RESUMO
O Programa de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB) busca induzir a ampliação do acesso e a melhoria da qualidade da atenção básica, com garantia de um padrão de qualidade permitindo maior transparência e efetividade das ações governamentais direcionadas à Atenção Básica em Saúde. O objetivo deste estudo foi descrever a estrutura e as condições de funcionamento dos serviços de saúde bucal na Atenção Primária no Brasil, utilizando dados do 2º ciclo do PMAQ-AB. Trata-se de um estudo descritivo, transversal, em que foram avaliados 16.202 serviços de saúde bucal, em relação à presença e a quantidade de 22 equipamentos odontológicos e 25 insumos. A presença de cada equipamento e insumo gerou ao serviço um escore, sendo o escore final a soma destes. Os escores foram comparados entre as regiões brasileiras. Os resultados foram analisados de forma descritiva utilizando o programa SPSS v. 25. Os dados quantitativos foram descritos por meio dos quartis e os categóricos por meio de frequência. A maioria das ESB apresentou equipamentos/insumos essenciais como cadeira odontológica, caneta de alta e baixa rotação, fotopolimerizador, amalgamador, equipamentos de proteção individual, rolo de algodão, gaze, flúor, ácido/adesivo, resina, anestésico com vasoconstritor, lâmina de bisturi, fio de sutura, brocas e material restaurador temporário. Houve deficiência de aparelhos como de raios X, jato de bicarbonato, ultrassom e insumos como amálgama (cápsula e preparo manual), anestésico sem vasoconstritor e medicação intracanal. Os serviços apresentaram uma mediana de 14 equipamentos e 22 insumos, as maiores medianas foram observadas na região Sul (16 e 23, respectivamente) e as menores medianas nas regiões Norte e Nordeste (14 e 21, respectivamente). De maneira geral, os serviços de saúde bucal apresentaram consultórios odontológicos em boas condições de disponibilidade de equipamentos e insumos para a realização de atividades clínicas. No entanto, ainda existem dificuldades de disponibilidade de equipamentos e insumos nas regiões Norte e Nordeste do Brasil.
ABSTRACT
The National Programme for Improving Access and Quality of Primary Care (PMAQAB in portuguese) seeks to induce the expansion of access and improvement of the quality of basic care, with a guarantee of a quality standard allowing greater transparency and effectiveness of the governmental actions directed to Basic Health Care. The aim of this study was to describe the structure of oral health services in primary health care in Brazil. This cross-sectional study evaluated 16,202 oral health services, adhering to the second cycle of the 'National Program for Improving Access and Quality of Primary Care' (PMAQ-AB), regarding 22 pieces of dental equipment and 25 dental supplies. The presence of each piece of equipment/dental supply generated a score for services. The sum of these scores was compared among different regions in Brazil. Quantitative data were described by quartiles and categorical data by frequencies, using the SPSS, version 25. Most evaluated services presented adequate equipment in use conditions and sufficient dental supplies for primary oral health care. Data also showed a lack of some equipment, such as X-rays, bicarbonate jet, and ultrasound devices, while dental supplies lacked amalgam (capsule and manual preparation), anesthesia without vasoconstrictors, and intracanal medication. The services presented a median of 14 pieces of equipment and 22 dental supplies. Of the Brazilian regions, the South presented the largest median, while the North and Northeast regions presented the smallest. The oral health services presented dental offices in good conditions of equipment/dental supply availability to perform clinical activities. However, differences in the structure of services between the Brazilian regions are still observed.
