RESUMEN
El dolor es uno de los síntomas más frecuentes que refieren los pacientes pediátricos en la consulta ortopédica, ya sea producto de traumatismos al realizar actividades propias de la edad, por patologías osteoarticulares, infecciones o por enfermedades de otros órganos y sistemas como trombofilia, linfomas, leucemia, talasemia, elevación plasmática de tiroxina libre y triyodotironina entre las más frecuentes, en las cuales se menosprecia el dolor óseo confinando su manejo a aliviar el dolor y no a reconocer su causa; de esta manera la enfermedad evoluciona y para el momento de su diagnóstico se encuentran en etapas irreversibles donde la limitación funcional y secuelas son permanentes pudiendo haber sido evitadas si se hubiera hecho un diagnóstico y manejo precoz. La Enfermedad de Legg-Calvé-Perthes debe ser sospechada en todo paciente pediátrico entre tres a nueve años que consulta por cojera, dolor de cadera, de ingle o rodilla unilateral; siendo, incluso, su primera radiografía normal debe ser valorado periódicamente ya que el dejar pasar por alto estos signos y síntomas en etapas incipientes se expone al paciente a secuelas que pueden comprometer su desarrollo sicomotor y la necesidad de implantar una prótesis de cadera, en la edad adulta.
ABSTRACT
Pain is one of the most frequent symptoms pediatric patients complain of in Orthopedics consultation, either as a result of traumatism doing activities age suited, osteoarticular pathologies, infections, or other organs or systems illnesses such as thrombophilia, lynphomas, leukemia, free thyroxine and triiodothyronine plasmatic increasing, among the most frequent. In all of those bony pain is underestimated limiting its handling to relieve pain, not recognizing its cause; this is why the disease continues and for when it is diagnosed it is in an irreversible stage in which functional limitations and sequels are permanent, when it was possible to avoid them provided diagnosis and handling would have been done earlier. Legg-Calvé-Perthes disease must be suspected in all pediatric patients whose age be between 3 and 9 years old consulting by lameness, one sided pain in the hip, groin or knee, even if his (her) first x-ray is normal, the patient should be evaluated on a regular basis, since overlooking these signs and symptoms in the initial stages is to expose the patient to sequels that will affect his (her) psycho-motor development and make necessary the use of a hip prosthesis when adult.
