Influencia de la nueva membrana Helixone® en la valoración nutricional de enfermería / Influence of the new helixone® membrane in the nutritional assessment of nursing

RESUMEN
Recientemente se ha descrito un nuevo síndrome, el MIA (malnutrición, inflamación y arteriosclerosis), donde la etiología de la desnutrición e inflamación es multifactorial, pudiendo influir negativamente el acumulo de toxinas urémicas de alto peso molecular. Objetivo principal describir y comparar el estado nutricional de los pacientes cuando utilizan la polisulfona® de baja permeabilidad, versus a la nueva helixone® de alta permeabilidad. La muestra de estudio la constituyeron 25 pacientes. Se registraron marcadores nutricionales antropométricos, bioquímico se inflamatorios, otros parámetros clínicos y otros datos. Se compararon los resultados nutricionales de los pacientes cuando usaban la polisulfona® en el año 2.001 versusa los resultados valorados en estos mismos pacientes en el año 2.003, cuando usaban la helixone® durante al menos12 meses. Los resultados indican a) El índice de masa corporal y la circunferencia braquial descendieron mínimamente, y el pliegue cutáneo subescapular lo hizo de forma significativa(p=0000), aunque sin validez estadística; b) La albúmina (p=0000) y la prealbúmina (p=0031) aumentaron significativamente y c) La proteína C reactiva ascendió muy discretamente y la β2-microglobulina descendió de forma muy significativa (p=0009).En el tiempo que ha durado nuestro estudio, la nueva membranahelixone® no ha demostrado una mejora del estado nutricional antropométrico, frente a la polisulfona®, en cambio, parece que influye positivamente sobre el estado nutricional proteico bioquímico, posiblemente justificado por la mejora del estado urémico al aumentar la eliminación de toxinas de alto y medio peso molecular
ABSTRACT
Recently a new syndrome has been described, MIA (malnutrition, inflammation and atherosclerosis), where the etiologyof malnutrition and inflammation is multifactorial, and inwhich the accumulation of uremic toxins with a high molecular weight may have a negative influence. Main aim:
to describe and compare the nutritional state of patients when they use low flux polysulphone® compared to new high fluxhelixone®.The study sample was made up of 25 patients. The following were recorded anthropometrical, biochemical and inflammatory nutritional markers, other clinical parameters and other data. The nutritional results of patients when they used polysulphone® in 2001 were compared with the results obtained in the same patients in 2003, when they used helixone® for at least 12 months. The results show a) The body mass index and brachial circumference fell minimally, and the subscapular skinfold decreased significantly (p=0.000), although without statistical validity; b) Albumin (p=0.000) and pre-albumin (p=0.031)increased significantly and c) C reactive protein increased very discreetly and β2-microglobulin fell very significantly(p=0.009).Throughout the time our study lasted, the new helixone®membrane has not shown an improvement in the anthropometrical nutritional state, compared to polysulphone®,while it does appear to have a positive influence on the biochemical proteic nutritional state, possibly because of theim provement in the uremic state by increasing the elimination of toxins with a high and medium molecular weight
