El inventario: Un instrumento de evaluación formativa de actitudes de los alumnos hacia los pacientes. Experiencia con los alumnos de ciclo inicial / The inventory: An instrument of formative evaluation of students' attitudes towards patients. Experience with students in the initial cycle

RESUMEN

Introducción: La educación médica es una experiencia de socialización profesional que comprende no solamente la adquisición de conocimientos y habilidades, sino también, y quizás como elemento más trascendente el desarrollo de actitudes, valores y un sentido de ética. Se presenta la experiencia con el uso del Inventario para la evaluación formativa de actitudes con alumnos de primer año de medicina. Materiales y Métodos: Inventario instrumento con veinte ítems que representan situaciones de relación médico paciente con cinco opciones, clasificadas como «centrados en el paciente¼ (CP) y «no centrados en el paciente¼ (NCP). La actividad se realizó con alumnos del primer año y tuvo dos momentos contestar el cuestionario en forma individual indicando sexo, edad y año de inscripción y discusión grupal de las respuestas. Para el análisis estadístico se utilizó el programa Statistical Package for the Social Sciences (SPSS). Resultados: se obtuvieron 132 registros de alumnos de primer año y se descartaron 6 por datos insuficientes. Femenino 72 (58%) y masculino 52 (42%). La elección de CP o NCP no dependió del sexo y de la edad. En 12 casos, de los 20 ítems el porcentaje de respuestas CP fueron mayor del 80 %. En sólo 2 casos, fueron menores del 40%. Las alternativas seleccionadas por menos de 5% de los alumnos fueron todas NCP. Discusión: la finalidad del uso de este inventario está orientada a la evaluación formativa de los estudiantes en cuanto a sus actitudes hacia los pacientes. El diseño de actividades para reflexionar acerca de las actitudes de los estudiantes puede ser una de las opciones para el desarrollo de las mismas a lo largo del programa.

ABSTRACT

Introduction: Medical education is a professional socialization experience that entails not only the acquisition of knowledge and skills but also, and may be most important, the development of attitudes, values, and an ethical sense. In this paper, experience with the use of the Inventory for a formative evaluation of attitudes in first year medical students is presented. Materials and Methods: The Inventory is an instrument that consists in twenty (20) items representing situations in a doctor-patient relationship and giving five options, classified as «patient centered¼ (PC) and «not patient centered¼ (NPC). The activity was performed with first year students and had two stages First, the students answered the questions individually indicating sex, age, and enrolment year and, second, there was a group discussion of the answers. For the statistical analysis of the results the program SPSS was used. Results: 132 records from first year students were obtained and only 6 were discarded because they contained insufficient data. In the study, there were 72 (58%) female students and 52 (42%) male students. The choice of PC or NPC neither depended on sex nor age. In 12 of the 20 items the percentage of PC answers was higher than 80%. Only in two it was lower than 40%. The alternatives chosen by less than 5% of the students were all NPC. Discussion: The aim of using this inventory is oriented towards the formative evaluation of stu- dents regarding their attitudes towards patients. The design of activities to think about students' attitudes may be one of the options for the development of such attitudes throughout the program.
