A controvérsia do parto domiciliar / The controversy of home childbirth

RESUMO

Esse estudo teve por objetivo avaliar através de pesquisa bibliográfica se a polêmica do parto domiciliar poderia ser esclarecida. Foi realizada pesquisa bibliográfica com o descritor em saúde “parto domiciliar” identificado em http//www.bireme.br, Terminologia em Saúde com seu equivalente “home childbirth”. Foram consultadas as bases de dados LILACS, SciELO e Biblioteca Cochrane através da Bireme e o PubMed através da Biblioteca National em Saúde (http//www.nl.nih.gov). A pesquisa identificou vasta bibliografia, que foi selecionada de acordo com a possibilidade de responder à questão, tendo sido uma revisão seletiva e não exaustiva. Foi conduzida no sentido de tentar esclarecer a polêmica respondendo a questões fundamentais, considerando o parto como evento biopsicossocial e não apenas médico. As questões consideradas foram a segurança da mãe e do feto, a liberdade de escolha por parte da paciente do local do parto, a satisfação da paciente com a forma de atendimento, o sistema de saúde para a assistência obstétrica e o seu custo.O estudo não permitiu esclarecer a polêmica do parto domiciliar em todo o mundo e o Brasil ainda não tem uma solução ideal para o local do parto para toda a população. Acreditamos que não temos, no momento, condições adequadas para realização do parto domiciliar.

ABSTRACT

This study aimed to evaluate through bibliographical research if the controversy of home childbirth could be clarified. It was performed a bibliographical research on health with the descriptor “childbirth” identified in http//www.bireme.br, Health Terminology, with its equivalent “home childbirth”. Were consulted databases the Cochrane Library, SciELO and LILACS by Bireme and PubMed through the National Library of Medicine (http//www.nlm.nih.gov). The survey identified vast bibliography, which was selected in accordance with the possibility to answer the question, having been a selective and non-exhaustive review. It was conducted in an attempt to clarify the key issues responding to controversy, considering childbirth as a biopsychosocial event and not just as a doctor one. The issues considered were the safety of the mother and the fetus, the freedom of choice on the part of local delivery by the patient, patient satisfaction with the way the care facilities, the obstetrical health system and its cost. The study could not clarify the question of the place of birth all over the world and Brazil has not the ideal obstetrical health system for its full population. We believe that today we have not the ideal conditions for home childbirth.
