Telaprevir en el paciente monoinfectado con fracaso del tratamiento anterior / Telaprevir in HCV-monoinfected patients with failure to prior treatment

RESUMEN

La introducción de telaprevir en el armamentario terapéutico de la hepatitis C ha significado un avance muy importante, también para pacientes no respondedores a un tratamiento previo con interferón y ribavirina. En este grupo de pacientes cabe descatar que la respuesta viral sostenida supera el 80% en los pacientes que presentaron una recaída y reciben triple terapia. En los pacientes respondedores parciales, más de la mitad va a conseguir curar la infección con la combinación de interferón pegilado, ribavirina y telaprevir. Desgraciadamente, los resultados no son tan buenos en pacientes respondedores nulos (null responders), especialmente si tenemos en cuenta que muchos de ellos son ya pacientes de cierta edad y con una fibrosis avanzada. Es en este grupo donde debe realizarse una selección minuciosa de los mejores candidatos a beneficiarse del tratamiento triple

ABSTRACT

The introduction of telaprevir in the therapeutic armamentarium for hepatitis C virus infection is a major advance, even in patients unresponsive to prior treatment with interferon and ribavirin. In this group of patients, sustained viral response is achieved in more than 80% of those with recurrence and who received triple therapy. In partial responders, more than half achieve cure with the triple combination of pegylated interferon, ribavirin and telaprevir. Unfortunately, the results are less good in null responders, especially given that many are of a certain age and have advanced fibrosis. Patients in this group should be carefully selected to identify those most likely to derive a benefit from triple therapy
