RESUMEN

Paciente de 64 años, raza blanca, femenina, comenzó 3 meses antes de su ingreso con dolor lumboabdominal izquierdo, acompañado de incremento progresivo de peso, aumento marcado del apetito, flushing facial, hipertensión arterial de debut e hirsutismo de aparición más tardía. Por todo esto es valorada por su médico de asistencia y se le indica Ultrasonido abdominal, constatándose imagen ultrasonográfica compatible con Tumor de suprarrenal izquierda, se valora por Cirugía indicándose Tomografía de abdomen que confirma el tumor, el cual por su tamaño era tributario de tratamiento quirúrgico. La intervención se realizó de inicio por Cirugía videolaparoscópica, constatándose una masa tumoral suprararrenal de 12 cm adherida a órganos vecinos, motivo por lo cual se convierte a Cirugía abierta, siendo necesaria además la esplenectomía. Postoperatorio de 72 horas con evolución poco favorable, síndrome general asociado, hipopotasemia, dolor en sitio de intervención quirúrgica, se realiza nueva Cirugía videolaparoscópica sin encontrar complicaciones locales, se decide entonces su traslado a Unidad de Cuidados Intermedios, valorándose la posibilidad de que su cuadro clínico fuera secundario a una deprivación marcada de glucocorticoides endógenos por tumor hiperfuncionante de corteza. Se inicia desde su ingreso tratamiento sustitutivo con Hidrocortisona 300 mg/día, logrando mejoría clínica evidente en 72 horas. A los 6 días de ingreso se traslada a Cirugía y después es dada de alta hospitalaria...(AU)

ABSTRACT

64-years-old patient, white, female patient, refers the beginning of left lumbar abdominal pain three months before entering the hospital, accompanied with a progressive weight increase, a remarked appetite increase, facial flushing, debuting arterial hypertension and, later, hirsuteness. For that reason she assisted to her physician and he indicated an abdominal ultrasound, finding an ultrasonographic image compatible with left suprarenal tumor. The surgeon indicated an abdominal computed tomography confirming the tumor, tributary of surgery treatment because of its size. The surgery began as a laparoscopy, but they found a 12 cm suprarenal tumor mass, adhered to near organs, and it became an open surgery, and the splenectomy was also necessary. The postsurgery period lasted 72 hours with a few favorable evolution, associated general syndrome, hypopotasemy, pain in the place of surgical intervention. We made a new videolaparoscopic surgery without finding local complications. We decided to transfer the patient to the Intermediate Care Unit, valuing the possibility of her clinical status being secondary to a remarked deprivation of endogenous glucocorticoids for hyper functioning tumor of the cortex. From the moment she entered we began a substitutive treatment with Hydrocortisone 300 mg/day, achieving an evident clinical improvement in 72 hours. After 6 days in the Intermediate Care Unit, she was transferred to the Surgery Unit and discharged from the hospital...(AU)
