BACKGROUND: In those patients who do not have timely access to primary angioplasty, the pharmaco-invasive approach, that is, the use of thrombolysis as a bridging measure prior to the coronary angiography, is a safe alternative. AIM: To describe the features of patients with an acute ST-elevation myocardial infarction (STEMI) treated with a pharmaco-invasive strategy. MATERIAL AND METHODS: Descriptive observational study of 144 patients with mean age of 46 years with STEMI who received a dose of thrombolytic prior to their referral for primary angioplasty at a public hospital between 2018 and 2021. RESULTS: There were no differences the clinical presentation according to the Killip score at admission between thrombolyzed and non-thrombolyzed patients (p = ns). Fifty-three percent of non-thrombolyzed patients were admitted with an occluded vessel (TIMI 0) compared with 27% of thrombolyzed patients (p < 0.001). The thrombolyzed group required significantly less use of thromboaspiration (3.5 and 8.4% respectively; p = 0.014). Despite this, 91 and 92% of non-thrombolyzed and thrombolyzed patients achieved a post-angioplasty TIMI 3 flow. Long-term survival was 91 and 86% in thrombolyzed and non-thrombolyzed patients, respectively (p = ns). CONCLUSIONS: The pharmaco-invasive strategy is a safe alternative when compared to primary angioplasty in centers that don't have timely access to Interventional Cardiology.

ABSTRACT: Background: Coronary artery disease represents the condition with the highest prevalence worldwide. The treatment of this disease is through Percutaneous Coronary Intervention (PCI). Aiming: To outline the clinical epidemiological profile of health users who have undergone PCI, in a Hospital in the state of Rio Grande do Sul. Methods: Exploratory research, with a quantitative approach carried out on 188 medical records of health users who were submitted to elective or emergency PCI, with the use of stents or not in the year 2018. Values of p <0.05 represented significant statistical differences. Results: Users with a diagnosis of unstable angina (n = 16; 17.8%), diagnostic symptoms of unstable angina (n = 61; 67.8%) and a diagnosis of angina pectoris (n = 52; 57.8%) presented significantly more likely to be submitted to the PCI in elective way (p <0.001). Users diagnosed with Acute Myocardial Infarction - AMI (n = 59; 60.2%) and symptoms with evolution greater than 24 hours (n = 30; 30.6%) and less than 24 hours (n = 24; 24.5%), with dyslipidemia (n = 38; 38.8%) and who underwent catheterization on the same day (n = 87; 88.8%) are significantly more likely to perform emergency surgery (p <0.05). In addition, the data showed that the diagnosis of unstable angina potentiates new cardiomyopathies (n = 9, 47.4%; p <0.001) and restenosis (n = 5; 26.3%; p = 0.002). Conclusion: It is necessary to create strategies to strengthen the Health Care Network (HCN) with actions for prevention, promotion and rehabilitation to health, aiming quality in diagnosis, treatment and rehabilitation. RESUMO: Introdução: A doença arterial coronariana representa a afecção de maior prevalência mundial. O tratamento desta doença se dá pela intervenção coronária percutânea (ICP). Objetivo: Delinear o perfil clínico-epidemiológico dos usuários de saúde submetidos à ICP em um Hospital do estado do Rio Grande do Sul. Métodos: Pesquisa exploratória, de abordagem quantitativa realizada em 188 prontuários de usuários de saúde submetidos à ICP eletiva ou de urgência com uso de stent ou não no ano de 2018. Valores de p< 0,05 representaram diferenças estatísticas significativas. Resultados: Usuários com diagnóstico de angina instável (n=16; 17,8%), sintomas diagnósticos de angina instável (n= 61; 67,8%) e diagnóstico de angina pectoris (n=52; 57,8%) apresentaram significativamente maior probabilidade de serem submetidos à ICP em caráter eletivo (p<0,001). Usuários com diagnóstico para infarto agudo do miocárdio (n=59; 60,2%) e sintomatologia com evolução maior que 24 horas (n=30; 30,6%) e menor que 24 horas (n=24; 24,5%), com dislipidemia (n=38; 38,8%) e que fizeram cateterismo no mesmo dia (n=87; 88,8%) são significativamente mais propensos a realizar cirurgia de urgência (p<0,05). Além disso, os dados evidenciaram que o diagnóstico de angina instável potencializa novas cardiomiopatias (n=9, 47,4%; p<0,001) e reestenose (n=5; 26,3%; p= 0,002). Conclusão: É necessário criar estratégias para fortalecer a Rede de Atenção à Saúde (RAS) com ações de prevenção, promoção e reabilitação à saúde, visando a qualidade no diagnóstico, tratamento e reabilitação. Resumen Objetivo: Mostrar los resultados en el corto y mediano plazo del tratamiento endovascular de angioplastia transluminal percutánea (ATP) con balón en pacientes en estado de isquemia crítica por enfermedad arterial obstructiva infrapoplítea. Materiales y Método: Estudio descriptivo, observacional, retrospectivo. Se incluyeron los pacientes hospitalizados entre 2009 y 2018 por isquemia crítica Fontaine III o IV sometidos a una ATP del territorio infrapoplíteo. Se observó como objetivos primarios la preservación de la extremidad afectada y la mortalidad posoperatoria a un año plazo, y como objetivos secundarios los procedimientos adicionales en pacientes con lesiones o necrosis distales, estadía hospitalaria, complicaciones posoperatorias y necesidad de reintervención. Resultados: Se incluyeron 42 pacientes con un promedio de edad de 66 años (46-82), con importantes comorbilidades. Un 83,3% ingresó en etapa Fontaine IV. En 16 casos se realizó una angioplastia percutánea en más de una arteria. No se colocó stents. Se presentaron complicaciones en 3 pacientes, 2 requirieron una amputación mayor y en otro se debió efectuar un nuevo procedimiento endovascular de rescate. La estadía hospitalaria promedio fue 22 días. No hubo mortalidad precoz posprocedimiento. La mortalidad global a un año fue 9,5%. A todos los pacientes en etapa Fontaine IV se les efectuó algún procedimiento adicional, a 31 una amputación menor, 3 cerraron sus lesiones por segunda intención y en otro se realizó un injerto dermo-epidérmico. De los 35 pacientes con seguimiento, 77% preservó su extremidad a un año. Conclusión: La reparación endovascular mediante una angioplastia percutánea en estos casos es un procedimiento seguro y tiene una alta tasa de preservación de la extremidad inicial a un año de seguimiento.


Aim: Show initial and midterms results of endovascular Percutaneous Transluminal Angioplasty (PTA) in critical limb isquemia (CLI) patients caused by below-the-knee arterial disease. Materials and Method: Observational, descriptive and retrospective study. 42 CLI patients admitted in our hospital from 2009 until 2018 with Fontaine III or IV treated by PTA in infrapopliteal arteries were analyzed, collecting demographic, clinical and surgical characteristics, additional procedures in Fontaine IV, hospital stay, postoperative complications, need of reintervention, limb preservation and mortality with one year follow-up after procedure. Results: 42 patients, average age 66 year-old (46-82), with significant comorbidities. Fontaine IV stage patients were 83.3%. In 16 cases more than one artery was intervened. No stent revascularization was performed. Complications occurred in 3 patients, 2 required major amputation and an urgent endovascular reintervention was required in another. Average hospital stay was 22 days with no post-operative mortality. One-year global mortality was 9.5 One-year follow-up in 35 patients shows that 77% preserved their limb. Conclusión: Percutaneous transluminal angioplasty procedure in this patients has a high rate of limb preservation in a one-year follow-up. There was no post-operative mortality.

SUMMARY OBJECTIVE: The objective of this study was to compare the interventions of percutaneous transluminal drug-coated balloon angioplasty (DCB PTA) and standard PTA in the treatment of patients with the below-the-knee peripheral artery disease (BTK PAD). METHODS: Overall, 196 patients (113 males and 83 females; mean age: 63.56±11.94 years; 45-83 years) were treated with PTA for BTK PAD between June 2014 and March 2019. RESULT: Standard PTA (group 1; 96 patients) and DCB PTA (group 2; 100 patients) results were analyzed and compared retrospectively. No statistically significant difference was found between the mean ages of group 1 and 2 patients (p=0.371, p>0.05). Demographic and clinical data were compared and no any statistically significant differences was found between the two groups. Comparing in terms of the iliac lesion, there was no statistically significant difference between the two groups. However, a statistically significant difference was found between the two groups in terms of frequency of popliteal lesions (p=0.001; p<0.05). There was not a statistically significant difference between the two groups in terms of other lesions. In addition, limb salvage rates were 82.0% (18 amputations) and 65.6% (33 amputations) in the drug-release balloon group and the naked balloon group, at the end of 1 year, respectively. No distal embolism, limb-threatening ischemia, and mortality were observed in any patients. CONCLUSIONS: Based on this study, patients in the DCB group had significantly higher rates of primary patency as compared with the other patients.

Subclavian steal syndrome is a group of symptoms resulting fromretrograde flow in the vertebral artery, "stealing" blood from the posterior intracranial circulation and other territories, caused by stenosis or occlusion of the subclavian artery proximal to the origin of the same vertebral artery, or even of the brachiocephalic trunk. Most of the time, it is an incidental finding in patients with other conditions or cerebrovascular risk factors. We report a series of 29 patients with an angiographic diagnosis, in which 7 received treatment (all endovascular), all with symptoms directly related to this condition. Advanced age, systemic arterial hypertension, diabetes mellitus, smoking and stroke were comorbidities frequently related. Six patients improved completely after the procedure and one remained with vertigo.

Objetivo: Comparar os tempos de tratamento dor-porta e porta-balão em indivíduos com infarto agudo do miocárdio com supradesnivelamento ST com os desfechos cardiovasculares em 30 dias. Métodos: Trata-se de uma coorte histórica, realizada por meio da pesquisa de prontuários eletrônicos e dos bancos de dados já existentes dos serviços de hemodinâmica de todos os indivíduos atendidos com diagnóstico de infarto agudo do miocárdio com supradesnivelamento ST e submetidos à angioplastia, no período de março de 2015 a setembro de 2016, em dois hospitais públicos de grande porte de Porto Alegre (RS). Os desfechos foram o óbito intra-hospitalar e em 30 dias e os eventos cardíacos maiores hospitalares e em 30 dias. Resultados: Foram avaliadas as informações de 808 indivíduos, sendo 26,9% provenientes do Hospital de Clínicas de Porto Alegre e 73,1% do Instituto de Cardiologia ­ Fundação Universitária de Cardiologia. Não houve diferença significativa na caracterização da amostra. Um terço dos indivíduos analisados apresentou tempo dor- -porta menor ou igual a 180 minutos, e 72% tiveram tempo porta-balão menor que 90 minutos. A mediana do tempo total de isquemia foi de 338 minutos. Na avaliação dos tempos não houve diferença significativa entre os dois hospitais. Para eventos cardíacos maiores e óbitos intra- -hospitalares, o único tempo que se mostrou significativo, após o ajuste multivariado, foi o porta-balão, em que os indivíduos com tempo maior que 90 minutos apresentaram razão de risco de 1,06 (IC95% 1,02-1,11) e 5,78 (IC95% 1,44-23,2), respectivamente, para eventos cardíacos maiores e óbitos intra-hospitalares. Para eventos cardíacos maiores total e óbito total, nenhum dos três tempos se associou significativamente com o desfecho após ajuste. Contudo, o tempo porta-balão maior ou igual a 90 minutos também foi significativo para razão de risco bruto para ambos, assim como a dor-porta para óbito total. Conclusão: Os dados da pesquisa corroboram as recomendações internacionais para cumprimento dos menores tempos de atendimento, em especial do tempo porta-balão, para o bom prognóstico. Infelizmente, no país, o tempo de isquemia miocárdica ainda está muito aquém do ótimo, necessitando de melhorias na área para melhorar os desfechos nesses indivíduos.


Objective: To compare symptom-onset-to-door and door- -to-balloon times in individuals with ST-segment elevation myocardial infarction to the 30-day cardiovascular outcomes. Methods: This is a historical cohort, using electronic medical records and the existing databases of hemodynamic services of all individuals diagnosed with ST-segment elevation myocardial infarction undergoing angioplasty between March 2015 and September 2016, in two large public hospitals in Porto Alegre. The outcomes were in-hospital death and death in 30 days, and major adverse cardiac events in hospital and in 30 days. Results: The information of 808 patients was evaluated, with 26.9% from Hospital de Clínicas de Porto Alegre, and 73.1% from the Instituto de Cardiologia ­ Fundação Universitária de Cardiologia. There was no significant difference in the characterization of the sample. One-third of the individuals evaluated presented symptom-onset-to-door of 180 minutes or less, and 72% had door-to- -balloon time below 90 minutes. The median total ischemic time was 338 minutes. In the evaluation of the times, there was no significant difference between the two hospitals. For more major cardiac events and intra-hospital deaths, the only time that proved to be significant after the multivariate adjustment was the door-to-balloon time, in which individuals with time higher than 90 minutes had a risk ratio of 1.06 (95% CI 1.02-1.11) for major cardiac events and 5.78 (95% CI 1.44-23.2), for intra-hospital deaths. For total major adverse cardiac events and total death, none of the 3 times was significantly associated with the outcome after adjustment; however, door-to-balloon of 90 minutes or more was also significant for crude risk ratio for both, as well as symptom-onset-to-door for total death. Conclusion: The research data corroborate the international recommendations to meet shorter service times, especially door-to-balloon time, for a good prognosis. Unfortunately, in the country, the time of myocardial ischemia is still far from optimal, requiring improvement in the area to improve the outcomes in these individuals.

Abstract Background The use of an adequate door-to-balloon time (≤ 90 minutes) is crucial in improving the quality of care provided to patients with ST-segment elevation myocardial infarction (STEMI). Objective To determine the door-to-balloon time in the management of STEMI patients in a cardiovascular emergency department in a hospital of northern Brazil. Methods This was a cross-sectional study based on review of medical records. A total of 109 patients with STEMI admitted to the emergency department of a referral cardiology hospital in Pará State, Brazil, between May 2017 and December 2017. Correlations of the door-to-balloon time with length of hospital stay and mortality rate were assessed, as well as whether the time components of the door-to-balloon time affected the delay in performing primary percutaneous coronary intervention. Quantitative variables were analyzed by Spearman correlation and the G test was used for categorical variables. A p<0.05 was set as statistically significant. Results Median door-to-balloon time was 104 minutes. No significant correlation was found between door-to-balloon time and length of hospital stay or deaths, but significant correlations were found between door-to-balloon time and door-to-ECG time (p<0.001) and ECG-to-activation (of an interventional cardiologist) time (p<0.001). Conclusion The door-to-balloon time was longer the recommended and was not correlated with the length of hospital stay or in-hospital mortality. Door-to-ECG time and ECG-to-activation time contributed to the delay in performing the primary percutaneous coronary intervention. Int J Cardiovasc Sci. 2020; [online].ahead print, PP.0-0

Abstract The SAFARI technique or Subintimal Arterial Flossing with Antegrade-Retrograde Intervention is an endovascular procedure that allows recanalization of Chronic Total Occlusive (CTO) lesions when conventional subintimal angioplasty is unsuccessful. Retrograde access is usually obtained through the popliteal, anterior tibial, dorsalis pedis artery, or posterior tibial arteries and may potentially provide more options for endovascular interventions in limb salvage. The case of an 81-year-old man with a history of uncontrolled hypertension, diabetes mellitus, and dyslipidemia is presented. He presented with a cutaneous ulcer on the right lower limb with torpid evolution and poor healing. The Doppler ultrasound and arteriographic study revealed a CTO lesion of the popliteal artery that was not a candidate for antegrade endovascular revascularization, but was successfully treated using the SAFARI technique. The patient had no perioperative complications, the wound showed better healing, and he was discharged with an indication of daily dressings and control by an external outpatient clinic.


Resumo A técnica SAFARI, ou Subintimal Arterial Flossing with Antegrade-Retrograde Intervention, é um procedimento endovascular que permite a recanalização de lesões por oclusão total crônica (OTC) em caso de fracasso da angioplastia subintimal convencional. O acesso retrógrado é geralmente obtido através da artéria poplítea, tibial anterior, pediosa ou tibial posterior e pode fornecer mais alternativas de intervenções endovasculares para o salvamento do membro. É apresentado o caso de um homem de 81 anos com histórico de hipertenção não controlada, diabetes melito e dislipidemia. Ele apresentava uma lesão ulcerativa cutânea no membro inferior direito com evolução tórpida e má cicatrização. O ultrassom Doppler e o estudo arteriográfico revelaram uma lesão por OTC na artéria poplítea. O paciente não era candidato a revascularização endovascular anterógrada; sendo assim, esta foi realizada com successo utilizando a técnica SAFARI. O paciente não apresentou complicações perioperatórias e recebeu alta com indicação de cuidados diários com a ferida e controle em um ambulatório externo. Além disso, a ferida apresentou melhor cicatrização.

Resumo A síndrome de Budd-Chiari é uma doença venosa hepática rara, mais incidente em adultos jovens, podendo se apresentar na forma aguda, subaguda ou crônica, o que resulta em hipertensão portal. O tratamento tradicional consiste em técnicas de trombólise e de shunts portossistêmicos intra-hepáticos, como pontes para o transplante hepático. Recentemente, técnicas de angioplastia com balão ou stents têm sido relatadas para o tratamento dessa afecção. Neste artigo, é relatado e discutido um caso de síndrome de Budd-Chiari por obstrução membranosa da via de saída da veia supra-hepática com trombose da veia hepática média em uma paciente de 24 anos. O tratamento estabelecido foi a angioplastia transjugular com balão, que obteve resultados satisfatórios e boa evolução clínica.


Abstract The Budd-Chiari syndrome is a rare hepatic venous disease. It is more prevalent in young adults and may present in acute, subacute, or chronic forms, causing portal hypertension. Traditional treatment consists of thrombolysis techniques and transjugular intrahepatic portosystemic shunt, as a bridge to liver transplantation. Recently, use of balloon or stent angioplasty techniques has been reported for treatment of this condition. In this article, we report and discuss a case of BCS by membranous obstruction in the hepatic vein outflow tract, with middle hepatic vein thrombosis, in a 24-year-old patient. The treatment chosen and employed was transjugular balloon angioplasty, which achieved satisfactory results and good clinical evolution.

Background: Chronic limb ischemia can lead to high rates of limb loss and mortality. Open surgery is the gold standard for treatment of distal disease. Endovascular surgery should have less complications with similar outcomes. Aim: To report a cohort of patients with distal arterial disease treated with endovascular surgery at our institution. Material and Methods: Review of angioplasty records of patients undergoing distal lower extremity endovascular procedures between 2016 and 2019. Demographics, comorbidities, form of presentation, type of intervention, perioperative complications, and length of stay were analyzed. The primary outcomes were wound healing, reinterventions and freedom from major amputation. Secondary outcomes were overall survival and amputation-free survival. Results: Forty-eight limbs of 41 patients with a mean age 75 years (78% males) were treated. Ninety-three percent had hypertension, 88% diabetes, 30% chronic kidney disease. 73% presented with major wounds. Plain balloon and drug coated balloon angioplasties were carried out in 65 and 31% of procedures respectively, with no difference in results. In 46% of the cases, only chronic total occlusions were treated. Wound healing was achieved in 85% of procedures and 90% of patients were free from amputation at one year of follow up. Complications were observed in 18% of procedures, perioperative mortality was 2% and one-year survival was 76%. Conclusions: Endovascular therapy achieves high rates of wound healing and freedom from amputation with low perioperative mortality and moderate complication rates.

OBJECTIVES: To assess clinical and coronary angiographic characteristics, previous medical history, and clinical course, by age group, in older adults after myocardial infarction who underwent primary percutaneous coronary intervention (pPCI). METHODS: Single-center, cohort study that enrolled all patients with ST-segment elevation myocardial infarction who underwent pPCI at a specialized cardiology reference center in the South of Brazil. Older adults were defined as age ≥60 years, as set out in Brazilian legislation. Patients in the following age groups were compared: 60 to 64 years, 65 to 69 years, 70 to 74 years, 75 to 79 years, and ≥80 years. Patients' clinical course was assessed at initial hospital admissions and after 2 years of clinical follow-up. Data were analyzed using SPSS 19, and significance was established at p <0.05. RESULTS: From December 2015 to December 2018, a total of 636 patients were enrolled consecutively. Angiographic success rates were around 90% in all age groups. There were no differences in medications used, except for glycoprotein IIb/IIIa inhibitors, which were more frequently used in patients of lower age groups. Older patients had more in-hospital acute renal failure and higher in-hospital mortality. Predictors of mortality were age over 75, chronic renal failure, need for ventilatory support, severe arrhythmia, and sepsis. CONCLUSIONS: pPCI in older adult patients is a safe procedure with a high success rate.


OBJETIVOS: Avaliar características clínicas e angiográficas, história clínica pregressa e evolução clínica, por faixa etária, em idosos submetidos a intervenção coronária percutânea primária (ICPp) após infarto do miocárdio. METODOLOGIA: Estudo de coorte, de centro único, que incluiu todos os pacientes com infarto do miocárdio com supradesnivelamento do segmento ST submetidos ICPp em um centro de referência especializado em cardiologia no sul do Brasil. Os idosos foram definidos como aqueles com idade ≥60 anos, conforme estabelecido na legislação brasileira. Os pacientes nas seguintes faixas etárias foram comparados: 60 a 64 anos, 65 a 69 anos, 70 a 74 anos, 75 a 79 anos e ≥80 anos. O curso clínico dos pacientes foi avaliado nas admissões hospitalares iniciais e após 2 anos de acompanhamento clínico. Os dados foram analisados usando o SPSS 19, e p <0,05 foi considerado significativo. RESULTADOS: De dezembro de 2015 a dezembro de 2018, 636 pacientes foram incluídos consecutivamente. As taxas de sucesso angiográfico foram de cerca de 90% em todas as faixas etárias. Não houve diferenças nos medicamentos utilizados, com exceção dos inibidores da glicoproteína IIb/IIIa, que foram mais frequentemente utilizados em pacientes em faixas etárias mais baixas. Pacientes mais velhos apresentaram mais insuficiência renal aguda intra-hospitalar e maior mortalidade hospitalar. Os preditores de mortalidade foram: idade superior a 75 anos, insuficiência renal crônica, necessidade de suporte ventilatório, arritmia grave e sepse. CONCLUSÕES: O ICPp em pacientes idosos é um procedimento seguro e com alta taxa de sucesso.

RESUMEN Con el objetivo de determinar las características clínicas y epidemiológicas de los pacientes con infarto de miocardio agudo (IMA), se realizó un estudio descriptivo en 175 pacientes de un hospital peruano de referencia. La edad promedio de los pacientes fue de 68,7 años y el 74,8% fueron de sexo masculino. La principal estrategia de reperfusión utilizada fue la angioplastia coronaria transluminal percutánea + stent, aunque su uso primario resultó ser bajo; además, el porcentaje de revascularización quirúrgica fue alto. Casi el 60% de los pacientes tuvo una estancia hospitalaria mayor a siete días. La mortalidad intra hospitalaria fue de 3,4%, siendo el shock cardiogénico la causa más frecuente de muerte. Se concluye que el IMA afecta principalmente a varones mayores de 60 años y que la principal estrategia de reperfusión es la angioplastia, además, los tiempos para la realización de la reperfusión son mayores a lo recomendado con porcentajes altos de revascularización quirúrgica.


ABSTRACT To determine the epidemiological and clinical characteristics of patients with acute myocardial infarc tion, a descriptive study was conducted in 175 patients in a referral hospital in Lima. The average age of the patients was 68.7 ± 10.8 years and 74.8% were male. The main reperfusion strategy used was percutaneous transluminal coronary angioplasty + stent, however, the use of primary angioplasty was low (19.5% of patients with ST elevation myocardial infarction). Time to reperfusion therapies (angio plasty or fibrinolysis) were longer than recommended and the percentage of surgical revascularization was high. Almost 60% of the patients had a length of stay longer than seven days. In-hospital mortality was 3.4%, with cardiogenic shock being the most frequent cause of death. Myocardial infarction mainly affects men over 60 years, the clinical and epidemiological variables are like other regional reports. The main reperfusion strategy is angioplasty, although the use of primary angioplasty is low. Time to perfor ming reperfusion is longer than recommended and the percentage of surgical revascularization is high.

Introdução. O infarto agudo do miocárdio apresenta significativas taxas de morbimortalidade. A reperfusão precoce por angioplastia primária é a intervenção que reduz a mortalidade e as complicações, e deve ser iniciada em até 12 horas, a fim de impedir a perda muscular irreversível. O tempo entre chegada do paciente ao hospital e a abertura da artéria acometida, tempo porta-balão, determina a morbimortalidade do paciente. Objetivo. Esse estudo busca analisar o potencial benefício do tratamento da reperfusão coronariana precoce, os fatores de risco, as possíveis complicações e o Killip em pacientes que sofreram infarto agudo do miocárdio relacionando-os a sua morbimortalidade. Materiais e métodos. Estudo observacional transversal realizado por meio de coleta de dados dos prontuários dos pacientes submetidos a angioplastia primária de um hospital privado. Resultados. A hipertensão arterial sistêmica foi a variável mais prevalente (75%), e que houve predomínio no sexo masculino (71%) e associação com a progressão da idade. 61% dos pacientes apresentaram um tempo porta balão menor que 90 minutos. Houve significância estatística entre o tempo porta balão e a evolução do Killip, evidenciando um tempo porta-balão maior que 90 minutos na maioria dos pacientes que obtiveram aumento da pontuação do Killip. Conclusão. A precocidade da intervenção no paciente com IAM impacta na morbimortalidade, visto que o tempo porta balão está diretamente associado a evolução da do Killip. Logo, deve-se identificar os fatores que interferem no atendimento, a fim de proporcionar uma intervenção otimizada. (AU)


Introduction. Acute myocardial infarction has significant rates of morbidity and mortality. Early reperfusion by primary angioplasty is the intervention that reduces mortality and complications, and should be started within 12 hours in order to prevent irreversible muscle loss. The time between the patient's arrival at the hospital and the opening of the affected artery, door-to-balloon time, determines the patient's morbidity and mortality. Objective. The proposition of this study is to analyze the potential benefits of early coronary reperfusion, associated with the risk factors, possible complications, and the Killip score in patients whit acute myocardial infarction (AMI) and the relation of those factors with the morbidity and mortality. Materials and methods. This is a transversal observational study and uses data collected of medical records of patients subjected to primary angioplasty in a private hospital. Results. Systemic arterial hypertension was the most prevalent one (75%), it was more common in males (71%) and associated with a higher age. In 61% of the patients port-balloon time was less than 90 minutes. There was statistical significance between port-balloon time and Killip score evaluation, that showed a higher score in patient with a port-balloon time that exceeded 90 minutes. Conclusion. Early intervention in patients with AMI impacts morbimortality, once that the port-balloon time is directly associated with the Killip score results. Therefore, all factors that can lead to a delay in their care of those patients should be identified with the objective of optimize the intervention. (AU)

Abstract A 67-year-old male diabetic patient with systemic arterial hypertension was admitted to the emergency department with a necrotic ulcer in the left external malleolus and no palpable popliteal or pedal pulses. Arterial Duplex ultrasound identified femoropopliteal occlusion, with popliteal refilling below the knee and a patent peroneal artery. An endovascular procedure was performed, requiring retrograde access to the popliteal artery to re-establish blood flow and deploy a popliteal stent. Technical success was achieved and the patient underwent debridement of the wound. Two days later, about 48 hours after the operation, the patient began to exhibit respiratory symptoms, with coughing and dyspnea. He immediately underwent a chest CT that identified ground glass opacities, the crazy-paving pattern, and bilateral air bronchogram in the lungs. A reverse transcription - polymerase chain reaction (RT-PCR) test was positive for SARS-Cov-2. The patient was moved to an intensive care unit and put on mechanical ventilation. Both hydroxychloroquine and azithromycin were administered. Despite appropriate treatment, the patient died 4 days after he was diagnosed with COVID-19.


Resumo Paciente do sexo masculino, 67 anos, diabético, hipertenso, foi admitido no pronto-socorro com histórico de úlcera necrótica no maléolo externo esquerdo, sem pulsos poplíteos e distais palpáveis. A ultrassonografia Doppler arterial identificou oclusão femoropoplítea, com reenchimento da artéria poplítea infragenicular e perviedade da artéria fibular. Foi realizado procedimento endovascular, com necessidade de acesso retrógrado na artéria poplítea para restabelecer o fluxo sanguíneo e realizar implante de stent poplíteo. O sucesso técnico foi alcançado e, em seguida, o paciente foi submetido ao desbridamento da ferida. No segundo dia, cerca de 48 horas de pós-operatório, o paciente apresentou quadro respiratório com tosse e dispneia. Foi submetido a uma tomografia computadorizada do tórax, que identificou opacidades em vidro fosco e broncograma aéreo bilateralmente nos pulmões, com teste de reverse transcription - polymerase chain reaction (RT-PCR) positivo para SARS-Cov-2. O paciente foi transferido para uma unidade de terapia intensiva, necessitando de ventilação mecânica. Recebeu hidroxicloroquina e azitromicina. Apesar do tratamento em suporte intensivo, o paciente morreu 4 dias após o diagnóstico de COVID-19.

Abstract Introduction: Radial access is the gold standard for ST-elevation myocardial infarction; nevertheless, there is scarce information in Mexico. Objectives: The objectives of this study were to describe the differences in radiation exposure, intervention time, fluoroscopy time, complications and temporal trends, and risk factors among radial and femoral access for coronary procedures. Materials and Methods: A total of 493 patients underwent coronary interventions by femoral or radial access. Sociodemographic and procedural data were recorded. A logistic regression model to determine risk factors for complications was performed. Results: The population included 346 men and 147 women, with a median age of 63 years, 159 underwent radial and 334 femoral approaches. Complications occurred in 18 patients (3.6%), 11 in radial and 7 in femoral access, with a higher trend in the first 5 months (n = 14). Vasospasm was the most common (n = 9) complication. Median fluoroscopy time was 12 min for radial and 9 min for femoral groups, with a total radiation dose of 2282 µGm2 and 2848 µGm2, respectively. Temporal trends showed that complications occurred most frequently during the first 6 months of the study. The main predictors for complications were intervention time and one-vessel disease. Conclusions: Radial access had higher frequency of complications than femoral approach and they were more common during the first 6 months. The main risk factor was intervention time longer than 60 min.


Resumen Introducción: El abordaje radial es el de elección para infarto de miocardio con elevación del segmento ST, sin embargo se desconoce información en México. Objetivos: Describir las diferencias en exposición a radiación, tiempo de intervención, tiempo de fluoroscopía, complicaciones y sus variaciones temporales, además de los factores de riesgo entre el abordaje radial y el femoral para procedimientos coronarios. Método: Se incluyeron 493 pacientes que fueron sometidos a estudio angiográfico o intervenciones coronarias por abordaje radial o femoral. Se recabaron datos sociodemográficos, antecedentes y variables del procedimiento. Se realizó un modelo de regresión logística para determinar los factores asociados a complicaciones. Resultados: Se incluyeron 346 hombres y 147 mujeres, con mediana de edad de 63 años. A 159 se les realizó acceso radial y a 334 femoral. Las complicaciones ocurrieron en 18 pacientes (3.65%): 11 en radial y 7 en femoral, teniendo mayor incidencia en los primeros 5 meses (n = 14) y siendo el vasoespasmo el más común (n = 9). La mediana de tiempo de fluoroscopía fue de 12 minutos para el radial y de 9 minutos para el femoral, con una dosis total de radiación de 2,282 µGm2 y 2,848 µGm2, respectivamente. Las tendencias temporales indicaron que las complicaciones fueron más frecuentes durante los primeros 6 meses. Los principales predictores fueron el tiempo de intervención y la enfermedad de 1 vaso. Conclusiones: La vía de acceso radial tuvo más complicaciones que la femoral. Se observaron más complicaciones en los primeros 6 meses del estudio. El principal predictor de complicaciones fue el tiempo de intervención mayor a 60 minutos.

Se evaluaron las características clínicas, demoras, resultados y morbimortalidad de 1142 pacientes tratados con angioplastia primaria (AP) dentro de las 36 horas del infarto, incluidos en el registro ARGEN-IAM-ST, de carácter prospectivo, transversal, multicéntrico y de alcance nacional. Edades: 61.2 ± 12 años, 88% varones, 20% diabéticos y 58% hipertensos; 77.6% en Killip y Kimball I y 6.2% en shock cardiogénico. El tiempo desde el inicio del dolor hasta el ingreso fue de 153 (75-316) minutos, y puerta-balón de 91 (60-150) minutos. Los casos derivados (17%) tuvieron mayor demora de ingreso, 200 minutos (195-420; p = 0.0001) y mayor tiempo puerta-balón, 113 minutos (55-207); p = 0.099. En 47.6% de los casos la AP se hizo en arteria descendente anterior, en 36.4% a coronaria derecha, en 14.8% a circunfleja y en 1.2% al tronco de coronaria izquierda; en 95% con stent (29% farmacológico). El 95% fue exitoso. El 1.3% presentó angina post-infarto (APIAM), 1.3% re-infarto, 8.8% shock y 3.2% sangrado. Se asociaron a mayor mortalidad edad > 64 años (OR 6.2 (IC 95%: 3.2-12), p < 0.001), diabetes (OR 2.5, IC 95% 1.6-3.9, p < 0.001), re-infarto o APIAM (OR 3.3, IC 95% 1.3-8.3, p = 0.011) y shock (OR 29.2 (15.6-54.8), p < 0.001). La mortalidad hospitalaria del infarto agudo de miocardio con elevación del segmento ST tratado con AP fue de 7.6%. La derivación se asoció a demora de tratamiento. El shock cardiogénico y la isquemia post-infarto se asociaron a alta mortalidad. No hubo variables del procedimiento asociadas a mortalidad.


Our objective was to evaluate clinical characteristics, results and morbi-mortality in primary angioplasty (PA), of patients treated with PA within 36 hours of a myocardial infarction (MI), included in a prospective, transversal, multicenter and national survey (ARGEN-IAM-ST). A total of 1142 patients treated with PA were registered, 61.2 ± 12 years old, 88% male, 20% diabetics and 58% with hypertension; 77.6% in Killip Kimball I and 6.2% in cardiogenic shock. The time from the onset of pain until admission was 153 (75-316) minutes, and door-balloon of 91 (60-150) minutes. The transferred patients (17%) showed longer delay to admission, 200 minutes (195-420; p = 0.0001) and door-to-balloon 113.5 minutes (55-207); p = 0.099. In 47.6% of the cases, the PA was made in the anterior descending artery, in 36.4% in the right coronary artery, in 14.8% in the circumflex artery and in 1.2% in the left coronary artery; in 95% with stent (29% pharmacological); 95% was successful, 1.3% presented post-infarct angina (APIAM), 1.3% re-infarct, 8.8% shock and 3.2% bleeding. Age > 64 years (OR 6.2 (95% CI: 3.2-12), p <0.001), diabetes (OR 2.5, 95% CI 1.6-3.9, p < 0.001), re-infarction or APIAM (OR 3.3, 95% CI 1.3-8.3, p = 0.011) and shock (OR 29.2 (15.6-54.8), p < 0.001) were independently associated with higher mortality. In-hospital mortality of acute myocardial infarction with ST-segment elevation treated with PA was 7.6%. Transference from other center was associated with delay in the admission and treatment. Cardiogenic shock and post-infarct ischemia were associated with high mortality. There were no procedural variables associated with mortality.

Abstract The Bridge Occlusion Balloon is a compliant balloon, specifically designed for temporary Superior vena cava occlusion in case of Superior Vena Cava laceration during lead extraction procedures. We here report the first case, using Bridge Occlusion Ballon for a venous angioplasty in a patient with dysfunctional pacemaker leads and symptomatic Superior Vena Cava occlusion. After successful lead extraction, venography was showing a narrow venous canal. Therefore, venous angioplasty using the Bridge balloon was performed. Especially for high-risk lead extraction cases in patients with Superior Vena Cava stenosis, the Bridge Occlusion Ballon might be used as a combination of a safety-net in case of Superior Vena Cava perforation and for Superior Vena Cava angioplasty.

OBJETIVO: Evaluar las características de los pacientes sometidos a angioplastía en miembros inferiores, así como establecer posibles asociaciones entre los factores que lleven a complicaciones de la misma. MATERIALES Y MÉTODO: Estudio descriptivo, observacional, retrospectivo de corte transversal durante 18 meses en el que se evaluaron aquellos pacientes con enfermedad arterial periférica sometidos a angioplastía de miembros inferiores, sus características demográficas, clínicas, quirúrgicas y las complicaciones postoperatorias. RESULTADOS: Se evaluaron 158 registros de pacientes con edades entre 30 y 95 años. El 65,2% (n = 103) de los procedimientos se realizaron de forma electiva, los vasos intervenidos con mayor frecuencia fueron la arteria femoral superficial (64%), el tipo de intervención endovascular más frecuente fue la combinación de balón + balón medicado (32,9%), el tratamiento farmacológico postoperatorio más usado fue la combinación de ácido acetilsalicílico (ASA) y clopidogrel (86,1%). Se presentaron 21 casos de complicaciones, la más frecuente fue la amputación menor (52%). Se encontró mayor riesgo de amputación menor cuando el procedimiento quirúrgico es realizado de forma urgente (p = 0,012; OR [IC 95%]: 4,8 [1,4-16,5]). DISCUSIÓN: La complicación posangioplastía con diferencia estadísticamente significativa fue la amputación menor cuando el procedimiento se realizó de manera urgente, esta asociación pudo estar relacionada con el estado clínico del paciente en el momento del ingreso y no con el procedimiento quirúrgico. CONCLUSIÓN: La angioplastía realizada de urgencias fue un procedimiento tan seguro como cuando se realiza de forma programada, dado por la misma proporción de sangrado o disección arterial.


AIM: Evaluate the characteristics of patients undergoing angioplasty in the lower limbs, as well as to establish possible associations between the factors leading to complications. MATERIALS AND METHODS: Observational descriptive, cross-sectional and retrospective study conducted during 18 months in which patients with peripheral arterial disease undergoing angioplasty of the lower limb were included, as well their demographic, clinical, and surgical characteristics and the postoperative complications. RESULTS: 158 records of patients between 30 and 95 years were evaluated. 65.2% (n = 103) of the procedures were performed not urgently, the most frequently intervened vessels were the superficial femoral artery (64%), the most frequent type of endovascular intervention was the combination of balloon + medicated ball (32.9%), the most used postoperative pharmacological treatment was the combination of acetylsalicylic acid (ASA) and clopidogrel (86.1%). There were 21 cases of complications, the most frequent complication was minor amputation (52%). A higher risk of minor amputation was found when the surgical procedure was performed urgently (p = 0.012, OR [95% CI]: 4.8 [1.4-16.5]). DISCUSION: The post-angioplasty complication with statistically significant difference was minor amputation when the procedure was performed urgently, this association was related to the clinical status of the patient at the time of admission and not to the surgical procedure. CONCLUSION: Angioplasty performed urgently is as safe as elective procedures, given by the same proportion of bleeding or arterial dissection.

Abstract Background: Primary angioplasty (PA) with placement of either bare metal or drug-eluting stents (DES) represents the main strategy in the treatment of ST-elevation myocardial infarction (STEMI). Diabetic patients, however, represent a special population in STEMI, with high rates of restenosis and unfavorable clinical outcomes, and with the use of DES, level of evidence A and indication class II, being indicated to reduce these damages. Objectives: To evaluate the DES rate of use in patients with STEMI and in the subgroup of diabetics assisted in the public versus private health network in Sergipe. Methods: This is a population-based, cross-sectional study with a quantitative approach using the data from the VICTIM Register. These were collected in the only four hospitals with capacity to perform PA in Sergipe, from December 2014 to March 2017. Results: A total of 707 patients diagnosed with STEMI were evaluated, of which 589 were attended at SUS and 118 at the private network. The use of DES in PA was lower in SUS compared to the private network in both the total sample (10.5% vs 82.4%, p<0.001) and in subgroup diabetic patients (8.7% vs 90.6%, p < 0.001), respectively. In all hypotheses tested, the level of significance was 5% (p < 0.05). Conclusions: The study reveals a disparity in the use of DES during the performance of PA between the public and private network, both in the total sample and the subgroup for diabetics, with lower rates for SUS users, demonstrating the challenges that need to be overcome in order to achieve quality improvements of the services provided.


Resumo Fundamento: A angioplastia primária (AP) com colocação de stent, seja ele convencional ou farmacológico, representa a principal estratégia no tratamento do infarto agudo do miocárdio com supradesnivelamento do segmento ST (IAMCSST). Os pacientes diabéticos, entretanto, representam população especial no IAMCSST, com altas taxas de reestenose e desfechos clínicos desfavoráveis, devendo-se indicar o uso de stents farmacológicos (SF), nível de evidência A e classe de indicação II, para redução destes danos. Objetivo: Avaliar a taxa de uso de SF em pacientes com IAMCSST e no subgrupo dos diabéticos assistidos na rede pública versus privada de saúde em Sergipe. Métodos: Trata-se de um estudo populacional, transversal, com abordagem quantitativa, que utilizou os dados do Registro VICTIM. Estes foram coletados nos quatro únicos hospitais com capacidade para realizar AP em Sergipe, no período de dezembro de 2014 a março de 2017. Em todas as hipóteses testadas, o nível de significância adotado foi de 5% (p < 0,05). Resultados: Foram avaliados 707 pacientes diagnosticados com IAMCSST, dos quais 589 foram atendidos pelo SUS e 118 pela rede privada. O uso de SF na AP foi menor no SUS em comparação com a rede privada, tanto no total da amostra (10,5% vs 82,4%; p < 0,001) quanto no subgrupo dos pacientes diabéticos (8,7% vs 90,6%; p < 0,001), respectivamente. Conclusões: O estudo revela disparidade no uso de SF durante a realização de AP entre a rede pública e privada, tanto na amostra total quanto no subgrupo dos diabéticos, com menores taxas para usuários do SUS, demonstrando os desafios que necessitam ser vencidos para se atingir melhorias na qualidade dos serviços prestados.
