Plastia escrotal en paciente con fascitis necrotizante de escroto: presentación de un caso / Scrotoplasty in a patient with necrotizing fasciitis of scrotum: a case presentation

RESUMEN

Fundamento la gangrena de Fournier está caracterizada por fascitis necrótica del periné y la pared abdominal junto con el escroto y el pene en los varones y la vulva en las mujeres. La pérdida de piel puede ser muy incapacitante y de difícil resolución. Se han usado varias técnicas para reconstruir la pérdida de tejidos. Objetivo: mostrar una técnica quirúrgica simple con posibilidad de utilizar tejido sobrante con resultados funcionales y estéticos satisfactorios. Caso clínico paciente masculino de 62 años de edad, de raza mestiza, que acude al cuerpo de guardia por presentar una fascitis necrotizante de escroto. Es ingresado en el Servicio de Urología del Hospital Universitario Celia Sánchez Manduley con tratamiento analgésico, antinflamatorio y antibioticoterapia que evolucionó hacia la necrosis de la piel del escroto. Se observó un paciente muy ansioso y preocupado por la apariencia de sus genitales. Se pudo apreciar que la piel del escroto que cubría el testículo derecho estaba casi intacta, además de que tiene la característica de ser bastante elástica, se le realizó la disección de la misma hasta que se obtuvo suficiente piel para cubrir el testículo izquierdo. Conclusiones: se logró un resultado estético y funcional adecuado, con la técnica aplicada se logró la curación del mismo con un mínimo de secuelas estéticas.

ABSTRACT

Background: Fournier gangrene is characterized by necrotizing fasciitis of perineum and of abdominal wall, as well as by necrotizing fasciitis of scrotum and penis in men and of vulva in women. The loss of skin can be disabling and hard to resolve. Some techniques have been used to reconstruct the loss of tissue. Objective: to show a simple surgical technique that gives the possibility of using redundant tissue with satisfactory functional and aesthetic results. Clinical case a sixty-two-year-old male patient of mixed race that comes to the emergency service with necrotizing fasciitis of scrotum. He is admitted in the Urology Service of the Celia Sanchez Manduley University Hospital and undergoes an analgesic, anti-inflammatory treatment and antibiotic therapy that evolved to the necrosis of the scrotum. It was noticed that the patient was eager and worried about the appearance of his genitals. It was seen that the skin of the scrotum that covered the right testicle was almost intact. The skin of this area has the characteristic of being very elastic; the dissection of the skin was made until there was enough tissue to cover the left testicle. Conclusions: functional and aesthetic results were achieved. Thanks to the applied technique, the patient recovered with a minimum of aesthetics sequelae.
