Uso de medicamentos em idosos que residem em uma instituição de longa permanência / Use of medication in elderly residing in a long-stay institution

RESUMO

A demanda pela institucionalização vem crescendo e o perfil do idoso que procura o asilo no Brasil vem se modificando, sendo necessário saber sobre o cenário atual. Assim, questionamos de que maneira é realizada a administração de medicamentos a idosos nas instituições de longa permanência? A metodologia consistiu em entrevistas com idosos que residem em um asilo situado em Salvador-Bahia, aliada à observação sistemática neste local. A análise do material empírico se deu a partir das categorias fundamentadas nas falas dos idosos com ênfase na temática das medicações, a qual constitui o interesse central deste estudo. Os resultados indicam que a administração direta da medicação usada pelos idosos é uma tarefa realizada pelos cuidadores. No entanto, devido às possibilidades de ocorrer trocas relacionadas à dosagem da medicação e/ou ao idoso que deveria recebê-la, verificou-se que a instituição ainda deveria avançar para planejar e programar melhor a administração dos medicamentos, superando improvisações e atuando de modo a não causar efeitos iatrogênicos. Concluímos ressaltando a importância de que nas instituições de longa permanência (ILP) sejam realizados estudos sobre a eficácia de medicamentos em idosos e que a capacitação do corpo técnico no que toca à administração da medicação seja continuada, evitando os erros e os abusos terapêuticos observados pelas pesquisadoras e percebidos pelos próprios idosos.(AU)

ABSTRACT

The present paper is a descriptive, exploratory research, of a qualitative approach in which interviews were performed between the months of August and October 2007, with six elderly people, residing in a long-stay institution located in Salvador - Bahia, allied to observation during a one-year period at this location. The research was evaluated by the Ethics Committee of the Collective Health Institute of the Federal University of Bahia (UFBA), report nr. 026-/07 CEP-ISC approved on 07/12/2007 and aimed check is performed as the administration of drugs in a long-stay institution for the elderly. The interpretation of the empirical material occurred from the categories based on the speech of the elderly with emphasis to the matter of medications, which forms the central interest of this study. The results indicate that direct administration of the medication used by the elderly is a duty performed by the carers. Nevertheless, due to the possibilities of occurring errors related to dosage of the medication and/or to the patient receiving the medication, the necessity for the institution to advance in order to better program the administration of the medication was verified, overcoming improvisations and practicing in a manner such as not to cause iatrogenic effects. It is primordial that studies be performed in the researched long-stay institution in relation to the efficiency of the medication to the elderly and continuous qualification of the collaborators in the case of administration of medication, avoiding errors and the therapeutic abuses observed by the researchers and perceived by the elderly patients themselves.(AU)
