIfTI header extension framework incorporates essential spectroscopic metadata and additional encoding dimensions. A detailed description of the specification is provided. An open-source command-line conversion program is implemented to convert single-voxel and spectroscopic imaging data to NIfTI-MRS. Visualization of data in NIfTI-MRS is provided by development of a dedicated plugin for FSLeyes, the FMRIB Software Library (FSL) image viewer. RESULTS: Online documentation and 10 example datasets in the proposed format are provided. Code examples of NIfTI-MRS readers are implemented in common programming languages. Conversion software, spec2nii, currently converts 14 formats where data is stored in image-space to NIfTI-MRS, including Digital Imaging and Communications in Medicine (DICOM) and vendor proprietary formats. CONCLUSION: NIfTI-MRS aims to solve issues arising from multiple data formats being used in the MRS community. Through a single conversion point, processing and analysis of MRS data are simplified, thereby lowering the barrier to use of MRS. Furthermore, it can serve as the basis for open data sharing, collaboration, and interoperability of analysis programs. Greater standardization and harmonization become possible. By aligning with the dominant format in neuroimaging, NIfTI-MRS enables the use of mature tools present in the imaging community, demonstrated in this work by using a dedicated imaging tool, FSLeyes, for visualization.

MOTIVATION: The i2b2 platform is used at major academic health institutions and research consortia for querying for electronic health data. However, a major obstacle for wider utilization of the platform is the complexity of data loading that entails a steep curve of learning the platform's complex data schemas. To address this problem, we have developed the i2b2-etl package that simplifies the data loading process, which will facilitate wider deployment and utilization of the platform. RESULTS: We have implemented i2b2-etl as a Python application that imports ontology and patient data using simplified input file schemas and provides inbuilt record number de-identification and data validation. We describe a real-world deployment of i2b2-etl for a population-management initiative at MassGeneral Brigham. AVAILABILITY AND IMPLEMENTATION: i2b2-etl is a free, open-source application implemented in Python available under the Mozilla 2 license. The application can be downloaded as compiled docker images. RESUMO

NHS knowledge and library specialists have been shown to save the time of busy healthcare professionals through the range of services they provide. To ensure continued effectiveness it is vital that appropriate learning and development opportunities are provided to maintain and enhance the knowledge and skills of this highly specialist staff group. This paper outlines recent progress in developing accredited continuing professional development opportunities for NHS knowledge and library specialists with a focus on the development of digital and data skills.
