ABSTRACT

Introduction: COVID-19 continues to drive research aimed at elucidating the disease's behavior and clinical aspects for improved diagnosis. Objective: To describe oral manifestations reported through a survey by dentate and denture-wearing Cuban individuals hospitalized due to confirmed COVID-19 infection. Methods: A cross-sectional descriptive study was conducted through a survey, involving Cuban individuals aged 18 years and above, confirmed COVID-19 positive by PCR. Exclusions encompassed smokers, alcoholics, regular medication users, those with poor oral hygiene, and individuals with pre-existing oral manifestations. A questionnaire was administered to over a thousand individuals, of which 264 met the criteria. Variables related to COVID-19 infection and oral hygiene were assessed. Data were processed using SPSS, adhering to ethical principles. Results: The study comprised 264 participants with an average age of 39.96 years. Xerostomia emerged as the most prevalent oral manifestation (40.2%), followed by mandibular pain, TMJ, or bone pain (18.9%), and non-dental mouth pain (12.5%). Xerostomia was more prevalent in the 35 to 39 age group, while mandibular pain predominated in the 50 to 54 age group. No statistically significant evidence was found for dentate individuals or denture wearers, but significance was observed for those requiring hospitalization, exhibiting painless tongue lesions, single ulcers, and painful tongue lesions. Conclusions: Xerostomia was the most prevalent oral manifestation, followed by mandibular pain, TMJ or bone pain, and non-dental mouth pain. A statistically significant association was noted between the need for hospitalization and certain oral manifestations. The use of dentures was not significantly related to the studied manifestations.


Introducción: La COVID-19 continúa generando interés en investigaciones que buscan esclarecer el comportamiento de la enfermedad y sus aspectos clínicos para facilitar el diagnóstico. Objetivo: Describir las manifestaciones orales informadas por individuos cubanos dentados, con prótesis, que fueron hospitalizados al dar positivo por COVID-19. Métodos: Se llevó a cabo un estudio descriptivo transversal a través de una encuesta con una muestra de individuos cubanos mayores de 18 años, infectados por COVID-19 y confirmados mediante PCR. Se excluyeron fumadores, alcohólicos, usuarios regulares de medicamentos, personas con mala higiene bucal y aquellos con manifestaciones bucales previas a la infección. Se aplicó un cuestionario a más de mil individuos, de los cuales 264 cumplieron con los criterios. Se utilizaron variables relacionadas con la infección por COVID-19 y la higiene bucal. Los datos se procesaron con SPSS, respetando los principios éticos. Resultados: El estudio incluyó a 264 participantes con una edad promedio de 39,96 años. La xerostomía fue la manifestación bucal más prevalente (40,2 %), seguida por el dolor mandibular, ATM o hueso (18,9 %) y el dolor de boca no dental (12,5 %). La xerostomía fue más frecuente en el grupo de 35 a 39 años, mientras que el dolor mandibular predominó en el grupo de 50 a 54 años. No se encontró evidencia estadística significativa para pacientes dentados o portadores de prótesis, pero sí para aquellos que necesitaron hospitalización, con lesiones en la lengua sin dolor, úlceras únicas y lesiones en la lengua con dolor. Conclusiones: La xerostomía fue la manifestación bucal más prevalente, seguida por el dolor mandibular, ATM o hueso, y el dolor de boca no dental. Se observó una asociación estadísticamente significativa entre la necesidad de hospitalización y ciertas manifestaciones bucales. No se encontró significativo el uso de prótesis en relación con las manifestaciones estudiadas.

ABSTRACT

The convergence of life sciences with neurosciences, nanotechnology, data management, and engineering has caused a technological diversification of the biotechnology, pharmaceutical, and medical technology industries, including the phenomenon of digital transformation, which has given rise to the so-called Fourth Industrial Revolution (Industry 4.0). Confronting the COVID-19 pandemic revealed the outstanding response capacity of the scientific community and the biopharmaceutical industry, based on a multidisciplinary and interinstitutional approach that has achieved an unprecedented integration in the history of biomedical science. Cuba, a small country, with scarce material resources, has had remarkable success in controlling the disease, which also highlights the impact of social factors. This report presents a summary of the most relevant presentations of selected topics during the scientific meeting, "BioHabana 2022: Cancer Immunotherapy and the COVID-19 Pandemic," which was held in Havana Cuba in April 2022.

ABSTRACT

El envejecimiento poblacional constituye uno de los logros y uno de los retos más importantes del desarrollo de las sociedades. La principal causa del envejecimiento es el descenso de la fecundidad de las mujeres, y en menor proporción la emigración externa, sobre todo de jóvenes. En Cuba, ya mueren más que los que nacen, debido fundamentalmente a los bajos niveles de reemplazo en la fecundidad, situación agravada en los años de la pandemia de COVID-19. Entre las consecuencias negativas del envejecimiento poblacional, se identifican cuatro principales: sanitarias, económicas, socioculturales y legales; entre las positivas, hay algunos reportes de cohortes de personas que llegan a edades avanzadas en mejor estado físico y mental. Los cambios ocurridos en la institución familiar en muchas sociedades y la discriminación por edad de los mayores, tienen consecuencias graves para la salud, el bienestar y los derechos humanos de los ancianos. Se considera que existe mucha información sobre el "diagnóstico" y el "pronóstico" del envejecimiento poblacional, pero hay menos acerca de acciones eficaces para su "tratamiento", aspecto complejo y difícil de enfrentar. Se exponen algunas propuestas de mitigación de estos inevitables cambios demográficos. El propósito de esta comunicación es comentar determinados aspectos del envejecimiento de la población cubana.


Population aging constitutes one of the achievements and one of the most important challenges in the development of societies. The main cause of aging is the decrease in the fertility of women and to a lesser extent external emigration, especially of young people. In Cuba, more people die than are born, mainly due to low replacement levels in fertility, a situation aggravated in the years of the COVID-19 pandemic. Among the negative consequences of population aging, four main ones are identified: health, economic, sociocultural and legal; among the positive ones, there are some reports of cohorts of people who reach advanced ages in better physical and mental condition. The changes that have occurred in the family institution in many societies and the age discrimination of the elderly have serious consequences for the health, well-being and human rights of the elderly. It is considered that there is a lot of information about the "diagnosis" and "prognosis" of population aging, but there is less about effective actions for its "treatment", a complex and difficult aspect to face. Some proposals are presented to mitigate these inevitable demographic changes. The purpose of this communication is to comment on certain aspects of the aging of the Cuban population.

Introducción: El coronavirus es una infección viral respiratoria nueva que se convirtió en una pandemia. La comunidad científica global sugirió la implementación de medidas preventivas ante el riesgo de contagio de la población. En Cuba, desde el inicio de la pandemia, el papel de la atención primaria de salud ha sido la prevención y el control de la epidemia, a través de un modelo de medicina familiar y comunitaria con mirada territorial e intersectorial. Objetivo: Profundizar sobre el papel desempeñado por la atención primaria de salud en la lucha contra el COVID-19 en Cuba. Métodos: Se consultaron un total de 27 referencias bibliográficas actualizadas sobre el tema, consistentes en revistas digitales y artículos de Internet en sitios científicos como PubMed, SciELO, Google Académico y Medline. Se utilizaron palabras claves de buscadores como: atención primaria de salud, COVID-19, pesquisa, estrategias, prevención, ciencias médicas, entre otras; se descartaron las que no se correspondían con la temática a estudiar. Conclusiones: En Cuba, la búsqueda activa de personas con sintomatología de infección respiratoria aguda y sus contactos se hace a través de los servicios de atención primaria comunitaria; esta ha demostrado ser uno de los pilares principales en el control y manejo del COVID-19. La pesquisa activa ayuda a identificar el estado de salud individual en grupos de población y a descubrir tempranamente, la morbilidad oculta, y constituye la principal opción para apoyar el trabajo del médico y la enfermera de las familias en Cuba.


Introduction: the coronavirus is a new respiratory viral infection that has become a pandemic. The global scientific community suggested the implementation of preventive measures in view of the risk of contagion of the population. In Cuba, since the beginning of the pandemic, the role of primary health care has been the prevention and control of the epidemic through a family and community medicine model with a territorial and intersectoral perspective. Objective: to deepen the role played by primary health care in Cuba in the fight against COVID-19. Methods: a total of 27 updated bibliographical references on the subject were consulted, consisting of digital journals and Internet articles published on scientific sites such as PubMed, SciELO, Google Scholar and Medline. Primary health care, COVID-19, screening, strategies, prevention, medical sciences, among others were the search engine keywords used; those that did not correspond to the theme to be studied were discarded. Conclusions: the active screening for people with symptoms of acute respiratory infection and their contacts is done in Cuba through the community primary care services; this has proven to be one of the main pillars in the control and management of COVID-19. Active screening helps to identify individual health status in population groups and discover hidden morbidity early, as well as constitutes the cornerstone executed by Cuba to support the work of family doctors and nurses.

BACKGROUND: In Cuba, there is neither a registry of ST Elevation Myocardial Infarction (STEMI), nor are analysis of performance measures widely reported. OBJECTIVE: A review of Cuban studies of patients with STEMI was carried out to describe quality of medical care. METHODS: Cochrane Library, EMBASE, PubMed, Scopus and SciELO, as well as archives of national journals, were all searched for articles on STEMI in Cuba, from 2000 to March 2020. They were included if they reported number or percentage of application of reperfusion therapy; administration of aspirin, enalapril-captopril (ACEI) or beta-blockers; status of patients at discharge; and patient or system delay times. Finally, 17 reports with 7823 patients were included. RESULTS: Thrombolytic therapy was administered to 3991 patients (51%), and 695 patients (8.9%) died. Only four studies, with 880 patients, presented data about prescription of ACEI, aspirin, and beta-blockers, which were administered to 381 (45.3%), 824 (93.6%), 464 (52.7%) patients, respectively. Coronary intervention was reported in 5 studies with 3422 patients, being performed in 661 (19.3%).  Conclusions: Quality of care of patients with STEMI seems to be poorer than reported in similar scenarios. Thrombolytic administration is still low, although mortality decreases in this period. Other pharmacological treatments were insufficiently fulfilled.

ABSTRACT

El aislamiento social por la pandemia ha repercutido negativamente en la salud mental; asimismo, han sido evidentes los síntomas psicológicos en pacientes que han padecido la COVID-19 en el contexto social cubano. Objetivo: Analizar los principales síntomas psicológicos en personas que han padecido la COVID-19. Métodos: Se realizó un estudio cualitativo y cuantitativo, desde el 12 de enero hasta el 12 de junio de 2021, de 51 sujetos que habían padecido la COVID-19 en la ciudad de Santiago de Cuba. Resultados: Los síntomas más relevantes fueron el estrés, la ansiedad, el miedo y la frustración, que resultaron de la exposición a la enfermedad y generaron comportamientos que modificaron la vida cotidiana. Conclusiones: Los sujetos entrevistados presentaron diferentes síntomas psicológicos, los cuales modificaron la cotidianidad, reafirmaron la protección de la salud, aumentaron la percepción del riesgo ante la enfermedad y la preocupación por el futuro de sus vidas y su país.


Social isolation has had a negative impact on mental health; moreover, psychological symptoms have been evident in patients who have suffered from COVID-19 in Cuban social context. Objective: To analyze the main psychological symptoms in people who have suffered from COVID-19. Methods: A qualitative and quantitative study of 51 subjects who had suffered from COVID-19, from January 12 to June 12, 2021, in the city of Santiago de Cuba, was carried out. Results: The most relevant symptoms were stress, anxiety, fear and frustration; these have been the result of exposure to the disease and have generated behaviors that modify daily life. Conclusions: The interviewed subjects presented different psychological symptoms, which modified daily life, reaffirmed health protection, and increased the coronavirus risk perception and concern for the future of their lives and their country.

ABSTRACT

Neuropsychiatric manifestations of viral infections (both per se and secondary to the neuroinflammatory reaction of the host) are mainly attributed to immunological reactions, so many aspects of their pathogenesis are still nuclear. Some novel therapeutic strategies are progressively emerging in which a vaccination may be having a particular impact on recovery and reduction of death. In this context, it is accepted that the SARS-CoV-2 virus is profoundly neurotropic and neuroinvasive, with various effects on the nervous system, although there is no complete understanding of the mechanism of neuroinvasion, brain injury, or short- or long-term neuropsychiatric sequelae. Therefore, it is necessary to understand the post-infectious manifestations of COVID-19 to guide the management of neuropsychiatric diseases. Thus, based on different research groups focused on this field, in this manuscript we summarize papers on COVID-19 and the nervous system (NS) published in a series of articles by Cuban authors. This review focuses on cognitive and affective emotional states, pathogenesis, biomarkers, clinical manifestations, and intervention strategies.

Introducción: En Cuba, la lucha contra la COVID-19 ha sido tarea de todos los que están relacionados a la atención sanitaria, con su consagración continúa, exponiendo sus capacidades y habilidades profesionales en el tratamiento y atención especializadas a los pacientes hospitalizados. Objetivo: Valorar las principales vivencias del personal de salud que trabajó en las zonas rojas, en los centros de aislamiento con casos sospechosos y positivos a la COVID-19. Métodos: Estudio descriptivo. Para realizar el análisis de las vivencias se aplicaron las técnicas de entrevista y observación. Se trabajó con una muestra de 43 sujetos, de ellos se seleccionaron 17 médicos, 10 enfermeras, siete técnicos y nueve del personal de servicio que participaron en los centros de aislamiento. Resultados: Las vivencias del personal de salud que laboraron en las zonas rojas se basaron en la expresión de los sentimientos, emociones, relaciones interpersonales, percepción de riesgo y vivencias únicas durante la labor realizada. Los principales sentimientos y emociones fueron: alegría, cooperación, altruismo, apoyo, gratitud, admiración, compromiso y las negativas fueron: miedo, ansiedad, estrés, depresión, rechazo y desesperanza en menor medida. Las relaciones interpersonales se desarrollaron en colaboración y aprendizaje grupal, la percepción de riesgo fue alto por la carga viral de la enfermedad. Conclusiones: Las vivencias de los sujetos investigados se expresaron en manifestaciones negativas al principio del tratamiento con los pacientes y evolucionaron hasta la conversión a estados positivos que permitieron elevar la calidad del trabajo realizado(AU)


Introduction: In Cuba, the fight against COVID-19 has been a task for everyone related to healthcare, with their continuous consecration, showing their professional capacities and skills in the specialized treatment of and care for hospitalized patients. Objective: To assess the main experiences of the health personnel who worked in red zones, in isolation centers, with suspected or positive cases of COVID-19. Methods: A descriptive study was carried out. The interview and observation techniques were used to analyze the experiences. The sample consisted of 43 subjects, of whom 17 physicians, 10 nurses, 7 technicians and 9 service employees who participated in isolation centers were selected. Results: The experiences of the health personnel who worked in the red zones were based on the expression of feelings, emotions, interpersonal relationships, perception of risk, and unique experiences during the work performed. The main feelings and emotions were joy, cooperation, altruism, support, gratitude, admiration, commitment; while the negative ones were fear, anxiety, stress, depression, rejection, and hopelessness to a lesser extent. Interpersonal relationships were developed in collaboration and group learning, while the perception of risk was high due to the viral load of the disease. Conclusions: The experiences of the studied subjects were expressed through negative events at the beginning of involvement with patients, and evolved until conversion to positive states that allowed raising the quality of the work performed(AU)

ABSTRACT

Para afrontar la reciente emergencia global por COVID-19, la comunidad científica y los profesionales de la salud en Cuba trabajan constantemente en el desarrollo de nuevos tratamientos y tecnologías que posibiliten el diagnóstico precoz de esta enfermedad, todo ello para facilitar el manejo general de esta pandemia.1) De ahí que se requiere el perfeccionamiento continuo de este proceso en correspondencia con los principios de la medicina, con una formación humanista, científica y con una visión integral de los problemas de salud como elemento esencial que caracteriza la formación de profesionales de la salud en territorio cubano donde la especialidad de Medicina General Integral constituye la principal disciplina integradora de contenidos, para ofrecer una atención preventiva, sistemática y continua al paciente a lo largo del tiempo, donde el médico en formación sea expresión de sus cualidades humanas. Coherente con esto, en 1984 se inició en Cuba la formación de especialistas en Medicina General Integral (MGI). Esta especialidad no tenía antecedentes en el país, por lo cual su propuesta constituye el perfeccionamiento del enfoque social de la medicina, que desde su implantación es la premisa básica del sistema de salud cubano. ABSTRACT

Introducción: La ciencia mediante la innovación de los servicios y la tecnología brinda importantes beneficios en función de la sociedad. Objetivo: Proporcionar información sistematizada sobre el impacto social de la ciencia y la tecnología cubana en el enfrentamiento a la COVID-19. Métodos: Se realizó una búsqueda bibliográfica en las bases de datos Scopus, SciELO, ScienceDirect y MEDLINE/PubMed. Para ello, se utilizaron los descriptores o palabras relacionadas con la temática (ciencia, tecnología, COVID-19). Se consultaron artículos de revisión, de posición, y metaanálisis de los años 2020, 2021 y 2022, de la búsqueda solo 30 artículos cumplieron con los criterios de selección. Resultados: En la etapa pandémica, Cuba depositó toda la confianza en sus científicos y sacó provecho a la industria biotecnológica en la búsqueda de salvaguardar la población. Gracias a la satisfactoria gestión del gobierno se obtuvieron resultados positivos en las investigaciones, a partir de la inventiva de los productos como jusvinza, nasalferon, biomodulina T, entre otros. Se inventó un ventilador pulmonar asistido de alta gama para el tratamiento de los adultos, de este modo se fortaleció el trabajo interinstitucional e intersectorial. Conclusiones: La ciencia, la tecnología y las innovaciones han sido cruciales para el manejo de la crisis sanitaria generada por la COVID-19. En Cuba, se obtuvieron valiosos resultados en diferentes niveles para el beneficio de la sociedad; como las vacunas soberana y Abdala(AU)


Introduction: Both science and technology justify their existence through the innovation of services and technologies for the benefit of society. Objective: To provide systematized information on the social impact of Cuban science and technology in the fight against COVID-19. Methods: A bibliographic search was carried out in Scopus, SciELO, ScienceDirect and MEDLINE/PubMed databases. Descriptors or words related to the theme (science, technology, COVID-19) were used. Review articles, position articles, and meta-analyses from 2020, 2021, and 2022 were consulted. Only 30 articles met the selection criteria. Results: In the pandemic stage, Cuba placed all its trust in its scientists and took advantage of the biotechnology industry in the search for the safeguarding of the population. Thanks to the satisfactory management of the government, good results were obtained in the investigations with the inventiveness of products such as Jusvinza, nasalferon, biomodulin T, among others. A high-end assisted lung ventilator was invented for treating adults, in the same way that inter-institutional and intersectoral work was strengthened. Conclusions: Science, technology, and innovations have been crucial in managing the health crisis caused by COVID-19. In Cuba, relevant results have been obtained at different levels for the benefit of society, among which Soberana and Abdala vaccines stand out(AU)

Introduction: An indisputable consequence has to do with the psychological impact of the COVID-19 on health professionals and students. In Cuba, these psychological manifestations have scarcely been investigated, and there are no reports in the dental undergraduate program. Objective: To analyze the evidences of validity and reliability of a depression, anxiety and stress scale in Cuban dental students in the context of COVID-19. Methods: An instrumental and cross-sectional study was carried out, which included students of all years and Cuban universities. A documentary measurement instrument was validated, using its main psychometric evidence. Convergent validity was analyzed and reliability was estimated using Cronbach's alpha coefficient. Results: 301 students were included, of which 74.09 % were women and whose ages ranged from 17 to 30 years. The asymmetry and kurtosis values of all items were adequate. The confirmatory factorial analysis showed that the factorial structure of 21 items distributed in 3 factors is satisfactory. The goodness of fit indices was equally satisfactory. The scale factors are directly and significantly related with the Generalized Anxiety Disorder-2 (p<0.01) and the Patient Health Questionnaire-2 (p<0.01); the scale shows evidences of convergent validity. With Cronbach's α coefficient, the scale scores were estimated to be reliable. Conclusions: The depression, anxiety and stress scale (DASS-21) in Cuban dental students in the context of COVID-19 turned out to be valid and reliable, in addition to allowing the evaluation of the dimensions of these three factors in this sector of the society.


Introducción: La COVID-19 produce un innegable impacto psicológico en estudiantes y profesionales de la salud. En Cuba, estas manifestaciones psicológicas apenas se han estudiado, y no existen informes al respecto en el programa de pregrado que corresponde a estomatología. Objetivo: Analizar las evidencias sobre validez y confiablidad de una escala para medir depresión, ansiedad y estrés en estudiantes de estomatología cubanos en el contexto de la COVID-19. Métodos: Se realizó un estudio instrumental y trasversal con estudiantes de todos los cursos y universidades cubanas. Se validó un instrumento de medición de tipo documental mediante su principal evidencia psicométrica. Se analizó la validez convergente y se estimó la confiabilidad mediante el coeficiente alfa de Cronbach. Resultados: Se incluyeron 301 estudiantes, el 74,09 % de los cuales son mujeres y cuyas edades oscilan de diecisiete a treinta años. Los valores de la asimetría y la curtosis de todos los ítems fueron adecuados. El análisis factorial confirmatorio mostró que la estructura factorial de los 21 ítems distribuidos en tres factores es satisfactoria. La bondad de los índices de ajuste también fue satisfactoria. Los factores de la escala están directa y significativamente relacionados con la escala para medir el trastorno de ansiedad gefneralizas-2 (p<0,01) y con el cuestionario sobre salud del paciente-2. La escala muestra evidencias de validez convergente. El coeficiente alfa de Cronbach permitió estimar la confiablidad de las puntuaciones del instrumento. Conclusiones: La escala para medir depresión, ansiedad y estrés (DASS-21) aplicada a estudiantes de estomatología cubanos en el contexto de la COVID-19 resulto ser válida y confiable, además de permitir la evaluación de las dimensiones de estos tres factores en este sector de la sociedad.

To monitor the COVID-19 epidemic in Cuba, data on several epidemiological indicators have been collected on a daily basis for each municipality. Studying the spatio-temporal dynamics in these indicators, and how they behave similarly, can help us better understand how COVID-19 spread across Cuba. Therefore, spatio-temporal models can be used to analyze these indicators. Univariate spatio-temporal models have been thoroughly studied, but when interest lies in studying the association between multiple outcomes, a joint model that allows for association between the spatial and temporal patterns is necessary. The purpose of our study was to develop a multivariate spatio-temporal model to study the association between the weekly number of COVID-19 deaths and the weekly number of imported COVID-19 cases in Cuba during 2021. To allow for correlation between the spatial patterns, a multivariate conditional autoregressive prior (MCAR) was used. Correlation between the temporal patterns was taken into account by using two approaches; either a multivariate random walk prior was used or a multivariate conditional autoregressive prior (MCAR) was used. All models were fitted within a Bayesian framework.

ABSTRACT

La situación epidemiológica originada por la COVID-19 provocó nuevas demandas a las ya existentes en el personal de salud. Objetivos: Describir las dimensiones del síndrome de burnout: agotamiento emocional, despersonalización y falta de realización profesional en el personal del área de salud, determinar los niveles de estrés, explorar el ambiente emocional en sus dimensiones de afectividad positiva y negativa y delimitar la relación entre estas variables. Estudio cuantitativo con un diseño descriptivo de corte transversal, en un Policlínico durante el mes de diciembre del 2021. En personal médico (n=23) y de enfermería (n=16), se aplica el Inventario de burnout de Maslach, la Escala sintomática de estrés y la Escala de afectividad positiva y negativa. Resultados: Se obtiene agotamiento emocional alto en el 30,8%, medio en 33,3%; despersonalización, en un sujeto fue alto, y en 6 medio; la eficacia profesional en 9 sujetos fue medio y en 1 bajo; el nivel de estrés fue patológico en 41%, la afectividad positiva es alta en 53,8%, la negativa en el 5,1%. No existen diferencias respecto al sexo, ocupación y haber padecido o no COVID-19, se constata relación inversa entre la despersonalización y la edad. Conclusiones: La relación directa entre el estrés, agotamiento emocional y afectividad negativa son expresión de un desgaste producto de las respuestas fisiológicas y emocionales a situaciones mantenidas de movilización de recursos en el entorno laboral, lo cual se manifiesta en el grupo estudiado. Se recomiendan acciones dirigidas a intervenir de modo participativo y prevenir el ambiente emocional negativo(AU)


The epidemiological situation originated by COVID-19 caused new demands to those already existing in health personnel. Objectives: Describe the dimensions of the burnout syndrome, determine the levels of stress explore the emotional environment in its positive and negative affectivity dimensions and define the relationship between these variables. Quantitative study with crosssectional design, in a polyclinic during the month of December 2021. In medical (n=23) and nursing (n=16) staff the Scale of Positive and Negative Affect -PANAS- the Symptomatic Scales of Stress and the Maslach Burnout Inventory were applied. Measures of central tendency, absolutes and relative frequencies, measures of association and correlation coefficients with a reliability of 95% were used. Results: High emotional exhaustion is obtained in 30.8%, medium in 33.3%, depersonalization in one subject was high and in 6 medium, professional efficacy in 9 subjects was medium and in 1 low; the level of pathological stress in 41%, the positive affectivity is high in 53,8%, the negative in the 5.1%. There are no differences regarding sex, occupation and having suffered or not COVID-19. An inverse relationship between depersonalization and age is verified. Conclusions: The direct relationship between stress, emotional exhaustion and negative affectivity are expressions of a wear due to the physiological and emotional response to chronic situations of mobilization of resource at work. Actions aimed at intervening in a participatory way and preventing the negative emotional environment is recommended(AU)

Introducción: Diferentes estudios han propuesto la relación entre los grupos sanguíneos del sistema ABO y la susceptibilidad a la COVID-19 y su importancia en el curso de la enfermedad. Objetivo: Determinar la frecuencia fenotípica de grupos sanguíneos ABO y RhD en un grupo de pacientes con infección activa por SARS-CoV-2. Métodos: Se realizó un estudio descriptivo, transversal y caso control en el Hospital Docente Clínico Quirúrgico "Freyre Andrade" de La Habana, para determinar la frecuencia fenotípica ABO y RhD en un grupo de pacientes con enfermedad activa por la COVID-19. Se estudiaron 928 muestras de sangre de pacientes con diagnóstico de COVID-19 y 1050 muestras pertenecientes a individuos sanos como grupo control. El grupo sanguíneo ABO y RhD se determinó por método de aglutinación en tubo con sueros hemoclasificadores anti-A, anti-B, anti-A+B, yanti-D. Los resultados fueron expresados en frecuencias absolutas y relativas y se determinó la asociación del grupo sanguíneo con la gravedad de la enfermedad por medio de la prueba no paramétrica de χ2 con un nivel de significación de p ≤ 0,05. Resultados: Se observó predominio del fenotipo O (49,24 %) seguido del A (35,79 %), B (12,68 %) y AB (2,27 %) respectivamente. Los fenotipos O y B presentaron significación estadística para la ocurrencia de la COVID-19, con valores de p < 0,05. No se encontró significación estadística en cuanto al predominio de un fenotipo particular y la necesidad de cuidados intensivos. Conclusiones: La relación del grupo sanguíneo ABO en la infectividad por SARS-CoV-2 y la gravedad de la enfermedad por la COVID-19 requiere estudios adicionales ya que los actuales no son concluyentes.


Introduction: Different studies have proposed the relationship between the blood groups of ABO system and the susceptibility to COVID-19 and its importance in the course of the disease. Objective: To determine the phenotypic frequency of ABO and RhD blood groups in a group of patients with active SARS-CoV-2 infection. Methods: A descriptive, cross-sectional, case-control study was conducted to determine the ABO and RhD phenotypic frequency in a group of patients with COVID-19 active disease. Nine hundred twenty eight blood samples and 1050 samples belonging to healthy individuals as control group were studied. The ABO and RhD blood group was determined by the tube agglutination method with anti-A, anti-B, anti-A+B, and anti-D blood classifying sera. The results were expressed in absolute and relative frequencies and the association of the blood group with the severity of the disease was determined by the non-parametric χ2 test with a significance level of p ≤ 0.05. Results: A predominance of phenotype O (49.24%) was observed, followed by A (35.79%), B (12.68%) and AB (2.27%) respectively. The O and B phenotypes showed statistical significance for the occurrence of COVID-19, with p values < 0.05. No statistical significance was found regarding the prevalence of a particular phenotype and the need for intensive care. Conclusions: The relationship of the ABO blood group in the infectivity of SARS-CoV-2 and the severity of the disease by COVID-19 requires additional studies, since the current ones are not conclusive.

ABSTRACT

OBJECTIVE: To report the experience of technology-enabling organisation and deployment of a large-scale free online scientific event about COVID-19, and to share leadership lessons learnt. METHODS: The 'First Brazilian Congress of Clinical Evidence on COVID-19' took place between 3 May and 7 May 2021 and was hosted by the Universidade Federal de Minas Gerais, one of Brazil's top federal universities. Online platforms and a website were used for registration, as well as live transmission of the event, such as Zoom, YouTube and Even. A Situational Leadership framework was used to lead the team. Participants' satisfaction was evaluated through an online questionnaire. RESULTS: There were overall 27 000 registrations. The transmission reached over 97 100 views, from Brazil, Cuba, Mexico and the UK. Conference's topics included the whole COVID-19 'system of care'. Speakers and moderators from all over Brazil and abroad were chosen according to their expertise on COVID-19 and on evidence-based medicine. Video testimonies were presented between sessions from people who could not work from home about what touched their heart during the pandemic. Accessibility was ensured through simultaneous translation to Brazilian sign language. From 2228 respondents of the satisfaction assessment questionnaire, 97.4% reported their expectations to be exceeded and 86.8% reported acquiring new knowledge about COVID-19. CONCLUSION: This experience showed that leadership, teamwork, motivation and technology enabled the dissemination of accessible scientific evidence on COVID-19 to a large audience through a free online event. Lessons learnt may be useful for the post-pandemic, for new-waves, as well as recovery.

Los avances en las tecnologías de la información y la comunicación han posibilitado trazar estrategias para el reordenamiento del proceso docente-educativo en estos tiempos de crisis sanitaria por la pandemia de COVID-19, que ha limitado la presencialidad en las aulas. En la Universidad de Ciencias Médicas de Santiago de Cuba, la Universidad Virtual de Salud integra varias plataformas para la enseñanza-aprendizaje de pregrado y posgrado, tales como el aula y la clínica virtuales, el repositorio, el directorio de cursos y humanidades; sin embargo, se observa un insuficiente aprovechamiento de estas potencialidades. Por tal razón, en el presente artículo se enfatiza en la necesidad de incrementar el uso del portal integrador para la gestión de la información en la Universidad Virtual de Salud.


The advances in technologies of information and communication have facilitated to trace strategies for the reorganization of the teaching-educational process in these times of sanitary crisis due to the pandemic of COVID-19 that has limited the presence in classrooms. In the University of Medical Sciences in Santiago de Cuba, the Health Virtual University integrates several platforms for the predegree and postdegree teaching-learning, such as classroom and virtual clinic, repository, courses and humanities directory; however, an insufficient use of these potentialities is observed. For such a reason, in this work the necessity of increasing the use of the integrative portal for information management in Health Virtual University is emphasized.

Introducción: La COVID-19 representa un panorama particularmente estremecedor en los adultos mayores, debido a la alta morbilidad y mortalidad. Diseñar acciones para su atención integral constituye una prioridad de la salud pública en Cuba y un poderoso instrumento para la prevención de esta enfermedad. Objetivo: Diseñar acciones comunitarias e intersectoriales para la COVID-19 en el adulto mayor. Métodos: Se realizó un estudio cuasiexperimental de intervención mediante acciones comunitarias e intersectoriales para prevenir la COVID-19 en los adultos mayores pertenecientes al consultorio 16 del Policlínico Guillermo Tejas Silva, en el período de abril a diciembre del 2020. El universo estuvo constituido por 134 gerontes y la muestra de 110. Se desarrolló en tres etapas. Se evaluó el impacto de la intervención. Resultados: Predominaron las gerontes femeninas (50,9 por ciento), las edades de 60-64 años (32,2 por ciento) y los independientes activos (45,5 por ciento). Con la aplicación de la estrategia se aumentó el nivel de conocimiento de los seniles (91,8 por ciento), se controló el 100 por ciento de las enfermedades, se redujeron los trastornos emocionales en un 78,4 por ciento y se diseñaron 185 acciones comunitarias para prevenir la COVID-19 en el adulto mayor. Conclusiones: La sostenibilidad en el control de la COVID -19 en el adulto mayor requiere de la movilización y participación de todos los sectores de la sociedad y la economía, unidos en un esfuerzo conjunto que sustente las acciones comunitarias(AU)


Introduction: COVID-19 is a particularly frightening scenario for elderly adults, due to high morbidity and mortality. Designing actions for the comprehensive management of this disease is a public health priority in Cuba and a powerful instrument for preventing it. Objective: To design community and intersectoral actions for COVID-19 in elderly people. Methods: A quasiexperimental intervention study was carried out by means of community and intersectoral actions for preventing COVID-19 in elderly people from the family doctor's office 16 of Policlínico Guillermo Tejas Silva, in the period from April to December 2020. The study universe was made up of 134 elderly people and the sample was 110. It was developed in three stages. The impact of the intervention was evaluated. Results: Female elderly people were predominant (50.9 percent), together with ages 60-64 years (32.2 percent) and the condition of active independent (45.5 percent) with the implementation of the strategy, the level of knowledge of the senile increased (91.8 percent), 100 percentof the diseases were controlled, emotional disorders were reduced by 78.4 percent, and 185 community actions were designed to prevent COVID-19 in the elderly people. Conclusions: Sustainability in the control of COVID-19 in the elderly people requires the mobilization and involvement of all sectors of society and the economy, united in a joint effort that sustains community actions(AU)

Introducción: Se desconocen las características de la ciencia generada por la Universidad de Ciencias Médicas de La Habana, publicada en revistas científicas estudiantiles nacionales. Los estudios métricos constituyen herramientas idóneas para abordar problemáticas de esta naturaleza. Objetivo: Caracterizar la producción científica de la Universidad de Ciencias Médicas de La Habana en revistas estudiantiles cubanas. Métodos: Se realizó un estudio métrico, descriptivo y transversal en 121 artículos publicados en 12 revistas entre 2017 y 2021. Resultados: Los artículos alcanzaron 149 citas y 65,8 citas corregidas. Hubo un alto índice de Subramanyan (0,76) y un bajo índice de colaboración entre facultades (0,16). Se registraron 207 autores; 2 de ellos fueron líderes. Predominaron los artículos originales (38,8 por ciento). HolCien ostentó el mayor índice de Price (0,87), mientras que 16 de abril y Universidad Médica Pinareña lideraron, respectivamente, en cuanto a número de citas (71) y de citas corregidas (32,5). La Facultad Manuel Fajardo resultó la más productiva (n = 29); y la Facultad Calixto García, la más citada (54 citas). En 2020 se alcanzó el mayor número de publicaciones (n = 51) y de citas (88), probablemente como consecuencia de la COVID-19, que constituyó el principal foco temático emergente de investigación. Conclusiones: La producción se caracterizó por ser baja, mostrar una tendencia al ascenso cuantitativo, ostentar altos índices de coautoría y originalidad científica, y contar con autores líderes. Fue débil en cuanto a la colaboración entre facultades, la publicación en inglés y la actualización de las referencias bibliográficas (AU)


Introduction: The characteristics of the science produced by the University of Medical Sciences of Havana, published in national student scientific journals, are unknown. Metric studies are ideal tools for approaching problem situations of this nature. Objective: To characterize the scientific production of the University of Medical Sciences of Havana published in Cuban student journals. Methods: A metric, descriptive and cross-sectional study was carried out with 121 articles published in twelve journals between 2017 and 2021. Results: The articles reached 149 citations and 65.8 corrected citations. There was a high Subramanyan index (0.76) and a low index of collaboration between schools (0.16). There were 207 reported authors; two of them were leaders. Original articles predominated (38.8 percent). HolCien had the highest Price index (0.87), while the journals 16 de Abril and Universidad Médica Pinareña led, respectively, in number of citations (71) and corrected citations (32.5). The Manuel Fajardo Medical School was the most productive (n=29), while the Calixto García Medical School was the most cited (54 citations). In 2020, the highest number of publications (n=51) and citations (88) was reached, probably as a consequence of COVID-19, which became the main emerging thematic focus of research. Conclusions: The production was characterized by being low, showing a quantitative upward trend, as well as high rates of coauthorship and scientific originality, and by having leading authors. It was weak in terms of collaboration between schools, publication in English and updating of bibliographic references(AU)

ABSTRACT

Introduction: The purpose of this study was to determine the impact of confinement due to COVID-19 on glycemic control in children and adolescents with type 1 diabetes mellitus. Material and methods: An observational, descriptive study with a prospective design was carried out in Cuba, between November 2020 and May 2021. The sample consisted of 41 children and adolescents from a Provincial Center for Care and Education for Diabetic Patients. Anthropometric measurements (weight, height, and BMI) were obtained from the patients' medical records at the time of debut and at the time of the follow-up visit during the study period. Glycemic control was determined from the value of glycosylated hemoglobin (HbA1c). The comparison of the means between independent groups of variables was carried out using the ANOVA analysis of variance of a Kruskal-Wallis factor when quantitative variables were brought, and Pearson's Chi square in the case of categorical variables. Results: It was shown that during confinement, 78.05% of the patients had poor glycemic control. In addition, the requirements for higher total doses of insulin (p=0.005), both fast-acting (p=0.011) and slow-acting (p=0.008), and the highest dosage by weight in kilograms (p=0.003), and the highest evolution time (p=0.011) were significantly related to poor glycemic control. Most of the variables that were associated with poor glycemic control during confinement were also associated before said period. Conclusions: The confinement had a negative impact on the glycemic control of children and adolescents with type 1 diabetes mellitus in Cuba, since during this period the patients required higher doses of insulin and showed higher HbA1c figures.


Introducción: El propósito de este estudio fue determinar el impacto del confinamiento por COVID-19 en el control glucémico de niños y adolescentes con diabetes mellitus tipo 1. Material y métodos: Se realizó un estudio observacional, descriptivo con diseño prospectivo en Cuba, entre noviembre de 2020 y mayo de 2021. La muestra estuvo constituida por 41 niños y adolescentes de un Centro Provincial de Atención y Educación al Paciente Diabético. Las mediciones antropométricas (peso, talla e IMC) fueron obtenidas de las historias clínicas de los pacientes al momento del debut y al momento de la consulta de seguimiento durante el periodo del estudio. El control glucémico fue determinado a partir del valor de la hemoglobina glucosilada (HbA1c). La comparación de las medias entre grupos de variables independientes fue llevada a cabo mediante el análisis de varianza ANOVA de un factor de Kruskal-Wallis cuando se trataba de variables cuantitativas, y Chi cuadrado de Pearson en el caso de las variables categóricas. Resultados: Se observó que durante el confinamiento el 78,05 % de los pacientes presentaban un mal control glucémico. Además, los requerimientos de mayores dosis totales de insulina (p=0,005), tanto de acción rápida (p=0,011) como lenta (p=0,008), y la mayor dosificación por peso en kilogramos (p=0,003), y el mayor tiempo de evolución (p=0,011) se relacionaron significativamente con el mal control glucémico. La mayoría las variables que se asociaron a un mal control glucémico durante el confinamiento, también lo hacían antes de dicho período. Conclusiones: El confinamiento tuvo un impacto negativo sobre el control glucémico de niños y adolescentes con diabetes mellitus tipo 1 en Cuba, ya que durante este período los pacientes requirieron mayores dosis de insulina y mostraron mayores cifras de HbA1c.

O controle glicêmico foi determinado a partir do valor da hemoglobina glicosilada (HbA1c). A comparação das médias entre grupos de variáveis ​​independentes foi realizada por meio da análise de variância ANOVA de um fator de Kruskal-Wallis no caso de variáveis ​​quantitativas e do Qui-quadrado de Pearson no caso de variáveis ​​categóricas. Resultados: Observou-se que durante o confinamento, 78,05% dos pacientes apresentaram mau controle glicêmico. Além disso, as necessidades de maiores doses totais de insulina (p=0,005), tanto de ação rápida (p=0,011) quanto de ação lenta (p=0,008), e a maior dosagem por peso em quilogramas (p=0,003), e o maior tempo de evolução (p=0,011) relacionaram-se significativamente com o mau controle glicêmico. A maioria das variáveis ​​que se associaram ao mau controle glicêmico durante o confinamento também se associaram antes desse período. ABSTRACT

Introduction: Covid-19 pandemic, threatened health worldwide, and overhelmed systemshealthcare in many countries. Performing and adequate identification of subjects with verylow risk to develop a severe form of the disease must result very cost-effective.Objective: To identify patients with very low risk of transition to severity in subjectsconfirmed with Covid-19 in Cuba.Methods: An environmental study was carried out based on the Cuban National Registerof confirmed Subjects covid-19. Between march 11th, 2020 and may 31st 2021, 10600persons were recorded. Admission in ICU and mortality related to covid-19, both measured30 days after disease confirmation, were the variables analyzed. As predictors, age and comorbidities were chosen. A classification tree ­ algorithm was applied to identify risk strata. To every stratum, its volume, the risk of transition to severity and the explained risk were calculated.Results: Subjects less than 65 years with no comorbidities (85,8% of the total) had a verylow risk (0,2%) of admission in ICU and death. Adjusted models showed a good calibration and discriminant ability.Conclusions: The study allowed the identification of subjects with very low risk of transition to severity. They could be followed by primary health care units with home surveillance and symptomatic treatment. The approaches used, permit the improvement of assigning admissions to different health care units' optimizing limited health resources.


Introducción: La pandemia de covid-19, amenaza para la salud mundial, desbordó los sistemas de salud en muchos países. Realizar una adecuada clasificación del riesgo de transmisión resulta de vital importancia. Objetivo: Identificar pacientes con muy bajo riesgo de transición a la severidad en pacientes  confirmados de covid-19 en Cuba. Métodos: Se realizó un estudio ecológico  que tuvo como base el registro nacional de pacientes confirmados de covid-19.  Se analizaron  10600 individuos entre el 11 de marzo de 2020 y el 31 de mayo de 2021. Las variables de respuesta seleccionadas fueron admisión en UCI y mortalidad, ambas medidas a los 30 días de la confirmación de la enfermedad. Los predictores fueron la edad  y la presencia de comorbilidades.  Se aplicó  un algoritmo de árboles de clasificación para identificar estratos de riego. Se determinó para cada estrato, su volumen, el riesgo de transición a la severidad y el riesgo explicado. Resultados: Se comprobó que los pacientes con  edad menor de 65 años y sin comorbilidades, que concentraba el 86,8% de la muestra, tenían muy bajo riesgo (0,2%)  de  admisión en UCI y de muerte. Los modelos demostraron una buena calibración y discriminación. Conclusiones: El estudio permitió identificar individuos con muy bajo riesgo de transición a la severidad seguidos desde la atención primaria de salud con  ingreso en el hogar y tratamiento sintomático. La metodología propuesta basado en la evidencia permitió perfeccionar los procedimientos de derivación hacia los distintos escenarios  y priorizar recursos de salud limitados. 


Introdução: A pandemia de covid-19, uma ameaça global à saúde, sobrecarregou os sistemas de saúde de muitos países. A realização de uma classificação adequada do risco de transmissão é de vital importância. Objetivo: Identificar pacientes com baixíssimo risco de transição para gravidade em pacientes confirmados com covid-19 em Cuba. Métodos: Foi realizado um estudo ecológico com base no registro nacional de pacientes confirmados com covid-19. Foram analisados ​​10.600 indivíduos entre 11 de março de 2020 e 31 de maio de 2021. As variáveis ​​de resposta selecionadas foram admissão na UTI e mortalidade, ambas medidas 30 dias após a confirmação da doença. Os preditores foram idade e presença de comorbidades. Um algoritmo de árvore de classificação foi aplicado para identificar os estratos de irrigação. Foi determinado para cada estrato, o seu volume, o risco de transição para gravidade e o risco explicado.Resultados: Verificou-se que os pacientes com menos de 65 anos e sem comorbidades, que perfizeram 86,8% da amostra, apresentaram risco muito baixo (0,2%) de internação em UTI e óbito. Os modelos demonstraram boa calibração e discriminação.Conclusões: O estudo permitiu identificar indivíduos com baixíssimo risco de transição para a gravidade seguidos de cuidados de saúde primários com internamento domiciliário e tratamento sintomático. A metodologia baseada em evidências proposta permitiu refinar os procedimentos de encaminhamento para os diferentes cenários e priorizar os recursos de saúde limitados.
