Inserção do programa de saúde da família na prevenção da doença renal crônica / The insertion of family health care program in the prevention of chronic kidney disease

RESUMO

O Programa de Saúde da Família (PSF) tornou-se o modelo assistencial da Atenção Primária à Saúde no Brasil e os profissionais que compõem as equipessão responsáveis pelo primeiro contato com os pacientes com a Doença Renal Crônica (DRC). O presente artigo propõe uma estratégia de inserção doPSF na prevenção da DRC com base nos pressupostos da integralidade, adscrição da população, atribuições da equipe e enfoque preventivo que norteiamo programa. Ressalta a importância da continuidade dos cuidados e propõe uma reflexão sobre os benefícios da atenção compartilhada entre o PSF e osnefrologistas para que a qualidade da proposta seja alcançada.

ABSTRACT

The Family Health Care Program (FHC) has became the main model of Primary Care to the Brazilian people, and the profissionals health who composeit are usually teams are usually those who have the first contact with patients with chronic kidney disease (CKD). In this review, the authors propose astrategy of insertion of FHC in the prevention of CKD, based on the premise of integrality, description of the population, attributions of the team and the preventive approach that guide the Program. It is highlighted the importance of the continuity of care, and it is proposed a reflection about the benefits of the shared-care model in which the FHC team and the nephrologists work together in order to optimize the quality of care to patients with CKD.
