Hyperglycemia and neurological outcome in patients with closed head injury

ABSTRACT
Data were collected from 10 patients admitted to the general intensive care unit and neurosurgical department in Assiut University Hospital over two years with severe closed head injury [GCS < 8]. On admission, each patient was evaluated using GCS. Blood sample was taken to determine serum glucose level, and CT scan results were recorded. Follow up of patients was done for 3-6 months and the final neurological outcome was assessed using the Glascow Outcome Scale. The results showed patients with normal blood sugar on admission have a favorable outcome than those with hyperglycemia. No significant correlation was found between the admission blood glucose and coma scale or CT scan findings. It is concluded that hyperglycemia should be avoided during the early management of patients with head injury and in those who are at risk for significant cerebral ischemia
