RESUMO

Localized prostate cancer exhibits multiple genomic alterations and heterogeneity at the proteomic level. Single-cell technologies capture important cell-to-cell variability responsible for heterogeneity in biomarker expression that may be overlooked when molecular alterations are based on bulk tissue samples. This study aims to identify prognostic biomarkers and describe the heterogeneity of prostate cancer and the associated microenvironment by simultaneously quantifying 36 proteins using single-cell mass cytometry analysis of over 1.6 million cells from 58 men with localized prostate cancer. We perform this task, using a high-dimensional clustering pipeline named Franken to describe subpopulations of immune, stromal, and prostate cells, including changes occurring in tumor tissues and high-grade disease that provide insights into the coordinated progression of prostate cancer. Our results further indicate that men with localized disease already harbor rare subpopulations that typically occur in castration-resistant and metastatic disease.

INTRODUCTION: The worldwide spread of SARS-COV2 had led to a delay in treatment of numerous urological pathologies, even in emergency conditions. We therefore sought to determine whether the timing of diagnosis and treatment and the postoperative outcome of patients with testicular torsion had been changed during the COVID pandemic. MATERIALS AND METHODS: We considered all patients evaluated in the emergency department (ED) for testicular torsion from February 2018 to August 2019 (pre-COVID period) and from February 2020 to August 2021 (during COVID pandemic). All patients underwent clinical and ultrasound evaluation and subsequently scrotal exploration. Primary outcomes were the time differences from pain onset to ED presentation and from ED presentation to surgical treatment. We also investigated whether the number or orchiectomies required changed during the pandemic. RESULTS: A total of 54 patients were divided in two groups: 40 patients in pre-COVID-19 group and 14 in the COVID-19 cohort. Mean time from symptoms onset to ED access was longer during the pandemic (4.2 ± 5.7 versus 39.6 ± 37.3 hours, p = 0.009). Mean time from ED access to surgery was similar (2.9 ± 1.1 versus 4.2 ± 2.3, p = 0.355). In addition, the number of orchiectomies was higher in COVID-19 group (2.5% versus 28.6%, p < 0.01), compared to a lower number of detorsions (97.5% versus 71.4%, p < 0.01). Elapsed time from pain onset to surgery was directly correlated with the increased white blood cell (WBC) count after surgery (r = 0.399, p = 0.002). DISCUSSION AND CONCLUSIONS: The current study identifies a significant delay in presentation of testicular torsion which resulted in a significant increase in orchiectomies with the expected decreased in detorsion/orchiopexy. In addition, there was an increase in the WBC at presentation associated with delayed presentation.

Systematic follow-up examinations of patients cured of testicular cancer first gained attention by caregivers in the 1980s only after the management of the disease had significantly been improved by the introduction of cisplatin-based chemotherapy and almost synchronously, by the implementation of computerized tomography (CT) and serum tumor markers. Follow-up involves three aims: early diagnosis of recurrence, detection of treatment-related toxicity, and detection of secondary diseases. As the clinical presentation of testicular cancer is very heterogeneous, there is no uniform follow-up for the disease. Instead, risk-adapted follow-up schedules are required. Since the release of the German AWMF S3 guideline for the management of testicular cancer in 2019, high level evidence has accumulated for the noninferiority of magnetic resonance imaging (MRI) to CT with regard to abdominal imaging. Therefore, it is appropriate to modify the recommendations for follow-up given in the 2019 issue of the S3 guidelines. The modifications recommended herein relate to three issues: (1) Only three risk groups (instead of formerly four) are identified, i.e., seminoma (all stages); nonseminoma clinical stage 1b (i.e., pT2, with lymphovascular invasion) on surveillance; nonseminoma all other stages. All patients cured from poor risk disease or from relapses require individual follow-up schedules not included in the recommendations tabulated herein. (2) CT and abdominal sonography are replaced by MRI. (3) Chest X­ray imaging during follow-up of seminoma patients is no longer recommended.

Castration can significantly enhance fat deposition in pigs, and the molecular mechanism of fat deposition caused by castration and its influence on fat deposition in different parts of pigs remain unclear. RNA-seq was performed on adipose tissue from different parts of castrated and intact Yorkshire pigs. Different ceRNA networks were constructed for different fat parts. GO and KEGG pathway annotations suggested that testosterone elevates cell migration and affects differentiation and apoptosis in back fat, while it predisposes animals to glycolipid metabolism disorders and increases the expression of inflammatory cytokines in abdominal fat. The interaction between M-7474, novel_miR_243 and SGK1 was verified by dual fluorescence experiments. This ceRNA relationship has also been demonstrated in porcine preadipocytes. Overexpression of M-7474 significantly inhibited the differentiation of preadipocytes compared to the control group. When 100 nM testosterone was added during preadipocyte differentiation, the expression of M-7474 was increased, and preadipocyte differentiation was significantly inhibited. Testosterone can affect preadipocyte differentiation by upregulating the expression of M-7474, sponging novel-miR-243, and regulating the expression of genes such as SGK1. At the same time, HSD11B1 and SLC2A4 may also be regulated by the corresponding lncRNA and miRNA, which ultimately affects glucose uptake by adipocytes and leads to obesity.

BACKGROUND: Salivary gland (SMG) degeneration and dysfunction are common symptoms that occur after sex hormone deprivation, but the underlying mechanisms remain largely unknown. Additionally, immunocastration, which causes drop of sex hormones, has been developed as an alternative to surgical castration, however whether it exerts similar effects as surgical castration on the salivary glands is unknown. Through histological and RNA-seq analysis, we assessed changes in morphology and transcriptome of SMG in response to immunocastration (IM) versus surgical castration (bilateral orchiectomy, ORC). RESULTS: Compared to entire males (EM), ORC caused severe degeneration of SMG in rats, as evidenced by both decreased (P < 0.01) SMG weight and organ index, and by decreased (P < 0.01) quantity of SMG acini and ducts. IM had minimal effects (P > 0.05) on SMG weight and organ index, but it still caused degeneration (P < 0.05) of the acini and ducts. Even though, the quantity of both SMG acini and ducts was much higher (P < 0.001) in IM than in ORC. Functional enrichment analysis of the common regulated genes by ORC/IM revealed disrupted epithelial cell development, angiogenesis, anatomical structure morphogenesis and enhanced cell death are associated with SMG degeneration in deprivation of androgens. Integrated data analysis shown that there existed a selective hyperfunction of SMG ribosome and mitochondrion in ORC but not in IM, which might be associated with more severe degeneration of SMG in ORC than in IM. CONCLUSIONS: Our findings suggested that both surgical castration and immunocastration caused SMG degeneration by disrupting epithelial cell development, angiogenesis, anatomical structure morphogenesis and enhancing cell death. But, surgical castration selectively induced hyperfunction of SMG ribosome and mitochondrion, thus causing more severe degeneration of SMG than immunocastration.

BACKGROUND: Androgen deprivation therapy (ADT) and androgen receptor signaling inhibitors (ARSI) are associated with deleterious physical effects, which exercise may mitigate; however, exercise has never been studied in patients initiating treatment with ADT and an ARSI. Our objective was to determine whether supervised exercise prior to and during initial therapy could mitigate adverse effects of ADT plus enzalutamide. METHODS: We conducted a single center trial in patients with recurrent prostate cancer treated with ADT and enzalutamide. We randomized 26 patients to 16 weeks of supervised exercise (aerobic and resistance), starting 4 weeks before initiation of ADT and enzalutamide, or usual care. The primary endpoint was change in peak oxygen uptake (VO2peak) as a measure of cardiorespiratory fitness (CRF). Secondary endpoints were functional capacity, maximal strength, body composition, patient-reported outcomes, safety, and feasibility. Analysis of covariance was used to compare outcomes for groups at Week 17 adjusted for baseline values. RESULTS: The usual care group (N = 13) showed declines from baseline to week 17 in both absolute CRF (-0.31 L/min, -10.9%; p < 0.01) and relative CRF (-3.2 mL/kg/min, -8.9%; p = 0.04); worse fatigue (p = 0.01); and worse quality of life (p = 0.01). At week 17, the exercise group (N = 13) demonstrated improved absolute CRF (between-group change +0.20 L/min, p = 0.05), leg strength (+48.6 kg, p < 0.01) and functional capacity (+21.0 m, p = 0.01) at week 17. CONCLUSIONS: This is the first randomized controlled trial demonstrating a clinically significant decline in CRF in patients initiating ADT and enzalutamide. We show the effectiveness of short-term supervised exercise to mitigate declines in absolute CRF, and improve maximal leg strength and functional capacity. GOV IDENTIFIER: NCT02256111.

To prevent boar taint, male piglets are commonly castrated without pain relief, causing them tremendous pain. There are, however, three alternatives, all of which have pros and cons: (1) surgical castration (SC) with pain relief, which removes boar taint but involves extra work for farmers and veterinarians; (2) raising non-castrated or entire male pigs (EM) in combination with a boar-taint detection method; and (3) immuno-castration (IC) by vaccination, which can lower the risk of boar taint acquired from GnRH pulses, but there are concerns about consumer response. The successful marketing of products from animals treated by alternatives to conventional castration depends on consumer acceptance. The current study (involving 3574 participants from Belgium, France, Spain, and Poland) aims to determine whether consumers' willingness to pay for meat from animals treated by alternatives depends on their attitude towards pork, attitude towards local ways of farming, and knowledge of animal welfare. We interpret these in the context of a meat-related moral dilemma and further investigate whether consumers resolve the moral dilemma by applying meat-eating-justification (i.e., apologetic or unapologetic) strategies. The results show that participants are least willing to pay for pork from castrates without pain relief. Willingness to pay for IC pork scores highest, followed by EM. Some consumer groups used an apologetic strategy to reduce the dissonance between moral dilemma and willingness to pay for meat from SC castrates. For the European market, it appears therefore feasible to market pork produced using IC or EM methods.

Dry-cured ham is a high-quality product elaborated through a long and complex process. To ensure the success of the process, it is necessary to select the most suitable pork leg and one of the major factors is pig castration. Due to animal welfare, pig castration is becoming a paramount issue in recent years. The proteomic differences of dry-cured ham from immunecastrated pigs against entire females, as well as, between immunecastrated pigs and surgically castrated males were analysed. The identification and quantification of proteins were carried out by sequential window acquisition of all theoretical mass spectra (SWATH-MS). A total of 249 proteins were identified across the samples of dry-cured ham, resulting in 17 and 37 differentially abundant proteins in the case of males and females, respectively. In the case of males, a high abundance of structural proteins in dry-cured ham from surgically castrated animals as well as a high abundance of trypsinogen and proteosome subunit C9-like protein with protease activity in samples from immunocastrated males suggests that immunocastration impact on myofibrils of dry-cured ham. Regarding females, the immunocastration provoked an increase of abundance in several structural proteins of the myosin heavy chain (MYH7, MYH7B and MYH4) and a decrease in others (ACTN2, TNNT3, MYL3 and TCAP) concerning entire. Overall, MYH4 and ACT were found to a greater degree in immunocastrated males and females indicating a potential for biomarkers.

Testicular epidermoid cysts are relatively rare, accounting for about 1% of all testicular tumors, and are often treated by high orchiectomy. We describe here the case of a testicular epidermoid cyst treated by testicle-sparing surgery due to a preoperative diagnosis. A 23-year-old man complained of a painless mass in the right scrotum. Physical examination revealed a firm little fingertip-sized smooth-surfaced mass in the right testis. Ultrasonography showed a hypoechoic lesion with an echogenic rim in the right testis. A T2-weighted magnetic resonance image showed a well-demarcated mass with a low signal outline. On the basis of a preoperative diagnosis of epidermoid cyst, intraoperative testicular frozen section was performed, and the mass was resected surgically while preserving the testis.

CONTEXT: Harmonisation of outcome reporting and definitions for clinical trials and routine patient records can enable health care systems to provide more efficient outcome-driven and patient-centred interventions. We report on the work of the PIONEER Consortium in this context for prostate cancer (PCa). OBJECTIVE: To update and integrate existing core outcome sets (COS) for PCa for the different stages of the disease, assess their applicability, and develop standardised definitions of prioritised outcomes. EVIDENCE ACQUISITION: We followed a four-stage process involving: (1) systematic reviews; (2) qualitative interviews; (3) expert group meetings to agree standardised terminologies; and (4) recommendations for the most appropriate definitions of clinician-reported outcomes. EVIDENCE SYNTHESIS: Following four systematic reviews, a multinational interview study, and expert group consensus meetings, we defined the most clinically suitable definitions for (1) COS for localised and locally advanced PCa and (2) COS for metastatic and nonmetastatic castration-resistant PCa. No new outcomes were identified in our COS for localised and locally advanced PCa. For our COS for metastatic and nonmetastatic castration-resistant PCa, nine new core outcomes were identified. CONCLUSIONS: These are the first COS for PCa for which the definitions of prioritised outcomes have been surveyed in a systematic, transparent, and replicable way. This is also the first time that outcome definitions across all prostate cancer COS have been agreed on by a multidisciplinary expert group and recommended for use in research and clinical practice. To limit heterogeneity across research, these COS should be recommended for future effectiveness trials, systematic reviews, guidelines and clinical practice of localised and metastatic PCa. PATIENT SUMMARY: Patient outcomes after treatment for prostate cancer (PCa) are difficult to compare because of variability. To allow better use of data from patients with PCa, the PIONEER Consortium has standardised and recommended outcomes (and their definitions) that should be collected as a minimum in all future studies.

OBJECTIVE: To investigate the effect of theatre temperature on body temperature in rabbits undergoing castration or ovariohysterectomy surgery during general anaesthesia. STUDY DESIGN: Prospective, clinical study. ANIMALS: A group of 88 rabbits presented for elective neutering. METHODS: Rabbits were divided into male (31/54) and female (23/54) groups and assigned to one of two theatre temperatures via coin toss. Theatre temperature was 23 °C (±2 °C) for group A (n = 37/54) and 28 °C (±2 °C) for group B (n = 17/54). During anaesthesia and recovery, theatre temperature and rectal temperature were recorded every 5 minutes. Time to resumption of feeding and passing faeces were recorded. Data are presented as median (interquartile range) or mean (± standard deviation). Statistical analyses comprised a mixed-effects model, with Sidak's multiple comparison test for post-hoc testing and Fisher's exact test; p < 0.05. RESULTS: A total of 54 rabbits completed the study, with median age 6 (4-9) months and median weight 1.53 (1.30-1.79) kg. In rabbits undergoing castration, theatre temperature did not significantly affect body temperature. Mean temperatures immediately after induction were 38.6 °C and 38.7 °C and at the end of the procedure 38.5 °C and 38.5 °C for group A and group B, respectively. In rabbits undergoing ovariohysterectomy, mean temperatures immediately after induction were 38.3 °C and 38.8 °C and at the end of the procedure 38.1 °C and 39.2 °C for group A and group B, respectively. Rabbits undergoing ovariohysterectomy at an ambient temperature of 28 °C had a significantly higher final temperature, mean ± 1.15 °C (95% confidence interval, 0.47-1.83), compared with 23 °C (p = 0.001). Theatre temperature did not affect return to feeding or defaecating. CONCLUSIONS AND CLINICAL RELEVANCE: During anaesthesia an ambient theatre temperature of 28 °C may reduce the risk of hypothermia in rabbits undergoing ovariohysterectomy or similarly invasive surgery.

INTRODUCTION: The present study analyzed piglet castration under isoflurane anesthesia in ten farms. The data of 202 male piglets in the age between three to eleven days and with a body weight of 1 - 4,7 kg were collected. The aim of this study was to compare the PorcAnest®, Pignap® and Pigsleeper® anesthesia machines used in Switzerland. Isoflurane, carbon dioxide (CO2) and oxygen (O2) values were measured in relation to their significance for anesthesia quality. Therefore, preoperative stress level and movements were assessed during skin incision and the dissection of the spermatic cords. Isoflurane, CO2 and O2 values were recorded in the inhaled and exhaled gases next to the nostrils at 30, 60, 75 and 90 seconds after the start of anesthesia. Movements to the skin incisions and dissection of the spermatic cord were observed in 32 % and 37 % respectively. Movements increased with the decrease of measured inspiratory and expiratory isoflurane values (FIIso and ETIso). Significantly lower inspiratory O2 values (FIO2: 12 - 21 volume % vs 21 - 95 volume %) and stronger movements were measured in anesthesia machines using room air compared to machines operated with medical oxygen. Results indicate a potential for improvement in anesthesia deph piglet castration under isoflurane inhalaton anesthesia and THE measured isoflurane, CO2 and O2 values differ significantly between PorcAnest®, Pignap® and Pigsleeper® anesthesia machines. These fluctuations could be recognized and reduced by annual professional maintenance and / or the addition of an anesthetic gas analyzer. In addition, a change to medical oxygen is indicated for all anesthetic machines. Anesthesia machines have to be optimized in consultation with the manufacturers as soon as possible and the farmers have to be trained and sensitized to ensure pain-free castration.


INTRODUCTION: La présente étude a analysé la castration de porcelets sous anesthésie à l'isoflurane dans dix exploitations. Les données de 202 porcelets mâles âgés de trois à onze jours et pesant entre 1 et 4,7 kg ont été recueillies. Le but de cette étude était de comparer les appareils d'anesthésie PorcAnest®, Pignap® et Pigsleeper® utilisés en Suisse. Les valeurs d>isoflurane, de dioxyde de carbone (CO2) et d>oxygène (O2) ont été mesurées en fonction de leur importance pour la qualité de l>anesthésie. Pour cela, le niveau de stress préopératoire et les mouvements ont été évalués lors de l'incision cutanée et la dissection des cordons spermatiques. Les valeurs d'isoflurane, de CO2 et d'O2 ont été enregistrées dans les gaz inhalés et expirés à côté des narines à 30, 60, 75 et 90 secondes après le début de l'anesthésie. Des mouvements lors des incisions cutanées et de la dissection du cordon spermatique ont été observés respectivement dans 32 % et 37 % des cas. Les mouvements ont augmenté avec la diminution des valeurs d'isoflurane inspiratoires et expiratoires mesurées (FIIso et ETIso). Des valeurs d'O2 inspiratoire significativement plus faibles (FIO2 : 12 - 21 % du volume contre 21 - 95 % du volume) et des mouvements plus importants ont été constatés avec les appareils d'anesthésie utilisant l'air ambiant par rapport à ceux fonctionnant avec de l'oxygène médical. Les résultats indiquent qu'il existe un potentiel d'amélioration de la profondeur d'anesthésie dans la castration des porcelets sous anesthésie par inhalation d'isoflurane et que les valeurs mesurées d'isoflurane, de CO2 et d'O2 diffèrent significativement entre les appareils d'anesthésie PorcAnest®, Pignap® et Pigsleeper®. Ces fluctuations pourraient être identifiées et réduites par un entretien professionnel annuel et/ou par l'ajout d'un analyseur de gaz anesthésique. De plus, un passage à l'oxygène médical est indiqué pour tous les appareils d'anesthésie. Les appareils d'anesthésie doivent être optimisés en concertation avec les fabricants dans les plus brefs délais et les éleveurs doivent être formés et sensibilisés pour assurer une castration sans douleur.

RESUMO

Splenogonadal fusion is a rare benign congenital anomaly in which there is an abnormal connection between the gonad and the spleen. It was first described over 100 years ago with limited reports in the literature since then. Its similarity in presentation to testicular neoplasia poses a significant challenge in diagnosis and management, often resulting in radical orchidectomy. We present the case of a 31-year-old man who presented with a rapidly growing left-sided testicular mass and suspicious ultrasound findings; histology from the subsequent radical inguinal orchidectomy showed findings consistent with splenogonadal fusion. We describe points for consideration in the clinical history, examination and imaging that could suggest splenogonadal fusion, including preoperative technetium-99m-sulfur colloid imaging and intraoperative frozen section evaluation, which may confirm the diagnosis and prevent unnecessary orchidectomy.

A 52-year-old man was referred to our hospital for evaluation of painless right scrotal swelling persisting for 3 months. Palpation detected swelling and induration centered on the head of the upper epididymis, and ultrasonography revealed a blood-filled nodular mass at the same site continuing to the spermatic cord. No abnormalities were observed in the bilateral testes. Blood tests were negative for tumor markers such as α fetoprotein and human chorionic gonadotropin-ß. Right radical inguinal orchiectomy was performed, and the pathological diagnosis was dedifferentiated liposarcoma of the spermatic cord. Although the spermatic cord stump was negative, the peri-spermatic cord stump, which had an exfoliated surface, was positive. No residual tumor was found on magnetic resonance imaging, but the tumor was suspected to remain. Thus, after approximately 1month, the tissue around the spermatic cord was resected. Eight months after the initial operation, no recurrence was observed. Here, we report a case of dedifferentiated liposarcoma of the spermatic cord, which is relatively rare, and review the related literature.

BACKGROUND: Valproic acid (VPA) pharmacological mechanisms are related to the anti-inflammatory and anti-viral effects. VPA is a histone deacetylases inhibitor and serves a role in its immunomodulatory impacts. VPA has complex effects on immune cell's mitochondrial metabolism. The SLC5A8 transporter of short fatty acids has an active role in regulating mitochondrial metabolism. The study aimed to investigate whether SLC5A8 expresses the sex-related difference and how SLC5A8 expression depends on gonadal hormones, VPA treatment, and NKCC1 expression in rat thymocytes. METHODS: Control groups and VPA-treated gonad-intact and gonadectomized Wistar male and female rats were investigated (n = 6 in a group). The VPA 300 mg/kg/day in drinking water was given for 4 weeks. The SLC5A8 (Slc5a8 gene) and NKCC1 (Slc12a2 gene) RNA expressions were determined by the RT-PCR method. RESULTS: The higher Slc5a8 expression was found in the gonad-intact males than respective females (p = 0.004). VPA treatment decreased the Slc5a8 expression in gonad-intact and castrated males (p = 0.02 and p = 0.03, respectively), and increased in gonad-intact female rats compared to their control (p = 0.03). No significant difference in the Slc5a8 expression between the ovariectomized female control and VPA-treated females was determined (p > 0.05). VPA treatment alters the correlation between Slc5a8 and Slc12a2 gene expression in thymocytes of gonad-intact rats. CONCLUSION: VPA effect on the Slc5a8 expression in rat thymocytes is gender- and gonadal hormone-dependent.

Összefoglaló. A splenogonadalis fúzió ritka fejlodési rendellenesség, melynek lényege az ivarmirigy (leggyakrabban a here) és a lépszövet rendellenes kapcsolódása. A szerzok egy féléves csecsemo esetét kívánják bemutatni, akinél már a megszületést követoen észlelheto volt a bal oldali here megnagyobbodása. A mutét elotti képalkotó vizsgálatok (ultrahang, MRI), a fizikális vizsgálat és az intraoperatív lelet a splenogonadalis fúzió lehetoségét vetette fel. A mutét során a makroszkóposan lépszövetnek imponáló elváltozás eltávolításra került, majd rutinorchiopexia történt. A hisztológiai vizsgálat alátámasztotta a feltételezett diagnózist. Az entitást 1883-ban írták le eloször, azóta kevesebb mint 200 esetrol számoltak be az angol nyelvu irodalomban. Az utóbbi 30 évben magyar szerzoktol egy közlemény jelent meg. Az elváltozás a banális szülési trauma okozta haematomára, újszülöttkori heretorzióra, malignus heretumorra is hasonlíthat, komoly differenciáldiagnosztikai problémát okozva. A splenogonadalis fúzió jóindulatú elváltozás, a helyes diagnózis esetén elkerülheto a felesleges orchiectomia. Orv Hetil. 2022; 163(7): 288-290. Summary. Splenogonadal fusion is a rare congenital malformation specified as the presence of splenic tissue connected to the gonads (mainly testis). The authors present the case of a 6-month-old male infant, in whom left inguino-scrotal mass was noticed soon after birth. The mass, based on preoperative diagnostic setup (ultrasound, MRI), physical examination and intraoperative findings were suspicious for splenogonadal fusion. At surgery, the mass was resected from the testis, which macroscopically resembled a splenic tissue. Following the resection, orchiopexy was performed. Histology confirmed splenogonadal fusion. Since the first description of the malformation in 1883, only less than 200 cases have been reported in the English literature. Only a single article has been published in Hungary in the last 30 years. Splenogonadal fusion represents a serious differential diagnostic problem. Hematoma caused by trauma, neonatal testicular torsion, benign or malignant testicular tumors may also present similarly. In our case, proper presumptive diagnosis made it possible to avoid unnecessary orchiectomy. Orv Hetil. 2022; 163(7): 288-290.

Primary testicular lymphoma is a very rare entity. However, its clinical course is poor with a high recurrence and low survival rate. Given its low prevalence, we present 2cases with an update on the treatment and progression of this disease.
