SUMMARY OBJECTIVE: The objective of this study was to compare the interventions of percutaneous transluminal drug-coated balloon angioplasty (DCB PTA) and standard PTA in the treatment of patients with the below-the-knee peripheral artery disease (BTK PAD). METHODS: Overall, 196 patients (113 males and 83 females; mean age: 63.56±11.94 years; 45-83 years) were treated with PTA for BTK PAD between June 2014 and March 2019. RESULT: Standard PTA (group 1; 96 patients) and DCB PTA (group 2; 100 patients) results were analyzed and compared retrospectively. No statistically significant difference was found between the mean ages of group 1 and 2 patients (p=0.371, p>0.05). Demographic and clinical data were compared and no any statistically significant differences was found between the two groups. Comparing in terms of the iliac lesion, there was no statistically significant difference between the two groups. However, a statistically significant difference was found between the two groups in terms of frequency of popliteal lesions (p=0.001; p<0.05). There was not a statistically significant difference between the two groups in terms of other lesions. In addition, limb salvage rates were 82.0% (18 amputations) and 65.6% (33 amputations) in the drug-release balloon group and the naked balloon group, at the end of 1 year, respectively. No distal embolism, limb-threatening ischemia, and mortality were observed in any patients. CONCLUSIONS: Based on this study, patients in the DCB group had significantly higher rates of primary patency as compared with the other patients.

Abstract Background Retroperitoneal open iliac conduits (ROIC) are used in patients with hostile iliac anatomy undergoing endovascular aortic repair (EVAR). Objectives We hereby report our experience of ROIC in patients subjected to EVAR. Methods This was a retrospective evaluation of 8 patients out of a total of 75 patients (11%) who underwent EVAR in the last 10 years. Pre-procedure computed tomography angiography was used to assess the dimensions of iliac and femoral arteries. Patients who had small arterial dimensions (i.e. smaller than the recommended access size for the aortic endograft device) were subjected to ROIC. Results The mean age of the 3 males and 5 females studied was 45.7 ± 15.2 years. The indication for ROIC was the small caliber ilio-femoral access site in 7 patients and atherosclerotic disease in 1 patient. All external grafts were anastomosed to the right common iliac artery except one which was anastomosed to the aortic bifurcation site because of a small common iliac artery. The procedural success rate was 100%. Local access site complications included infection (n=1), retroperitoneal hematoma (n=1), and need for blood transfusion (n=3). The median post-intervention hospital stay was 10 days. All patients had favorable long-term outcomes at a median follow-up of 18 months. Conclusions Female patients require ROIC during EVAR more frequently. Adjunctive use of iliac conduit for EVAR was associated with favorable perioperative and short-term outcomes.


Resumo Contexto Os condutos ilíacos abertos retroperitoneais são utilizados em pacientes submetidos a reparo endovascular de aneurisma (REVA) com anatomia ilíaca hostil. Objetivos Relatamos a nossa experiência com os condutos ilíacos em pacientes submetidos a REVA. Métodos Trata-se de uma avaliação retrospectiva de oito pacientes, de um total de 75 (11%), os quais foram submetidos a REVA nos últimos 10 anos. Foi realizada angiotomografia computadorizada antes do procedimento para avaliar as dimensões das artérias ilíaca e femoral. Os pacientes com dimensões arteriais menores, abaixo do tamanho de acesso recomendado para o dispositivo de endoprótese aórtica, foram submetidos a condutos ilíacos. Resultados A média de idade dos participantes foi de 45,7±15,2 anos, sendo três do sexo masculino e cinco do sexo feminino. As indicações para condutos ilíacos foram local de acesso ilíaco femoral de pequeno calibre, para sete pacientes, e doença aterosclerótica, para um paciente. Todas as próteses externas foram anastomosadas na artéria ilíaca comum direita, com exceção de uma, que foi anastomosada no local da bifurcação aórtica por apresentar artéria ilíaca comum menor. A taxa de sucesso do procedimento foi de 100%. As complicações no local de acesso incluíram infecção (n = 1), hematoma retroperitoneal (n = 1) e necessidade de transfusão de sangue (n = 3). O tempo mediano de internação hospitalar pós-intervenção foi de 10 dias. Todos os pacientes apresentaram desfechos de longo prazo favoráveis no seguimento mediano de 18 meses. Conclusões As pacientes do sexo feminino necessitaram de condutos ilíacos durante REVA com maior frequência. O uso adjuvante de condutos ilíacos com REVA foi associado a desfechos perioperatórios e de curto prazo favoráveis.

Abstract Introduction: One of the most important points of the acute type A aortic dissection surgery is how to perform cannulation regarding cerebral protection concerns and the conditions of arterial structures as a pathophysiological consequence of the disease. Objective: In this study, femoral and axillary cannulation methods were compared in acute type A aortic dissection operations. Methods: The study retrospectively evaluated 52 patients who underwent emergency surgery for acute type A aortic dissection. Patients without malperfusion according to Penn Aa classification were chosen for preoperative standardization of the study groups. The femoral arterial cannulation group was group 1 (n=22) and the axillary arterial cannulation group was group 2 (n=30). The groups were compared in terms of perioperative and postoperative results. Results: There was no statistically significant difference in terms of preoperative data. In terms of postoperative parameters, especially early mortality and new-onset cerebrovascular event, there was no statistically significant difference. Mortality rates in group 1 and group 2 were 13.6% (n=3) and 10% (n=3), respectively (P=0.685). Postoperative new-onset cerebral events ratio was found in 5 (22.7%) in the femoral cannulation group and 6 (20%) in the axillary cannulation group (P=0.812). Conclusion: Both femoral and axillary arterial cannulation methods can be safely performed in patients with acute type A aortic dissection, provided that cerebral protection strategies should be considered in the first place. The method to be performed may vary depending on the patient's current medical condition or the surgeon's preference.

Abstract Introduction: Radial access is the gold standard for ST-elevation myocardial infarction; nevertheless, there is scarce information in Mexico. Objectives: The objectives of this study were to describe the differences in radiation exposure, intervention time, fluoroscopy time, complications and temporal trends, and risk factors among radial and femoral access for coronary procedures. Materials and Methods: A total of 493 patients underwent coronary interventions by femoral or radial access. Sociodemographic and procedural data were recorded. A logistic regression model to determine risk factors for complications was performed. Results: The population included 346 men and 147 women, with a median age of 63 years, 159 underwent radial and 334 femoral approaches. Complications occurred in 18 patients (3.6%), 11 in radial and 7 in femoral access, with a higher trend in the first 5 months (n = 14). Vasospasm was the most common (n = 9) complication. Median fluoroscopy time was 12 min for radial and 9 min for femoral groups, with a total radiation dose of 2282 µGm2 and 2848 µGm2, respectively. Temporal trends showed that complications occurred most frequently during the first 6 months of the study. The main predictors for complications were intervention time and one-vessel disease. Conclusions: Radial access had higher frequency of complications than femoral approach and they were more common during the first 6 months. The main risk factor was intervention time longer than 60 min.


Resumen Introducción: El abordaje radial es el de elección para infarto de miocardio con elevación del segmento ST, sin embargo se desconoce información en México. Objetivos: Describir las diferencias en exposición a radiación, tiempo de intervención, tiempo de fluoroscopía, complicaciones y sus variaciones temporales, además de los factores de riesgo entre el abordaje radial y el femoral para procedimientos coronarios. Método: Se incluyeron 493 pacientes que fueron sometidos a estudio angiográfico o intervenciones coronarias por abordaje radial o femoral. Se recabaron datos sociodemográficos, antecedentes y variables del procedimiento. Se realizó un modelo de regresión logística para determinar los factores asociados a complicaciones. Resultados: Se incluyeron 346 hombres y 147 mujeres, con mediana de edad de 63 años. A 159 se les realizó acceso radial y a 334 femoral. Las complicaciones ocurrieron en 18 pacientes (3.65%): 11 en radial y 7 en femoral, teniendo mayor incidencia en los primeros 5 meses (n = 14) y siendo el vasoespasmo el más común (n = 9). La mediana de tiempo de fluoroscopía fue de 12 minutos para el radial y de 9 minutos para el femoral, con una dosis total de radiación de 2,282 µGm2 y 2,848 µGm2, respectivamente. Las tendencias temporales indicaron que las complicaciones fueron más frecuentes durante los primeros 6 meses. Los principales predictores fueron el tiempo de intervención y la enfermedad de 1 vaso. Conclusiones: La vía de acceso radial tuvo más complicaciones que la femoral. Se observaron más complicaciones en los primeros 6 meses del estudio. El principal predictor de complicaciones fue el tiempo de intervención mayor a 60 minutos.

Abstract Objective: Over the past 10 years, the rate of patients who have undergone coronary artery bypass graft (CABG) surgery has increased twofold in cases of coronary angiography. Today, transradial access is the first choice for coronary angiography. We aimed to compare the efficacy and reliability of radial versus femoral access for coronary angiography in post-CABG surgery in this study. Methods: Data from 442 patients who underwent post-CABG surgery between 2012-2017 were retrospectively compared. The right radial route was used in 120 cases, the left radial route in 148, and femoral route in 174. These three pathways were compared in terms of procedure time and fluoroscopy time, efficacy, and complication development. Comparisons among the three groups were performed with Bonferroni test for continuous variables and chi-square or Fisher's exact test for nominal variables as a binary. Results: Comparison results indicate that femoral access was better than left radial access and the left radial access was better than right radial access in terms of fluoroscopy time (10.71±1.65, 10.94±1.25, 16.12±5.28 min, P<0.001) and total procedure time (17.28±1.68, 17.68±2.34, 23.04±5.84 min, P<0.001). The left radial pathway was the most effective way of viewing left internal mammary artery (LIMA). No statistically significant differences were found among the three groups in other graft visualizations, all minor complications, total procedure and fluoroscopy time "Except LIMA imaging". Mortality due to processing was not observed in all three groups. Conclusion: The left radial route is preferred over right radial access for post-CABG angiography because the left radial pathway is close to the LIMA and is similar to the femoral pathway. In LIMA graft imaging, right radial access is a reliable route, even though it is not as effective as other pathways. We hope that the right radial pathway will improve with physician experience and innovations.

Background: In coronary procedures, although the radial approach protects patients from hemorrhagic complications, it is technically more complex than the femoral approach. Objectives: To test the hypothesis that the radial approach is the procedure of choice in ACS patients due to the high risk of bleeding; and to identify independent predictors of the choice for radial access. Methods: Patients admitted for ACS who underwent invasive coronary procedure were included. We registered the type of access (femoral or radial) chosen by the physician for the first angiography; the investigators did not interfere with this choosing process. Student's t-test was used for comparisons between the CRUSADE and ACUITY scores. Predictors of radial access were compared between the groups. Statistical significance was defined by p < 0,05.Results: Radial access was chosen in 67% of 347 consecutive patients. Patients who underwent radial approach had lower risk of bleeding determined by CRUSADE (30 ± 14 vs. 37 ± 15; p < 0.001) as compared with femoral access. In multivariate analysis, four variables were identified as independent predictors negatively associated with radial access ­ age (OR = 0.98; 95%CI = 0.96 ­ 0.99), creatinine (OR = 0.54; 95%CI = 0.3 ­ 0.98), signs of left ventricular failure (OR = 0.45; 95% CI = 0.22 ­ 0.92) and previous CABG (OR = 0.022; 95%CI = 0.003 ­ 0.166). Conclusion: The propensity to choose radial over femoral access in coronary intervention was not primarily influenced by patients' bleeding risk. Predictors of this decision, identified in the study, indicated less complex patients, suggesting that the difficulty in performing the technique was a stronger determinant than its potential antihemorrhagic effect

Resumen Objetivo: Evaluar la efectividad obtenida con la compresión ecoguiada como primera elección para lograr la trombosis del pseudoaneurisma iatrogénico post estudio o intervencionismo. Material y Método: Estudio retrospectivo observacional de 9 pacientes en quienes se realizó la compresión ecoguiada como primera alternativa de manejo frente a pseudoaneurismas iatrogénicos secundarios a procedimientos diagnósticos o terapéuticos ocurridos entre agosto de 2012 y diciembre de 2015 en el Hospital Regional de Talca. Resultados: De 4.070 procedimientos se presentó la complicación en 9 pacientes, un 0,22%; 7 posterior a procedimientos terapéuticos (0,4%), y 2 en el grupo de diagnósticos (0,08%). La muestra se compone por 6 hombres y 3 mujeres de edad promedio 70 años. El vaso comprometido fue principalmente la arteria femoral con un 77%. Los pseudoaneurismas ocurrieron mayoritariamente posteriores a procedimientos terapéuticos (77%); y el éxito de la compresión ecoguiada se obtuvo en el 67%. No hubo complicaciones asociadas al tratamiento. Conclusiones: La compresión ecoguiada tiene un porcentaje de éxito menor en esta pequeña serie que los obtenidos por otros autores, pero al no presentar complicaciones y por su bajo costo creemos debe mantenerse como primera alternativa.


Objective: To evaluate the effectiveness obtained with echo guided compression as the first choice to achieve thrombosis of iatrogenic pseudoaneurysm after study or interventionism. Material and Method: Retrospective observational study of 9 patients who underwent echo-guided compression as the first management alternative to iatrogenic pseudoaneurysms following diagnostic or therapeutic procedures that occurred between August 2012 and December 2015 at the Regional Hospital of Talca. Results: Of 4,070 procedures the complication was presented in 9 patients, 0.22%; 7 after therapeutic procedures (0.4%), and 2 in the diagnostic group (0.08%). The sample consists of 6 men and 3 women of average age 70 years. The involved vessel was mainly the femoral artery with 77%. Pseudoaneurysms occurred mostly after therapeutic procedures (77%); and the success of the echogenic compression was obtained in 67%. There were no complications associated with treatment. Conclusions : Image guided compression has a lower success rate in this small series than those obtained by other authors, but since there are no complications and because of its low cost, we believe that it should be kept as the first alternative.

Abstract Objective: Midline sternotomy is the preferred approach for device migration following transcatheter device closure of ostium secundum atrial septal defect. Results of patients operated for device migration were retrospectively reviewed after transcatheter closure of atrial septal defect. Methods: Among the 643 patients who underwent atrial septal defect with closure device, 15 (2.3%) patients were referred for device retrieval and surgical closure of atrial septal defect. Twelve patients underwent device retrieval and surgical closure of atrial septal defect through right antero-lateral minithoracotomy with femoral cannulation. Three patients were operated through midline sternotomy. Results: Twelve patients operated through minithoracotomy did not require conversion to sternotomy. Due to device migration to site of difficult access through thoracotomy, cardiac tamponade and hemodynamic instability, respectively, three patients were operated through midline sternotomy. Mean aortic cross-clamp time and cardiopulmonary bypass time were 28.1±17.7 and 58.3±20.4 minutes, respectively. No patient had surgical complication or mortality. Mean intensive care unit and hospital stay were 1.6±0.5 days and 7.1±2.2 days, respectively. Postoperative echocardiography confirmed absence of any residual defect and ventricular dysfunction. In a mean follow-up period of six months, no mortality was observed. All patients were in New York Heart Association class I without wound or vascular complication. Conclusion: Minithoracotomy with femoral cannulation for cardiopulmonary bypass is a safe-approach for selected group of patients with device migration following transcatheter device closure of atrial septal defect without increasing the risk of cardiac, vascular or neurological complications and with good cosmetic and surgical results.

Abstract Objective: To evaluate our experience following the introduction of a percutaneous program for endovascular treatment of aortic diseases using Perclose Proglide® assessing efficacy, complications and identification of potential risk factors that could predict failure or major access site complications. Methods: A retrospective cohort study during a two-year period was performed. All the patients submitted to totally percutaneous endovascular repair (PEVAR) of aortic diseases and transcatheter aortic valve implantation since we started the total percutaneous approach with the preclosure technique from November 2013 to December 2015 were included in the study. The primary endpoint was major ipsilateral access complication, defined according to PEVAR trial. Results: In a cohort of 123 patients, immediate technical success was obtained in 121 (98.37%) patients, with only two (0.82%) cases in 242 vascular access sites that required intervention immediately after the procedure. Pairwise comparisons revealed increased major access complication among patients with >50% common femoral artery (CFA) calcification vs. none (P=0.004) and > 50% CFA calcification vs. < 50% CFA calcification (P=0.002). Small artery diameter (<6.5 mm) also increased major access complication compared to bigger diameters (> 6.5 mm) (P=0.027). Conclusion: The preclosure technique with two Perclose Proglide® for PEVAR is safe and effective. Complications occur more often in patients with unfavorable access site anatomy and the success rate can be improved with proper patient selection.

OBJECTIVES:Compare the use of carbon dioxide contrast medium with iodine contrast medium for the endovascular treatment of ilio-femoral occlusive disease in patients without contraindications to iodine.MATERIALS AND METHODS:From August 2012 to August 2014, 21 consecutive patients with ilio-femoral occlusive disease who were eligible for endovascular treatment and lacked contraindications to either iodine contrast or carbon dioxide were randomized into the carbon dioxide or iodine groups and subjected to ilio-femoral angioplasty.We analyzed the feasibility of the procedures, the surgical and clinical outcomes, the procedure lengths, the endovascular material costs, the contrast costs and the quality of the angiographic images in each group.RESULTS:No conversions to open surgery and no contrast media related complications were noted in either group. A post-operative femoral pulse was present in 88.9% of the iodine group and 80% of the carbon dioxide group. No differences in procedure length, endovascular material cost or renal function variation were noted between the groups. Four patients in the carbon dioxide group required iodine supplementation to complete the procedure. Contrast media expenses were reduced in the carbon dioxide group. Regarding angiographic image quality, 82% of the carbon dioxide images were graded as either good or fair by observers.CONCLUSIONS:The use of carbon dioxide contrast medium is a good option for ilio-femoral angioplasty in patients without contraindications to iodine and is not characterized by differences in endovascular material costs, procedure duration and surgical outcomes. In addition, carbon dioxide has lower contrast expenses compared with iodine.

Introdução: O Registro Angiocardio, que agrega dados de cinco diferentes centros do Estado de São Paulo, oferece a oportunidade de estudar a associação de fatores de risco cardiovasculares com a extensão da doença arterial coronária (DAC), avaliada pela cinecoronariografia. Este estudo buscou analisar preditores independentes de DAC multiarterial em nosso meio. Métodos: De agosto de 2006 a janeiro de 2014, 16.320 pacientes com DAC foram incluídos e divididos nos grupos uni e multiarterial. Resultados: Pacientes multiarteriais (n = 9.512, 58,3%) apresentaram-se, em média, 3 anos mais idosos, com maior prevalência do sexo masculino (69,4% vs. 65,4%; p < 0,01), hipertensão arterial (80,0% vs. 73,5%; p < 0,01), diabetes (40,7% vs. 28,8%; p < 0,01), dislipidemia (39,6% vs. 34,9%; p < 0,01), antecedentes familiares de DAC precoce (23,0% vs. 21,3%; p < 0,01), insuficiência renal crônica (4,1% vs. 2,6%; p < 0,01), acidente vascular cerebral prévio (3,8% vs. 2,8%; p < 0,01), doença vascular periférica (4,0% vs. 3,3%; p = 0,02) e infarto do miocárdio prévio (18,4% vs. 13,9%; p < 0,01), além de menor prevalência de tabagismo (20,0% vs. 24,5%; p < 0,01). À análise multivariada, foram preditores independentes de lesão multiarterial idade > 40 anos (odds ratio - OR = 1,996; IC 95% 1,52-2,63; p < 0,01), sexo masculino (OR = 1,202; IC 95% 1,12-1,28; p < 0,01), hipertensão arterial (OR = 1,439; IC 95% 1,34-1,55; p < 0,01), diabetes (OR = 1,697; IC 95% 1,59-1,81; p < 0,01), dislipidemia (OR = 1,223; IC 95% 1,15-1,30; p < 0,01), infarto agudo do miocárdio prévio (OR = 1,393; IC 95% 1,28-1,52; p < 0,01) e insuficiência renal crônica (OR = 1,597; IC 95% 1,33-1,91; p < 0,01). Conclusões: A DAC multiarterial em nosso meio é associada aos fatores de risco tradicionais para aterosclerose, com exceção do tabagismo. A idade, o diabetes e a insuficiência renal crônica foram os fatores preditores mais fortes para a DAC multiarterial avaliada pela cinecoronariografia


Background: The Angiocardio Registry, which comprises data from five different centers in the state of São Paulo, offers the opportunity to study the association between cardiovascular risk factors and coronary artery disease (CAD) extent, as assessed by coronary angiography. This study aimed to evaluate independent predictors of multivessel CAD in Brazil. Methods: From August 2006 to January 2014, 16,320 patients with CAD were included and divided into the single- and multivessel groups. Results: Patients with multivessel disease (n = 9,512, 58.3%) were on average 3 years older, with a higher prevalence of males (69.4% vs. 65.4%; p < 0.01), arterial hypertension (80.0% vs. 73.5%; p < 0.01), diabetes (40.7% vs. 28.8%; p < 0.01), dyslipidemia (39.6% vs. 34.9%; p < 0.01), family history of CAD (23.0% vs. 21.3%; p < 0.01), chronic renal failure (4.1% vs. 2.6%; p < 0.01), previous stroke (3.8% vs. 2.8%; p < 0.01), peripheral vascular disease (4.0% vs. 3.3%; p = 0.02) and previous myocardial infarction (18.4% vs. 13.9%; p < 0.01), as well as a lower prevalence of smoking (20.0% vs. 24.5%; p < 0.01). At the multivariate analysis, the following were independent predictors of multivessel lesion: age > 40 years (odds ratio - OR = 1.996; 95% CI: 1.52-2.63; p < 0.01), male gender (OR = 1.202; 95% CI: 1.12-1.28; p < 0.01), arterial hypertension (OR = 1.439; 95% CI: 1.34-1.55; p < 0.01), diabetes (OR =1.697; 95% CI: 1.59-1.81; p < 0.01), dyslipidemia (OR = 1.223; 95% CI: 1.15-1.30; p < 0.01), previous acute myocardial infarction (OR = 1.393; 95% CI: 1.28-1.52; p < 0.01), and chronic renal failure (OR = 1.597; 95% CI: 1.33-1.91; p < 0.01). Conclusions: Multivessel CAD in Brazil is associated with traditional risk factors for atherosclerosis, except for smoking. Age, diabetes, and chronic renal failure were the strongest predictors for multivessel CAD assessed by coronary angiography

As intervenções endovasculares na artéria femoral superficial para o tratamento da doença arterial oclusiva periférica têm crescido nas últimas décadas. A primeira e a segunda geração de stents na artériafemoral superficial falharam em demonstrar a melhora da perviedade do vaso tratado, devido às altas taxas defratura. O objetivo deste estudo foi avaliar os desfechos clínicos no curto prazo com o uso de stents de nitinolsuperflexíveis de terceira geração no tratamento de lesões ateroscleróticas na artéria femoral superficial. Métodos: Trata-se de um estudo retrospectivo, realizado em único centro, no período de junho de 2013 a maio de 2014. Um total de 27 pacientes foi submetido à angioplastia com stents de nitinol superflexíveis de terceira geração em lesões ateroscleróticas da arterial femoral superficial. Resultados: A média de idades foi de 68 ± 12 anos, 55,6% eram do sexo feminino e 74,1%, diabéticos. Os pacientes foram classificados em TASC B e C em 77,7% dos casos. O sucesso técnico foi de 100%. Houve aumento do índice tornozelo-braquial de 0,35 ± 0,1 pré-intervenção para 0,75 ± 0,2 na alta hospitalar. O seguimento médio dos pacientes foi de 6,7 ± 2,3 meses. A taxa de patência primária foi de 96,3%. A taxa de salvamento de membro foi de 100%. Não ocorreram fraturas de stent documentadas por raios X. Endovascular interventions in the superficial femoral artery for the treatment of peripheral arterial occlusive disease have increased over the last decades. The first- and second-generation stents in the superficial femoral artery have failed to demonstrate improved patency of the treated vessel due to high fracture rates. The aim of this study was to evaluate the clinical, short-term outcomes of using third-generation superflexible nitinol stents in the treatment of atherosclerotic lesions in the superficial femoral artery. Methods: This was a retrospective study carried out in a single center, from June 2013 to May 2014. A total of 27 patients underwent angioplasty with third-generation superflexible nitinol stents in atherosclerotic lesions of the superficial femoral artery. Results: The mean age was 68 ± 12 years, 55.6% were females, and 74.1% were diabetics. Patients were classified as TASC B and C in 77.7% of cases. Technical success was 100%. There was an increase in the anklebrachial index from 0.35 ± 0.1 before the intervention to 0.75 ± 0.2 at hospital discharge. The mean followupof patients was 6.7 ± 2.3 months. The primary patency rate was 96.3%. The limb salvage rate was 100%. There were no stent fractures documented by X-rays. O implante de valva aórtica transcateter (TAVI) é uma alternativa para pacientes com estenose aórtica de alto risco cirúrgico e para muitos daqueles considerados inoperáveis. Apesar de sua característica minimamente invasiva, podem ocorrer complicações relacionadas ao procedimento. Obstrução coronária durante o TAVI é uma complicação rara, com incidência inferior a 1%, mas potencialmente letal. Em nosso país, essa complicação foi encontrada em 0,72% dos procedimentos − 3 de 418 casos do Registro Brasileiro de Implante de Bioprótese Aórtica por Cateter − com mortalidade hospitalar de 100%. Apresentamos, neste relato de caso, medidas de prevenção e tratamento de oclusão coronária após o TAVI.


Transcatheter aortic valve implantation (TAVI) is an alternative for patients with aortic stenosis at high surgical risk and for many of those considered inoperable. Despite its minimally invasive features,complications related to the procedure may occur. Coronary obstruction during TAVI is a rare (incidence rate of less than 1%) but potentially lethal complication. In Brazil, this complication was found in 0.72% of procedures – three of 418 cases from the Brazilian Transcatheter Aortic Valve Implantation Registry – with an in-hospital mortality rate of 100%. This case report presents prevention and treatment measures forcoronary occlusion after TAVI.

Procedimentos coronários invasivos são comuns em pacientes com revascularizaçãomiocárdica cirúrgica prévia. Dados acerca do real papel e das possíveis limitações do acesso radial nesse subgrupo de pacientes são infrequentes. O objetivo deste estudo foi avaliar a factibilidade e a segurança do acesso radial em pacientes revascularizados cirurgicamente e que foram submetidos a procedimentos coronários invasivos diagnósticos ou terapêuticos subsequentes, comparando-o ao acesso femoral. Métodos: Entre maio de 2008 e novembro de 2014, foram analisados 959 procedimentos, sendo 539 realizados pelo acesso radial e 420 pelo femoral. Todos os operadores estavam familiarizados com ambos os acessos vasculares, cabendo a eles a decisão final sobre a via a ser utilizada. Resultados: A prevalência de insucesso foi de 6,1% vs. 0,5% (p < 0,0001), favorecendo a técnica femoral. As taxas de eventos cardíacos adversos graves (0,4% vs. 0,7%) e de complicações vasculares (1,5% vs. 1,9%)foram baixas, sem diferença entre os grupos. A opção pela técnica radial implicou em maior tempo de fluoroscopia e necessidade de cruzamento entre vias de acesso, principalmente em procedimentos diagnósticos. Conclusões: O acesso radial representou uma opção segura e eficaz para a realização de procedimentos coronários invasivos em pacientes cirurgicamente revascularizados, notadamente para os procedimentos terapêuticos...


Invasive coronary procedures are common in patients with previous coronary artery by-pass graft surgery. Data on the actual role and possible limitations of the radial approach in this subgroup of patients are sparse. The objective of this study was to evaluate the feasibility and safety of radial accessin patients surgically revascularized and who under went subsequent invasive diagnostic or therapeutic coronary procedures, comparing it to the femoral access. Methods: Between May 2008 and November 2014, 959 procedures were included; 539 performed by radial access and 420 by femoral access. All operators were familiar with both vascular accesses, and the final decision on the route to be used was left to the operators discretion. Results: The failure rate was 6.1% vs. 0.5% (p < 0.0001), favoring the femoral approach. Major adverse cardiac events (0.4% vs. 0.7%) and vascular complications (1.5% vs. 1.9%) rates were low, with no difference between groups. The choice of the radial approach resulted in greater fluoroscopy time and crossover rate between access routes, especially in diagnostic procedures. Introdução: A técnica de acesso arterial radial tem sido incorporada em muitos centros como técnica de escolha para procedimentos invasivos cardíacos. No entanto, ainda há resistências relacionadas principalmente a possibilidade de crossover para via femoral, causadas por dificuldades técnicas ou alterações anatômicas vasculares. O objetivo deste estudo foi identificar as razões para a utilização da via femoral em um centro de médio volume de intervenções, que recentemente adotou essa técnica como primeira escolha na realização de procedimentos invasivos cardíacos. Métodos: Estudo prospectivo, que incluiu pacientes consecutivos submetidos a cateterismo cardíaco e coronariografia de forma eletiva. O preenchimento de formulário foi realizado com informações pré-, per- e pós-procedimento, e foi dada ênfase à avaliação das causas da utilização da via femoral (crossover ou por escolha primária do operador). Resultados: No período de novembro de 2013 a agosto de 2014, 1.290 pacientes foram submetidos a procedimento diagnóstico eletivo. A via femoral foi utilizada em 10,9% dos pacientes, por escolha do operador em 6,6% ou por crossover em 4,3% dos casos. O crossover ocorreu por punção inadequada (3,4%), espasmo arterial (0,6%) ou tortuosidade vascular (0,3%). As complicações imediatas foram observadas em seis pacientes (0,5%) que desenvolveram hematomas locais (tipos I e II). Background: The radial access has been incorporated in many centers as the technique of choice for cardiac invasive procedures. However, there is still resistance to its use, which is mainly related to the possibility of crossover to the femoral access, caused by technical difficulties or vascular anatomic alterations. The aim of this study was to identify the reasons for the use of the femoral access in a center with moderate volume of interventions, which recently adopted it as the technique of choice for invasive cardiac procedures. Methods: Prospective study including consecutive patients undergoing elective cardiac catheterization and coronary angiography. A data form was filled out containing pre-, peri-, and postprocedure information, with emphasis on the evaluation of the causes to use the femoral access (crossover or first choice). Results: From November 2013 to August 2014, a total of 1,290 patients underwent an elective diagnostic procedure. The femoral access was used in 10.9% of the patients, as the operator's first choice in 6.6% and due to crossover in 4.3% of the cases. Crossover resulted from puncture failure (3.4%), arterial spasm (0.6%), or vascular tortuosity (0.3%). Immediate complications were observed in six patients (0.5%) who developed local hematoma (type I and type II). Background: The radial access provides a lower risk of bleeding and vascular complications related to the puncture site in comparison to the femoral access. Recent studies have suggested a reduction in mortality associated with the radial access in patients with acute myocardial infarction undergoing percutaneous coronary intervention. Objective: To compare the occurrence of adverse cardiovascular ischemic and hemorrhagic events in patients undergoing primary angioplasty according to the type of arterial access route. Methods: From August 2010 to December 2011, 588 patients undergoing primary percutaneous coronary intervention during acute ST-segment elevation myocardial infarction were assessed; they were recruited from 47 centers participating in the ACCEPT registry. Patients were grouped and compared according to the arterial access used for the procedure. Results: The mean age was 61.8 years; 75% were males and 24% had diabetes mellitus. There was no difference between groups as regards the procedure success rate, as well as regards the occurrence of death, reinfarction, or stroke at six months of follow-up. Severe bleeding was reported in 1.1% of the sample analyzed, with no statistical difference related to the access used. Conclusions: The femoral and radial accesses are equally safe and effective for the performance of primary percutaneous coronary intervention. The low rate of cardiovascular events and of hemorrhagic complications reflects the quality of the participating centers and the operators expertise with the use of both techniques. .


Fundamentos: O acesso radial promove menor risco de sangramento e complicações vasculares relacionadas ao sítio de punção quando comparado ao acesso femoral. Estudos recentes sugerem redução de mortalidade favorável ao primeiro em pacientes com infarto agudo do miocárdio submetidos à intervenção coronária percutânea. Objetivo: Comparar a ocorrência de eventos cardiovasculares adversos isquêmicos e hemorrágicos em pacientes submetidos à angioplastia primária conforme a via de acesso arterial. Métodos: No período de agosto de 2010 a dezembro de 2011, foram avaliados 588 pacientes que realizaram intervenção coronária percutânea primária na vigência de um infarto agudo do miocárdio com supradesnivelamento de ST, incluídos em 47 centros participantes do registro ACCEPT. Os pacientes foram agrupados e comparados de acordo com a via de acesso arterial utilizada para a efetivação do procedimento. Resultados: A média de idade foi de 61,8 anos, sendo 75% pertencentes ao sexo masculino e 24% portadores de diabetes melito. Não houve diferença entre os grupos na taxa de sucesso do procedimento, bem como na ocorrência de óbito, reinfarto ou acidente vascular encefálico aos seis meses de seguimento. Sangramento grave foi relatado em 1,1% da amostra analisada, sem diferença estatística conforme a via de acesso utilizada. Conclusões: As vias de acesso femoral e radial são igualmente seguras e eficazes para a realização de intervenção coronária percutânea primária. A baixa taxa de eventos cardiovasculares, bem como de complicações hemorrágicas, reflete a qualidade dos centros participantes e a experiência dos operadores com a utilização de ambas as técnicas. .

Introdução: Estudos demonstram que o acesso via artéria radial diminui o risco de complicações vasculares e hemorrágicas associadas à intervenção coronária percutânea. Nosso objetivo foi avaliar os resultados hospitalares da utilização da via radial em pacientes idosos submetidos à intervenção coronária percutânea. Métodos: Registro prospectivo, que incluiu pacientes ≥ 70 anos, tendo sido comparados os desfechos de segurança e de eficácia entre os grupos tratados pelas vias radial e femoral. Resultados: Incluímos 225 pacientes, sendo 117 (52%) tratados por via radial e 108 por via femoral. À exceção da idade, as demais características clínicas não mostraram diferenças entre os grupos. Predominaram os pacientes do sexo masculino (60%), 36,7% eram diabéticos e mais de um terço foi tratado na vigência de quadro de síndrome coronária aguda. As variáveis angiográficas e do procedimento não mostraram diferenças entre os grupos. Na comparação das taxas de complicações vasculares, somente os hematomas < 5 cm (5,1% vs. 17,6%; p < 0,01) foram mais prevalentes no acesso femoral. Sangramentos maiores, pelo critério ACUITY (zero vs. 5,6%; p = 0,01), e menores, pelo critério TIMI (zero vs. 7,4%; p < 0,01), também foram mais frequentes no grupo femoral. Os desfechos clínicos hospitalares óbito (0,9% vs. 5,6%; p = 0,06) e infarto não fatal (zero vs. 3,7%; p = 0,05) incidiram mais frequentemente nos pacientes tratados por via femoral. Conclusões: Em uma população não selecionada de pacientes com idade ≥ 70 anos, a intervenção coronária percutânea por via radial esteve associada à menor incidência de desfechos clínicos hospitalares...


Background: Studies demonstrate that radial artery access reduces the risk of vascular and bleeding complications associated to percutaneous coronary intervention. Our objective was to evaluate in-hospital results of the transradial approach in elderly patients undergoing percutaneous coronary intervention. Methods: Prospective registry including patient's ≥ 70 years of age; safety and efficacy endpoints were compared for the radial and femoral artery access groups. Results: We included 255 patients, 117 (52%) treated using the radial approach and 108 using the femoral approach. Except for age, the remaining clinical characteristics did not show differences between groups. Male patients prevailed (60%), 36.7% were diabetic and over one third were diagnosed with acute coronary syndrome. Angiographic and procedure-related variables did not show differences between groups. When vascular complication rates were compared only hematomas < 5 cm (5.1% vs. 17.6%; p < 0.01) were more prevalent with the femoral access. Major bleedings, according to the ACUITY criteria (zero vs. 5.6%; p = 0.01) and minor bleedings, according to the TIMI criteria (zero vs. 7.4%; p < 0.01), were also more frequent in the femoral group. In-hospital clinical endpoints, death (0.9% vs. 5.6%; p = 0.06) and non-fatal infarction (zero vs. 3.7%; p = 0.05) were more frequent in patients treated by the femoral access.
