La oxigenación hiperbárica como tratamiento del infarto agudo del miocardio, en pacientes no tributarios del uso de estreptoquinasa recombinante / Hyperbaric oxygenation in the treatment of acute myocardial infarction in patients not tax the use of recombinant streptokinase

RESUMEN

En un estudio con 50 pacientes que presentaban Infarto Agudo del Miocardio (IMA) y contraindicación para emplear Estreptoquinasa Recombinante, se les administró Nitropental y Acido Acetil Salicílico ( ASA). A 25 de ellos se les aplicó tratamiento con Oxigenación Hiperbárica (OHB), y se mantuvo el resto de la medicación para este tipo de afección y de las enfermedades concomitantes. Se utilizó la cámara hiperbárica PDK-2U administrándose Oxígeno Hiperbárico (ATA) por un período de 60 minutos cada 12 horas durante el primer día, luego una sesión diaria hasta completar 10 días de tratamiento. Se mantuvo el paciente con Monitoreo Cardíaco Continuo (MCC), se le realizó Electrocardiograma (ECG) al concluir cada sesión, ecocardiograma simple antes del egreso hospitalario y a los 90 días; con esta periodicidad se efectuó la ergometrìa de esfuerzo, inicialmente submáxima, se incorporó al Plan de Rehabilitación Cardíaca Integral (RCI), finalmente se realizó la ergometría de esfuerzo máxima. Predominio del sexo masculino, edad promedio 59 años, topografía diafragmática y septal, dolor con localización precordial. Se encontró que la enzima Creatinin Fosfoquinasa (CPK) mostró un rápido ascenso a las 24 horas con regresión brusca a las 48 horas de iniciado el tratamiento, la enzima Transaminasa Glutámico Oxalacética (TGO) triplicó el nivel basal a las 72 horas del inicio, el supradesnivel del segmento S-T disminuyó el 39 por ciento del inicial y se normalizó en el 40 por ciento de los casos después de aplicada la primera sesión de tratamiento; con carga máxima el doble producto varió 3 y media veces lo alcanzado por el tratamiento sin OHB, mientras que el Consumo de Oxígeno (VO2) lo duplicaba. Puede decirse que la OHB evitó una mortalidad del 16 por ciento de los casos y un 8 por ciento de reingreso(AU)

In a study with 50 patients that Sharp Infarto of the Miocardio presented (IMA) and contraindication to use Estreptoquinasa Recombinante, was administered Nitropental and Sour Acetil Salicilico (it ROASTS).
