RESUMO

This paper introduces a new prevention strategy against child sexual abuse (both offline and online) in the United States. The Global Prevention Project is a supportive treatment program designed for nonoffending minor-attracted persons who reside in the community. Attraction to minors and the underlying scientific terms (pedophilia/hebephilia) are discussed and a framework is provided for how to implement such a program in the United States. Our treatment modality is described to provide transparency in our clinical work. We discuss challenges encountered in this domain with possible solutions and the legal ramifications of preventing child sexual abuse behaviors by targeting nonoffending individuals.

El maltrato sexual infantil es un tema complejo que debe tratarse de manera seria y responsable, sobre todo porque ha venido incrementándose de manera alarmante en los últimos tiempos. En este artículo se aborda tan importante tema y se ofrece evidencias de 4 casos de abuso sexual en niños indefensos, cuyos sucesos no fueron denunciados a las autoridades judiciales por parte de los padres, debido a prejuicios y lazos de familiaridad o de amistad con los victimarios. El objetivo es despertar en los padres y familiares, sobre todo de los niños más desprotegidos (por discapacidades físicas o mentales o por la edad), la agudeza para detectar a tiempo un posible acto de esta índole y evitarlo, así como sensibilizar a todos en la protección de la inocencia infantil contra perpetradores inescrupulosos.


Child sexual abuse is a complex topic which should be treated in a serious and responsable way, above all because it has been increasing in an alarming way in the last times. In the work, this important topic is analized, and evidencies of 4 sexual abuses in defenseless children are presented, cases which were not denounced to the judicial authorities by their parents due to prejudices and familiarity or friendship links with the perpetrator. The objective is to awake in parents and relatives, above all from defenseless children (due to physical or mental discapacities or due to age), the acuity to detect on time a possible case and to avoid it, as well as to raise awareness to all in the protection of childhood inocence against unscrupulous perpetrators.

The prevalence of online child pornography is a major societal issue. The criminal justice system has struggled with assessing the risk of individuals involved in online sexual offenses against children, especially when it involves the possession of child pornography. Research suggests there are different categories of offenders involved in this type of behavior (e.g., Online Child Pornography Offenders, Dual Offenders, Contact Offenders), with each category having different motivations, contributing factors, and levels of risk to re-offend or escalate their criminal behavior to more serious offenses (i.e., collecting pictures to contact offending). Determining the risk that individuals involved in online sexual offenses against children pose to re-offend or escalate their criminal behavior has been problematic. Traditional sexual offender risk measures have lower predictive validity when dealing with online child pornography offenders. This article discusses the need for a formalized hybrid risk assessment model that combines the current online sex offenses against children risk measures with digital forensics artifact analysis. The evidence derived from digital forensics artifact analysis can supplement the predictive risk factors obtained from these risk assessment tools, thus increasing the reliability and validity of the risk assessment.

OBJECTIVE: To study the changes of evoked EEG oscillations in response to erotic visual stimuli to identify specific patterns in subjects with different variants of pedophilia. MATERIAL AND METHODS: Forty-eight right-handed male offenders who have committed sexual acts against children participated in the study. Seventeen subjects without paraphilic disorders were included in group 1, 17 subjects with heterosexual pedophilia were included in group 2, and 13 subjects with homosexual pedophilia were included in group 3. The stimuli were colored pictures of neutral, heterosexual normative, hetero- and homosexual pedophilic content shortly demonstrated with backward masking. Spectral power of delta, theta, alpha, and beta evoked oscillations (EO) was analyzed. Further, the number of significant changes of EO power in response to erotic stimuli relative to neutral ones was calculated, and topography changes of EO power in response to specific erotic pictures compared to neutral ones were analyzed for each group. RESULTS: The diminished number of significant reactions to specific stimuli was found in the pedophilia groups (18 and 4 in groups 2 and 3, respectively) in comparison with the group without paraphilia (25). The specific changes of EO power in response to specific erotic visual pictures were found for each group. Increased power of delta, theta and alpha EO in the 104-164 ms time window, and decreased power of alpha EO in the 344-920 ms time window were found in group 1. In group 2, specific stimulation elicited decline of delta and theta EO power, possibly related to impaired motivational and emotional aspects of erotic stimuli perception. The weak changes were found in group 3 - increased power of beta EO in the frontal area. CONCLUSION: The obtained frequency and spatial patterns of EO changes in response to erotic visual pictures are related to specificity of hetero- and homosexual pedophilia.

There are few empirical studies examining sexological features of sexual interest in children among females. A non-representative sample of 20 females and 208 males who self-identified as having a sexual interest in children completed an anonymous survey. The primary purpose of the present study was to examine sexological features of sexual interest in children among females. Most females reported interest in prepubescent and pubescent children and rated their interest in different age categories similarly. Most females reported an interest in boys. Females first experienced sexual attractions during childhood, but awareness of sexual interest in children occurred later in their adolescence.

The purpose of our study is to carry out a comprehensive analysis of the phenomenon of pedophilia in the context of child protection from sexual abuse, as well as to substantiate the need for treatment of persons with sexual perversions as a way to prevent sexual violence against children. Despite the normative consolidation of the inadmissibility of violence against children, including sexual violence, and the current system of preventive measures, today minors are the most victimized group of people. Of particular concern is not only the physical harm caused to a child as a result of sexual violence, but also the negative effects on mental health, which further contributes to the emergence of various psychosomatic illnesses, depressed psycho-emotional state, mental health disorders, suicidal behavior. Analysis of domestic and foreign studies cited in the article shows that the number of criminal attacks on the sexual inviolability of the child is steadily increasing. Most of the victims of such crimes are young children, and the perpetrator is often a close relative or someone known to the child. The use of the digital environment and modern methods of communication by pedophiles in order to harass and coerce children into sexual intercourse contributes to the increase in the number of such crimes. The methodology of the study is based on the dialectical method of knowledge, legal and organizational foundations of preventive activity concerning crimes against sexual freedom and sexual inviolability of the child, interrelation and interdependence of domestic and international experience. The results of the study show that, in order to reduce the number of infringements on the sexual inviolability of the child, a set of legal and medical measures aimed at preventing real and potential threats, primarily at identifying and treating persons with sexual perversions, in particular pedophilia, is necessary. This is especially necessary during and after serving a sentence for relevant crimes.

OBJECTIVES: A pilot study was conducted in order to construct a Polish adaptation of emotional Stroop test in assessment of pedophilia. METHODS: The study consisted of two stages. The first stage involved creating test material by ranking words in adequate lists by competent experts. The second stage consisted of empirical verification of the principle of emotional Stroop test in a non-clinical population. RESULTS: Based on the assessment of five competent experts, words were ordered from the most to the least sexually arousing (Kendall's W from 0.368 to 0.693). Six ranked lists were obtained, and the competent experts were subsequently asked to assess whether these lists were suitable for the study (Lawshe's Content Validity Ratio from 0.6 to 1.0). Two categories of words were merged. Five ranked lists were obtained, and the competent experts were subsequently asked again to assess whether these lists were suitable for the study (Lawshe's Content Validity Ratio 1.0). The created lists of words were approved by allcompetent experts. Based on the experimental study conducted on a non-clinical population, it was shown that, in accordance with the principle of the test, the mean response time for sexually related words was longer that for neutral words. The mean response time for children-related words did not differ significantly from response time for neutral words. CONCLUSIONS: Based on the study with competent experts and conducted experiments, an initial Polish adaptation of the emotional Stroop test for diagnosis of pedophilia has been created. Further studies with persons with pedophilia are needed to implement the test in clinical setting.

A 65-year-old right-handed man gradually became socially indifferent and less active. Four years later, he was indicted for molesting children on multiple occasions. Psychomotor slowness and executive impairment contrasted with sparing of language, semantic memory, visuospatial perception, construction praxis, and right-left orientation. Neuroimaging showed asymmetric atrophy of dorsomedial frontal and anterior temporal lobes, and hypoperfusion of medial prefrontal cortex consistent with a diagnosis of behavioral variant of frontotemporal dementia. Further information revealed that the patient exhibited pedophilic behavior several years prior to symptom onset. We conclude that preexisting developmental pedophilia was "unmasked" by the underlying progressive frontotemporal degeneration.

Paedophilia is a paraphilia characterised by behaviour, drives or sexual fantasies that are intense and recurring and that generally involve children. This sexual deviation is often linked to personal distress, to a significant alteration of personal, family, professional and social life, in a general sense. In our modern Western societies, paedophilia and the perversions associated with it are punishable by law. Paedophilia is a concept that is very ancient, complex and particularly diverse in its many clinical presentations. Its societal impact is very important and contrasts with the real knowledge of this issue among non-specialists, or even among specialists. In this second part of the article, the authors will develop the aetiologies of paedophilia, the therapeutic options and the forensic aspects.

Its societal impact is very important and contrasts with the real knowledge of this issue among non-specialists, or even among specialists. Based on expert clinical experience gathered at the courts and on an exhaustive review of current literature on the subject, the authors propose an update of the concept of paedophilia. In the first part of this work, the authors will discuss several aspects such as the historical background and the clinical aspects and nosography of paedophilia. This literature review will be accompanied by short clinical vignettes, based on the authors' clinical experience.

OBJECTIVE: We quantitatively reviewed the construct validity evidence for all cognitively based indirect measures of sexual interest in prepubescent children (pedophilic interest) and pubescent children (hebephilic interest) using meta-analysis. METHOD: Studies were included if they presented scores on a cognitively based indirect measure of pedohebephilic interest for a sample of adolescent or adult males who had committed a sexual offense against a child 16 years of age or younger, or who reported sexual interest in children, and for a comparison group. Studies were also included if they reported on the strength of association between scores on an indirect measure and an independent indicator of pedohebephilic interest in a sample of males. We used meta-analysis with robust variance estimation to summarize effect sizes and metaregression to test potential moderators. RESULTS: Cognitively based indirect measures of pedohebephilic interest showed a moderate difference between pedohebephilic (n = 2,552) and nonpedohebephilic males (n = 2,434), d = 0.61, 95% CI [0.46, 0.76], k = 39. A small-to-moderate correlation was also observed between indirect measures and independent indicators of pedohebephilic interest, r = .23, 95% CI [0.17, 0.28], k = 23, n = 3,623. These effects were qualified by substantial heterogeneity; however, most moderators we tested did not account for a significant amount of heterogeneity. CONCLUSIONS: Findings suggest that publication bias did not substantially distort the results. However, the lack of significant moderators suggests more research is needed to understand the conditions under which indirect measures best reflect pedohebephilic interest. Androgen deprivation therapy (ADT) is a libido-inhibiting medication that may be necessary to reduce recidivism in the treatment of paraphilic disorders, especially in those with a pedophilic disorder. However, there is a significant risk to develop osteoporosis while using ADT and thereby an increased risk to develop fractures. These risks and benefits must be carefully weighed in the treatment of these patients. We describe a case in which this dilemma is further explained and clarified. We recommend to request a second opinion and a structured risk assessment. If the risk for recidivism remains increased, despite psychotherapeutic interventions, we advise to suspend further rehabilitation into society, and let the reduction of the risk of recidivism prevail over the wishes of the patient.

Pedophilia is a disorder of public concern because of its association with child sexual offense and recidivism. Previous neuroimaging studies of potential brain abnormalities underlying pedophilic behavior, either in idiopathic or acquired (i.e., emerging following brain damages) pedophilia, led to inconsistent results. This study sought to explore the neural underpinnings of pedophilic behavior and to determine the extent to which brain alterations may be related to distinct psychopathological features in pedophilia. To this aim, we run a coordinate based meta-analysis on previously published papers reporting whole brain analysis and a lesion network analysis, using brain lesions as seeds in a resting state connectivity analysis. The behavioral profiling approach was applied to link identified regions with the corresponding psychological processes. While no consistent neuroanatomical alterations were identified in idiopathic pedophilia, the current results support that all the lesions causing acquired pedophilia are localized within a shared resting state network that included posterior midlines structures, right inferior temporal gyrus and bilateral orbitofrontal cortex. These regions are associated with action inhibition and social cognition, abilities that are consistently and severely impaired in acquired pedophiles. This study suggests that idiopathic and acquired pedophilia may be two distinct disorders, in line with their distinctive clinical features, including age of onset, reversibility and modus operandi. Understanding the neurobiological underpinnings of pedophilic behavior may contribute to a more comprehensive characterization of these individuals on a clinical ground, a pivotal step forward for the development of more efficient therapeutic rehabilitation strategies.

Pedophilic interest is a central risk factor for sexual offending against children. Multiple measures exist to assess pedophilic interest, and the present study aims to provide validity evidence for three of these measures in a sample of men convicted of sexual offenses. The association between a phallometric test for pedophilic interest, the Screening Scale for Pedophilic Interest (SSPI), and the sexual deviance factor of the Violence Risk Scale-Sexual Offense (VRS-SO) version was examined in a sample of 261 men who participated in sexual violence reduction services. The association between these measures and sexual recidivism, both as sole predictors and while controlling for static risk, was also assessed. The second aim of the study was to examine the validity of different methods for modeling the distribution of pedophilic interests, using phallometric test scores, based on the findings in recent taxometric research. The measures generally showed a positive and moderate relationship with each other and predicted sexual recidivism. However, the SSPI did not significantly predict sexual recidivism, and when controlling for static risk, only the VRS-SO Sexual Deviance factor significantly predicted this outcome. Modeling phallometric test scores continuously and trichotomously produced significant associations with sexual recidivism; however, only a trichotomous model with two levels remained predictive after controlling for static risk. The results are broadly supportive of measures of pedophilic interest and underscore the importance of appropriately modeling the latent structure of pedophilic interest.

Individuals who identify as pedophilic or hebephilic, and who do not offend, are increasingly visible in online discourse and as a focus of research. Developing knowledge about this population will offer insights into their psychological needs and, potentially, into the mechanisms and strategies individuals use to live offense-free lives. This study examined coping strategies among members of an online forum supporting pedohebephilic individuals who do not wish to offend. Forum users' posts were analyzed using thematic analysis. Eleven themes emerged, which were classifiable into three superordinate themes around (a) the acceptance of pedophilia, (b) strategies to stay safe, and (c) dealing with sexual arousal. These themes offer insight into the varying strategies used by these individuals to cope with stress and/or to remain offense-free. Understanding whether these strategies are adaptive or maladaptive may help develop better support services for those who have not offended and may inform prevention efforts.

This study examined whether men who committed undetected sexual offenses would show more evidence of sexual interest in children than their detected counterparts. It also considered whether denial or minimization of offending history explained observed differences between undetected and detected men. In an archival database of 2,236 men, 96 were undetected and the remainder were detected men who varied in the extent to which they admitted their offending: complete deniers, partial deniers, those who admitted their offense history, and those who disclosed additional unknown offenses. There were differences in self-reported sexual interest in children, but relatively few differences when sexual interest in children was assessed by phallometry or sexual victim history. There were no differences between undetected and detected men who admitted to additional victims. The results suggest that denial is likely more important in understanding group differences on sexual interest in children than detection status.

The Explicit and Implicit Sexual Interest Profile (EISIP) is a multimethod measure of sexual interest in children and adults. It combines indirect latency-based measures such as the Implicit Association Test (IAT), Viewing Time (VT), and explicit self-report measures. This study examined test-retest reliability and absolute temporal agreement of the EISIP over a 2-week interval in persons who were convicted of sexual offenses against children (n = 33) and nonoffending controls (n = 48). Test-retest reliability of the aggregated EISIP measures was high across the whole sample (rtt = .90, intraclass correlation coefficient [ICC] = .90) with the IAT yielding the lowest retest correlations (rtt = .66, ICC = .66). However, these indicators of relative reliability only quantify the temporal stability of individual differences within the group, not the detectability of individual change. Absolute temporal agreement as assessed via Bland-Altman plots ranged from one fourth to three thirds of a standardized unit in the sexual preference scores. This implies that individual change has to exceed medium to large standardized effect sizes to be distinguishable from spontaneous temporal variation in the EISIP measures. Overall, scores of combined measures were largely superior to single measures in terms of both absolute and relative reliability.

Except for pedophilia, little is known about public attitudes toward paraphilias and psychological conditions that are considered risk factors for sexual offending. In the present study we sought to compare the stigma attached to pedophilia with attitudes toward sexual sadism and antisocial tendencies (Study 1, N = 720) and with attitudes toward necrophilia and zoophilia (Study 2, N = 210) in convenience samples of community individuals. When investigating social distance intentions both studies explicitly referred to people with paraphilic sexual interests or antisocial tendencies who had not committed any crimes. In both studies, people with pedophilia emerged as highly stigmatized even though most participants showed awareness that pedophilic interests cannot be chosen or changed at will. The present studies solidify the evidence that pedophilia occupies a place in the public consciousness as less deserving of acceptance than most other sexual offending risk-relevant conditions, including ones that compare in terms of rarity.

The present study aimed to differentiate pedophilic child sex offenders (CSOs) from nonoffending controls (CTLs), as well as contact from noncontact CSOs. For this purpose, we investigated 21 contact CSOs, 20 noncontact CSOs (child pornography offenders), as well as 21 CTLs on neuropsychological test measures and indirect test measures of sexual interest. Multiple logistic regression models showed that three parameters of indirect tests and two neuropsychological test parameters allowed the differentiation of CSOs from CTLs with a maximum accuracy of 87%. The profile of contact and noncontact CSOs was remarkably similar and the optimal model for this group differentiation had a maximum accuracy of 66%, with slightly increased levels of risk-taking behavior and greater susceptibility for perceptual interference in contact CSOs than in noncontact CSOs. The findings suggest that standardized, objective methods can support the assessment of sexual offenders against children in forensic psychiatry and legal psychology.
