OBJECTIVES: We aimed to explore the effects of diet on the inflammatory response in middle-aged and elderly people with hypertension. METHODS: Thirty overweight or obese patients with stage one hypertension (age range, 45-75 years) were allocated to either the intervention or control group (n=15 per group; age- and sex-matched). Patients in the intervention group consumed a food powder supplement (100 g) instead of a regular meal. The control group maintained their normal dietary habits. This study lasted for six weeks. Blood pressure, inflammatory marker levels, and energy intake were measured before and after the study. RESULTS: After 6 weeks, the diet composition of the intervention group changed significantly (p<0.05). The intake of proteins, dietary fibre, monounsaturated fat, and polyunsaturated fat increased significantly (p<0.05), while the total energy intake trended towards an increase (p>0.05). In the control group, the total energy intake decreased significantly (p<0.05). The levels of nuclear factor-κB (NF-κB), soluble intercellular adhesion molecule-1 (sICAM-1) and high sensitivity C-reactive protein (hs-CRP) decreased, and adiponectin increased significantly in the intervention group (p<0.05); however, no significant changes were observed in the inflammatory marker levels of the control group. In the intervention group, systolic blood pressure decreased significantly (p<0.05), and diastolic blood pressure also exhibited a decreasing trend. No significant change in blood pressure was observed in the control group. CONCLUSION: The consumption of a food powder supplement can improve diet composition, decrease blood pressure and reduce inflammation in middle-aged and elderly overweight or obese hypertensive patients. The food powder supplement may also have an anti-atherosclerotic effect in hypertensive patients.

OBJETIVO: Quantificar a concentração de vitamina A no leite maduro de nutrizes assistidas em maternidade pública no município do Rio de Janeiro e avaliar a sua relação com variáveis sociodemográficas e grau de conhecimento de nutrição. MÉTODOS: Para a quantificação do retinol, foram coletadas amostras de 10 mL de leite maduro. Os pontos de corte adotados para identificação da deficiência de vitamina A e constituição de reserva hepática foram de < 1,05 µmol/L e > 2,3 µmol/L, respectivamente. Foram avaliadas, ainda, as variáveis: grau de conhecimento de nutrição, sexo, idade, renda familiar, escolaridade, condições de saneamento básico, número de moradores na residência, idade materna e assistência pré-natal. Resultados: Participaram do estudo 196 nutrizes, com concentração média de vitamina A no leite maduro de 1,76±0,85 µmol/L e prevalência de 20,5% de deficiência de vitamina A. Não houve diferença na concentração de vitamina A no leite das nutrizes segundo as variáveis sociodemográficas e conhecimento de nutrição. Apenas 38,9% das nutrizes apresentavam concentração de vitamina A no leite suficiente para constituição de reserva hepática dos lactentes. RESULTADOS: Participaram do estudo 196 nutrizes, com concentração média de vitamina A no leite maduro de 1,76±0,85 µmol/L e prevalência de 20,5% de deficiência de vitamina A. Não houve diferença na concentração de vitamina A no leite das nutrizes segundo as variáveis sociodemográficas e conhecimento de nutrição. Apenas 38,9% das nutrizes apresentavam concentração de vitamina A no leite suficiente para constituição de reserva hepática dos lactentes. CONCLUSÕES: Os achados aqui apresentados revelam alta prevalência de inadequação do estado nutricional de vitamina A materno e do lactente, concordante com a prevalência nacional descrita em mulheres em idade fértil e crianças brasileiras, e que as medidas de intervenção para combate à carência devem ser estendidas a todas as gestantes e puérperas, independentemente das condições sociodemográficas e do grau de conhecimento de nutrição, visando melhorar a saúde do binômio mãe/filho.


OBJECTIVE: To quantify vitamin A levels in mature milk of 196 nursing women who were treated at the Maternity School of Rio de Janeiro and to evaluate its correlation with sociodemographic variables and degree of nutrition knowledge. METHODS: To quantify retinol concentrations, 10 mL of mature milk were collected by manual expression of one breast, 2 hours after the last feed, in the morning period. Values below 1.05 µmol/L and 2.3 µmol/L were considered inadequate to meet satisfactory intake and to constitute vitamin A liver reserve, respectively. The following variables were also assessed: sex, age, familiar income, maternal education, basic sanitation conditions, number of people in the household, maternal age, prenatal care, and degree of nutrition knowledge. RESULTS: Among the 196 lactating mothers analyzed, the average vitamin A concentration observed in mature milk was 1.76±0.85 µmol/L and prevalence of vitamin A deficiency was observed in 20.5% mothers. There was no significant difference between vitamin A levels in maternal milk and the variables socioeconomic status and nutrition knowledge. Only 38.9% of lactating women presented enough vitamin A concentrations in milk for the infants' liver reserves (2.3 µmol/L). CONCLUSIONS: These findings reveal high prevalence of inadequate vitamin A nutritional status of mothers and infants, consistent with the national prevalence reported in women of childbearing age and Brazilian children, and that the intervention measures to fight this shortage should be extended to all pregnant and postpartum women, regardless of sociodemographic conditions and degree of nutrition knowledge, in order to improve the health of mother and child.

Background: During 2005, the Chilean Ministry of Health included a fortified milk beverage to the Elderly Food Supplementation Program, to improve the compliance with the program and to increase the provision of micronutrients. This beverage was added to a soup that was distributed since 1999. Aim: To evaluate the long term acceptability, tolerance and consumption of the Milk Beverage and analyze the nutritional contribution of the products provided by the supplementation program to the usual diet of the elderly. Material and methods: One hundred twenty five elderly subjects, aged 77 ± 4 (52 males) living in Metropolitan Santiago, aged over 70 years, beneficiaries of the food supplementation program, were interviewed. The acceptability, consumption and tolerance of the Milk Beverage were evaluated by specially designed surveys. The contribution of the food suplementation products to daily nutrient intake was also analyzed by 24 hours dietary recalls. Results: The mean body mass index of the interviewed elderly subjects was 27.0 ±4.5 Kg/m². Monthly delivery of the products included in the program increased significantly after the incorporation of the Milk Beverage to the program. The milk beverage had a significantly higher daily consumption and a lower family dilution than the soup. The consumption of supplementation products increased significantly the daily intake of energy and most micronutrients. Noteworthy was the case for vitamin B12, C, E and zinc, whose intake doubled (p <0.01). Conclusions: The Milk Beverage had an excellent approval and consumption rate. The consumption of food supplementation products improved daily intake of most nutrients in elderly subjects.

Background: Zinc intake is well below recommendation among Chilean free living elderly adults of low socioeconomic level Aim: To assess the effect of the consumption of a food supplement on plasma zinc concentrations in elderly adults (EA). Material and Methods: Ambulatory EA (z70) with controlled type 2 diabetes mellitus or high blood pressure, ascribed to public family health centers were studied. They were separated in a control and intervention group, without blinding techniques. The intervention group consumed daily 50 g of a special nutritional supplement prepared as a soup or porridge, provided by the Government, for 3 months. The control group did not receive the supplement. A good compliance with the supplement was defined as a consumption of 7 portions per week. A fasting venous blood sample was obtained to determine plasma zinc at the beginning and end of the study. Results: Forty three supplemented EA aged 76±5 years (21 women) and 31 non supplemented EA aged 78±5 years (20 women), completed 3 months of follow up. Mean compliance with the supplement was 40.5 percent (95 percent confidence intervals (CI) 40.3-40.6 percent). General characteristics of the study subjects upon recruitment were similar, except for the literacy that was higher in the intervention group. We fitted a multiple linear regression model which explained 39 percent of the variance, where the consumption of the nutritional supplement increased the concentration ofplasmatic zinc by 4.14 fig/dL (95 percent CI 0.25-8.02) (p<0.037), after controlling for sex, age, energy, vitamin E and calcium consumption. Conclusions: The consumption of a food supplement significantly increased plasma zinc concentrations in Chilean elderly adults.
